Abstract:Jones-Wonni et al. provide an excellent review of current gene therapy strategies to treat individuals with hemoglobinopathies. The authors quickly and succinctly summarize the major technical strategies of gene addition via retrovirus, gene silencing via shRNA, and editing via zinc fingers, TALENs, CRISPR-Cas9, base editing, prime editing, and the even newer Programmable Addition via Site-Specific Targeting Elements (PASTE). Two of these strategies, gene addition and CRISPR-Cas9, have been FDA approved for use in hemoglobinopathies. This is an exciting time for the field of hemoglobinopathies; it is gratifying to be able to tell individuals with hemoglobinopathies and their families about the availability of a powerful new therapy. However, with this promise come several challenges.

Abstract:In the last ten years, a consistent number of clinical studies have evaluated different gene approaches for the treatment of patients with sickle cell disease (SCD) and transfusion-dependent β-thalassemia (TDT). Initial studies of gene therapy for hemoglobinopathies involved the use of lentiviral vectors to add functional copies of the gene encoding β-globin in defective CD34 cells; more recently, gene editing techniques have been used involving either CRISPR-Cas9, transcription activation-like effector protein nuclease, zinc finger nuclease, and base editing to either induce fetal hemoglobin production at therapeutic levels or to genetically repair the underlying molecular defect causing the disease. Here, we review recent gene editing studies that have started the development of a new era in the treatment of hemoglobinopathies and, in general, monoallelic hereditary diseases.

Abstract:Hemoglobinopathies, namely β-thalassemia and sickle cell disease (SCD), are hereditary diseases, characterized by molecular genetic aberrations in the beta chains of hemoglobin. These defects affect the normal production of hemoglobin with severe anemia due to less or no amount of beta globins in patients with β-thalassemia (quantitative disorder), while SCD is a serious disease in which a mutated form of hemoglobin distorts the red blood cells into a crescent shape at low oxygen levels (qualitative disorder). Despite the revolutionary progress in recent years with the approval of gene therapy and gene editing for specific patients, there is an unmet need for highlighting the mechanisms influencing hemoglobin production and for the development of novel drugs and targeted therapies. The identification of the transcription factors and other genetic modifiers of hemoglobin expression is of utmost importance for discovering novel therapeutic approaches for patients with hemoglobinopathies. The aim of this review is to describe these complex molecular mechanisms and pathways affecting hemoglobin expression and to highlight the relevant investigational approaches or pharmaceutical interventions focusing on restoring the hemoglobin normal function by linking the molecular background of the disease with the clinical perspective. All the associated drugs increasing the hemoglobin expression in patients with hemoglobinopathies, along with gene therapy and gene editing, are also discussed.

Abstract:Gene therapy has shown significant potential in treating various diseases, particularly inherited blood disorders such as hemophilia, sickle cell disease, and thalassemia. Advances in understanding the regulatory network of disease-associated genes have led to the identification of additional therapeutic targets for treatment, especially for β-hemoglobinopathies. Erythroid regulatory factor BCL11A offers the most promising therapeutic target for β-hemoglobinopathies and reduction of its expression using the commercialized gene therapy product Casgevy was approved for use in the UK and USA in 2023. Notably, the emergence of innovative gene editing technologies has further broadened the gene therapy landscape, presenting new possibilities for treatment. Intensive studies indicate that base editing and prime editing, built upon CRISPR technology, enable precise single-base modification in hematopoietic stem cells for addressing inherited blood disorders ex vivo and in vivo. In this review, we present an overview of the current landscape of gene therapies, focusing on clinical research and gene therapy products for inherited blood disorders, evaluation of potential gene targets, and the gene editing tools employed in current gene therapy practices, which provides an insight for the establishment of safer and more effective gene therapy methods for a wider range of diseases in the future.

Abstract:The root cause of sickle cell anemia has been known for seven decades, yet no curative therapies have been available other than allogeneic bone marrow transplantation, applicability is limited. Two potentially curative therapies based on gene therapy and gene editing strategies are about to enter the final stages of the FDA approval process. This review surveys the nature of these therapies and the opportunities and issues raised by the prospect of definitive genetically based therapies being available in clinical practice.

Abstract:Autologous hematopoietic stem cell (HSC)-targeted gene therapy provides a one-time cure for various genetic diseases including sickle cell disease (SCD) and β-thalassemia. SCD is caused by a point mutation (20A > T) in the β-globin gene. Since SCD is the most common single-gene disorder, curing SCD is a primary goal in HSC gene therapy. β-thalassemia results from either the absence or the reduction of β-globin expression, and it can be cured using similar strategies. In HSC gene-addition therapy, patient CD34+ HSCs are genetically modified by adding a therapeutic β-globin gene with lentiviral transduction, followed by autologous transplantation. Alternatively, novel gene-editing therapies allow for the correction of the mutated β-globin gene, instead of addition. Furthermore, these diseases can be cured by γ-globin induction based on gene addition/editing in HSCs. In this review, we discuss HSC-targeted gene therapy in SCD with gene addition as well as gene editing.

Abstract:β-Thalassemia is an inherited hematological disorder that results from genetic changes in the β-globin gene, leading to the reduced or absent synthesis of β-globin. For several decades, the only curative treatment option for β-thalassemia has been allogeneic hematopoietic cell transplantation (allo-HCT). Nonetheless, rapid progress in genome modification technologies holds great potential for treating this disease and will soon change the current standard of care for β-thalassemia. For instance, the emergence of the CRISPR/Cas9 genome editing platform has opened the door for precision gene editing and can serve as an effective molecular treatment for a multitude of genetic diseases. Investigational studies were carried out to treat β-thalassemia patients utilizing CRISPR-based CTX001 therapy targeting the fetal hemoglobin silencer BCL11A to restore γ-globin expression in place of deficient β-globin. The results of recently carried out clinical trials provide hope of CTX001 being a promising one-time therapeutic option to treat β-hemoglobinopathies. This review provides an insight into the key scientific steps that led to the development and application of novel CRISPR/Cas9–based gene therapies as a promising therapeutic platform for transfusion-dependent β-thalassemia (TDT). Despite the resulting ethical, moral, and social challenges, CRISPR provides an excellent treatment option against hemoglobin-associated genetic diseases.

Abstract:Purpose of review Gene therapy for sickle cell disease (SCD) is advancing rapidly, with two transformative products recently approved by the US Food and Drug Administration and numerous others under study. All current gene therapy protocols require ex vivo modification of autologous hematopoietic stem cells (HSCs). However, several SCD-related problems impair HSC collection, including a stressed and damaged bone marrow, potential cytotoxicity by the major therapeutic drug hydroxyurea, and inability to use granulocyte colony stimulating factor, which can precipitate severe vaso-occlusive events. Recent findings Peripheral blood mobilization of HSCs using the CXCR4 antagonist plerixafor followed by apheresis collection was recently shown to be safe and effective for most SCD patients and is the current strategy for mobilizing HSCs. However, exceptionally large numbers of HSCs are required to manufacture an adequate cellular product, responses to plerixafor are variable, and most patients require multiple mobilization cycles, increasing the risk for adverse events. For some, gene therapy is prohibited by the failure to obtain adequate numbers of HSCs. Summary Here we review the current knowledge on HSC collection from individuals with SCD and potential improvements that may enhance the safety, efficacy, and availability of gene therapy for this disorder.

Abstract:Hereditary anemia has various manifestations, such as sickle cell disease (SCD), Fanconi anemia, glucose-6-phosphate dehydrogenase deficiency (G6PDD), and thalassemia. The available management strategies for these disorders are still unsatisfactory and do not eliminate the main causes. As genetic aberrations are the main causes of all forms of hereditary anemia, the optimal approach involves repairing the defective gene, possibly through the transplantation of normal hematopoietic stem cells (HSCs) from a normal matching donor or through gene therapy approaches (either in vivo or ex vivo) to correct the patient's HSCs. To clearly illustrate the importance of cell and gene therapy in hereditary anemia, this paper provides a review of the genetic aberration, epidemiology, clinical features, current management, and cell and gene therapy endeavors related to SCD, thalassemia, Fanconi anemia, and G6PDD. Moreover, we expound the future research direction of HSC derivation from induced pluripotent stem cells (iPSCs), strategies to edit HSCs, gene therapy risk mitigation, and their clinical perspectives. In conclusion, gene-corrected hematopoietic stem cell transplantation has promising outcomes for SCD, Fanconi anemia, and thalassemia, and it may overcome the limitation of the source of allogenic bone marrow transplantation.

Abstract:Hematopoietic stem cell (HSC) gene therapy has shown potential as a therapeutic approach for thalassemia in recent years. However, a comparison of the varying gene therapy methods of HSC gene therapy in thalassemia has never been reviewed. This study aims to evaluate the utilization of HSC gene therapy approaches in animal models of thalassemia. A systematic review was conducted in five databases: PubMed, EBSCOHost, Science Direct, SCOPUS, and Proquest using a combination of the terms hematopoietic stem cell or hematopoietic stem cell or HSC, thalassemia, genetic therapy or gene therapy and animal model. Only journals published in English between 2008 and 2023 were included. This literature included six studies analyzing the use of HSC gene therapy in thalassemic mice models. The three outcomes being assessed in this review were globin levels, hematological parameters, and red blood cell (RBC) phenotypes. Gene therapy approaches for thalassemia using HSC showed significant improvement in β‐globin levels and RBC phenotypes. Phenotypic improvements were also observed. These outcomes indicate good efficacy in gene therapy for thalassemia in mice models. Furthermore, more studies assessing the efficacy of HSC gene therapy in the human model should be done in future studies.

[Expectations of established neurologists and general practitioners regarding the psychiatric clinic].

Abstract:OBJECTIVE Psychiatrists' and general practitioners' expectations regarding psychiatric hospitals were evaluated using qualitative methodology. DESIGN A content analysis was carried out of interviews with 27 psychiatrists and 24 general practitioners. The statements were analysed quantitatively to assess their relative importance. RESULTS Immediate delivery of discharge summaries and continuous patient-related communication were mentioned most frequently. In addition, psychiatrists assigned much importance to therapy referral after completion of inpatient care and to the prescription of less expensive drugs. Nearly half of all statements related to the communication process between local and clinic physicians, a quarter to aspects of referral, and a quarter to quality of inpatient care. Differences between psychiatrists and general practitioners mainly concerned aspects of medical education. CONCLUSION Taking into account these findings, more attention should be paid to psychiatrists' and general practitioners' expectations to reduce problems of interaction between clinicians and their colleagues in private practice; this might be helpful to improve the continuity of psychiatric care.

Abstract:Evolutionary spectral theory provides a means for defining a decomposition of signal energy jointly over time and frequency for processes which exhibit an oscillatory behavior. The oscillatory model of behavior effectively assumes that a process is composed sinusoidal components with slowly-varying time-dependent amplitudes. We expand evolutionary spectral theory by allowing the frequency of the sinusoidal components to vary with time. An estimator of this generalized spectrum is described and examples are presented that illustrate the relative merits of this new approach.

Abstract:Human hemoglobin, a tetramer containing two α globins and two β globins, is responsible for oxygen transportation in the body. Globin genes are clustered in the genome and their expressions are regulated by a variety of cis-acting elements and trans-acting factors, exhibiting a developmental- and tissue-specific manner. β-thalassemia and sickle cell diseases are two of the most common autosomal recessive disorders caused by mutations in the β-globin gene. Besides α- and β-globins, the human genome also has a third globin gene-γ-globin. Like β-globin, γ-globin also has oxygen-carrying capabilities. Unlike β-globin, γ-globin is mainly expressed at the fetal stage and remains intact in β-thalassemia and sickle cell disease patients. Thus, reactivating the expression of the γ-globin gene in adult patients to ameliorate their clinical symptoms has become one of the best therapeutic strategies to treat β-thalassemia and sickle cell diseases. Some drugs have been developed clinically to increase γ-globin gene expression for those patients. With the development of genome editing technologies, precision gene therapy for these diseases is underway. This review focuses on the main transcription factors and epigenetic modifiers that are involved in γ-globin gene regulation, and some applications for clinical treatment for β-thalassemia and sickle cell diseases based on these studies. We hope to provide a useful reference for in-depth studies on transcriptional regulation of γ-globin gene expression in the future.

Abstract:Photoreactivation is a DNA repair pathway that requires the presence of a photoreactivating enzyme, DNA photolyase. DNA photolyase recognizes and binds specifically to ultraviolet radiation (UVR)-induced cyclobutane pyrimidine dimers in DNA. Exposure of the photolyase-dimer complex to wavelengths in the range of 300-500 nm results, upon absorption of a photon, in the return of the dimerized pyrimidines to their monomeric form (1). While Kelner (2) is generally credited with the discovery of photoreactivation in 1949, a photoreactivation type of process was reported in 1933 by Hausser and von Oehmcke (see ref. 3), who observed that UVR-induced darkening of banana skin could be prevented by post-UVR exposure to nearUVR (366 nm) and visible light. Clear evidence for photoreactivation has been observed in numerous prokaryotes (3) and certain eukaryotes including fish (4) and marsupials (5). Evidence for photoreactivation in placental mammals is less convincing. Sutherland (6) reported in 1974 that DNA photolyase was present in human leukocytes. Since that time, photolyase has been reported to be present in human and murine cells in culture (7, 8) and in human skin (9, 10). The occurrence of photoreactivation in these studies was dependent upon the medium in which the cells were grown (7), the tissue from which extracts were prepared (9), and the manner in which the photoreactivation treatments were administered-i.e., no photoreactivation was observed following a single exposure to UVR and subsequent visible light, but 40% removal of dimers was measured following three cycles of UVR/visible light given at 2.5-hr intervals (10). In the marsupial Monodelphis domestica, removal of pyrimidine dimers by photoreactivation is readily demonstrated. The process of photoreactivation in M. domestica appears to occur in all tissues (11-13) and with a wide variety of photoreactivation protocols (unpublished observations). Attempts to measure the capacity of photoreactivation to suppress UVR-induced injury to human skin have yielded unimpressive results. As regards erythema induction in man, post-UVR exposure to longer wavelengths of light has been reported to enhance erythema induction (14) or to have a mild (15) or small (16) effect on prevention of erythema. On the other hand, photoreactivation in M. domestica clearly reduced the severity of such UVR-induced pathologic changes in the skin as erythema, edema, desquamation, hyperplasia, sunburn-cell formation and the appearance ofmelanotic and nonmelanotic tumors (17-19). In addition, photoreactivation reduced the incidence and severity of UVR-induced preneoplastic and neoplastic changes in the eyes of M. domestica (19, 20). If the same light-activated repair process found in marsupials occurs in human skin, why is it so difficult to demonstrate in humans cells in a reproducible manner? Recent results from the laboratory of A. Sancar, published in this issue of the Proceedings (21), clearly indicate that photoreactivation is either missing or of no practical significance in human cells. This observation is of considerable importance as regards the significance, or potential significance, of photoreactivation to human health. If the same lightactivated repair pathway found in marsupials occurred consistently in human skin, the detrimental effects of human exposure to UVR could be readily diminished by post-UVR exposure to the appropriate light sources, either natural or artificial, that emit radiation in the wavelength range of 300-500 nm. Thus, photoreactivation might be expected to prevent sunburn and the appearance of nonmelanoma and melanoma skin cancer in humans. However, based on the evidence at hand, it seems unlikely at this time to think that photoreactivation could serve as an effective therapy for the prevention of pathological conditions in sun-exposed human skin. 1. Current and Future Therapeutics for Treating Patients with Sickle Cell Disease

Abstract:Sickle cell disease (SCD) is the most common genetic blood disorder in the United States, with over 100,000 people suffering from this debilitating disease. SCD is caused by abnormal hemoglobin (Hb) variants that interfere with normal red blood cell (RBC) function. Research on SCD has led to the development and approval of several new SCD therapies in recent years. The recent FDA-approved novel gene therapies are potentially curative, giving patients an additional option besides a hematopoietic bone marrow transplant. Despite the promise of existing therapies, questions remain regarding their long-term pharmacological effects on adults and children. These questions, along with the exorbitant cost of the new gene therapies, justify additional research into more effective therapeutic options. Continual research in this field focuses on not only developing cheaper, more effective cures/treatments but also investigating the physiological effects of the current therapies on SCD patients, particularly on the brain and kidneys. In this article, we undertake a comprehensive review of ongoing clinical trials with completion dates in 2024 or later. Our exploration provides insights into the landscape of current therapeutics and emerging novel therapies designed to combat and potentially eradicate SCD, including the latest FDA-approved gene therapies.

Abstract:Sickle cell disease reflects a complex interplay of genetics, hematology, and clinical symptoms. The intricate pathogenesis of the disease offers a landscape with various targets and treatment options. Anticipated advancements in treatment include combining fetal hemoglobin inducers with other drug classes to improve the efficacy beyond monotherapy. Sickle cell disease (SCD) is an autosomal recessive genetic disorder that occurs due to the point mutation in the β‐globin gene, which results in the formation of sickle hemoglobin (HbS) in the red blood cells (RBCs). When HbS is exposed to an oxygen‐depleted environment, it polymerizes, resulting in hemolysis, vaso‐occlusion pain, and impaired blood flow. Still, there is no affordable cure for this inherited disease. Approved medications held promise but were met with challenges due to limited patient tolerance and undesired side effects, thereby inhibiting their ability to enhance the quality of life across various individuals with SCD. Progress has been made in understanding the pathophysiology of SCD during the past few decades, leading to the discovery of novel targets and therapies. However, there is a compelling need for research to discover medications with improved efficacy and reduced side effects. Also, more clinical investigations on various drug combinations with different mechanisms of action are needed. This review comprehensively presents therapeutic approaches for SCD, including those currently available or under investigation. It covers fundamental aspects of the disease, such as epidemiology and pathophysiology, and provides detailed discussions on various disease‐modifying agents. Additionally, expert insights are offered on the future development of pharmacotherapy for SCD.

Abstract:Hemoglobinopathies are among the most common monogenic diseases worldwide. Approximately 1–5% of the global population are carriers for a genetic thalassemia mutation. The thalassemias are characterized by autosomal recessive inherited defects in the production of hemoglobin. They are highly prevalent in the Mediterranean, Middle East, Indian subcontinent, and East and Southeast Asia. Due to recent migrations, however, the thalassemias are now becoming more common in Europe and North America, making this disease a global health concern. Currently available conventional therapies in thalassemia have many challenges and limitations. A better understanding of the pathophysiology of β-thalassemia in addition to key developments in optimizing transfusion programs and iron-chelation therapy has led to an increase in the life span of thalassemia patients and paved the way for new therapeutic strategies. These can be classified into three categories based on their efforts to address different features of the underlying pathophysiology of β-thalassemia: correction of the globin chain imbalance, addressing ineffective erythropoiesis, and improving iron overload. In this review, we provide an overview of the novel therapeutic approaches that are currently in development for β-thalassemia.

Therapeutic perspective for children and young adults living with thalassemia and sickle cell disease

Abstract:Hemoglobinopathies, including thalassemias and sickle cell disease, are the most common monogenic diseases worldwide, with estimated annual births of more than 330,000 affected infants. Hemoglobin disorders account for about 3.4% of deaths in children under 5 years of age. The distribution of these diseases is historically linked to current or previously malaria-endemic regions; however, immigration has led to a worldwide distribution of these diseases, making them a global health problem. During the last decade, new treatment approaches and novel therapies have been proposed, some of which have the potential to change the natural history of these disorders. Indeed, the first erythroid maturation agent, luspatercept, and gene therapy have been approved for beta-thalassemia adult patients. For sickle cell disease, molecules targeting vaso-occlusion and hemoglobin S polymerization include crizanlizumab, which has been approved for patients ≥ 16 years, voxelotor approved for patients ≥ 12 years, and L-glutamine for patients older than 5 years. Conclusion: We herein present the most recent advances and future perspectives in thalassemia and sickle cell disease treatment, including new drugs, gene therapy, and gene editing, and the current clinical trial status in the pediatric populations. What is Known: • Red blood cell transfusions, iron chelation therapy and hematopoietic stem cell transplantation have been the mainstay of treatment of thalassemia patients for decades. • For sickle cell disease, until 2005, treatment strategies were mostly the same as those for thalassemia, with the option of simple transfusion or exchange transfusion. In 2007, hydroxyurea was approved for patients ≥ 2 years old. What is New: • In 2019, gene therapy with betibeglogene autotemcel (LentiGlobin BB305) was approved for TDT patients ≥ 12 years old non β0/β0 without matched sibling donor. • Starting from 2017 several new drugs, such as L-glutamine (approved only by FDA), crizanlizumab (approved by FDA and EMA for patients ≥ 16 years), and lastly voxelotor (approved by FDA and EMA for patients ≥ 12 years old).
CRISPR/Cas-based gene editing in therapeutic strategies for beta-thalassemia

Abstract:Beta-thalassemia (β-thalassemia) is an autosomal recessive disorder caused by point mutations, insertions, and deletions in the HBB gene cluster, resulting in the underproduction of β-globin chains. The most severe type may demonstrate complications including massive hepatosplenomegaly, bone deformities, and severe growth retardation in children. Treatments for β-thalassemia include blood transfusion, splenectomy, and allogeneic hematopoietic stem cell transplantation (HSCT). However, long-term blood transfusions require regular iron removal therapy. For allogeneic HSCT, human lymphocyte antigen (HLA)-matched donors are rarely available, and acute graft-versus-host disease (GVHD) may occur after the transplantation. Thus, these conventional treatments are facing significant challenges. In recent years, with the advent and advancement of CRISPR (clustered regularly interspaced short palindromic repeats)/Cas9 (CRISPR-associated protein 9) gene editing technology, precise genome editing has achieved encouraging successes in basic and clinical studies for treating various genetic disorders, including β-thalassemia. Target gene-edited autogeneic HSCT helps patients avoid graft rejection and GVHD, making it a promising curative therapy for transfusion-dependent β-thalassemia (TDT). In this review, we introduce the development and mechanisms of CRISPR/Cas9. Recent advances on feasible strategies of CRISPR/Cas9 targeting three globin genes (HBB, HBG, and HBA) and targeting cell selections for β-thalassemia therapy are highlighted. Current CRISPR-based clinical trials in the treatment of β-thalassemia are summarized, which are focused on γ-globin reactivation and fetal hemoglobin reproduction in hematopoietic stem cells. Lastly, the applications of other promising CRISPR-based technologies, such as base editing and prime editing, in treating β-thalassemia and the limitations of the CRISPR/Cas system in therapeutic applications are discussed.
