Abstract:Cryptosporidium is an obligate intracellular parasite reported from all over the world. This protozoan infects a wide range of animals as well as humans. Cryptosporidium parvum and Cryptosporidium hominis are the most prevalent infecting species with mild and self-limiting infection in healthy people. The protozoan oocyst is resistant to common water purifiers. Based on emerging evidence, Cryptosporidium is one of waterborne parasites considered a major public health problem in developing and developed countries. In this study, 42 samples were collected from 14 rivers in the catchment area of Lake Urmia in northwest of Iran. Moreover, amplification of SSU rRNA gene was performed, and polymerase chain reaction products were sequenced. The results of sequencing and comparing the sequences with those in the GenBank revealed that all the 17 positive samples were C. parvum, a zoonotic species and one of the most frequent human-infecting species. Considering these data, it is highly important to inhibit the spread of this protozoan by treating livestock and preventing human and animal effluents from entering the water.

Abstract:Intestinal protozoan pathogens cause significant diarrhoeal diseases in children. However, to date, there has been limited genetic study of the intestinal pathogens Cryptosporidium, Giardia and Enterocytozoon in humans in China, with the exception of research in a small number of cities/provinces. In the present study, PCR-based tools were used to detect and characterise these protistan parasites from 500 children with a history of diarrhoea in Wuhan and environs, Hubei province, China. Genomic DNAs from faecal samples were screened for the particular protists by PCR utilising regions in the small subunit (SSU) of the nuclear ribosomal RNA, the 60kDa glycoprotein (gp60), the internal transcribed spacer of nuclear ribosomal DNA (ITS) and/or the triose phosphate isomerase (tpi) genes as markers. Cryptosporidium meleagridis subtype IIIb (10/500, 2.0%), Giardia duodenalis assemblage A (7/500, 1.4%) and Enterocytozoon bieneusi genotype D (1/500, 0.2%) were identified in small percentages of the 500 samples. No significant gender- or age-associated differences in the prevalence of Cryptosporidium and Giardia infections were found. Future studies might focus on the occurrence of these protists in children as well as animals, with an emphasis on Cryptosporidium meleagridis in pets and agriculturally important birds, in different parts of Hubei province.

Abstract:Purpose: To investigate the response to intravitreal ranibizumab after failure of intravitreal bevacizumab in patients with diabetic macular edema (DME). Methods: Demographics, visual acuity (VA), central macular thickness (CMT), and HbA1C were retrospectively collected from DME patients treated with second-line intravitreal ranibizumab at a tertiary hospital in 2012-2013 and followed for at least 3 months. Results: Twenty-two patients (26 eyes) were included in the study, with a mean (±SD) age of 66 ± 8.1 years and followed for an average of 28.36 months. The mean number of intravitreal bevacizumab injections was 7.3 ± 2.8, and of intravitreal ranibizumab injections 5.11 ± 2.4. After 3 ranibizumab injections, 57% of eyes showed improvement in VA. The change in VA was statistically significant (p = 0.044) in those eyes where the pretreatment acuity for the second-line therapy was <20/40 (logMAR 0.3). CMT decreased from 435.95 ± 83.28 to 373.69 ± 44.39 µm (p = 0.01). The number of ranibizumab injections was significantly correlated with the change in CMT (p = 0.037). Conclusion: Intravitreal treatment with ranibizumab can be efficacious in eyes with DME that have failed to respond to bevacizumab.

Abstract:Cryptosporidiosis is considered to be one of the most devasting gastrointestinal diseases in calves. The aim of this study was to investigate Cryptosporidium parvum infection (C. parvum) in buffalo-calves with both copro-microscopic examination and enzyme linked immunosorbent assay (ELISA) using two C. parvum prepared antigens with regards to their cytokines profile; interferon- γ (IFN-γ), interleukin (IL)-12 and IL-14 to achieve a proper diagnosis. All collected buffalo- calves' fecal samples were examined by modified Ziehl–Neelsen staining technique. ELISA was performed to evaluate the diagnostic accuracy of the two C. parvum prepared antigens; crude whole oocyst (CWO) and crude sonicated oocyst (CSO) in detection of anti-C. parvum IgG in buffalo-calves' sera. As well, concentrations of INF-γ, IL-12 and IL-14 in the buffalo-calves' serum samples were estimated. The results revealed that the overall parasitological incidence of cryptosporidiosis was 40%. However, the serological diagnosis by ELISA assay showed 53.75% and 27.5% when using CWO and CSO antigen, respectively. Also, the diagnostic efficacy parameters of both antigens; CWO and CSO showed a significant high specificity (83.3%) achieved by CSO antigen and a high sensitivity (71.8%) by CWO antigen. The levels of INF-γ, IL-12 and IL-14 were significantly increased in positive Cryptosporidium infected group by both coprological and serological assays followed by the group which was positive for cryptosporidiosis by copro-microscopic examination only. The present study concluded that a combination of coprological and serological examination with reference to the cytokines profile is needed for proper diagnosis of cryptosporidiosis in buffalo-calves.

Abstract:Cryptosporidium parvum is an important zoonotic parasite that causes significant economic loss in the animal husbandry industry, especially the cattle industry. As there is no specific vaccine or drug against Cryptosporidium, a rapid and accurate method for the detection of C. parvum is of great significance. In this study, colloidal gold strips were developed based on Cryptosporidium parvum virus 1 (CSpV1) for the detection of C. parvum infection in cattle fecal samples. The colloidal gold solution was prepared by reducing trisodium citrate and the CSpV1 #5 monoclonal antibody was labeled with colloidal gold. A polyclonal antibody against the CSpV1 capsid protein and an anti-mouse IgG antibody were coated on the colloidal gold strips for use in the test and control lines, respectively. Our results showed that the detection sensitivity in fecal samples was up to a 1:64 dilution. There was no cross-reaction with Cryptosporidium andersoni or Giardia in the fecal samples. The different preservation conditions (room temperature, 4°C, and 37°C) and preservation time (7, 30, 60, and 90 days) were analyzed. The data showed that the strips could be preserved for 90 days at 4°C and for 60 days at room temperature or 37°C. The colloidal gold strips were used to detect the samples of 120 clinical fecal in Changchun, China. The results indicated that the rate of a positive test was 5% (6/120). This study provides a rapid and accurate method for detecting C. parvum infection in cattle and humans.

Abstract:Cryptosporidium, a worldwide protozoan parasite, is one of the most common causes of diarrhoea in humans and animals. The aim of the present study was to determine Cryptosporidium species/genotypes in pre-weaned calves in Shaanxi Province using PCR and sequencing based on the small subunit rRNA gene. A total of 258 faecal samples were collected from pre-weaned calves in 19 different farms from six areas in Shaanxi Province, north-western China. Cryptosporidium infection was detected in 14 of 19 farms (73.7 %), with a total prevalence of 20.2% (52/258). Both dairy and Qinchuan (beef) cattle were found with Cryptosporidium infection. Three Cryptosporidium species, namely Cryptosporidium bovis (n=26), Cryptosporidium andersoni (n=14) and Cryptosporidium ryanae (n=12), were detected in pre-weaned calves in Shaanxi Province, with C. bovis (in 12 farms) identified as the most common species on cattle farms. Two additional and previously unknown C. ryanae genotypes, CRTypes III and IV, were observed in the present study. However, the zoonotic species, Cryptosporidium parvum, was not detected in this study, which suggested a low zoonotic potential in Cryptosporidium-infected pre-weaned calves in this province.

Abstract:Cryptosporidium spp. are diarrheagenic intestinal parasites with multiple hosts worldwide. A total of 1252 fresh fecal samples of sheep were collected from 10 large-scale farms in southern Xinjiang. Based on the small subunit ribosomal ( SSU rRNA) gene of Cryptosporidium , 100 Cryptosporidium -positive samples (8.0%, 100/1252) were detected by PCR. Nine out of 10 farms were positive for Cryptosporidium , with the highest infection rate being 18.4% (23/125) on farm 9 in Qira. The infection rates of Cryptosporidium in pre-weaned lambs, weaned lambs, fattening sheep, and adult sheep were 20.3% (61/301), 10.3% (34/329), 0.9% (3/327), and 0.7% (2/295), respectively. Three Cryptosporidium species were identified, namely, C. xiaoi ( n = 61), C. parvum ( n = 22), and C. ubiquitum ( n = 17). Of them, C. xiaoi was detected on all positive farms and in different age groups of sheep. The subtypes of C. parvum and C. ubiquitum were identified by PCR at the 60 kDa glycoprotein ( gp60 ) gene. Two C. parvum subtypes were identified: IIdA19G1 ( n = 21) and IIdA15G1 ( n = 1). One C. ubiquitum subtype was identified with XIIa ( n = 17). These results indicated the common transmission and genetic diversity of Cryptosporidium in sheep in southern Xinjiang, and further investigations are needed on the zoonotic potential of C. parvum and C. ubiquitum in this region.

Study on Seroprevalence and Leptospiral Antibody Distribution Among Livestock Breeders and Farmers in Golestan Province

Abstract:Background: The high prevalence of leptospirosis in humans is of great public health concern, particularly in tropical and subtropical regions. Objective: This study aimed to determine the seroprevalence of leptospiral antibodies and the distribution of serovars in livestock breeders and farmers in Golestan province. Methods: Seventy samples of serum collected from farmers and ranchers suspected of leptospirosis were examined using an ELISA method for surveying Immunoglobulin M (IgM) anti-leptospiria. Also from samples, DNA was extracted and PCR was performed using by primers for 16s rRNA. Demographic properties of positive patients were analysed. Results: Chi-square statistical test shows a statistically significant difference between the gender and prevalence leptospirosis (p-value = 0.004). Also, by examining the age, it was shown that 68.57% of patients are in the middleaged rang. According to the results obtained from the study and investigation of blood serum IgM-and Immunoglobulin G (IgG) in people suspected of leptospirosis, 3 cases of the patients had IgM higher than 11 and were known to be positive for leptospirosis. Also, by examining the IgG level of patients, 5 cases had intermediate results, and 2 cases were found to be positive for IgG. The PCR results showed that 41.42% of patients tested positive for the 16s rRNA. Conclusion: Leptospirosis is a common disease among farmers, and in Golestan province, considering traditional farming methods, it is considered an important infectious disease. Therefore, health and safety measures should be expanded to control and prevent this disease. Also, by employing mechanised agricultural methods, the prevalence of leptospirosis in this region can be significantly reduced.
Prevalence and Molecular Characterization of Cryptosporidium spp., Giardia duodenalis, and Enterocytozoon bieneusi in Diarrheic and Non-Diarrheic Calves from Ningxia, Northwestern China

Abstract:Cryptosporidium spp., Giardia duodenalis, and Enterocytozoon bieneusi are significant parasitic gastrointestinal pathogens with global distribution in humans and domestic animals, including calves. The main symptoms of calf infection are severe diarrhea, dehydration, growth retardation, and sometimes even death. To date, there has been limited information on the prevalence of Cryptosporidium spp., G. duodenalis, and E. bieneusi infections in calves in Ningxia, China, especially between diarrheic and non-diarrheic calves. A total of 438 fecal samples were collected from diarrheic (201) and non-diarrheic (237) calves in Ningxia. PCR and DNA sequencing were used to find the prevalence of Cryptosporidium spp. at 46.8% (205/438), G. duodenalis at 16.9% (74/438), and E. bieneusi at 10.0% (44/438). The prevalence of Cryptosporidium spp. infection in diarrheic and non-diarrheic calves was 54.0% (128/237) and 38.3% (77/201), respectively, and statistical analysis showed a positive correlation between the prevalence of Cryptosporidium spp. infection and calf diarrhea (p 0.05). Furthermore, four known Cryptosporidium species were successfully identified by comparing them with SSU rRNA gene sequences, including C. parvum, C. bovis, C. ryanae, and C. andersoni. In addition, all 74 G. duodenalis-positive samples were identified as assemblage E by comparative analysis of bg gene sequences. Among the 44 E. bieneusi-positive samples sequenced in the present study, 4 distinct E. bieneusi genotypes were successfully identified by comparative analysis of ITS sequences, including 3 known genotypes (J, BEB4, and N) and 1 novel genotype, the latter of which was identified and designated as NX1. These findings indicated that the high genetic diversity and complex population structures of Cryptosporidium spp., G. duodenalis, and E. bieneusi in Ningxia diarrhea calves and non-diarrhea calves, which provide new data for understanding the epidemiological status of Cryptosporidium spp., G. duodenalis, and E. bieneusi in Ningxia calves.

Abstract:This study was designed to determine the prevalence of Cryptosporidium infections in sheep herds around tributaries in Pakistan and to evaluate the influence of potential risk factors. Four hundred forty-eight samples from 26 sheep herds were screened microscopically and then confirmed through PCR amplification of the 18S SSU rRNA gene. Our result revealed that out of 448 samples, 107 were found positive by microscopy and 77 positive by PCR, with prevalence rates of 23.88% and 17.18% in different breeds of sheep. A high statistical difference exists between the P-value (P <0.05) when comparing diarrheic and non-diarrheic sheep, breeds, ages, genders, and seasons. The highest infection rate was found in the Australian breed (8.25%); season-wise, the highest prevalence was recorded in summer. Furthermore, sheep-secreting diarrhoeal faces were 41% young sheep and lambs. This reveals that young animals are more susceptible to infection than adults, and cryptosporidial infection decreases with increasing age of the animal. This is the first report on the prevalence of Cryptosporidium in sheep sampled near the tributaries in Pakistan. The implications of this study's findings are clear; immediate action is necessary to prevent further spread and potentially devasting harm.

Abstract:Cryptosporidiosis, primarily caused by Cryptosporidium parvum , is a significant cause of diarrhea in pre-weaned dairy calves. To investigate the prevalence of Cryptosporidium among pre-weaned diarrheic dairy calves and identify potential sources of infection in northern China, 234 fecal samples from 18 farms in six regions were analyzed for Cryptosporidium . Furthermore, 217 bedding samples from both occupied and unoccupied calf hutches, heating lamp pens, and individual calving pens in eight farms in Beijing were also examined for the presence of the parasite. All samples were screened for Cryptosporidium spp. using nested PCR targeting the SSU rRNA gene fragment, and C. parvum was subtyped with nested PCR targeting the 60 kDa glycoprotein gene. The prevalence of Cryptosporidium was 33.3%, with C. parvum and C. bovis constituting 29.9% and 3.4% of cases, respectively. The positive rate of Cryptosporidium in 1- to 4-week-old calves ranged from 9.6 to 63.6%. Analysis of the gp60 fragment of C. parvum revealed four subtypes: IIdA15G1, IIdA17G1, IIdA19G1, and IIdA20G1. Besides the bedding samples in heating lamp pens, both C. parvum and C. bovis were detected in bedding samples throughout the other regions. A significant positive correlation between the detection rate of Cryptosporidium in fecal samples and that in the bedding materials of occupied calf hutches ( R = 0.93, P = 0.002). These findings suggest that C. parvum is the predominant species among pre-weaned diarrheic dairy calves in northern China. Contaminated bedding materials may act as sources of infection for newborn calves.

Abstract:This study was conducted with the aim of the molecular identification of the protozoan parasite Cryptosporidium spp. in calves in the early stage of their development on a dairy farm in Eastern Slovakia. Twenty-five Holstein and Holstein cross calves were included in the study and monitored from their birth to the fifth week of life (1–5 weeks). Fresh fecal samples were collected from the same group of calves each week, except during the fourth week, and with the exception of Sample 8. All samples were analyzed using the Ziehl–Neelsen staining method and coproantigen was tested using the ELISA test as the screening method. Using the ELISA method, the highest incidence of cryptosporidiosis was observed in the second week of life of the calves, while the antigen was detected in 21 (91.6%) calves. Using the Ziehl–Neelsen staining method, the highest incidence was also observed in the second week, with an incidence rate of 62.5%. Positive isolates confirmed by the ELISA test were molecularly characterized. The species and subtypes of Cryptosporidium in the positive isolates were identified using PCR and the sequence analysis of the small subunit of the ribosomal 18S RNA (ssu rRNA) and the 60 kDa glycoprotein (gp60) genes of the parasite. The sequence analysis of 29 isolates at the 18S rRNA loci confirmed the presence of two species—Cryptosporidium parvum and Cryptosporidium ryanae. Out of 29 isolates, 25 were assigned to the species C. parvum, with the gp60 locus identified as genotype IIaA17G1R1. Among the individual animal groups, calves are the most common reservoirs of the C. parvum zoonotic species. This disease has significant public health implications as contact with livestock and their feces and working with barn manure are major sources of infection, not only for other animals but also for humans.

Abstract:Cryptosporidium is a gastro-intestinal protozoan parasite that has been found to infect both humans and livestock. This study investigated the parasite in 998 fecal samples from Bangladeshi children ( n = 299) and calves ( n = 699) to determine its prevalence, genetic variation, and zoonotic importance. The nested PCR and sequencing of the SSU rRNA gene in the samples showed a Cryptosporidium infection rate of 2.3% (7/299) in children and 15.7% (110/699) in calves. Statistical analysis revealed insignificant variations in Cryptosporidium infections among children across age, gender, and study area, while in calves, the infection rate significantly differed based on location and breed. Genotyping of seven human isolates of Cryptosporidium confirmed C. hominis ( n = 5) and C. parvum ( n = 2). After characterizing 110 Cryptosporidium isolates from calves, C. andersoni ( n = 55), C. ryanae ( n = 29), C. bovis ( n = 14), C. parvum ( n = 10), C. ubiquitum (n = 1), and C. occultus (n = 1) were identified. Cryptosporidium hominis and C. parvum -positive samples were further subjected to nested PCR and sequencing of the glycoprotein 60 ( gp60 ) gene for subtyping. Four C. hominis subtypes (IaA19R3, IaA23R3, IbA9G3, and IdA15G1) and one C. parvum subtype (IIdA15G1) were observed. In conclusion, Cryptosporidium was prevalent in calves but less common in children in the study locations, and the presence of zoonotic Cryptosporidium species and subtypes in calves raises concerns regarding zoonotic transmission to humans.

Abstract:Objective: This study was conducted to molecularly identify and classify Cryptosporidium spp. in fecal samples (n=150) from patients with diarrhea received at the microbiology laboratory of a private hospital in Denizli. Methods: In this study, the positivity of Cryptosporidium spp. in fecal samples was investigated using direct microscopy, Kinyoun's acid-fast staining method, and Nested polymerase chain reaction (PCR) techniques. Positive PCR products were sequenced. Results: In the examined fecal samples of patients with diarrhea, no parasites were detected through direct microscopic examination. Using the Kinyoun acid-fast staining method, Cryptosporidium spp. was identified in 2.7% (n=4) of the samples, while Nested PCR detected it in 4.67% (n=7) of the samples. The four positive samples were sequenced using primers that amplify the 18S rRNA gene region. The sequencing results identified the isolates as C. parvum. Conclusion: Cryptosporidiosis is an important public health issue as it is a zoonotic disease caused by the Cryptosporidium parasite that can be transmitted from animals to humans. This study focuses on the molecular characterization of Cryptosporidium species detected in human fecal samples, which is significant for understanding which specific strains or species are involved in human infections. According to the findings, it is recommended that control measures be implemented to reduce the risk of exposure to Cryptosporidium in both humans and animals in Türkiye.
Diagnostic advancements: Isolating Mycobacterium avium ssp. paratuberculosis and unveiling its molecular identity with nested-PCR

Abstract:Mycobacterium avium subspecies paratuberculosis (MAP) is the causative agent of paratuberculosis, which is currently prevalent in many parts of Iran and produces severe economic loss. It is hence necessary to identify and isolate the animals infected with this bacterium, so this research aimed to isolate MAP from milk and fecal samples of ELISA-positive animals and determine the molecular identity of isolates. After performing ELISA on 3,700 bovine blood samples, 115 samples of milk and feces were taken from ELISA-positive cattle and were cultured on Herald's egg yolk medium with and without mycobactin-J and then the acid-fastness of positive samples was determined using Ziehl-Neelsen staining. The 16S rRNA-PCR test was performed after DNA extraction to determine the molecular identity of isolates. Primers IS6110 and IS901 were employed to ensure that the isolates were not related to members of M. tuberculosis complex and M. avium, respectively. Primer IS900 was also used to determine the molecular identity of MAP isolates. Also, expression levels of MAP-related genes (IS900, ISMAP02, F57, MAP2191, MAP4027) were evaluated via qPCR. Finally, positive samples were confirmed based on the Nested-PCR. Results showed that a total of 9 isolates were obtained from the culture of 90 ELISA-positive samples. The results revealed that all grown samples were positive for acid-fastness. The 16S rRNA-PCR test revealed the 543 bp band, which confirms the presence of Mycobacterium in the samples. The PCR test with Primer IS900 generated the 398 bp fragment in the first step and the 298 bp fragment in the second step, indicating the presence of MAP in samples. Also, relative expression analysis revealed that MAP-related genes were significantly higher in ELIZA-positive samples than in negative ones. Based on the study findings, it can be concluded that MAP-infected animals can be identified by ELISA. In addition, mycobacterium can be isolated by culturing the samples on appropriate media and then its molecular identity can be determined by using nested-PCR.

Characteristics of nonurgent emergency department use in the first 3 months of life

Abstract:Objectives To determine the characteristics of nonurgent emergency department (ED) visits in the first 3 months of life. Methods The study cohort consisted of full-term newborns admitted to and discharged from one newborn nursery from September 1, 1992, to May 1, 1994. All visits in the first 90 days of life to one large urban ED were analyzed to determine whether they were nonurgent, based on history of present illness and final diagnosis or disposition. The principal outcomes of interest were the frequency and pattern of nonurgent ED visits. Risk factors for nonurgent ED use were also studied. Results A total of 2137 patients with 965 ED visits were analyzed; 20.4% of the patients had nonurgent visits, and 60.1% of all visits were nonurgent. Of all patients with nonurgent visits, 24.1% had more than one. Younger maternal age, Medicaid, maternal parity, and nonwhite race all resulted in increased nonurgent ED use. One third of all ED visits were made when the primary care physician’s offices were open, and 57.6% of these visits were nonurgent. Conclusions Maternal and economic factors affected nonurgent ED utilization. Other critical factors still need to be explored. Interventions focused on decreasing nonurgent ED use in early infancy should be targeted at patients with the identified risk factors.

Abstract:Sarcocystis species are cyst-forming intracellular protozoan parasites. Cattle are mainly infected with Sarcocystis cruzi, Sarcocystis hominis and Sarcocystis hirsuta. Water buffaloes are intermediate hosts for Sarcocystis fusiformis, Sarcocystis levinei (S. cruzi-like species), Sarcocystis dubeyi, Sarcocystis sinensis (S. hominis-like species) and Sarcocystis buffalonis (S. hirsuta- like species). The aim of this study was Identification of Sarcocystis spp. in slaughtered cattle and water buffaloes in Ahvaz, Khuzestan province by PCR-restriction fragment length polymorphism. Meat inspection was done on 124 cattle and 147 water buffaloes. From each animal tissue samples (each 50 g) from heart, esophagus, diaphragm and intercostal muscle were collected during meat inspection. Samples examined with digestion method. Genomic DNA of 80 positive samples was extracted and their 18S rRNA gene was amplified. PCR products were digested by restricted enzymes (FokI, SspI and DraI). S. cruzi in cattle and S. fusiformis in water buffaloes were identified. Our study clarified that sarcocystosis in cattle in Ahvaz district may be results acute infection according to determined species, but in buffaloes as S. fusiformis was detected we may expect only economic loss follow up slaughterhouse inspection.
