Insomnia and sleep apnea in the entire population of US Army soldiers: Associations with deployment and combat exposure 2010-2019, a retrospective cohort investigation

Abstract:Objectives: This retrospective cohort study examined clinically diagnosed insomnia and sleep apnea and analyzed associations with deployment and combat exposure in active-duty soldiers (n=1,228,346) from 2010 to 2019. Methods: Retrospective data were obtained from the Soldier Performance, Health, and Readiness database. Results: Overseas soldier deployments peaked in 2010, decreasing thereafter as soldiers were withdrawn from Iraq and Afghanistan. From 2010 to 2012 insomnia incidence increased at a rate of 6.7 cases/1000 soldier-years, then decreased after 2012 at 5.3 cases/1000 soldier-years. Sleep apnea increased 2010-2016 at 1.9 cases/1000 soldier-years and generally declined thereafter. Risk of insomnia increased with deployment (hazard ratio=1.51; 95% confidence interval=1.49-1.52) and combat exposure (hazard ratio=1.15; 95% confidence interval=1.13-1.17). Risk of sleep apnea was increased by deployment (hazard ratio=1.89; 95% confidence interval, 1.86-1.92) and combat exposure (hazard ratio=1.09; 95% confidence interval, 1.07-1.11). Most relationships remained after accounting for other factors in multivariable analyses, except that the association between sleep apnea and combat exposure was reduced (hazard ratio=0.94; 95% confidence interval=0.92-0.97). Conclusions: Insomnia risk decreased in the period nearly in parallel with a reduction in the number of deployments; nonetheless deployment and combat exposure increased insomnia risk in the period examined. Risk of sleep apnea increased in the period and was related to deployment but not combat exposure after accounting for demographics and comorbid conditions. Despite reductions in insomnia incidence and a slowing in sleep apnea incidence, sleep disorders remain highly prevalent, warranting continued emphasis on sleep-disorder screening and improving the soldier sleep habits.

Abstract:Abstract Introduction Insomnia symptoms during inpatient rehabilitation are associated with slower recovery progression and longer lengths of stay. In addition to addressing environmental and behavioral factors, understanding medical and psychiatric comorbidities may help clinicians identify patients in most need of insomnia-related assessment, monitoring, or treatment. The present preliminary analysis aimed to characterize the comorbidity profiles of patients with insomnia symptoms and daytime sleepiness newly admitted to a VA subacute rehabilitation neighborhood. Methods 48 Veterans (Mage=72.8±12.1; 97.9%male; 79.2%white) were admitted to the neighborhood following hospital discharge during a 6-month period. Reasons for admission included short-term rehabilitation following acute illness or injury or worsening of an existing medical condition. Within 7 days of admission, Veterans completed the Insomnia Severity Index(ISI) and Epworth Sleepiness Scale(ESS). Medical and psychiatric diagnoses were obtained from electronic medical records. Comorbidity profiling was determined through frequency analyses. Results 52.1%(n=25) of Veterans endorsed at least subthreshold insomnia (ISI≥8), and 33.3% (n=16) of Veterans endorsed excessive daytime sleepiness (ESS>10). For Veterans with at least subthreshold insomnia OR excessive daytime sleepiness, the following medical comorbidities were most prevalent: circulatory disease (e.g., coronary artery disease, hypertension) (80%;93.8%) and pain disorder (e.g., spinal stenosis) (76%;75%), respectively. For Veterans with at least subthreshold insomnia OR excessive daytime sleepiness, the following psychiatric comorbidities were most prevalent: anxiety disorder (44%;68.8%), and history of substance use disorder (e.g., alcohol use disorder) (44%;43.8%), respectively. Prevalence of circulatory disease (89%) and pain disorder (90.1%) were more pronounced among Veterans with clinical insomnia (n=11)(ISI ≥ 15), though these findings were not significant. Conclusion While this work is preliminary, the findings support that it may be beneficial for clinicians to assess sleep difficulties upon admission to subacute rehabilitation settings. This may be especially important for patients with medical comorbidities, such as circulatory disease and pain disorders, and/or psychiatric comorbidities, such as anxiety disorder and substance use disorder. Future evaluations should include larger, more diverse samples and longitudinal designs to identify which comorbidities are primary risk factors for greater insomnia severity and daytime sleepiness during subacute rehabilitation. Understanding these risk factors would be essential to addressing insomnia and daytime sleepiness during rehabilitation in order to optimize recovery. Support (if any)

Abstract:Summary Military veterans have high rates of psychiatric conditions such as posttraumatic stress disorder, which can complicate the clinical management of insomnia. Population‐based data are lacking on the prevalence, characteristics and mental health burden of veterans with co‐occurring posttraumatic stress disorder and insomnia. The current cross‐sectional study analysed data from a nationally representative sample of 4069 US veterans examining the prevalence and comorbidity between posttraumatic stress disorder and insomnia, and their associations with psychiatric and medical comorbidities, suicidality, and psychosocial functioning. Results revealed that 4.0% of US veterans screened positive for posttraumatic stress disorder + insomnia, 7.4% for insomnia only, and 3.2% for posttraumatic stress disorder only. Compared with controls, higher odds of major depressive disorder and generalized anxiety disorder were observed in the posttraumatic stress disorder + insomnia and posttraumatic stress disorder only groups. Moreover, compared with the control group, posttraumatic stress disorder + insomnia and posttraumatic stress disorder only groups had higher odds of current suicidal ideation, while the posttraumatic stress disorder + insomnia group had also higher odds of attempting suicide. Relative to the posttraumatic stress disorder only group, the posttraumatic stress disorder + insomnia group scored substantially lower on measures of cognitive, emotional and social functioning (d = 1.05, 1.04 and 0.87, respectively). This study provides contemporary data regarding current prevalence, correlates, and psychiatric and functional burden of posttraumatic stress disorder + insomnia among US veterans. The results underscore the importance of assessing, monitoring and treating posttraumatic stress disorder and insomnia as part of the efforts to mitigate suicide risk and promote multi‐domain functioning in this population.

Abstract:Abstract Introduction New paramedics commencing emergency work and adjusting to shift work can experience an increase in sleep problems. Sleep disturbances have been closely associated with poor mental health in emergency personnel. It is unknown, however, whether poor sleep experienced in the initial months of emergency work represents an early risk factor for future mental health problems in paramedics. The present study examined whether sleep disturbances in paramedics after 6-months of shift and emergency work were associated with mental health symptoms later in their career. Methods Sleep disorder and mental health symptoms were examined in new recruit paramedics (n=105) after their first 6- and 12-months of shift and emergency work. At each timepoint, participants completed validated self-report measures of insomnia (Insomnia Severity Index), shift work disorder (SWD; SWD Screening Questionnaire), obstructive sleep apnoea (OSA; Berlin Questionnaire), depression (Patient Health Questionnaire-9), anxiety (Generalised Anxiety Disorder-7), post-traumatic stress disorder (PTSD; PTSD Checklist-5) and trauma exposure (Life Events Checklist-5). Linear regressions examined whether sleep at 6-months (i.e., insomnia, SWD, and OSA) predicted mental health symptoms (i.e., depression, anxiety, and PTSD) at the 12-month follow-up, while controlling for demographics (i.e., age and sex) and mental health symptoms at the 6-month timepoint. Additionally, the regression models examining sleep and PTSD also controlled for trauma exposure at 6-months via the Life Events Checklist-5. Results Increased insomnia symptoms at 6-months of emergency work and shift work significantly predicted higher PTSD symptoms at the 12-month timepoint (b=0.09 [0.03,0.15], p=0.01) while controlling for trauma exposure, PTSD symptoms and demographics at 6-months. Insomnia at 6-months was not, however, related to depression (p=0.36) or anxiety (p=0.42) at follow-up, and OSA and SWD risk at 6-months were not related to any of the mental health outcomes at follow-up (all p>0.05). Conclusion Our findings highlight insomnia symptoms in the initial months of emergency work as an early risk factor for the development of PTSD later in new paramedics’ careers. Given sleep is a largely modifiable factor, this finding highlights the need to investigate interventions that target insomnia early in paramedics’ careers to reduce the risk of future mental health problems in this high-risk occupation. Changes in insomnia severity with advanced PAP therapy in patients with posttraumatic stress symptoms and comorbid sleep apnea: a retrospective, nonrandomized controlled study

Abstract:Background: Sleep disorders frequently occur in posttraumatic stress disorder (PTSD) patients. Chronic insomnia is a common feature of and criteria for the diagnosis of PTSD. Another sleep disorder, obstructive sleep apnea (OSA), also occurs frequently in PTSD, and emerging research indicates OSA fuels chronic insomnia. Scant research has investigated the impact of OSA treatment on insomnia outcomes (Insomnia Severity Index, ISI) in trauma survivors. Methods: OSA patients with moderately severe posttraumatic stress symptoms were studied in a retrospective chart review. Ninety-six patients who failed CPAP therapy due to expiratory pressure intolerance or complex sleep apnea or both underwent manual titration with advanced PAP modes [autobilevel (ABPAP); adaptive servo-ventilation (ASV)], which were subsequently prescribed. PAP use measured by objective data downloads divided the sample into three groups: compliant regular users (C-RU): n = 68; subthreshold users (SC-RU): n = 12; and noncompliant users (NC-MU): n = 16. The average follow-up was 11.89 ± 12.22 months. Baseline and posttreatment ISI scores were analyzed to assess residual insomnia symptoms as well as cure rates. Results: The C-RU group showed significant improvements in insomnia with very large effects compared to those in the NC-MU reference group (P = 0.019). Insomnia severity significantly decreased in all three groups with large effects (C-RU, P = 0.001; SC-RU, P = 0.027; NC-MU, P = 0.007). Hours of weekly PAP use and insomnia severity were inversely correlated (P = 0.001, r = - 0.321). However, residual insomnia symptoms based on established ISI cut-offs were quite common, even among the C-RU group. Post hoc analysis showed that several categories of sedating medications reported at baseline (hypnotics, anti-epileptic, opiates) as well as actual use of any sedating medication (prescription or nonprescription) were associated with smaller insomnia improvements than those in patients not using any sedating agents. Conclusions: In a retrospective, nonrandomized analysis of a select sample of sleep clinic patients with OSA and PTSD symptoms, advanced PAP therapy was associated with significant improvement in insomnia severity for both compliant and partial users. However, residual insomnia symptoms persisted, indicating that PAP therapy provides only limited treatment. RCTs are warranted to assess the effect of ABPAP and ASV modes of therapy on adherence and sleep outcomes, and their potential impact on posttraumatic stress symptoms. Treatment arms that combine PAP with CBT-I would be expected to yield the greatest potency.
1087 Recognizing Sleep Disorders in the US Military Health System: Differences Between On-base and Private Sector Care

Abstract:Abstract Introduction Sleep disorders such as insomnia and obstructive sleep apnea (OSA) are common and costly in the US Military Health System (MHS). Further, demand for sleep specialty care greatly exceeds available supply. Many MHS beneficiaries are referred off-base to the TRICARE network to access private sector care. The purpose of this study was to compare military, demographic, and clinical characteristics between MHS beneficiaries newly diagnosed with either insomnia or OSA on-base (direct care) and off-base (private sector care). Methods We identified MHS beneficiaries with a first diagnosis of insomnia or OSA between years 2016-2021 within the Military Data Repository (MDR). This large data repository includes encounter, procedure, medication, and durable medical equipment information for Active Duty personnel, dependents, National Guard, and Reserves. Demographic and military information was obtained from the MDR. Sleep disorders and medical and psychiatric comorbidities were defined based on ICD-10 diagnostic codes. Determination of direct vs private sector care was based on an indicator in the MDR. Results 235,823 MHS beneficiaries were diagnosed with either insomnia (n=143,877) or OSA (n=91,946). Of beneficiaries with insomnia, the majority (n=119,923, 83.4%) were diagnosed in direct care. Of beneficiaries with OSA, the majority (n=51,014, 55.5%) were diagnosed in private sector care. Between-groups differences (direct vs private sector care) were observed in service branch (standard mean difference [SMD]=0.46) and beneficiary category (i.e., Active Duty, dependent, National Guard, or Reserve; SMD=0.37), with most Active Duty (n=131,321, 81.6%) and dependent (n=26,987, 53.5%) beneficiaries being treated via direct care. No between-groups differences were observed in medical and psychiatric comorbidities. In terms of comorbid sleep disorders, of beneficiaries diagnosed with hypersomnia (n=12,403), the majority (n=10,055, 81.1%) were diagnosed in private sector care. Conclusion Differences in service branch and beneficiary category were observed between individuals diagnosed with insomnia or OSA in direct and private sector care. Insomnia was more likely to be diagnosed in direct care, and OSA was more likely to be diagnosed in private sector care. Future research should examine health and economic outcomes associated with direct and private sector sleep care and identify opportunities (e.g., telehealth) to bring sleep care back into the MHS. Abstract:Abstract Introduction Suicide represents a public health emergency among veterans, who die by suicide at alarming rates and show elevated sleep disturbances. Selective interventions for military suicide indication are scarce, and available treatments remains mismatched to the acute nature of a suicidal crisis. We sought to develop and test preliminary efficacy of a fast-acting, behavioral insomnia intervention for antisuicidal response in the context of a sham-controlled, randomized trial among veterans. Methods A multicomponent behavioral sleep intervention was developed, integrating: CBT for Insomnia (CBTi) and Imagery Rehearsal Treatment (IRT) within a rapid (4-session) format (SERVE: Sleep Enhancement for Returning Veterans). This was compared to active control, Arousal-Based Treatment of Insomnia (ABTI). Treatments were manualized and matched for therapist contact, materials (manuals, powerpoints, guidesheets), and passage of time. Raters were blind to treatment assignment. Inclusion criteria: (a) age>18, (b) clinically-significant sleep disturbance (Insomnia Severity Index (ISI>10)), (c) DSM-V-Defined Depression, or (d) current suicidal ideation (Columbia Suicidal Severity Rating Scale (CSSRS>1)). A comprehensive data and safety monitoring plan supported standardized risk assessment, safety planning, and triage. Outcomes were assessed at Baseline and Posttreatment (1,3 mos) according to primary (suicidal ideation), secondary (sleep indices), and exploratory (mood, stress indices) outcomes. Results Recruitment occurred by way of comprehensive military partnerships, clinic referrals, and community flyering. Of n=753 new contacts, n=436 participants were screened for inclusion. Of these, n=112 were interviewed by full-battery eligibility assessment, resulting in n=77 veterans enrolled and randomized to: Active Treatment (n=39) vs. Active Control (n=38). Feasibility analyses supported high rates of acceptance, tolerability, and safety. For the full sample, t-tests revealed large posttreatment reductions in suicidal ideation (CSSRS, p<.001; Depressive Symptom Inventory Suicidality Subscale (DSISS)), p<.001), sleep, mood, and stress outcomes (ISI, p<.01; DDNSI, p<.01; QIDS-SR, p<.01; PCL, p<.001). Effects were significant compared to control (DSISS, p<.01; ISI, p<.01). Conclusion A brief, non-pharmacological insomnia intervention resulted in antisuicidal response and large posttreatment improvements in sleep and well-being. This is the first known report testing non-medication insomnia treatment within a sham-controlled, randomized trial for military suicidal behaviors, where results support feasibility, safety, and therapeutic impact to suicidal risk. Insomnia severity predicts depression, anxiety, and posttraumatic stress disorder in veterans with spinal cord injury or disease: a cross-sectional observational study

Abstract:Study objectives: To assess the association of insomnia symptoms and psychiatric symptoms in patients with spinal cord injury or disease (SCI/D). Methods: In this cross-sectional observational study, veterans with SCI/D (n = 72; mean = 59.85 ± 10.4 years; 92% male) completed baseline measures, including the Insomnia Severity Index (ISI) during the baseline phase of a clinical trial on treatment of sleep disorders in veterans with SCI/D. Depression severity was measured by the Patient Health Questionnaire (PHQ-9; sleep items excluded), anxiety severity was measured by the Generalized Anxiety Disorder screener (GAD-7), and probable posttraumatic stress disorder (PTSD) was measured by the Primary Care PTSD screener. Blocked regression was used to evaluate the impact of insomnia symptoms (ISI) on mental health measures after accounting for demographics and level of spinal cord injury/disease. Results: On average, participants scored in the mild range for depression (PHQ-9 = 7.4 ± 5.9) and anxiety severity (GAD-7 = 6.1 ± 6.1). In total, 36.1% (n = 26) screened positive for probable PTSD. ISI explained 19% of the variance in PHQ-9 and 20% of the variance in GAD-7 (P < .001) over and above demographics and SCI/D level of injury/disease. Odds of probable PTSD were increased 1.22-fold for each 1 unit increase in ISI (P = .001) after accounting for demographics and level of injury/disease. Conclusions: In veterans with SCI/D, insomnia severity was linked to depression and anxiety symptom severity and risk of PTSD. Study results warrant further research to evaluate the impact of insomnia treatment on depression, anxiety, and PTSD in patients with SCI/D. Abstract:Abstract Introduction Disordered sleep, including insomnia and obstructive sleep apnea, is common among post-9/11 veterans returning from Southwest Asia and Afghanistan. Environmental exposures, such as elevated respirable particulate matter, may contribute to the development of sleep apnea. Additional occupational exposures sustained during military service may contribute to poor sleep, for example hyperarousal and resultant insomnia. We sought to describe sleep quality and prevalence of sleep disorders among a veteran population exposed to airborne hazards with unexplained dyspnea. Methods Veterans from across five sites of the Post-Deployment Cardiopulmonary Evaluation Network (PDCEN) completed, as part of a more comprehensive evaluation, questionnaires inclusive of demographic information and the Pittsburgh Sleep Quality Index (PSQI). Limited polysomnography data was also available through chart review at the Baltimore site. Descriptive statistical analysis was performed, with means and standard deviations or medians and interquartile ranges reported for all questionnaire and PSG outcomes, as appropriate. Results In total, 168 veterans completed the PSQI and demographic questionnaires. The majority of respondents were aged 45±9 years, male (82%), and white (73%). Approximately half had served in the Army branch of the military. BMI was available for only 127 veterans, with mean 31.9±5.9 kg/m2. The group overall reported prolonged sleep onset latency (56±51 minutes) and reduced total sleep time (5±1 hours). Overall PSQI scores were elevated at 13±5. In Baltimore (N=38), PSQI data was similar. Sleep study data was available for 30/38 veterans, of whom 27 had a diagnosis of obstructive sleep apnea (OSA). Among those with OSA, the median AHI was 13.5 (IQR 14.5) events/hour and the mean BMI was 31.0±5.2 kg/m2. Conclusion Among a cohort of post-9/11 veterans with exposure to airborne hazards, insufficient and disturbed sleep was common. In a limited subset with sleep study data, the majority had at least mild OSA. Future work will evaluate the presence of comorbid medical and mental health conditions. This early descriptive data is consistent with prior reports among similar veteran populations, and highlights the need for further research to elucidate causes of sleep disturbances and possible relation to exposure to airborne hazards. Abstract:The identification of overexpressed miRNAs in multiple myeloma (MM) has progressively added a further level of complexity to MM biology. miRNA and gene expression profiles of two large representative MM datasets, available from retrospective and prospective series and encompassing a total of 249 patients at diagnosis, were analyzed by means of in silico integrative genomics methods, based on MAGIA2 and Micrographite computational procedures. We first identified relevant miRNA/transcription factors/target gene regulation circuits in the disease and linked them to biological processes. Members of the miR-99b/let-7e/miR-125a cluster, or of its paralog, upregulated in t(4;14), were connected with the specific transcription factors PBX1 and CEBPA and several target genes. These results were validated in two additional independent plasma cell tumor datasets. Then, we reconstructed a non-redundant miRNA-gene regulatory network in MM, linking miRNAs, such as let-7g, miR-19a, mirR-20a, mir-21, miR-29 family, miR-34 family, miR-125b, miR-155, miR-221 to pathways associated with MM subtypes, in particular the ErbB, the Hippo, and the Acute myeloid leukemia associated pathways.
Postsurgical opioid prescribing among veterans using community care for orthopedic surgery at non-VA hospitals compared to a VA hospital with a transitional pain service: a retrospective cohort study

Abstract:Background The USA provides medical services to its military veterans through Veterans Health Administration (VHA) medical centers. Passage of recent legislation has increased the number of veterans having VHA-paid orthopedic surgery at non-VHA facilities. Methods We conducted a retrospective cohort study among veterans who underwent orthopedic joint surgery paid for by the VHA either at the Salt Lake City VHA Medical Center (VAMC) or at non-VHA hospitals between January 2018 and December 2021. 562 patients were included in the study, of which 323 used a non-VHA hospital and 239 patients the VAMC. The number of opioid tablets prescribed at discharge, the total number prescribed by postdischarge day 90, and the number of patients still filling opioid prescriptions between 90 and 120 days after surgery were compared between groups. Results Veterans who underwent orthopedic surgery at a non-VHA hospital were prescribed more opioid tablets at discharge (median (IQR)); (40 (30–60) non-VHA vs 30 (20–47.5) VAMC, p<0.001) and in the first 90 days after surgery than patients who had surgery at the Salt Lake City VAMC (60 (40–120) vs 35 (20–60), p<0.001). Patients who had surgery at Salt Lake City VAMC were also significantly less likely to fill opioid prescriptions past 90 days after hospital discharge (OR (95% CI) 0.06 (0.01 to 0.48), p=0.007). Conclusion These results suggest that veterans who have surgery at a veterans affairs hospital with a transitional pain service are at lower risk for larger opioid prescriptions both at discharge and within 90 days after surgery as well as persistent opioid use beyond 90 days after discharge than if they have surgery at a community hospital.

Abstract:Abstract Introduction Safety behaviors may perpetuate anxiety, maintain an individual’s perception of threats, and may lead to psychosocial impairment. Safety behaviors are commonly seen in those suffering from insomnia and post-traumatic stress disorder (PTSD). Despite their prevalence and associated impairments, there exists no data on the relationship between insomnia and safety behavior in those suffering from PTSD. The aim of this study is to evaluate the relationship between safety behavior and insomnia in those with PTSD. If such a relationship is seen, does the severity of nightmares or overall PTSD mediate this relationship between insomnia and safety behaviors? Methods The study extracted data from baseline assessments of a treatment study conducted at the VA Medical Center at Philadelphia. The subjects were treatment-seeking patients with PTSD (N =43). Safety behavior was evaluated using the 12-item brief Safety Behavior Assessment Form (SBAF), PTSD was evaluated using the PTSD checklist for DSM-5 (PCL-5), and insomnia with Insomnia Severity Index (ISI) scales. Bivariate analyses evaluated the relationship between SBAF total score and age, education, income, PCL-5, and ISI total scores. Nightmare severity was evaluated using item number 2 from the PCL-5 questionnaire. An analysis evaluated the role of the severity of nightmares or PTSD (PCL-5 total score after excluding the sleep and safety items) as independent mediators in the relationship between SBAF and ISI total scores. Results The mean±standard deviation of their age was 56.1±11.6 years and 13.1±2.1 years of education. They were mostly males (86.0%), black (61.9%) and unemployed over the past month (76.8%). The mean total score was 17.1±5.7 on the SBAF, 17.3±7.4 on the ISI, and 52.5±14.5 on the PCL-5. Bivariate analysis demonstrated that SBAF total score was associated with the ISI (β=0.32, p=0.0005 and PCL-5 (β=0.22, p=0.0004) total scores. Mediation analysis demonstrated a significant mediation effect of nightmares (R2=24.6%) and PTSD severity (R2=33.3%) on the association between safety behaviors and insomnia. Conclusion About sixty percent of the variance in safety behaviors was explained by insomnia and its association with PTSD or nightmare severity. Abstract:Abstract Introduction Insomnia and Sleep Apnea (SA), the two most prevalent sleep disorders, can occur separately or concurrently as comorbid insomnia and sleep apnea (COMISA). This study utilized a large national Veterans Health Administration (VA) database to compare all-cause mortality among them. Methods We constructed a cohort of veterans who used any sleep services in the VA from 10/1999 to 10/2023. Using ICD 9/10 codes and a VA-validated algorithm, we identified veterans with confirmed diagnoses of insomnia and/or SA. For insomnia, we required the presence of relevant medications or CBT-I. We extracted mortality data for three groups: insomnia-only, SA-only, and COMISA. We estimated the association of these conditions using multivariable logistic regression, adjusting for age, sex, race, ethnicity, Charlson Comorbidity Index (CCI), and exposure time (time from the initial insomnia or SA diagnosis). Results Out of 4,722,693 veterans who used VA sleep services, we identified 2,159,412 participants with a confirmed diagnosis of either SA or insomnia, who had ≥2 specialty visits in the past three years. We observed 518,574 (24%) with insomnia-only (mean age 58.5, 77.2% male, 71.4% white, 22.6% with CCI > 2), 1,343,035 (63%) with SA-only (mean age 57.6, 93.5% male, 70.1% white, 25% with CCI > 2), and 297,803 (13.8%) with COMISA (mean age 52.5, 89.2% male, 68.3% white, 18.5% with CCI > 2). All-cause mortality rates were 33.2%, 23.3%, and 16.2% for insomnia-only, SA-only, and COMISA, respectively. Compared to the SA-only group as a reference, the insomnia-only group was associated with a 73% increased risk (adjusted Odds Ratio [aOR] 1.73; 95% Confidence Interval [CI]: 1.72, 1.75) and the COMISA group with a 12% decreased risk of mortality (aOR 0.88; 95%CI: 0.87, 0.89). Conclusion In a cohort referred to VA sleep centers, SA, insomnia, and COMISA were prevalent. Mortality was highest in the insomnia-only group, compared to the SA-only and COMISA groups. Further studies are needed to explore the causes of this increased mortality. Abstract:Abstract Introduction Disturbed sleep is a modifiable risk factor for a variety of co-morbidities. Treatment of sleep disorders can provide a key target for improving cardiometabolic health and overall wellness. Despite evidence suggesting higher prevalence of disturbed sleep in Veterans with spinal cord injury (SCI) compared to the non-injured population, the intersectional effects of sleep-wake disruption remain undefined. The purpose of this study is to examine existing data from the spinal cord injury-pressure injury resource (SCI-PIR) database to assess prevalence and identify relationships among sleep disorders and cardiometabolic risk after spinal cord injury. Methods This retrospective longitudinal national database cohort study utilized the VA’s SCI-PIR database containing comprehensive clinical data for 29,000 individual Veterans with SCI with over 120,000 encounters. SCI-PIR data includes clinical characteristics of Veterans with SCI, demographics, and comorbidities identified as being relevant to sleep and cardiometabolic risk. We queried the database for ICD9 diagnosis codes related to cardiovascular, metabolic, psychologic, and sleep conditions to identify sleep disorder subgroups defined by clustering of cardiometabolic risk factors and sleep diagnoses within SCI individuals diagnosed with sleep disorders. Results In the sample of 18,894 Veterans, sleep disturbances were diagnosed in only 16.25% of the sample. Sleep apnea (6.7%) and insomnia (4.3%) were the most common sleep diagnosis. Correspondence analysis of sleep, cardiac disorders, and other co-morbidities demonstrated two clusters. The first cluster showed a robust link between sleep apnea, hypersomnia, heart failure and arrhythmias, and secondary associations with coronary artery disease, chronic kidney disease, obesity, diabetes, and hyperlipidemia. The second cluster showed a strong relationship between insomnia, anxiety, and post-traumatic stress disorder. Conclusion Sleep disturbances are common in Veterans with SCI, but underdiagnosed. Our findings support the link between medical complications and consequences of disturbed sleep and cardiometabolic risk. This will enable development of SCI validated sleep instruments and clinical decision tools that identify those at highest risk for sleep disorders lead to the development of targeted interventions and treatments to advance precision health for Veterans with SCI. Abstract:We previously described five trajectories of insomnia (each defined by a distinct pattern of insomnia severity over 12 months following traumatic brain injury [TBI]). Our objective in the present study was to estimate the association between insomnia trajectory status and trajectories of mental health and neurocognitive outcomes during the 12 months after TBI. In this study, participants included N = 2022 adults from the Federal Inter-agency Traumatic Brain Injury Repository database and Transforming Research and Clinical Knowledge in TBI (TRACK-TBI) study. The following outcome measures were assessed serially at 2 weeks, and 3, 6, and 12 months post-injury: Insomnia Severity Index, Patient Health Questionnaire, Post-Traumatic Stress Disorder (PTSD) Checklist for Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5), Patient Reported Outcomes Measurement Information System-Pain, and Quality of Life After Brain Injury-Overall Scale. Neurocognitive performance was assessed at 2 weeks, and 6 and 12 months using the Wechsler Adult Intelligence Scales Processing Speed Index and the Trails Making Test Parts A and B. Results indicated that greater insomnia severity was associated with greater abnormality in mental health, quality of life, and neuropsychological testing outcomes. The pattern of insomnia over time tracked the temporal pattern of all these outcomes for all but a very small number of participants. Notably, severe insomnia at 3 or 6 months post-TBI was a risk factor for poor recovery at 12 months post-injury. In conclusion, in this well-characterized sample of individuals with TBI, insomnia severity generally tracked severity of depression, pain, PTSD, quality of life, and neurocognitive outcomes over 12 months post-injury. More intensive sleep assessment is needed to elucidate the nature of these relationships and to help inform best strategies for intervention.
Impact of complementary health approaches on opioid prescriptions among veterans with musculoskeletal disorders - A retrospective cohort study

Abstract:To examine whether complementary and integrative health approaches mitigate opioid prescriptions for pain and whether the relationship differs by post-dramatic stress disorder (PTSD) diagnosis, we followed 1,993,455 Veterans with musculoskeletal disorders during 2005-2017 using Veterans Healthcare Administration electronic health records. Complementary and integrative health (CIH) approaches were defined as ≥ 1 primary care visits for meditation, Yoga, and acupuncture etc using natural language processing. Opioid prescriptions were ascertained from pharmacy dispensing records. A propensity score was estimated and used to match one control Veteran to each CIH recipient. Over the 2-year follow-up period after the index diagnosis, 140,902 (7.1 %) Veterans received ≥ 1 modalities. Among the matched analytic sample (272,296 Veterans), the likelihood of dispensing opioid prescriptions was significantly lower for Veterans in the CIH group than their controls [adjusted hazard ratio (aHR), 0.45 (95 % Confidence Intervals (CI): 0.44-0.46)]. The association did not differ between Veterans with [aHR: 0.46 (95 % CI: 0.45-0.47)] and without [aHR: 0.44 (95 % CI: 0.43-0.45)] PTSD. In sensitivity analyses, the exposure group had 3.82 (95 % CI: 3.76-3.87) months longer restricted mean survival time to opioid initiation, 2 % (95 % CI: 4 %-1 %) lower morphine equivalent and 17 % lower total days' supply (95 % CI: 18 %-16 %). The relationship remains significant but was attenuated after eliminating waiting time for the exposure group (aHR, 0.63 (95 % CI: 0.62-0.64)). These observations suggest that CIH approaches may help reduce opioid prescriptions for Veterans with musculoskeletal disorders and related pain. The impact of the timing of receiving such approaches warrants further investigation. PERSPECTIVE: This article presents a quasi-experimental investigation into potential benefit of complementary and integrative health approaches (CIH) on de-prescribing opioids. The findings may potentially help clinicians who are seeking non-pharmacological alternative options to manage patient pain and opioid dependence".
Toward personalized care for insomnia in the US Army: development of a machine-learning model to predict response to pharmacotherapy

Abstract:Study objectives: Although many military personnel with insomnia are treated with prescription medication, little reliable guidance exists to identify patients most likely to respond. As a first step toward personalized care for insomnia, we present results of a machine-learning model to predict response to insomnia medication. Methods: The sample comprised n = 4,738 nondeployed US Army soldiers treated with insomnia medication and followed 6-12 weeks after initiating treatment. All patients had moderate-severe baseline scores on the Insomnia Severity Index (ISI) and completed 1 or more follow-up ISIs 6-12 weeks after baseline. An ensemble machine-learning model was developed in a 70% training sample to predict clinically significant ISI improvement, defined as reduction of at least 2 standard deviations on the baseline ISI distribution. Predictors included a wide range of military administrative and baseline clinical variables. Model accuracy was evaluated in the remaining 30% test sample. Results: 21.3% of patients had clinically significant ISI improvement. Model test sample area under the receiver operating characteristic curve (standard error) was 0.63 (0.02). Among the 30% of patients with the highest predicted probabilities of improvement, 32.5.% had clinically significant symptom improvement vs 16.6% in the 70% sample predicted to be least likely to improve (χ21 = 37.1, P < .001). More than 75% of prediction accuracy was due to 10 variables, the most important of which was baseline insomnia severity. Conclusions: Pending replication, the model could be used as part of a patient-centered decision-making process for insomnia treatment, but parallel models will be needed for alternative treatments before such a system is of optimal value. Abstract:The military lifestyle often includes continuous operations whether in training or deployed environments. These stressful environments present unique challenges for service members attempting to achieve consolidated, restorative sleep. The significant mental and physical derangements caused by degraded metabolic, cardiovascular, skeletomuscular, and cognitive health often result from insufficient sleep and/or circadian misalignment. Insufficient sleep and resulting fatigue compromises personal safety, mission success, and even national security. In the long-term, chronic insufficient sleep and circadian rhythm disorders have been associated with other sleep disorders (e.g., insomnia, obstructive sleep apnea, and parasomnias). Other physiologic and psychologic diagnoses such as post-traumatic stress disorder, cardiovascular disease, and dementia have also been associated with chronic, insufficient sleep. Increased co-morbidity and mortality are compounded by traumatic brain injury resulting from blunt trauma, blast exposure, and highly physically demanding tasks under load. We present the current state of science in human and animal models specific to service members during- and post-military career. We focus on mission requirements of night shift work, sustained operations, and rapid re-entrainment to time zones. We then propose targeted pharmacological and non-pharmacological countermeasures to optimize performance that are mission- and symptom-specific. We recognize a critical gap in research involving service members, but provide tailored interventions for military health care providers based on the large body of research in health care and public service workers.
