Abstract:Background: Although the use of expanded-criteria donors (ECDs) alleviates the problem of organ shortage, it significantly increases the incidence of delayed graft function (DGF). DGF is a common complication after kidney transplantation; however, the effect of DGF on graft loss is uncertain based on the published literature. Hence, the aim of this study was to determine the relationship between DGF and allograft survival. Methods: We conducted a retrospective, multicenter, observation cohort study. A total of 284 deceased donors and 541 recipients between February 2012 and March 2017 were included. We used logistic regression analysis to verify the association between clinical parameters and DGF, and Cox proportional hazards models were applied to quantify the hazard ratios of DGF for kidney graft loss. Results: Among the 284 deceased donors, 65 (22.8%) donors were ECD. Of the 541 recipients, 107 (19.8%) recipients developed DGF, and this rate was higher with ECD kidneys than with standard-criteria donor (SCD) kidneys (29.2% vs. 17.1%; P = 0.003). The 5-year graft survival rate was not significantly different between SCD kidney recipients with and without DGF (95.8% vs. 95.4%; P = 0.580). However, there was a significant difference between ECD kidney recipients with and without DGF (71.4% vs. 97.6%; P = 0.001), and the adjusted hazard ratio (HR) for graft loss for recipients with DGF was 1.885 (95% confidence interval [CI] = 1.305-7.630; P = 0.024). Results showed that induction therapy with anti-thymocyte globulin was protective against DGF (odds ratio = 0.359; 95% CI = 0.197-0.652; P = 0.001) with all donor kidneys and a protective factor for graft survival (HR = 0.308; 95% CI = 0.130-0.728; P = 0.007) with ECD kidneys. Conclusion: DGF is an independent risk factor for graft survival in recipients with ECD kidneys, but not SCD kidneys.
Delayed Graft Function in Kidney Retransplantation: United Network for Organ Sharing Data With Linked Primary and Retransplant

Abstract:Introduction: There are several articles exploring the risk factors for primary delayed graft function (DGF). However, current literature does not include many resources on the risk factors for DGF when it is a recipient's second kidney transplant or look at short-term graft and patient survival of DGF retransplants. Methods: United Network for Organ Sharing data from January 2008 to June 2021 were analyzed. Pancreas transplants, multi-organ transplants, and lost to follow-up transplants were excluded. Second transplant patients with DGF were identified. Multivariate logistic regression models based on the primary and second transplant characteristics were created. Survival analysis was performed with Kaplan-Meier methodology and assessed with log-rank test. Results: A total of 2964 second kidney transplants were identified. Rate of DGF in the second transplant was 28.4% (843/2964) and 49.2% of them had a prior DGF in their first transplant (P < 0.001). Multivariate analysis confirmed that occurrence of DGF (odds ratio [OR] 1.5, P < 0.001) and graft loss due to acute rejection (OR 1.2, P < 0.005) in the primary transplant were predictors of reappearing DGF in the second transplant. Dialysis at transplant was the greatest risk factor from the second transplant (OR 3.539, P < 0.001). There was a decreased graft survival after 12 mo (77% versus 49% with log t-test <0.001) in the second transplant. However, DGF was not significantly associated with patient survival. Conclusions: This study shows the interaction between primary and second transplant in developing DGF. Survival analysis shows lower graft survival for retransplants in the case of DGF. This study opens the possibility of identifying additional risk factors for patients undergoing retransplant surgeries.
Risk Factors and Outcome of Prolonged Recovery from Delayed Graft Function after Deceased Kidney Transplantation

Abstract:Objective To investigate the risk factors of prolonged recovery from delayed graft function (DGF) after deceased kidney transplantation and to evaluate the effect of prolonged recovery on the outcome. Methods We retrospectively analyzed 91 recipients with DGF after deceased kidney transplantation from 2007 to 2016 in our center. DGF recovery time is defined as the time required to achieve stable serum creatinine level. Recipients with DGF recovery time ≥ the median were assigned to the prolonged recovery group while the rest to the rapid recovery group. Result Ninety-one recipients with regular follow-up until graft function recovery were further divided into fast and prolonged recovery group. The median recovery time was 27 days (Interquartile range: 15-45 days). Donor terminal glomerular filtration rate (GFR) was significantly lower in the prolonged recovery group (N = 46) compared with the rapid recovery group (N = 45) (median 24.9 vs 65.4 ml/min/1.73m2, P=0.0036). There was a significant decrease in the GFR at 1 year post-transplant in the prolonged recovery group compared with the rapid recovery group (50.6±20.0 vs 63.5±21.4 ml/min/1.73m2, P =0.005). The prolonged recovery group was also at higher risk of composite end-point (acute rejection, pneumonia, graft failure and death; hazards ratio 2.604, 95% confidence interval 1.102-6.150, P=0.029) compared with the rapid recovery group. Conclusion Donor acute kidney injury is a risk factor of prolonged recovery from DGF after deceased kidney transplantation. Prolonged recovery time is associated with restricted recovery of graft function and poor transplant outcome.

Early readmissions post kidney transplantation: lessons learned

Abstract:Introduction: Kidney transplantation (KT) is the gold standard treatment for end-stage renal disease (ESRD) patients. Hospital readmissions post-transplant is a common complication and can be considered an indication of avoidable morbidity and hospital quality, and there is a significant correlation between EHR and adverse patient outcomes. This study aimed to assess the readmission rate following kidney transplants, the underlying causes, and possible ways to prevent it. Material and methods: We retrospectively reviewed the medical records of recipients from January 2016 to December 2021 in a single center. The primary objective of this study is to find the readmission rate for kidney transplants and the variables that contribute to readmission. Post-transplant complications that were resulted in the readmission categorized into surgical complications, graft-related complications, infections, DVT, and other medical complications. Results: Four hundred seventy-four renal allograft recipients met our inclusion criteria and were included in the study. 248 (52.3%) of the allograft recipients had at least one readmission during the first 90 days after the transplantation. 89 (18.8%) allograft recipients had more than one readmission episode in the first 90 days post-transplant. The perinephric fluid collection was the most common surgical complication (52.4%), and UTI was the most common infection (50%), causing readmission in the first 90 days post-transplant. The readmission odd ratio was significantly higher in patients above 60 years old and in kidneys with KDPI ≥ 85, and in recipients with DGF. Conclusion: Early hospital readmission (EHR) following a kidney transplant is a common complication. Identifying the causes not only helps the transplant centers to take further steps to prevent some incidents and help to improve the patients' morbidities and mortalities, but also it can reduce the unnecessary costs of readmissions.

Abstract:Background: The protracted recovery of renal function may be an actionable marker of post-transplant adverse events, but a paucity of data are available to determine if the duration of graft recovery serves to stratify risk. Materials and Methods: Single-center data of adult-isolated deceased-donor kidney transplant (KTX) recipients between 1 July 2015 and 31 December 2018 were stratified by delayed graft function (DGF) duration, defined as time to serum creatinine < 3.0 mg/dL. Results: Of 355 kidney transplants, the time to creatinine < 3.0 mg/dL was 0–3 days among 96 cases (DGF ≤ 3), 4–10 days among 85 cases (DGF4-10), 11–20 days among 93 cases (DGF11-20), and ≥21 days for 81 cases (DGF ≥ 21). DGF ≥ 21 recipients were significantly more likely to be male, non-sensitized, and receive kidneys from donors that were older, with donation after circulatory death, non-mandatory share, hypertensive, higher KDPI, higher terminal creatinine, and longer cold and warm ischemia time. On multivariate analysis, DGF ≥ 21 was associated with a 5.73-fold increased odds of 12-month eGFR < 40 mL/min compared to DGF ≤ 3. Lesser degrees of DGF had similar outcomes. Conclusions: Prolonged DGF lasting over 20 days signifies a substantially higher risk for reduced eGFR at 1 year compared to lesser degrees of DGF, thus serving as a threshold indicator of increased risk.

Abstract:Background/Aims: Hemodialysis (HD) or peritoneal dialysis (PD) is an important renal replacement method in patients with delayed graft function (DGF) after kidney transplantation; however, it is not clear which dialysis modality is superior. This study determined the impact of different dialysis modalities on patients with DGF. Methods: It was a single-center, retrospective and descriptive study. We performed 673 kidney transplants from donors after cardiac death (DCD) between January 2010 and December 2016 at our center and 138 (20.5%) recipients developed DGF after transplantation. We classified the recipients into two groups according to post-transplant dialysis: DGF-HD (n = 96) and DGF-PD (n = 42). We analyzed the outcomes of the different dialysis modalities 30 days and 1 year post-transplantation. Results: There were no differences in baseline factors between patients with post-transplant HD (n = 96) or PD (n = 42). There were 10 patients with conversion from PD to HD during DGF. The DGF-PD patients had a higher rate of treatment failure than the DGF-HD patients (23.8% vs. 0%, p < 0.001), peritonitis (7.1% vs. 0%, p = 0.027), and longer duration of dialysis dependence (10.5 vs. 9 days, p = 0.003). There was no statistically significant difference between both groups with respect to acute rejection, hemorrhage, and patient and graft survival at 1 year. Conclusion: In renal transplant recipients with DGF, post-transplant PD led to increased treatment failure. PD did not result in rapid recovery of transplanted renal function, and had a high probability of peritonitis. (C) 2018 The Author(s) Published by S. Karger AG, Basel

Abstract:The effect of delayed graft function (DGF) recovery on long-term graft outcome is unclear. The aim of this study was to examine the association of DGF recovery status with long-term outcome. We analyzed 385 recipients who underwent single kidney transplantation from brain-dead donors between 2004 and 2015. Patients were grouped according to renal function at 1 month post-transplantation: control (without DGF); recovered DGF (glomerular filtration rate [GFR] ≥ 30 mL/min/1.73 m2); and incompletely recovered DGF group (GFR < 30 mL/min/1.73 m2). DGF occurred in 104 of 385 (27%) recipients. Of the DGF patients, 70 recovered from DGF and 34 incompletely recovered from DGF. Death-censored graft survival rates for control, recovered DGF, and incompletely recovered DGF groups were 95.3%, 94.7%, and 80.7%, respectively, at 5 years post-transplantation (P = 0.003). Incompletely recovered DGF was an independent risk factor for death-censored graft loss (HR = 3.410, 95%CI, 1.114-10.437). DGF was associated with increased risk for patient death regardless of DGF recovery status. Mean GFRs at 5 years were 65.5 ± 20.8, 62.2 ± 27.0, and 45.8 ± 15.4 mL/min/1.73 m2 for control, recovered, and incompletely recovered DGF groups, respectively (P < 0.001). Control group and recovered DGF patients had similar renal outcomes. However, DGF was associated with increased risk for patient death regardless of DGF recovery status.

Abstract:Abstract Background and Aims Delayed graft function (DGF) is a common complication after renal transplant. Definitions of DGF are based on analysis of estimated glomerular filtration rate (eGFR) underlying the plasma creatinine fluctuations after renal transplant. We tried to validate an early parameter of DGF in a renal transplant population based on the kinetic estimation of glomerular filtration rate, since this formula adds a quantitative dimension to the assessment of kidney function, and consequently predicting DGF could get a better post-transplant management. Method This is a longitudinal study of 886 renal transplant recipients from our center during the first post-transplant month. As we are searching for functional biochemical markers in renal transplant from decesased donors, we excluded patients with primary gtaft disfunction.The final simple was composed of 574 adult kidney transplant recipients, 348 renal transplant from donors after circulatory death (DCD) and 226 from donors after brain death (DBD). The following are analyzed 5creatinine concentrations, serum Lactate dehydrogenase (LDH) and urinary creatinine excretion rate (CER). Results In DCD there was a significant correlation between DGF duration and all KeGFR and CER determinations during the first post-transplant week. The parameters that achieved the best correlation to predict DGF were KeGFR (r = 0.515; p<0.001) and CER on the fourth day (r = 0.584; p<0.001) (Figure 1). In the DBD, a weak correlation was observed between DGF duration and KeGFR determinations in the first days, being KeGFR at the fourth day the one that presented the highest correlation (r = 0.59 p<0.001). We observed that serum LDH on the first day in the DBD group is associated with worse renal graft function at first, third month and one year after transplantation (p<.045, p<.05 and p<.067 respectively). Conclusion The determination of KeGFR and CER could predict the duration of DGF, especially in DCD recipients. DCD recipients with DGF have significantly better graft survival at one year than DBD recipients with DGF. (p<.001).

Abstract:Kidney transplantation has become a routine procedure for end-stage renal disease. Although improvement in short term graft patency has been achieved, delayed graft function (DGF) is one of the important complications following kidney transplantation. DGF results from ischemia-reperfusion injury and involves a multitude of contributing elements including donor characteristics and etiology of the end-stage renal disease. The neutrophil-lymphocyte ratio (NLR) and platelet-lymphocyte ratio (PLR) are easily applicable methods for evaluation of systemic inflammation. We hypothesized that these parameters could refine the detection of the patients with DGF in post- renal transplantation and guide management. To test our hypothesis, we investigated whether the NLR, PLR, and creatinine reduction ratio CRR could serve as prognostic indicators for identifying patient cohorts at an elevated risk of negative treatment outcomes following renal transplantation. We also assessed demographic and clinical factors as an independent predictor to detect negative early graft function outcomes in patients after renal transplantation. This single center retrospective cohort trial comprising 55 patients who received a kidney graft from deceased or living donors was performed in Northern Cyprus Ministry of Health’s Dr. Burhan Nalbantoglu State Hospital, over a 6 year period. The demographic and clinical variables included age, gender, body mass index, the etiology of end-stage renal disease, kidney graft from living or deceased donors, NLR, PLR and CRR. DGF was defined as GFR<60 on the 21th day following renal transplantation. Of the patients included in this study, 26 (47.3%) and 29 (52.7%) received deceased and living donor kidney transplantation, respectively. On the 21st day following transplantation, 27 patients (49%) had an eGFR<60, whereas 28 patients (51%) had an GFR≥60. None of the patients had undergone dialysis within the first week after transplantation. There were no significant differences in demographic and clinical characteristics between the patients with eGFR 60 and eGFR 60. In logistic regression analysis, none of the predictors including etiology of the end-stage renal disease, source of donor, or demographic baseline characteristic were associated with DGF. We suggest that the NLR and PLR parameters are not associated with the presence of DGF after renal transplantation. Abstract:Delayed graft function (DGF) is an early posttransplant complication predictive of adverse outcomes. This “acute kidney injury of transplantation” is often defined as allograft dysfunction requiring renal replacement within 7 d posttransplantation. DGF is an important area of study because it is emerging with efforts to expand the donor pool and address the supply-demand gap in kidney transplantation. DGF is often caused by severe kidney injury mechanisms because of multiple donors, recipients, and immunologic factors. The role of kidney biopsy, particularly in prolonged DGF, is an ongoing area of research and inquiry for clinicians and researchers alike to better define, manage, and predict outcomes of this early posttransplant event. This review aims to provide an in-depth, comprehensive summary of the literature to date on the histopathology of DGF and the role of kidney transplant biopsies in prolonged DGF.

Risk Factors of Kidney Re-Transplantation Recipients and Graft Survival During the Long-Term Follow-Up

Abstract:Objective To investigate the risk factors of kidney re-transplantation recipients and graft survival during the long-term follow-up. Methods The clinical data of kidney transplant donors and recipients performed at the Kidney Disease Center of the First Affiliated Hospital of Zhejiang University from January 1, 1991 to December 31, 2017 were retrospectively analyzed. A total of 5 374 cases of first kidney transplantation and 37 cases of re-transplantation were included. In addition, the recipients of re-transplantation were divided into long-term survival group (19 cases, >5 years) and short-term survival group (18 cases, ≤5 years). The age of donor and recipient, the interval between first and second kidney transplantation, HLA mismatch and cold/warm ischemia time of donor and recipient in long-term and short-term survival groups were compared by group t test. Chi-square test was used to compare the gender of recipients, type of donor kidney, rate of positive panel reactive antibodies before and after re-transplantation, initial graft nephrectomy, immune induction before re-transplantation, delayed graft function (DGF) and acute rejection in two groups. The 1, 5 and 10-year patient/graft survival rates were calculated by Kaplan-Meier method. The Cox proportional hazard model was used to analyze the possible risk factors on long-term survival of transplanted kidney. P<0.05 was statistically significant. Results As of March 1, 2018, the median follow-up time of re-transplantation group was 152 months (11-323 months), 2 recipients died, 18 reipients suffered from renal allograft dysfunction and 17 patients had stable renal transplant function. The median follow-up time of the first transplantation group was 108.9 months (0.1-350.0 months). 459 cases died and 1 343 cases suffered from renal transplant failure. The 1, 5 and 10-year recipient/graft survival rates in the re-transplantation group were 86%/81%, 86%/62% and 82%/36%, and the 1, 5 and 10-year recipient/graft survival rates in the first transplantation group were 99%/98%, 93%/89% and 88%/80%. The 1, 5 and 10-year recipients survival rates of re-transplantation group were lower than those in the first transplantation group (χ2= 60.816, 25.110 and 43.900, P<0.05), and the 1-year recipients survival rate in the re-transplantation group was lower than the first transplantation group (χ2= 40.409, P<0.05). Univariate analysis showed that there were significant differences in DGF and acute rejection rates between long-term and short-term survival groups (χ2=4.039 and 4.748, P<0.05). Cox regression analysis showed that DGF and acute rejection were independent risk factors for long-term survival of transplanted kidneys in re-transplanted recipients (RR=4.317 and 4.571, P<0.05). Conclusions The survival rate of re-transplant recipients is lower than that of the first renal transplant recipients. DGF and acute rejection are independent risk factors affecting the survival of re-transplant recipients. Effect of delayed graft function on longer‐term outcomes after kidney transplantation from donation after circulatory death donors in the United Kingdom: A national cohort study

Abstract:<p>Kidneys from donation after circulatory death (DCD) donors are utilized variably worldwide, in part due to high rates of delayed graft function (DGF) and putative associations with adverse longer‐term outcomes. We aimed to determine whether the presence of DGF, and its duration, were associated with poor longer‐term outcomes after kidney transplantation from DCD donors. Using the UK transplant registry, we identified 4714 kidney‐only transplants from controlled DCD donors to adult recipients between 2006‐2016; 2832 recipients (60·1%) had immediate graft function and 1882 (39.9%) had DGF. Of the 1847 recipients with DGF duration recorded, 926 (50·1%) had DGF <7 days, 576 (31·2%) had DGF 7‐14 days, and 345 (18·7%) had DGF >14 days. After risk adjustment, the presence of DGF was not associated with inferior long‐term graft or patient survivals. However, DGF duration of >14 days was associated with an increased risk of death‐censored graft failure (hazard ratio 1·7, p=0·001) and recipient death (hazard ratio 1·8, p<0·001) compared to grafts with immediate function. This study suggests that shorter periods of DGF have no adverse influence on graft or patient survival after DCD donor kidney transplantation and that DGF >14 days is a novel early biomarker for significantly worse longer‐term outcomes.</p>

Abstract:Introduction: Delayed graft function (DGF) is a common condition that necessitates dialysis during the first week after transplantation. Although DGF rarely occurs following living-donor kidney transplantation (LDKT), it may eventually lead to acute or chronic graft rejection. This study aimed to assess the risk factors for DGF in patients who underwent LDKT. Methods: A systematic review and meta-analysis of studies published before August 2022 was conducted using the PubMed, Science Direct, Cochrane, and Directory of Open Access Journal (DOAJ) databases. The review included studies that assessed the incidence of DGF following LDKT, and examined its risk factors, while excluding studies involving deceased donors. Potential risk factors were analyzed using pooled mean differences or odds ratios with 95% confidence intervals (CIs). Review Manager 5.3 was used for the meta-analysis. Results: Among the 13 included studies, 3685 cases of DGF were identified in a total of 113,261 patients (3.25%). Potential risk factors for DGF following LDKT were examined across several aspects, including donor, recipient, donor/recipient relationship, and immunological and intraoperative factors. The identified risk factors included older donors (P = 0.07), male recipients (P < 0.0001), higher recipient body mass index (BMI) (P < 0.0001), non-white recipients (P < 0.0001), pre-existing diabetes (P < 0.0001), pre-existing hypertension (P = 0.01), history of dialysis (P < 0.0001), re-transplantation (P = 0.004), unrelated donor/recipient (P = 0.02), ABO incompatibility (P < 0.0001), higher panel reactive antibody (PRA) levels (P < 0.0001), utilization of right kidney (P < 0.0001), and longer cold ischemia time (CIT) (P = 0.004). Conclusion: Several factors related to the donor, recipient, donor/recipient relationship, and immunological and intraoperative aspects were identified as potential risk factors for the development of DGF following LDKT. Addressing and optimizing these factors may improve the long-term outcomes of LDKT.
The Prognostic Implications of Renal Function Recovery and Delayed Graft Function in Kidney Transplantation

Abstract:Introduction In the general population, lack of renal function recovery (RFR), 90 days after acute kidney injury (AKI) is a contributor to inferior renal outcomes and mortality. Ischemia and reperfusion during kidney transplantation (KT) contributes to AKI and delayed graft function (DGF), a severe form of AKI. Our aim was to analyze the long-term prognostic implications of RFR at 90 days, in recipients with and without DGF. Methods This was a retrospective analysis of adult deceased donor KT recipients (1996 -2016). Outcome of interest was death censored graft failure (DCGF). RFR was calculated using the formula (observed eGFR&sol;predicted eGFR) X 100. Predicted eGFR was half of the donor eGFR plus pre-emptive recipient eGFR, and observed eGFR was the average of 3 best values, 90 days post-KT. Grafts with primary non-function were excluded. The Chronic Kidney Disease Epidemiology Collaboration prediction equation was used to determine the eGFR. Results 941 KT recipients were eligible for analysis, of which 25&percnt; had DGF. RFR was divided into 3 tertiles <75&percnt; (360), 75-100&percnt; (218) and >100&percnt; (363). Higher DCGF was noted in recipients that developed DGF, and had RFR<75&percnt; (Figure 1). In recipients that developed DGF, RFR had no further prognostic implications on DCGF (Table 1). However, in those that did not develop DGF, RFR<75&percnt; was associated with an 82&percnt; and 89&percnt; higher DCGF in univariate and multivariate analysis, respectively. Figure. No caption available. Table. No title available. Conclusion In recipients that do not develop DGF, 90-day RFR<75&percnt; was associated with DCGF. This indicates the prognostic relevance of RFR as a short-term outcome as most KT do not develop DGF. We propose that to truly capture the impact of AKI and ischemia and reperfusion injury during the transplantation surgery, one must quantify RFR over 90 days after KT.

Abstract:Introduction: The demographics of donor and recipient candidates for kidney transplantation (KT) have substantially changed. Recipients tend to be older and polymorbid and KT to suboptimal recipients is associated with delayed graft function (DGF), prolonged hospitalization, inferior long-term allograft function, and poorer patient survival. In parallel, donors are also older, suffer from several comorbidities, and donations coming from circulatory death (DCD) predominate, which in turn leads to early and late complications. However, it is unclear how donor and recipient risk factors interact. Methods: In this retrospective cohort study, we assess the impact of a KT from suboptimal donors to suboptimal recipients. We focused on: 1) DGF; 2) hospital stay and number of dialysis days after KT and 3) allograft function at 12 months. Results and discussion: Among the 369 KT included, the overall DGF rate was 25% (n = 92) and median time from reperfusion to DGF resolution was 7.8 days (IQR: 3.0-13.8 days). Overall, patients received four dialysis sessions (IQR: 2-8). The combination of pre-KT anuria (<200 ml/24 h, 32%) and DCD procurement (14%) was significantly associated with DGF, length of hospital stay, and severe perioperative complications, predominantly in recipients 50 years and older.

Abstract:Delayed graft function ( DGF ) remains a major barrier to improved outcomes after kidney transplantation. High‐risk transplant recipients can be identified, but no definitive prediction model exists. Novel biomarkers to predict DGF in the first hours post‐transplant, such as neutrophil gelatinase‐associated lipocalin ( NGAL ), are under investigation. Donor management to minimize the profound physiological consequences of brain death is highly complex. A hormonal resuscitation package to manage the catecholamine “storm” that follows brain death is recommended. Donor pretreatment with dopamine prior to procurement lowers the rate of DGF . Hypothermic machine perfusion may offer a significant reduction in the rate of DGF vs simple cold storage, but costs need to be evaluated. Surgically, reducing warm ischemia time may be advantageous. Research into recipient preconditioning options has so far not generated clinically helpful interventions. Diagnostic criteria for DGF vary, but requirement for dialysis and/or persistent high serum creatinine is likely to remain key to diagnosis until current work on early biomarkers has progressed further. Management centers on close monitoring of graft (non)function and physiological parameters. With so many unanswered questions, substantial reductions in the toll of DGF in the near future seem unlikely but concentrated research on many levels offers long‐term promise.
