Abstract:BACKGROUND:Late-onset Alzheimer's disease (AD) is a polygenic neurodegenerative disease. Identifying the neuroimaging phenotypes behind the genetic predisposition of AD is critical to the understanding of AD pathogenesis. Two major questions which previous studies have led to are: (1) should the general "polygenic hazard score" (PHS) be a good choice to identify the individual genetic risk for AD; and (2) should researchers also include inter-modality relationships in the analyses considering these may provide complementary information about the AD etiology.METHODS:We collected 88 healthy controls, 77 patients with mild cognitive impairment (MCI), and 22 AD patients to simulate the AD continuum included from the ADNI database. PHS-guided multimodal fusion was used to investigate the impact of PHS on multimodal brain networks in AD-continuum by maximizing both inter-modality association and reference-modality correlation. Fractional amplitude of low frequency fluctuations, gray matter (GM) volume, and amyloid standard uptake value ratios were included as neuroimaging features. Eventually, the changes in neuroimaging features along AD continuum were investigated, and relationships between cognitive performance and identified PHS associated multimodal components were established.RESULTS:We found that PHS was associated with multimodal brain networks, which showed different functional and structural impairments under increased amyloid deposits. Notably, along with AD progression, functional impairment occurred before GM atrophy, amyloid deposition started from the MCI stage and progressively increased throughout the disease continuum.CONCLUSION:PHS is associated with multi-facets of brain impairments along the AD continuum, including cognitive dysfunction, pathological deposition, which might underpin the AD pathogenesis.

Abstract:Neuroimaging genetics has attracted growing attention and interest, which is thought to be a powerful strategy to examine the influence of genetic variants (i.e., single nucleotide polymorphisms (SNPs)) on structures or functions of human brain. In recent studies, univariate or multivariate regression analysis methods are typically used to capture the effective associations between genetic variants and quantitative traits (QTs) such as brain imaging phenotypes. The identified imaging QTs, although associated with certain genetic markers, may not be all disease specific. A useful, but underexplored, scenario could be to discover only those QTs associated with both genetic markers and disease status for revealing the chain from genotype to phenotype to symptom. In addition, multimodal brain imaging phenotypes are extracted from different perspectives and imaging markers consistently showing up in multimodalities may provide more insights for mechanistic understanding of diseases (i.e., Alzheimer's disease (AD)). In this work, we propose a general framework to exploit multi-modal brain imaging phenotypes as intermediate traits that bridge genetic risk factors and multi-class disease status. We applied our proposed method to explore the relation between the well-known AD risk SNP APOE rs429358 and three baseline brain imaging modalities (i.e., structural magnetic resonance imaging (MRI), fluorodeoxyglucose positron emission tomography (FDG-PET) and F-18 florbetapir PET scans amyloid imaging (AV45)) from the Alzheimer's Disease Neuroimaging Initiative (ADNI) database. The empirical results demonstrate that our proposed method not only helps improve the performances of imaging genetic associations, but also discovers robust and consistent regions of interests (ROIs) across multi-modalities to guide the disease-induced interpretation.

Abstract:Concomitant neuropsychiatric symptoms (NPS) are associated with accelerated Alzheimer's disease (AD) progression. Identifying multimodal brain imaging patterns associated with NPS may help understand pathophysiology correlates AD. Based on the AD continuum, a supervised learning strategy was used to guide four-way multimodal neuroimaging fusion (Amyloid, Tau, gray matter volume, brain function) by using NPS total score as the reference. Loadings of the identified multimodal patterns were compared across the AD continuum. Then, regression analyses were performed to investigate its predictability of longitudinal cognition performance. Furthermore, the fusion analysis was repeated in the four NPS subsyndromes. Here, an NPS-associated pathological-structural-functional covaried pattern was observed in the frontal-subcortical limbic circuit, occipital, and sensor-motor region. Loading of this multimodal pattern showed a progressive increase with the development of AD. The pattern significantly correlates with multiple cognitive domains and could also predict longitudinal cognitive decline. Notably, repeated fusion analysis using subsyndromes as references identified similar patterns with some unique variations associated with different syndromes. Conclusively, NPS was associated with a multimodal imaging pattern involving complex neuropathologies, which could effectively predict longitudinal cognitive decline. These results highlight the possible neural substrate of NPS in AD, which may provide guidance for clinical management.

Abstract:Graphical, voxel, and region-based analysis has become a popular approach to studying neurodegenerative disorders such as Alzheimer's disease (AD) and its prodromal stage [mild cognitive impairment (MCI)]. These methods have been used previously for classification or discrimination of AD in subjects in a prodromal stage called stable MCI (MCIs), which does not convert to AD but remains stable over a period of time, and converting MCI (MCIc), which converts to AD, but the results reported across similar studies are often inconsistent. Furthermore, the classification accuracy for MCIs vs. MCIc is limited. In this study, we propose combining different neuroimaging modalities (sMRI, FDG-PET, AV45-PET, DTI, and rs-fMRI) with the apolipoprotein-E genotype to form a multimodal system for the discrimination of AD, and to increase the classification accuracy. Initially, we used two well-known analyses to extract features from each neuroimage for the discrimination of AD: whole-brain parcelation analysis (or region-based analysis), and voxel-wise analysis (or voxel-based morphometry). We also investigated graphical analysis (nodal and group) for all six binary classification groups (AD vs. HC, MCIs vs. MCIc, AD vs. MCIc, AD vs. MCIs, HC vs. MCIc, and HC vs. MCIs). Data for a total of 129 subjects (33 AD, 30 MCIs, 31 MCIc, and 35 HCs) for each imaging modality were obtained from the Alzheimer's Disease Neuroimaging Initiative (ADNI) homepage. These data also include two APOE genotype data points for the subjects. Moreover, we used the 2-mm AICHA atlas with the NiftyReg registration toolbox to extract 384 brain regions from each PET (FDG and AV45) and sMRI image. For the rs-fMRI images, we used the DPARSF toolbox in MATLAB for the automatic extraction of data and the results for REHO, ALFF, and fALFF. We also used the pyClusterROI script for the automatic parcelation of each rs-fMRI image into 200 brain regions. For the DTI images, we used the FSL (Version 6.0) toolbox for the extraction of fractional anisotropy (FA) images to calculate a tract-based spatial statistic. Moreover, we used the PANDA toolbox to obtain 50 white-matter-region-parcellated FA images on the basis of the 2-mm JHU-ICBM-labeled template atlas. To integrate the different modalities and different complementary information into one form, and to optimize the classifier, we used the multiple kernel learning (MKL) framework. The obtained results indicated that our multimodal approach yields a significant improvement in accuracy over any single modality alone. The areas under the curve obtained by the proposed method were 97.78, 96.94, 95.56, 96.25, 96.67, and 96.59% for AD vs. HC, MCIs vs. MCIc, AD vs. MCIc, AD vs. MCIs, HC vs. MCIc, and HC vs. MCIs binary classification, respectively. Our proposed multimodal method improved the classification result for MCIs vs. MCIc groups compared with the unimodal classification results. Our study found that the (left/right) precentral region was present in all six binary classification groups (this region can be considered the most significant region). Furthermore, using nodal network topology, we found that FDG, AV45-PET, and rs-fMRI were the most important neuroimages, and showed many affected regions relative to other modalities. We also compared our results with recently published results.

Abstract:Alzheimer's disease (AD) has become a severe medical challenge. Advances in technologies produced high-dimensional data of different modalities including functional magnetic resonance imaging (fMRI) and single nucleotide polymorphism (SNP). Understanding the complex association patterns among these heterogeneous and complementary data is of benefit to the diagnosis and prevention of AD. In this paper, we apply the appropriate correlation analysis method to detect the relationships between brain regions and genes, and propose "brain region-gene pairs" as the multimodal features of the sample. In addition, we put forward a novel data analysis method from technology aspect, cluster evolutionary random forest (CERF), which is suitable for "brain region-gene pairs". The idea of clustering evolution is introduced to improve the generalization performance of random forest which is constructed by randomly selecting samples and sample features. Through hierarchical clustering of decision trees in random forest, the decision trees with higher similarity are clustered into one class, and the decision trees with the best performance are retained to enhance the diversity between decision trees. Furthermore, based on CERF, we integrate feature construction, feature selection and sample classification to find the optimal combination of different methods, and design a comprehensive diagnostic framework for AD. The framework is validated by the samples with both fMRI and SNP data from ADNI. The results show that we can effectively identify AD patients and discover some brain regions and genes associated with AD significantly based on this framework. These findings are conducive to the clinical treatment and prevention of AD.

Abstract:Alzheimer's disease (AD) is associated with heterogeneous atrophy patterns. We employed a semi-supervised representation learning technique known as Surreal-GAN, through which we identified two latent dimensional representations of brain atrophy in symptomatic mild cognitive impairment (MCI) and AD patients: the "diffuse-AD" (R1) dimension shows widespread brain atrophy, and the "MTL-AD" (R2) dimension displays focal medial temporal lobe (MTL) atrophy. Critically, only R2 was associated with widely known sporadic AD genetic risk factors (e.g., APOE ε4 ) in MCI and AD patients at baseline. We then independently detected the presence of the two dimensions in the early stages by deploying the trained model in the general population and two cognitively unimpaired cohorts of asymptomatic participants. In the general population, genome-wide association studies found 77 genes unrelated to APOE differentially associated with R1 and R2. Functional analyses revealed that these genes were overrepresented in differentially expressed gene sets in organs beyond the brain (R1 and R2), including the heart (R1) and the pituitary gland, muscle, and kidney (R2). These genes were enriched in biological pathways implicated in dendritic cells (R2), macrophage functions (R1), and cancer (R1 and R2). Several of them were "druggable genes" for cancer (R1), inflammation (R1), cardiovascular diseases (R1), and diseases of the nervous system (R2). The longitudinal progression showed that APOE ε4 , amyloid, and tau were associated with R2 at early asymptomatic stages, but this longitudinal association occurs only at late symptomatic stages in R1. Our findings deepen our understanding of the multifaceted pathogenesis of AD beyond the brain. In early asymptomatic stages, the two dimensions are associated with diverse pathological mechanisms, including cardiovascular diseases, inflammation, and hormonal dysfunction—driven by genes different from APOE —which may collectively contribute to the early pathogenesis of AD. Abstract:Unbiased data-driven omic approaches are revealing the molecular heterogeneity of Alzheimer disease. Here, we used machine learning approaches to integrate high-throughput transcriptomic, proteomic, metabolomic, and lipidomic profiles with clinical and neuropathological data from multiple human AD cohorts. We discovered 4 unique multimodal molecular profiles, one of them showing signs of poor cognitive function, a faster pace of disease progression, shorter survival with the disease, severe neurodegeneration and astrogliosis, and reduced levels of metabolomic profiles. We found this molecular profile to be present in multiple affected cortical regions associated with higher Braak tau scores and significant dysregulation of synapse-related genes, endocytosis, phagosome, and mTOR signaling pathways altered in AD early and late stages. AD cross-omics data integration with transcriptomic data from an SNCA mouse model revealed an overlapping signature. Furthermore, we leveraged single-nuclei RNA-seq data to identify distinct cell-types that most likely mediate molecular profiles. Lastly, we identified that the multimodal clusters uncovered cerebrospinal fluid biomarkers poised to monitor AD progression and possibly cognition. Our cross-omics analyses provide novel critical molecular insights into AD.

Machine Learning Based on the Multimodal Connectome Can Predict the Preclinical Stage of Alzheimer’s Disease: a Preliminary Study

Abstract:OBJECTIVES:Subjective cognitive decline (SCD) may be a preclinical stage of Alzheimer's disease (AD). Neuroimaging studies suggest that abnormal brain connectivity plays an important role in the pathophysiology of SCD. However, most previous studies focused on single modalities only. Multimodal combinations can more effectively utilize various information and little is known about their diagnostic value in SCD.METHODS:One hundred ten SCD individuals and well-matched healthy controls (HCs) were recruited in this study (the primary sample: 35 SCD and 36 HC; the validation sample: 21 SCD and 18 HC). Multimodal imaging data were used to construct functional, anatomical, and morphological networks, respectively. These networks were used in combination with a multiple kernel learning-support vector machine to predict SCD individuals. We validated our model on another independent sample. Multiple linear regression (MLR) analyses were conducted to investigate the relationships among network metrics, cognition, and pathological biomarkers.RESULTS:We found that the characteristics identified from the multimodal network were primarily located in the default mode network (DMN) and salience network (SN), achieving an accuracy of 88.73% (an accuracy of 79.49% for an independent sample) based on the integration of the three modalities. MLR analyses showed that increased AV45 SUVRs were significantly associated with impaired memory function, the enhanced functional connectivity, and the decreased morphological connectivity.CONCLUSION:This study suggests that abnormal multimodal connections within DMN and SN can be used as effective biomarkers to identify SCD and provide insight into understanding the pathophysiological mechanisms underlying SCD.KEY POINTS:• Multimodal brain networks improve the detection accuracy of SCD. • Abnormal connections within DMN and SN can be used as effective biomarkers for the identification of SCD.

Abstract:Alzheimer's disease (AD) is a neurodegenerative disease that affects the elderly and leads to cognitive decline and memory loss. Treatments for stopping or slowing the progression of AD have not been discovered yet; therefore, delaying the progression of AD is the only option, which makes early diagnosis of AD crucial. Additionally, although plaques and tau proteins are considered the causes of early AD, few studies have used this information to diagnose early AD. In this study, a middle-fusion multimodal model is proposed for the diagnosis of early AD. The proposed multimodal model extracts features without loss using a depthwise separable convolution block without an activation function. Subsequently, middle fusion is applied using mix skip connection and sharing weight convolution blocks, both designed to learn the complex relationships between modalities. In contrast to other studies, the proposed approach has three main novelties. 1) A middle-fusion multimodal model is proposed for the early diagnosis of AD. 2) The proposed model is evaluated using the entire ADNI series, including T1-weighted magnetic resonance imaging (T1w MRI) and 18F-FluoroDeoxyGlucose positron emission tomography (FDG PET) from the ADNI1 dataset, as well as PET and tau protein PET from ADNI2 and ADNI3 datasets. 3) A novel region-of-interest (ROI) extraction method is proposed for the hippocampus, middle temporal, and inferior temporal regions, which are known to be affected in the early stages of AD. In the experimental results, the proposed multimodal model achieved a balanced accuracy of 1.00, for the task of Alzheimer's disease vs cognitive normal (CN) and 0.76 for the task of mild cognitive impairment vs cognitive normal.

Abstract:Abstract Alzheimer’s disease (AD) is a devastating neurological disorder primarily affecting the elderly. An estimated 6.2 million Americans age 65 and older are suffering from Alzheimer’s dementia today. Brain magnetic resonance imaging (MRI) is widely used for the clinical diagnosis of AD. In the meanwhile, medical researchers have identified 40 risk locus using single-nucleotide polymorphisms (SNPs) information from Genome-wide association study (GWAS) in the past decades. However, existing studies usually treat MRI and GWAS separately. For instance, convolutional neural networks are often trained using MRI for AD diagnosis. GWAS and SNPs are frequently used to identify genomic traits. In this study, we propose a multi-modal AD diagnosis neural network that uses both MRIs and SNPs. The proposed method demonstrates a novel way to use GWAS findings by directly including SNPs in predictive models. We test the proposed methods on the Alzheimer’s Disease Neuroimaging Initiative dataset. The evaluation results show that the proposed method improves the model performance on AD diagnosis and achieves 93.5% AUC and 96.1% AP, respectively, when patients have both MRI and SNP data. We believe this work brings exciting new insights to GWAS applications and sheds light on future research directions.

Abstract:Alzheimer's disease is a progressive neurological disorder characterized by cognitive impairment and memory loss. With the increasing aging population, the incidence of AD is continuously rising, making early diagnosis and intervention an urgent need. In recent years, a considerable number of teams have applied computer-aided diagnostic techniques to early classification research of AD. Most studies have utilized imaging modalities such as magnetic resonance imaging (MRI), positron emission tomography (PET), and electroencephalogram (EEG). However, there have also been studies that attempted to use other modalities as input features for the models, such as sound, posture, biomarkers, cognitive assessment scores, and their fusion. Experimental results have shown that the combination of multiple modalities often leads to better performance compared to a single modality. Therefore, this paper will focus on different modalities and their fusion, thoroughly elucidate the mechanisms of various modalities, explore which methods should be combined to better harness their utility, analyze and summarize the literature in the field of early classification of AD in recent years, in order to explore more possibilities of modality combinations.

An interpretable generative multimodal neuroimaging-genomics framework for decoding Alzheimer's disease

Abstract:Alzheimer's disease (AD) is the most prevalent form of dementia with a progressive decline in cognitive abilities. The AD continuum encompasses a prodormal stage known as Mild Cognitive Impairment (MCI), where patients may either progress to AD or remain stable. In this study, we leveraged structural and functional MRI to investigate the disease-induced grey matter and functional network connectivity changes. Moreover, considering AD's strong genetic component, we introduce SNPs as a third channel. Given such diverse inputs, missing one or more modalities is a typical concern of multimodal methods. We hence propose a novel deep learning-based classification framework where generative module employing Cycle GANs was adopted to impute missing data within the latent space. Additionally, we adopted an Explainable AI method, Integrated Gradients, to extract input features relevance, enhancing our understanding of the learned representations. Two critical tasks were addressed: AD detection and MCI conversion prediction. Experimental results showed that our model was able to reach the SOA in the classification of CN/AD reaching an average test accuracy of $0.926\pm0.02$. For the MCI task, we achieved an average prediction accuracy of $0.711\pm0.01$ using the pre-trained model for CN/AD. The interpretability analysis revealed significant grey matter modulations in cortical and subcortical brain areas well known for their association with AD. Moreover, impairments in sensory-motor and visual resting state network connectivity along the disease continuum, as well as mutations in SNPs defining biological processes linked to amyloid-beta and cholesterol formation clearance and regulation, were identified as contributors to the achieved performance. Overall, our integrative deep learning approach shows promise for AD detection and MCI prediction, while shading light on important biological insights.

Using Multi-Scale Genetic, Neuroimaging and Clinical Data for Predicting Alzheimer’s Disease and Reconstruction of Relevant Biological Mechanisms

Abstract:Alzheimer’s Disease (AD) is among the most frequent neuro-degenerative diseases. Early diagnosis is essential for successful disease management and chance to attenuate symptoms by disease modifying drugs. In the past, a number of cerebrospinal fluid (CSF), plasma and neuro-imaging based biomarkers have been proposed. Still, in current clinical practice, AD diagnosis cannot be made until the patient shows clear signs of cognitive decline, which can partially be attributed to the multi-factorial nature of AD. In this work, we integrated genotype information, neuro-imaging as well as clinical data (including neuro-psychological measures) from ~900 normal and mild cognitively impaired (MCI) individuals and developed a highly accurate machine learning model to predict the time until AD is diagnosed. We performed an in-depth investigation of the relevant baseline characteristics that contributed to the AD risk prediction. More specifically, we used Bayesian Networks to uncover the interplay across biological scales between neuro-psychological assessment scores, single genetic variants, pathways and neuro-imaging related features. Together with information extracted from the literature, this allowed us to partially reconstruct biological mechanisms that could play a role in the conversion of normal/MCI into AD pathology. This in turn may open the door to novel therapeutic options in the future.

Abstract:Alzheimer's disease (AD) is a debilitating neurodegenerative disorder with a global impact, yet its pathogenesis remains poorly understood. While age, metabolic abnormalities, and accumulation of neurotoxic substances are potential risk factors for AD, their effects are confounded by other factors. To address this challenge, we first utilized multi-omics data from 87 well phenotyped AD patients and generated plasma proteomics and metabolomics data, as well as gut and saliva metagenomics data to investigate the molecular-level alterations accounting the host-microbiome interactions. Second, we analyzed individual omics data and identified the key parameters involved in the severity of the dementia in AD patients. Next, we employed Artificial Intelligence (AI) based models to predict AD severity based on the significantly altered features identified in each omics analysis. Based on our integrative analysis, we found the clinical relevance of plasma proteins, including SKAP1 and NEFL, plasma metabolites including homovanillate and glutamate, and Paraprevotella clara in gut microbiome in predicting the AD severity. Finally, we validated the predictive power of our AI based models by generating additional multi-omics data from the same group of AD patients by following up for 3 months. Hence, we observed that these results may have important implications for the development of potential diagnostic and therapeutic approaches for AD patients.

Abstract:INTRODUCTION: Previous studies have applied normative modeling on a single neuroimaging modality to investigate Alzheimer Disease (AD) heterogeneity. We employed a deep learning-based multimodal normative framework to analyze individual-level variation across ATN (amyloid-tau-neurodegeneration) imaging biomarkers. METHODS: We selected cross-sectional discovery (n = 665) and replication cohorts (n = 430) with available T1-weighted MRI, amyloid and tau PET. Normative modeling estimated individual-level abnormal deviations in amyloid-positive individuals compared to amyloid-negative controls. Regional abnormality patterns were mapped at different clinical group levels to assess intra-group heterogeneity. An individual-level disease severity index (DSI) was calculated using both the spatial extent and magnitude of abnormal deviations across ATN. RESULTS: Greater intra-group heterogeneity in ATN abnormality patterns was observed in more severe clinical stages of AD. Higher DSI was associated with worse cognitive function and increased risk of disease progression. DISCUSSION: Subject-specific abnormality maps across ATN reveal the heterogeneous impact of AD on the brain.

Abstract:Alzheimer's disease (AD) is a major neurodegenerative disease and the most common cause of dementia. Currently, no treatment exists to slow down or stop the progression of AD. There is converging belief that disease-modifying treatments should focus on early stages of the disease, that is, the mild cognitive impairment (MCI) and preclinical stages. Making a diagnosis of AD and offering a prognosis (likelihood of converting to AD) at these early stages are challenging tasks but possible with the help of multimodality imaging, such as magnetic resonance imaging (MRI), fluorodeoxyglucose (FDG)-positron emission topography (PET), amyloid-PET, and recently introduced tau-PET, which provides different but complementary information. This article is a focused review of existing research in the recent decade that used statistical machine learning and artificial intelligence methods to perform quantitative analysis of multimodality image data for diagnosis and prognosis of AD at the MCI or preclinical stages. We review the existing work in 3 subareas: diagnosis, prognosis, and methods for handling modality-wise missing data-a commonly encountered problem when using multimodality imaging for prediction or classification. Factors contributing to missing data include lack of imaging equipment, cost, difficulty of obtaining patient consent, and patient drop-off (in longitudinal studies). Finally, we summarize our major findings and provide some recommendations for potential future research directions.

Abstract:Alzheimer's disease (AD) is a significant neurological disorder characterized by progressive cognitive decline and memory loss. One essential task is understanding the molecular mechanisms underlying brain disorders of AD. Detecting biomarkers that contribute significantly to the classification of AD is an effective means to accomplish this essential task. However, most machine learning methods used to detect AD biomarkers require lengthy training and are unable to rapidly and effectively detect AD biomarkers. To detect biomarkers for AD accurately and efficiently, we proposed a novel approach using the Multi-Kernel Support Vector Machine (SVM) with Apriori algorithm to mine strongly associated feature sets from functional magnetic resonance imaging (fMRI) and gene expression profiles. Firstly, we downloaded the imaging data and genetic data of 121 participants from the Alzheimer's Disease Neuroimaging Initiative (ADNI) and transformed gene sequences into labeled sequences by encoding the four types of bases (A, T, C, and G) into distinct labels. Subsequently, we extracted the first 130 temporal sequences of brain regions and employed Pearson correlation analysis to construct "brain region gene pairs". The integration of these data allowed us to explore the correlations between genes and brain regions. To improve classification accuracy and feature selection, we applied the Apriori algorithm to the multi-kernel SVM, dynamically building feature combinations and continuously validating classification results. By iteratively generating frequent itemsets, we obtained important brain region gene pairs. Experimental results show the effectiveness of our proposed approach. The Multi-Kernel SVM with Apriori model achieves an accuracy of 92.9%, precision of 95%, and an F1 score of 95% in classifying brain region-gene pairs within the AD–Late mild cognitive impairment (AD-LMCI) group. The amygdala, BIN1, RPN2, and IL15 associated with AD have been identified and demonstrate potential in identifying potential pathogenic factors of AD. The selected brain regions and associated genes may serve as valuable biomarkers for early AD diagnosis and better understanding of the disease's molecular mechanisms. The integration of fMRI and gene data using the Multi-Kernel SVM–Apriori model holds great potential for advancing our knowledge of brain function and the genetic basis of neurological disorders. This approach provides a valuable tool for neuroscientists and researchers in the field of genomics and brain imaging studies.
