Abstract:Background: In China, the rates of early initiation and exclusive breastfeeding are low. The high cesarean delivery rates further contribute to low breastfeeding outcomes. Skin-to-skin contact, a key component of early essential newborn care, is known to be associated with improved breastfeeding initiation and exclusivity; however, the necessary duration has not been subjected to a randomized control trial. Objective: This study aimed to determine the association of the duration of skin-to-skin contact after cesarean delivery with breastfeeding outcomes and maternal and neonatal health outcomes in China. Study design: This was a multicentric randomized controlled trial that was conducted at 4 hospitals in China. A total of 720 participants at ≥37 gestational weeks with a singleton pregnancy and who received an elective cesarean delivery with epidural anesthesia or spinal anesthesia or combined spinal-epidural anesthesia were randomly divided into 4 groups at a ratio of 1:1:1:1. The control group received routine care. Intervention group 1 (G1), 2 (G2), and 3 (G3) received 30, 60, and 90 minutes of skin-to-skin contact immediately after the cesarean delivery, respectively. Results: Between January 3 and October 14, 2021, 659 participants were recruited, including 173 in the control group, 176 in G1, 146 in G2, and 164 in G3. Among G1, G2, and G3, the rate of early initiation of breastfeeding within 60 minutes of birth was 56%, 71%, and 72%, respectively, compared with 22% in the control group (P<.001). The exclusive breastfeeding rate at discharge was 69%, 62%, and 71%, respectively, compared with 57% in the control group (P=.003). Early essential newborn care practices were associated with a reduction in postpartum blood loss and neonatal intensive care unit or neonatal ward admission (P<.001; P=.022) . Conclusion: Our findings highlight that prolonged skin-to-skin contact after a cesarean delivery was associated with higher initiation and exclusive breastfeeding at discharge rates. It also found associations with reduced postpartum blood loss and neonatal intensive care unit or neonatal ward admission.

Abstract:Introduction: Our aim was to describe feasibility and outcomes of skin-to-skin care (SSC) that began during cesarean surgery and continued, uninterrupted, for about 5 hours. We described maternal/newborn measures of physiologic stability and stress; maternal measures of comfort; maternal satisfaction with surgery and SSC; and exclusive breast milk feeding at discharge.Materials and Methods: We used a quasiexperimental, time-interrupted design and randomly assigned women to receive SSC that began during surgery (Group 1, intervention; n = 20) or after surgery, before transfer to recovery (Group 2, standard care; n = 20). We analyzed differences across time and for five observations: before transfer to the operating room (OR); in the OR, about 20 minutes after birth; in the recovery room, about 1 hour after admission; in the New Family Center (NFC), about 1 hour after admission; and in the NFC, about 2 hours after admission.Results: Group 1 began SSC an average of 0.89 minutes after birth and continued an average of 300 minutes and Group 2 began an average of 46 minutes after birth and continued an average of 126 minutes. Women who began SSC during surgery were more satisfied with the experience (p = 0.015) and had lower levels of salivary cortisol across time (p = 0.003). We found no negative effects on maternal or newborn measures of physiologic stability and no difference in exclusive breast milk feeding rates at discharge.Conclusion: Immediate and uninterrupted SSC during medically uncomplicated cesarean surgery is a feasible, low-cost intervention that can safely begin during surgery and continue, uninterrupted, for extended durations.

The effect of mother-infant skin to skin contact on success and duration of first breastfeeding: A systematic review and meta-analysis

Abstract:Breast-feeding initiation within the first half hour after birth is one of the World Health Organization recommendations. However, in most hospitals, mother-infant contact and breast-feeding initiation are delayed due to routine mother and infant care. This study aimed to determine the effect of mother-infant skin to skin contact (SSC) immediately after birth on the success rate and duration of the first breast-feeding. In this review, databases of PubMed, Scopus, Cochrane, Google Scholar, SID and Magiran and reference sections of relevant articles were searched for both Persian and English randomized clinical trials from 2000 to December 2017, using the keywords of "(Breast-feeding OR Lactation) AND (mother-infant SSC OR KMC) AND (breast-feeding success OR breast-feeding duration)". A total of nine trials were ultimately included. Data analysis was performed with Comprehensive Meta-analysis (CMA) software version 2. In total 597 participants were assigned to the intervention group and 553 participants were assigned to the comparison group. Quantitative analysis Based on mean differences or odds ratio showed that Mother-Infant SSC had a significantly positive effect on success in first breast-feeding (MD:1.90, 95%; CI 0.958-2.856; p = 0.00, OR: 2.771 95%; CI 1.587-4.838; p = 0.00) and first breast-feeding duration (MD:26.627 95%; CI 1.070-52.184; p = 0.041). Mother-infant SSC after birth has beneficial effects on breast-feeding and can increase the success rate and duration of the first lactation. Therefore, the results of this study can be used by healthcare providers in evidence-based decision-making about ways to increase breast-feeding rates.

Abstract:Aim and objectives This study aimed to delineate optimal practices for early mother-infant skin-to-skin contact (SSC) by synthesizing the best available evidence, thereby facilitating its implementation and promotion. Design and methods: We referred to the Joanna Briggs Institute (JBI) streamlined rapid review approach to conducted this study. The guide websites, institution websites, national and international databases, and clinical decision support systems were comprehensive searched. AGREE II (Appraisal of Guidelines for Research and Evaluation II), AMSTAR 2 (A Measurement Tool to Assess Systematic Reviews), and JBI appraisal tools were utilized to assess the quality of the included literature. Results 14 guidelines, 9 expert consensus reports, five evidence summaries, and one systematic review were finally included. In total, six domains comprising 18 recommendations were extracted: (1) target population; (2) preparations beforehand; (3) timing of initiation; (4) practices during SSC; (5) precautions during exposure; (6) breastfeeding promotion. Conclusions and relevance to clinical practice: Based on the synthesized evidence, immediate or early uninterrupted SSC is recommended regardless of delivery mode. Newborns should be placed prone on the mother's abdomen or chest as soon as possible after birth for a minimum of 60 or 90 minutes, or until completion of the first breastfeeding. Notably, we compared and selected evidence to address inconsistencies in practices. Our recommendations offer clarity on the SSC target population, preparation, precautions during exposure, and breastfeeding promotion. However, explicit duration and initiation time for SSC remain uncertain. These findings serve as a valuable reference for clinical practice and research endeavors.

Abstract:Abstract Skin-to-skin care (SSC) is an important part of parent and infant care during the neonatal period and into infancy. SSC should be initiated immediately after birth and practiced as a standard of care in all settings, as well as in the home. There is strong evidence that SSC has a positive effect on breastfeeding and human milk feeding in both term and preterm infants, as well as on mortality, cardiopulmonary stability, and thermoregulation. SSC reduces pain and infant stress, enhances parent–infant bonding, has neurodevelopmental benefits, and has positive effects on parental mental health. The safety and feasibility of providing SSC has been established in term and preterm infants, and SSC is recommended as best practice for all infants. The benefits of SSC outweigh the risks in most situations, and despite challenges, care providers should implement procedures and accommodations to ensure that SSC occurs as a safe and positive experience for the parent, family, infant, and health care team. This statement includes all families as defined and determined by themselves, and recognizes that health communication, language, and terminology must be individualized to meet specific family needs by the health care team.

In vitro diagnosis of farmers' IgE‐mediated allergy by Phadiatop® and three new multiallergen RAST analyses

Abstract:Identification of persons with atopic allergy in a population is difficult if the pattern of sensitizing allergens is unique. Three new multiallergen tests, Professional Allergy Mixes (PAX) 1–3 (Pharmacia Diagnostics, Uppsala, Sweden), have recently been inttoduced to screen IgE‐mediated allergy among farmers. PAX 1 contains animal danders and feathers, PAX 2 mites and insects, and PAX 3 pollens and molds. The value of these new tests for the determination of IgE‐mediated allergy among fanners was compared with that of Phadiatop®, a test for general atopy. Of 440 farmers, 91 were positive to Phadiatop and 97 to any of PAX 1–3. None of the farmers were positive in all tests. Only 70/97 sera positive in RAST to PAX 1–3 had a positive Phadiatop, whereas of 91 sera positive in Phadiatop, 70 had specific IgE antibodies to PAX 1–3. The value of using either Phadiatop or PAX 1–3 is limited (sensitivity 74–79%), but, together, these four tests will detect almost all (95.9%) farmers with IgE‐mediated sensitization and all (100%) with symptoms. We have also shown that the grain weevil Sitophilus grcmarius and the botfly Gaslerophihts intestinalis are important sources of allergens in rural environments.

Abstract:The gut microbiota is vital for human body development and function. Its development in early life is influenced by various environmental factors. In this randomized controlled trial, the gut microbiota was obtained as a secondary outcome measure in a study on the effects of one hour of daily skin-to-skin contact (SSC) for five weeks in healthy full-term infants. Specifically, we studied the effects on alpha/beta diversity, volatility, microbiota maturation, and bacterial and gut-brain-axis-related functional abundances in microbiota assessed thrice in the first year. Pregnant Dutch women ( n = 116) were randomly assigned to the SSC or care-as-usual groups. The SSC group participants engaged in one hour of daily SSC from birth to five weeks of age. Stool samples were collected at two, five, and 52 weeks and the V4 region was sequenced. We observed significant differences in the microbiota composition, bacterial abundances, and predicted functional pathways between the groups. The SSC group exhibited lower microbiota volatility during early infancy. Microbiota maturation was slower in the SSC group during the first year and our results suggested that breastfeeding duration may have partially mediated this relation. Our findings provide evidence that postpartum SSC may influence microbiota development. Replication is necessary to validate and generalize these results. Future studies should include direct stress measurements and extend microbiota sampling beyond the first year to investigate stress as a mechanism and research SSC's impact on long-term microbiota maturation trajectories.

Abstract:Over the observation period, skin‐to‐skin infants displayed a more relaxed and drowsy state compared to those held dressed in their mothers' arms. There were no significant differences in vital signs between the groups. Infants in the skin‐to‐skin group initiated breastfeeding significantly faster (p = 0.005) than those in the mothers' arms group. Background The purpose of this study was to compare the effects of two caregiving models on full‐term healthy infants' wakefulness, rooting and sucking reflexes, initiation of breastfeeding, and physiological parameters when reunited with their mothers after a mother–infant separation of 130 min after elective cesarean birth. Methods Ninety‐five mother–infant pairs participated in a randomized controlled trial, in which full‐term healthy infants were allocated to be either dressed in their mothers' arms (n = 56) or skin‐to‐skin with their mother (n = 39) when reunited with the mother within 130 min after cesarean birth. Data were collected by the Neonatal Behavioral Assessment Scale (NBAS) to assess the infants' wakefulness and prebreastfeeding behaviors. Physiological parameters were assessed at 15 min intervals, from 130 to 205 min after birth. Time to first breastfeed was measured in minutes from the reunion with the mother. Results The primary finding was that physiologic parameters did not differ but time for initiation of breastfeeding after the reunion with the mother was significantly faster in the skin‐to‐skin group compared to the infants in the mothers' arms group (p = 0.005). Over the full study period, a more relaxed state and drowsy were found in the skin‐to‐skin group compared to the infants in the mothers' arms group. Conclusion Healthy full‐term infants born by elective cesarean, who were cared for by their mothers when reunited within 130 min of separation and cared for by their fathers during the mother–infant separation, initiated breastfeeding successfully and showed stable physiological patterns.

Exploring outcomes for women and neonates having skin-to-skin contact during caesarean birth: A quasi-experimental design and qualitative study

Abstract:Problem: Women having an elective caesarean birth are often separated from their babies at birth with newborns transferred to a postnatal ward with the significant other. Background: Two midwives were employed in 2019 to provide skin-to-skin contact for women who planned for elective caesarean births in a public hospital in metropolitan New South Wales with 4000 births per year and a 39% CB rate (57.8% of these births being elective). Aim: To compare the outcomes for women and their newborns on the effects of skin-to-skin contact at elective caesarean births within the first five minutes of birth to those who did not have skin-to-skin contact and to explore the lived experiences of women having skin-to-skin contact during their elective caesarean births. Methods: A quasi-experimental design study with a qualitative component of in-depth interviews. Quantitative analyses included independent t-tests, chi square and logistic regression. Thematic analysis was used for the qualitative data. Findings: In the quantitative results, there was a reduction in the time to the first feed (t(100) = -11.32, p < 0.001) (M = 38.9, SE = 20.7) (M = 124.9, SE = 50.1) and the first breastfeed (t(100) = -5.2, p < 0.001) (M = 53.2, SE = 82.5) (M = 277, SE = 295.8) with increased breastfeeding on discharge for women that had skin-to-skin contact at caesarean birth in comparison to those who did not receive skin-to-skin contact χ2(1) = 10.22, p < 0.05. In the qualitative results, women who had skin-to-skin contact during their caesarean birth had a positive experience with improved bonding and reported less anxiety and depression than their previous caesarean birth. Conclusion: This study provides evidence of the benefits of skin-to-skin contact during a caesarean birth.

Abstract:The study aimed to compare the effects of breastfeeding education and skin-to-skin contact on breastfeeding efficiency and maternal attachment. This study was a three-group randomized controlled study. This study was conducted with 92 women in a delivery room in Türkiye between October 2021 and May 2022. The study consists of continuous early SSC (Group A), breastfeeding education (Group B), and control groups (Group C). Group A received SSC education and Group B received structured breastfeeding education. The groups were homogeneous in terms of sociodemographic and obstetric characteristics ( p > 0.05). The levels of maternal attachment and breastfeeding activity of mothers in Group A were effective ( p < 0.001). While SSC education was more effective than breastfeeding education in terms of these variables ( p < 0.001), no significant difference was found in the control group. These findings may encourage professional support in enhancing breastfeeding efficiency and maternal attachment. . Trial registration: ClinicalTrials.gov identifier: NCT05433818.

Abstract:Background: Skin-to-skin contact (SSC) is crucial in neonatal intensive care unit (NICU) care; yet, challenges exist in its implementation. This study addresses a knowledge gap by examining the most effective ways to introduce SSC in the NICU, emphasizing the need to seamlessly integrate this practice for optimal infant and parent care. One way, among many, to implement SSC is to increase staff's knowledge and awareness about SSC. Objective: To describe the effect of an educational intervention on SSC time, the number of SSC sessions per day, and the number of infants cared for with SSC within their first day of life. Methods: A pre- and postinterventional study design was used to determine the effect of an educational intervention on SSC time, the number of SSC sessions per day, and the number of infants cared for with SSC within their first day of life. Results: The prevalence of infants receiving SSC during their first 24 hours of life, the number of SSC sessions per day, and the duration of each SSC session increased after the intervention, and these effects persisted over 2 years. Implications for practice: Our study highlights the impact of staff education and reflective discussions on SSC in the NICU. Integrating these with practical training, parental education, and NICU environment modifications is pivotal for enhancing and sustaining optimal SSC practices, benefiting both infants and parents.

Abstract:Background: Twenty-to-forty percent of women experience postpartum depressive symptoms, which can affect both the mother and infant. In preterm infants, daily skin-to-skin contact (SSC) between the mother and her infant has been shown to decrease maternal postpartum depressive symptoms. In full-term infants, only two studies investigated SSC effects on maternal depressive symptoms and found similar results. Research in preterm infants also showed that SSC improves other mental and physical health outcomes of the mother and the infant, and improves the quality of mother-infant relationship. This randomized controlled trial will investigate the effects of a SSC intervention on maternal postpartum depressive symptoms and additional outcomes in mothers and their full-term infants. Moreover, two potential underlying mechanisms for the relation between SSC and the maternal and infant outcomes will be examined, namely maternal oxytocin concentrations and infant intestinal microbiota. Methods/design: Design: A parallel-group randomized controlled trial. Participants: 116 mothers and their full-term infants. Intervention: Mothers in the SSC condition will be requested to provide daily at least one continuous hour of SSC to their infant. The intervention starts immediately after birth and lasts for 5 weeks. Mothers in the control condition will not be requested to provide SSC. Maternal and infant outcomes will be measured at 2 weeks, 5 weeks, 12 weeks and 1 year after birth. Primary outcome: maternal postpartum depressive symptoms. Secondary maternal outcomes: mental health (anxiety, stress, traumatic stress following child birth, sleep quality), physical health (physical recovery from the delivery, health, breastfeeding, physiological stress), mother-infant relationship (mother-infant bond, quality of maternal caregiving behavior). Secondary infant outcomes: behavior (fussing and crying, sleep quality), physical health (growth and health, physiological stress), general development (regulation capacities, social-emotional capacities, language, cognitive and motor capacities). Secondary underlying mechanisms: maternal oxytocin concentrations, infant intestinal microbiota. Discussion: As a simple and cost-effective intervention, SSC may benefit both the mother and her full-term infant in the short-and long-term. Additionally, if SSC is shown to be effective in low-risk mother-infant dyads, then thought could be given to developing programs in high-risk samples and using SSC in a preventive manner. Abstract:Abstract Introduction The World Health Organization recommends that infants be exclusively breastfed for at least the first 6 months of life, and then partially breastfed until 2 years of age and onward, if preferred. However, the choice to breastfeed an infant is influenced by cultural, biological, and social factors. For instance, factors which may promote breastfeeding include bedsharing, but the current literature requires more longitudinal examinations and follow-ups. The current project aims to investigate the associations between bedsharing at 6 months and the duration of breastfeeding. Methods The analysis included data from 294 mother-infant dyads (52 bedshared) from the Maternal Adversity, Vulnerability and Neurodevelopment cohort. Maternal reports of their infant’s sleep location were reported at 6 months of infancy. Infants who shared a bed with their mother were considered to bedshare, whereas those who slept in the same room as their mother, in their own room alone, or in their own room with someone else where not considered to bedshare. A retroactive measure of breastfeeding was given to mothers (until infants were 24 months), in which they were asked how old the infant was when they stopped breastfeeding. An independent samples t-test was conducted to compare breastfeeding duration between infants who bedshare and those who do not. Results Bedsharing infants were breastfed for a longer period (in months; M=12.87, SD=5.21) than infants who did not bedshare (M=10.64, SD=3.97; t(292)=-3.48, p=<.001). Conclusion These results show that infants who bedshare with their mother are likely to be breastfed for about 2 months more. These results provide support for the protective nature of bedsharing at 6 months on breastfeeding duration. Although our nurses verbalized the benefits for postpartum patients and newborns, our rates of skin-to-skin contact for vaginal and cesarean birth and initiation of chestfeeding were below the recognized benchmarks, particularly in our highest ethnic group.

Abstract:As an essential part of Early Essential Newborn Care, 90 minutes of mother-infant skin-to-skin contact is significant in improving maternal and infant outcomes. However, due to human resource constraints and the consideration of maternal and infant safety, it is difficult to achieve continuous uninterrupted skin-to-skin contact for at least 90 minutes during and after cesarean delivery. The aim of this study was to investigate the efficacy and safety of the continuous uninterrupted skin-to-skin contact for at least 90 minutes during and after cesarean section for exclusive breastfeeding rate during hospitalization and maternal and infant health indicators during and after cesarean delivery.
