Abstract:OBJECTIVE To develop a bacteria expression system to produce the fusion protein of humanized anti-HBsAg scFV and interferon-alpha. METHODS The expression vector was constructed after cleaving the plasmids harboring the humanized anti-HBsAg scFv and interferon alpha respectively and ligating to linearized pET22b subsequence. The expression of fusion protein in E.coli was analyzed by SDS-PAGE. The binding activity and antiviral activity of the fusion protein was characterized by competing inhibition test and cytopathic effect reduction. RESULTS The plasmid harboring the in frame arranged fusion gene was constructed and identified. After induction for 12h, a new band close to 4.5 10(4) was observed using SDS-PAGE. Results of competing ELISA and cytopathic effect reduction showed the fusion protein retained its specific binding activity and antiviral activities. CONCLUSIONS The construction and expression of the fusion gene of humanized anti-HBsAg scFv and interferon in E.coli are successful.

Abstract:Objective: To investigate the correlation between cone-beam CT (CBCT) morphological stage of midpalatal suture (MPS) and cervical vertebral maturation stage (CS) of Chinese children and youth. Methods: This study was a retrospective observational study. A total of 1 076 CBCT images (male: 401, female: 675, age≥ 6.0 years old and<21.0 years old) taken from January 2013 to April 2018 in Peking University School and Hospital of Stomatology were included. MPS and CS of each sample were recorded according to previous studies. MPS included five stages: MPS 1, MPS 2, MPS 3, MPS 4s1 (substage), MPS 4s2 (substage), and MPS 5. CS included six stages: CS 1-6. Correlation between MPS and CS (ordinal categorical variables) was investigated by Spearman correlation coefficient and diagnostic test analysis. Results: Spearman coefficient between CS and MPS was 0.803. Diagnostic pairs with the best diagnostic performance include: CS 6-MPS 5, CS 5-MPS 4s2, CS 1-MPS 1, CS 4-MPS 4s1, CS 2-MPS 1, CS 3-MPS 3. The 90.9% (309/340) of individuals at CS 1-3 were at MPS 1-3 and 9.1% (31/340) were at MPS 4s1. The 33.0% (107/324) of individuals at CS 4 were at MPS 3 and 46.6% (151/324) were at MPS 4s1. Conclusions: CS has a close relationship with MPS. Individuals at CS 4 are recommended to take pre-treatment CBCT, so as to estimate feasibility of skeletal expansion with traditional tooth-borne rapid maxillary expansion.
Establishment of an Intelligent Cervical Vertebrae Maturity Assessment System Based on Cone Beam CT Data.

Abstract:To establish an intelligent cervical vertebra maturity assessment system, and to evaluate the reliability and clinical value of the system. Sixty children aged were recruited in the study. Lateral cephalometric radiograph and cone beam CT (CBCT) were taken at the same period. Based on the CBCT data, the system automatically extracted the patient's facial area through Otsu's method, intercepted the sagittal plane by three-dimensional least squares method, captured the second to fourth cervical vertebrae by superpixel segmentation. And then selected points were marked automatically through morphological algorithm and manual method. Consistency test was performed on the two sets of data to compare the reliability of automated cervical morphology capture. According to the parameters of morphological identification, positioning and staging algorithms were designed to form the intelligent cervical vertebra maturity assessment system. The cervical vertebra maturity was also judged manually on the lateral cephalometric radiograph. The weighted Kappa test and the Gamma correlation coefficient were subsequently applied to evaluate the consistency and correlation. The results showed that the cervical vertebra features automatically captured based on CBCT data had a high accuracy on the overall morphological recognition. In the prediction of 8 inflection points out of 13 points, there was no significant difference between automatic and manual method on both X and Y axes (all >0.05). The assessment results of the cervical vertebra maturity of the intelligent system had strong consistency and correlation with the manual recognition results (weighted Kappa value=0.877, Gamma value=0.991, both <0.05). The intelligent cervical vertebrae maturity assessment system based on CBCT data established in this study presents reliable outcome and high degree of automation, indicating that the system may be used clinically.

Abstract:Abstract Background (1) Establish an improved morphological staging method of midpalatal suture. (2) Investigate the correlation between morphological stages of midpalatal suture (MPS stage) and cervical vertebrate maturation (CVM) stages. (3) Compare effects of traditional tooth-borne rapid maxillary expansion (RME) at different MPS stages. Methods 1076 CBCT images (675 females, 401 males, 6.0 to 21.0 years old) are included. MPS stages are classified based on observation of horizontal view. Correlation between MPS stage and CVM stage are investigated by Spearman correlation coefficient and diagnostic test analysis. Effects of traditional tooth-borne RME at different MPS stages were analyzed based on pre- and post-expansion dental casts. Results Five stages of midpalatal suture are established. Spearman coefficient between MPS stage and CVM stage is 0.803. Diagnostic pairs with the largest area under ROC curve (AUC) value are: CS 6 - MPS 5,CS 5 - MPS 4s2,CS 1 - MPS 1, CS 4 - MPS 4s1, CS 2 - MPS 1, CS 3 - MPS 3. 309/340 (90.9%) of individuals at CS 1-3 are at MPS 1-3, 31/340 (9.1%) are at MPS 4s1. 107/324 (31.2%) of individuals at CS 4 are at MPS 3, 151/324 (46.6%) are at MPS 4s1. Conclusion (1) An improved morphological staging method of midpalatal suture is established. (2) MPS stages have close relationship with CVM stages. (3) Pre-treatment CBCT is recommended for individuals at CS 4 to estimate the possibility of skeletal expansion with traditional tooth-borne RME.
Chinese Morphological Stages of Midpalatal Suture and Its Correlation with Demirjian Dental Age

Abstract:OBJECTIVE:To investigate the correlation between morphological stages of midpalatal suture (MPS) and Demirjian dental age. METHODS:In this retrospective study, 1 076 cone-beam CT (CBCT) images (female: 675, male: 401; age range: 6.0 to 21.0 years) were included. Horizontal view of each sample's CBCT images was observed, each sample's MPS stage was recorded, and dental age. MPS stage and dental age were ordered with categorical variables. Therefore, their correlation was investigated through Spearman correlation coefficient analysis and diagnostic test analysis. RESULTS:(1) For left lower second premolar: 95.2% of those in dental age stage B-D were in MPS 1-2, accounting for the largest proportion. 85.3% of those in dental age stage E were in MPS 1-2, still accounting for the largest proportion. Another 14.7% were in MPS 3.45.1% of those in dental age stage F were in MPS 3, 46.1% in MPS 1-2, and another 8.8% in MPS 4s1.49.8% of those in dental age stage G were in MPS 3.24.9% in MPS 4s1, and 18.9% in MPS 1-2.80.1% of those in dental age stage H were in MPS 4-5. Another 16.3% were in MPS 3. (2) For left lower second molar: 89.7% of those in dental age stage B-D were in MPS 1-2, accounting for the largest proportion. 67.4% of those in dental age stage E were in MPS 1-2, still accounting for the largest proportion. Another 26.1% were in MPS 3.55.3% of those in dental age stage F were in MPS 3, 34.2% in MPS 1-2, and another 10.5% in MPS 4s1.50.7% of those in dental age stage G were in MPS 3, 24.3% in MPS 4s1, and 16.8% in MPS 1-2.83.8% of those in dental age stage H were in MPS 4-5, another 14.2% were in MPS 3. (3) To diagnose MPS stage with dental age, diagnostic pairs with good performance included: Dental age of left lower second molar-MPS: H-4s2, H-5, D-1; Dental age of left lower second premolar-MPS: H-4s2, H-5, G-3. Other diagnostic pairs were of ordinary diagnostic efficiency. (4) For dental age-MPS Spearman correlation analysis, dental age of left lower second molar-MPS had the highest Spearman coefficient (0.68), dental age of left lower second premolar-MPS was the second high (0.64). (5) Dental age stage H of left lower second molar or left lower second premolar indicated that the individual was later than MPS 4s2. CONCLUSION:Dental age's diagnostic efficiency for MPS stage is ordinary on the whole, except for some pairs with good performance. Therefore, pre-treatment CBCT examination should be considered as assistance for evaluating maturation and fusion status of midpalatal suture.
Correlation of spheno-occipital synchondrosis and mandibular condylar cortication with chronological age using computed tomography in Indian population- A cross-sectional study

Abstract:Background: Forensic age estimation is a procedure which utilises many methods to estimate the age of both living and deceased individuals, including those who have died in natural disasters or man-made catastrophes. The pattern and closure of spheno-occipital synchondrosis (SOS) fusion, along with subchondral ossification of the mandibular condyle, can be used to estimate age. Aim and objectives: This study aims to estimate age using computed tomographic (CT) images of spheno-occipital synchondrosis fusion (SOS) and mandibular condylar cortication (MCC), and to correlate these findings with chronological age. Materials and methods: The present study included 435 CT images of individuals aged 10-25 years. SOS fusion was assessed using a four-stage system, and MCC was assessed bilaterally using a three-stage system on the sagittal plane. Data on fusion stages and cortication types were entered along with chronological age, and then statistically analysed. Results: SOS fusion stage 2 occurred at similar age in males (19.82 ± 2.67 years) and females (19.23 ± 2.93 years). Earlier fusion of other stages was observed in females by a mean age of 2 years. MCC was completed 1 year earlier in females, with statistically significant differences (p ≤ 0.001). When comparing cortication types and different fusion stages, only type II cortication showed statistically significant differences compared to different fusion stages (p ≤ 0.001). Conclusion: Mandibular condylar cortication (MCC) and spheno-occipital synchondrosis (SOS) fusion were positively correlated with chronological age, suggesting that these parameters can be used as an adjunct method for age estimation.
Study on the correlation between the structure of TMJ and vertical craniofaciai pattern, occlusion in Class Ⅱ1 Children

Abstract:Objective:To investigate the relationship between the structure of TMJ and the craniofacial pattern,occlusion in ClassⅡ1mandibular retrusion children. Methods:TMJ MRI,lateral cephalometric were taken respectively from 30 ClassⅡ1 patients (14 boys and 16 girls,the average age was 10.8±1.1 years). The condyle position and disc position were measured. The correlation of condyle and disc position with craniofacial pattern and occlusion were analyzed metrically and compared with pearson test.Results:There were low positive correlation between MP-SN angle and anterial space. No correlation between posterior-anterior ratio and other data were found. The posterior facial height and Dp-Cc/Cs-Cc angle had low positive correlation. The anterior facial height and Dp-Cc/Cs-Cc、Ca-Ca'、Ca-Cp showed low negative correlation,and little correlation was found between condyle position and overbite and overjet. Overbite had no correlation with disc-condyle-fossa relationship. However,The positive correlation was found between the overjet. and Ca-Dm and Dp-Cc/Cs-Cc,and Pearson coefficient is 0.420 and 0.460.Conclusion:Vertical craniofacial patter has relation to forward displacement of condyle and backward displacement of disc. A deeper overjet is related with a higher risk of abnormal disc-condyle-fossa relationship.
Correlation Assessment Between Three-Dimensional Facial Soft Tissue Scan and CBCT Data in Orthodontic Diagnosis for Skeletal Class Ⅲ Adolescents

Abstract:Objective The aim of the current study was to investigate correlations between measurements on 3D facial soft tissue scan and CBCT skeletal structures.Methods The study sample consisted of 20 subjects living in Beijing area.A 3D face scan analysis including two distance values and 22 angle values based on 14 landmarks,and the corresponding CBCT analysis with three distance values and 9 angle values were performed for all subjects.All measurements were operated twice by the same researcher.Then,the reproducibility was evaluated by intraclass correlation coefficient (ICC),and the correlations were evaluated by Pearson correlation coefficient analysis.Besides,the thickness of facial soft tissue at relative points to explore possible reasons between the correlations.Results The reproducibility of measurements related to soft tissue gonion is medium.For sagittal measurement,medium correlations were found between soft tissue subspinale and hard tissue point A,as well as the relative position of maxilla and mandible.For vertical measurements,soft tissue gonions,mandibular plane angles showed medium correlations with hard tissue,and medium correlation were found between anterior upper and lower facial heights with corresponding hard tissue measurements and hard tissue mandibular plane angle.And the thickness of facial soft tissue at subspinale was larger than that at alare.Conclusions Soft tissue measurements of alare,the relative position between maxilla and mandible,upper anterior facial height and lower anterior facial height are reliable in providing a soft tissue diagnosis reference to the CBCT analysis.While the measurements related to soft tissue gonion should be used with caution.

Abstract:Objectives This study aimed to assess the relative growth rates (RGRs) of the maxilla and mandible at varying fusion stages of the spheno‐occipital synchondrosis (SOS), thereby elucidating the potential of SOS stages in predicting maxillomandibular growth. Materials and Methods A total of 320 subjects (171 boys and 149 girls), aged 6 to 18 years, were retrospectively included. Each subject had a minimum of two longitudinal cone‐beam computed tomography (CBCT) images, with no more than one interval of SOS fusion stage change between the two scans. Subjects were categorized based on their SOS fusion stages and genders. The RGRs of the maxilla and mandible at various SOS fusion stages were measured and compared using longitudinal CBCT images. Results Significant statistical differences were observed in maxillomandibular RGRs across various SOS fusion stages. Abstract:Background: Temporomandibular joint (TMJ) is a ginglymo-diarthroidial joint with fibroelastic cartilage. The chondrogenesis initiates from the 12th week of intrauterine life and the development of condyle is associated with growth. The condylar cortication shows distinct morphological variation for each individual in each stage of their life. The cortical bone around the condyle could be used as a factor for chronological age assessment and it can act as a tool in forensic medicine. Aim and Objective: The study was carried out to evaluate the cortical grading in mandibular condyle using two different applications and to correlate their grades with chronological age. Setting and Design: Hospital-based retrospective observational cross-sectional study. Materials and Methods: The study was carried out in 40 patients and 80 TMJs were assessed for cortication grades in Carestream 3D imaging and Image J applications. These grading from both the applications were correlated with the chronological age. Statistical Analysis: SPSS (Statistical Analysis for the Social Science) – Cohen's Kappa inter-examiner reliability and Spearman's correlation coefficient were used. Results: The radiological assessment of condylar cortication in individual application showed significant results and the relationship of cortication with chronological age showed a significant correlation. Conclusion: The condylar cortication grading is a simple technique and can be used as a factor for chronological age assessment. This is an initial study which used two different applications to view the cortication of the mandibular condyle and to correlate the cortication with chronological age. Hence, a large sample size-based study is required for further research.
Correlation Between the Extension of the Maxillary Sinus Floor and Malocclusion in Adolescent Patients Using a Cone-Beam Computed Tomography

Abstract:Abstract Background: The correlation between extension of the maxillary sinus floor and vertical facial skeletal patterns is important for designing orthodontic treatment plans. We correlated the extension of the maxillary sinus (MS) floor with different facial skeletal malocclusion in adolescent patients aged 10-19 years old using a cone-beam computed tomography. Methods: The relationship between the root tips of upper posterior teeth and the sinus floor was typed and scored (0-4) using cone-beam computed tomography (CBCT) images. Scores of each tooth and each patient were formulated.The cephalometric radiographs were analyzed with Dolphin software to diagnose patients' facial skeletal types into three groups according to Frankfort mandibular plane angle (FMA) as high-angle, average-angle and low-angle groups and ANB angle as Class I, Class II and Class III groups separately. The influences caused by age, sex, and facial skeletal malocclusion on the tooth and patient score were analyzed. Results: In high-angle group, the first molar (FM) score was significantly higher than that in the average-angle and low-angle groups (P = 0.018). Age has a positive effect on the patient score and some of the tooth scores, such as the second premolar (SPM) score, FM score, and the second molar (SM) score, and in the skeletal facial malocclusion. With increasing age, SM score increased along with the Frankfort mandibular plane angle (FMA). No significant difference was found in the distribution of patient and tooth scores by ANB angles. Conclusion: The relationship between the maxillary posterior roots and the sinus floor is correlated with facial skeletal malocclusion in 10- to 19-year-old adolescents. More first molar root tips were inside the sinus in the high-angle group compared with the average-angle and low-angle skeletal patterns. With advancing age, more second molar root tips were more close to contact the sinus floor in the high-angle group. Consideration of the root-sinus position in the posterior maxillary area before establishing an orthodontic treatment plan is recommended.

Abstract:Introduction This study aimed to evaluate the midpalatal suture (MPS) maturation stages using the cone-beam computed tomography classification method in Chinese children aged 5-10 years, adolescents aged 11-15 years, and postadolescents aged 16-20 years and identify a correlation between maturation stage and age and sex. Methods Axial sections of tomographic images from 717 participants (369 female and 348 male participants) aged 5-20 years were used to classify the maturation stage of the MPS (stages A, B, C, D, and E). Kappa statistics were used to evaluate the measurement error. The chi-square test was applied to analyze the differences in the distribution of MPS stages by age group and by sex among all participants, as well as the adolescent group. The Fisher exact test was employed to assess the differences in MPS stage distribution by sex among children aged 5-10 years and among the postadolescent group. The Mann-Whitney U test was used to assess the potential variance in age distribution between stages C and D. Results The most prevalent maturation stage was stage C (40.3%). Of the total population, 69.4% had MPS in stages A, B, or C. A significant difference in age distribution was observed between stage C and stage D ( P <0.001). The distribution of the MPS maturation stage significantly varied by age group ( P <0.001) and sex ( P <0.001). Conclusions The distribution of participants in advanced maturation stages increases with age. Female patients generally experience earlier MPS maturation than male patients, particularly between the ages of 11-20 years.

Abstract:Objective: To use cone beam computed tomography (CBCT) scans to analyze mandibular symmetry, the occlusal plane and their correlations in patients with high-angle skeletal class III malocclusion and jaw deformity. Design: Reconstructed images simulated of CBCT data of 20 adult patients with high-angle skeletal class III malocclusion and jaw asymmetry (Experimental group) and 20 adult patients of skeletal class I with normal faces (Control group) were measured with a three-dimensional (3D) geometric morphological measurement system. Related landmarks, lines and planes were marked and relevant distances and angles of 3D craniofacial structures were measured. The Student t-test was performed to assess symmetry. With the help of the Pearson correlation and Linear regression, we explored the correlations and other relationships among them. Results: Compared with control group, the symmetrical differences in experimental group were concentrated in condyle, coracoid process, mandibular angle, mandibular first molar area and the chin (P < 0.05). A strong positive correlation was found between the degree of mandibular deviation and the inclination of the occlusal plane in the coronal position (r = 0.860, P < 0.001) and a linear relationship between them was found: Y = 1.300 + 0.419X (the inclination of occlusal plane was Y, and the mandibular deviation was X). Conclusions: Certain characteristics of mandibular symmetry and the occlusal plane were found in patients with high-angle skeletal class III malocclusion and jaw asymmetry. These findings provide references for doctors to pay attention to the degree and location of mandibular asymmetry and to determine the occlusal plane.
Evaluation of Cervical Vertebral Maturation Angular Measurements as a Tool to Assess Skeletal Maturity: A Radiological Study

Abstract:Aim: The purpose of the present research was to evaluate the comparison between the Skeletal Maturation phases of Fishman's hand-wrist skeletal maturation index (SMI) and cervical vertebral maturation angle (CVMA). Methodology: Assessment and comparison of Skeletal Maturation utilizing the hand-wrist bone radiograph by Fishman's system of skeletal maturation indicator (SMI), cervical vertebra maturation stages (CVMS), and lateral cephalogram by cervical vertebral maturation angle (CVMA) conducted on 150 subjects (75 male and 75 female). Chi-square was used to reveal the significance of study parameters on a categorical scale between SMI, CVMS, CVMA, and multiple regression analysis conducted to envisage the connotation of the dependent variable. Results: Multiple regression analysis models exhibited 61.9% of the variance for CVMA with autonomous variables. In multiple regression analysis, there is no substantial association (P > 0.05) between age and CVMA. The correlation of SMI and CVMA values utilizing Pearson's correlation coefficient for males and females presented positive association with R value of 0.801 for males and 0.834 for females and a P value of < 0.001. Conclusion: As compared to other maturity indicators like hand and wrist, cervical vertebral angular measurements were reliable enough for assessing the degree of skeletal maturation.

Abstract:Introduction: Lateral cephalograms are routinely used in orthodontics for accurate assessment and treatment planning of malocclusion. Paranasal air sinuses, such as the maxillary and frontal sinuses, are visible in the lateral cephalogram. However, a direct correlation between the area of these sinuses and the tendency to develop skeletal discrepancies has not been studied in the literature. Aim: To determine the cephalometric correlation between different skeletal malocclusions and the dimensions of the frontal and maxillary sinuses. Materials and Methods: A cross-sectional cephalometric study was conducted at a Tertiary Care Centre in Siliguri district of West Bengal, India. The duration of the study was two years, from August 2019 to September 2021. A total of 150 patients were included and they were divided into three equal groups: skeletal class I (n=50), skeletal class II (n=50), and skeletal class III (n=50). Multiple cephalometric parameters were traced on the radiographs using Dolphin and AutoCAD software, and they were compared among the three groups. The data were analysed using Pearson’s correlation test. Results: The results showed a statistically significant correlation between Maxillary Sinus Area (MSA) and Gonial angle (r=0.468, p=0.001) in skeletal class I malocclusion. In skeletal class II malocclusion, a significant negative correlation was found between Frontal Sinus Area (FSA) and total mandibular length (r=-0.30, p=0.009). However, no significant negative correlation was found between MSA and any cephalometric parameter in skeletal class II malocclusion. Conclusion: The present study highlights there was an increase in the size of the maxillary and frontal sinuses was observed in skeletal class II and skeletal class III malocclusion. However, no correlation could be found between skeletal class I malocclusion and the areas of the frontal sinus.
The Cervical Vertebral Maturation (CVM) Method for the Assessment of Optimal Treatment Timing in Dentofacial Orthopedics

Abstract:The present study introduces a further modified version of the Cervical Vertebral Maturation (CVM) method for the detection of the peak in mandibular growth, based on the analysis of the second through fourth cervical vertebrae in a single cephalogram. The morphology of the bodies of the second (C2 –odontoid process), third (C3), and fourth (C4) cervical vertebrae were analyzed in 6 consecutive cephalometric observations (T1 through T6) of 30 orthodontically untreated subjects. Observations for each subject consisted of two consecutive cephalograms comprising the interval of maximum mandibular growth (as assessed by means of the maximum increment in total mandibular length, Condylion –Gnathion: Co-Gn), together with two earlier consecutive cephalograms and two later consecutive cephalograms. The analysis consisted of both visual and cephalometric appraisals of morphological characteristics of the three cervical vertebrae. The construction of this new modified version of the CVM method was based on the results of both ANOVA for repeated measures with post hoc Scheffé’s test (P < 0.05) and discriminant analysis. The new clinically improved CVM method is comprised of six maturational stages (cervical stage 1 through cervical stage 6, ie, CS1 through CS6). CS1 and CS2 are prepeak stages; the peak in mandibular growth occurs between CS3 and CS4. CS6 is recorded at least 2 years after the peak. The use of the CVM method enables the clinician to identify optimal timing for the treatment of a series of dentoskeletal disharmonies in all three planes of space.

Abstract:The association between craniocervical posture and craniofacial structures in the various sagittal skeletal malocclusion during different growth stages has been the focus of intense interest in fields of orthodontics, but it has not been conclusively demonstrated. Thus, this study aimed to investigate the association between craniofacial morphology and craniocervical posture in patients with sagittal skeletal malocclusion during different growth periods. A total of 150 from a large pool of cephalograms qualified for the inclusion and exclusion were evaluated and classified into three groups according to the Cervical Vertebral Maturation (CVM) by examining the morphological modifications of the second through fourth cervical vertebrae, each group consisted of 50 cephalograms. In each growth period, for the comparison of head and cervical posture differences among various skeletal classes, the radiographs were further subdivided into skeletal Class I (0° < ANB < 5°, n = 16), skeletal Class II (ANB ≥ 5°, n = 18), and skeletal Class III (0° ≤ ANB, n = 16) on the basis of their ANB angle. There was no significant difference in gender ( P > 0.05). Some variables were found to be significant during pubertal growth and later in patients with sagittal skeletal malocclusion ( P < 0.05). Most indicators describing craniocervical posture were largest in skeletal Class II and smallest in skeletal Class III during the peak growth periods and later. Cervical inclination variables were greater in skeletal Class III than in skeletal Class II. Variables of craniofacial morphology and craniocervical posture are more correlated during the pubertal growth period and later in patients with sagittal skeletal malocclusion. A tendency is an indication of the close interrelationship that a more extended head was in skeletal Class II while a flexed head was in skeletal Class III. Nevertheless, with the considerations of some limitations involved in this study, further longitudinal studies with large samples are required to elucidate the relationship clearly.

Abstract:OBJECTIVES: To investigate the reliability of regional three-dimensional registration and superimposition methods for assessment of temporomandibular joint condylar morphology across subjects and longitudinally.METHODS: The sample consisted of cone beam CT scans of 36 patients. The across-subject comparisons included 12 controls, mean age 41.3 ± 12.0 years, and 12 patients with temporomandibular joint osteoarthritis, mean age 41.3 ± 14.7 years. The individual longitudinal assessments included 12 patients with temporomandibular joint osteoarthritis, mean age 37.8 ± 16.7 years, followed up at pre-operative jaw surgery, immediately after and one-year post-operative. Surface models of all condyles were constructed from the cone beam CT scans. Two previously calibrated observers independently performed all registration methods. A landmark-based approach was used for the registration of across-subject condylar models, and temporomandibular joint osteoarthritis vs control group differences were computed with shape analysis. A voxel-based approach was used for registration of longitudinal scans calculated x, y, z degrees of freedom for translation and rotation. Two-way random intraclass correlation coefficients tested the interobserver reliability.RESULTS: Statistically significant differences between the control group and the osteoarthritis group were consistently located on the lateral and medial poles for both observers. The interobserver differences were ≤0.2 mm. For individual longitudinal comparisons, the mean interobserver differences were ≤0.6 mm in translation errors and 1.2° in rotation errors, with excellent reliability (intraclass correlation coefficient >0.75).CONCLUSIONS: Condylar registration for across-subjects and longitudinal assessments is reliable and can be used to quantify subtle bony differences in the three-dimensional condylar morphology.

Abstract:The purpose of this study was to establish a quantitative cervical vertebral maturation (CVM) system for adolescents with normal occlusion. Methods: Mixed longitudinal data were used. The subjects included 87 children and adolescents from 8 to 18 years old with normal occlusion (32 boys, 55 girls) selected from 901 candidates. Sequential lateral cephalograms and hand-wrist films were taken once a year for 6 years. The lateral cephalograms of all subjects were divided into 11 maturation groups according to the Fishman skeletal maturity indicators. The morphologic characteristics of the second, third, and fourth cervical vertebrae at 11 developmental stages were measured and analyzed. Results: Three characteristic parameters (H4/W4, AH3/PH3, @2) were selected to determine the classification of CVM. With 3 morphologic variables, the quantitative CVM system including 4 maturational stages was established. An equation that can accurately estimate the maturation of the cervical vertebrae was established: CVM stage = -4.13 + 3.57 x H4/W4 + 4.07 x AH3/PH3 + 0.03 x @2. Conclusions: The quantitative CVM method is an efficient, objective, and relatively simple approach to assess the level of skeletal maturation during adolescence. Correlation between cervical vertebral maturation and chronological age in a group of Iranian females

Abstract:BACKGROUND: Correlation between chronological age at different stages of cervical vertebral maturation (CVM) is important in clinical orthodontic practice. The objective of this study was to evaluate the correlation between CVM stage and chronological age in a group of Iranian female patients.MATERIALS AND METHODS: This study was conducted on 196 digital lateral cephalometry of female patients with the age ranged 9-14 years. The CVM stage was determined with two calibrated examiners, using the method developed by Baccetti and its correlation with mean chronological age was assessed by the Spearman rank-order. The intra and inter-agreements were evaluated by weighted Kappa statistics in overall diagnosis of stages, in addition to determination of presence or absent of concavities at the lower border of second, third and fourth cervical vertebrae and the shapes of the third and fourth vertebrae. P < 0.05 was considered as significant.RESULTS: The correlation coefficient between CVM stages and chronological age was relatively low (r = 0.62). The least amount of inter-observer agreement was determined to be at the clinical decision of the shape of the fourth vertebra.CONCLUSION: Regarding the low reported correlation, the concomitant usage of other skeletal indicators seems necessary for precise determination of physiological age of the patients.
