Abstract:Introduction: The aim of this study was to assess the results of the local pre-treatment verifications of online adaptive prostate SBRT plans performed by dosimetrists METHODS AND MATERIALS: Prostate SBRT treatments are planned in our department using an online adaptive method developed and validated by our group. The adaptive plans were computed on the daily CBCT scan using the Acuros XB v. 16.1 algorithm of the Varian Eclipse treatment planning system. Adaptive plans consisted of a single VMAT with 6 MV flattening-filter-free (FFF) energy performed on a Varian TrueBeam linac. Pre-treatment verification of the adaptive "plan-of-the-day" (POD) created in each treatment session was performed using the Mobius 3D v. 3.1 secondary dose calculation program (M3D). Commissioning of M3D included the tuning of the dosimetric leaf gap correction (DLGc) parameter. Generic and specific DLGc values were then derived using a set of plans for typical sites (prostate, head and neck, brain, lung and bone palliative) and another set were determined for specific online SBRT PODs (gDLGc and sDLGc, respectively). The first 50 prostate patients treated with the PACE-B schedule (5 × 7.25 Gy) were included, i.e., 250 adaptive SBRT PODs were collected in this study. For each online adaptive POD, a global 3D gamma comparison between the Eclipse 3D dose and the M3D dose in the patient CBCT was performed. Gamma passing rates (GPRs) for the whole external patient contour (Body) and the PTV were recorded, using the 5 % global /3 mm criteria. The target mean dose and target coverage differences between the Eclipse and M3D doses were also analyzed (ΔDmean and ΔD90 %, respectively). The accuracy of M3D was assessed against PRIMO Monte Carlo software. Twenty-five online prostate SBRT PODs were randomly selected from the set of 250 adaptive plans and simulated with PRIMO. Results: Values of -1 mm and -0.14 mm were found as optimal gDLGc and sDLGc, respectively. Over the 250 online adaptive PODs, excellent GPR values ∼ 100 % were obtained for the Body and PTV structures, regardless the type of DLGc used. The use of the sDLGc instead of the gDLGc provided better results for ΔDmean (0.1 % ± 0.5% vs. -1.9 ± 0.7 %) and ΔD90 % (-1.0 % ± 0.5 %. vs. -3.5 % ± 0.8 %). This issue was also observed when M3D calculations were compared to PRIMO simulations. Conclusions: M3D can be effectively used for independent pre-treatment verifications of online adaptive prostate SBRT plans. The use of a specific DLGc value is advised for this SBRT online adaptive technique.

Abstract:Background Quality assurance (QA) remains unstandardized for CT‐guided online adaptive radiotherapy (CTgART) platforms (Ethos, Varian Medical Systems, Inc., Palo Alto, CA), as they become more clinically prevalent. A secondary dose calculation software (Mobius3D, Varian Medical Systems, Inc., Palo Alto, CA) is provided for this closed CTgART system. However, the clinical impact of tuning dosimetric leaf gap (DLG) correction values for specific delivery techniques for CTgART secondary dose calculations remains uninvestigated. Purpose Tuning the DLG correction value for different delivery techniques of the independent secondary dose verification software for adaptive online QA. Methods A total of 31 volumetric arc therapy (VMAT) and 13 fixed‐gantry intensity modulated radiation therapy (IMRT) plans were selected from representative anatomical sites treated in our clinic. All plans were evaluated on a patient CT dataset and a QA dataset of a solid water phantom with an embedded ion chamber placed at the center in both primary treatment planning systems (TPS) and secondary dose verification software. Primary TPS plan doses were compared with secondary calculation doses on patient CT by calculating 3D gamma passing criteria under different DLG correction values ranging from –2 to 2 mm to determine the optimal DLG correction range. Ion chamber verification measurements were then compared to secondary calculation dose to determine the optimal DLG correction value by minimizing the difference for IMRT and VMAT plans, separately. Results The optimal DLG correction values for VMAT and IMRT techniques were –0.3 and 0.4 mm respectively for the selected clinical cohort of patients. The mean gamma passing rate between primary and secondary doses for VMAT and IMRT were 99.0% ± 1.0% and 99.9% ± 0.1% with 3%/2 mm and excluding 10% low dose criteria. The mean ion chamber agreement for VMAT and IMRT were 0.0% ± 2.1% and 0.2% ± 1.4%. Conclusion DLG correction value should be tuned for each delivery technique (VMAT and IMRT) separately to maximize the robustness of CTgART online QA programs.

Study of PlanIQ Feasibility DVH Usage to Improve Cervical Cancer Treatment Plan

Abstract:The implementation of the optimal treatment plan by determining what is reasonably achievable is a challenge, influenced also by the planner's experience and available time. The aim of this study is to explore the clinical advantage of PlanIQ Feasibility DVH (FDVH) module in the design process of cervical cancer treatment plan. Tomotherapy clinically delivered treatment plans (CdP) of twenty cervical cancer patients randomly selected were included in this retrospective study. Using the same clinical dose prescriptions as CdP, an ideal plan (IP) was generated for each patient by PlanIQ FDVH using the minimal dose that any voxel outside the targets must receive given 100% target coverage. A new treatment plan (NP) was therefore optimized using as endpoint the ideal target dose distribution and organs-at-risk (OAR) dose limit determined by IP. Dose volume histogram dosimetric parameters D95, conformity and homogeneity index (HI, CI, respectively) for the target and the mean dose (Dm), V20 and V30 for the OARs were used to compare CdP and NP. Data were analyzed with the t-Student test; p-value less than 0.05 was considered statistically significant. The OARs and target mean dosimetric values obtained for CdP and NP were reported in Table 1. The results show that both plan groups achieve the clinical requirements. The target dosimetric parameters D95, CI, HI present no statistical differences; for the rectum, the bladder and the small intestine, Dm, V20 and V30 values were significant lower for the NP than for CdP (p < 0.05). Impact of Multileaf Collimator Configuration Parameters on the Dosimetric Accuracy of 6-MV Intensity-Modulated Radiation Therapy Treatment Plans

Abstract:The purpose of this study was to evaluate the impact of selected configuration parameters that govern multileaf collimator (MLC) transmission and rounded leaf offset in a commercial treatment planning system (TPS) (Pinnacle3, Philips Medical Systems, Andover, MA, USA) on the accuracy of intensity-modulated radiation therapy (IMRT) dose calculation. The MLC leaf transmission factor was modified based on measurements made with ionization chambers. The table of parameters containing rounded-leaf-end offset values was modified by measuring the radiation field edge as a function of leaf bank position with an ionization chamber in a scanning water-tank dosimetry system and comparing the locations to those predicted by the TPS. The modified parameter values were validated by performing IMRT quality assurance (QA) measurements on 19 gantry-static IMRT plans. Planar dose measurements were performed with radiographic film and a diode array (MapCHECK2) and compared to TPS calculated dose distributions using default and modified configuration parameters. Based on measurements, the leaf transmission factor was changed from a default value of 0.001 to 0.005. Surprisingly, this modification resulted in a small but statistically significant worsening of IMRT QA gamma-index passing rate, which revealed that the overall dosimetric accuracy of the TPS depends on multiple configuration parameters in a manner that is coupled and not intuitive because of the commissioning protocol used in our clinic. The rounded leaf offset table had little room for improvement, with the average difference between the default and modified offset values being -0.2 ± 0.7 mm. While our results depend on the current clinical protocols, treatment unit and TPS used, the methodology used in this study is generally applicable. Different clinics could potentially obtain different results and improve their dosimetric accuracy using our approach.

Abstract:Objective: This study is part of ongoing efforts aiming to transit from measurement-based to combined patient-specific quality assurance (PSQA) in intensity-modulated proton therapy (IMPT). A Monte Carlo (MC) dose-calculation algorithm is used to improve the independent dose calculation and to reveal the beam modeling deficiency of the analytical pencil beam (PB) algorithm. Methods: A set of representative clinical IMPT plans with suboptimal PSQA results were reviewed. Verification plans were recalculated using an MC algorithm developed in-house. Agreements of PB and MC calculations with measurements that quantified by the gamma passing rate were compared. Results: The percentage of dose planes that met the clinical criteria for PSQA (>90% gamma passing rate using 3%/3 mm criteria) increased from 71.40% in the original PB calculation to 95.14% in the MC recalculation. For fields without beam modifiers, nearly 100% of the dose planes exceeded the 95% gamma passing rate threshold using the MC algorithm. The model deficiencies of the PB algorithm were found in the proximal and distal regions of the SOBP, where MC recalculation improved the gamma passing rate by 11.27% (p < 0.001) and 16.80% (p < 0.001), respectively. Conclusions: The MC algorithm substantially improved the gamma passing rate for IMPT PSQA. Improved modeling of beam modifiers would enable the use of the MC algorithm for independent dose calculation, completely replacing additional depth measurements in IMPT PSQA program. For current users of the PB algorithm, further improving the long-tail modeling or using MC simulation to generate the dose correction factor is necessary. Advances in knowledge: We justified a change in clinical practice to achieve efficient combined PSQA in IMPT by using the MC algorithm that was experimentally validated in almost all the clinical scenarios in our center. Deficiencies in beam modeling of the current PB algorithm were identified and solutions to improve its dose-calculation accuracy were provided.

Abstract:To the Editor.— Fluoxetine hydrochloride (Prozac) is a new antidepressant characterized by selective inhibition of central nervous system neuronal up-take of serotonin. 1 Fibromyalgia is a common, chronic, rheumatologic pain disorder that may respond to antidepressant treatment. 2 Only one patient with fibromyalgia has been reported to have been treated successfully with fluoxetine. 3 We describe two additional patients. Case 1.— A 41-year-old woman was diagnosed in 1987 as having fibromyalgia by the criteria of Yunus et al. 4 Over the past 3 years, response to trials of trazodone hydrochloride, nortriptyline hydrochloride, and doxepin hydrochloride had been poor. One week after adding 20 mg of fluoxetine hydrochloride every other day to 25 mg of doxepin hydrochloride every evening, the patient reported markedly decreased pain and increased energy, as well as moderately improved sleep. Three months later, she has maintained improvement of her symptoms by taking 20 mg of fluoxetine hydrochloride
Dosimetric Impact of Optimization Resolutions Using the New Photon Optimizer Algorithm in Eclipse Treatment Planning System

Abstract:Objective To investigate the effect of optimization resolutions of new photon optimizer in Eclipse treatment planning system on the dose to planning target volume (PTV) and organs-at-risk (OAR) of preoperational simultaneous-integrated-boosting volumetric modulated arc therapy plans for patients with rectal cancer,and provide clinical guidelines of resolution selection to strike a balance between plan quality and efficiency.Methods After the RapidPlan model applied in the research was set with a resolution of 1.25,2.50 and 5.00 mm,respectively,the dose-volume histogram (DVH) were estimated,respectively,and Application Programming Interface (API) script was used to export these data for comparison.Herein,3 plans using different resolutions were generated for 20 patients,and their DVH was exported.We applied the in-house MATLAB code to calculate the mean DVH,and SigmaPlot and SPSS soltware for plotting and statistical analysis.Time for DVH estimation and optimization were also recorded and analyzed.Results According to the API exportation,the DVH estimation and optimized parameters acquired by various resolutions were exactly the same.However,the optimization time using a resolution of 1.25 mm was about 3-4 times of that using the resolution of 2.50 and 5.00 mm.All the resolutions satisfied clinical requirements for target dose coverage.The differences in the conformity index of PTV and planning gross tumor volume,and the homogeneity index of PTV were not statistically significant (P＞0.05).Though the homogeneity index of planning gross tumor volume was significantly different among 3 plans,the magnitude of deviations was within 0.01,which was considered clinically negligible.Relative to the resolutions of 2.50 and 5.00 mm,1.25 mm did not always guarantee the best OAR sparing.The differences in mean DVH were marginal.Conclusion The impacts of optimization resolutions on the dose to target areas and OAR are trivial,but the differences in optimization time are dramatic.The plan with a resolution of 2.50 mm was most cost-effective.
Dose Calculation Deviations Induced by Fractional Image-Guided-couch-shifts for Varian Halcyon MV Cone Beam CT

Abstract:Purpose: To assess the criticality of calculation deviations induced by fractional image-guided-couch-shifts for Halcyon MV cone beam CT (CBCT) dose, which is incorporated as part of total treatment dose. Methods: Eclipse-calculated imaging dose was first validated in 'Cheese Phantom' by measurement. Then, the actual imaging dose (D-act) for 18 historical patients of various sites were recalculated based on 513 MV CBCTguided-couch-shift data, and compared with reference computations based on treatment isocentre (D-ref). Patient-and plan-specific dose from treatment fields was integrated with D-act and D-ref respectively for comparison. Results: The average absolute relative disagreements between the measured and calculated dose were less than 1.23%. The mean +/- 1SD of gamma passing rates of the accumulated imaging dose and total dose were 80.71 +/- 6.22% and 99.81 +/- 0.32% respectively based on 3 mm/3%/local/10% threshold criteria. The accumulated errors of minimum imaging dose to PTV were no larger than -14.38 cGy, which were reduced to -0.82 cGy after the heterogeneous treatment dose was overlaid. The mean relative discrepancies of PTV minimum dose were - 0.61 cGy ( - 0.71%) and - 0.00 (0.00%), before and after incorporating the treatment dose respectively. Conclusions: The Eclipse-calculated Halcyon MV CBCT dose was validated. Although the isocentre displacement-induced imaging dose calculation errors for Halcyon MV CBCT were partially cancelled out by couch shifts of various directions and distances, especially after the incorporation of heterogeneous treatment dose, it was still advisable to monitor the accumulated deviations and replan when unacceptable target under-dose or organ overdose were observed.
Quantification of Dosimetry Improvement With or Without Patient Surface Guidance

Abstract:Purpose: Noncoplanar beams and arcs are routinely used to improve dosimetry for intracranial cases, but their application for extracranial cases has been hampered by the risk of collision. This has led to conservative beam selection whose impact on plan dosimetry has not been previously studied. Methods and materials: A full-body 3-dimensional patient surface was acquired using optical cameras for a single lung patient at the time of computed tomography simulation. Eight stereotactic body radiation therapy (SBRT) plans were created for the patient, with varying degrees of noncoplanarity and deliverability. The plans included volumetric modulated arc therapy and intensity modulated radiation therapy (IMRT) plans ranging from simple, coplanar arcs to multiple noncoplanar arcs and IMRT beams. A total of 70 fields were created across the 8 plans, of which 21 fields were undeliverable with a 5-cm buffer. Organs-at-risk (OARs) metrics including R50, Dmax 2 cm from the PTV, lung V20, and chest wall V30 were evaluated. Five expert SBRT dosimetrists from 5 institutions evaluated field deliverability, with or without the guidance of the clearance map. Results: In the dosimetry evaluation, a clear trend in increasing dosimetric compactness and OAR sparing is observed with increasing plan noncoplanarity. R50, Dmax 2 cm, lung V20, and chest wall V30 decreased 41%, 39%, 43%, and 57%, respectively, from plan 1 (2 coplanar partial arcs) to plan 8 (19 noncoplanar IMRT beams). In the observer tests, the expert dosimetrists' ability to accurately discern beam deliverability because of collision significantly increases with the clearance map. The errors in predicting colliding fields were eliminated using the whole-body surface and clearance map, and the user was able to select fields based on plan quality and patient comfort instead of being overly conservative. Conclusion: The study shows that incorporating a personalized, whole-body clearance map in the treatment planning workflow can facilitate the adoption of noncoplanar beams or arcs that benefit the SBRT plan dosimetry.

Abstract:Background For patient‐specific quality assurance (PSQA) for small targets, the dose resolution can change depending on the characteristics of the dose calculation algorithms. Purpose This study aimed to evaluate the influence of the dose calculation algorithms Acuros XB (AXB), anisotropic analytical algorithm (AAA), photon Monte Carlo (pMC), and collapsed cone (CC) on a helical diode array using volumetric‐modulated arc therapy (VMAT) for small targets. Materials and methods ArcCHECK detectors were inserted with a physical depth of 2.9 cm from the surface. To evaluate the influence of the dose calculation algorithms for small targets, rectangular fields of 2×100, 5×100, 10×100, 20×100, 50×100, and 100×100 mm2 were irradiated and measured using ArcCHECK with TrueBeam STx. A total of 20 VMAT plans for small targets, including the clinical sites of 19 brain metastases and one spine, were also evaluated. The gamma passing rates (GPRs) were evaluated for the rectangular fields and the 20 VMAT plans using AXB, AAA, pMC, and CC. Results For rectangular fields of 2×100 and 5×100 mm2, the GPR at 3%/2 mm of AXB was 10×100 mm2, the GPR at 3%/2 mm was > 88.1% and comparable for all dose calculation algorithms. For the 20 VMAT plans, the GPRs at 3%/2 mm were 79.1 ± 15.7%, 93.2 ± 5.8%, 94.9 ± 4.1%, and 94.5 ± 4.1% for AXB, AAA, pMC, and CC, respectively. Conclusion The behavior of the dose distribution on the helical diode array differed depending on the dose calculation algorithm for small targets. Measurements using ArcCHECK for VMAT with small targets can have lower GPRs owing to the coarse dose resolution of AXB around the detector area.

Voxel-Based Dose Prediction with Multi-Patient Atlas Selection for Automated Radiotherapy Treatment Planning

Abstract:Automating the radiotherapy treatment planning process is a technically challenging problem. The majority of automated approaches have focused on customizing and inferring dose volume objectives to used in plan optimization. In this work we outline a multi-patient atlas-based dose prediction approach that learns to predict the dose-per-voxel for a novel patient directly from the computed tomography (CT) planning scan without the requirement of specifying any objectives. Our method learns to automatically select the most effective atlases for a novel patient, and then map the dose from those atlases onto the novel patient. We extend our previous work to include a conditional random field for the optimization of a joint distribution prior that matches the complementary goals of an accurately spatially distributed dose distribution while still adhering to the desired dose volume histograms. The resulting distribution can then be used for inverse-planning with a new spatial dose objective, or to create typical dose volume objectives for the canonical optimization pipeline. We investigated six treatment sites (633 patients for training and 113 patients for testing) and evaluated the mean absolute difference (MAD) in all DVHs for the clinical and predicted dose distribution. The results on average are favorable in comparison to our previous approach (1.91 vs 2.57). Comparing our method with and without atlas-selection further validates that atlas-selection improved dose prediction on average in Whole Breast (0.64 vs 1.59), Prostate (2.13 vs 4.07), and Rectum (1.46 vs 3.29) while it is less important in Breast Cavity (0.79 vs 0.92) and Lung (1.33 vs 1.27) for which there is high conformity and minimal dose shaping. In CNS Brain, atlas-selection has the potential to be impactful (3.65 vs 5.09), but selecting the ideal atlas is the most challenging.

Abstract:Purpose: To present a two-stage optimization approach that explicitly considers aperture-specific dose distributions prior to dose optimization. Method and Materials: Approximate and fast non-Monte Carlo dose calculation models have typically been utilized for pencil beam weight optimization. Such approximate dose calculations do not take into account aperture specific collimator effects. This leads to potentially significant differences between the dose distribution corresponding to the optimization and a final more accurate dose calculation is typically used to evaluate the treatment plan (conventional planning). We propose a 2-stage approach in which the aperture specific dose distributions were calculated using Monte Carlo and then optimized using sequential linear programming techniques. Our approach was compared with the “conventional” IMRT planning approach and a single-stage process known as “direct aperture optimization”. Results: We have tested our approach on a head and neck case in the which, the target involved the primary tumor plus the nodal volume. The dose prescription was a simultaneous integrated boost. The conventional plan used 159 apertures, the 2-stage plan used 123 apertures (36 of the original apertures yielded weights of zero) and the DMPO plan used 89 apertures. The 2-stage plan outperformed both the conventional IMRT plan and the DMPO plans. The 2-stage optimization approach resulted in an improvement of almost 10–14% for the RP compared with the other optimization approaches and in the lowest maximum cord and brainstem doses. The 2-stage approach also resulted in the least volume of the CTV (both primary and nodal volumes) receiving “hot spots”. These improvements were achieved with greater delivery efficiency relative to the conventional plan. Conclusion: Our results demonstrate that the 2-stage optimization process in which an initial set of apertures is obtained and then re-optimized using linear programming yields superior results relative to the conventional approach and to the DMPO direct aperture-based approach.
Research on the correction method for radiotherapy verification plans based on displaced electronic portal imaging device

Abstract:Background It has been observed that under the single isocenter conditions, the potential shifts of the electronic portal imaging devices (EPID) may be introduced when executing portal dosimetry (PD) plans for bilateral breast cancer, pleural mesothelioma, and lymphoma. These shifts are relative to the calibration positions of EPID and result in significant discrepancies in the plan verification results. Purpose To explore methods including correction model and specific correction matrices to revise the data obtained from displaced EPID. Methods Two methods, the correction model and the specific correction matrices, were applied to correct the data. Five experiments were designed and conducted to build correction model and to validate the effectiveness of these two methods. Gamma passing rates were calculated and data profiles along X‐axis and Y‐axis were captured. Results The gamma passing rates for the EPID‐displaced IMRT validation plans after applying correction model, along with the application of specific correction matrices to VMAT and IMRT validation plans, exhibit results that are comparable to the cases with non‐displaced EPID. Except for the VMAT plans applied correction model which showed larger discrepancies (0.041 ± 0.028, 0.049 ± 0.030), the other three exhibit minimal differences in discrepancy values. In all profiles, the corrected data from displaced EPID exhibit a high level of agreement with data obtained from non‐displaced EPID. Good consistency is observed in actual application of the correction model and the specific correction matrices between gamma passing rates of data corrected and those of non‐displaced data. Conclusions The proposed methods involving correction model and specific correction matrices can correct the data collected from the displaced EPID, and the gamma passing rates of the corrected data show results that are comparable to some extent with those of non‐displaced data. Particularly, the results corrected by specific correction matrices closely resemble the data from non‐displaced EPID.

Dosimetric Systems in Pre-Treatment QA for Stereotactic Treatments: Correlation Agreements and Target Volume Dependency

Abstract:Aim: This study comprehensively investigated pre-treatment quality assurance (QA) for 100 cancer patients undergoing stereotactic treatments (SRS/SRT) using various detectors. Methods: The study conducted QA for SRS/SRT treatments planned with a 6MV SRS beam at a dose rate of 1,000 MU/min, utilizing Eclipse v13.6 Treatment Planning System (TPS). Point dose measurements employed 0.01cm3 and 0.13cm3 cylindrical ionization chambers, while planar dose verification utilized Gafchromic EBT-XD Film and Portal Imager (aS1000). Plans were categorized by target volume, and a thorough analysis compared point dose agreements, planar dose gamma pass rates, and their correlations with chamber volume mean dose, detector type, and point dose agreement. Additionally, the consistency between different ionization chambers was assessed. Results: Point dose agreement generally improved with increasing target volume, except for volumes over 10cm3 with 0.01cm3 chambers, showing a contrary trend. Significant differences (p<0.05) were observed between TPS and measured doses for both chambers. Gamma pass rate improved with increasing target volume in EBT XD and aS1000 analyses, except for the >10cm3 group in EBT XD. EBT XD demonstrated better agreement with TPS for target volumes up to 10cm3 compared to aS1000, with a statistically significant difference (p<0.05) between the detectors. Strong correlations were found between chamber point dose and chamber volume mean dose agreement, as well as between the two gamma criteria analyses of the same detector type in the planar dose correlation analysis. However, weak correlations were discovered for other analyses. Conclusion: This study found weak correlation between different detector types in pre-treatment QA for point dose and planar dose evaluation. However, within a specific detector type, strong correlation was observed for different point dose evaluation methods and gamma criteria. This highlights the importance of cautious interpretation of QA results, particularly for SRS QA, due to the lack of correlation between detector types.
Atlas-based Dose Optimization Via Linear Programming for IMRT Using a High-throughput Computing Environment

Abstract:To develop and evaluate an atlas-based dose optimization (DO) scheme using linear programming (LP) for disease site specific IMRT treatment planning using a high-throughput computing environment (HTC). Ten locally advanced head and neck cases were retrospectively selected for this study. The treatment targets involved primary tumor, high risk and low risk nodal volumes. Organs-at-risk (OARs) included both parotids and the spinal cord. Seven-beam IMRT plans were generated using the Pinnacle3 planning system. More than 2000 apertures from these 10 cases were extracted (on average 30 apertures per beam per case). These apertures served as the basis for the atlas from which prospective case-specific apertures were derived. Aperture-based 3D dose distributions were calculated in parallel using Monte Carlo-based Kernel Superposition (MCKS) and facilitated using Condor in the HTC environment consisting of 48 2.26GHz CPUs. Condor provided the job queuing mechanism, priority scheme, resource monitoring, and resource management. Aperture weights of each case were optimized via an in-house developed sequential linear program (SLP) DO approach. The SLP approach involved solving the DO problem by dividing the problem into sub-problems (due to the size of the DO problem), each of which was solved using LP while maintaining a global view of the full problem. Our approach was compared with three approaches: conventional IMRT, two-arc intensity-modulated arc therapy (IMAT) (generated using Pinnacle SmartArc 9.0) and SLP corresponding to apertures derived from the conventional IMRT plans for the specific case. Approximately 200 apertures per case were returned with non-zero weight assignments via the atlas-based SLP scheme and 40% of the selected atlas apertures were from other cases. Superior plan quality was achieved with atlas-based plans. On average, at dose constraint levels parotid gland sparing improved 30% (±7%), 31% (±9%) and 18% (±5%), respectively using the atlas-based DO scheme compared with conventional IMRT, two-arc IMAT and the SLP scheme using apertures derived from the conventional IMRT plans. The improvement in the maximum dose to the spinal cord was 10% (±4%), 20% (±6%) and 6% (±2%), respectively when compared with the other three approaches. t-test results showed that these improvements were significant (p < 0.001). Dose homogeneity for all PTVs (tumor and nodes) was higher for the atlas-based plans compared with the other approaches. The atlas-based approach also resulted in lower integral dose to non-PTV and non-OAR tissues. Our results suggest that atlas-based DO using the SLP approach results in superior plan quality compared with conventional IMRT and IMAT.

Abstract:In a joint study, 2061 patterns (1346 women and 715 men) were patch tested with chlorhexidine gluconate 1% in Water. included in a standard lest series. 4H patients (2.3%) showed a positive reaction. These were relatively marc common in men (3.2%) than in women (1.9%), Positive reactions were most common in patients with leg eczema (6.8%) or leg ulcer (10.9%). Of the 48 patients who were patch test positive. 14 (2 with leg ulcer) were retested with chlorhexidine gluconate 0.01 and 1%. Only one with a leg ulcer was positive. These findings indicate that false positive reactions, known as “the incited skin syndrome”, may arise from testing eczema patients in a standard series The sensitizing potential of chlorhexidine may be very low, but the potential in patients with an eczema or ulcer of the leg has to be further evaluated.
Beam mask and sliding window-facilitated deep learning-based accurate and efficient dose prediction for pencil beam scanning proton therapy

Abstract:Purpose: To develop a DL-based PBSPT dose prediction workflow with high accuracy and balanced complexity to support on-line adaptive proton therapy clinical decision and subsequent replanning. Methods: PBSPT plans of 103 prostate cancer patients and 83 lung cancer patients previously treated at our institution were included in the study, each with CTs, structure sets, and plan doses calculated by the in-house developed Monte-Carlo dose engine. For the ablation study, we designed three experiments corresponding to the following three methods: 1) Experiment 1, the conventional region of interest (ROI) method. 2) Experiment 2, the beam mask (generated by raytracing of proton beams) method to improve proton dose prediction. 3) Experiment 3, the sliding window method for the model to focus on local details to further improve proton dose prediction. A fully connected 3D-Unet was adopted as the backbone. Dose volume histogram (DVH) indices, 3D Gamma passing rates, and dice coefficients for the structures enclosed by the iso-dose lines between the predicted and the ground truth doses were used as the evaluation metrics. The calculation time for each proton dose prediction was recorded to evaluate the method's efficiency. Results: Compared to the conventional ROI method, the beam mask method improved the agreement of DVH indices for both targets and OARs and the sliding window method further improved the agreement of the DVH indices. For the 3D Gamma passing rates in the target, OARs, and BODY (outside target and OARs), the beam mask method can improve the passing rates in these regions and the sliding window method further improved them. A similar trend was also observed for the dice coefficients. In fact, this trend was especially remarkable for relatively low prescription isodose lines. The dose predictions for all the testing cases were completed within 0.25s.

Abstract:Purpose To propose an efficient collimator angle optimization method by combining island blocking (IB) and parked gap (PG) problem to reduce the radiotherapy dose for normal tissue. The reduction will be done with single‐isocenter multi‐lesion volumetric modulated arc therapy (VMAT) for the stereotactic body radiation therapy (SBRT) of liver cancer. Methods A novel collimator angle optimization algorithm was developed based on the two‐dimensional projection of targets on a beam's eye view (BEV) plane as a function of gantry and collimator angle. This optimization algorithm minimized the sum of the combined IB and PG (IB & PG) areas from all gantry angles for each arc. For comparison, two SBRT plans were respectively generated for each of the 20 retrospective liver cancer cases with multiple lesions. One plan was optimized using the IB & PG algorithm, and the other plan was optimized with a previously reported optimization algorithm that only considered the IB area. Plans were then evaluated and compared using typical dosimetric metrics. Results With the comparable target coverage, IB & PG plans had significantly lower D500cc, D700cc, mean dose (Dmean), and V15 of normal liver tissues when compared to IB plans. The median percent reductions were 3.32% to 5.36%. The D1cc, D5cc, and Dmean for duodenum and small intestine in IB & PG plans were significantly reduced in a range from 7.60% up to 16.03%. Similarly, the median integral dose was reduced by 3.73%. Furthermore, the percentage of normal liver Dmean sparing when IB & PG plans compared to IB plans, was found to be positively correlated (ρ = 0.669, P = 0.001) with the inter‐target distance. Conclusion The proposed IB & PG algorithm has been demonstrated to outperform the IB algorithm in almost all normal tissue sparing, and the magnitude of liver sparing was positively correlated with inter‐target distance.

Abstract:PURPOSE: To perform experimental as well as independent Monte Carlo (MC) evaluation of the MC algorithm implemented in RADIANCE version 4.0.8, a dedicated treatment planning system (TPS) for 3D electron dose calculations in intraoperative radiation therapy (IOERT).METHODS AND MATERIALS: The MOBETRON 2000 (IntraOp Medical Corporation, Sunnyvale, CA) IOERT accelerator was employed. PDD and profiles for five cylindrical plastic applicators with 50-90 mm diameter and 0°, 30° beveling were measured in a water phantom, at nominal energies of 6, 9 and 12 MeV. Additional PDD measurements were performed for all the energies without applicator. MC modeling of the MOBETRON was performed with the user code BEAMnrc and egs_chamber of the MC simulation toolkit EGSnrc. The generated phase space files of the two 0°-bevel applicators (50 mm, 80 mm) and three energies in both RADIANCE and BEAMnrc, were used to determine PDD and profiles in various set-ups of virtual water phantoms with air and bone inhomogeneities. 3D dose distributions were also calculated in image data sets of an anthropomorphic tissue-equivalent pelvis phantom. Image acquisitions were realized with a CT scanner (Philips Big Bore CT, Netherlands). Gamma analysis was applied to quantify the deviations of the RADIANCE calculations to the measurements and EGSnrc calculations. Gamma criteria normalized to the global maximum were investigated between 2%, 2 mm and 3%, 3 mm.RESULTS: RADIANCE MC calculations satisfied the gamma criteria of 3%, 3 mm with a tolerance limit of 85% passing rate compared to in- water phantom measurements, except for the dose profiles of the 30° beveled applicators. Mismatches lay in surface doses, in umbra regions and in the beveled end of the 30° applicators. A very good agreement to the EGSnrc calculations in heterogeneous media was observed. Deviations were more pronounced for the larger applicator diameter and higher electron energy. In 3D dose comparisons in the anthropomorphic phantom, gamma passing rates were higher than 96 % for both simulated applicators.CONCLUSIONS: RADIANCE MC algorithm agrees within 3%, 3 mm criteria with in-water phantom measurements and EGSnrc MC dose distributions in heterogeneous media for 0°-bevel applicators. The user should be aware of missing scattering components and the 30° beveled applicators should be used with attention.
