Abstract:BACKGROUND: Dynapenia is a risk factor of mortality. Therefore, the development of low-cost and easy-to-apply tools is essential to optimize the health surveillance actions of older people.OBJECTIVES: To compare the time spent on habitual physical activity (HPA) and sedentary behavior (SB) among dynapenic and non-dynapenic older adults and ascertain the predictive ability of these behaviors on outcome.DESIGN AND SETTING: A cross-sectional population epidemiological survey was conducted involving 208 older adults.METHODS: HPA and SB were quantified using the International Physical Activity Questionnaire, and dynapenia was identified by handgrip strength (women: 18.37 kgf; men: 26.75 kgf).RESULTS: The prevalence was 24.50%. In both sexes, dynapenic individuals reported a HPA median time of 70.00 minutes/week (min/wk), while non-dynapenic women and men reported HPA median times of 240.00 and 280.00 min/wk, respectively (P < 0.05). For SB among dynapenic individuals, a median of 388.75 min/day was observed in women and 428.57 min/d in men. In contrast, non-dynapenic women and men had 291.42 and 274.28 min/day in SB (P < 0.05), respectively. The best cutoff HPA to discriminate the outcome was 150.00 min/wk in women (sensitivity: 73.30%; specificity: 60.67%) and 140.00 min/wk in men (sensitivity, 71.43%; specificity, 61.54%). The best cutoff SB was 381.43 min/day in women (sensitivity, 53.30%; specificity, 84.80%) and 351.43 min/day in men (sensitivity, 71.43%; specificity, 73.85%).CONCLUSION: Older individuals with dynapenia spent less time on HPA and more time in SB. Furthermore, HPA was found to be a better discriminator of dynapenic individuals, and SB better discriminated non-dynapenic individuals.

Abstract:PURPOSE: Skeletal muscle function is an important indicator of physical fitness and health status in adults. Although insufficient nutrition intakes contribute to loss of muscle function, the association between inappropriate eating behaviors and muscle function has not been examin. Therefore, this population-based study aims to examine the association of several eating behaviors with muscular strength in Japanese adults. METHODS: A cross-sectional study was carried out between 2008 and 2011. One thousand three hundred and ten Japanese adult employees working at Sendai Oroshisho Center in Sendai, Japan, provided their written informed consent to participate in this study. These participants’ age ranged from 30 to 83 years, among them were 313 (23.9%) women. Eating behaviors were comprised of skipping breakfast (breakfast consumption frequency < 4 days/week), eating speed (slow, moderate, fast), and late-night eating habit (eating within 2 hours before bedtime). They were assessed by using a self-reported questionnaire. In addition, as an indicator of skeletal muscle function, handgrip strength was measured by a handheld digital dynamometer. The association between eating behaviors and handgrip strength was analyzed by analysis of covariance after adjustment for potential confounding factors, such as socio-demographic, lifestyle-related and health-related factors. A statistical significance was considered as P<0.05. RESULTS: Analysis of covariance showed that subjects who skipped breakfast had a poorer handgrip strength than those with regular breakfast consumption habit (mean and 95% confidence interval: 37.6 [37.0-38.0] vs. 38.3 [37.9-38.7], P=0.02). Further, lower eating speed tended to be associated with poorer handgrip strength (low eating speed, 36.9 [35.8-38.0]; moderate eating speed, 38.1 [37.7-38.5]; high eating speed, 38.2 [37.7-38.8], P for trend=0.04). On the other hand, there was no significant association between late-night eating habit and handgrip strength. CONCLUSION: This population-based cross-sectional study shows that the adults who skip breakfast and eat slowly may be associated with poorer muscular strength. Prospective studies are warranted to further confirm these observations and to clarify causality. Prevalence and Risk Factors for Hand-Grip-Determined Dynapenia in the Korean Population: A Cross-Sectional Study

Abstract:Dynapenia refers to muscle weakness related to aging and is defined as a loss of muscle strength associated with muscle quality. The purpose of this study is to identify the prevalence of dynapenia and associated risk factors by gender and age in domestic adults and to provide customized basic data for the prevention of dynapenia through its management. Based on the data from 2014 to 2019 of the Korea National Health and Nutrition Survey, 20,950 adults over the age of 20 who participated in grip strength tests and health surveys were selected as participants. Factors related to dynapenia were analyzed using complex sample multilogistic regression analysis. The prevalence of dynapenia in domestic adults was 6.3%, 4.1% in men, and 8.7% in women. Prevalence in the second decade was 5.3%, in the third decade was 3.2%, in the fourth decade was 3.3%, in the fifth decade was 4.8%, in the sixth decade was 8.9%, and was 24.6% for participants in their seventh decade or beyond. Among the factors related to dynapenia, age, BMI, and alcohol status were common in both men and women; education level, HDL-C, and resistance exercise were common in men; and high blood pressure, high blood sugar, and aerobic exercise were common in women. Our findings indicate that several factors are associated with dynapenia, which should be considered as potential targets for interventions focused on both individual and public health measures.

Abstract:Background and aims: Increased levels of circulating adiponectin in the elderly cause a negative impact on physical function and health status, which suggests that circulating adiponectin may be related to skeletal muscle function. However, data on the relationship between circulating adiponectin levels and skeletal muscle function is limited. Our objective was to investigate the association between serum adiponectin levels and muscle strength in adults.Methods and results: This cross-sectional study is a part of the Oroshisho Study of adult employees in Japan from 2008 to 2011. In our study, we used data gathered in 2008-2010 that had included serum adiponectin measurements (n = 1378; age, 19-83 years). From this population, 1259 subjects were evaluated for grip strength (949 men, 310 women), and 965 subjects were evaluated for leg extension power (716 men, 249 women). Multivariate linear regression analyses showed that adiponectin was associated significantly and negatively with both grip strength (beta and standard error [SE]: men, -0.09 [0.01], p = 0.010; women, -0.20 [0.03], kg, p = 0.002) and leg extension power (men, -0.09 [0.02], p = 0.014; women, -0.14 [0.07], W, p = 0.032) after adjusting for age, physical activity, nutrient intake, depressive symptoms, metabolic syndrome, C-reactive protein, body mass index, and other lifestyle-related potential confounders.Conclusion: This population-based cross-sectional study indicates an inverse association between serum adiponectin levels and muscle strength in adults. Dynapenic Abdominal Obesity and Metabolic Health in Non-Geriatric Patients with Obesity

Abstract:Background: Dynapenic abdominal obesity (DAO) is the combination of low muscle strength and high central adiposity, which can lead to functional and cardiometabolic impairments. Objective: The aim of this study was to examine the relationship between DAO and metabolic parameters in non-geriatric patients with obesity. Methods: All patients seen in the obesity outpatient clinic during a random month, along with 30 healthy individuals, were included in the study. Body mass index (BMI), waist circumference (WC), hip circumference (HC), serum fasting blood glucose, triglyceride, high-density lipoprotein, low-density lipoprotein (LDL), insulin, and glycated hemoglobin (HbA1c) levels were measured. Muscle function tests were conducted. Hypertension and diabetes mellitus diagnoses were recorded. DAO was determined using handgrip strength and WC. Metabolic syndrome was defined based on International Diabetes Federation (IDF) criteria. Data were analyzed using SPSS. Results: A total of 106 individuals participated in the study, including 76 individuals with obesity as the case group and 30 healthy individuals as the control group. In the case group, BMI, WC, HC, and chair stand test results were higher compared to the control group. There was no significant difference between the case and control groups in terms of gait speed or handgrip test results. In the case group, 30 people (39.5%) had dynapenic obesity (DO), and in the control group, 8 people (26.7%) had DO. There was no significant difference in DO rates between the case and control groups. In the case group, the HbA1c level was higher in DO (+) group than DO (-) group. In the control group, DO (+) group had higher LDL and HbA1c level than DO (-) group. No significant difference was observed in other search parameters between the DO (+) and DO (-) groups in both the case and control groups. Conclusion: DAO is common among people with obesity and contributes to a poor prognosis, increased morbidity, and mortality. Therefore, including its treatment in the management of obesity is crucial for individuals of all age ranges.
Association between upper limb muscle quality and knee osteoarthritis in dynapenia: an observational cross-sectional study

Abstract:Neurological and skeletal muscle properties are suggested causes of dynapenia. This study aimed to evaluate the relationship between upper limb muscle quality (grip strength/upper extremity muscle mass) and knee osteoarthritis in dynapenia, and to identify dynapenia-associated factors. Elderly individuals who responded to a public call for screening in Wakasa Town, Fukui Prefecture between June 2019 and November 2021 were included. The analysis included 433 participants (304 women aged 76.0 ± 7.1 years). Examination comprised (consecutively) a basic interview, physical function measurement, body composition measurement, and explanation of results. Dynapenia was observed in 67 patients. Binomial logistic regression analysis revealed that age, upper limb muscle quality score, and knee osteoarthritis were independent factors for dynapenia. Receiver operating characteristic analysis of the relationship between dynapenia and upper limb muscle quality showed an area under the curve of 0.806 (95% confidence interval: 0.658-0.953) for men (cut-off value, 14.3 kg/kg) and 0.849 for women (95% confidence interval: 0.858-0.968; cut-off value, 14.0 kg/kg). In conclusion, age, upper limb muscle quality, and knee osteoarthritis were independent factors of dynapenia. We demonstrated that upper limb muscle quality has good accuracy in detecting dynapenia in both men and women.

Abstract:[Purpose] This study clarified the relationship between sedentary behavior and dynapenia in community-dwelling elderly using ambulatory service implementation facilities. [Participants and Methods] The subjects were 161 users of two outpatient nursing homes who met the conditions. A cross-sectional design was adopted for this study. Determination of dynapenia was performed via grip strength (28 kg for men, and 18 kg for women) with reference to the definition of the Asian Working Group for Sarcopenia. [Results] Sedentary behavior showed an association with dynapenia even after controlling for age, sex, physical activity, cognitive function, and comorbidities. [Conclusion] Interventions focusing on sedentary behavior are important for preventing the development and exacerbation of geriatric syndromes in the elderly due to the progression of dynapenia.
Low Normalized Grip Strength is a Biomarker for Cardiometabolic Disease and Physical Disabilities among US and Chinese Adults

Abstract:Background: Evidence highlights the importance of muscular strength as a protective factor for health and function across aging populations. The purpose of this study was to examine the extent to which low normalized grip strength (NGS) serves as a biomarker for both cardiometabolic disease and physical disability in U. S. and Chinese adults. Methods: Middle aged and older adults from the U. S. National Health and Nutrition Examination Survey 2011-2012 and 2013-2014 combined surveys (n = 4,544), and the 2011 wave of the China Health and Retirement Longitudinal Study (n = 6,030) were included. Strength was assessed using a handgrip dynamometer, and was normalized to body mass. Weighted logistic regression models were used to assess the association between NGS and diabetes, hyperglycemia, hypertriglyceridemia, low HDL-cholesterol, hypertension, and physical disability status, while controlling for age, sex, and sociodemographic characteristics. Results: Every 0.05 lower NGS was independently associated with a 1.49 (95% confidence interval [ CI]: 1.42-1.56) and 1.17 (95% CI: 1.11-1.23) odds for diabetes; a 1.46 (95% CI: 1.39-1.53) and 1.11 (95% CI: 1.07-1.15) odds of hyperglycemia; a 1.15 (95% CI: 1.07-1.25) and 1.11 (95% CI: 1.08-1.14) odds of hypertriglyceridemia; a 1.22 (95% CI: 1.17-1.27) and 1.15 (95% CI: 1.12-1.18) odds of low HDLcholesterol; a 1.19 (95% CI: 1.14-1.24) and 1.10 (95% CI: 1.07-1.14) odds of hypertension; and a 1.36 (95% CI: 1.29-1.42) and 1.10 (95% CI: 1.05-1.15) odds for physical disability status in U. S. and Chinese adults, respectively. Conclusions: NGS was robustly associated with both cardiometabolic disease risk and physical disabilities in U. S. and Chinese aging adults.

Abstract:The diagnosis of long COVID is troublesome, even when functional limitations are present. Dynapenia is the loss of muscle strength and power production that is not caused by neurologic or muscular diseases, being mostly associated with changes in neurologic function and/or the intrinsic force-generating properties of skeletal muscle, which altogether, may partially explain the limitations seen in long COVID. This study aimed to identify the distribution and possible associations of dynapenia with functional assessments in patients with long COVID. A total of 113 patients with COVID-19 were evaluated by functional assessment 120 days post-acute severe disease. Body composition, respiratory muscle strength, spirometry, six-minute walk test (6MWT, meters), and hand-grip strength (HGS, Kilogram-force) were assessed. Dynapenia was defined as HGS < 30 Kgf (men), and < 20 Kgf (women). Twenty-five (22%) participants were dynapenic, presenting lower muscle mass (p < 0.001), worse forced expiratory volume in the first second (FEV 1 ) (p = 0.0001), lower forced vital capacity (p < 0.001), and inspiratory (p = 0.007) and expiratory (p = 0.002) peek pressures, as well as worse 6MWT performance (p < 0.001). Dynapenia, independently of age, was associated with worse FEV 1 , maximal expiratory pressure (MEP), and 6MWT, (p < 0.001) outcomes. Patients with dynapenia had higher intensive care unit (ICU) admission rates (p = 0.01) and need for invasive mechanical ventilation (p = 0.007) during hospitalization. The HGS is a simple, reliable, and low-cost measurement that can be performed in outpatient clinics in low- and middle-income countries. Thus, HGS may be used as a proxy indicator of functional impairment in this population.

Association between bone health and dynapenic obesity in postmenopausal women

Abstract:We examined the associations between dynapenic obesity and cortical bone thickness and trabecular bone density. Dynapenic obesity was found to be associated with cortical bone thinning that might compromise bone health. Aim The combination of dynapenia (age‐related muscle weakness) and obesity is referred to as dynapenic obesity. We examined the associations between dynapenic obesity and cortical bone thickness and trabecular bone density. Methods The participants were 797 community‐dwelling postmenopausal women (with an average age of 62.5 years) who were stratified into normopenia without obesity, dynapenia without obesity (dynapenia), normopenia with obesity (obesity) and dynapenia with obesity (dynapenia obesity) groups based on their grip strength and body fat percentage. Cortical bone thickness and trabecular bone density were measured using ultrasonic bone densitometry. The participants were further divided into those with low cortical bone thickness and low trabecular bone density. Logistic regression analysis was used to identify associated factors. Results Individuals with dynapenia (odds ratio [OR] 1.77, 95% confidence interval [CI] 1.16–2.68), obesity (OR 2.46, 95% CI 1.62–3.75) and dynapenic obesity (OR 4.07, 95% CI 2.44–6.79) all significantly increased the odds of low cortical bone thickness. Conversely, the odds of low trabecular bone density were significantly lower in the obesity group (OR 0.65, 95% CI 0.43–0.99) and dynapenic obesity group (OR 0.60, 95% CI 0.37–0.97). Conclusions Dynapenic obesity was found to be associated with cortical bone thinning that might compromise bone health. Postmenopausal women with dynapenic obesity might need to be closely monitored for preserving bone health. Geriatr Gerontol Int 2024; ••: ••–••.

Abstract:Background: The coexistence of obesity and low muscle strength—denoted dynapenic obesity (DO)—has been associated with an unhealthy metabolic profile and increased risk for metabolic syndrome. However, there is a lack on studies investigating if DO exhibits higher cardiometabolic risk than non-dynapenic obesity. Objectives: To assess if individuals with DO exhibit elevated cardiometabolic risk compared to non-dynapenic obesity. Methods: a cross-sectional study that analyzed the data of workers from a quaternary care hospital collected between November 2018 and March 2020. Participants were stratified into the following anthropometrical and peripheral muscle strength profiles: non-obese/non-dynapenic (NOND), non-obese/dynapenic (NOD), obese/non-dynapenic (OND), and obese dynapenic (OD). Cardiovascular risk was evaluated by Atherogenic Index (AI), Plasma Atherogenic Index (PAI), Hypertriglyceridemic Waist (HW), A Body Shape Index (ABSI), Atherogenic Dyslipidemia (AD), Castelli Indices I and II, and Framingham Score (FS). Results: the OD group had significantly lower HDL compared to all others (p = 0.009), and despite exhibited lower prevalence of HW compared to OND (p < 0.01), a higher cardiometabolic risk compared to OND profile was observed assessing AI (p = 0.05), Castelli I (p < 0.05) and Castelli II (p < 0.05) scores. Conclusions: in the studied population, individuals with DO exhibit elevated cardiometabolic risk compared to other anthropometrical and peripheral muscle strength profiles.

Abstract:Introduction: Muscle strength and endurance are key determinants of physical fitness, cardiometabolic profile, and overall health. Clinically, they are tested by estimating Handgrip Strength (HGS) and Handgrip Endurance (HGE) through an isometric contraction of the flexors of the forearm and hand. Body Mass Index (BMI) and body fat content can both influence muscle performance, but there are wide variations regarding their effects. BMI and body fat also do not show unequivocal similarity to each other. Aim: The aim of this study was to determine and compare the effects of BMI and body fat on muscle strength and endurance. Materials and Methods: A descriptive cross-sectional study was conducted at the Department of Physiology, Himalayan Institute of Medical Sciences, Swami Rama Himalayan University, Jolly Grant, Dehradun from April 2019 to March 2020. A total of 100 subjects who fulfilled the selection criteria were included. BMI and body fat were measured, and HGS and HGE were recorded using a handgrip dynamometer. Statistical analysis was performed using SPSS version 17.0. Descriptive statistics, independent sample t-test, and one-way ANOVA were used for comparison. Pearson’s correlation coefficient and linear regression were used to determine and quantify the nature of the effect, with a p-value of ≤0.05 considered statistically significant. Results: The participants had a mean BMI of 23.809±3.798 kg/ m2 and a mean body fat percentage of 25.309±7.162%. The mean HGS and HGE of the participants were 36.08±9.956 kg and 18.46±14.2 seconds, respectively. Body fat percentage showed a negative and moderately significant correlation (p-value≤0.001) with both muscle strength and endurance. Approximately one-fifth of the variations in muscle strength and one-third of the variations in muscle endurance were determined by body fat content. HGS and HGE were higher in males, while body fat percentage was higher in females. Gender variation in body fat contributed to more than 40% of the variance. Conclusion: The study revealed a negative correlation between total body fat and muscle strength and performance. Keeping body fat under control can improve muscle performance, which, in turn, enhances cardiometabolic health.

Abstract:Background There is little epidemiological evidence on the relationship of dynapenic abdominal obesity (DAO) and the development of functional disability, particularly in Asian populations. We aimed to investigate the association of DAO with new-onset functional disability in Chinese adults. Methods A total of 7881 participants aged ≥45 years from China Health and Retirement Longitudinal Study (CHARLS) in 2011 and 2015 were included in the study. Dynapenia and abdominal obesity were respectively defined based on handgrip strength (< 28 kg for male and < 18 kg for female) and waist circumference (≥ 90 cm for male and ≥ 85 cm for female). The sample was divided into four groups: non-dynapenic/non-abdominal obesity (ND/NAO), non-dynapenic/abdominal obesity (ND/AO), dynapenic/non-abdominal obesity (D/NAO) and dynapenic/abdominal obesity (D/AO). Functional status was assessed by basic activities of daily living (BADL) or instrumental activities of daily living (IADL). Logistic regression model was used to explore the longitudinal association between dynapenic abdominal obesity and incident functional disability. Results After a 4-year follow-up, 1153 (14.6%) developed BADL disability and 1335 (16.9%) developed IADL disability. The multivariable-adjusted odds ratios (95% CIs) for the D/AO versus ND/NAO were 2.21 (1.61-3.03) for BADL disability, and 1.68 (1.23-2.30) for IADL disability. In addition, DAO was associated with an increased risk for functional dependency severity (odds ratio, 2.08 [95% CI, 1.57-2.75]). Conclusions DAO was significantly associated with greater risk of functional disability among Chinese middle-aged and older adults. Our findings indicated that interventions targeted DAO might be effective in the primary prevention of functional disability.

The Relationships Between Upper and Lower Extremity Muscle Strength, Rate of Force Development, and Fatigue in Adults

Abstract:Electronic handgrip dynamometry allows for multiple muscle function aspects to be feasibly measured, yet their relationship with lower extremity muscle function is unknown. We sought to determine the relationships between upper and lower extremity mechanical isometric muscle strength, rate of force development (RFD), and endurance by limb dominance in resistance trained adults. The analytic sample included 30 adults aged 32.1 ± 13.5 years. An electronic handgrip dynamometer ascertained upper extremity strength capacity, RFD, and endurance. Lower extremity strength, RFD, and endurance were collected with the isometric feature on an isokinetic knee dynamometer. Limb dominance was self-reported. Pearson correlations were used for the analyses. Each muscle function attribute on the dominant limb of the upper and lower extremities were correlated: r = 0.76 (p < 0.01) for strength, r = 0.37 (p = 0.04) for RFD, and r = -0.48 (p < 0.01) for endurance. Although strength from the non-dominant limbs were correlated (r = 0.67; p < 0.01), no significant correlations were observed for RFD (r = 0.20; p = 0.29) and endurance (r = -0.21; p = 0.26). For adults aged 18-34 years, only upper and lower extremity strength was correlated on the dominant (r = 0.69; p < 0.01) and non-dominant limbs (r = 0.75; p < 0.01); however, strength (r = 0.88; p < 0.01) and endurance (r = -0.68; p = 0.01) were correlated in adults aged 35-70 years. Upper and lower extremity fatigability was likewise correlated in females (r = -0.56; p = 0.01). Our findings suggest that electronic handgrip dynamometry derived strength, RFD, and endurance could be a whole-body indicator of these muscle function attributes given their relationships with the lower extremities. These findings underscore the promise of handgrip dynamometry in routine muscle function assessments across different age groups.
Association of physical capacity with heart rate variability based on a short-duration measurement of resting pulse rate in older adults with obesity

Abstract:BACKGROUND: Obesity can limit physical capacity and lower physical activity levels in elderly people. Low physical activity levels may be mediated by autonomic dysfunction with decreased heart rate variability (HRV). However, the relationship between autonomic dysfunction and low physical capability remains unclear. This cross-sectional study investigated the association of low physical capability with HRV in older adults with obesity.MATERIALS AND METHODS: We recruited 231 old man and 210 old women with a mean (range) age of 65.5 (51-78) and 62.9 (52-76) years, respectively. Physical capability was measured using mobility tasks, including functional reach, single-leg stance (SLS), gait speed (GS), timed up and go, and timed chair rise (TCR), and the scores on these tasks were merged and transformed into a global physical capability score (GPCS). HRV was measured using a 7-min resting pulse-based technique, and the time- and frequency-domain indices of HRV were obtained including standard deviation of normal-to-normal intervals (SDNN), root mean square of successive differences at rest (rMSSD), and high-frequency (HF) power. All HRV indices were natural log (ln) transformed for analysis. Participants were divided into high, moderate, and low physical-capability groups according to their physical performance. Multivariate analysis of covariance was performed to test differences in HRV indices among physical-capability groups with participants' characteristics serving as covariates. A stepwise regression model was established to identify the determinants of HRV indices. We used hierarchical regression analysis to identify the association of the GPCS with HRV indices.RESULTS: In both men and women, the low physical-capability group exhibited significantly increased heart rate (P <0.05) and decreased HRV in terms of a decreased ln[SDNN] (P <0.001), ln[rMSSD] (P <0.05) and ln[HF] (P <0.05), compared with the high physical-capability group. GS positively predicted ln[SDNN], whereas SLS, GS, and TCR were determinants of ln[HF], regardless of gender. The GPCS in older men and women independently accounted for 29.9% (P <0.001) and 23.7% (P <0.001), respectively, in variance in ln[SDNN].CONCLUSIONS: A low physical-capability level is an independent determinant of decreased HRV in older adults with obesity.

Assessment of estrogenic activity in Tunisian water and wastewater by E-screen assay.

Abstract:Wastewater and surface water samples from three wastewater treatment plants (WWTPs) and three rivers in Tunisia were assayed for estrogenic activity using the E-screen assay and enzyme-linked immunosorbent assay (ELISA). Results showed that all the Tunisian raw wastewater samples as well as the Roriche river water sample induced a strong proliferative response in human MCF-7 breast cancer cells. Tunisian raw wastewater had an average 17beta-estradiol content of 2,705.4 pg/ml, whereas that of the Roriche river was 36.7 pg/ml, which is sufficient for inducing endocrine-mediated responses in aquatic organisms. Results further showed that the Mornag WWTP, which uses the activated-sludge treatment system, has a higher estrogen removal efficiency than the stabilization ponds of the Gammart and pilot WWTPs. This study, which is the first of such studies in Tunisia, and probably the first in the North African region, underscores the need to detect and monitor the estrogenic activity of water and wastewater, given the scarcity of water in Tunisia and the detrimental impact of endocrine-disrupting compounds on the physiology of both animals and humans.
Depressive Symptoms, Handgrip Strength, and Weight Status in US Older Adults

Abstract:Background: Handgrip strength is a valid indicator of broader physical functioning. Handgrip strength and weight status have been independently associated with depressive symptoms in older adults, but no study has yet investigated the relationships between all three in older US adults. This study investigated the relationship between physical function and depressive symptoms by weight status in older US adults. Methods: Cross-sectional data were analysed from the National Health and Nutrition Examination Survey waves 2011 to 2012 and 2013 to 2014. Physical function was assessed using a grip strength dynamometer. Depressive symptoms were assessed using the self-reported Patient Health Questionnaire-9. Weight status was assessed using Body Mass Index (BMI) and participants were categorised as normal weight (< 25 kg/m(2)), overweight (25 to< 30 kg/m(2) ), and obese (>= 30.0 kg/m(2)). Associations between depressive symptoms and hand grip strength were estimated by gender-specific multiple linear regressions and BMI stratified multivariable linear regression. Results: A total of 2,812 adults (54% female, mean age 69.2 years, mean BMI 29.2 kg/m(2)) were included. Women with moderate to severe depressive symptoms had 1.60 kg (95% CI: 0.91 to 2.30) lower hand grip strength compared to women with minimal or no depressive symptoms. No such association was observed in men. Among those with obesity, men (-3.72 kg, 95% CI: -7.00 to -0.43) and women (-1.83 kg, 95% CI: -2.87 to -0.78) with moderate to severe depressive symptoms both had lower handgrip strength. Conclusion: Among older US adults, women and people who are obese and depressed are at the greatest risk of decline in physical function.

Abstract:The relationships between different functional fitness performance and abdominal obesity among the elderly have not been widely discussed in the literature. The present study aimed to investigate the associations between functional fitness test performance and abdominal obesity in Taiwanese elderly people. A total of 22,399 items of data from the National Physical Fitness Survey Databases in Taiwan (NPFSD 2014-15) were reviewed and analyzed. The quartiles of functional fitness test results were identified as the dependent variable in the multiple linear regression analysis to examine the association between functional fitness and abdominal obesity status. The results showed that body balance (odds ratios (ORs) listed from 1.18 to 2.29, p < 0.05) and flexibility (ORs listed from 1.23 to 2.16, p < 0.05) were critically associated with abdominal obesity. However, measurements related to muscle strength revealed the limited effect sizes for abdominal obesity. From a disability prevention perspective, the development of muscle strength in the elderly should be emphasized and encouraged to maintain their daily capabilities instead of satisfaction with a lean body.
