Abstract:Neurodevelopmental disability (ND) poses a significant challenge to infants' socio-emotional and cognitive development, as well as to caregiving dynamics, such as parental sensitivity and mother-infant interaction. Prior research highlights the crucial role of early parenting support interventions in enhancing parental behaviors, serving as a protective developmental factor for children at risk for or diagnosed with ND. This single-cohort, multicentric study aims to evaluate the efficacy of early video-feedback intervention (VFI) in improving maternal parenting behaviors. Thirty-four mothers with their children diagnosed with ND participated in the study. Different dimensions of parenting behavior (i.e., affection, responsiveness, encouragement, teaching) were assessed during 10-minute mother-infant interactions, both before (T0) and after (T1) completing six weekly VFI sessions. Additionally, mothers completed standardized questionnaires assessing parenting stress, anxiety, and depressive symptoms. The results indicated significant improvements in maternal responsiveness, encouragement, and teaching following the intervention. Furthermore, mothers reported a state anxiety reduction at T1. Additionally, higher scores in affection and responsiveness were correlated with reduced maternal stress. These findings support the utility of VFI as a valuable tool for enhancing positive parenting skills in the context of ND, and provide insights into the dyadic processes through which parenting behaviors may promote socio-emotional and cognitive development in children with ND.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Aim</h3><p>To evaluate the effectiveness of early visual training and environmental adaptation on visual function and neurological development in infants with visual impairment.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Method</h3><p>This was a pilot intervention clinical trial study. Thirty infants (mean age 5.9mo, SD 2.1mo, range 4–11mo; 16 males, 14 females) with peripheral visual impairment (PVI, <i>n</i>=15) or cerebral visual impairment (CVI, <i>n</i>=15) participated in a 6‐month visual intervention programme. Thirty matched infants (mean age 6mo, SD 1.4mo, range 4–9mo; 18 males, 12 females) served as a comparison group. Primary outcome measures were visual acuity, contrast sensitivity, and qualitative ocular motor functions. Secondary outcomes were scores on the Griffiths Mental Developmental Scales (GMDS). </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>The treatment group showed a significant improvement in all the primary outcomes (<i>p</i><0.01). The comparison group improved only in visual acuity and contrast sensitivity (<i>p</i><0.01). The treatment group showed greater improvement than the comparison group in visual fixation (<i>p</i>=0.033) and smooth pursuit (<i>p</i><0.01). The CVI subgroup showed greater improvement in visual acuity than the PVI subgroup (<i>p</i><0.01). GMDS subscales of hand–eye coordination (<i>p</i>=0.01) and performance (<i>p</i><0.01) increased in the treatment group, while the total score of the comparison group decreased, driven by language (<i>p</i>=0.039) and hand–eye coordination (<i>p</i>=0.025) subscales. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Interpretation</h3><p>Results suggest that, in infants with visual impairment, visual function and certain developmental outcomes improve in response to early visual training and environmental adaptation, in an interactive context.</p></section>

Abstract:Cycles of miscommunication often occur within the caregiver–child dyad when a child has a visual or visual-and-intellectual disability, influencing sensitive and responsive caregiver behaviour and child happiness. This study aims to examine the efficacy of using interactive technology, the Barti-mat, to promote sensitive and responsive caregiver behaviour, specifically mirroring behaviour, and increase the happiness of children with a visual or visual-and-intellectual disability. The secondary aim is to examine the social validity of the Barti-mat. A mixed-method approach was used, combining quantitative data from a multiple within-series single-case design and qualitative data comparing play-as-usual with play on the Barti-mat. Eleven caregiver–child dyads participated in a home-based study. No significant effects were found for Attunement nor for Valence. Significant improvements were found for Total Mirroring, Happiness, and Arousal. Overall, the caregivers enjoyed using the Bart-mat and were enthusiastic about the development of specialized play material for children with visual impairments. Results of the current study suggest that the Barti-mat can act as catalyst for caregiver mirroring behaviour and improve the happiness of a child with a visual or visual-and-intellectual disability. Caregivers were generally motivated to use and recommend the Barti-mat again. Minor product improvement recommendations were made. The Barti-mat is appropriate for a diverse group of caregivers and children with a visual or visual-and-intellectual disability and would be a good adjunct to preventive attachment-based interventions.
Feasibility and Acceptability of Using FirstPlay® to Enhance Mother-Child Interaction: A pilot study of mothers' perspectives

Abstract:Background: Although there is evidence that human touch facilitates mother-child bonding, mothers' understanding of how to connect with and develop the emotional regulation of their babies remains unclear. Objectives: This study used a Storytelling Massage program to investigate mothers' experience of practicing reciprocal interactions with their children. Specifically, it explored the efficacy of multi-sensory activities for building healthy parent-child bonds. Method: Participants included 12 mothers with children between 8 and 23 months. These mothers participated in a 6-session program on FirstPlay Infant Storytelling-Massage Intervention (FirstPlay Therapy) and attended an individual semi-structured interview after the program. Data were analyzed using a phenomenological approach. Results: The FirstPlay program positively affected the participants' self-efficacy in parent-child bonding and parenting beliefs. Five themes were identified: "bonding with the child-connect and engage," "attending to the child's uniqueness and needs," "developing a structure and a daily routine," "feeling calm and relaxed as a person," and "gaining confidence as a mom." Conclusion: The results of this study further support the need for low-cost, high-impact programs that enhance parent-child interactions. Limitations of this study are discussed. Future research and practical implications are also suggested.
Parental Satisfaction with the Quality of Care in an Early Intervention Service for Children with Visual Impairment: A Retrospective Longitudinal Study

Abstract:The fundamental role of vision during development and the nurturing role of early intersubjectivity have enabled the Robert Hollman Foundation to develop an early intervention program providing holistic support to visually impaired children and their families, where fostering parent-infant interactions is at the heart of our care. The aim of this study is to understand how parents perceive this approach. It is an eleven-year retrospective study of children following the Robert Hollman Foundation’s early intervention program, in which parents’ (n = 1086) perceptions of quality of care were measured through the administration of a specifically designed 4-point scale questionnaire. Annual longitudinal trends of parents’ perceptions were calculated for every single response. Parents reported a very high satisfaction value in 21/23 questions (Mean > 3.7 out of a maximum score of 4, with the highest scores in human and soft skills of professionals) with a statistically positive trend (p < 0.05), throughout the period considered. Our core approach, based on an individualized nurturing relational support, has been appreciated and confirmed by the high satisfaction reported in the questionnaires by parents of children with visual impairment. We therefore hypothesize that parent-infant relationship-based and individualized approaches may help parents achieve better health, well-being, and quality of daily life for their children.

Abstract:A 2-year-old child with cerebral visual impairment (CVI) was brought by the mother to the pediatric ophthalmology clinic. She underwent a detailed functional vision and developmental assessment that revealed impairments in visual developmental skills, sensory, motor, and social skills. She was admitted into an integrated, parent-centered, structured early intervention program specifically designed for her for 3-months after which there were improvements noticed in visual developmental skills such as visual fixation, attention, and tracking; sensory skills such as tactile, olfactory, and gustatory; motor skills, including neck control and fine motor grasping; and age-appropriate social and communication skills. Through this case report, we aim to bring out the benefits of an evidence-based systematic early intervention approach in a child with CVI.

Abstract:This study investigated the effects of a sensory integration program on posture control and language abilities in preschoolers with developmental delay at the age of 3. The study included a single subject, a 3-year-old child, with the intervention period lasting from April to May 2024, consisting of 8 weeks with sessions conducted 3 times a week for 40 minutes each, totaling 12 sessions. Language therapy was conducted for a total of 8 sessions, twice a week for 40 minutes each over 8 weeks. One occupational therapist evaluated K-DDST-II, K-CSP-II, and a combined assessment of K-DDST-II and K-CSP-II to determine if the participant met the selection criteria before the screening and intervention. Language development assessments, including SELSI and PRES, were conducted by one speech therapist. As a result of the study, the child's K-CSP-II quadrant shifted from showing more behaviors than peers in the ‘Registration/Bystander’ area to having behaviors similar to peers. Additionally, in the sensory session, there was a shift from showing more behaviors than peers to behaviors similar to peers in the ‘Body Positioning Processing’ category. In the K-DDST-II results, changes were observed in fine motor skills, adaptation, language, motor development, and final outcome categories. In the language assessment results from SELSI, changes were noted in expressive language scores. Furthermore, in PRES, there was a change from initially scoring at a 28-month level in receptive language, expressive language, and integrated language to scoring at a 35-month, 33-month, and 34-month level, respectively, after intervention. While this study found positive effects of early intervention on sensory processing, postural control, and language development, it is not possible to generalize the results of this study to all children in developmental stages. Therefore, future research should focus on conducting multidimensional studies on the effectiveness of early intervention services targeting early identified infants and toddlers.
Bridging the gap: fostering interactive stimming between non-speaking autistic children and their parents

Abstract:Non-speaking autistic individuals grapple with the profound impact of the 'double empathy problem' in their daily interactions with speaking others. This study rethinks the communication challenges faced by non-speaking autistic individuals, challenging traditional approaches that predominantly focus on speech and complex communication devices (AAC). By spotlighting the natural phenomenon of "interactive stimming," a powerful mode of communication among autistic individuals, this study advocates for a shift from a conventional emphasis on speech towards the foundational role of the body in autistic communication. Central to this exploration is the introduction of the Magical Musical Mat (MMM), an innovative interactive environment mapping interpersonal touch to musical sounds. Through a robust mixed-methods approach integrating video-based fieldwork and design-based research, this paper engages three non-speaking autistic children and their mothers in a 5-day empirical intervention. Results reveal significant transformations in parent–child interactions as both parents and children are acquainted with touch in a new environment. Children assert their autonomy, exploring at their own pace, and discovering sensory features of the environment. Notably, the introduction of sound prompts heightened awareness of the stims, leading to diversified and expressive stim movements. Foregrounding interpersonal touch eventually guides parents into their children's sensory activities where parents attune to the stims of their children by joining in and facilitating their expressiveness, co-creating extended, evolving patterns of repetitive cycles. The collaborative stim cycles can be likened to free improvisation, where dynamical coherence between individuals occurs through a blend of stability and active flexibility. By shifting the focus from speech to co-created sensory experiences, this paper sheds light on the value of transformative multisensory environments, envisioning a world where varied modes of communication are valued and embraced.

Abstract:The ability to perspective-take (cognitive awareness of another’s state) and empathise (emotional/affective response) are important characteristics for sensitive, co-operative and constructive parenting, which assists in developing adaptive functioning for children. For the first time, immersive virtual reality was used to place parents in the position of a child in order to assess impact on perspective-taking and empathy. This novel study was conducted with 20 non-high risk Spanish mothers (a pilot study with 12 mothers is reported in supplementary files). Mothers were virtually embodied as a 4-year-old child, experienced from the first-person perspective and with virtual and real body movements synchronised. They interacted with a ‘mother avatar’, which responded either in a Positive or Negative way. Participants reported a strong body ownership illusion for the child body that led to cognitive, emotional and physical reactions. Experiencing negative maternal behavior increased levels of empathy. In addition, the Negative mother led to increased feelings of fear of violence. Physiological data indicated greater stress in the Negative than Positive condition. Although further research is required to assess the effectiveness of such methods, any improvement in empathy that leads to a change in parenting behavior has the potential to impact on developmental outcomes for children.

Abstract:Background: Interindividual variability is important in the evolution of adaptative profiles of children with ASD having benefited from an early intervention make up for deficits in communication, language and social interactions. Therefore, this paper aimed to determine the nature of factors influencing the efficacy variability of a particular intervention technique i.e., “Play-based communication and behavior intervention” (PCBI). Methods: The participants comprised 70 13–30-month-old toddlers with ASD enrolled in PCBI for 12 weeks. The Autism Treatment Evaluation Checklist (ATEC) was used to evaluate the efficacy of PCBI. Video recordings of 5 min of free-play before and after PCBI were used to examine behaviors of mothers and children and parent-child dyadic synchrony. Hierarchical multiple regression analyses and machine learning algorithms were performed to explore the effect of these potential predictors (mothers' factors, children's factors and videotaped mother-child interaction) of intervention efficacy. Results: The hierarchical regression analysis and the machine learning algorithms indicated that parenting stress, level of completion of training at home and mother-child dyadic synchrony were crucial factors in predicting and monitoring the efficacy of PCBI. Conclusions: In summary, the findings suggest that PCBI could be particularly beneficial to children with ASD who show a good performance in the mother-child dyadic synchrony evaluation. A better dyadic mother-child synchrony could enhance the PCBI efficacy through adapted emotional and behavioral responses of the mother and the child and has a beneficial influence on the child's psychological development.

Abstract:Infant attention is a cognitive function that underlines sensory–motor integration processes at the interface between the baby and the surrounding physical and socio-relational environment, mainly with the caregivers. The investigation of the role of non-visual inputs (i.e., vocal and tactile) provided by the caregivers in shaping infants' attention in the context of visual impairment is relevant from both a theoretical and clinical point of view. This study investigated the social attention (i.e., gaze orientation) skills in a group of visually impaired (VI) and age-matched sighted controls (SCs) between 9 and 12 months of age. Moreover, the role of VI severity and maternal vocalizations and touch in shaping the social attention were investigated. Overall, 45 infants and their mothers participated in a video-recorded 4 min interaction procedure, including a play and a still-face episode. The infants' gaze orientation (i.e., mother-directed, object-directed, or unfocused) and the types of maternal vocalizations and touch (i.e., socio-cognitive, affective) were micro-analytically coded. Maternal vocalizations and touch were found to influence gaze orientation differently in VI infants compared SCs. Moreover, the group comparisons during the play episode showed that controls were predominantly oriented to the mothers, while VI infants were less socially oriented. Visual impairment severity did not emerge as linked with social attention. These findings contribute to our understanding of socio-cognitive developmental trajectories in VI infants and highlight the need for tailored interventions to promote optimal outcomes for VI populations.

A Case Study in an Italian Community-Based Setting

Abstract:Background: Gestures or manual signing are valid options for augmentative and alternative communication. However, the data in the literature are limited to a few neurodevelopmental disorders, and less is known about its application in the community setting. Objectives: This case report explores the feasibility and preliminary efficacy of tact training with augmentative gestural support intervention for a child affected by a language disorder with challenging behaviors in a community setting. Methods: Baseline assessments were conducted using the Verbal Behavior Milestone Assessment and Placement Program (VB-MAPP) and Griffiths Mental Developmental Scale-III (GMDS-III). The patient received six months of standard treatment, consisting of neuropsychomotor and speech therapy each twice a week, with improved cooperation in proposed activities, but no improvement in language. Afterward, a total of 24 sessions of tact training with augmentative gestural support interventions were performed. Data were collected by two independent observers and analyzed to measure language and behavioral outcomes. Results: VB-MAPP scores increased form minimal communication and social interaction at T0 (baseline) to improved compliance but unchanged language skills at T1 (after standard therapy). After tack training with augmentative gestural support (T2), VB-MAPP scores showed significant improvements, with notable increases in verbal operants, independence in communication, and intersubjectivity skills. GMDS-III scores at T2 also demonstrated growth in social, communicative, and cognitive skills. Additionally, challenging behaviors were reduced by more than 70% and nearly resolved by the end of the intervention. Conclusions: Personalized approaches appear to be essential for interventions tailored to developmental age. Further research is needed to determine the effectiveness of these approaches for other neurodevelopmental disorders, identify patient characteristics that may be predictors of outcomes to tailor the intervention, and explore the generalization of the results obtained with these strategies.

Abstract:Importance: The scope of early intervention (EI) programs, which mostly focus on motor skills, needs to be expanded. Objective: To examine the effects of an EI on developmental domains and parent-child interactions. Design: Randomized controlled study. Participants: Children ages 24-36 mo with developmental delay (DD; intervention group, n = 30; control group, n = 40). Setting: Pediatric occupational therapy unit of Hacettepe University. Intervention: The intervention group received an intervention that used the Goal Activity and Motor Enrichment (GAME) approach and a home program, whereas the control group received a home program only. Both groups received the same intervention dosage. Outcomes and measures: Developmental domains were assessed using the Bayley Scales of Infant and Toddler Development Third Edition (Bayley III) and the Ages and Stages Questionnaires (ASQ). Sensory processing was assessed with the Infant/Toddler Sensory Profile 2 (ITSP-2), social-emotional development with the ASQ:Social-Emotional (ASQ:SE), and parent-child interaction with the Parenting Interactions with Children: Checklist of Observations Linked to Outcomes (PICCOLO™). A compliance checklist was used to determine compliance with the home program. Results: A significant between-groups difference was found on all Bayley III and PICCOLO subscales and on the Gross Motor, Fine Motor, Problem-Solving, and Social-Emotional domains of the ASQ and ASQ:SE in favor of the intervention group (p < .05). The intervention group showed more typification (normal sensory processing) in sensory quadrants and general processing. Both groups were 100% compliant with the home program. Conclusions and relevance: The GAME-based intervention for children with DD, including sensory and cognitive activities, is effective in improving developmental domains and parent-child interactions. Plain-Language Summary: The scope of early intervention programs mostly focuses on motor skills and needs to be expanded to include the essential roles of children's sensory and cognitive abilities. This study provides important insights into early interventions by occupational therapists and the impact on children with developmental delay and parent-child interactions. We found that the Goal Activity and Motor Enrichment (GAME) intervention for children with developmental delay, which includes sensory and cognitive activities, is effective in improving developmental domains and parent-child interactions.
Touch and look: The role of affective touch in promoting infants' attention towards complex visual scenes

Abstract:In a complex social environment, stimuli from different sensory modalities need to be integrated to decode communicative meanings. From very early in life, infants have to combine a multitude of sensory features with social and affective attributes. Of all senses, touch constitutes a privileged channel to carry affective‐motivational meanings and foster social connection. In the present study, we investigate whether sharing sensory stimulation that varies for its affective value differentially affects infants' attention towards visual stimuli. 6 to 11‐month‐old infants (N = 42) were familiarized with two characters respectively matched with tactile (affective or non‐affective) and auditory stimulation; then repeatedly exposed to scenes where the two characters moved towards target objects. Our results showed a main effect of stimulation (sound vs. touch) on looking times during familiarization, with longer looking times when sound is provided. During scenes presentation, a main effect of the type of touch (affective vs. non affective) emerged, with longer looking times in infants that previously experienced affective touch, suggesting that this sensory experience may critically engage the self and modulate infant attention. Overall, these findings suggest that while sound acts as attention getter, affective touch supports sustained attention towards complex visual scenes beyond the stimulation period itself.

Abstract:Social communication skills, especially eye contact and joint attention, are frequently impaired in autism spectrum disorder (ASD) and predict functional outcomes. Applied behavior analysis is one of the most common evidence-based treatments for ASD, but it is not accessible to most families in low- and middle-income countries (LMICs) as it is an expensive and intensive treatment and needs to be delivered by highly specialized professionals. Parental training has emerged as an effective alternative. This is an exploratory study to assess a parental intervention group via video modeling to acquire eye contact and joint attention. Four graded measures of eye contact and joint attention (full physical prompt, partial physical prompt, gestural prompt, and independent) were assessed in 34 children with ASD and intellectual disability (ID). There was a progressive reduction in the level of prompting required over time to acquire eye contact and joint attention, as well as a positive correlation between the time of exposure to the intervention and the acquisition of abilities. This kind of parent training using video modeling to teach eye contact and joint attention skills to children with ASD and ID is a low-cost intervention that can be applied in low-resource settings.

Abstract:Infancy, a formative period for development, has been identified as an advantageous time to provide family-centered support for children with delays. Families should be included as stakeholders during development of such interventions to ensure social value. We describe a preliminary randomized controlled trial evaluating Infant Achievements (IA), an 8-week (16-session) parent-mediated intervention for parents of infants 8–12 months old with social and communication delays. This study reports our qualitative examination of the intervention's social validity. We conducted focus groups with seven IA parent participants and analyzed their responses using conventional content analysis. Five themes emerged: (a) difficulty identifying resources to address concerns; (b) seeking validation of concerns; (c) seeking support to enhance children's development; (d) experiencing a supportive coaching relationship; and (e) creating meaningful interactions to support children's communication and social skills. Our results emphasize the necessity of partnering with families in the development of evidence-based, family-centered interventions for infants with developmental delays to maximize their social value.

AACessTalk: Fostering Communication between Minimally Verbal Autistic Children and Parents with Contextual Guidance and Card Recommendation

Abstract:As minimally verbal autistic (MVA) children communicate with parents through few words and nonverbal cues, parents often struggle to encourage their children to express subtle emotions and needs and to grasp their nuanced signals. We present AACessTalk, a tablet-based, AI-mediated communication system that facilitates meaningful exchanges between an MVA child and a parent. AACessTalk provides real-time guides to the parent to engage the child in conversation and, in turn, recommends contextual vocabulary cards to the child. Through a two-week deployment study with 11 MVA child-parent dyads, we examine how AACessTalk fosters everyday conversation practice and mutual engagement. Our findings show high engagement from all dyads, leading to increased frequency of conversation and turn-taking. AACessTalk also encouraged parents to explore their own interaction strategies and empowered the children to have more agency in communication. We discuss the implications of designing technologies for balanced communication dynamics in parent-MVA child interaction.

Abstract:This study aimed to explore the effects of a four-week intensive eye-tracking intervention on children with dyskinetic cerebral palsy (DCP), focusing on goal attainment, communication competencies, stress levels, subjective workload, and caregivers' perception of psychosocial impact. A multiple case study design with non-concurrent, staggered multiple baselines was employed, involving three children aged 7, 12, and 13 years. The study included a randomized baseline period of two or three weeks, an intensive eye-tracking intervention, and a six-month follow-up. Two individual eye-tracking goals were identified and assessed using the Goal Attainment Scale, while communication competencies were evaluated with the Augmentative and Alternative Communication Profile: A Continuum of Learning. Stress levels were monitored through Heart Rate Variability measured by the Bittium Faros 360° ECG Holter during eye-tracking tasks. Subjective workload and psychosocial impact were assessed using pictograms and the Psychosocial Impact of Assistive Devices Scale, respectively. Descriptive statistics were applied for analysis. All participants attained and retained their eye-tracking goals, regardless of their initial functional profiles or prior experience with eye-tracking technology. Post-intervention improvements in communication competencies were maintained at the six-month follow-up. Variations in stress levels, subjective workload, and psychosocial impact were observed among participants across different phases of the study, aiding the interpretation of the results. The study concludes that a structured, tailored, four-week intensive eye-tracking intervention can yield successful results in children with DCP, irrespective of their baseline communication abilities or functional profile. Recommendations for future research, including more robust methodologies and reliable computerized tests, are provided.

Abstract:Purpose: We investigated effects of an adaptive telehealth coaching model on caregiver implementation of enhanced milieu teaching (EMT) with newly diagnosed toddlers with autism. Method: Three caregiver–child dyads participated in a multiple-baseline-across-behaviors, single-case design. Caregivers were taught EMT via telehealth using the teach–model–coach–review approach. EMT strategies were taught sequentially in four components corresponding to design tiers. Caregivers reported their needs for support and adaptation via weekly surveys. Primary outcomes were measures of caregiver's implementation, including (a) a fidelity checklist for wholistic use of EMT and (b) a percentage of correct use of a subset of key EMT strategies (e.g., matched turns, target talk, expansions, play actions, milieu episodes). Generalization and maintenance of caregiver strategy use in uncoached home activities were measured. The number of different words used by children was measured as a secondary, descriptive outcome. Social validity data were collected through ratings and interviews at the end of the study. Results: There was a functional relation between the intervention and caregiver's implementation of EMT for all dyads. Caregiver's use of EMT strategies often generalized and maintained post-intervention. Child response to intervention was variable. Social validity data indicated that the model was beneficial to caregivers and children. Conclusions: An adaptive telehealth coaching model is effective for teaching caregivers of toddlers with autism to implement EMT and potentially helps to bridge the gap between diagnosis and comprehensive intervention. Further exploration of the relation between caregiver fidelity and dosage of active ingredients and child spoken language outcomes is needed.
