Abstract:OBJECTIVE:To study the effect and safety of vacuum stretcher combined with feeding in cranial magnetic resonance imaging (MRI) examination for neonates. METHODS:A prospective study was performed for the neonates with hyperbilirubinemia, with a gestational age of >34 weeks and stable vital signs, who needed cranial MRI examination and did not need oxygen inhalation hospitalized in the Department of Neonatology, Children's Hospital of Zhejiang University School of Medicine, from September to November, 2019. The neonates were randomly divided into a vacuum stretcher combined with feeding group and a conventional sedation group. Vital signs were monitored before, during, and after MRI examination. The success rate of MRI procedure was recorded. RESULTS:A total of 80 neonates were enrolled in the study, with 40 neonates in the vacuum stretcher combined with feeding group and 40 in the conventional sedation group. The vacuum stretcher combined with feeding group had a significantly higher success rate of MRI procedure than the conventional sedation group (P<0.05). As for the neonates who underwent successful MRI examination, the fastest heart rate after examination in the vacuum stretcher combined with feeding group was significantly lower than that in the conventional sedation group (P<0.05), while there were no significant differences between the two groups in transcutaneous oxygen saturation, respiratory rate, and body temperature before and after MRI examination (P>0.05). No complications, such as apnea, acute allergic reactions, and malignant fever, were observed. CONCLUSIONS:Vacuum stretcher combined with feeding can improve the success rate of MRI procedure and reduce the use of sedatives, and meanwhile, it does not increase related risks.

Abstract:Objective The study was aimed to analyze the clinical application of magnetic compatible cardiac implantable electronic device (CIED) and influence of magnetic resonance investigation (MRI) on the parameters of CIED from our center.MethodsAll of the 545 patients implanted magnetic compatible CIED from August 2011 to December 2016 in the Second Affiliated Hospital,Zhejiang University School of Medicine were enrolled retrospectively.The baseline characteristics and the necessity of MRI were analyzed.We observed the changes of the parameters of CIED after MRI.ResultsAmong 545 patients,307 were male,and 238 were female,with an average age of (69.2±13.0) years old.Hypertension was found in 324(59.4%) patients,diabetes in 90(16.5%) patients,stroke in 72(13.2%) patients,coronary heart disease in 138(25.3%) patients,osteoarthropathy in 30(5.5%) patients,cancer in 52(9.5%) patients.Thirty-seven patients had single-chamber pacemaker,486 patients had dual-chamber pacemaker,17 patients had implantable cardiac defibrillator (ICD),2 patients had cardiac resynchronization therapy pacemaker(CRT-P)and 3 patients had cardiac resynchronization therapy defi brillator(CRT-D).Before the implantation,131 patients underwent 145 times of MR scan.In which 106 times for head,4 times for extremities,20 times for spine,7 times for hepatobiliary and pancreatic diseases,4 times for the heart and 4 times for other parts of the body.Post-procedural MR scan were carried out in 21 patients for 24 times.Among them,13 times were for the head,3 times for extremites,3 times for spine,3 times for hepatobiliary and pancreatic diseases,and 2 times for other parts of the body.The observation showed there was no significant change of parameters of CIED after MRI scanning.ConclusionThere is a growing trend for applying magnetic compatible CIED,and the request for MR scan in Chinese population might differ from those of foreign countries.MR scan is safe and effective for the patients with magnetic compatible CIED according to our center's experience.
Individualised MRI training for paediatric neuroimaging: A child-focused approach

Abstract:Magnetic Resonance Imaging (MRI) in paediatric cohorts is often complicated by reluctance to enter the scanner and head motion-related imaging artefacts. The process is particularly challenging for children with neurodevelopmental disorders where coping with novel task demands in an unfamiliar setting may be more difficult due to symptom-related deficits or distress. These issues often give rise to excessive head motion that can significantly reduce the quality of images acquired, or render data unusable. Here we report an individualised MRI training procedure that enables children with Autism Spectrum Disorders (ASD) to better tolerate the MRI scanner environment based on a child-focused approach and individualised familiarisation strategies, including a pre-visit interview, familiarisation package, and personalised rewards. A medical imaging mobile application was utilised to familiarise participants to multi-sensory aspects of the neuroimaging experience through a variety of themed mini-games and activities. The MRI training procedure was implemented for monozygotic twins (n = 12; 6 twin pairs; age range 7.1-12.9 years) concordant or discordant for ASD. MRI image quality indices were better or comparable to images acquired from a large independent multi-centre ASD cohort. Present findings are promising and suggest that child-focused strategies could improve the quality of paediatric neuroimaging in clinical populations.

Abstract:Exploring and analyzing the effectiveness of an intelligent pacifying strategy information system based on assisted decision-making in reducing the sedation rate of children in short-duration magnetic resonance scans. A total of 125 children aged 3–5 years who underwent MRI scans at a children's hospital from July to December 2021 participated in this study, during which 62 children were assigned to a control group from July to September, and 63 children were assigned to an intervention group from October to December. In the intervention group, the pacifier used the intelligent pacifying strategy information system based on assisted decision-making to assess children's temperament, and utilization of a system-generated pacification plan according to assessment results. In the control group, the pacification plan was formulated by the pacifier based on their own experience and discussion with families of the participating children. The success rate of pacification, duration of pacification, and image quality of the two groups were compare. Compared with the control group, the intervention group had a higher success rate of pacification and lower duration of pacification, with statistically significant differences ( P < 0.05). There was no difference in image quality between the two groups ( P > 0.05). The intelligent pacifying strategy information system can help reduce the use of the sedative drugs in children aged 3–5 years who underwent a short-duration MRI scan.
Feasibility and success of a non-sedated brain MRI training protocol in 7-year-old children from rural and semi-rural Colombia

Abstract:Background: Brain magnetic resonance imaging (MRI) is a crucial tool for clinical evaluation of the brain and neuroscience research. Obtaining successful non-sedated MRI in children who live in resource-limited settings may be an additional challenge. Objective: To present a feasibility study of a novel, low-cost MRI training protocol used in a clinical research study in a rural/semi-rural region of Colombia and to examine neurodevelopmental factors associated with successful scans. Materials and methods: Fifty-seven typically developing Colombian children underwent a training protocol and non-sedated brain MRI at age 7. Group training utilized a customized booklet, an MRI toy set, and a simple mock scanner. Children attended MRI visits in small groups of two to three. Resting-state functional and structural images were acquired on a 1.5-Tesla scanner with a protocol duration of 30-40 minutes. MRI success was defined as the completion of all sequences and no more than mild motion artifact. Associations between the Wechsler Preschool and Primary Scale of Intelligence (WPPSI), Movement Assessment Battery for Children (MABC), Behavioral Rating Inventory of Executive Function (BRIEF), Child Behavior Checklist (CBCL), and Adaptive Behavior Assessment System (ABAS) scores and MRI success were analyzed. Results: Mean (SD) age at first MRI attempt was 7.2 (0.2) years (median 7.2 years, interquartile range 7.1-7.3 years). Twenty-six (45.6%) participants were male. Fifty-one (89.5%) children were successful across two attempts; 44 (77.2%) were successful on their first attempt. Six (10.5%) were unsuccessful due to refusal or excessive motion. Age, sex, and scores across all neurodevelopmental assessments (MABC, TVIP, ABAS, BRIEF, CBCL, NIH Toolbox Flanker, NIH Toolbox Pattern Comparison, WPPSI) were not associated with likelihood of MRI success (P=0.18, 0.19, 0.38, 0.92, 0.84, 0.80, 1.00, 0.16, 0.75, 0.86, respectively). Conclusion: This cohort of children from a rural/semi-rural region of Colombia demonstrated comparable MRI success rates to other published cohorts after completing a low-cost MRI familiarization training protocol suitable for low-resource settings. Achieving non-sedated MRI success in children in low-resource and international settings is important for the continuing diversification of pediatric research studies.

Abstract:Pediatric neuroimaging datasets are rapidly increasing in scales. Despite strict protocols in data collection and preprocessing focused on improving data quality, the presence of head motion still impedes our understanding of neurodevelopmental mechanisms. Large head motion can lead to severe noise and artifacts in magnetic resonance imaging (MRI) studies, inflating correlations between adjacent brain areas and decreasing correlations between spatial distant territories, especially in children and adolescents. Here, by leveraging mock-scans of 123 Chinese children and adolescents, we demonstrated the presence of increased head motion in younger participants. Critically, a 5.5-minute training session in an MRI mock scanner was found to effectively suppress the head motion in the children and adolescents. Therefore, we suggest that mock scanner training should be part of the quality assurance routine prior to formal MRI data collection, particularly in large-scale population-level neuroimaging initiatives for pediatrics.

Abstract:Background: A noninvasive and sensitive imaging tool is needed to assess the fast-evolving baby brain. However, using MRI to study non-sedated babies faces roadblocks, including high scan failure rates due to subjects motion and the lack of quantitative measures for assessing potential developmental delays. This feasibility study explores whether MR Fingerprinting scans can provide motion-robust and quantitative brain tissue measurements for non-sedated infants with prenatal opioid exposure, presenting a viable alternative to clinical MR scans. Assessment: MRF image quality was compared to pediatric MRI scans using a fully crossed, multiple reader multiple case study. The quantitative T1 and T2 values were used to assess brain tissue changes between babies younger than one month and babies between one and two months. Statistical Tests: Generalized estimating equations (GEE) model was performed to test the significant difference of the T1 and T2 values from eight white matter regions of babies under one month and those are older. MRI and MRF image quality were assessed using Gwets second order auto-correlation coefficient (AC2) with its confidence levels. We used the Cochran-Mantel-Haenszel test to assess the difference in proportions between MRF and MRI for all features and stratified by the type of features. Results: In infants under one month of age, the T1 and T2 values are significantly higher (p<0.005) compared to those between one and two months. A multiple-reader and multiple-case study showed superior image quality ratings in anatomical features from the MRF images than the MRI images. Conclusions: This study suggested that the MR Fingerprinting scans offer a motion-robust and efficient method for non-sedated infants, delivering superior image quality than clinical MRI scans and additionally providing quantitative measures to assess brain development.

Tactile Sensation Assisted Motor Imagery Training for Enhanced BCI Performance: A Randomized Controlled Study

Abstract:Objective: Independent of conventional neurofeedback training, in this study, we propose a tactile sensation assisted motor imagery training (SA-MI Training) approach to improve the performance of MI-based BCI. Methods: Twenty-six subjects were recruited and randomly divided into a Training-Group and a Control-Group. All subjects were required to perform three blocks of MI tasks. In the Training-Group, during the second block (SA-MI Training block), tactile stimulation was applied to the left or right wrist while the subjects were performing the left or right-hand MI task, while during the first block (Pre-Training block) and the third block (Post-Training block), subjects performed pure MI tasks without the tactile sensation assistance. In contrast, in the Control-Group, subjects performed the left and right-hand MI tasks in all three blocks. Results: The performance of the Post-Training block (83.2 $\pm$ 11.4%) was significantly ( p = 0.0014) higher than that of the Pre-Training block (73.2 $\pm$ 16.3%). By contrast, in the Control-Group, no significant difference was found among the three blocks. Moreover, after the SA-MI Training, the motor-related cortex activation (i.e., ERD/ERS) and the R $^{2}$ coefficient in the alpha-beta band were enhanced, while no training effect was found in the Control-Group. Conclusion: The proposed SA-MI Training approach can significantly improve the performance of MI, which provides a novel training framework for MI-based BCI. Significance: It may be especially beneficial to those who are with difficulty in convention neurofeedback training or performing pure MI mental tasks to gain BCI control.
High success rates of sedation-free brain MRI scanning in young children using simple subject preparation protocols with and without a commercial mock scanner–the Diabetes Research in Children Network (DirecNet) experience

Abstract:BackgroundThe ability to lie still in an MRI scanner is essential for obtaining usable image data. To reduce motion, young children are often sedated, adding significant cost and risk.ObjectiveWe assessed the feasibility of using a simple and affordable behavioral desensitization program to yield high-quality brain MRI scans in sedation-free children.Materials and methods222 children (4–9.9 years), 147 with type 1 diabetes and 75 age-matched non-diabetic controls, participated in a multi-site study focused on effects of type 1 diabetes on the developing brain. T1-weighted and diffusion-weighted imaging (DWI) MRI scans were performed. All children underwent behavioral training and practice MRI sessions using either a commercial MRI simulator or an inexpensive mock scanner consisting of a toy tunnel, vibrating mat, and video player to simulate the sounds and feel of the MRI scanner.Results205 children (92.3%), mean age 7 ± 1.7 years had high-quality T1-W scans and 174 (78.4%) had high-quality diffusion-weighted scans after the first scan session. With a second scan session, success rates were 100% and 92.5% for T1-and diffusion-weighted scans, respectively. Success rates did not differ between children with type 1 diabetes and children without diabetes, or between centers using a commercial MRI scan simulator and those using the inexpensive mock scanner.ConclusionBehavioral training can lead to a high success rate for obtaining high-quality T1-and diffusion-weighted brain images from a young population without sedation.

Abstract:BACKGROUND: Young children undergoing magnetic resonance imaging (MRI) require sedation. In June 2013, Tokyo Metropolitan Ohtsuka Hospital (TMOH) introduced an oral sedation protocol for young children undergoing MRI; the protocol included instructions on fasting before sedation, and recommended a shorter duration of sleep the night before MRI. We compared the MRI success rate before and after the introduction of this protocol.METHODS: The eligible subjects were children under 3 years old who underwent MRI by appointment at TMOH between October 2012 and March 2014, under sedation with triclofos sodium. All those who underwent MRI in or after June 2013 were enrolled prospectively as a post-protocol group. All patients who underwent MRI before June 2013 were enrolled retrospectively as a pre-protocol group, with data collected from chart review.RESULTS: Seventy-four patients were enrolled in the post-protocol group, and 42 in the pre-protocol group. The MRI success rate was significantly higher in the post-protocol group than in the pre-protocol group (98.7% vs 88.1%), as was the rate of on-time starting of MRI (86.5% vs 71.4%). The post-protocol group woke up earlier on the day of examination (6:18 a.m. vs 6:43 a.m.), resulting in a significantly longer time between awakening and the beginning of sedation (289.8 min vs 265.9 min), and a significantly shorter average duration of sleep on the previous night (504.8 min vs 532.3 min).CONCLUSIONS: Implementation of a hospital-wide sedation protocol for young children undergoing MRI significantly improved the MRI success rate.

The study and cooperation of children's care during brain perfusion imaging

Abstract:Objective Taking good care is a very important part and quality assurance to make a successful study during children's nuclear examination for nervous system. This paper aims at summarizing the caring experience so as to coordinate the SPECT examination better. Methods 140 children underwent brain perfusion imaging in our department. The main procedures are as follows: take psychological care, orally take potassium chlorate 400mg and block out the vision and hearing. Then inject 99mTc-ECD 14.8MBq/kg intravenously.Sedate those in-cooperated patients. The equipment for the examination is SIEMENS E. CAM/ICON SPECT. Results In the 140 cases of brain perfusion imaging, 138 cases were successful, while 2 patients moved their head during the examination. Conclusion Through the care of the 140 patients during the examination, we realize that it is very important to communicate with the children actively to get their cooperation. To those autistic children, the operation should be quick and precise. After the administration of sedative, more attention should be paid to the children's respiration and pulse. Make sure that the imaging reagents are injected into the vessel entirely.
Analysis of the Application Effect of Dual-Perspective Care Mode in Patients with Magnetic Resonance Imaging Examination

Abstract:Objectives: This paper aims to explore application effect of the dual-perspective care mode for safety and comfort needs in patients with magnetic resonance imaging (MRI) examination. Methods: A total of 132 patients with MRI examination who were admitted to our hospital from September 2018 to February 2020 were selected as the research objects, and they were divided into control group and intervention group by random number table method, with 66 cases in each group. Patients in the control group received routine nursing care, while patients in the intervention group received the dual-perspective nursing care. The psychological state, nursing safety quality, nursing satisfaction, and quality of life scores of the two groups were compared and analyzed. Results: The Self-Rating Anxiety Scale (SAS) (32.05 ± 2.63) and Self-Rating Depression Scale (SDS) (33.14 ± 4.15) of patients in the intervention group receiving dual-perspective nursing mode were lower than those in the control group, and the improvement level of psychological states such as anxiety and depression was significantly better than that in the control group, the difference was statistically significant (P < 0.05). Intervention group patients in early treatment of dangerous situation (3.26 ± 1.08), implementation of safety measures (3.32 ± 1.15), prevention of accidental injury (3.17 ± 0.95), and other eight nursing safety quality index scores were higher than the control group (P < 0.05). The nursing satisfaction of patients in the intervention group (93.94%) was significantly higher than that in the control group (75.76%), and the differences were statistically significant (P < 0.05). In addition, the five life quality indexes of the intervention group were higher than those of the control group, including physiological (90.51 ± 6.39), psychological (91.33 ± 5.76), and emotional (90.45 ± 5.39). Conclusion: The dual-perspective nursing intervention for the safety and comfort needs of patients with MRI examination can effectively improve the anxiety and depression in the diagnosis and treatment, ensure the safety and comfort of the examination process, improve patients’ satisfaction with nursing, so as to ensure the effectiveness and accuracy of the examination results, which has great clinical application value.
Feasibility and efficacy of low-field magnetic resonance imaging in critically unwell children: A developing country experience

Abstract:Introduction and aim A novel portable low-field MRI (pMRI) system has been recently developed and approved for use in different care settings. This study aims to describe the experience of using pMRI at the bedside in pediatric intensive care unit (PICU) of a developing country and evaluate its efficacy for diagnosing acute brain injury (ABI) in critically ill children compared to standard neuroimaging, conventional magnetic resonance imaging (cMRI) and computed tomography (CT) scan. Patients and methods In this prospective pilot cohort study, children admitted to PICU (aged 0–8 years), between May 2021 – June 2022, and underwent cMRI or CT scan brain were included. pMRI scans were performed within a 24-h window of standard neuroimaging, at the bedside, excluding clinically unstable and agitated children who required sedation. pMRI Images were transferred to online system for reporting by a trained pediatric radiologist, while standard neuroimaging was reported as per routine. The radiological results of both modalities were compared, and kappa-statistic computed to assess the level of agreement. Results A total 108 eligible patients were identified, 88 patients underwent pMRI within 24-h of standard neuroimaging, 85 (96·6%) patients completed scan and 73 were included in final analysis. One patient was agitated, one became unstable, and one required another therapeutic procedure so pMRI couldn't be completed. Mean age was 4·8 (±5.2) years. Indications for neuroimaging; unexplained encephalopathy 29 (39·7%), seizures 38 (52·1%), focal neurologic deficit 19 (26%), traumatic brain injury 6 (8·2%) and diplopia 2 (2·7%). The pMRI scan time was 51·3 min, provisional diagnosis of edema 27 (37·0%), hydrocephalus 12 (16·4%), Infarction 13 (17·8%), midline shift 13 (17·8%), intra-parenchymal hemorrhage 7 (9·6%), and intracranial mass 4 (5·5%) were made through pMRI. Compared to a standard neuroimaging, most pMRI findings agreed with an overall moderate agreement 73·91% (k 0·406, P 0·0003) between both modalities. Conclusion pMRI is safe and feasible modality for diagnosing ABI in critically ill children and shows promising results in children admitted to PICU.

The objectives of this study were to examine (1) child- and parent-reported information status before magnetic resonance imaging (MRI), (2) experiences during an MRI and (3) needs and suggestions for improvement. Methods: Children (≥8 years) and parents answered questionnaires (before and after planned MRI examination) covering mental condition, information status/needs, preparation for MRI, and potential stressors. Before MRI n = 132 accompanying parents and n = 91 children provided data (after MRI: n = 93 parents; n = 71 children). The mean age of the children undergoing MRI was 10.5 years (SD = 4.9). Children had on average seven previous MRIs before our survey (Range: 1-33). Twenty-three percent of the children were to be sedated during the examination. Results: Parents and children reported low to medium levels of burden, high knowledge and high information status. For the children, most stressful factors during the MRI were boredom and noise. Main information resources were the radiologists or self-searches. Parents of children with their first MRI reported higher anxiety levels in their children and stated a worse information status (main information source: self-searches/internet). Parents reported needs regarding organization of the MRI (e.g., reduction of waiting times), age-appropriate information and communication during the MRI. Children wished to visit the MRI room prior the examination (44%) and to get more information (44%). Children suggested improvements such as better sound quality of films/music, more transparency about the procedure, being in contact with a parent and shorter examinations. Conclusion: Results suggest that preparation, information provision and care is perceived positively by families. Needs and suggestions for improvement were, for example, higher transparency of procedures, better communication and reduction of stress. Findings indicate that preparation in routine-care should follow an individualized child-focused approach, should focus on families without previous MRIs and should address children with high anxiety levels.
Application of Quantitative Magnetic Resonance Imaging in the Diagnosis of Autism in Children

Abstract:Objective To explore the application of quantitative magnetic resonance imaging in the diagnosis of autism in children. Methods Sixty autistic children aged 2–3 years and 60 age- and sex-matched healthy children participated in the study. All the children were scanned using head MRI conventional sequences, 3D-T1, diffusion kurtosis imaging (DKI), enhanced T2*- weighted magnetic resonance angiography (ESWAN) and 3D-pseudo continuous Arterial Spin-Labeled (3D-pcASL) sequences. The quantitative susceptibility mapping (QSM), cerebral blood flow (CBF), and brain microstructure of each brain area were compared between the groups, and correlations were analyzed. Results The iron content and cerebral blood flow in the frontal lobe, temporal lobe, hippocampus, caudate nucleus, substantia nigra, and red nucleus of the study group were lower than those in the corresponding brain areas of the control group ( P < 0.05). The mean kurtosis (MK), radial kurtosis (RK), and axial kurtosis (AK) values of the frontal lobe, temporal lobe, putamen, hippocampus, caudate nucleus, substantia nigra, and red nucleus in the study group were lower than those of the corresponding brain areas in the control group ( P < 0.05). The mean diffusivity (MD) and fractional anisotropy of kurtosis (FAK) values of the frontal lobe, temporal lobe and hippocampus in the control group were lower than those in the corresponding brain areas in the study group ( P < 0.05). The values of CBF, QSM, and DKI in frontal lobe, temporal lobe and hippocampus could distinguish ASD children (AUC > 0.5, P < 0.05), among which multimodal technology (QSM, CBF, DKI) had the highest AUC (0.917) and DKI had the lowest AUC (0.642). Conclusion Quantitative magnetic resonance imaging (including QSM, 3D-pcASL, and DKI) can detect abnormalities in the iron content, cerebral blood flow and brain microstructure in young autistic children, multimodal technology (QSM, CBF, DKI) could be considered as the first choice of imaging diagnostic technology. Effects of Transcranial Magnetic Stimulation Combined with Computer-Aided Cognitive Training on Cognitive Function of Children with Cerebral Palsy and Dysgnosia

Abstract:Objective: This study is aimed at researching transcranial magnetic stimulation (TMS) effects combined with computer-aided cognitive training (CACT) on cognitive function of children suffering from cerebral palsy and dysgnosia. Methods: From December 2019 to October 2021, 86 children with cerebral palsy and dysgnosia who were treated at our hospital were recruited and assigned into observation and control groups (n = 43, each) using the random number table technique. The observation group received TMS combined with CACT (TMS+CACT), whereas the control group received only TMS. Chinese Wechsler Young Children Scale of Intelligence (C-WYCSI) and Chinese-Wechsler Intelligence Scale for Children (C-WISC) were used to evaluate the intelligence level of the two groups; Gross Motor Function Measure-88 (GMFM-88) of Fudan Chinese version was employed for evaluating the gross motor function of the two groups; a comparison was drawn among the two groups for the cerebral hemodynamic parameters before and after the treatment. Results: For young children, the verbal intelligence quotient (VIQ) scores at 6 and 12 weeks of treatment in the observation group were increased when compared to those in the control group (48.91 ± 3.70 vs. 47.32 ± 3.33, 54.25 ± 4.46 vs. 49.48 ± 3.36), and the observation group's performance intelligence quotient (PIQ) score at 12 weeks of treatment was higher as to that of the control group (65.38 ± 4.23 vs. 62.81 ± 4.74, all P < 0.05). For older age children, the observation group's VIQ and PIQ scores were greater than the control group's at 6 and 12 weeks of treatment, with statistical significance (63.80 ± 3.76 vs. 59.50 ± 5.32, 74.64 ± 12.04 vs. 65.08 ± 6.30; 63.91 ± 5.96 vs. 58.42 ± 3.70, 72.73 ± 5.06 vs. 66.42 ± 5.93; all P < 0.05). The GMFM-88 scale scores in both groups were increased after 6 and 12 weeks of treatment. After treatment for 12 weeks, the observation group's A-E scores were greater than those of the control group (all P < 0.05). The peak systolic velocity (V s), end-diastolic velocity (V d), and mean velocity (V m) at the anterior cerebral artery (ACA), middle cerebral artery (MCA), and posterior cerebral artery (PCA) in the observation group were dramatically increased than those in the control group (all P < 0.05) after 12 weeks of treatment. Conclusion: TMS+CACT can effectively improve the intelligence level, cognitive ability, gross motor function, and cerebral blood flow of children suffering from cerebral palsy and intellectual disability.

Abstract:Purpose: This trial aims to investigate the efficacy and safety of virtual reality (VR) combined with repetitive transcranial magnetic stimulation (rTMS) for improving musculoskeletal pain and motor development in children with unilateral spastic cerebral palsy (CP). Methods: This study protocol is for a randomized controlled trial consisting of 2 treatment sessions (3 days/week for 4 weeks in each session, with a 1-week interval between sessions). We will recruit children aged 3-10 years with unilateral spastic CP (Gross Motor Function Classification System level I or II). Participants will be randomly divided into 3 groups: the VR + rTMS group (immersive VR intervention, rTMS and routine rehabilitation therapy), rTMS group (rTMS and routine rehabilitation therapy), and control group (sham rTMS and routine rehabilitation therapy). VR therapy will involve a daily 40-minute movement training session in a fully immersive environment. rTMS will be applied at 1 Hz over the primary motor cortex for 20 min on the contralateral side. The stimulation intensity will be set at 90% of the resting motor threshold, with 1200 pulses applied. A daily 60-minute routine rehabilitation therapy session including motor training and training in activities of daily living will be administered to all participants. The primary outcome will be pain intensity, assessed by the Revised Face, Legs, Activity, Cry, and Consolability Scale (R-FLACC). The secondary outcomes will include motor development, evaluated by the 66-item version of the Gross Motor Function Measure (GMFM-66) and Fine Motor Function Measure (FMFM); balance capacity, measured by the interactive balance system; activities of daily living; and quality of life, measured by the Barthel index and the Chinese version of the Cerebral Palsy Quality of Life scale for Children (C-CP QOL-Child). Safety will be monitored, and adverse events will be recorded during and after treatment. Discussion: Combined application of VR therapy and rTMS may reveal additive effects on pain management and motor development in children with spastic CP, but further high-quality research is needed. The results of this trial may indicate whether VR therapy combined with rTMS achieves a better analgesic effect and improves the motor development of children with spastic CP. Abstract:Repetitive Transcranial magnetic stimulation (rTMS) is emerging as a new investigation as well as treatment tool for various neurological and psychiatric diseases. Recent studies showed its application as treatment tool in movement disorders, where rTMS stimulation on primary motor cortex alters physiological patterns of motor threshold; motor evoked potential and cortical plasticity which induces motor activity. Recent studies on rTMS combined with rehabilitation therapy demonstrated functional improvement in motor activities of spastic cerebral palsy (sCP) children. Thus, this study was designed to evaluate the effect of rTMS on hand function of sCP patients. Forty-five children diagnosed as sCP participated in this study after written consent from their parents or guardians. They were divided into three groups- control (CG) and interventional group (IG-A and IG-B). Participants in CG were provided only physical therapy (PT) of 30 minutes duration daily for 20 days and those in IG were administered rTMS frequency of 5Hz (IG-A) and 10Hz (IG-B) for 15 minutes consisting of 1500 pulses daily for 20 days; followed by PT as given to CG. Quality of upper extremity skill test (QUEST) scoring was used for evaluating the improvement in hand function of sCP patients. The pre (before starting any therapy) versus post (after completion of 20 sessions) mean QUEST score between different groups were statistically significant (p<0.01) and the mean change was 0.61, 2.46 and 2.87 in CG, IG-A and IG-B respectively. However, encouraging functional improvement in hand function was observed in diplegic patients in the age group of 2-6 years employing 5Hz frequency and higher frequency (10Hz) induced better activity in hemiplegic and quadriplegic patient of older age groups (7-16 years).
