Abstract:Bone fracture plates are usually made from steel or titanium, which are much stiffer than cortical bone. This may cause bone 'stress shielding' (i.e. bone resorption leading to plate loosening) and delayed fracture healing (i.e. fracture motion is less than needed to stimulate callus formation at the fracture). Thus, the authors previously designed, fabricated, and mechanically tested novel 'hybrid' composites made from inorganic and organic materials as potential bone fracture plates that are more flexible to reduce these negative effects. This is the first study to measure the cytotoxicity of these composites via the survival of rat cells. Cubes of carbon fiber/flax fiber/epoxy and glass fiber/flax fiber/epoxy had better cell survival vs. Kevlar fiber/flax fiber/epoxy (57% and 58% vs. 50%). Layers and powders made of carbon fiber/epoxy and glass fiber/epoxy had higher cell survival than Kevlar fiber/epoxy (96%-100% and 100% vs. 39%-90%). The presence of flax fibers usually decreased cell survival. Thus, carbon and glass fiber composites (with or without flax fibers), but not Kevlar fiber composites (with or without flax fibers), may potentially be used for bone fracture plates.

Abstract:Field studies of releases and recaptures of diurnal moths painted with yellow to resemble the edible tiger swallowtail and of black moths that resemble a toxic species of swallowtail produced these results: (i) A greater proportion of the black moths were recaptured; (ii) daily trapping for a week after each release showed that the black moths survived longer than the yellow-painted moths; (iii) an analysis of wing injuries shows that most attacks can be attributed to birds and that the yellow-painted moths were attacked more often, more vigorously, or more persistently than the black moths. These results are interpreted as showing a greater predation pressure on the yellow-painted than on the black moths and, therefore, as confirming the Batesian theory of mimicry.

Abstract:Bone plates play a vital role in bone fracture healing by providing the necessary mechanical fixation for fracture fragments through modulating biomechanical microenvironment adjacent to the fracture site. Good treatment effect has been achieved for fixation of bone fracture with conventional bone plates, which are made of stainless steel or titanium alloy. However, several limitations still exist with traditional bone plates including loosening and stress shielding due to significant difference in modulus between metal material and bone tissue that impairs optimal fracture healing. Additionally, due to demographic changes and non-physiological loading, the population suffering from refractory fractures, such as osteoporosis fractures and comminuted fractures, is increasing, which imposes a big challenge to traditional bone plates developed for normal bone fracture repair. Therefore, optimal fracture treatment with adequate fixation implants in terms of materials and design relevant to special conditions is desirable. In this review, the complex physiological process of bone healing is introduced, followed by reviewing the development of implant design and biomaterials for bone plates. Finally, we discuss recent development of hybrid bone plates that contains bioactive elements or factors for fracture healing enhancement as a promising direction. This includes biodegradable Mg-based alloy used for designing bone screw-plates that has been proven to be beneficial for fracture healing, an innovative development that attracts more and more attention. This paper also indicates that the tantalum bone plates with porous structure are also emerging as a new fracture internal fixation implants. The reduction of the stress shielding is verified to be useful to accelerate bone fracture healing. Potential application of biodegradable metals may also avoid a second operation for implant removal. Further developments in biometals and their design for orthopedic bone plates are expected to improve the treatment of bone fracture, especially the refractory fractures.

Abstract:This study aims to address the challenges associated with conventional metallic bone fixation plates in biomechanical applications, such as stainless steel and titanium alloys, including stress shielding, allergic reactions, corrosion resistance, and interference with medical imaging. The use of materials with a low elastic modulus is regarded as an effective approach to overcome these problems. In this study, the impact of different types of chopped carbon fiber-reinforced polyether ether ketone (CCF/PEEK) functionally graded material (FGM) bone plates on stress shielding under static and instantaneous dynamic loading was explored using finite element analysis (FEA). The FGM bone plate models were established using ABAQUS and the user's subroutine USDFLD and VUSDFLD, and each model was established with an equivalent overall elastic modulus and distinctive distributions. The results revealed that all FGM bone plates exhibited lower stress shielding effects compared to metal bone plates. Particularly, the FGM plate with an elastic modulus gradually increased from the centre to both sides and provided maximum stress stimulation and the most uniform stress distribution within the fractured area. These findings offer crucial insights for designing implantable medical devices that possess enhanced mechanical adaptability.

Abstract:Polymers and Polymer Composites, Volume 32, Issue , January-December 2024. Facial bone fractures in facial injuries resulting from severe trauma can be treated using plate and screw systems. Depending on the location of the fracture, the fracture lines are brought end to end and fixed with plates and screws. Metallic implants are at risk of endogenous infection and there are inherent problems associated with their use, such as stress shielding phenomena, pain, and local irritation. After the bone has healed, a second surgery is needed to remove the metallic fixation. Considering these problems, it is important to use biodegradable plates instead of metal plates. Biodegradable fracture fixation plates degrade slowly in the human body, causing less pain and stress, and there is no need for a second procedure to remove the synthetic material. As these plates decompose, the load will also decrease. There are no toxic and mutagenic effects in an absorbable implant. However, there are some problems with the use of these implants, such as inflammatory response, rapid loss of initial implant strength, higher fracture rates, insufficient hardness of the implants, and weakness compared to metallic implants. In this study, biodegradable plates were designed for use in maxillofacial bone injuries, and treatment plates were prepared using extrusion and injection molding. Different proportions of trimethylene carbonate (TMC), poly-L-lactate (LPLA), poly(D,L-lactate) (DLPLA), polyglycolate (PGA) and beta-tricalcium phosphate (β-TCP) are mixed by extrusion to produce composites by injection molding. Plates were obtained. The degradation, morphological, thermal, and mechanical behaviors of the obtained products were examined.

Assessment of carbon fibre composite fracture fixation plate using finite element analysis

Abstract:In the internal fixation of fractured bone by means of bone-plates fastened to the bone on its tensile surface, an on-going concern has been the excessive stress shielding of the bone by the excessively-stiff stainless-steel plate. The compressive stress shielding at the fracture-interface immediately after fracture-fixation delays callus formation and bone healing. Likewise, the tensile stress shielding in the layer of bone underneath the plate can cause osteoporosis and decrease in tensile strength of this layer. In this study a novel forearm internal fracture fixation plate made from short carbon fibre reinforced plastic (CFRP) was used in an attempt to address the problem. Accordingly, it has been possible to analyse the stress distribution in the composite plates using finite-element modelling. A three-dimensional, quarter-symmetric finite element model was generated for the plate system. The stress state in the underlying bone was examined for several loading conditions. Based on the analytical results the composite plate system is likely to reduce stress-shielding effects at the fracture site when subjected to bending and torsional loads. The design of the plate was further optimised by reducing the width around the innermost holes.
Biomechanical design of a new proximal humerus fracture plate using alternative materials

Abstract:Comminuted proximal humerus fractures are often repaired by metal plates, but potentially still experience bone refracture, bone "stress shielding," screw perforation, delayed healing, and so forth. This "proof of principle" investigation is the initial step towards the design of a new plate using alternative materials to address some of these problems. Finite element modeling was used to create design graphs for bone stress, plate stress, screw stress, and interfragmentary motion via three different fixations (no, 1, or 2 "kickstand" [KS] screws across the fracture) using a wide range of plate elastic moduli (EP = 5-200 GPa). Well-known design optimization criteria were used that could minimize bone, plate, and screw failure (i.e., peak stress < ultimate tensile strength), reduce bone "stress shielding" (i.e., bone stress under the new plate ≥ bone stress for an intact humerus, titanium plate, and/or steel plate "control"), and encourage callus growth leading to early healing (i.e., 0.2 mm ≤ axial interfragmentary motion ≤ 1 mm; shear/axial interfragmentary motion ratio <1.6). The findings suggest that a potentially optimal configuration involves the new plate being manufactured from a material with an EP of 5-41.5 GPa with 1 KS screw; but, using no KS screws would cause immediate bone fracture and 2 KS screws would almost certainly lead to delayed healing. A prototype plate might be fabricated using alternative materials suggested for orthopedics and other industries, like fiber-metal laminates, fiber-reinforced polymers, metal foams, pure polymers, shape memory alloys, or 3D-printed porous metals.

Abstract:Abstract This study aimed to pioneer a transformative approach in orthopedic implant design by developing and analyzing a groundbreaking carbon‐flax reinforced bioepoxy hybrid composite bone plate. The primary objectives of the present research were to enhance the bio‐mechanical performance of orthopedic implants and explore the potential applications of the novel material for orthopedic implants. Hybrid composite plate was fabricated mimicking the human bone with the soft inner core and a rigid outer coating. Mechanical properties for the hybrid composite were obtained through material characterization studies as per ASTM standards. The hybrid composite bone plates were tested as per bio‐mechanical test standard and the results were correlated with the finite element simulations. The maximum stress value in the experiments for the biomechanical four‐point bending tests was 331.74 MPa, and the corresponding strain value was 0.0337. The maximum equivalent stress and strain values obtained from simulation were in line with the findings of the experiments. The current research signifies a paradigm shift in orthopedic implant technology. The carbon‐flax bioepoxy hybrid composite offers remarkable potential for orthopedic applications, promising safer and more durable solutions for patients in need of bone repair or replacement.

Carbon fiber reinforced PEEK Optima--a composite material biomechanical properties and wear/debris characteristics of CF-PEEK composites for orthopedic trauma implants

Abstract:Background: The advantageous properties of carbon fiber reinforced polyetheretherketone (CF-PEEK) composites for use as orthopedic implants include similar modulus to bone and ability to withstand prolonged fatigue strain. Methods: The CF-PEEK tibial nail, dynamic compression plate, proximal humeral plate and distal radius volar plate were compared biomechanically (by four-point bending, static torsion of the nail, and bending fatigue) and for wear/debris (by amount of the debris generated at the connection between the CF-PEEK plate and titanium alloy screws) to commercially available devices. Results: Four-point bending stress of the tibial nail and dynamic and distal radius plates yielded characteristics similar to other commercially available devices. The distal volar plate bending structural stiffness of the CF-PEEK distal volar plate was 0.542 Nm2 versus 0.376 Nm2 for the DePuy's DVR anatomic volar plate. The PHILOS proximal humeral internal locking system stainless steel plate was much stronger (6.48 Nm2) than the CF-PEEK proximal humeral plate (1.1 Nm2). Tibial nail static torsion testing showed similar properties to other tested nails (Fixion, Zimmer and Synthes). All tested CF-PEEK devices underwent one million fatigue cycles without failure. Wear test showed a lower volume of generated particles in comparison to the common implants in use today. Interpretation: Thus, these tested implants were similar to commercially used devices and can be recommended for use as implants in orthopedic surgery.

Abstract:The persistent failure of conventional materials used in manufacturing orthopedic implants was due to the deficiency or poor integrations of implant materials to the juxtaposed bone and stress-strain imbalances between the interfaces of tissues and implant materials. Therefore, the fabrication of a suitable bioactive scaffold for bone tissue engineering is considered a vital requisite to mimic the extracellular bone matrix. Numerous researches were reported concerning the fabrication of a suitable bioactive scaffold to improve cell adhesion, proliferation, and differentiation so far. However, for the past two decades, the research on carbon nanotubes (CNTs)-reinforced composites employed in the biomedical field is increasing day-by-day because of its outstanding properties. Moreover, it is essential to choose a biocompatible polymer with greater affinity to act as an extracellular matrix as well as to attract CNTs and in facilitating the homogeneous distribution of CNTs in aqueous and organic solvents. The development of CNTs-based composites in bone tissue engineering is presented in this review based on the last 10 years of research. Investigations on the Mechanical Properties of Glass Fiber/Sisal Fiber/Chitosan Reinforced Hybrid Polymer Sandwich Composite Scaffolds for Bone Fracture Fixation Applications

Abstract:This study aims to explore the mechanical properties of hybrid glass fiber (GF)/sisal fiber (SF)/chitosan (CTS) composite material for orthopedic long bone plate applications. The GF/SF/CTS hybrid composite possesses a unique sandwich structure and comprises GF/CTS/epoxy as the external layers and SF/CTS/epoxy as the inner layers. The composite plate resembles the human bone structure (spongy internal cancellous matrix and rigid external cortical). The mechanical properties of the prepared hybrid sandwich composites samples were evaluated using tensile, flexural, micro hardness, and compression tests. The scanning electron microscopic (SEM) images were studied to analyze the failure mechanism of these composite samples. Besides, contact angle (CA) and water absorption tests were conducted using the sessile drop method to examine the wettability properties of the SF/CTS/epoxy and GF/SF/CTS/epoxy composites. Additionally, the porosity of the GF/SF/CTS composite scaffold samples were determined by using the ethanol infiltration method. The mechanical test results show that the GF/SF/CTS hybrid composites exhibit the bending strength of 343 MPa, ultimate tensile strength of 146 MPa, and compressive strength of 380 MPa with higher Young’s modulus in the bending tests (21.56 GPa) compared to the tensile (6646 MPa) and compressive modulus (2046 MPa). Wettability study results reveal that the GF/SF/CTS composite scaffolds were hydrophobic (CA = 92.41° ± 1.71°) with less water absorption of 3.436% compared to the SF/CTS composites (6.953%). The SF/CTS composites show a hydrophilic character (CA = 54.28° ± 3.06°). The experimental tests prove that the GF/SF/CTS hybrid composite can be used for orthopedic bone fracture plate applications in future.

Finite element investigation into the use of carbon fibre reinforced PEEK laminated composites for distal radius fracture fixation implants

Abstract:Carbon fibre reinforced PEEK (CF/PEEK) laminates provide mechanical advantages over homogenous metal osteo-synthesis implants, e.g. radiolucency, fatigue strength and strength to weight ratio. Implants can be designed with custom anisotropic material properties, thus enabling the engineer to tailor the overall stiffness of the implant to the specific loading conditions it will experience in vivo. In the current study a multi-scale computational investigation of idealised distal radius fracture fixation plate (DRP) is conducted. Physiological loading conditions are applied to macro-scale finite element models of DRPs. The mechanical response is compared for several CF/PEEK laminate layups to examine the effect of ply layup design. The importance of ply orientation in laminated DRPs is highlighted. A high number of 0° plies near the outer surfaces results in a greater bending strength while the addition of 45° plies increases the torsional strength of the laminates. Intra-laminar transverse tensile failure is predicted as the primary mode of failure. A micro-mechanical analysis of the CF/PEEK microstructure uncovers the precise mechanism under-lying intra-laminar transverse tensile crack to be debonding of the PEEK matrix from carbon fibres. Plastic strains in the matrix material are not sufficiently high to result in ductile failure of the matrix. The findings of this study demonstrate the significant challenge in the design and optimisation of fibre reinforced laminated composites for orthopaedic applications, highlighting the importance of multi-scale modelling for identification of failure mechanisms.
Failure analysis and design improvement of retrieved plates from revision surgery

Abstract:Background: The fracture of bone plate can cause considerable pain for the patient and increase the burden on the public finances. This study aims to explore the failure mechanism of 49 plates retrieved from revision surgery and introduce pure magnesium (Mg) block to improve the biomechanical performance of the plate via decreasing the stiffness and to stimulate the biological response of the plate potentially by the degradation of Mg block. Methods: The morphological analysis and component analysis of the plates were conducted to determine the fracture reason of the plates combining the clinical data. According to the structural feature, the 49 retrieved plates were divided into: traditional plate (TP), asymmetrical plate (AP), reconstructive plate (RP) and central enhancement plate (CEP), and their structure features are normalized in a commercial plate, respectively. The biomechanical performance of the plates was evaluated using a validated femoral finite element model. A block of pure Mg with a thickness of 1 mm, 1.5 mm and 2 mm was also incorporated into the CEP to be assessed. Results: The results indicated that the retrieved plates mainly failed due to fatigue fracture induced by delayed union or nonunion (44/49), and using pure titanium plates in weight-bearing areas increased the risk of fracture compared with Ti alloy plates when the delayed union or nonunion occurred. The TP demonstrated the highest compression resistance and bending resistance, while CEP had the highest rotational resistance. As the thickness of the Mg block was increased, the stress on the plate in compression decreased, but the stress in rotation increased. The plate with a 1.5 mm Mg block demonstrated excellent compression resistance, bending resistance and rotational resistance. Conclusion: Fatigue fracture resulting from the delayed union or nonunion is the primary failure reason of plates in clinic. The incorporation of Mg block into plate improves the biomechanical performance and has the potential to promote bone healing. The plate with a 1.5 mm Mg block may be suitable for use in orthopaedics. The translational potential of this article: This study assessed the failure mechanism of retrieved bone plates and used this data to develop a novel plate incorporating a 1.5 mm block of pure Mg block at the position corresponding to the fracture line. The novel plate exhibited excellent compression resistance, bending resistance and rotational resistance due to the alleviation of stress concentrations. The Mg block has the potential to degrade over time to promote fracture healing and prevents fatigue fracture of plates.
Biomechanical comparison between stainless steel, titanium and carbon-fiber reinforced polyetheretherketone volar locking plates for distal radius fractures

Abstract:Introduction: As the popularity of volar locked plate fixation for distal radius fractures has increased, so have the number and variety of implants, including variations in plate design, the size and angle of the screws, the locking screw mechanism, and the material of the plates. Hypothesis: Carbon-fiber reinforced polyetheretherketone (CFR-PEEK) plate features similar biomechanical properties to metallic plates, representing, therefore, an optimal alternative for the treatment of distal radius fractures. Materials and methods: Three different materials-composed plates were evaluated: stainless steel volar lateral column (Zimmer); titanium DVR (Hand Innovations); CFR-PEEK DiPHOS-RM (Lima Corporate). Six plates for each type were implanted in sawbones and an extra-articular rectangular osteotomy was created. Three plates for each material were tested for load to failure and bending stiffness in axial compression. Moreover, 3 constructs for each plate were evaluated after dynamically loading for 6000 cycles of fatigue. Results: The mean bending stiffness pre-fatigue was significantly higher for the stainless steel plate. The titanium plate yielded the higher load to failure both pre and post fatigue. After cyclic loading, the bending stiffness increased by a mean of 24% for the stainless steel plate; 33% for the titanium; and 17% for the CFR-PEEK plate. The mean load to failure post-fatigue increased by a mean of 10% for the stainless steel and 14% for CFR-PEEK plates, whereas it decreased (-16%) for the titanium plate. Statistical analysis between groups reported significant values (p<001) for all comparisons except for Hand Innovations vs. Zimmer bending stiffness post fatigue (p=.197). Discussion: The significant higher load to failure of the titanium plate, makes it indicated for patients with higher functional requirements or at higher risk of trauma in the post-operative period. The CFR-PEEK plate showed material-specific disadvantages, represented by little tolerance to plastic deformation, and lower load to failure. Level of evidence: N/A.

However, in recent years, composite structures arise as an attractive alternative due to high strength to weight ratio and other advantages like, for example, radiolucency. According to Food and Drug Administration (FDA), the only unidirectionally reinforced composite allowed to be implanted in human bodies is carbon fiber (CF)-reinforced poly-ether-ether-ketone (PEEK). In this work, the healing process of long bone assembled with CF/PEEK plates with cross- and angle-ply lay-up configurations is studied in the framework of finite element method. The healing is simulated by making use of the mechanoregulation model basing on the Prendergast theory. Cells transformation is determined by the octahedral shear strain and interstitial fluid velocity. The process runs iteratively assuming single load cycle each day. The fracture is subjected to axial and transverse forces. In the computations, the Abaqus program is used. It is shown that the angle-ply lamination scheme of CF/PEEK composite seems to provide better conditions for the transformation of the soft callus into the bone tissue.

Abstract:The work reports antibacterial and antifungal activity of different solvent extracts of Origanum vulgare. The antimicrobial activity of methanol, chloroform and aqueous extracts were determined against nine different gram negative and gram positive bacterial strains and three fungal stains. The bacterial strains were Bacillus subtilis (ATCC 6633), Staphylococcus aureus (ATCC 29213), Micrococcus luteus (ATCC 9341), Pseudo-monas aeruginosa (ATCC 33347), Escherichia coli (ATCC 25922), Salmonella typhi (ATCC 19430), Shig-ella flexneri (ATCC 25929), Salmonella para typhi A (ATCC 9150) and Proteus mirabilis (ATCC 49565) and fungal strains were Aspergillus flavus, Aspergillus nigar and Aspergillus pterus. Agar well diffusion method was followed in this study. The comparative analysis of antibacterial activity reflects that among these three extracts, chloroform and methanol extracts shows promising result by exhibiting maximum anti-bacterial activity, whereas aqueous extract is not active against most of these strains. The analysis of anti-fungal activity reveals chloroform extract as most efficacious unlike methanol and Aqueous extracts.
Aspirin and prostacyclin treatment of diabetic dogs.

Abstract:Spontaneously diabetic dogs were treated initially (the first 1/2 yr) with i.v. prostacyclin and oral aspirin and then for 1 yr with varying doses of aspirin alone. At this time cataracts prevented visualization of retinae, and one eye of each animal was removed for examination of the vasculature of the trypsin digested retinae. Studies of the blood during the course of treatment showed an elevation of plasma cyclic adenylic acid and marked lowering of inordinately high platelet count and platelet aggregation. Treatment was stopped and after an additional 1/2 year the animals were euthanized and the fellow eye removed for retinal study. Although just three diabetic animals survived for binocular retinal histology, there were no discernible differences in the retinal vasculature in the fellow eyes of a given animal, and in only one of these three animals were there clear signs of diabetic retinopathy. We must conclude, therefore, that in spite of relative normalization of platelet function in the parameters measured during the course of treatment, we cannot yet claim a salutary effect of treatment on the diabetic retina. Cataracts prevented us from having better information on diabetic retinae prior to treatment and a redesigned study would have to include unilateral histological retinal examination before any treatment is started to determine if, indeed, improved platelet physiology slows or reverses changes in the diabetic retina.
A novel biomimetic trabecular bone metal plate for bone repair and osseointegration

Abstract:Abstract Fracture is one of the most common traumatic diseases in clinical practice, and metal plates have always been the first choice for fracture treatment because of their high strength. However, the bone plates have high elastic modulus and do not match the biomechanics of human bone, which adversely affects callus formation and fracture healing. Moreover, the complex microenvironment in the human body can induce corrosion of metallic materials and release toxic ions, which reduces the biocompatibility of the bone plate, and may necessitate surgical removal of the implant. In this study, tantalum (Ta) was deposited on porous silicon carbide (SiC) scaffolds by chemical vapor deposition technology to prepare a novel porous tantalum (pTa) trabecular bone metal plate. The function of the novel bone plate was evaluated by implantation in an animal fracture model. The results showed that the novel bone plate was effective in fracture fixation, without breakage. Both X-ray and microcomputed tomography analysis showed indirect healing by both pTa trabecular bone metal plates and titanium (Ti) plates; however, elastic fixation and obvious callus formation were observed after fixation with pTa trabecular bone metal plates, indicating better bone repair. Histology showed that pTa promoted the formation of new bone and integrated well with the host bone. Therefore, this novel pTa trabecular bone metal plate has good prospects for application in treating fractures.

Abstract:Patients suffering bone fractures in different parts of the body require implants that will enable similar function to that of the natural bone that they are replacing. Joint diseases (rheumatoid arthritis and osteoarthritis) also require surgical intervention with implants such as hip and knee joint replacement. Biomaterial implants are utilized to fix fractures or replace parts of the body. For the majority of these implant cases, either metal or polymer biomaterials are chosen in order to have a similar functional capacity to the original bone material. The biomaterials that are employed most often for implants of bone fracture are metals such as stainless steel and titanium, and polymers such as polyethene and polyetheretherketone (PEEK). This review compared metallic and synthetic polymer implant biomaterials that can be employed to secure load-bearing bone fractures due to their ability to withstand the mechanical stresses and strains of the body, with a focus on their classification, properties, and application.
