Abstract:In this paper, we are proposing a blockchain-based architectural model to ensure the integrity of healthcare-sensitive data in an AI-based medical research context. In our approach, we will use the HL7 FHIR standardized data structure to ensure the interoperability of our approach with the existing hospital information systems (HIS). Indeed, structuring the data coming from several heterogeneous sources would enhance its quality. In addition, a standardised data structure would help establish a more accurate security and data protection model throughout the process of data collection cleansing and processing. Hence, we designed our architecture to be interoperable with all FHIR-based HISs to add a trust layer to the current medical research process. In this paper we are to achieve our goal, we will combine continua healthcare IoT architecture and Hyperledger fabric architecture. Our trust layer model is composed of four components: (1) an architecture that integrates with the HL7 FHIR data exchange framework, which extends an open protocol that supports efficient standards-based healthcare data exchange (2) a blockchain layer to support access control and auditing of FHIR health records that are stored in the data exchange network databases; (3) a distributed architecture consisting of multiple trusted nodes ensure privacy protection for health data; and (4) an application programming interface (API) will be available for use by the network.

Abstract:The healthcare sector deals with extremely sensitive information that must be administered in a safe and confidential way. The objective of the proposed framework is to utilize Blockchain Technology (BT) for tracking medical prescriptions and the implementation is carried out using the Hyperledger Fabric platform, an enterprise-grade open-source distributed ledger technology platform designed for Bigdata applications. Multiple entities, including patients, e-pharmacies, pharmacies, doctors and hospitals can establish connections by introducing several nodes in the Fabric chain. A web-centered application is provided for doctors, connecting them with participating pharmacies, hospitals and e-pharmacies through which, they can share patient prescription. Pharmacies and e-pharmacies have access to this data and can notify patients about the availability of prescribed medicines. Additionally, reminders for refills, such as heart medication, can be sent for patients requiring long-term medication. Patients can also try with nearby pharmacies and the availability of their prescribed medicines. The inclusion of a wallet feature in the application enables patients to use mobile tokens for making purchases. Patient data is treated with the utmost confidentiality, kept private, and accessed only upon request and with the consent of the concerned parties. This privacy is ensured through the use of zero-knowledge proof. Patients retain access to their complete medical history, facilitating interactions with doctors without the need for repetitive information sharing.

SCALHEALTH: Scalable Blockchain Integration for Secure IoT Healthcare Systems

Abstract:Internet of Things (IoT) devices are capable of allowing for far-reaching access to and evaluation of patient data to monitor health and diagnose from a distance. An electronic healthcare system that checks patient data, prepares medicines and provides financial assistance is necessary. Providing safe data transmission, monitoring, decentralization, preserving patient privacy, and maintaining confidentiality are essential to an electronic healthcare system. In this study, we introduce (SCALHEALTH) which is a blockchain-based scheme of the Hyperledger Fabric consortium. In this study, we use authentication to agree on a common key for data encryption to send data confidentially. Also, sending data through IPFS is decentralized. Non-fungible token (NFT) is used to send patient prescriptions to pharmacies and insurance companies to ensure the authenticity of patient prescriptions. As the system's main body, blockchain creates authorization and validation for all devices and institutions. Also, all metadata in the system is recorded on the blockchain to maintain integrity, transparency, and timely data monitoring. The proposed study uses two types of blockchain: a health blockchain and a financial blockchain. The financial blockchain is for financial transactions and is based on Ethereum. The health blockchain also introduces a mechanism that allows several blockchains to be active in parallel, instead of only one blockchain. The prototype of this mechanism is simulated in two scenarios. In comparison to the normal state, the proposed plan has superior results.

Abstract:Data sharing in the health sector represents a big problem due to privacy and security issues. Health data have tremendous value for organisations and criminals. The European Commission has classified health data as a unique resource owing to their ability to enable both retrospective and prospective research at a low cost. Similarly, the Organisation for Economic Co-operation and Development (OECD) encourages member nations to create and implement health data governance systems that protect individual privacy while allowing data sharing. This paper proposes adopting a blockchain framework to enable the transparent sharing of medical information among health entities in a secure environment. We develop a laboratory-based prototype using a design science research methodology (DSRM). This approach has its roots in the sciences of engineering and artificial intelligence, and its primary goal is to create relevant artefacts that add value to the fields in which they are used. We adopt a patient-centric approach, according to which a patient is the owner of their data and may allow hospitals and health professionals access to their data.

Abstract:The Hyperledger Fabric (HF) framework is widely studied for securing electronic health records (EHRs) in the healthcare sector. Despite the various cross-domain blockchain technology (BCT) applications, little is known about the role of the HF framework in healthcare. The purpose of the systematic literature review (SLR) is to review the existing literature on the HF framework and its applications in healthcare. This SLR includes literature published between January 2015 and March 2023 in the ACM digital library, IEEE Xplore, SCOPUS, Springer, PubMed, and Google Scholar databases. Following the inclusion and exclusion criteria, a total of 57 articles emerged as eligible for this SLR. The HF framework was found to be useful in securing health records coming from the Internet of Medical Things (IoMT) and many other devices. The main causes behind using the HF framework were identified as privacy and security, integrity, traceability, and availability of health records. Additionally, storage issues with transactional data over the blockchain are reduced by the use of the HF framework. This SLR also highlights potential future research trends to ensure the high-level security of health records.

Abstract:The management and exchange of electronic health records (EHRs) remain critical challenges in healthcare, with fragmented systems, varied standards, and security concerns hindering seamless interoperability. These challenges compromise patient care and operational efficiency. This paper proposes a novel solution to address these issues by leveraging distributed ledger technology (DLT), including blockchain, to enhance data security, integrity, and transparency in healthcare systems. The decentralized and immutable nature of DLT enables more efficient and secure information exchange across platforms, improving decision-making and coordination of care. This paper outlines a strategic implementation approach, detailing timelines, resource requirements, and stakeholder involvement while addressing crucial privacy and security concerns like encryption and access control. In addition, it explores standards and protocols necessary for achieving interoperability, offering case studies that demonstrate the framework's effectiveness. This work contributes by introducing a DLT-based solution to the persistent issue of EHR interoperability, providing a novel pathway to secure and efficient health data exchanges. It also identifies the standards and protocols essential for integrating DLT with existing health information systems, thereby facilitating a smoother transition toward enhanced interoperability.

Abstract:Electronic health record (EHR) management systems require the adoption of effective technologies when health information is being exchanged. Current management approaches often face risks that may expose medical record storage solutions to common security attack vectors. However, healthcare-oriented blockchain solutions can provide a decentralized, anonymous and secure EHR handling approach. This paper presents PREHEALTH, a privacy-preserving EHR management solution that uses distributed ledger technology and an Identity Mixer (Idemix). The paper describes a proof-of-concept implementation that uses the Hyperledger Fabric's permissioned blockchain framework. The proposed solution is able to store patient records effectively whilst providing anonymity and unlinkability. Experimental performance evaluation results demonstrate the scheme's efficiency and feasibility for real-world scale deployment.

Abstract:Objectives: Electronic Health Record (EHR) systems are increasingly used as an effective method to share patients' records among different hospitals. However, it is still a challenge to access scattered patient data through multiple EHRs. Our goal is to build a system to access patient records easily among EHRs without relying on a centralized supervisory system. Methods: We apply consortium blockchain to compose a distributed system using Hyperledger Fabric incorporating existent EHRs. Peer nodes hold the same ledger on which the address of a patient record in an EHR is written. Individual patients are identified by unique certificates issued by a local certificate authorities that collaborate with each other in a channel of the network. To protect a patient's privacy, we use a proxy re-encryption scheme when the data are transferred. We designed and implemented various chaincodes to handle business logic agreed by member organizations of the network. Results: We developed a prototype system to implement our concept and tested its performance including chaincode logic. The results demonstrated that our system can be used by doctors to find patient's records and verify patient's consent on access to the data. Patients also can seamlessly receive their past records from other hospitals. The access log is stored transparently and immutably in the ledger that is used for auditing purpose. Conclusions: Our system is feasible and flexible with scalability and availability in adapting to existing EHRs for strengthening security and privacy in managing patient records. Our research is expected to provide an effective method to integrate dispersed patient records among medical institutions.

Abstract:Medical data involves a large amount of personal information and is highly privacy sensitive. In the age of big data, the increasing informatization of healthcare makes it vital that medical information is stored securely and accurately. However, current medical information is subject to the risk of privacy leakage and difficult to share. To address these issues, this paper proposes a healthcare information security storage solution based on Hyperledger Fabric and the Attribute-Based Access Control (ABAC) framework. The scheme first utilizes attribute-based access control, which allows dynamic and fine-grained access to medical information, and then stores the medical information in the blockchain, which can be secured and tamper-proof by formulating corresponding smart contracts. In addition, this solution also incorporates IPFS technology to relieve the storage pressure of the blockchain. Experiments show that the proposed scheme combining access control of attributes and blockchain technology in this paper can not only ensure the secure storage and integrity of medical information but also has a high throughput when accessing medical information.

Abstract:Medical data are important in diagnosis, treatment, recovery, and medical accident investigation. The integrity and availability of medical data are the basic guarantee for the smooth operation of these activities. The privacy of medical data is a natural demand from the sensitivity of medical data. At present, there are mainly two ways to protect the privacy of medical data. One way is to store medical data in a local database and set up an access control strategy of the database. The other way is to encrypt medical data with the patient's key and to share the key when needed. The problem with the first method is that the data in the local database may be modified or deleted. The problem with the second method is that the key cannot be shared when the patient dies during the diagnosis and treatment. These two problems will damage the availability of data. This paper proposes to establish a shared key that could be reconstructed by the legitimate parties before the process of diagnosis and treatment begins. The data in the diagnosis and treatment process is encrypted and stored in a blockchain using the shared key. The proposal meets the integrity, availability and privacy requirements of medical data. It uses the sibling intractable function families (SIFF) to establish a shared key, and uses the Hyperledger Fabric to store encrypted data. The simulation shows that the system has good efficiency. Additionally, it is the first time to introduce SIFF to a blockchain application.

Abstract:The set of distributed ledger architectures known as blockchain is best known for cryptocurrency applications such as Bitcoin and Ethereum. These permissionless block chains are showing the potential to be disruptive to the financial services industry. Their broader adoption is likely to be limited by the maximum block size, the cost of the Proof of Work consensus mechanism, and the increasing size of any given chain overwhelming most of the participating nodes. These factors have led to many cryptocurrency blockchains to become centralized in the nodes with enough computing power and storage to be a dominant miner and validator. Permissioned chains operate in trusted environments and can, therefore, avoid the computationally expensive consensus mechanisms. Permissioned chains are still susceptible to asset storage demands and non-standard user interfaces that will impede their adoption. This paper describes an approach to addressing these limitations: permissioned blockchain that uses off-chain storage of the data assets and this is accessed through a standard browser and mobile app. The implementation in the Hyperledger framework is described as is an example use of patient-centered health data management.

Abstract:Introduction This paper addresses the dilemmas of accessibility, comprehensiveness, and ownership related to health data. To resolve these dilemmas, we propose and justify a novel, globally scalable reference architecture for a Personal Health Data Space (PHDS). This architecture leverages decentralized content-addressable storage (DCAS) networks, ensuring that the data subject retains complete control and ownership of their personal health data. In today's globalized world, where people are increasingly mobile for work and leisure, healthcare is transitioning from episodic symptom-based treatment toward continuity of care. The main aims of this are patient engagement, illness prevention, and active and healthy longevity. This shift, along with the secondary use of health data for societal benefit, has intensified the challenges associated with health data accessibility, comprehensiveness, and ownership. Method The study is structured around four health data use case scenarios from the Estonian National Health Information System (EHIS): primary medical use, medical emergency use, secondary use, and personal use. We analyze these use cases from the perspectives of accessibility, comprehensiveness, and ownership. Additionally, we examine the security, privacy, and interoperability aspects of health data. Results The proposed architectural solution allows individuals to consolidate all their health data into a unified Personal Health Record (PHR). This data can come from various healthcare institutions, mobile applications, medical devices for home use, and personal health notes. Discussions The comprehensive PHR can then be shared with healthcare providers in a semantically interoperable manner, regardless of their location or the information systems they use. Furthermore, individuals maintain the autonomy to share, sell, or donate their anonymous or pseudonymous health data for secondary use with different systems worldwide. The proposed reference architecture aligns with the principles of the European Health Data Space (EHDS) initiative, enhancing health data management by providing a secure, cost-effective, and sustainable solution.

Designing a Distributed Ledger Technology System for Interoperable and General Data Protection Regulation–Compliant Health Data Exchange: A Use Case in Blood Glucose Data

Abstract:BACKGROUND: Distributed ledger technology (DLT) holds great potential to improve health information exchange. However, the immutable and transparent character of this technology may conflict with data privacy regulations and data processing best practices.OBJECTIVE: The aim of this paper is to develop a proof-of-concept system for immutable, interoperable, and General Data Protection Regulation (GDPR)-compliant exchange of blood glucose data.METHODS: Given that there is no ideal design for a DLT-based patient-provider data exchange solution, we proposed two different variations for our proof-of-concept system. One design was based purely on the public IOTA distributed ledger (a directed acyclic graph-based DLT) and the second used the same public IOTA ledger in combination with a private InterPlanetary File System (IPFS) cluster. Both designs were assessed according to (1) data reversal risk, (2) data linkability risks, (3) processing time, (4) file size compatibility, and (5) overall system complexity.RESULTS: The public IOTA design slightly increased the risk of personal data linkability, had an overall low processing time (requiring mean 6.1, SD 1.9 seconds to upload one blood glucose data sample into the DLT), and was relatively simple to implement. The combination of the public IOTA with a private IPFS cluster minimized both reversal and linkability risks, allowed for the exchange of large files (3 months of blood glucose data were uploaded into the DLT in mean 38.1, SD 13.4 seconds), but involved a relatively higher setup complexity.CONCLUSIONS: For the specific use case of blood glucose explored in this study, both designs presented a suitable performance in enabling the interoperable exchange of data between patients and providers. Additionally, both systems were designed considering the latest guidelines on personal data processing, thereby maximizing the alignment with recent GDPR requirements. For future works, these results suggest that the conflict between DLT and data privacy regulations can be addressed if careful considerations are made regarding the use case and the design of the data exchange system.

Abstract:The privacy of Electronic Health Records (EHRs) is facing a major hurdle with outsourcing private health data in the cloud as there exists danger of leaking health information to unauthorized parties. In fact, EHRs are stored on centralized databases that increases the security risk footprint and requires trust in a single authority which cannot effectively protect data from internal attacks. This research focuses on ensuring the patient privacy and data security while sharing the sensitive data across same or different organisations as well as healthcare providers in a distributed environment. This research develops a privacy-preserving framework viz Healthchain based on Blockchain technology that maintains security, privacy, scalability and integrity of the e-health data. The Blockchain is built on Hyperledger fabric, a permissioned distributed ledger solutions by using Hyperledger composer and stores EHRs by utilizing InterPlanetary File System (IPFS) to build this healthchain framework. Moreover, the data stored in the IPFS is encrypted by using a unique cryptographic public key encryption algorithm to create a robust blockchain solution for electronic health data. The objective of the research is to provide a foundation for developing security solutions against cyber-attacks by exploiting the inherent features of the blockchain, and thus contribute to the robustness of healthcare information sharing environments. Through the results, the proposed model shows that the healthcare records are not traceable to unauthorized access as the model stores only the encrypted hash of the records that proves effectiveness in terms of data security, enhanced data privacy, improved data scalability, interoperability and data integrity while sharing and accessing medical records among stakeholders across the healthchain network.

Abstract:Healthcare data is crucial and sensitive, as it contains absolute information about a patient's medical history, treatments, and actions; this information gets shared among the stakeholders on a routine basis. Patients' information is vital and should be kept accurate, up-to-date, and secret; it should be available only to authorized users. Most of the existing systems are centralized and may breach data privacy. This study mainly focuses on protecting the privacy and security of sensitive healthcare data while sharing with multiple stakeholders. This work presents a privacy-preserving and access-control blockchain-based framework that uses consensus-driven decentralized data management on top of peer-to-peer distributed computing platforms to ensure the privacy, security, accessibility, and integrity of healthcare data. Blockchain technology helps to protect transactions from manipulation due to its features of irreversibility and immutability. Additionally, we thoroughly examine the security requirements afforded by blockchain-enabled systems by incorporating stakeholders like patients, doctors, chemists, and pathology labs as system entities; they can only share information through a proper channel. The proposed framework has been implemented and evaluated using Hyperledger Fabric. We observe that the proposed framework reveals promising benefits in security, regulation compliance, reliability, flexibility, and accuracy.

Abstract:Medical data sharing needs to be done with the utmost respect for privacy and security. It contains intimate data of the patient and any access to it must be highly regulated. With the emergence of vertical solutions in healthcare institutions, interoperability across organisations has been hindered. The authors of this paper propose a blockchain based medical-data sharing solution, utilising Hyperledger Fabric to regulate access to medical data, and using the InterPlanatory File System for its storage. We believe that the combination of these two distributed solutions can enable patients to access their medical records across healthcare institutions while ensuring non-repudiation, immutability and providing data-ownership. It would enable healthcare practitioners to access all previous medical records in a single location, empowering them with the data required for the effective diagnosis and treatment of patients. Making it safe and straightforward, it would also enable patients to share medical data with research institutions, leading to the creation of reliable data sets, laying the groundwork required for the creation of personalised medicine.

Abstract:The rapid development of wearable sensors and the 5G network empowers traditional medical treatment with the ability to collect patients’ information remotely for monitoring and diagnosing purposes. Meanwhile, the health-related mobile apps and devices also generate a large amount of medical data, which is critical for promoting disease research and diagnosis. However, medical data is too sensitive to share, which is also a common issue for IoT (Internet of Things) data. The traditional centralized cloud-based medical data sharing schemes have to rely on a single trusted third party. Therefore, the schemes suffer from single-point failure and lack of privacy protection and access control for the data. Blockchain is an emerging technique to provide an approach for managing data in a decentralized manner. Especially, the blockchain-based smart contract technique enables the programmability for participants to access the data. All the interactions are authenticated and recorded by the other participants of the blockchain network, which is tamper resistant. In this paper, we leverage the K-anonymity and searchable encryption techniques and propose a blockchain-based privacy-preserving scheme for medical data sharing among medical institutions and data users. To be specific, the consortium blockchain, Hyperledger Fabric, is adopted to allow data users to search for encrypted medical data records. The smart contract, i.e., the chaincode, implements the attribute-based access control mechanisms to guarantee that the data can only be accessed by the user with proper attributes. The K-anonymity and searchable encryption ensure that the medical data is shared without privacy leaking, i.e., figuring out an individual patient from queries. We implement a prototype system using the chaincode of Hyperledger Fabric. From the functional perspective, security analysis shows that the proposed scheme satisfies security goals and precedes others. From the performance perspective, we conduct experiments by simulating different numbers of medical institutions. The experimental results demonstrate that the scalability and performance of our scheme are practical.

Consequently, health information systems need to be developed using cooperation and distributed trust principles to allow protected data sharing between multiple domains or entities (e.g., health data service providers, hospitals and research labs). In this paper, we present a novel health information sharing system viz., HonestChain that uses Blockchain technology to allow organizations to have incentive-based and trustworthy cooperation to either access or provide protected healthcare records. More specifically, we use a consortium Blockchain approach coupled with chatbot guided interfaces that allow data requesters to: (a) comply with data access standards, and (b) allow them to gain reputation in a consortium. We also propose a reputation scheme for creation and sustenance of the consortium with peers using Requester Reputation and Provider Reputation metrics. We evaluate HonestChain using Hyperledger Composer in a realistic simulation testbed on a public cloud infrastructure. Our results show that our HonestChain performs better than the state-of-the-art requester reputation schemes for data request handling, while choosing the most appropriate provider peers. We particularly show that HonestChain achieves a better tradeoff in metrics such as service time and request resubmission rate. Additionally, we also demonstrate the scalability of our consortium platform in terms of the Blockchain transaction times.

Abstract:With the assistance of the Internet of Things, the fast developing Healthcare Internet of Things (H-IoT) has promoted the healthcare ecosystem into the era of Health 5.0 and enables many promising medical applications, such as remote healthcare that is crucial in pandemic (e.g., coronavirus disease 2019). Healthcare participants can make accurate diagnosis, treatment, and research based on the shared personal health records (PHRs) sensed from remote H-IoT devices. However, current H-IoT systems fall short of a secure and trustworthy PHR sharing service in remote healthcare, which is able to prevent user privacy leakage and PHR integrity violation together with high efficiency in key distribution alongside efficient data retrieval and fine-grained access control. In response, we present a blockchain-based hierarchical data sharing framework (BHDSF) to provide fine-grained access control and efficient retrieval over encrypted PHRs with low consumed hierarchical key distribution and key leakage resistance. Compared with the existing solutions, the BHDSF takes both untrusted cloud and malicious auditor into consideration simultaneously and achieves trustworthy PHR integrity auditing and metadata verification by leveraging the blockchain technique. Besides, the BHDSF enables efficiently aggregative authentication for the trustworthiness of source records from H-IoT devices, which is lacked in most of the existing data sharing frameworks. Finally, we demonstrate the feasibility of the BHDSF by conducting extensive empirical tests over a real-world dataset.

Abstract:As healthcare systems worldwide continue to grapple with the challenges of interoperability, data security, and accessibility, integrating emerging technologies becomes imperative. This paper investigates the implementation of blockchain technology, specifically Hyperledger Fabric, for Electronic Health Records (EHR) management at Frere Hospital in the Eastern Cape province of South Africa. The paper examines the benefits and challenges of integrating blockchain into healthcare information systems. Hyperledger Fabric's modular architecture is harnessed to create a secure, transparent, and decentralized platform for storing, managing, and sharing EHRs among stakeholders. The study used a mixed-methods approach, integrating case studies and data collection methods through observation and informal questions, with the specific goal of understanding current record management methods and challenges. This method offers practical insights and validates the approach. The result demonstrates the role of blockchain in transforming healthcare, framed within a rigorous exploration and analysis. The findings of this study have broader implications for healthcare institutions seeking advanced solutions to address the persistent challenges in electronic health record management. Ultimately, the research underscores the transformative potential of blockchain technology in healthcare settings, fostering trust, security, and efficiency in the management of sensitive patient data.
