Abstract:Tuberculosis (TB) is a major health issue with high mortality rates worldwide. Recently, tremendous researches of artificial intelligence (AI) have been conducted targeting at TB to reduce the diagnostic burden. However, most researches are conducted in the developed urban areas. The feasibility of applying AI in low-resource settings remains unexplored. In this study, we apply an automated detection (AI) system to screen a large population in an underdeveloped area and evaluate feasibility and contribution of applying AI to help local radiologists detect and diagnose TB using chest X-ray (CXR) images. First, we divide image data into one training dataset including 2627 TB-positive cases and 7375 TB-negative cases and one testing dataset containing 276 TB-positive cases and 619 TB-negative cases, respectively. Next, in building AI system, the experiment includes image labeling and preprocessing, model training and testing. A segmentation model named TB-UNet is also built to detect diseased regions, which uses ResNeXt as the encoder of U-Net. We use AI-generated confidence score to predict the likelihood of each testing case being TB-positive. Then, we conduct two experiments to compare results between the AI system and radiologists with and without AI assistance. Study results show that AI system yields TB detection accuracy of 85%, which is much higher than detection accuracy of radiologists (62%) without AI assistance. In addition, with AI assistance, the TB diagnostic sensitivity of local radiologists is improved by 11.8%. Therefore, this study demonstrates that AI has great potential to help detection, prevention, and control of TB in low-resource settings, particularly in areas with more scant doctors and higher rates of the infected population.

Abstract:Tuberculosis (TB) continues to be a leading cause of death in children despite global efforts focused on early diagnosis and interventions to limit the spread of the disease. This challenge has been made more complex in the context of the coronavirus pandemic, which has disrupted the "End TB Strategy" and framework set out by the World Health Organization (WHO). Since the inception of artificial intelligence (AI) more than 60 years ago, the interest in AI has risen and more recently we have seen the emergence of multiple real-world applications, many of which relate to medical imaging. Nonetheless, real-world AI applications and clinical studies are limited in the niche area of paediatric imaging. This review article will focus on how AI, or more specifically deep learning, can be applied to TB diagnosis and management in children. We describe how deep learning can be utilised in chest imaging to provide computer-assisted diagnosis to augment workflow and screening efforts. We also review examples of recent AI applications for TB screening in resource constrained environments and we explore some of the challenges and the future directions of AI in paediatric TB.

Abstract:With the increasing incidence and mortality of pulmonary tuberculosis, in addition to tough and controversial disease management, time-wasting and resource-limited conventional approaches to the diagnosis and differential diagnosis of tuberculosis are still awkward issues, especially in countries with high tuberculosis burden and backwardness. In the meantime, the climbing proportion of drug-resistant tuberculosis poses a significant hazard to public health. Thus, auxiliary diagnostic tools with higher efficiency and accuracy are urgently required. Artificial intelligence (AI), which is not new but has recently grown in popularity, provides researchers with opportunities and technical underpinnings to develop novel, precise, rapid, and automated implements for pulmonary tuberculosis care, including but not limited to tuberculosis detection. In this review, we aimed to introduce representative AI methods, focusing on deep learning and radiomics, followed by definite descriptions of the state-of-the-art AI models developed using medical images and genetic data to detect pulmonary tuberculosis, distinguish the infection from other pulmonary diseases, and identify drug resistance of tuberculosis, with the purpose of assisting physicians in deciding the appropriate therapeutic schedule in the early stage of the disease. We also enumerated the challenges in maximizing the impact of AI in this field such as generalization and clinical utility of the deep learning models.

Artificial intelligence-based computer aided detection (AI-CAD) in the fight against tuberculosis: Effects of moving health technologies in global health

Abstract:Computer Aided Detection software based on Artificial Intelligence (AI-CAD), combined with chest X-rays have recently been promoted as an easy fix for a complex problem: ending TB by 2030. WHO has recommended the use of such imaging devices in 2021 and many partnerships have helped propose benchmark analysis and technology comparisons to facilitate their "market access". Our aim is to examine the socio-political and health issues that stem from using AI-CAD technology in a global health context conceptualized as a set of practice and ideas organizing global intervention "in the life of others". We also question how this technology, which is not yet fully implemented in routine use, may limit or amplify some inequalities in the care of tuberculosis. We describe AI-CAD through Actor-Network-Theory framework to understand the global assemblage and composite activities associated with detection through AI-CAD, and interrogate how the technology itself may consolidate a specific configuration of "global health". We explore the various dimensions of AI-CAD "health effects model": technology design, development, regulation, institutional competition, social interaction and health cultures. On a broader level, AI-CAD represents a new version of global health's accelerationist model centered on "moving and autonomous-presumed technologies". We finally present key aspects in our research which help discuss the theories mobilized: AI-CAD ambivalent insertion in global health, the social lives of its data: from efficacy to markets and AI-CAD human care and maintenance it requires. We reflect on the conditions that will affect AI-CAD use and its promises. In the end, the risk of new detection technologies such as AI-CAD is indeed that the fight against TB could be reduced to one that is purely technical and technological, with neglect to its social determinants and effects.

Abstract:Tuberculosis (TB) remains a significant global health concern, particularly with the emergence of multidrug-resistant tuberculosis (MDR-TB) and extensively drug-resistant tuberculosis (XDR-TB). Traditional methods for diagnosing drug resistance in TB are time-consuming and often lack accuracy, leading to delays in appropriate treatment initiation and exacerbating the spread of drug-resistant strains. In recent years, artificial intelligence (AI) techniques have shown promise in revolutionizing TB diagnosis, offering rapid and accurate identification of drug-resistant strains. This comprehensive review explores the latest advancements in AI applications for the diagnosis of MDR-TB and XDR-TB. We discuss the various AI algorithms and methodologies employed, including machine learning, deep learning, and ensemble techniques, and their comparative performances in TB diagnosis. Furthermore, we examine the integration of AI with novel diagnostic modalities such as whole-genome sequencing, molecular assays, and radiological imaging, enhancing the accuracy and efficiency of TB diagnosis. Challenges and limitations surrounding the implementation of AI in TB diagnosis, such as data availability, algorithm interpretability, and regulatory considerations, are also addressed. Finally, we highlight future directions and opportunities for the integration of AI into routine clinical practice for combating drug-resistant TB, ultimately contributing to improved patient outcomes and enhanced global TB control efforts.

Abstract:As we enter the era of digital interdependence, artificial intelligence (AI) emerges as a key instrument to transform health care and address disparities and barriers in access to services. This viewpoint explores AI's potential to reduce inequalities in cancer care by improving diagnostic accuracy, optimizing resource allocation, and expanding access to medical care, especially in underserved communities. Despite persistent barriers, such as socioeconomic and geographical disparities, AI can significantly improve health care delivery. Key applications include AI-driven health equity monitoring, predictive analytics, mental health support, and personalized medicine. This viewpoint highlights the need for inclusive development practices and ethical considerations to ensure diverse data representation and equitable access. Emphasizing the role of AI in cancer care, especially in low- and middle-income countries, we underscore the importance of collaborative and multidisciplinary efforts to integrate AI effectively and ethically into health systems. This call to action highlights the need for further research on user experiences and the unique social, cultural, and political barriers to AI implementation in cancer care.
Integrating Artificial Intelligence in the Diagnosis of COPD Globally: A Way Forward

Abstract:The advancement of artificial intelligence (AI) capabilities has paved the way for a new frontier in medicine, which has the capability to reduce the burden of COPD globally. AI may reduce health care-associated expenses while potentially increasing diagnostic specificity, improving access to early COPD diagnosis, and monitoring COPD progression and subsequent disease management. We evaluated how AI can be integrated into COPD diagnosing globally and leveraged in resource-constrained settings.AI has been explored in diagnosing and phenotyping COPD through auscultation, pulmonary function testing, and imaging. Clinician collaboration with AI has increased the performance of COPD diagnosing and highlights the important role of clinical decision-making in AI integration. Likewise, AI analysis of computer tomography (CT) imaging in large population-based cohorts has increased diagnostic ability, severity classification, and prediction of outcomes related to COPD. Moreover, a multimodality approach with CT imaging, demographic data, and spirometry has been shown to improve machine learning predictions of the progression to COPD compared to each modality alone. Prior research has primarily been conducted in high-income country settings, which may lack generalization to a global population. AI is a World Health Organization priority with the potential to reduce health care barriers in low- and middle-income countries. We recommend a collaboration between clinicians and an AI-supported multimodal approach to COPD diagnosis as a step towards achieving this goal. We believe the interplay of CT imaging, spirometry, biomarkers, and sputum analysis may provide unique insights across settings that could provide a basis for clinical decision-making that includes early intervention for those diagnosed with COPD.

Abstract:For decades, tuberculosis (TB) is an unavoidable lung disease and epidemic for several developing nations. It proceeds to be the main cause of demises worldwide. It is because of poor access to medical diagnosis, where tuberculosis disease is common. Further for this medical diagnosis problem, chest X-ray (CXR) is recognized to be a convenient, cost-effective, and primary tuberculosis diagnosis tool. However, reading each CXR manually for TB localization is a hectic task for the radiologist where TB disease is common. To overcome this limitation, in this paper we have discussed the iDoc-X model, which is a seamlessly integrated software of iDoc.ai (an initiative of Teleglobal Consulting LTD). iDoc-X diagnoses the TB disease using the AI model and gives the prioritized list to a medical practitioner. In addition to this, we have also performed and discussed the accuracy test of the iDoc-X model. This will overcome the restrictions of the TB diagnosis workflow and provide better assistance to the medical practitioner, where the TB disease is common.
AI Clinics on Mobile (AICOM): Universal AI Doctors for the Underserved and Hard-to-Reach

Abstract:This paper introduces Artificial Intelligence Clinics on Mobile (AICOM), an open-source project devoted to answering the United Nations Sustainable Development Goal 3 (SDG3) on health, which represents a universal recognition that health is fundamental to human capital and social and economic development. The core motivation for the AICOM project is the fact that over 80% of the people in the least developed countries (LDCs) own a mobile phone, even though less than 40% of these people have internet access. Hence, through enabling AI-based disease diagnostics and screening capability on affordable mobile phones without connectivity will be a critical first step to addressing healthcare access problems. The technologies developed in the AICOM project achieve exactly this goal, and we have demonstrated the effectiveness of AICOM on monkeypox screening tasks. We plan to continue expanding and open-sourcing the AICOM platform, aiming for it to evolve into an universal AI doctor for the Underserved and Hard-to-Reach.

Abstract:Mycobacterium tuberculosis is a bacterium that causes disease known as Tuberculosis. Tuberculosis is highly contagious and can result in high mortality rate if left untreated. In order to screen individuals suspected of the disease, medical expert relies on several conventional approaches which are hindered by several limitations which include time consuming, high workload, false positive results, etc. This calls for the need to develop smart and automatic approaches that can address these challenges. Majority of existing studies reported the use of 1 or 2 pretrained models and the use of SoftMax as the classifier. Moreover, majority of the studies trained models using a single type of dataset which are mostly curated from public accessible domains. Thus, this study addressed these challenges by: (1) The use of several pretrained models (2) The use of 2 classifiers which include SVM and KNN and (3) Training and validating pretrained models fused with classifiers on microscopic slide and chest X-ray images. The result achieved in this study highlights the prospect of computer-assisted techniques in triaging and screening of TB. The integration of Internet of Everything (IoE) in medical diagnosis has the potential to increase healthcare outcome, boost productivity, and reduce workload.

Democratizing Infectious Disease Modeling: An AI Assistant for Generating, Simulating, and Analyzing Dynamic Models

Abstract:Understanding and forecasting infectious disease spread is pivotal for effective public health management. Traditional dynamic disease modeling is an essential tool for characterization and prediction, but often requires extensive expertise and specialized software, which may not be readily available in low-resource environments. To address these challenges, we introduce an AI-powered modeling assistant that utilizes advanced capabilities from OpenAI's latest models and functionality. This tool enhances the accessibility and usability of infectious disease models and simulation frameworks by allowing users to generate or modify model configurations through intuitive natural language inputs or by importing explicit model descriptions. Our prototype integrates with an established open-source disease simulation framework called the Compartmental Modeling Software (CMS) to provide a seamless modeling experience from setup to analysis. The AI assistant efficiently interprets disease model parameters, constructs accurate model files, executes simulations in a controlled environment, and assists in result interpretation using advanced analytics tools. It encapsulates expert knowledge and adheres to best practices to support users ranging from novices to expert modelers. Furthermore, we discuss the limitations of this AI assistant, particularly its performance in complex scenarios where it might generate inaccurate specifications. By enhancing the ease of disease modeling and supporting ongoing capacity-building initiatives, we believe that AI assistants like this one could significantly contribute to global health efforts by empowering researchers, especially in regions with limited resources, to develop and refine their disease models independently. This innovative approach has the potential to democratize disease modeling in global health, offering a scalable solution that adapts to diverse needs across a wide-range of geographies, languages, and populations.

Development and application of survey-based artificial intelligence for clinical decision support in managing infectious diseases: A pilot study on a hospital in central Vietnam

Abstract:Introduction: In this study, we developed a simplified artificial intelligence to support the clinical decision-making of medical personnel in a resource-limited setting. Methods: We selected seven infectious disease categories that impose a heavy disease burden in the central Vietnam region: mosquito-borne disease, acute gastroenteritis, respiratory tract infection, pulmonary tuberculosis, sepsis, primary nervous system infection, and viral hepatitis. We developed a set of questionnaires to collect information on the current symptoms and history of patients suspected to have infectious diseases. We used data collected from 1,129 patients to develop and test a diagnostic model. We used XGBoost, LightGBM, and CatBoost algorithms to create artificial intelligence for clinical decision support. We used a 4-fold cross-validation method to validate the artificial intelligence model. After 4-fold cross-validation, we tested artificial intelligence models on a separate test dataset and estimated diagnostic accuracy for each model. Results: We recruited 1,129 patients for final analyses. Artificial intelligence developed by the CatBoost algorithm showed the best performance, with 87.61% accuracy and an F1-score of 87.71. The F1-score of the CatBoost model by disease entity ranged from 0.80 to 0.97. Diagnostic accuracy was the lowest for sepsis and the highest for central nervous system infection. Conclusion: Simplified artificial intelligence could be helpful in clinical decision support in settings with limited resources.

Bridging the Skills Gap: Evaluating an AI-Assisted Provider Platform to Support Care Providers with Empathetic Delivery of Protocolized Therapy

Abstract:Despite the high prevalence and burden of mental health conditions, there is a global shortage of mental health providers. Artificial Intelligence (AI) methods have been proposed as a way to address this shortage, by supporting providers with less extensive training as they deliver care. To this end, we developed the AI-Assisted Provider Platform (A2P2), a text-based virtual therapy interface that includes a response suggestion feature, which supports providers in delivering protocolized therapies empathetically. We studied providers with and without expertise in mental health treatment delivering a therapy session using the platform with (intervention) and without (control) AI-assistance features. Upon evaluation, the AI-assisted system significantly decreased response times by 29.34% (p=0.002), tripled empathic response accuracy (p=0.0001), and increased goal recommendation accuracy by 66.67% (p=0.001) across both user groups compared to the control. Both groups rated the system as having excellent usability.

Abstract:Abstract With the rapid advancement of artificial intelligence (AI), the field of infectious diseases (ID) faces both innovation and disruption. AI and its subfields including machine learning, deep learning and large language models can support ID clinician’s decision making and streamline their workflow. AI models may help ensure earlier detection of disease, more personalized empiric treatment recommendations, and allocation of human resources to support higher yield antimicrobial stewardship and infection prevention strategies. AI is unlikely to replace the role of ID experts, but instead augment it. However its limitations will need to be carefully addressed and mitigated to ensure safe and effective implementation. ID experts can be engaged in AI implementation by participating in training and education, identifying use cases for AI to help improve care for their patients, designing, validating and evaluating algorithms, and continuing to advocate for their vital role in patient care.

Abstract:Tuberculosis (TB) is a chronic infectious disease worldwide and remains a major cause of death globally. Of the estimated 9 million people who developed TB in 2013, over 80% were in South-East Asia, Western Pacific, and African. The majority of the infected populations was from resource-poor and marginalized communities with weak healthcare infrastructure. Reducing TB diagnosis delay is critical in mitigating disease transmission and minimizing the reproductive rate of the tuberculosis epidemic. The combination of machine learning and mobile computing techniques offers a unique opportunity to accelerate the TB diagnosis among these communities. The ultimate goal of our research is to reduce patient wait times for being diagnosed with this infectious disease by developing new machine learning and mobile health techniques to the TB diagnosis problem. In this paper, we first introduce major technique barriers and proposed system architecture. Then we report two major progresses we recently made. The first activity aims to develop large-scale, real-world and well-annotated X-ray image database dedicated for automated TB screening. The second research activity focus on developing effective and efficient computational models (in particularly, deep convolutional neural networks (CNN)-based models) to classify the image into different category of TB manifestations. Experimental results have demonstrated the effectiveness of our approach. Our future work includes: (1) to further improve the performance of the algorithms, and (2) to deploy our system in the city of Carabayllo in Perú, a densely occupied urban community and high-burden TB.
A Methodology for a Scalable, Collaborative, and Resource-Efficient Platform to Facilitate Healthcare AI Research

Abstract:Healthcare AI holds the potential to increase patient safety, augment efficiency and improve patient outcomes, yet research is often limited by data access, cohort curation, and tooling for analysis. Collection and translation of electronic health record data, live data, and real-time high resolution device data can be challenging and time-consuming. The development of real-world AI tools requires overcoming challenges in data acquisition, scarce hospital resources and high needs for data governance. These bottlenecks may result in resource-heavy needs and long delays in research and development of AI systems. We present a system and methodology to accelerate data acquisition, dataset development and analysis, and AI model development. We created an interactive platform that relies on a scalable microservice backend. This system can ingest 15,000 patient records per hour, where each record represents thousands of multimodal measurements, text notes, and high resolution data. Collectively, these records can approach a terabyte of data. The system can further perform cohort generation and preliminary dataset analysis in 2-5 minutes. As a result, multiple users can collaborate simultaneously to iterate on datasets and models in real time. We anticipate that this approach will drive real-world AI model development, and, in the long run, meaningfully improve healthcare delivery.

Abstract:Computer-aided detection algorithms based on artificial intelligence are increasingly being tested and used as a means for detecting tuberculosis in countries where the epidemic is still present. Computer-aided detection tools are often presented as a global solution that can be deployed in all the geographical areas concerned by tuberculosis, but at the same time, they need to be adjusted and calibrated according to local populations’ characteristics. The aim of this article is to analyze the tensions between the standardization of computer-aided detection algorithms and their local adaptation and the political issues associated with these tensions. We undertook a qualitative analysis of practices associated with tuberculosis detection algorithms in different contexts, contrasting the perspectives of various stakeholders. Algorithms embed the promise of standardization through automation and the bypassing of variable human expertise such as that of radiologists, they are nonetheless objects of local practices that we have characterized as “tweaking.” This work of tweaking reveals how the technology is situated but also the many concerns of the users and workers (insertion in care, control over infrastructure, and political ownership). This should be better considered to truly make computer-aided detection innovative tools for tuberculosis management in global health.

Abstract:Artificial intelligence (AI) technology has been increasingly used in the implementation of advanced Clinical Decision Support Systems (CDSS). Research demonstrated the potential usefulness of AI-powered CDSS (AI-CDSS) in clinical decision making scenarios. However, post-adoption user perception and experience remain understudied, especially in developing countries. Through observations and interviews with 22 clinicians from 6 rural clinics in China, this paper reports the various tensions between the design of an AI-CDSS system (“Brilliant Doctor”) and the rural clinical context, such as the misalignment with local context and workflow, the technical limitations and usability barriers, as well as issues related to transparency and trustworthiness of AI-CDSS. Despite these tensions, all participants expressed positive attitudes toward the future of AI-CDSS, especially acting as “a doctor’s AI assistant” to realize a Human-AI Collaboration future in clinical settings. Finally we draw on our findings to discuss implications for designing AI-CDSS interventions for rural clinical contexts in developing countries.
