Abstract:Global use of the internet has become commonplace, and smart phones have paved the way for technological mobility. Incorporation of smart phone technology has the potential to positively affect health outcomes through use of health-directed applications (apps), particularly for those patients living in medically underserved areas. The Bridge Mobile App for Burn Patients (fka: HealthySteps), is a pilot project that was developed to address the unique recovery needs of patients with major burn injuries who are being discharged from a regional burn center. App content was developed from three focus groups to explore and elucidate on stakeholders' understandings of the bio-psycho-social education and messages that they believed would improve short-term outcomes for newly discharged burn patients. The app will provide burn patients with accessible support 24h a day, seven days a week. Original recovery-stage appropriate bio-psycho-social content, instructional videos and links to burn-supportive web sites are delivered directly to patients' smart phones for the first 90days following discharge. The primary goal for the Bridge App is to decrease unplanned hospital re-admissions, while supporting increased quality of life and resilience in short-term recovery. In addition, the Bridge Mobile App is designed to collect patient data reflecting pain, anxiety, mood, itching, medication compliance, social participation, self-efficacy and return to work on a password protected, HIPPA compliant, encrypted mainframe.
Development of an Artificial Intelligence–Based Tailored Mobile Intervention for Nurse Burnout: Single-Arm Trial

Abstract:Background: Nurse burnout leads to an increase in turnover, which is a serious problem in the health care system. Although there is ample evidence of nurse burnout, interventions developed in previous studies were general and did not consider specific burnout dimensions and individual characteristics. Objective: The objectives of this study were to develop and optimize the first tailored mobile intervention for nurse burnout, which recommends programs based on artificial intelligence (AI) algorithms, and to test its usability, effectiveness, and satisfaction. Methods: In this study, an AI-based mobile intervention, Nurse Healing Space, was developed to provide tailored programs for nurse burnout. The 4-week program included mindfulness meditation, laughter therapy, storytelling, reflective writing, and acceptance and commitment therapy. The AI algorithm recommended one of these programs to participants by calculating similarity through a pretest consisting of participants' demographics, research variables, and burnout dimension scores measured with the Copenhagen Burnout Inventory. After completing a 4-week program, burnout, job stress, stress response using the Stress Response Inventory Modified Form, the usability of the app, coping strategy by the coping strategy indicator, and program satisfaction (1: very dissatisfied; 5: very satisfied) were measured. The AI recognized the recommended program as effective if the user's burnout score reduced after the 2-week program and updated the algorithm accordingly. After a pilot test (n=10), AI optimization was performed (n=300). A paired 2-tailed t test, ANOVA, and the Spearman correlation were used to test the effect of the intervention and algorithm optimization. Results: Nurse Healing Space was implemented as a mobile app equipped with a system that recommended 1 program out of 4 based on similarity between users through AI. The AI algorithm worked well in matching the program recommended to participants who were most similar using valid data. Users were satisfied with the convenience and visual quality but were dissatisfied with the absence of notifications and inability to customize the program. The overall usability score of the app was 3.4 out of 5 points. Nurses' burnout scores decreased significantly after the completion of the first 2-week program ( t =7.012; P <.001) and reduced further after the second 2-week program ( t =2.811; P =.01). After completing the Nurse Healing Space program, job stress ( t =6.765; P <.001) and stress responses ( t =5.864; P <.001) decreased significantly. During the second 2-week program, the burnout level reduced in the order of participation ( r =–0.138; P =.04). User satisfaction increased for both the first ( F =3.493; P =.03) and second programs ( F =3.911; P =.02). Conclusions: This program effectively reduced burnout, job stress, and stress responses. Nurse managers were able to prevent nurses from resigning and maintain the quality of medical services using this AI-based program to provide tailored interventions for nurse burnout. Thus, this app could improve qualitative health care, increase employee satisfaction, reduce costs, and ultimately improve the efficiency of the health care system.

A Web-Based Mobile App (INTERACCT App) for Adolescents Undergoing Cancer and Hematopoietic Stem Cell Transplantation Aftercare to Improve the Quality of Medical Information for Clinicians: Observational Study

Abstract:Background: A growing number of cancer and hematopoietic stem cell transplant (HSCT) survivors require long-term follow-up with optimal communication schemes, and patients' compliance is crucial. Adolescents have various unmet needs. Regarding self-report of symptoms and health status, users of mobile apps showed enhanced compliance. Currently, HSCT aftercare at the HSCT outpatient clinic of the St. Anna Children's Hospital in Vienna, Austria, is based on handwritten diaries, carrying various disadvantages. Recently, we developed the prototype of a web-based, self-monitoring gamified mobile app tailored for adolescents: the INTERACCT (Integrating Entertainment and Reaction Assessment into Child Cancer Therapy) app. Objective: This observational, prospective study evaluated the usability of the INTERACCT app for tracking real-time self-reported symptoms and health status data in adolescent HSCT patients and a healthy matched control group. The primary outcome of the study was the quality of the self-reported medical information. We hypothesized that the mobile app would provide superior medical information for the clinicians than would the handwritten diaries. Methods: Health data were reported via paper diary and mobile app for 5 consecutive days each. The quality of medical information was rated on a 5-point scale independently and blinded by two HSCT clinicians, and the duration of use was evaluated. A total of 52 participant questionnaires were assessed for gaming patterns and device preferences, self-efficacy, users' satisfaction, acceptability, and suggestions for improvement of the mobile app. Interrater reliability was calculated with the intraclass correlation coefficient, based on a two-way mixed model; one-way repeated-measures analysis of variance and t tests were conducted post hoc. Descriptive methods were used for correlation with participants' demographics. For users' satisfaction and acceptability of the mobile app, the median and the IQR were calculated. Results: Data from 42 participants-15 patients and 27 healthy students-with comparable demographics were evaluated. The results of our study indicated a superiority of the quality of self-reported medical data in the INTERACCT app over traditional paper-and-pencil assessment (mobile app: 4.14 points, vs paper-based diary: 3.77 points, P=.02). The mobile app outperformed paper-and-pencil assessments mainly among the patients, in particular among patients with treatment-associated complications (mobile app: 4.43 points, vs paper-based diary: 3.73 points, P=.01). The mobile app was used significantly longer by adolescents (≥14 years: 4.57 days, vs ≤13 years: 3.14 days, P=.03) and females (4.76 days for females vs 2.95 days for males, P=.004). This corresponds with a longer duration of use among impaired patients with comorbidities. User satisfaction and acceptability ratings for the mobile app were high across all groups, but adherence to entering a large amount of data decreased over time. Based on our results, we developed a case vignette of the target group. Conclusions: Our study was the first to show that the quality of patient-reported medical information submitted via the INTERACCT app embedded in a serious game is superior to that submitted via a handwritten diary. In light of these results, a refinement of the mobile app supported by a machine learning approach is planned within an international research project.
A Process Evaluation of a Mobile App for Medical Students Aimed at Increasing Resilience and Decreasing Stigma in Mental Health

Abstract:Objective: The purpose of this pilot study was to conduct a process evaluation of a mental health and wellness mobile health (mHealth) application for medical students designed to increase resilience and decrease mental health stigma. Methods: The customized application, MindfulMEDS, was developed with peer-focused interactive modules specific to medical students within an existing system called Sharpen®. The Sharpen® system contains an extensive library of didactic and experiential mental health and wellness content built specifically to promote evidence-based protective factors for resilience. A mixed-methods approach including surveys and focus groups assessed participant resiliency, perception of mental health within the context of medical school, and evaluation of the app. Assessments were conducted at baseline (n = 66), six months (n = 30), and one year (n = 24). Demographic information was collected once at baseline as a part of the initial survey. Results: A total of 215 users were registered in MindfulMEDS, consumed 83 courses, and engaged in 1,428 "connect clicks" to community resources and crisis-response supports. Resilience levels did not change significantly between surveys; however, a significant decrease in the perception of mental health stigma associated with utilizing mental health resources was observed. Focus group participants (n = 11) reported the screening tools to be useful, encouraged expansion, and suggested additional reminders to access the app to increase engagement. Conclusion: Findings of this pilot study demonstrate the feasibility of implementing MindfulMEDS (an mHealth app focused on mental health and wellness) among medical students. Students found the app experience valuable, accessed mental health screeners embedded within the app, utilized the app to seek help, and engaged with the app to learn more about mental health. There was also a decrease in mental health stigma observed during the course of the study. Based on these results, we propose that medical schools incorporate mobile-based technology into their mental health support programs.

Abstract:This research study serves, on the one hand, to assess the prevalence of burnout among the nursing staff. On the other hand, to determine the socio-demographic and professional characteristics associated with burnout. The aim is to propose an interactive tool to cope with burnout. This is an observational study of a cross-sectional type, conducted among a population of 30 nurses over 4 months. A two sections self-administered survey was used to determine: 1) Sociodemographic and professional characteristics; 2) the Copenhagen Burnout Inventory to evaluate burnout. In total, 17% of nurses display a high level of burnout in both its dimensions and that more than 50% are highly susceptible to it. Moreover, 83% of participants agreed to use the technology as a solution. The proposed prevention solution to cope with burnout was developed by the scratch programming code using the app inventor tool, it consists of three online services (Training, Assessment, Support).

Abstract:Isatin (1H-indole-2,3-dione) and its derivatives demonstrate a diverse array of biological and pharmacological activities including anticonvulsant, antibacterial, antifungal, antiviral and anticancer properties. This broad spectrum of biochemical targets has been facilitated by the synthetic versatility of isatin, which has allowed the generation of a large number of structurally diverse derivatives including analogues derived from substitution of the aryl ring, and/or derivatisation of the isatin nitrogen and C2/C3 carbonyl moieties. The recent FDA approval of the oxindole sunitinib malate, as a kinase inhibitor for the treatment of advanced renal carcinoma and gastrointestinal stromal tumours, underscores the increasing interest in isatins as a new class of antineoplastic agents. In addition to potent kinase inhibition, the mechanism of action of other isatin derivatives includes the inhibition and/or modulation of proteases, translation initiation, neo-vascularisation and tubulin polymerisation. It was therefore the objective of this review to systematically evaluate the cytotoxic and anticancer properties of various substituted isatins and collate these findings to be used as a guide for future structure-activity relationship and mode of action studies. This is the first review to comprehensively discuss the in vitro and in vivo anticancer activities of isatin and its substituted derivatives.
A Resilience-Building App to Support the Mental Health of Health Care Workers in the COVID-19 Era: Design Process, Distribution, and Evaluation

Abstract:Background The COVID-19 pandemic has resulted in increased strain on health care systems and negative psychological effects on health care workers (HCWs). This is anticipated to result in long-term negative mental health effects on the population, with HCWs representing a particularly vulnerable group. The scope of the COVID-19 pandemic necessitates the development of a scalable mental health platform to provide services to large numbers of at-risk or affected individuals. The Mount Sinai Health System in New York City was at the epicenter of the pandemic in the United States. Objective The Center for Stress, Resilience, and Personal Growth (CSRPG) was created to address the current and anticipated psychological impact of the pandemic on the HCWs in the health system. The mission of the Center is to support the resilience and mental health of employees through educational offerings, outreach, and clinical care. Our aim was to build a mobile app to support the newly founded Center in its mission. Methods We built the app as a standalone digital platform that hosts a suite of tools that users can interact with on a daily basis. With consideration for the Center’s aims, we determined the overall vision, initiatives, and goals for the Wellness Hub app, followed by specific milestone tasks and deliverables for development. We defined the app’s primary features based on the mental health assessment and needs of HCWs. Feature definition was informed by the results of a resilience survey widely distributed to Mount Sinai HCWs and by the resources offered at CSRPG, including workshop content. Results We launched our app over the course of two phases, the first phase being a “soft” launch and the second being a broader launch to all of Mount Sinai. Of the 231 HCWs who downloaded the app, 173 (74.9%) completed our baseline assessment of all mental health screeners in the app. Results from the baseline assessment show that more than half of the users demonstrate a need for support in at least one psychological area. As of 3 months after the Phase 2 launch, approximately 55% of users re-entered the app after their first opening to explore additional features, with an average of 4 app openings per person. Conclusions To address the mental health needs of HCWs during the COVID-19 pandemic, the Wellness Hub app was built and deployed throughout the Mount Sinai Health System. To our knowledge, this is the first resilience app of its kind. The Wellness Hub app is a promising proof of concept, with room to grow, for those who wish to build a secure mobile health app to support their employees, communities, or others in managing and improving mental and physical well-being. It is a novel tool offering mental health support broadly.
A Mobile Health Approach for Improving Outcomes in Suicide Prevention (SafePlan)

Abstract:Background: Suicide is a prominent cause of death worldwide, particularly among young people. It was the second leading cause of death among those aged 15-29 years globally in 2016. Treatment for patients with suicidal thoughts or behaviors often includes face-to-face psychological therapy with a mental health professional. These forms of interventions may involve maintaining and updating paper-based reports or worksheets in between sessions. Mobile technology can offer a way to support the implementation of evidence-based psychological techniques and the acquisition of protective coping skills. Objective: This study aims to develop a mobile app to facilitate service users' access to mental health support and safety planning. This process involved eliciting expert input from clinicians who are actively engaged in the provision of mental health care. Methods: A survey was distributed to targeted health care professionals to determine what features should be prioritized in a new mobile app relating to suicide prevention. On the basis of the survey results, a clinical design group, comprising 6 members with experience in fields such as mobile health (mHealth), clinical psychology, and suicide prevention, was established. This group was supplemented with further input from additional clinicians who provided feedback over three focus group sessions. The sessions were centered on refining existing app components and evaluating new feature requests. This process was iterated through regular feedback until agreement was reached on the overall app design and functionality. Results: A fully functional mobile app, known as the SafePlan app, was developed and tested with the input of clinicians through an iterative design process. The app's core function is to provide an interactive safety plan to support users with suicidal thoughts or behaviors as an adjunct to face-to-face therapy. A diary component that facilitates the generalization of skills learned through dialectical behavior therapy was also implemented. Usability testing was carried out on the final prototype by students from a local secondary school, who are representative of the target user population in both age and technology experience. The students were asked to complete a system usability survey (SUS) at the end of this session. The mean overall SUS rating was 71.85 (SD 1.38). Conclusions: The participatory process involving key stakeholders (clinicians, psychologists, and information technology specialists) has resulted in the creation of an mHealth intervention technology that has the potential to increase accessibility to this type of mental health service for the target population. The app has gone through the initial testing phase, and the relevant recommendations have been implemented, and it is now ready for trialing with both clinicians and their patients.
Designing an App to Support Measurement-Based Peer Supervision of Frontline Health Workers Delivering Brief Psychosocial Interventions in Texas: Multimethod Study

Abstract:Background: The unmet need for mental health care affects millions of Americans. A growing body of evidence in implementation science supports the effectiveness of task sharing in the delivery of brief psychosocial interventions. The digitization of training and processes supporting supervision can rapidly scale up task-shared interventions and enable frontline health workers (FLWs) to learn, master, and deliver interventions with quality and support. Objective: We aimed to assess the perceived feasibility and acceptability of a novel mobile and web app designed and adapted to support the supervision, training, and quality assurance of FLWs delivering brief psychosocial interventions. Methods: We followed human-centered design principles to adapt a prototype app for FLWs delivering brief psychosocial interventions for depression, drawing from an app previously designed for use in rural India. Using a multimethod approach, we conducted focus group sessions comprising usability testing and group interviews with FLWs recruited from a large health system in Texas to assess the feasibility and acceptability of the app. The positive System Usability Scale was used to determine the app's overall usability. We also calculated the participants' likelihood of recommending the app to others using ratings of 0 to 10 from least to most likely (net promoter score). Focus group transcripts were coded and analyzed thematically, and recommendations were summarized across 4 key domains. Results: A total of 18 FLWs varying in role and experience with client care participated in the study. Participants found the app to be usable, with an average System Usability Scale score of 72.5 (SD 18.1), consistent with the industry benchmark of 68. Participants' likelihood of recommending the app ranged from 5 to 10, yielding a net promoter score of 0, indicating medium acceptability. Overall impressions of the app from participants were positive. Most participants (15/18, 83%) found the app easy to access and navigate. The app was considered important to support FLWs in delivering high-quality mental health care services. Participants felt that the app could provide more structure to FLW training and supervision processes through the systematic collection and facilitation of performance-related feedback. Key concerns included privacy-related and time constraints regarding implementing a separate peer supervision mechanism that may add to FLWs' workloads. Conclusions: We designed, built, and tested a usable, functional mobile and web app prototype that supports FLW-delivered psychosocial interventions in the United States through a structured supervision mechanism and systematic collection and review of performance measures. The app has the potential to scale the work of FLWs tasked with delivering these interventions to the hardest-to-reach communities they serve. The results of this project will inform future work to evaluate the app's use and efficacy in real-world settings to support task-shared mental health programs across the United States.
Surviving & Thriving; a healthy lifestyle app for new US firefighters: usability and pilot study protocol

Abstract:In the United States (US), new firefighters' fitness and health behaviors deteriorate rapidly after fire academy graduation. Over the long-term, this increases their risks for chronic diseases. This study protocol describes the proposed usability testing and pilot study of a newly designed and developed healthy lifestyle smartphone app, "Surviving & Thriving", tailored towards young US firefighters. "Surviving & Thriving" will provide interactive educational content on four lifestyle factors; nutrition, sleep, physical activity, and resilience, and include a personalized journey, habit tracker, and elements of gamification to promote engagement and long-term healthy behavior change. The first phase of the app development entails alpha testing by the research team and pre-beta testing by a fire service expert panel which will help refine the app into a pre-consumer version. Upon completion of the full app prototype, beta 'usability' testing will be conducted among new fire academy graduates from two New England fire academies to collect qualitative and quantitative feedback via focus groups and satisfaction surveys, respectively. A last phase of piloting the app will evaluate the app's efficacy at maintaining/improving healthy lifestyle behaviors, mental health metrics, and physical fitness metrics. We will also evaluate whether firefighters' perceived "health cultures" scores (ratings of each fire station's/fire department's environments as to encouraging/discouraging healthy behaviors) modify the changes in health metrics after utilizing the app for three to six months. This novel user-friendly app seeks to help new firefighters maintain/improve their health and fitness more effectively, reducing their risk of lifestyle-related chronic disease. Firefighters who can establish healthy habits early in their careers are more likely to sustain them throughout their lives.

568 Clinician Survey on Emotional Challenges in Treating Self-Immolation or Self-Inflicted Burn Patients

Abstract:Abstract Introduction Self-immolation (SIM) is an uncommon method of attempted suicide involving flammable substances, with suicidal intent. By contrast self-inflicted (SIF) burn injuries utilize a chemical or heated object to cause injury, without suicidal intent. Caring for either patient population can have emotional and ethical challenges. The purpose of this study was to better understand the perspectives and experiences of the healthcare clinician when caring for these patients and identify the need for enhanced support. Methods An 11-item survey was distributed to burn center professionals via SurveyMonkey. Questions were developed to seek healthcare professionals’ attitudes, emotional and ethical challenges in treating these patient populations. Respondents were stratified into two groups, those that responded yes to having found it emotionally challenging to care for this patient population (Y) and those that responded no (N). Results A total of 65 individuals responded to the survey, and 58 surveys were fully completed. The majority, 65%, of respondents were between ages 22-39, (n=41). Most were burn nurses, 46.7% (n = 29), followed by rehabilitation professionals (rehab), 12.9% (n = 8), advanced practice practitioners (APP), 9.6% (n=6) and physicians 8% (n=5). When stratified, 53% (n=31) responded yes (Y) when asked if they found it emotionally challenging to care for this patient population and 48% (n=28) responded no (N). There were no significant differences between groups for age, gender, years in profession or years in burn care. When examined by profession, 75% of rehab, 67% of psychosocial, 50% of nurses, 40% of physicians and 33% of APP respondents answered yes that care of this population was emotionally challenging. The majority of both groups responded that they received support from team members. When asked if care of these patients created an ethical or moral dilemma 42% of Y and 25% of N responded yes. Y were more likely to have experienced a change in their own mental health 52% vs 7% N. When asked if same day support would be helpful 74% (n=23) of Y and 42% (n=12) responded yes. The preferred format for help was 1:1 or group. Conclusions The majority of staff reported emotional and/or ethical challenges when caring for this population created. Of those who responded that care of these patients was not emotionally challenging, interestingly, 25% experienced ethical or moral dilemmas and 43% felt that having same day support would be beneficial. Identification of the need for enhanced support for staff in the hospital setting is critical to avoid burnout, secondary traumatic stress and compassion fatigue for healthcare professionals. Applicability of Research to Practice This study highlights the need for a specialized in-house enhanced support program for burn professionals to address their emotional and mental well-being.

Abstract:Background: Reactive arthritis is an HLA-B27-associated disease triggered by Chlamydia trachomatis. Results: Three chlamydial peptides endogenously presented by HLA-B27 were identified. All were homologous to human-derived sequences, and one showed conformational similarity to a self-derived HLA-B27 ligand. Conclusion: Molecular mimicry between chlamydial and self-derived HLA-B27 ligands is not uncommon. Significance: Molecular mimicry may contribute to the pathology of reactive arthritis. Reactive arthritis (ReA) is an HLA-B27-associated spondyloarthropathy that is triggered by diverse bacteria, including Chlamydia trachomatis, a frequent intracellular parasite. HLA-B27-restricted T-cell responses are elicited against this bacterium in ReA patients, but their pathogenetic significance, autoimmune potential, and relevant epitopes are unknown. High resolution and sensitivity mass spectrometry was used to identify HLA-B27 ligands endogenously processed and presented by HLA-B27 from three chlamydial proteins for which T-cell epitopes were predicted. Fusion protein constructs of ClpC, Na+-translocating NADH-quinone reductase subunit A, and DNA primase were expressed in HLA-B27+ cells, and their HLA-B27-bound peptidomes were searched for endogenous bacterial ligands. A non-predicted peptide, distinct from the predicted T-cell epitope, was identified from ClpC. A peptide recognized by T-cells in vitro, NQRA(330–338), was detected from the reductase subunit. This is the second HLA-B27-restricted T-cell epitope from C. trachomatis with relevance in ReA demonstrated to be processed and presented in live cells. A novel peptide from the DNA primase, DNAP(211–223), was also found. This was a larger variant of a known epitope and was highly homologous to a self-derived natural ligand of HLA-B27. All three bacterial peptides showed high homology with human sequences containing the binding motif of HLA-B27. Molecular dynamics simulations further showed a striking conformational similarity between DNAP(211–223) and its homologous and much more flexible human-derived HLA-B27 ligand. The results suggest that molecular mimicry between HLA-B27-restricted bacterial and self-derived epitopes is frequent and may play a role in ReA.

Abstract:Poor well-being and burnout among the nursing workforce were heightened by the COVID-19 pandemic. The purpose of this study was to deliver, spread, and sustain an evidence-based wellness program, Workforce Engagement for Compassionate Advocacy, Resilience, and Empowerment (WE CARE), for nurse leaders, staff registered nurses (RNs), and patient care technicians (PCTs) to ameliorate or prevent burnout, promote resilience, and improve the work environment. The program included Community Resiliency Model (CRM) training provided by a certified 6-member wellness team. A baseline and 6-month follow-up survey included measures of well-being, moral distress, burnout, resilience, perceived organizational support (POS), job satisfaction, intent to leave (ITL), and work environment. A total of 4900 inpatient RNs, PCTs, and leaders of a 1207-bed academic medical center in the southeastern United States were analyzed. From baseline (n = 1533) to 6-month follow-up (n = 1457), well-being, moral distress, burnout, job satisfaction, and work environment improved; however, resilience, POS, and ITL did not. Although we have seen some improvements in well-being and mental health indicators, it is still early in the intervention period to have reached a critical mass with the training and other interventions. The mental health and work environment issues among nurses are so complex, no one-size-fits-all intervention can resolve.

Abstract:Abstract Introduction The percent total body surface area (TBSA) burned is a critical determinant of required level of care, initial management, and prognosis in burn patients. The current gold standard for TBSA estimation, the Lund-Browder (LB) chart, requires familiarity with its construction and is not necessarily feasible in the field. Recently, many mobile applications (apps) have been developed to calculate TBSA. However, few have been rigorously validated. In this study, we present a novel TBSA calculator app developed for first responders and validate its accuracy. Methods Infant, pediatric, and adult mannequins were fabricated with eight simulated burns (Figure 1). Thirteen first- and second-year medical students with no experience in burn care were tasked with calculating the TBSA of these burns using both LB and our app. Students then completed a questionnaire to assess their experience using both methods. A paired t-test was used to compare absolute mean differences in TBSA between student estimates using both methods and those of two expert burn surgeons using LB. Results The app performed significantly better than LB for three of the simulated burns with mean TBSA values better approximating expert values (Burn 2 p = 0.002, Burn 5 p = 0.039, and Burn 8 p = 0.006). For the remaining five simulated burns, there was no significant difference in performance between the two methods (Table 1). Students overwhelmingly reported the app was easier and faster to use (92%, n = 12), and universally preferred it over LB for the calculation of TBSA. Students cited the visual nature of the app interface and the lack of need for manual calculations as reasons for their preference. Conclusions The app offered an easier, faster, and more accurate alternative to LB for calculation of TBSA among inexperienced users. Future directions include replicating this study with first responders. Applicability of Research to Practice Our app may be a valid alternative to LB for TBSA calculation.

Abstract:Abstract Successful embryo implantation involves complex interactions between the embryo and the uterus and is critical in establishing pregnancy. Proprotein convertase (PC) 6 (PC6) is one of the PC endoproteases regulating protein function through posttranslational activation of precursor proteins, including growth and differentiation factors. Here we show that PC6 protein is induced in the uterine stromal cells specifically at the site of embryo attachment during early pregnancy in mice. In vivo blocking of uterine production of PC6 protein using morpholino antisense oligonucleotides in mice resulted in total inhibition of implantation, revealing a vital role for PC6 in modulating the uterus for embryo implantation. Studies in primates (rhesus monkey and human) showed a dramatic upregulation of endometrial PC6 during the phase of uterine receptivity and at implantation, particularly during a critical uterine cell differentiation process termed decidualization. Thus, the current studies have demonstrated that PC6 is an essential molecule in modulating uterine function to support the establishment of embryo implantation. Interestingly, PC6 is one of the PCs identified to be important in processing the coat protein of HIV; inhibition of PCs has been suggested to be an effective approach to reduce HIV transmission. We therefore propose the novel concept that PC6 could be a potential nonhormonal target in the female reproductive tract for dual protection for women, both in preventing pregnancy and reducing HIV infection.
Workplace mental health resilience: usability and impact study of a mental health coping mobile app on a corporate setting

Abstract:Introduction Mental health resilience is crucial to professional wellbeing and productivity, being that 57% of company employees are reporting burnout. Additionally, early-stage preventive mental health interventions are not common, and typically employees only have access to mid-stage professional care. Objectives The aim of this project is to provide employees with a preventive self-coping tool, enabling open and inclusive care. Specifically, the aim is to assess the receptivity, usability and impact of the usage of a mobile app that provides coping strategies based on positive psychology and a burnout-risk screening. Methods A mobile app was used with the purpose of being accessible to everyone, independently of their financial capacity. The app also promotes inclusiveness, by aggregating several approaches and methods for mental health coping, which are recommended given the needs of each user. The app was released to a large Portuguese company with 700 employees, in which employees could download it voluntarily. Results After 7 months, the results showed 37% receptivity rate, 24% improvement on anxiety levels, 36% improvement on workplace wellbeing, 23% increase on mental health self-coping skills, and 21% improvement on burnout-risk levels. These metrics were acquired via app's back-end, self-reporting, and our model for burnout-risk screening. Conclusions First results showcase the positive impact of adding such a mobile solution to the employees' mental healthcare. Next steps will be conducting a longer study, adding control groups and productivity assessment. Disclosure of Interest None Declared

Abstract:Health Informatics Journal, Volume 30, Issue 4, October-December 2024. SAFE is a mobile application co-created for and by people who have experienced self-harm, either themselves or as next of kin. This study intended to integrate SAFE into an Emergency Department (ED) to help patients share experiences of self-harm and to support professionals in conducting treatment as usual (TAU). Objective: This study was a part of a Co-operative Inquiry in which a learning intervention was implemented, followed by an interview study exploring ED nurses' reflections and learnings while integrating SAFE into their practice. Methods: Thirteen semi-structured interviews were analysed using reflexive thematic analysis. Results: The nurses imagined that SAFE could be a positive game changer. However, they were hesitant due to uncertainty about the ED context, the value of the app and their skills. Conclusions: Supplying TAU with technology is challenging and future digital solutions must be created in partnership to ensure the solutions are customised to the target group.

Abstract:I n contrast with COPD patients, patients with obstructive sleep apnea (OSA) have a priori normal respiratory mechanics and normal gas exchange during wakefulness. Increased upper airway resistance on sleep onset in OSA patients leads to intermittent or sustained alveolar hypoventilation during the night, but this process usually reverses within a short time after the patients wake up. However, about 30 to 40% of patients with OSA will continue to exhibit elevated Pco2 levels throughout the waking period, despite the absence of severe lung disease. In other words, OSA patients are at high risk for hypoventilation syndrome. Therefore, our primary gut feeling would be to attribute the daytime alveolar hypoventilation in patients with OSA to obesity, since it has been clearly implicated in the pathophysiology of OSA patients.1–4 Furthermore, we have now all become familiar with obesity hypoventilation syndrome (OHS), a complex condition that associates morbid obesity with severe daytime gas exchange abnormalities,5 such that we may have intuitively associated the daytime hypercapnia of OSA patients with obesity. In this issue of CHEST, we are reminded that, as is usual in medicine, things are not so simple. Indeed, Akashiba and colleagues (see page 415) report on their analysis of potential correlates of daytime hypercapnia in 143 male patients with OSA. Using a stepwise logistic regression approach, these investigators found that 43% of the variance in daytime Paco2 levels could be attributed to the mean oxyhemoglobin saturation during sleep and to the degree of vital capacity dysfunction, while the body mass index and the apnea index did not emerge from this analysis as exerting any additional effect on the predictive value of the model. Taken together, these findings would suggest that respiratory mechanics and nighttime hypoxemia may interact during sleep and during wakefulness in patients with OSA. How do we reconcile these findings with those from patients with OHS? A unifying concept for the pathogenesis of OHS has been formulated in a recent editorial in CHEST by Teichtahl.6 Respiratory muscle fatigue and diminished ventilatory response to a respiratory load,7 adverse respiratory mechanics and decreased leptin levels or leptin receptor density, which may attenuate hypercapnic ventilatory responsiveness,8–10 may all interact to promote the development of OHS. However, while the possibility of respiratory muscle fatigue in patients with OSA has not been explored systematically,11 leptin levels are not low but, rather, have been found to be elevated in these patients!12 Indeed, the mean ( SD) leptin levels were 13.7 1.3 ng/mL in 32 male patients with OSA whose conditions were newly diagnosed and had never been treated, and 9.2 1.2 ng/mL in 32 similarly obese, closely matched subjects (p 0.02).12 Thus, if leptin is indeed a modulator of central chemosensitivity, then OSA patients would be expected to display enhanced rather than attenuated CO2 ventilatory responses. An alternative explanation to the daytime hypercapnia of OSA patients could involve the characteristics of the events occurring during sleep. This explanation, which is based on the observation that eucapnic and hypercapnic OSA patients do not exhibit major differences in the number and duration of apneic episodes that they experience during the night,13 proposes that the ventilatory output during sleep will play a major deterministic role in the daytime ventilation characteristics of OSA patients.14 Rapoport and colleagues14 identified the following two major components affecting daytime gas exchange in OSA: (1) the occurrence of sustained hypoventilation independent of apnea and therefore not reversed by continuous positive airway pressure therapy, which would primarily reflect a low level of ventilatory drive in the awake state; and (2) a critical interdependency between the overall ventilation during the time spent awake and asleep, and the degree of CO2 loading induced by the apneic episodes that is not compensated for during the postapnea period.14 This second component was reversible with treatment of the apnea, such that conceptually, the balance between the kinetics of CO2 accumulation and elimination during the apnea and interapnea periods would impose the resultant Paco2 level during the daytime. Thus, we could interpret the findings of Akashiba and colleagues as being indicative of close interactions between a ventilatory debt incurred during OSA-disturbed sleep (which can be modified by effective therapy) and intrinsic individual factors associated with respiratory drive and gas exchange. In this context, breath-by-breath measurements of CO2 and O2 levels during sleep and wakefulness may provide important insights into the potential imbalances in CO2 homeostasis that are generated during sleep in patients with eucapnia and hypercapnia during the

Abstract:Occupational burnout is a significant global problem that has impacted clinical outcomes, patient safety, and patient-centered care across healthcare settings (World Health Organization) [WHO], 2019. The classic definition of burnout, as defined by Dr. Christina Maslach, is “a psychologic syndrome involving emotional exhaustion, depersonalization, and a diminished sense of personal accomplishment (Maslach & Jackson, 1982). Burnout affects cognitive, behavioral and emotional aspects of human behavior. It also interferes with how individuals process and interact with others at an individual and organizational level. Others have noted that burn-out individuals tend to focus on negative things rather than positive emotions (Bianchi & Laurent, 2015). Nurse and physician burnout was recognized before the COVID-19 pandemic as a growing problem. Evidence now suggests that more than 50% of nurses, physician assistants and physicians report syndromes of burnout. The problem has become even more pronounced when the work environment contributes stressors associated with technological advancements, EMRs, increased patient acuity, and financial cost cutting measures. It is now recognized that individuals and organizations burnout. As a public health issue, burnout and well-being are multifactorial and recognize that individuals and organizations contribute significant factors to the burnout syndrome. It appears that burnout rates are rising across specialties and settings resulting in increase of costs to individuals and organizations (National Academy of Medicine [NAM]), 2018. What is needed is a clear map to make decisions on clinical burnout. Healthcare workers (HCW) need confidential access to wellness activities, support systems and mental health consultation. A new model is needed to further refine the work-life balance and create a caring corporate culture. Healthcare leaders need to rethink how to support employees across disciplines when it comes to burnout and well-being.
