Abstract:OBJECTIVES: There are many studies concerning sarcopenia prevalence from all over the world. However, to our knowledge, only two compared urban and rural sarcopenia and to date, none have been conducted in the Americas. The aim of this study was to evaluate the prevalence of sarcopenia in a convenience sample of women ≥60 y of age who underwent bone densitometry and live in urban and rural areas of southern Brazil.METHODS: This was a cross-sectional study comprising 205 women ≥60 y of age who had undergone bone densitometry. Sarcopenia was defined according to the criteria recommended by the European Working Group on Sarcopenia in Older People. The diagnosis combined the evaluation of muscle mass (assessed by dual-energy x-ray absorptiometry), muscle strength (measured using a manual digital dynamometer), and muscular performance (evaluated by the 4-m walking speed test). Sociodemographic data, smoking status, chronic conditions, number of falls and fractures in the past year, and level of physical activity also were collected.RESULTS: Sarcopenia was present in 2.4% of the total sample. Urban and rural populations significantly differed in terms of schooling (P < 0.001), occupation (P = 0.010), socioeconomic status (P = 0.001), and smoking status (P = 0.006). The environment in which the women lived was independently associated with sarcopenia (odds ratio, 9.561; 95% confidence interval, 1.021-89.523; P = 0.048). The prevalence of sarcopenia was significantly higher in the urban women than in the rural group (5.7 versus 0.7%, respectively; P = 0.047). After multivariate analysis, the environment of the women's residence remained independently associated with sarcopenia.CONCLUSION: Urban elderly women are more vulnerable to sarcopenia than rural elderly women.

Abstract:Objective: to identify the prevalence of sarcopenia in elderly primary care users, and the relationship of nutritional status with sarcopenia. Material and methods: the study was a cross-sectional study with elderly users of the primary health care network. The participants were evaluated for the diagnosis of sarcopenia, which involved calculation of muscle mass, grip strength, and walking speed, as well as sociodemographic, anthropometric and nutritional variables. Nutritional status was evaluated through the body mass index (BMI) and the Mini Nutritional Assessment (MNA) instrument. The study was conduted at eleven family health centers in Marau, Rio Grande do Sul, and included elderly subjects aged 65 years and over (n = 148). Results: of the elderly subjects evaluated, 72.3% were female; mean age was 73.6 years (SD: 5.5), ranging from 65 to 89 years. The prevalence of sarcopenia was 14.2%, 47.3% of the sample had low manual grip strength, and 53.7% had inadequate walking speed. Regarding BMI, 10.8% of participants were classified as underweight, and 75% of these were diagnosed with sarcopenia. Sarcopenia was significantly associated with the oldest age group (p = 0.046) and with higher BMI (p < 0.001). Conclusions: results show the importance of assessing nutritional status and a potential diagnosis of sarcopenia, mainly since this syndrome is highly associated with inappropriate food intake, which is often impaired among the elderly because of economical and/or physiological reasons.
Probable sarcopenia is associated with cognitive impairment among community-dwelling older adults: results from the FIBRA study

Abstract:ABSTRACT Background: The link between sarcopenia and cognitive impairment has not yet been thoroughly evaluated, especially among older adults. Objective: To evaluate the relationship between probable sarcopenia and cognitive impairment among community-dwelling older adults in two Brazilian cities. Methods: Probable sarcopenia was assessed using the EWGSOP2 (2018) criteria. Thus, participants were classified as probably having sarcopenia if they had SARC-F (Strength, Assistance in walking, Rise from a chair, Climb stairs and Falls) ≥4 points and low grip strength. Cognitive function was evaluated through the Mini-Mental State Examination (MMSE), verbal fluency (VF) and clock drawing test (CDT). Results: In a sample of 529 older adults (mean age 80.8±4.9 years; mean education 4.2±3.67 years; 70.1% women), 27.3% of the participants had SARC-F≥4, 38.3% had low grip strength and 13.6% were classified as probable sarcopenia cases. After adjusting for possible confounders (age, sex, education, depression, diabetes, hypertension, leisure-time physical activity and obesity), probable sarcopenia was found to be associated with impairment in the MMSE (OR 2.52; 95%CI 1.42‒4.47; p=0.002) and in VF (OR 2.17; 95%CI 1.17‒4.01; p=0.014). Low grip strength was found to be associated with impairment in the MMSE (OR 1.83; 95%CI 1.18‒2.82; p=0.006) and in the CDT (OR 1.79; 95%CI 1.18‒2.73; p=0.006). SARC-F scores were found to be associated with impairment in the MMSE (OR 1.90; 95%CI 1.18‒3.06; p=0.008). Conclusion: The results suggested that probable sarcopenia and its components present a significant association with cognitive deficits among community-dwelling older adults. Future longitudinal studies will further explore the causal relationship.

Abstract:Background Low muscle strength and mass with possible impaired physical performance characterizes the muscle disease known as sarcopenia. When not diagnosed or treated, the risk of falls, fractures, hospitalization, hospital costs, longer hospital stays, and negative outcomes increases. In the presence of comorbidities, such as cardiovascular diseases, sarcopenia can be significant and serious. In these patients, muscle weakness has attracted considerable attention in recent years since it is deemed an independent risk factor for disability and a strong predictor of premature death. The SARC-F (simple questionnaire to rapidly diagnose sarcopenia), a valid and consistent instrument to detect people at risk of adverse outcomes associated with sarcopenia, including functional decline, can be used as a screening tool. It consists of 5 items self-reported by patients based on their perceptions regarding strength limitations, the ability to walk, rise from a chair, climb stairs and a number of falls. Objectives Analyze the risk of sarcopenia and identify associated factors in hospitalized older adults with cardiovascular disease. Methods This is a cross-sectional study conducted with 23 hospitalized older adults with cardiovascular disease in a tertiary hospital of the Federal District, Brazil, classified into two groups: with or without risk of sarcopenia according to the SARC-F. Demographic (age and sex), clinical (number of medications and body mass index –BMI) and physical data (muscle strength and mass) were collected. Muscle strength was assessed using handgrip dynamometry and the Medical Research Council (MRC) scale, and mass by means of electrical bioimpedance. These data were compared between the groups using the student´s t and Mann-Whitney U tests. Results There was a risk of sarcopenia in 39.1% (n=9) of the sample. In comparison analyses, the group of older adults at risk of sarcopenia were older (mean difference: 14.39 years [95%CI 8.23 – 20.55]; p=0.001), exhibited lower handgrip strength (mean difference: 9.36 KgF [95%CI 0.874 – 17.854]; p=0.032), more frequent muscle weakness on the MRC scale (X²(1)=5.367, p=0.047) and lower appendicular muscle mass (mean difference: 0.763 [95%CI 0.113 – 1.414]; p=0.024). The groups showed no differences for sex, BMI and number of continuous use drugs (p>0.05). Conclusion This study found that older adults at risk of sarcopenia are older and obtain worse physical results when compared to their risk-free counterparts. The SARC-F was able to identify hospitalized patients with lower strength and muscle mass. Implications The findings show the need to identify hospitalized older adults at risk of sarcopenia using a simple, fast, low-cost, noninvasive assessment, which may contribute to the establishment of early identification strategies in a hospital setting, aimed at developing more assertive measures.

Abstract:In Brazil, sarcopenic obesity—a condition characterized by simultaneous muscle mass loss and excess fat accumulation—affects 17% of the population, with significant regional disparities. This systematic review and meta‐analysis highlights the urgent need for public health strategies to address this growing challenge. Aim Sarcopenic obesity (SO) is characterized by the coexistence of the loss of muscle mass and function with excess adipose tissue. Its prevalence has increased concomitantly with population aging, becoming one of the most significant challenges for public health, threatening the quality of life and the physical and mental health of the elderly population. Methods This study estimated the prevalence and factors associated with SO among adults and the elderly in Brazil's macro‐regions, utilizing MEDLINE, Embase, Web of Science, and the Virtual Health Library databases. Primary outcomes included SO prevalence overall and by subgroups, with secondary outcomes identifying associated factors. Results A systematic review of 33 studies with 10 266 participants up to July 2023 showed a 17% prevalence of SO (95% confidence interval: 13–21%), using a random effects meta‐analysis. The Central‐West region had the highest occurrence (27%). Dual‐energy X‐ray absorptiometry (26%) and bioimpedance (6%) were the top diagnostic methods. Protective factors were chronic kidney disease and healthy lifestyles; risk factors included socioeconomic status, functionality, lifestyle, biochemical parameters, and comorbidities such as osteoarthritis and apnea. Conclusions The prevalence of SO in Brazil is significant, emerging as a critical public health problem. It is essential to direct attention to changes in prevalence rates in the coming years, given the rising obesity trends and the absolute increase in the elderly population. Abstract:Background Sarcopenia and cognitive impairment are worrisome age and public health-related problems due to the high risk of functional decline, hospitalization and death. Sarcopenia is a muscle disease identified by clinical parameters of low muscle strength and muscle mass which, when added to poor physical performance, characterize severe sarcopenia. Cognitive impairment is the decline in normal functioning of one or more brain functions, affecting the activities of daily living (ADLs) of individuals, and may present as a mild or major cognitive disorder. Objectives To investigate the association of clinical parameters of sarcopenia with cognitive impairment in elderly people. Methods Cross-sectional study, with 263 elderly people (≥60 years old) users of a public specialized care service. Sociodemographic and clinical variables characterized the sample, and the clinical parameters of sarcopenia (strength, muscle mass and physical performance) were evaluated, respectively, using Handgrip Strength (HGS), calf circumference (CC) and the Timed Up and Go (TUG). The Mini Mental State Examination (MMSE) was used to assess cognitive status. Associations were investigated by simple and multiple linear and logistic regressions considering clinical parameters of sarcopenia (independent variables) and cognitive status (dependent variable), adjusted for age, gender, years of schooling, number of medications, nutritional status and functional capacity. Results Of the participants with cognitive impairment, 59.6% had low muscle strength. Cognitive status was explained by muscle strength in 21.5%, muscle mass in 12.3% and physical performance in 7.6% in simple linear regression analyses, maintaining strength and muscle mass as explanatory variables of the cognitive state in unadjusted multiple analyzes and only muscle strength when adjusted (OR=0,846; [95%CI: 0,774 – 0,924] p<0,001). Only strength remained significantly associated with cognitive status in the adjusted multiple logistic regression (OR=0.846; [95% CI: 0.774 – 0.924]; p<0.001). Conclusion Low muscle strength was the sarcopenia parameter independently associated with cognitive impairment. Implications This information is useful to pay attention to the probability of cognitive impairment when low muscle strength is identified in older people.

Prevalence and factors associated with sarcopenia among urban and rural Indian adults in middle age: A cross-sectional study from Western India

Abstract:Sarcopenia is the age-related loss of muscle mass and function. India has 8.6% of the global elderly (>60 years) population, and this is expected to increase to 20% by 2050. Around 70% of Indians live in rural areas where lifestyle factors like diet and physical activity differ from urban areas. Understanding age, sex and location-specific sarcopenia prevalence in India is crucial. Thus, our aim was to assess the prevalence and determinants of sarcopenia in urban and rural community-dwelling men and women aged 40 years and older, representing the next generation of older Indian adults. This cross-sectional study included 745 adults (400 women) from urban and rural areas near Pune, Western India. Assessments included socio-demography, diet by-24-hr recall, physical activity, anthropometry (height, weight), muscle mass measurement by dual-energy X-ray absorptiometry, muscle strength (hand grip) & muscle function by Short Physical Performance Battery (SPPB). Sarcopenia was defined by Asian Working Group on Sarcopenia-2019 guidelines Mean age of participants was 53±7.6yrs. Overall prevalence of sarcopenia was 10% and of severe sarcopenia was 4.2%. Sarcopenia prevalence was higher in rural (14.8%) than urban (6.8%) participants and in men (12.5%) than women (8%, all p<0.05). Muscle mass, grip strength and SPPB score were all higher in urban than rural participants (p<0.05). Older age, rural residence, inadequate protein intake, and lower socio-economic status were independently associated with sarcopenia. In this middle-aged group, sarcopenia prevalence was similar to that observed in older Western populations, over 100% higher among rural than urban participants, and higher amongst men than women. Age, location, protein intake and socioeconomic status were factors associated with sarcopenia. Given this rapidly increasing population of older adults in India there is an urgent need to plan strategies for early sarcopenia diagnosis and management, especially in rural populations.

Abstract:Abstract Introduction The SARC-F questionnaire can be rapidly implemented by clinicians to identify patients with probable sarcopenia. A score ≥4 is predictive of sarcopenia and poor outcome. We sought to identify the prevalence and demographic correlates of probable sarcopenia (SARC-F score ≥4) in community-dwelling older adults. Methods 480 participants (219 men, 261 women) identified from Primary Care completed a questionnaire ascertaining demographic, lifestyle factors, comorbidities, nutrition risk score (DETERMINE) and SARC-F score. Participant characteristics in relation to probable sarcopenia were examined using sex-stratified logistic regression. Age was included as a covariate. Results The median (lower quartile, upper quartile) age was 79.8 (76.9, 83.5) years. 12.8% of men and 23% of women had probable sarcopenia. Self-reported walking speed strongly associated with probable sarcopenia (men: odds ratio (OR) 10.39 (95% CI: 4.55, 23.72), p<0.001; women: 11.42 (5.98, 21.80), p<0.001 per lower band). Older age was associated with probable sarcopenia in both sexes (p=0.01) as was higher DETERMINE score (men: 1.30 (1.12, 1.51), p=0.001; women: 1.32 (1.17, 1.50), p<0.001 per unit increase). Among men, being married or in a civil partnership or cohabiting was protective against probable sarcopenia (0.39 (0.17, 0.89), p=0.03) as was reporting drinking any alcohol (0.34 (0.13, 0.92), p=0.03) while in women generally similar relationships were seen though these were weaker. Higher BMI (1.14 (1.07, 1.22), p<0.001 per unit increase) and presence of comorbidities (1.61 (1.34, 1.94), p<0.001 per extra medical condition) were also associated with probable sarcopenia in women. All associations were robust after adjustment for age. Conclusions Probable sarcopenia (SARC-F score ≥4) was common in older adults living in their own homes. As expected, self-reported walking speed was highly predictive of probable sarcopenia. In addition to advancing age and malnutrition, socio-demographic factors were also important. Identifying these factors in clinical practice should trigger sarcopenia screening in older adults.

Abstract:Background: The use of antiretrovirals has increased the survival of People Living with HIV/AIDS (PLWHA), resulting in an aging population and a rise in the incidence of sarcopenia. The lack of uniformity among the prevalences found in studies may be associated with the use of different diagnostic criteria, highlighting the need for local studies to determine the prevalence of sarcopenia. Methods: Cross-sectional study to estimate the prevalence and associated factors of sarcopenia using the revised criteria of the European Working Group on Sarcopenia in Older People (EWGSOP2). This study included PLWHA of both sexes, aged 40 years or older, who were treated at the infectious disease outpatient clinic of a tertiary hospital from 2019 to 2021. Muscle mass was quantified through electrical bioimpedance, using resistance and reactance to calculate appendicular lean mass (ALM) in kg/m². Muscle strength, measured in kg, was assessed using a manual dynamometer, and muscle function was evaluated using the gait speed test (m/s). Numerical variables were analyzed using measures of central tendency and dispersion. The chi-square test was used to assess associations in categorical variables. Odds ratios (OR) and 95% Confidence Intervals (CI) were calculated to evaluate the strength of associations. Results: Among the 218 PLWHA, the prevalence of sarcopenia was 8.7% (95% CI: 5.6 to 13.3). The mean age of the study population was 51.8 ± 8.3 years; 53.7% were male, 72.9% were brown/Black, 97.7% reported not using illicit drugs, and 24.8% were classified as obese. Multivariate analysis showed that the time since HIV diagnosis (P = 0.022) and the use of illicit drugs were associated with the diagnosis of sarcopenia. Conclusion: The prevalence of sarcopenia using the EWGSOP2 criteria was low. People with a longer duration of HIV infection and those using illicit drugs were more likely to develop sarcopenia.
Association of Sarcopenia With Performance on Multiple Cognitive Domains: Results From the ELSA-Brasil Study

Abstract:Abstract Background Sarcopenia and cognitive impairment share pathophysiological paths and risk factors. Our aim was to investigate the association of sarcopenia and its defining components with cognitive performance in middle-aged and older adults. Methods This cross-sectional analysis included 5,038 participants from the ELSA-Brasil Study, aged ≥ 55 years. Muscle mass was evaluated by bioelectrical impedance analysis and muscle strength by handgrip strength. Sarcopenia was defined according to the Foundation for the National Institutes of Health. Cognition was evaluated using delayed word recall test, semantic verbal fluency test, and trail making test version B. Possible confounders included sociodemographic characteristics, lifestyle, and clinical comorbidities. Results The frequencies of sarcopenia, low muscle mass, and low muscle strength were 1.8%, 23.3%, and 4.4%, respectively. After adjustment for possible confounders, poorer performance on the verbal fluency test was associated with sarcopenia (β = −0.20, 95% confidence interval [CI] = −0.38; −0.01, p = .03) and low muscle mass (β = −0.08, 95% CI = −0.14; −0.01, p = .02). Low muscle strength was associated with poorer performance in the delayed word recall test (β = −0.14, 95% CI = −0.27; −0.02, p = .02), verbal fluency test (β = −0.14, 95% CI = −0.26; −0.02, p = .03), and trail making test (β = −0.15, 95% CI = −0.27; −0.03, p = .01). Conclusions Sarcopenia was associated with poorer performance on the verbal fluency test, and low muscle strength was associated with poorer performance in all cognitive tests in middle-aged and older adults.
Prevalence and risk factors of primary sarcopenia in community-dwelling outpatient elderly: a cross-sectional study

Abstract:Abstract No previous study has investigated the prevalence and risk factors for primary sarcopenia in outpatient setting. This study aims to evaluate the prevalence and factors associated with primary sarcopenia in outpatient elderly. Additionally, we compared the severity of sarcopenia based on the 2014 and 2019 Asian Working Group for Sarcopenia (AWGS) criteria. This cross-sectional study was performed in 330 subjects aged over 60 years in an outpatient setting. The muscle strength, muscle performance and muscle mass were assessed using the handheld dynamometer, 6-m gait speed, and bioelectrical impedance analysis, respectively. The prevalence of sarcopenia was 10% as per the 2014 and 2019 AWGS criteria. The development of sarcopenia was positively correlated with the age with an odds ratio (OR) of 6.87 [95% confidence interval (CI) 1.63–28.88] in the middle-old group (70–79 years), and 13.71 (95%CI 3.66–51.41; p = 0.009) in the very old group (≥ 80 years). Prefrailty and low physical activity were significantly associated with sarcopenia with an OR of 4.75 (95%CI 1.90—11.89) in prefrailty, 15.35 (95%CI 1.69–139.47) in the middle activity group, and 17.99 (95%CI 1.95–165.73) in the lowest activity group. In conclusion, primary sarcopenia was found in one-tenth of outpatient elderly. Age, prefrailty, and low activity were independent factors associated with sarcopenia.

Abstract:BACKGROUND: Sarcopenia is defined as a progressive loss of skeletal muscle mass and strength related to age and comorbidities. Worldwide sarcopenia's prevalence varies between 10-40%, being associated with functional impairment, lower quality of life, and higher mortality. Sarcopenia can be estimated based on age, calf circumference, and handgrip strength (Ishii's formula). Early diagnosis is essential because treatment with nutritional support and rehabilitation programs can prevent complications. The aim of the study was to assess the prevalence of probable sarcopenia in hospitalized patients using Ishii's <i>score</i> and possible associated risk factors.METHODS: We developed an observational prospective study in a medicine inpatient ward of a Central Hospital. We applied Ishii's formula to the patients admitted to the medical ward in December 2021 and January 2022. Patients should be aged 60 or above and able to collaborate with the tests. One year later, we analyzed re-hospitalization and mortality rates. Patients with edema of the lower limbs, who were not able to follow instructions, and who were admitted exclusively for symptomatic treatment were excluded.RESULTS: Our final sample was 49 patients (55% males, mean age 78 ± 8.88 years). Only one patient had a previous diagnosis of sarcopenia. Estimated sarcopenia´s prevalence was 73.5% N=36), higher in men and people with three or more comorbidities. In the sarcopenic group, 77% had some degree of functional dependency and positive markers for malnutrition. After one year of follow-up, we found a higher mortality rate in the sarcopenic group (44.4% against 7.6%) and a higher number of re-hospitalizations (1.03 hospitalizations per patient, against 0.31).CONCLUSIONS: Our data showed that the prevalence of probable sarcopenia is high, but this pathology is still underdiagnosed. Traditional diagnosis is complex in some hospital settings and a simple tool such as Ishii's score can help to improve diagnostic rates. We suggest screening all patients at admission to provide early rehabilitation and nutritional support.
A Combination of Serum Biomarkers in Elderly Patients with Sarcopenia: A Cross-Sectional Observational Study

Abstract:Background. The pathogenesis of sarcopenia in the elderly has not yet been fully understood. This study aimed to explore the relationship between sarcopenia and several serum biomarkers in elderly population. Methods. It was an observational cross‐sectional study of data collected from 70 patients. According to the criteria of the Asian Working Group for Sarcopenia (AWGS), subjects were divided into the sarcopenia group and nonsarcopenic group. We compared age, body mass index (BMI), biochemical indexes, smoking status, underlying disease, muscle mass, handgrip strength (HS), gait speed (GS), skinfold thickness, muscle thickness, and IL-6, IL-10, IL-17A, and TNF-α levels between these groups. Results. Of the 70 subjects, 35 patients were diagnosed with sarcopenia. The number of men was higher than that of women in both groups. The patients with sarcopenia were older and had lower BMI and muscle thickness but higher SARC-F questionnaire scores. However, the difference in smoking status and skinfold thickness between these two groups were not statistically significant. Higher IL-6, IL-17A, and TNF-α levels were observed in participants with sarcopenia P < 0.05 . Patients with sarcopenia had a lower IL-10 level. Positive associations were present between the severity of sarcopenia and IL-6, IL-17A, and TNF-α levels, while there was an inverse correlation between the presence of sarcopenia and IL-10 level. Conclusions. Our research found that in sarcopenic elderly subjects, the serum levels of several biomarkers, such as IL-6, IL-17A, and TNF-α, were higher than those in nonsarcopenic elderly persons. Further studies are needed to explore the possible molecular mechanisms and discover new therapeutic targets.
Sarcopenia and sarcopenic obesity among community-dwelling Peruvian adults: A cross-sectional study

Abstract:Introduction Sarcopenia and sarcopenic obesity (SO) have emerged as significant contributors to negative health outcomes in the past decade. We aimed to estimate the prevalence of probable sarcopenia, sarcopenia, and SO in a community-dwelling population of 1151 adults aged ≥55 years in Lima, Peru. Methods This cross-sectional study was conducted between 2018 and 2020. Sarcopenia was defined as the presence of low muscle strength (LMS) and low muscle mass (LMM) according to European (EWGSOP2), US (FNIH) and Asian (AWGS2) guidelines. We measured muscle strength by maximum handgrip strength and muscle mass using bioelectrical impedance analyzer. SO was defined as a body mass index ≥ 30 kg/m 2 and sarcopenia. Results The study participants had a mean age of 66.2 years (SD 7.1), age range between 60 to 92 years old, of which 621 (53.9%) were men. Among the sample, 41.7% were classified as obese (BMI ≥30.0 kg/m2). The prevalence of probable sarcopenia was estimated to be 22.7% (95%CI: 20.3–25.1) using the EWGSOP2 criteria and 27.8% (95%CI: 25.2–30.4) using the AWGS2 criteria. Sarcopenia prevalence, assessed using skeletal muscle index (SMI), was 5.7% (95%CI: 4.4–7.1) according to EWGSOP2 and 8.3% (95%CI: 6.7–9.9) using AWGS2 criteria. The prevalence of sarcopenia based on the FNIH criteria was 18.1% (95%CI: 15.8–20.3). The prevalence of SO, considering different sarcopenia definitions, ranged from 0.8% (95%CI: 0.3–1.3) to 5.0% (95%CI: 3.8–6.3). Conclusion Our findings reveal substantial variation in the prevalence of sarcopenia and SO, underscoring the necessity for context-specific cut-off values. Although the prevalence of SO was relatively low, this result may be underestimated. Furthermore, the consistently high proportion of probable sarcopenia and sarcopenia point to a substantial public health burden.

Abstract:Objective To assess the longitudinal association between cognitive impairment and sarcopenia in a sample of Brazilian community-dwelling older adults. Design Nine-year observational prospective study. Setting and Participants A total of 521 community-dwelling older adults from 2 Brazilian sites of the Frailty in Brazilian Older Adults (FIBRA in Portuguese) study. Methods Sarcopenia was defined as low hand-grip strength and low muscle mass. Cognitive impairment was determined at baseline using the Mini-Mental State Examination, with education-adjusted cutoff scores. The logistic regression model was used to assess the association between cognitive impairment and incident sarcopenia after adjusting for gender, age, education, morbidities, physical activity, and body mass index. Inverse probability weighting was applied to correct for sample loss at follow-up. Results The mean age of the study population was 72.7 (±5.6) years, and 365 were women (70.1%). Being 80 years and older (odds ratio [OR], 4.62; 95% CI, 1.38–15.48; P = .013), being under- and overweight (OR, 0.29; 95% CI, 0.11–0.76; P = .012, and OR, 5.12; 95% CI, 2.18–12.01; P < .001, respectively) and having cognitive impairment (OR, 2.44; 95% CI, 1.18–5.04; P = .016) at baseline predicted sarcopenia after 9 years. Conclusion and Implications Cognitive impairment may predict sarcopenia in Brazilian older adults. More studies are necessary to identify the main mechanisms shared by sarcopenia and cognitive decline, which could support the development of prevention interventions.

Abstract:Abstract Objective To associate clinical parameters of sarcopenia with cognitive impairment in older people. Method Cross-sectional study with 263 older adults (≥60 years) treated at a specialized public health facility. Sociodemographic and clinical variables were used to characterize the sample and the clinical parameters of sarcopenia (muscle strength, muscle mass and physical performance) were assessed based on handgrip strength (HGS), calf circumference (CC) and the Timed Up and Go (TUG) test. The Mini-Mental State Examination (MMSE) was used to evaluate cognitive status. Associations were analyzed by simple and multiple linear and logistic regression considering the clinical parameters of sarcopenia (independent variables) and cognitive status (dependent variable), adjusted for age, sex, years of schooling, number of medications, nutritional status and functional capacity. Results Of participants with cognitive impairment, 59.6% exhibited low muscle strength. Conclusion Low muscle strength was the sarcopenia parameter independently associated with cognitive impairment. This information is useful in highlighting the likelihood of cognitive impairment when poor muscle strength is identified in older people.

Abstract:Introduction: Sarcopenia, characterized by progressive skeletal muscle loss, has emerged as a significant public health concern, with a global prevalence of 10-27%. While traditionally studied in geriatric populations, recent evidence indicates its impact on individuals aged 40 and above, with early manifestations of muscle decline. Primary care settings play a pivotal role in the early identification and management of sarcopenia, facilitating timely diagnosis and intervention. This study aims to evaluate the prevalence of sarcopenia and its associated factors among individuals aged 40 and above attending a family medicine outpatient clinic. Methods: A cross-sectional study was conducted in a family medicine outpatient clinic, including participants aged 40 and above. Participants underwent handgrip strength measurements, calf circumference measurements, and a 4-m walking test. Based on the criteria of the European Working Group on Sarcopenia in Older People 2 (EGSWOP2), individuals with sarcopenia and those at risk of sarcopenia were identified. The Mini Nutritional Assessment (MNA) was used to assess nutritional status. Sociodemographic characteristics, comorbidities, and laboratory values were recorded. A comparison was made between individuals with normal muscle strength and those at risk of sarcopenia. Results: Among 213 individuals, 33 (15.4%) were at risk of sarcopenia (probable sarcopenia), and 12 (5.6%) were diagnosed with sarcopenia. There was a negative correlation observed between age and muscle strength (r=-0,339, p<0,001), and positive correlation was found between muscle strength and MNA score (r=0.301, p<0.001). Individuals with higher education and income levels exhibited higher muscle strength. Participants at risk of sarcopenia had higher prevalence rates of comorbidities such as diabetes mellitus (DM), chronic kidney disease, and cardiovascular disease (p=0.20, p<0.01, p=0.015, respectively). Conclusion: Our study highlights the prevalence and associated factors of sarcopenia in individuals aged 40 and above emphasizing the need for screening and intervention strategies in primary care settings. The study findings support the role of primary care in addressing sarcopenia and improving patient outcomes.

Abstract:Sarcopenia, a clinical syndrome primarily associated with reduced muscle mass in the elderly, has a negative impact on quality of life and survival. It can occur secondarily to other diseases such as heart failure (HF), a complex clinical syndrome with high morbidity and mortality. The simultaneous occurrence of these two conditions can worsen the prognosis of their carriers, especially in the most severe cases of HF, as in patients with reduced left ventricular ejection fraction (LVEF). However, due to the heterogeneous diagnostic criteria for sarcopenia, estimates of its prevalence present a wide variation, leading to new criteria having been recently proposed for its diagnosis, emphasizing muscle strength and function rather than skeletal muscle mass. The primary objective of this study is to evaluate the prevalence of sarcopenia and/or dynapenia in individuals with HF with reduced LVEF according to the most recent criteria, and compare the gene and protein expression of those patients with and without sarcopenia. The secondary objectives are to evaluate the association of sarcopenia and/or dynapenia with the risk of clinical events and death, quality of life, cardiorespiratory capacity, ventilatory efficiency, and respiratory muscle strength. The participants will answer questionnaires to evaluate sarcopenia and quality of life, and will undergo the following tests: handgrip strength, gait speed, dual-energy X-ray absorptiometry, respiratory muscle strength, cardiopulmonary exercise, as well as genomic and proteomic analysis, and dosage of N-terminal pro-B-type natriuretic peptide and growth differentiation factor-15. An association between sarcopenia and/or dynapenia with unfavorable clinical evolution is expected to be found, in addition to reduced quality of life, cardiorespiratory capacity, ventilatory efficiency, and respiratory muscle strength.

Abstract:BackgroundThe European Working Group on Sarcopenia in Older People (EWGSOP) recently updated the definition and diagnostic criteria to assess sarcopenia, which can result in important changes in sarcopenia prevalence in older adults.AimTo compare the prevalence of sarcopenia through the diagnostic criteria and definition proposed by the first (EWGSOP1) and recent (EWGSOP2) European consensus in older adults. We also aimed to evaluate which sarcopenia consensus is better associated with unfavorable health outcomes.MethodsThe review followed PRISMA guidelines. Embase, Medline (PubMed), Scopus and Web of Science were searched from 2018 to February 2021. The systematic review protocol was registered at PROSPERO (CRD42020213303). The search, selection, and evaluation processes were done in a duplicate and independent manner.ResultsOf the 298 potentially eligible articles, 9 were included in this review. The prevalence of sarcopenia was 17.7% by EWGSOP1 and 11% by EWGSOP2. Evaluating all the studies, the sarcopenia prevalence ranged from 6.2 to 35.3% for the EWGSOP1, and from 3.2 to 26.3% for the EWGSOP2. Five studies have evaluated the association between the prevalence of sarcopenia (EWGSOP1 versus EWGSOP2) and unfavorable health outcomes, in which three studies showed that EWGSOP1 was better associated with increased risk of hospitalization and/or mortality.ConclusionIn comparison with EWGSOP1, the prevalence of sarcopenia in older adults decreased when diagnosed according to EWGSOP2. Based on limited evidence, EWGSOP2 seems to be worse for predicting unfavorable outcomes compared with EWGSOP1.
