Abstract:Introduction Electroconvulsive therapy (ECT) is a well established treatment for catatonia. Although studies are limited, ECT is safe and effective in geriatric patients even those with serious medical comorbidities. Untreated or refractory catatonia can lead to medical complications including malnutrition, venous thromboembolism, aspiration pneumonia and skin breakdown. Mortality from catatonia is typically mentioned in the context of malignant catatonia; however, non-malignant catatonia can also be fatal. The discussion of ECT from a palliative perspective is limited in the literature thus far. There is one case series by Rasmussen and Richardson (2011) outlining the role ECT plays in palliative care. Two of these cases described continuation of ECT during comfort care as a quality of life improving measure. In a series of three cases of severe catatonia from the Psychiatry Consult-Liaison Service at University of Cincinnati Medical Center, we expand this discussion of the role of ECT from a palliative perspective. Methods Case 1 is a 76 year old female with bipolar I disorder admitted for UTI and catatonia after multiple similar admissions successfully treated with ECT. ECT was discontinued. She discharged to hospice and died a month later. Case 2 is a 75 year old male with no prior psychiatric history admitted after a suicide attempt. Rapid functional decline plus catatonia triggered extensive neurological workup. He then completed 20 sessions of ECT on inpatient psychiatry with good response. On return to neurology, ECT was discontinued and catatonia recurred. When ECT resumed, he developed aspiration pneumonia. Once he stabilized, family declined further ECT. He discharged to facility then was readmitted a few days later after cardiac arrest with anoxic brain injury and ultimately palliatively extubated. Case 3 is an 84 year old male with PTSD admitted for hyperglycemia and confusion. Psychiatry was consulted and catatonia suspected. Palliative care was involved and he was DNRCC. He underwent 15 ECT sessions with good response and discharged to facility. Results In Case 1, although catatonia abated with prior ECT, her level of functioning had not returned to baseline. During the final ECT treatments, family shared that she had expressed discomfort due to confusion post ECT. She was suffering from malnutrition but a feeding tube was not in line with goals of care. They understood withdrawal of ECT was predicted to worsen catatonia and the decision was made to discharge to hospice. They were very grateful for the opportunity to communicate with her and say goodbye. Case 2 was complicated by the diagnostic uncertainty of the likely comorbid neurodegenerative process. Attribution of aspiration pneumonia to ECT was unclear as it is also an outcome of catatonia itself. His family was very grateful for the improvement patient experienced prior to resurgence of catatonia. This period allowed them to communicate. Palliative care was not involved in this case until the final admission post cardiac arrest but assistance could have been helpful earlier in the course. In Case 3, goals of care for patient and his family remained restorative and to maximize time with family. ECT was the clear choice to attempt to resolve catatonia. Conclusions These cases highlight the importance of viewing ECT from a palliative perspective. Common goals of care near end of life are maximizing time with loved ones, avoiding hospitalization and unnecessary procedures and maintaining a meaningful level of functionality. Given its overall safety and efficacy in catatonia, ECT should not be reflexively disregarded as an invasive and unnecessary procedure near end of life. However, in severe catatonia, it is a life prolonging treatment. Especially when ECT does not bring the patient back to a level of functioning and quality of life consistent with their values or causes significant distress due to side effects or burden of time/logistics, withdrawal of care should be considered. ECT also has its place as a palliative treatment in itself when it provides relief from symptoms of catatonia which prevent a patient from interacting with family, controls agitation and/or allows for a lower level of care. When catatonia becomes the driving force for a patient's medical deterioration, the psychiatrist has an essential role in engaging family in discussions of overall prognostication and course of illness. It is also important to ensure, family has an accurate sense of what the procedure entails and is not deterred solely by stigma or misunderstanding.
