Abstract:BACKGROUND:Primary non-Hodgkin's lymphoma in lung is very rare, and the most common among them is mucosa-associated lymphoid tissue lymphoma (MALToma), whose clinical features and laboratory characteristics are poorly defined, making diagnosis difficult. The purpose of this study was to study the diagnosis and treatment of pulmonary MALToma.METHODS:The clinical data of 12 patients treated for MALToma between August 1992 and December 2005 were analyzed.RESULTS:No specific symptoms or signs, or results of bronchoscopy, ultrasonagraphy or bone marrow examination could be found in the 12 patients. Only radiography was useful in diagnosis, though the final diagnosis of all the patients was based on histology and immunohistochemistry. Two patients also had gastric MALToma. Operations were performed on 6 patients, including 5 radical operations and 1 partial resection: 4 patients also received adjuvant chemotherapy. One patient experienced recurrence 152 months after the operation, while the other 5 patients have survived disease-free. Four patients were treated with chemotherapy alone, two of whom experienced complete remission and the others partial remission. The final 2 patients received no treatment and had survived for 7 and 27 months respectively. All the patients were still alive at the most recent follow-up, 7 to 160 months (mean 71.3 months).CONCLUSIONS:Except radiography, no specific clinical manifestations could be identified for pulmonary MALToma. The final diagnosis should be based on histology and immunohistochemistry. Several treatment methods can be used to achieve good outcomes.
Solitary Pulmonary Mass in a Patient with a History of Lymphoma: a Case Report

Abstract:Introduction With the progress made in treatments, the survival rate for patients with malignant lymphoma in the last 30 years has significantly improved. However, the risk of experiencing a second primary malignancy or other disease has increased significantly. Case presentation A 44-year-old Mongolian man with a large mass in his right lower abdomen was admitted to our hospital 15 years previously. The mass was removed, and confirmed via pathological examination to be a malignant B-cell lymphoma in the appendix and distal small bowel. Post-operative chemotherapy with standard cyclophosphamide, hydroxydaunomycin, vincristine (Oncovin®) and prednisolone regimen was given for six cycles. No obvious recurrence was detected over the following 12 years. Subsequently, a mass in the right lung was found on a regular X-ray follow-up; our patient did not report chills, fever or cough. Chest computed tomography and positron emission tomography scans confirmed the mass. A primary lung carcinoma was considered to be the most likely diagnosis. However, after an exploratory thoracotomy and right upper lobectomy was performed a pathological examination of tissue samples demonstrated a lung cryptococcal granuloma, with positive staining for periodic acid Schiff and periodic acid-silver metheramine. Conclusions Compared to the normal population, second primary malignancy (in particular leukaemia and lung cancer) in patients with malignant lymphoma during their long-term survival has been seen occasionally. However, other diagnoses should also be considered such as pulmonary cryptococcosis. Other than computed-tomography-guided needle biopsy, surgery for some patients is a much more appropriate choice, which could also help attain correct diagnosis and treatment.
Malignant glomus tumor of the lung with multiorgan metastases: case report and literature review

It rarely occurs in the visceral organs where glomus body may be sparse or even absent, such as the stomach, intestines, mediastinum, and respiratory tract. It is unusual for a glomus tumor to demonstrate atypical or malignant histopathological characteristics. It is also rare for such a tumor to express clinically aggressive behavior. However, when metastasis does occur,this disease is often fatal. We herein report an interesting case of a middle-age woman admitted due to progressive cough and hemoptysis. A polypoid mass was found to occlude the left lingular lobar bronchus. Final histopathologic examination showed the presence of malignant glomus tumor, confirmed by immunoreactivity for smooth muscle actin and vimentin. Two months later, the patient developed abdominal distension and gastrointestinal bleeding. Further evaluation lead to the discovery of widespread metastatic disease to the gastrointestinal tract, spleen, and the left adrenal gland. We further entail a review of the literature on the clinicopathologic features and diagnosis of this uncommon tumor. Pulmonary Cryptococcosis Coexisting with Central Type Lung Cancer in an Immuocompetent Patient: a Case Report and Literature Review

Abstract:Pulmonary Cryptococcosis is a common fungal infection mainly caused by Cryptococcus neoformans/C.gattii species in immunocompromised patients. Cases of pulmonary cryptococcosis in patients with normal immune function are increasingly common in China. Clinical and radiographic features of pulmonary cryptococcosis are various and without obvious characteristics, so it is often misdiagnosed as pulmonary metastatic tumor or tuberculosis. When coexisting with malignant lung tumors, it was more difficult to differentiate from metastatic lung cancer, although the coexistence of pulmonary cryptococcosis and central type lung cancer is rare. Reviewing the imaging manifestations and diagnosis of the case and the relevant literature will contribute to recognition of the disease and a decrease in misdiagnoses. A 72-year-old immunocompetent Han Chinese man had repeated dry cough for more than half a year. CT examination of chest showed an irregular mass at the left hilum of the lung, and two small nodules in the right lung, which were considered as the left central lung cancer with right lung metastasis. However, the patient was diagnosed with pulmonary cryptococcosis coexisting with central type lung cancer based on the results of laboratory examination, percutaneous lung biopsy, fiberoptic bronchoscopy, and surgical pathology. The patient underwent surgical resection of the left central type lung cancer and was placed on fluconazole treatment after a positive diagnosis was made. Five years after the lung cancer surgery, the patient had a recurrence, but the pulmonary cryptococcus nodule disappeared. Our case shows that CT findings of central type lung cancer with multiple pulmonary nodules are not necessarily metastases, but may be coexisting pulmonary cryptococcosis. CT images of cryptococcosis of the lung were diverse and have no obvious characteristics, so it was very difficult to distinguish from metastatic tumors. CT-guided percutaneous lung biopsy was a simple and efficient method for identification.

Abstract:Objective To help improve the diagnosis and treatment of pulmonary mucosa-associated lymphoid tissue (MALT) lymphoma by reporting a case and reviewing relevant literature. Methods Clinical data of 1 case diagnosed with MALT lymphoma by pathology was retrospectively analyzed, and literature related to this disease was reviewed. Results An 80 year old male patient was admitted to the hospital for a pulmonary shadow, which had been diagnosed as primary bronchogenic carcinoma 5 years earlier without any treatment. The patient had a history of airway foreign body aspiration. During hospitalization, chest CT showed a pulmonary shadow larger than on a previous CT scan and signs of pleural effusion. A diagnosis of pulmonary MALT lymphoma was made through a percutaneous lung biopsy. Conclusions Primary MALT lymphoma is uncommon and pulmonary MALT lymphoma is even more rare and often shows untypical clinical presentations, which can be easily confused radiologically with those of lobar pneumonia and lung cancer. Pulmonary MALT lymphoma progresses slowly, has a high 5-year survival rate in untreated cases, and is often diagnosed based on anatomic pathology and immunohistochemistry. Lipomatous apocrine mixed tumor of the skin associated with chondroid and ossiferous stroma

Abstract:Dear Editor, Mixed tumor of the skin (chondroid syringoma) is a benign sweat gland neoplasm composed of both epithelial and mesenchymal components and is classified into two groups, apocrine and eccrine types, with the former being more common.1,2 The mesenchymal component usually consists of myxoid and/or chondroid stroma, and ossification is only rarely associated.3,4 The adipocyte component is also common in the stroma; however, an extensive adipocyte component is extremely rare.3,4 Only two such cases of extensive adipocyte component have been reported recently as lipomatous (apocrine) mixed tumor of the skin.5,6 We herein report an additional case of lipomatous apocrine mixed tumor of the skin associated with chondroid and ossiferous stroma. A 68-year-old woman presented with several years’ history of a nodular lesion on her right cheek. Physical examination showed a well-circumscribed, skincolored, firm nodule measuring 1.5–1.7 cm (Fig. 1). The nodule was surgically excised under the clinical diagnosis of epidermal cyst, and a gray-yellowish subcutaneous nodule, mimicking lipoma, was removed. The lesion was histopathologically a wellcircumscribed, subcutaneous nodule with fibrous encapsulation. Nearly half of the lesion was an epithelial component and the other half was a mesenchymal component (Fig. 2a). The epithelial component was made up of elongated tubular structures and solid aggregations (Fig. 2b). The luminal cells of the tubular structures were usually flattened or cuboidal, but were rarely columnar with apical snouts of decapitation secretions. The solid aggregations were composed mostly of polygonal cells. Characteristically, approximately 80% of the mesenchymal component consisted of numerous adipocytes (Fig. 3a), and the remaining 20% was composed of myxoid stroma associated with chondroid elements and a focus of ossification (Fig. 3b,c). Thus, nearly 40% of all the neoplastic components were composed of adipocytes. The two recently reported cases of lipomatous mixed tumor of the skin were apocrine, similar to this case,6 with one assessed as apocrine based on the presence of long tubular structures.5 These cases were composed of adipose tissue, comprising nearly 50%6 and 90%5 of all the neoplastic components, respectively. Abstract:BACKGROUND:Pulmonary nodular lymphoid hyperplasia (PNLH) is a rare benign disease lacking systematically clinical assessment. This study aims to explore the clinical features of PNLH and evaluate the safety and efficacy of different surgical approaches for PNLH.METHODS:Patients with confirmed PNLH in our department between March 2007 and August 2017 were retrospectively reviewed. The detail information of patients, including demographic characteristics, medical history, preoperative investigations, perioperative findings, postoperative conditions, and recurrence status, was collected and analyzed.RESULTS:A total of 67 patients with PNLH were diagnosed in our department, and 15 of them were suspected to have lung cancer by radiological examination before the surgery. 43 of 67 patients underwent sublobar resection and 24 patients underwent lobectomy. Compared with the lobectomy, sublobar resection showed more significant advantages in operation time (P<0.001), blood loss (P=0.001), volume of chest drainage (P=0.007), duration of chest drainage (P=0.001) and postoperative hospital stay (P=0.001).CONCLUSIONS:The diagnosis of PNLH before operation is challenging and it should be taken into consideration in the differential diagnosis of lung nodules. Compared with lobectomy, sublobar resection is an alternative approach for the treatment of PNLH.

Abstract:Pulmonary B-cell lymphoma in the extranodal marginal zone of mucosa-associated lymphoid tissue (MALT), a rare tumor originating from bronchial mucosa-associated lymphoid tissue, is the major histologic type of primary pulmonary lymphoma. Combined lung squamous cell carcinoma with pulmonary MALT lymphoma is rare. A 63-year-old male patient presented to the hospital because of a dry cough, and chest CT showed soft tissue density nodules in the upper lobe of the right lung, the boundary was visible lobulation and spiculation, and the middle lobe of the right lung showed patchy shadow, moderate enhancement, associated with bronchial traction. After a multidisciplinary diagnosis and treatment (MDT) discussion, surgical resection was done for the patient, and postoperative pathological results showed pulmonary MALT lymphoma combined with lung squamous carcinoma. For complex pulmonary multiple lesions, judgment needs to be made after MDT discussion, and timely intervention is required for lesions suspicious of malignancy. There are no uniform recommendations for the management of mixed tumors of the lung, and an individualized treatment plan needs to be developed based on the patient's actual condition.

Abstract:OBJECTIVE To compare the quantity of ankle, knee, hip, and trunk corrective actions shown during single-leg stance. DESIGN Counter-balanced crossover design. Single-leg stance under the conditions of eyes open on firm, foam, and multiaxial surfaces and eyes closed on a firm surface were recorded for 12 seconds in 18 participants. SETTING A university neuromuscular research laboratory. PARTICIPANTS Eighteen healthy and recreationally active college students. INTERVENTIONS Not applicable. MAIN OUTCOME MEASURE Average angular displacement magnitude between successive sampling instances for the ankle, knee, hip, and trunk. RESULTS A significant condition by joint interaction was revealed. Post hoc comparisons revealed that the ankle dominated as the source of corrective action across each of the testing conditions. As the challenge became greater because of foam surface or eyes closed, more corrective action occurred at proximal joints (hip and/or knee). CONCLUSIONS The ankle is of primary importance during single-leg stance on firm, foam, and multiaxial surfaces, with proximal joints having an increased role under more challenging conditions. These results provide a scientific basis for clinicians' and researchers' decisions about support surface and visual condition during single-leg postural control testing and training.
Langerhans cell histiocytosis of the rib in an adult: A case report and review of the literature

Abstract:Introduction and importance: Langerhans cell histiocytosis (LCH) is a rare neoplastic hyperplasia with an unknown etiology. It is clinically rare for patients with solitary rib lesion and pathological fracture. In this article, we report a case of LCH in solitary involvement of rib and provide a review of the available literature.Case presentation: A 24-year-old female patient complained of right chest and back pain for 10 days. CT showed a fracture in the right 6th rib. Findings on X-ray, and CT were suggestive of homogeneous osteolytic lesion of the right 6th rib. The rib tumor was then resected and the surrounding muscles and soft tissues were accordingly resected.Clinical discussion: The patient was diagnosed with pathological rib fracture, and the patient was pathologically diagnosed with LCH. After surgery, no local recurrence or distant metastasis was reported during the one-year follow-up.Conclusions: Most of the solitary tumorous lesions in rib in adults call for various differential diagnoses. Although single-site, single-system LCH of the rib is one of the rarest causes of bone tumor in adults, it can be treated successfully with surgical intervention. LCH should be considered in the diagnosis of an adult patient with a rib mass.
Clinica analysis of three cases of pulmonary langerhans cell histocytosis

Abstract:Objective To highlight the characteristics of pulmonary langerhans cell histocytosis (PLCH).Methods The clinical,radiological and pathological data of three patients with PLCH were analyzed,and relevant literature was reviewed.Results Three male smoker presented with cough,one of them with chest pain.The chest CT scan revealed multiple,bilateral nodules distributing randomly in one case and a mix of nodules and cysts in the lungs in other two cases.Two patients were diagnosed after lung biopsy and another one by bone biopsy,the pathological examination of three patients revealed a mixture of langerhans cell,eosinophils infiltration around the bronchioles and positive immunohistoehemical staining for CD1a,S100,which was consistent with PLCH.The patients showed significant clinical and radiological improvement after therapy.Two patient was stable and the other one died of severe infection because of chemotherapy.Conclusions Patients of PLCH without special clinical manifestations are diagnosed by radiographic and pathologic data.If the imaging characteristics is atypical,pathology could help us.
Clinical presentation and prognostic analysis of adult patients with Langerhans cell histiocytosis with pulmonary involvement

Abstract:Abstract Background The study aimed to investigate the clinical features and prognosis factors of adult patients with Langerhans cell histiocytosis (LCH) with pulmonary involvement, especially multisystem (MS) LCH with pulmonary involvement. Methods We retrospectively analyzed the demographic materials, clinical features and treatment outcomes of 119 adult LCH patients with pulmonary involvement at our center from January 1990 to November 2019. Results Among 119 patients, 13 (10.9%) had single-system (SS) LCH, and 106 (89.1%) had MS-LCH with pulmonary involvement. SS-LCH patients had higher smoking rate (84.6% vs 52.8%, P = 0.026) and smoking index (300 vs 200, P = 0.019) than MS-LCH patients. The percentage of respiratory symptoms of SS-LCH patients was higher than MS-LCH patients (84.6% vs 53.8%, P = 0.034). Pulmonary function was impaired in 83.8% of the patients, and DLCO was the parameter most frequently impaired, accounting for 81.1%. The median DLCO was 65.1% predicted. Patients with pneumothorax had significantly worse DLCO ( P = 0.022), FEV1 ( P = 0.000) and FEV1/FVC ( P = 0.000) than those without pneumothorax. During the follow-up, 72.4% of the patients had stable pulmonary function, and 13.8% showed improvements after chemotherapy. The estimated 3-year OS and EFS were 89.7 and 58.3%, respectively. Patients with a baseline FEV1 ≤ 55% predicted had worse OS. A history of pneumothorax indicated worse EFS and cytarabine based therapy predicted better EFS. Conclusions An FEV1 ≤ 55% predicted and a history of pneumothorax at diagnosis indicated a poor prognosis. Cytarabine based regimen may arrest the decline in pulmonary function in LCH patients with pulmonary involvement and improve EFS.

Clinical And Pathological Characteristics Of Pulmonary Mucosa-Associated Lymphoid Tissue Lymphoma: A Case Report

Abstract:Introduction: Pulmonarymucosa-associated lymphoid tissue (P-MALT) lymphomais a relative rare pulmonary disease. The purpose of our report is to characterize the symptoms, diagnosis and treatment of P-MALT lymphoma. Case report: We reported a case with a history of cough, expectoration, night sweat and multiple lung nodules on chest computer tomography. The pathological analysis of the lung biopsy through VATS demonstrated P-MALT lymphoma. The patient did not take any further treatment. During the following up after the surgery, no changes of symptoms and radiological presentation were detected. Discussions: P-MALT lymphoma is a relative rare entity and could be easily misdiagnosed. A proper diagnosis could be established by pathological examination on lung biopsy samples. The disease progresses slowly and patients can benefit from chemical and surgical therapy.

Abstract:Diffuse pulmonary lymphangiomatosis (DPL) is a rare disease that is characterized by diffuse proliferation of abnormal pulmonary lymphatic channels. DPL occurs mostly in children and young adults and often undergoes a progressive clinical course, eventually causing deterioration of the lung. Both the clinical diagnosis and treatment of DPL remain a challenge. Here, we report a case of DPL in a 53-year-old Chinese woman with comprehensive investigations including pulmonary function tests, computer tomography (CT), bronchoscopy and histological examination of the lung biopsy, and review the literature.

Abstract:BACKGROUND:Cystic airspace is an uncommon imaging manifestation involved in non-small lung cancer (NSCLC). Diffuse cystic lesion is even rarer as pulmonary manifestation of NSCLC. In the present study, we reported a rare case of NSCLC associated with progressive diffusion of cystic lesions misdiagnosed as Pulmonary langerhans cell histocytosis (PLCH), finally diagnosed by transbronchial cryobiopsy (TBCB).CASE PRESENTATION:A 52-year-old woman was admitted to our hospital due to cough and dyspnea. High-resolution computed tomography (HRCT) presented diffuse cystic shadow mostly, concomitantly with nodular densities in bilateral lungs. A lung biopsy revealed poorly differentiated adenocarcinoma with vascular tumor emboli. The epidermal growth factor receptor (EGFR) mutation on exon 18 (G719X, G719) was detected by mutation test. The patient received treatment of tyrosine kinase inhibitor (afatinib).CONCLUSIONS:Diffuse cystic lesion can be a rare manifestation of lung cancer. It was important to improve the recognition of diffuse cystic lung diseases to avoid misdiagnosis.

Abstract:Object. To investigate the clinicopathological features, imaging findings, treatment, and prognosis of primary pulmonary lymphoepitheliomatoid-like carcinoma. Methods. The clinical data of six patients with primary pulmonary lymphoepithelioma-like carcinoma treated in Zhejiang Taizhou Hospital of Taizhou Enze Medical Center (Group) from May 2014 to December 2018 were summarized and analyzed. Combined with the relevant literature, the primary pulmonary lymphoepithelioma-like carcinoma was analyzed retrospectively. Results. The main manifestations of six patients were respiratory symptoms, and cough was the most common. The imaging features of six patients were mainly round-like high-density mass shadow or nodule shadow. All patients were diagnosed by pathology. Microscopically, the cancer cells were nested, with large nuclei and vacuoles and abundant lymphocyte infiltration in the tumor stroma. The positive rates of EBER, p63, CK5/6, and Ki-67 were high, and TTF-1 was negative. Five patients received surgical treatment. One patient developed brain metastasis 12 months after operation and received craniocerebral radiotherapy. The other patients did not receive radiotherapy and chemotherapy, and one patient did not receive treatment. After follow-up, four patients survived so far, the longest survival time was 82 months, one patient lost follow-up, and one patient died of lung metastasis 24 months after operation. Conclusion. Primary pulmonary lymphoepitheliomatoid-like carcinoma is a rare lung malignant tumor, whose pathogenesis is related to Epstein-Barr virus infection. The clinical manifestations are nonspecific, but with unique pathological characteristics. Surgical resection is the proper treatment for early-stage patients, and comprehensive treatment with surgery as the main treatment is suitable for late-stage patients. The prognosis is good.

Abstract:Langerhans cell histiocytosis (LCH) is a rare histiocytic disorder of unknown etiopathogenesis. Its clinical presentation is variable and ranges from isolated skin or bone disease to a life-threatening multisystem condition. LCH can occur at any age but is more frequent in the pediatric population. The diagnosis depends on clinical, histopathological and radiographic examination and should be confirmed by immunohistochemical study with CD1a, S100 protein and langerin, three markers used widely for identifying Langerhans cells. Herein, we report an adolescent with acute myeloid leukemia (AML-M2) who was treated just with surgical management alone for LCH. As far as we know, this is the first case that the LCH patient without chemotherapy evolved into AML and was successfully cured. Cooperative studies of large numbers of LCH patients are needed to evaluate a possible association between LCH and acute leukemia, and to identify common risk factors or predisposing agents if such be present. The previously reported cases of LCH concomitant with other hematological disorders are also summarized and described compared with the present case.

Abstract:This clinical case highlights the diagnostic challenges encountered in a young adult smoker presenting with undifferentiated pulmonary nodules. Initial investigations were inconclusive, necessitating surgical lung biopsy due to the nodules' size and location. The histopathological examination revealed pulmonary Langerhans cell histiocytosis (PLCH). This emphasizes the importance of considering PLCH in the differential diagnosis of pulmonary nodules, particularly in smokers. Moreover, it underscores the value of surgical biopsy in cases where other diagnostic techniques are limited. Early recognition and accurate diagnosis are crucial for optimal management and outcomes in PLCH.
