Abstract:Purpose: To examine the influence of subfoveal choroidal thickness (SFCT) and choroidal vascularity index (CVI) on axial length (AL) elongation over a 2-year period in highly myopic children. Methods: In this is prospective, longitudinal, observational study, 163 participants (74%), who were 8 to 18 years of age with bilateral high myopia (sphere ≤ -6.0 D) and without pathologic myopia, completed follow-up visits over 2 years. All participants underwent baseline and follow-up ocular examinations, including swept-source optical coherence tomography (SS-OCT) and AL measurements. SFCT and CVI were derived from SS-OCT scans using a deep-learning-based program for choroidal structure assessment. Results: The mean age of the participants at baseline was 15.0 years (±2.3), with males constituting 47% of the cohort. An inverse relationship was observed between AL elongation and increases in baseline age, baseline SFCT, and CVI, as well as a decrease in baseline AL. Adjusting for other factors, every 10-µm increase in SFCT and each 1% increase in CVI were associated with decreases in AL elongation of 0.007 mm (95% confidence interval [CI], -0.013 to -0.002; P = 0.011) and 0.010 mm (95% CI, -0.019 to 0.000; P = 0.050), respectively. The incorporation of SFCT or CVI into predictive models improved discrimination over models using only age, gender, and baseline AL (both P < 0.05, likelihood ratio test). Conclusions: Our findings suggest a possible association between a thinner choroid and increased AL elongation over 2 years in children with high myopia, after adjusting for potential baseline risk factors such as age, gender, and initial AL.
Two-year longitudinal change in choroidal and retinal thickness in school-aged myopic children: exploratory analysis of clinical trials for myopia progression

Abstract:Abstract Background With increasing axial length and myopia progression, the micro-structure of the retina and choroid gradually changes. Our study describes the longitudinal changes in retinal and choroidal thickness in school-aged children with myopia and explores the relationship between changes in choroidal thickness and myopia progression. Methods An exploratory analysis of a randomized trial was performed. Children (n = 168, aged 7 to 12 years) with myopia from − 0.75 dioptre (D) to − 4.00 D were enrolled in this prospective longitudinal study. Cycloplegic refraction, axial length (AL), retinal and choroidal thicknesses were measured at baseline and at 1- and 2-year follow-ups. “Rapid progression myopia” was defined as increasing in myopia > 1.00 D and “stable progression myopia” was ≤ 1.00 D during the 2-year follow-up. Factors affecting the changes in choroidal thickness were analysed using linear mixed models. Results AL significantly increased by 0.67 ± 0.24 mm with a myopic shift of − 1.50 ± 0.64 D over the 2 years. The overall retinal thickness increased from 251.12 ± 15.91 μm at baseline to 253.47 ± 15.74 μm at the 2-year follow-up (F = 23.785, P < 0.001). The subfoveal choroidal thickness decreased from 231.03 ± 54.04 μm at baseline to 206.53 ± 59.71 μm at the 2-year follow-up (F = 73.358, P < 0.001). Choroidal thinning was significantly associated with AL elongation (β = − 43.579 μm/mm, P = 0.002) and sex (β = − 17.258, P = 0.001). Choroidal thickness continued to decrease in subjects with rapid progression (F = 92.06, P < 0.001) but not in those with steady progression (F = 2.23, P = 0.119). Conclusion Significant choroidal thinning was observed and was associated with rapid progression and sex. These findings indicate a need to understand the role of the choroid in eye growth and myopia development. Synopsis/Precis The macular choroidal thickness of myopic children is relevant to different degrees of myopic progression in this 2-year longitudinal study. Abstract:Abstract Objectives To characterize choroidal vascular changes in children with different refractive status. Methods A study including 5864 children aged 6–9 years was performed to investigate the choroidal vascular index (CVI) in myopic, emmetropic and hyperopic eyes. Each participant had a comprehensive ocular examination with cycloplegic autorefraction performed, axial length (AL) measured and Swept Source-Optical Coherence Tomography (SS-OCT) scans acquired. Choroidal thickness (ChT) was measured by built-in software, and CVI was calculated using a previously validated self-developed algorithm. Results The mean ChT and CVI were 275.88 ± 53.34 μm and 34.91 ± 3.83 in the macula region, and 191.96 ± 46.28 μm and 32.35 ± 4.21 in the peripapillary region. CVI was significantly lowest for myopes, followed by emmetropes and hyperopes ( P < 0.001). CVI varied between different sectors separated by the Early Treatment of Diabetic Retinopathy Study (ETDRS) grid ( P < 0.001). Macular CVI decreased horizontally from nasal to temporal quadrant with lowest in center fovea, and vertically from superior to inferior quadrants. Peripapillary CVI was highest in the nasal and lowest in the inferior sector. Multiple regression showed that spherical equivalent (SE), AL, intraocular pressure (IOP), ChT, age, and gender were significantly related to CVI ( P < 0.05). Conclusions In children, the distribution of CVI in the posterior pole is not uniform. A decreased CVI was observed from hyperopia to myopia and was associated with decreased SE, elongated AL, and choroidal thinning. Further study of changes in CVI during myopia onset and progression is required to better understand the role of the choroidal vasculature in myopia development.

Abstract:Purpose To determine the relationship of subfoveal choroidal thickness (ChT), refraction, and axial length in children, and evaluate the evolution of subfoveal ChT with time in myopic versus nonmyopic eyes. Methods A total of 229 eyes of 115 children aged 2 to 16 years were included in the study. Refraction under cycloplegia, axial length, and subfoveal ChT were measured at baseline with comparative investigations at 15 months follow-up. Results The probability for the subfoveal ChT to be thinner in myopic children compared to nonmyopic children was 0.9999. We found a relation between subfoveal ChT and axial length. At 15 months follow-up, subfoveal ChT was found to have increased in the nonmyopic eyes, but decreased in myopic patients. Conclusions A number of studies have already shown the choroid to play an important role in the process of emmetropization. We found that ChT had a different evolution in myopic children compared to nonmyopic children. A thinner choroid may predict the onset, or progression, of myopia. Further studies, with longer follow-up, are necessary to confirm this hypothesis.

Abstract:The choroid is a vascular tissue which plays a range of critical roles in the normal physiology of the eye, such as supplying the outer retina with oxygen and nutrients and the regulation of intraocular pressure. There is also substantial evidence, particularly from animal studies, that the choroid plays an important role in the regulation of eye growth and the development of common refractive errors like myopia. In recent years, advances in optical coherence tomography technology have improved our ability to image and measure the choroid in the human eye. Research using this technology over the past decade has dramatically improved our knowledge of the normal choroid, and its potential role in the regulation of eye growth and refractive error development. This review aims to provide an overview of recent work examining the normal human choroid, its changes with myopia and the possible role of the choroid in the mechanism regulating eye growth. Studies have demonstrated that choroidal thinning accompanies the development and progression of myopia, and have established a close link between eye growth and choroidal thickness changes. Dramatic thinning of the choroid is seen with high myopia, and associations are also observed between choroidal thinning and reduced vision, and the development of retinal pathology associated with high myopia. In the short-term, environmental factors known to be associated with myopia development and more rapid eye growth typically lead to a thinning of the choroid, whereas factors linked to a slowing of eye growth are typically associated with short-term choroidal thickening. Collectively, these findings suggest that the choroid is an important biomarker of eye growth in the human eye, and additional research to better understand the human choroid is likely to further our knowledge of the signals and pathways regulating eye growth, myopia development and progression.

Dynamic changes of choroidal vasculature and its association with myopia control efficacy in children during 1-year orthokeratology treatment

Abstract:Purpose: To investigate the dynamic changes of choroidal vasculature and its association in mediating ocular elongation over 1 year of orthokeratology (ortho-k) treatment. Methods: This observational study included 8-14 year old myopic children who were willing to receive ortho-k treatment or wear single vision spectacle lenses (SVL). Axial length (AL) and choroidal characteristics, including submacular luminal area (LA), stromal area (SA), total choroidal area (TCA), choroidal vascularity index (CVI), subfoveal choroidal thickness (SFCT) and choriocapillaris flow deficits (CcFD) from OCT/OCTA images, were evaluated at baseline, 3, 6 and 12 months. Results: After 1 year, AL elongation was significantly less in 25 children treated with ortho-k than in 29 children wearing SVL (0.17 ± 0.14 v.s. 0.28 ± 0.16 mm, P<0.01). In the ortho-k group, LA, SA, TCA and SFCT were significantly increased by 0.033 ± 0.059 mm2, 0.024 ± 0.035 mm2, 0.058 ± 0.087 mm2, 8.9 ± 17.4 μm, respectively, at 3 months (all P<0.05). These parameters returned to near baseline levels over the following 9 months. Meanwhile, CcFD decreased by 0.83 ± 1.09 % over 12 months, but CVI remained steady during this period. No significant changes in these choroidal parameters (except for CcFD, decreased by -0.56 ± 1.23 %) were observed in the SVL group over 12 months. Increases in SFCT, LA, SA and TCA over 12 months were significantly greater in the ortho-k group than in the SVL group. Mediation analyses showed that 44.8 % of effect on inhibiting AL elongation through ortho-k was mediated via TCA (in particular LA). Conclusions: The decelerated ocular elongation during ortho-k treatment is accompanied by increases in content of choroidal vascular lumen and stromal component. Dynamic changes of choroidal vasculature provide a possible mechanism underlying myopia control efficacy via ortho-k.
Choroidal vascular analysis in myopic eyes: evidence of foveal medium vessel layer thinning

Abstract:PurposeTo analyse morphologic features of the choroid in Non-pathological myopic eyes using spectral-domain (SD) optical coherence tomography (OCT).MethodsRetrospective analysis of enhanced depth SD-OCT images of Non-pathological myopic eyes in comparison with age-matched healthy controls was performed. Choroidal thickness (CT) and large choroidal vessel thickness (LCVT) were measured at the fovea, 750 µm nasally from fovea (N750) and 750 µm temporally (T750) from fovea. Medium choroidal vessel thickness (MCVT) was calculated by subtracting LCVT from CT. Choriocapillaris was encompassed by MCVT, given its reduced thickness. Linear regression analysis evaluated the relationship between age and axial with CT, LCVT and MCVT.ResultsThe study group comprised 42 eyes of 31 patients (mean age 46.13 ± 15.63; 15 females). Control group included 57 eyes of 34 patients (mean age of 42.3 ± 15.29; 24 females). Mean axial length in myopic eyes and control group was 26.57 ± 1.27 and 23.59 ± 0.99 mm respectively. Myopic eyes showed significant thinning of MCVT and CT at all locations (p < 0.0001) compared to controls, unlike LCVT (p > 0.05). With each decade, thinning of up to 37 µm in CT was noted along with thinning of LCVT (up to 22.6 µm) and MCVT (up to 25 µm). Each mm increase in axial length caused 38.2 µm thinning of choroid along with LCVT (<10 µm), however, MCVT showed more notable thinning (>30 µm).ConclusionSignificant thinning of MCVT was noted in non-pathological myopic eyes in comparison to healthy subjects. It appears that MCVT has stronger relationship with age and axial length.
LONGITUDINAL CHANGES IN CHOROIDAL AND RETINAL THICKNESSES IN CHILDREN WITH MYOPIC SHIFT.

Abstract:Purpose: To elucidate the development of the choroid and retina in children, and to explore changes in these during myopic shift. Methods: A total of 118 children aged 7 to 12 years participated in this 1-year longitudinal study. Children underwent several examinations at baseline and follow-up, including cycloplegic refraction, axial length measurement, and swept-source optical coherence tomography. Thickness changes in the choroid and retina were compared among children with or without myopic shift. Results: Eighty-eight children (74.6%) developed a myopic shift after 1 year, and their central foveal choroid was significantly attenuated (P < 0.01). No significant change was observed in choroids of children without myopic shift (P = 0.83). Choroidal thickness decreased in all subfields during myopic shift, whereas the thickness of the retinal layers increased or were unchanged in most subfields. Axial length increase and central foveal choroidal thinning were associated with myopic shift (R-2 = 0.157, P < 0.01), but axial length increase was not significantly related to choroidal thinning (P > 0.05). Conclusion: Choroidal thinning occurs early in myopic progression. Axial length increase and choroidal thinning are independently associated with myopic shift.
Compartmental analysis of three-dimensional choroidal vascularity and thickness of myopic eyes in young adults using SS-OCTA

Abstract:Purpose: We aimed to investigate the change of three-dimensional (3D) choroidal thickness (ChT), choroidal vessel volume (CVV), and choroidal vessel index (CVI) in young myopic adults using swept-source optical coherence tomography angiography (SS-OCTA) and compare the difference of these indicators in different quadrants of the macula and optic disc. Methods: A total of 248 eye samples from 135 participants were used in this cross-sectional study. Each participant underwent detailed history taking and ocular examinations. Based on axial length (AL), patients were divided into the emmetropia (EM) group, mild-myopia (MIM) group, moderate-myopia (MOM) group, and high-myopia (HM) group. 6 mm × 6 mm (1,024 × 1024 B-scans) SS-OCTA scans were performed centered on the fovea and optic disc. 3D ChT, CVV, and CVI were measured based on a built-in deep learning algorithm. Differences in ChT, CVV, and CVI were analyzed in different regions and different myopic groups. Results: Significant reduction in the global CVV were found in the HM group (1.930 ± 0.865) in comparison with the EM (3.486 ± 0.992), MIM (3.238 ± 1.033), and MOM (2.589 ± 1.083) groups ( p < 0.001). The global CVI was also lower in the HM group (0.258 ± 0.061) than in the EM (0.320 ± 0.055), MIM (0.320 ± 0.051), and MOM (0.286 ± 0.066) groups ( p < 0.001). The ChT was thinner in eyes with HM (242.753 ± 65.641) than in eyes with EM (377.532 ± 80.593), MIM (348.367 ± 78.191), or MOM (300.197 ± 87.175) ( p < 0.001). Compartmental analysis revealed that ChT, CVV, and CVI in the nasal quadrant of the macula and temporal and inferior quadrants of the optic disc were much lower than those in other quadrants ( p < 0.05). Correlation analyses found that ChT, CVV, and CVI were negatively correlated with AL and spherical equivalence. Conclusion: 3D ChT, CVV, and CVI gradually decreased as the degree of myopia increased. The changes were more dramatic on the nasal side of the macula and the temporal and inferior sides of the optic disc. These findings demonstrated the 3D choroidal change and highlighted the papillo-macular bundle as a sensitive region in myopic development.

Abstract:Purpose To determine the characteristics of longitudinal choroidal thickness (CT) and axial length (AL) changes in a group of Chinese young adults with various refractive errors. Methods In this 2 year prospective cohort study, a total of 291 (314 enrolled at baseline) Chinese medical freshmen aged 18 to 22 years (mean age, 18.7 ± 0.9 years) underwent eye examinations at baseline and follow-up visits, including cycloplegic refraction, ocular biometry measurements, and swept-source optical coherence tomography. Choroidal thickness measurements were performed at nine locations in the macular area. Results At baseline, the CT and AL was significant differences among groups of emmetropia, mild to moderate myopia and high myopia. During a two-year period, there were significant differences found in the changes of the subfoveal CT ( p < 0.001) and parafoveal CT of 7 locations between emmetropia and mild to moderate myopia, and the changes of the subfoveal CT ( p = 0.002) and parafoveal CT of 6 locations between emmetropia and high myopia. But there were no differences for AL and SE ( p > 0.05). The multivariable linear regression analysis showed that baseline subfoveal CT (per 1 μm) was a significant factor affecting the changes of subfoveal CT ( p < 0.001), whereas age, gender, and baseline AL were not significantly associated. Conclusion The longitudinal change in CT varies with refractive errors in Chinese young adults aged 18 to 22 years over a two-year period. The changes of subfoveal CT were significantly associated with the baseline subfoveal CT, but not associated with baseline AL.

Abstract:Aims To investigate the longitudinal changes of peripapillary retinal nerve fibre layer (RNFL) and choroidal thickness during myopic axial elongation. Methods Peripapillary RNFL and choroidal thickness were prospectively evaluated by spectral-domain optical coherence tomography (SD-OCT) in 46 eyes of 23 myopic children over the course of 4 years. Using serial OCT images acquired based on a fixed scan circle in the glaucoma progression analysis mode, general and sectoral RNFL thicknesses were acquired at the same position and the angular location of the peak was measured. The peripapillary choroidal thickness likewise was measured at eight positions in serial OCT images. Results The mean age at the baseline was 9.6±1.7 years. The mean axial length increased from 24.80±1.28 mm to 25.64±1.35 mm. The global peripapillary RNFL thickness was 98.54±12.06 μm at baseline. The global and sectoral RNFL thicknesses did not change during the 4 years. The angular location of RNFL peaks was also stable and was located in the superotemporal (64.18±10.85°) and inferotemporal (293.98±11.62°) sectors. The global peripapillary choroidal thickness was 145.40±28.67 μm at the baseline. The global and sectoral choroidal thicknesses did not change during the 4 years. Conclusions The peripapillary RNFL and choroidal thicknesses as well as the locations of the RNFL peaks had been preserved, during the 4-year follow-up on myopic children, when traced and measured from the same location.

Longitudinal Changes in Choroidal Thickness in Children with a History of Prematurity: An 18-month Prospective Cohort Study

Abstract:Purpose: To evaluate choroidal changes over time in school-aged children with a history of prematurity. Methods: A study of 416 eyes, including 88, 190, 36, 56, and 46 eyes in the full-term control, preterm, spontaneously regressed retinopathy of prematurity (ROP), intravitreal bevacizumab (IVB)-treated ROP, and laser-treated ROP groups, respectively. The choroidal thickness was measured 4 times at 6-month intervals by optical coherence tomography. Results: Of all the groups, the laser-treated children had the thinnest choroid compared with full-term children (−52.3 μm, P = .04). Preterm children exhibited greater attenuation in choroidal thickness over time than did full-term children (−6.3 ± 26.9 and −1.1 ± 12.8 μm/year, P = .03), whereas no difference was observed between IVB and laser treatments (−4.6 ± 18.9 and −2.0 ± 15.7 μm/year, P = .46). In all groups, the changes in axial length were negatively associated with the changes in choroidal thickness (all P < .05). Conclusion: A greater attenuation in choroid thickness over time was observed in preterm children than in full-term children, but this attenuation did not differ between IVB and laser treatments. Axial elongation was associated with choroidal thinning in school-aged children.

Abstract:Purpose This study evaluated the structural features of the retinal and choroidal regions and their correlations with ocular biometric and vascular parameters in Chinese children using optical coherence tomography angiography (OCTA). Methods A total of 159 children, 6–13 years of age, were included in this prospective study. The sample consisted of 55 emmetropes (spherical equivalent ≤ +0.75 and > −0.50 D), 53 low‐moderate myopes (≤ −0.50 to > −6.00 D) and 51 high myopes without pathological changes (≤ −6.00 D). Optical coherence biometry was used to measure axial length (AL) and anterior segment parameters. Swept‐source optical coherence tomography/OCTA was used to assess the macular structures and vascular characteristics in a 6 × 6 mm region centred on the macula. Results In a comprehensive analysis adjusting for age, sex, AL, macular blood perfusion, intraocular pressure and anterior segment parameters, retinal thickness (RT) showed a significant positive association with deep retinal vascular density and superficial retinal vascular density in the foveal area, but not with AL. Moreover, RT exhibited a significant negative association with AL in the parafoveal and perifoveal regions. Further, a significant positive correlation was observed between choroidal thickness and both choroidal vascular volume and choriocapillaris perfusion area, along with a negative correlation with AL across the entire macular region. Conclusions This study showed that the thickness of retina and choroid in Chinese children was not only associated with AL but also showed dynamic properties such as the blood perfusion of the retina and choroid, particularly in the foveal area.

Abstract:Aims/Purpose: Assessing axial eye growth is a key component in myopia management; however, access to biometry is limited in primary eye care settings. This study evaluated choroidal thickness, compared to spherical equivalent refraction, as a potential biomarker of axial elongation. Methods: The Myopia Outcome Study of Atropine in Children (MOSAIC) is a double‐blind, randomized controlled trial of 0.01% atropine eye drops. Irish myopic participants aged 6–16 years used either 0.01% atropine or placebo eye drops nightly for 2 years. Spherical equivalent refraction (SER) was measured by cycloplegic autorefraction, axial length (AL) by partial coherence interferometry and choroidal thickness (ChT) by Swept Source‐OCT. The respective 12‐month changes in both eyes were included in mixed linear regression analysis, while controlling for treatment group and within‐subject correlation. ROC analysis was conducted to assess the ability of ChT and/or SER to identify eyes in the top 25% (fast progressors) of AL change. Results: Of the 250 participants enrolled in MOSAIC (mean age 11.8 years, 62% female), 217 (87%) completed the 12‐month visit. Mean 12‐month change in AL, ChT and SER were 0.268 ± 0.19 mm, −3.62 ± 33.3 um and −0.437 ± 0.346 DS respectively. Analysis revealed a significant association between change in ChT and AL (−0.01 ± 0.002 mm per 10 μm; p < 0.001) and change in SER and AL (−0.28 ± 0.02 mm per 1D; p < 0.001). When both SER and ChT were combined in a single model, a 1D myopic change in SER and a 10um decrease in ChT were associated with a 0.27 mm ( p < 0.001) and 0.007 mm increase in AL ( p = 0.002), respectively. ROC analysis revealed that change in ChT was a stronger predictor of fast AL progression (AUC = 0.77) compared to SER change (AUC = 0.72), with the combined model performing better than either variable alone (AUC = 0.80). Conclusions: ChT change was significantly associated with AL change over 12 months and better than SER alone at identifying fast AL progressors, with a combined SER and ChT model achieving the best results.

Differences in Retinal and Choroidal Vasculature and Perfusion Related to Axial Length in Pediatric Anisomyopes

Abstract:The purpose of this study was to evaluate the interocular differences in choroidal vasculature, choriocapillaris perfusion, and retinal microvascular network, and to explore their associations with interocular asymmetry in axial lengths (ALs) in children with anisomyopia.Refractive error, AL, and other biometric parameters were measured in 70 children with anisomyopia. Using optical coherence tomography (OCT) and OCT-angiography, we measured the submacular choroidal thickness (ChT), total choroidal area (TCA), luminal area (LA), stromal area (SA), choroidal vascularity index (CVI), choriocapillaris flow deficit (CcFD), retinal vessel density (VD), and foveal avascular zone (FAZ) area.The mean interocular differences in spherical equivalent refraction and AL were -2.26 ± 0.94 diopters and 0.95 ± 0.46 mm, respectively. Submacular ChT, TCA, LA, SA, and CVI were all significantly lower in the more myopic (longer AL) eyes than in the less myopic (shorter AL) fellow eyes. In eyes with longer ALs, both the CcFD and FAZ areas were significantly greater, whereas the superficial and deep retinal VDs were significantly less. After adjusting for corneal power and intraocular pressure, interocular differences in LA (β = -0.774), SA (β = -0.991), and CcFD (β = 0.040) were significantly associated with interocular asymmetry in AL (all P < 0.05).In pediatric anisomyopes, eyes with longer ALs tended to have lower choroidal vascularity and choriocapillaris perfusion than the contralateral eyes with shorter ALs. Longitudinal investigations would be useful follow-ups to test for a causal role of choroidal circulation in human myopia.
Assessment of the effects of myopic and hyperopic anisometropia on choroidal vascular structure in children using SS‐OCTA

Abstract:Objective To compare large‐ and medium‐sized choroidal vascularity and the choriocapillaris (CC) flow area in children with different refractive errors using swept‐source optical coherence tomography angiography (SS‐OCTA). Methods Forty‐two anisometropic children were enrolled and divided into hyperopic anisometropia (HA) and myopic anisometropia (MA) groups. SS‐OCTA was performed to analyse choroidal vascularity. Mean choroidal thickness (CT), choroidal vascularity volume (CVV), choroidal vascularity index (CVI) and CC flow area were compared between the two eyes. The inter‐ocular differences between the two groups were also determined. Results Mean CT and CVV were highest in eyes with shorter axial lengths in both refractive groups, and the difference between the two eyes was positively correlated with the difference in axial length at the foveal region. Significant differences in the CVI in the MA group were only found in the parafoveal region. Inter‐ocular differences in the CC were significantly reduced in eyes with longer axial lengths in the foveal and parafoveal regions of the HA and MA groups, respectively. Comparing inter‐ocular differences, CC was significantly greater in the parafoveal region of the MA group than the HA group. Conclusions All layers of choroidal vasculature were thinner in eyes with longer axial lengths in all groups. The inter‐ocular CC difference was greater in the MA than in the HA group, with similar differences in axial length. This suggests that both medium‐to‐large choroidal vascular and choroidal capillaries may play a role in myopia development.

Abstract:Purpose:To investigate topographical maps of the three-dimensional choroidal vascularity index (3D-CVI) in children with different levels of myopia. Methods:We enrolled 274 eyes from 143 children with various severity of myopia, including emmetropia (EM), low myopia (LM), and moderate-high myopia (MHM). The choroidal vessel volume (CVV), choroidal stroma volume (CSV), and 3D-CVI in different eccentricities (fovea, parafovea, and perifovea) and quadrants (nasal, temporal, superior, and inferior) were obtained from swept-source optical coherence tomography angiography (SS-OCTA) volume scans. All choroidal parameters were compared among groups, and the associated factors contributing to different 3D-CVIs were analyzed. Results:Compared to the less myopic group, the more myopic group showed a significant decrease in CVV and CSV (MHM < LM < EM) and a significant increase in the 3D-CVI (MHM > LM > EM) in most areas (all P < 0.05). The nasal quadrant had the greatest 3D-CVI and lowest CSV and CVV, and vice versa in the temporal quadrant. The 3D-CVIs of the EM and LM groups gradually increased from the fovea to the perifovea, whereas the 3D-CVI of the MHM group first decreased and then increased. Regression analysis showed that axial length was an independent risk factor affecting foveal and parafoveal 3D-CVIs. Restricted cubic spline analysis revealed that the 3D-CVI increased with spherical equivalent (SE) when the SE was less than threshold and decreased when the SE was greater than threshold (SE thresholds for foveal, parafoveal, and perifoveal 3D-CVIs were -5.25 D, -5.125 D, and -2.00 D, respectively; all P < 0.05). Conclusions:Children with myopia exhibited decreased CSV and CVV, increased 3D-CVIs, and altered 3D-CVI eccentricity characteristics (from the fovea to the perifovea). The quadratic relationship between the 3D-CVI and SE should be explored in longitudinal investigations.
Choroidal Morphologic and Vascular Features in Patients With Myopic Choroidal Neovascularization and Different Levels of Myopia Based on Image Binarization of Optical Coherence Tomography

Abstract:Purpose: To characterize the choroidal morphologic and vascular features in different levels of myopes and patients with myopic choroidal neovascularization (mCNV).Methods: A total of 148 subjects were enrolled in this cross-sectional study, including 78 low-to-moderate myopes (LMM), 53 high myopes (HM), and 17 high myopic patients with mCNV. Ocular biometrics were measured using an optical low-coherence reflectometry device. Retinal and choroidal imaging was performed using enhanced depth imaging (EDI) spectral domain optical coherence tomography (OCT). Retinal parameters including retinal thickness and retinal volume were obtained from a built-in software. Binarization technique was adopted to investigate choroidal parameters including choroidal thickness (CT), vascular area, stromal area, and choroidal vascularity index (CVI). Choroidal parameters were measured at five locations to cover as much area of choroid as possible, and their patterns of distribution were further analyzed.Results: Patients with mCNV had an atrophic retina of comparable thickness to HM (273.65 +/- 17.28 vs. 276.49 +/- 13.29 mu m, p = 0.47), but the choroid was thinner than that of HM (153.94 +/- 15.12 vs. 236.09 +/- 38.51 mu m, p < 0.001). Subfoveal CVI was greatest in the mCNV eyes (0.651 +/- 0.009), followed by HM (0.645 +/- 0.012) and LMM eyes (0.636 +/- 0.012). Similar to CT, CVI was also found significantly different among these three groups at all five locations (p for trend < 0.001 for all locations). Axial length (AL) was negatively correlated with retinal volume (r = -0.236, p = 0.009), which is the only significant finding in associations between ocular factors and retinal parameters. Strong, negative correlations were identified between AL and subfoveal choroidal thickness (SFCT, r = -0.820, p < 0.001). However, AL was positively correlated with subfoveal CVI (r = 0.668, p < 0.001). CVI was greater in myopic eyes with thinner choroid (r = -0.578, p < 0.001). BCVA exhibited no significant association with CVI (r = 0.139, p = 0.092), but was negatively correlated with SFCT (r = -0.386, p < 0.001) and positively correlated with AL (r = 0.351, p < 0.001).Conclusion: Choroid in patients with mCNV was thinner yet more vascularized than that in HM and LMM subjects. CVI increased with a longer AL which was associated with a smaller SFCT, choroidal vascular area (VA), and total choroidal area (TCA). Better BCVA was achieved in subjects with thicker SFCT and shorter AL.
Choroidal vascular changes in early-stage myopic maculopathy from deep learning choroidal analysis: a hospital-based SS-OCT study

Abstract:Background: The objective of this study is to illustrate the changes in the choroidal vasculature in individuals with diffuse chorioretinal atrophy (DCA, early-stage myopic maculopathy) and investigate the association between them. Methods: This study included 1418 highly myopic eyes from 720 participants aged 18 - 60 years from the Wenzhou High Myopia Cohort Study. These participants underwent comprehensive ophthalmic assessments. Myopic maculopathy classification followed the Meta-PM system, with pathological myopia defined as myopic maculopathy of DCA or severer. Eyes with myopic maculopathy categorized as no macular lesions (C0), tessellated fundus (C1), and DCA (C2) were enrolled in the analysis. Choroidal images were obtained from swept-source optical coherence tomography (SS-OCT), and the images were processed with a deep learning-based automatic segmentation algorithm and the Niblack auto-local threshold algorithm. Results: DCA was detected in 247 eyes (17.4%). In comparison to eyes with C0, those with C2 exhibited significant reductions in choroidal thickness (ChT), luminal area (LA), and stromal area (SA) across all evaluated regions (all P < 0.001). An increase in choroidal vascular index (CVI) was observed in all regions, except for the nasal perifoveal (N2) and inferior perifoveal (I2) regions (all P < 0.01). Multivariable logistic regression analysis revealed a negative association between the presence of DCA and increases in choroidal LA and SA (odds ratio ≤ 0.099, P < 0.001). Multivariable linear regression analysis showed that the mean deviation of the visual field test was positively associated with LA and SA at the vertical meridian (B = 1.512, P < 0.001 for LA; B = 1.956, P < 0.001 for SA). Furthermore, the receiver operating characteristic curve analyses showed the optimal ChT to diagnose pathological myopia was 82.4 µm in the N2 region, the LA was 0.076 mm2 and the SA was 0.049 mm2, with area under the curves of 0.916, 0.908, and 0.895, respectively. Conclusions: The results of this study indicated that both the presence of DCA and visual function impairment were associated with reductions in choroidal perfusion and stromal components. Moreover, we established threshold values for choroidal parameters in diagnosing pathological myopia, offering valuable references for clinical diagnosis and management.
