Abstract:Abstract OBJECTIVES To review our surgical experiences in patients with complex pathology of the aortic arch who have undergone antero-lateral thoracotomy with partial sternotomy (ALPS). METHODS From October 2019 to November 2023, 23 patients underwent one-stage repair of complex pathology of the aortic arch through the ALPS approach. The mean age was 61.9 ± 16.7 years old. The aortic pathology was as follows: aorta-related infection in eleven (aorto-oesophageal fistula: four, graft infection: six, native aortic infection: one), aortic dissection in nine including shaggy aorta in two, non-dissecting aneurysm in one, and coarctation of the aorta (CoA) in two. RESULTS The extent of aortic replacement was as follows: 18 patients underwent replacement from either the sinotubular junction or the ascending aorta to the descending aorta, 1 patient underwent it from the aortic root to the descending aorta (redo Bentall procedure and extensive aortic arch replacement), 3 patients underwent it from the aortic arch between the left carotid artery and left subclavian artery to the descending aorta, and 1 patient underwent the descending aortic replacement. Ten patients underwent omentopexy, latissimus dorsi muscle flap installation, or both procedures. The hospital mortality rate was 13.0% (3/23). The overall survival and freedom from aortic events were 73.3%±10.2% and 74.1%±10.2%, respectively, at the 3-year follow-up. There was an absence of aorta-related mortality, and no recurrent infections were identified. CONCLUSIONS The short-term outcomes using the ALPS approach for the treatment of complex pathologies of the aortic arch were feasible. Further studies will be required to determine the long-term results.

Abstract:BACKGROUND:High mortality and morbidity rates are associated with the conventional surgery for aortic arch aneurysm because of cardiopulmonary bypass and deep hypothermic circulatory arrest. In this report, we describe a "hybrid" treatment for aortic arch aneurysm that combines the surgical debranching procedure and the stenting technique.METHODS:A surgical bypass graft is created from the ascending aorta to every main branch of the aortic arch. Subsequently, an endovascular stent graft is deployed retrogradely through the femoral artery with the aid of digital subtraction angiography.RESULT:The patient was discharged on postoperative day 7. A computed tomography scan was routinely performed at 1 week postoperatively to confirm the good patency of all grafts and the stent.CONCLUSION:The feasibility and the benefit of the hybrid treatment for aortic arch aneurysm are confirmed.
Endovascular Total Arch Repair Using In Situ Fenestration for Arch Aneurysm and Chronic Type A Dissection

Abstract:Background: Total arch replacement and ascending aorta and arch replacement are the gold standard treatments for aortic arch aneurysm and are possible treatment strategies for chronic type A dissection, with good reported outcomes. However, because total arch replacement is extremely invasive, it can be difficult to perform in some patients. We designed an endovascular total arch repair procedure with the use of in situ fenestration and commercially available devices, and we present our initial experience. Methods: Seven patients (chronic type A aortic dissection, n = 3; thoracic arch aneurysm, n = 4) who underwent endovascular total arch repair with the use of in situ fenestration thoracic endovascular aortic repair from a zone 0 landing were retrospectively analyzed. All the procedures were performed under general anesthesia and cardiopulmonary bypass. The triple branches were manually punctured in a retrograde manner with the use of an 18-gauge/30-cm needle or a 21-gauge aspiration needle, and all the branches were reconstructed with the use of stent grafts; balloon touch-up was performed if appropriate. Results: The procedure was successful in 6 of the 7 patients; the remaining 1 patient additionally underwent an axillary-axillary artery bypass during the operation because the left subclavian artery was difficult to puncture. All the patients had an acceptable postoperative course, with no 30-day and in-hospital deaths. None of the patients had endoleaks, and all the patients exhibited a reduction in aneurysm diameter or thrombosed false lumen during a mean follow-up period of 17.6 months. Conclusions: Endovascular total arch repair with the use of in situ fenestration can be performed with commercially available devices with acceptable interim results.

Abstract:Abstract Background Abdominal aortic aneurysms (AAA) with iliac artery occlusive diseases are not uncommon. When an occlusion extends from iliac artery to common femoral artery (CFA), adjunctive procedures such as endareterectomy of CFA and angioplasty of iliac artery are performed prior to endovascular aneurysm repair (EVAR). Alternatively, aorto-uni-iliac stentgrafting with femoro-femoro bypass surgery could be performed. If run off vessels such as superficial femoral artery (SFA) and profunda femoris artery (PFA) are both occluded in addition to the CFA, surgical procedures may become extremely complex, with much longer procedure time. We present an unusual case of AAA with arterial occlusion ranging from external iliac artery (EIA) to superficial and profunda femoris arteries, which was fully managed with endovascular means. Case presentation The patient was a 76 year old male who was found incidentally to have a fusiform infrarenal AAA, the size of which was 55 mm in maximal transverse diameter. Despite the occlusions of left EIA, CFA and proximal parts of SFA and PFA, he did not have ischemic symptoms in his left leg due to the development of abundant collateral networks from left internal iliac artery. The patient had a past history of endarterectomy of left CFA. Since a repeated endarterectomy or interposition grafting of the CFA were deemed extremely difficult, without any patent runoff vessel, EVAR was performed using the occluded vessel simply as a conduit for the delivery of the endograft, without revascularizing the vessel. An angioplasty balloon was delivered from right CFA to the occluded left CFA through a subintimal space. A percutaneous puncture of the expanded balloon was done at the occluded left CFA under fluoroscopy, inserting the guidewire into the punctured balloon, finally establishing the through and through wire. EVAR was successfully performed using AFX unibody stentgraft without any complication. Conclusion AAA with access vessel occlusions from EIA to SFA was successfully treated with EVAR with the aid of the balloon oriented percutaneous puncture technique. Having the technique as an armamentarium can broaden the application of EVAR for AAA with the complicated access.

Abstract:Abstract Background Aortic aneurysms involving the proximal aortic arch, which require hemiarch-type repair, typically require circulatory arrest with antegrade cerebral perfusion. Left carotid antegrade cerebral perfusion (LCP) via distal arch cannulation without circulatory arrest was used in this study’s patient population. The goal was to assess the operative efficiency and clinical outcomes of using a distal arch cannulation technique that would not require any hypothermic circulatory arrest (HCA) time compared with more traditional brachiocephalic artery cannulation with right-sided unilateral antegrade cerebral perfusion (RCP) and HCA. Methods A single-center retrospective review of patients with replacement of the distal ascending aorta involving the proximal arch was performed. Patients with an intramural hematoma or dissection were excluded. Between January 2015 and December 2019, 68 adult patients had undergone a hemiarch repair because of aneurysmal disease. Analysis of baseline demographics, operative data, and clinical outcomes was performed. Results Comparing the 68 patients: 21 patients were treated with RCP (via brachiocephalic artery graft with HCA), and 47 patients were treated with LCP (via distal aortic arch cannulation with cross-clamp between the brachiocephalic and left common carotid arteries without HCA). Baseline characteristics and outcomes were evaluated for both groups. The LCP group was younger (LCP median [IQR] age, 60 [53-65] years vs RCP median [IQR] age, 67 [59-71] years]. Sex, race, body mass index, comorbidities, and ejection fraction were similar between the groups. Cardiopulmonary bypass time (LCP, 123 minutes vs RCP, 149 minutes) and unilateral cerebral perfusion time (LCP, 17 minutes vs RCP, 22 minutes) were longer in the RCP group. Bleeding, prolonged ventilatory support, kidney failure, and length of stay were similar. In-hospital mortality was 2% in the LCP group vs 0% in the RCP group. Stroke occurred in 2 patients (4.2%) in the LCP group and in 0% of the RCP group. Mortality at 6 months in the LCP and RCP groups was 3% and 10%, respectively. Conclusion Distal arch cannulation with LCP without HCA is a reasonable and safe alternative strategy for patients requiring hemiarch replacement for aneurysmal disease. This technique may provide additional benefits by avoiding circulatory arrest in these complex cases.

Three-Dimensional Printing to Guide the Application of Modified Prefenestrated Stent Grafts to Treat Aortic Arch Disease

Abstract:Background: The aim of this study was to summarize the experience and outcomes of total endovascular repair of aortic arch disease using three-dimensional (3D) printing to guide the application of modified prefenestrated/branched stent grafts. Patients and methods: From April 2018 to March 2019, 17 patients with aortic arch disease were treated in our department. Five patients had an aortic arch aneurysm and 12 had undergone an aortic arch dissection. Thirteen men and 4 were women, with an average age of 57.82 ± 10.47 years. Preoperatively, a 3D-printed model of the aorta was made according to computed tomography data. Then, under the guidance of the 3D-printed aortic model, modified prefenestrated/branched stent grafts were prepared, and the diameter of the stent grafts was reduced intraoperatively by a physician for total endovascular repair. Aortic computed tomography angiography was performed 3 and 6 months after the surgery. Results: All procedures were completed in one stage with no conversions to sternotomy. Among all 17 patients, the operation was successful in 16. One patient was treated with a chimney graft and a stent graft fenestrated in situ because of distortion of the stent. The success rate of the technique was 94.18%. The average operation time was 4.18 ± 1.57 hr, and no patients died. No neurologic complications, such as cerebral infarction or paraplegia, were observed during the follow-up period. Conclusions: Three-Dimensional printing can be used to help guide the treatment of aortic arch disease using modified prefenestrated/branched stent grafts. This minimally invasive total treatment technique is accurate, allows quick recovery, and has a low complication rate. The short-term follow-up data show the safety and reliability of the method; however, further research and development are needed.

Abstract:Journal of Endovascular Therapy, Ahead of Print. Purpose:To demonstrate the Modified Balloon Nose Cone Technique to avoid passage of the aortic valve in endovascular branched arch repair.Technique:The technique is demonstrated in a 54-year-old patient after previous open repair of the ascending aorta and mechanical aortic valve replacement due to type A aortic dissection. The delivery system of a custom-made stent-graft with 3 inner branches was modified by subtotally sawing off its nose cone. Using a right transaxillary access, a sheath was introduced over a through-and-through (TAT) wire exiting the left groin and meeting the branched stent-graft in a rendezvous fashion. A balloon was used to bridge the gap between both sheaths. Passing the innominate artery, both sheaths were pushed into the ascending aorta to establish a loop configuration of the balloon until the correct landing zone was reached. The main body was deployed, and the supra-aortic target vessels were connected. After the procedure, the mechanical aortic valve showed unimpaired function.Conclusion:The Modified Balloon Nose Cone Technique broadens the technical armamentarium in endovascular aortic arch repair offering a feasible solution to overcome the need of aortic valve passage. Especially patients with mechanical aortic valve could benefit from this method.Clinical ImpactTo describe the feasibility and safety of the Modified Balloon Nose Cone Technique in a severely comorbid patient with residual Type A aortic dissection and history of ascending aortic repair and mechanical replacement of the aortic valve to broaden the technical armamentarium in endovascular aortic arch repair offering a feasible solution to overcome the need of aortic valve passage.

Abstract:Simple coarctation of the aorta is repaired in an infant by direct end-to-end anastomosis of the aorta or subclavian flap aortoplasty. However, some cases are not detected until late childhood. For school-age patients, greater consideration must be given to risks such as postoperative limb ischemia and the potentially harmful effects of any artificial material on future growth. Here, we describe our technique for these patients, in whom the value of direct anastomosis is uncertain, to minimize the amount of synthetic graft material used while achieving successful anatomical repair.

Abstract:Since Parodi et all reported the first successful case of endovascular treatment for abdominal aortic aneurysm (AAA) in 1991, the endovascular repair has become an important option for the surgical treatment of aortic aneurysms. 2-4 The occluder is a newly-developed device introduced intraluminally to block the blood flow in certain arteries. It was mainly adopted by cardiologists in occluding the ventricular septal defect (VSD) and atrial septal defect (ASD). 5-6 Afterwards, vascular surgeons found it could also be used in endovascular repair of aortic aneurysms, especially when the aorto-uni-iliac (AUI) stent-grafts were implanted. 7 Herein we present 6 patients with AAA, who were treated with endovascular deployment of AUI stent-grafts combined with a newly-designed occluder.

Abstract:Objectives: In recent years, minimally invasive approaches have been used with increasing frequency, even for more complex aortic procedures. However, evidence on the practicability and safety of expanding minimally invasive techniques from isolated operations of the ascending aorta towards more complex operations such as the hemiarch replacement is still scarce to date. Methods: A total of 86 patients undergoing elective surgical replacement of the ascending aorta with (n = 40) or without (n = 46) concomitant proximal aortic arch replacement between 2009 and 2023 were analyzed in a retrospective single-center analysis. Groups were compared regarding operation times, intra- and postoperative complications and long-term survival. Results: Operation times and ventilation times were significantly longer in the hemiarch replacement group. Despite this, no statistically significant differences between the two groups were observed for the duration of the ICU and hospital stay and postoperative complication rates. At ten-year follow-up, overall survival was 82.6% after isolated ascending aorta replacement and 86.3% after hemiarch replacement (p = 0.441). Conclusions: Expanding the indication for minimally invasive aortic surgery towards the proximal aortic arch resulted in comparable postoperative complication rates, length of hospital stay and overall long-term survival compared to the well-established minimally invasive isolated supracommissural ascending aorta replacement.

Abstract:Because atrophy of the jaws in edentulous patients is a major health problem, how best to reconstruct the atrophic mandible is a dilemma that frequently confronts the medical and dental professions. After loss of the natural dentition, the reduction of the residual ridges is progressive, irreversible, and cumulative. The cause and pathogenesis of mandibular atrophy is discussed. In the evaluation of a patient, the amount of residual bone at the symphysis should be measured on the lateral cephalometric radiograph as an aid to treatment planning. This measurement allows the degree of atrophy to be classified as minor, moderate, or severe. The current techniques to rehabilitate the edentulous mandible, including relative and absolute heightening techniques and implants, are reviewed with illustrative examples. A modification of a previous absolute heightening osteotomy technique utilizing pedicle bone flaps, is presented.
[Total Arch Replacement with Open Stent Grafting for Chronic Dissecting Aortic Aneurysm with Aberrant Right Subclavian Artery:Report of a Case]

Abstract:A 59-year-old man was referred to our hospital for surgery for a dissecting aortic aneurysm with an aberrant right subclavian artery( ARSA). He had a history of surgery for atrial septal defect at the age of 3 and developed Stanford type B aortic dissection at the age of 53. The maximum diameter of the aortic aneurysm was 68 mm, and the entry was located close to the ARSA origin. We established cardiopulmonary bypass using the femoral artery and vein and performed a median re-sternotomy. We performed total arch replacement with the open stent-grafting technique. The ARSA was ligated from the right thoracic cavity. Three arch branches were reconstructed in situ, and the right axillary artery was bypassed with a 9 mm Dacron graft. Six months after that operation, reduction of the false lumen was observed. This strategy is considered to be effective for chronic aortic dissection with ARSA.
Repair of Acute Type A Aortic Dissections Using Open Replacement with Triple-Branched Stent Grafts

Abstract:Background. Total arch replacement to treat type A aortic dissection is a complex procedure. To simplify the procedure, we used a triple-branched stent graft for total aortic arch replacement.Methods. Between February 2010 and June 2011, 27 patients (mean age, 52.7 +/- 11.5 years), with acute type A aortic dissection underwent open surgical placement of a triple-branched stent graft for total arch reconstruction.Results. All patients were discharged from the hospital. Mean cardiopulmonary bypass time was 160.5 +/- 25.2 minutes, aortic cross-clamp time was 85.5 +/- 18.4 minutes, and lower body arrest time was 30.2 +/- 11.8 minutes. The mean drainage after the operation was 560 +/- 120 mL, and the mean blood transfusion was 500 +/- 150 mL. The mean length of stay after surgery was 13.5 +/- 3.2 days. The mean follow-up time was 19.3 +/- 7.6 months (range, 18 to 33 months). Follow-up was 100%. Endoleaks occurred in 3 patients after the operation, and 1 of them died 9 months after the operation. Follow-up computed tomography scans at 3 months showed the elimination of a false lumen in 24 patients.Conclusions. Using triple-branched stent grafts for total arch reconstruction in acute type A aortic dissections may be an effective technique to eliminate flow in the false lumen. Abstract:Purpose: We report our initial experience with the PAS-Port proximal anastomosis system (Cardica Inc, Redwood City, CA) using full-skeletonized radial artery (RA) in patients requiring off-pump coronary artery bypass grafting. Description: The PAS-Port system (Cardica Inc) was used in 25 patients undergoing off-pump coronary artery bypass surgery. All patients received at least one RA graft using the PAS-Port system on the proximal anastomosis. The radial arteries were harvested in a fully skeletonized fashion before loading to the PAS-Port system. Evaluation: Our attempt to use the PAS-Port system for proximal anastomosis of the RA was successful in 25 anastomoses. Postoperative angiography showed 24 grafts to be widely patent. During the mean postoperative follow-up of 9.2 +/- 3.1 months, there was no cardiac-related event in any patient. Mid-term patency collected from the first 8 patients was 87.5% (mean follow-up, 12.8 +/- 2.8 months). Conclusions: The PAS-Port system does not require aortic clamping and enables the creation of uniform and widely patent anastomosis with use of RA grafts.
Proximal and extended aortic arch replacement in acute DeBakey type I aortic dissection

Abstract:Objective This study aimed to compare the short- and long-term outcomes of proximal repair vs. extensive arch surgery for acute DeBakey type I aortic dissection. Subjects From April 2014 to September 2020, 121 consecutive patients with acute type A dissection were surgically treated at our institute. Of these patients, 92 had dissections extending beyond the ascending aorta. Methods Of the 92 patients, 58 underwent proximal repair, including aortic root and/or hemiarch replacement, and 34 underwent extended repair, including partial and total arch replacement. Perioperative variables and early and late postoperative results were statistically analyzed. Results The duration of surgery, cardiopulmonary bypass, and circulatory arrest was significantly shorter in the proximal repair group ( p < 0.01). The overall operative mortality rate was 10.3% in the proximal repair group and 14.7% in the extended repair group ( p = 0.379). The mean follow-up period was 31.1 ± 26.7 months in the proximal repair group and 35.3 ± 26.8 months in the extended repair group. During follow-up, the cumulative survival and freedom from reintervention rates at 5 years were 66.4% and 92.9% in the proximal repair group, and 76.1% and 72.6% in the extended repair group, respectively ( p = 0.515 and p = 0.134). Conclusions No significant differences were found in the rates of long-term cumulative survival and freedom from aortic reintervention between the two surgical strategies. These findings suggest limited aortic resection achieves acceptable patient outcomes.
