Abstract:The goal of this clinical report was to present an alternative to traditional flat bone reduction guides, using a custom-designed 3-dimensional (3D)–printed guide according to the future gingival margin of the planned dentition. A 61-year-old woman with concerns regarding her smile appearance was presented. The initial examination revealed excessive gingival show accompanied by excessive overjet. The dentition was in a failing situation. The proposed treatment plan, relying on the sufficient amount of bone and keratinized tissue, consisted of recontouring of the alveolar ridge and gingiva and placement of 6 implants and an FP-1 prosthesis after extraction of all remaining maxillary teeth. Digital smile design was completed, and a fully digitally guided surgery was planned. This consisted of using 3 surgical guides, starting with the fixation pin guide, continuing with the scalloped hard- and soft-tissue reduction guide, and finally the implant placement template. Following the surgery, the patient received a temporary restoration, and on the 4-month follow-up, a new polymethyl meta-acrylate temporary prosthesis was delivered. The patient's 7-month follow-up is presented in the article. The report of this triple-template guided surgery indicated that digital 3D planning is a considerably predictable tool to properly establish and evaluate future occlusal plane, smile line, and lip support. Scalloped guides seem to be an excellent alternative to conventional bone reduction guides since they require less bone removal and improve patient comfort during surgery.

Abstract:Computer-aided design and computer-aided manufacturing (CAD/CAM) dentistry have significantly changed workflows in recent years. Restorations and devices can now be digitally designed and 3D-printed for dental care purposes. This clinical case report provides straightforward protocols for the digital design and 3D manufacture of gingivectomy and tooth preparation guides. These types of guides improved the gingival architecture of the anterior teeth and provided controllable tooth preparations prior to labial ceramic veneers. Thoughtful clinical evaluation started with listening to the patient's chief complaint and extra- and intra-oral evaluations. Then a digital wax-up was performed, followed by an intra-oral mock-up, to evaluate the shape of the proposed restorations. After patient acceptance, the clinical procedure started with the gingivectomy and tooth preparation. Hand-crafted porcelain veneers were bonded under rubber dam isolation to avoid any contamination and maximize the bonding protocol. The esthetic and functional demands were fully satisfied. Predictable outcomes can be obtained whenever a meticulous evaluation and execution of all the steps are performed. Three dimensional printing technology allows the fabrication of devices such as gingivectomy and tooth reduction guides that help accomplish the desired results.

Abstract:AIM To describe a digital workflow utilizing 3D printing technology to guide esthetic crown lengthening and control tooth preparation. CLINICAL CONSIDERATIONS After the initial intraoral and face scans, an esthetic treatment plan was performed digitally based on the patient's personality and face type using artificial intelligence-based 3D smile design software. A 3D-printed tray relined with silicone over a 3D-printed model was used for the mock-up. A 3D-printed guide was implemented to assist esthetic crown lengthening by incorporating, simultaneously, information about the desired free gingival line and the alveolar bone level. Based on the initial planning, a set of reduction guides was 3D printed to check and correct the tooth preparation. Prior to the start of construction of the final monolithic restorations, their design was verified using 3D-printed prototypes. A stabilization splint was digitally designed and 3D printed to protect and maintain the final result. CONCLUSIONS Technologic advances can improve the predictability of an interdisciplinary esthetic approach. Digital planning can be transferred to clinical reality using a digital workflow, utilizing a set of appropriate 3D-printed guides, which can help to control clinical procedures based on the initial planning. CLINICAL SIGNIFICANCE By following the proposed step-by-step workflow, clinicians can achieve predictable results through an interdisciplinary approach, guiding both the periodontal plastic surgery and the restorative treatment after an individualized CAD/CAD procedure for 3D-printed guides.

Abstract:This paper proposed a novel method for dental implant guide, based on computer aided design technique with CT data and dental 3D data. We analyze the patient's 3D dental data, and then plan the implant area and support area interactively. With the help of 2D image, we can accurately calculate the anatomical implants direction and a safe distance. With biomechanical principles as well as aesthetical and functional requirements as precondition, this method can make full use of the remaining teeth to ensure the support's stability. First of all, this method needs import and preprocess the model which needs repairing. Secondly, the implant direction was automatically computed. After that, the implant and support area was designed semi-automatically. Then, we design the dental platform and implant parametrically with the help of CT image registration. Finally, the dental implant guide is generated after Boolean operations. In the end of this paper, a case research explained the whole process of surgical guide. The results validated the correctness and feasibility of this method which has a great significance to enhance the quality and accuracy of implant surgery.

Abstract:Objective: complex rehabilitations present multiple difficulties, regarding both the planification of the surgery and the design of the prothesis. A digital approach can support the workflow, as well as the degree of intraoperative precision, and improve the long-term prognosis. Methods: A surgical guide was designed for implant placement. An extensive regeneration of the upper jaw was performed with contextual implant insertion, and a delayed load rehabilitation was chosen. After four months, a second surgery and a simultaneous soft tissue augmentation was performed, and a 3D-printed temporary restoration was placed. After another two months, new dental and facial scans, smile design, and facial bite registrations were obtained. Upper and lower dentures were built using an exclusively digital workflow. Both metal substructures were passivated and cemented in one session; in the following appointment, the aesthetic and occlusal checks were carried out. During the third visit, both prostheses were delivered. Results: Careful case planning and the surgical guide made it possible to achieve primary stability and acceptable emergence profiles in an extremely reabsorbed upper jaw. Leukocyte-Platelet Rich Fibrin (L-PRF) made the extensive bone regeneration more approachable and lowered the post-operative pain and swelling, while speeding up the soft tissue healing process. During the re-entry surgery, the volumes of soft tissues were increased to improve aesthetics, and the amount of keratinized gingiva around the six implants was also increased. Smile design and facial scans have provided the means to create acceptable aesthetics and function in a few sessions with minimal patient discomfort. Conclusions: Computer-assisted implantology is a safe and precise method of performing dental implant surgery. Preliminary studies have a high degree of accuracy, but further studies are needed to arrive at a fully digital clinical protocol at all stages.

Abstract:The provisional prosthesis is a prerequisite for prosthodontic rehabilitation. The purpose of this technique was to register facial and intraoral scans predictably. A 3D virtual patient was created through facial scans, intraoral scans, digital face-bow transfer, and digital cross-articulation technique. This virtual patient allowed predictable positioning of intraoral scans to a virtual articulator by using digital face-bow transfer. The resulting virtual patient facilitated the design of definitive prostheses following a facially generated treatment planning principle. In addition, the virtual articulator was used to improve occlusal design on the definitive prostheses and reduce the need for intraoral adjustment.

Abstract:This article describes a case with a full digital procedure to prefabricate an implant-supported interim restoration based on the preoperative digital implant planning. A fully guided surgical template is designed and printed, and then an interim restoration is fabricated based on the planned implant position through a dental computer-aided design (CAD) software. Once the implant was placed at the predetermined position through the fully guided surgical guide, the prefabricated interim restoration could be inserted immediately after the surgery, which can guide the healing of the soft tissue and enhance the esthetic outcomes. This novel technique eliminates the conventional impression making to insert an implant-supported interim restoration immediately after the implant placement surgery, which can guide the healing of the soft tissue, minimize the chairside time and optimize the clinical workflow.

Abstract:Objective The aim of this report is to present the workflow of a surgical procedure involving U‐shaped splitting in an envelope flap with simultaneous implant placement using two digital guides for patients with insufficient bone width and labial concavity. Clinical Consideration Two digital guides and an envelope flap are proposed in this report. An implant drilling guide and an osteotomy guide guarantee the precise execution of surgical procedures. Using an envelope flap enhances the integrity of the labial mucoperiosteum, minimizing surgical trauma and improving the effect of bone augmentation. Conclusion With the assistance of two digital guides, precise U‐shaped splitting in an envelope flap can minimize surgical trauma and have a significant effect on horizontal bone augmentation. Clinical Significance This workflow considers the implant positions, the scope of osteotomy, and the kind of flap. It ensures minimal surgical trauma and excellent bone augmentation effect, which means predictable esthetic outcomes and stability in oral implantology, and provides a considerable solution for patients with insufficient bone width and labial concavity.

Abstract:This case report presented a fully digital workflow in esthetic dental restoration. A 51-year-old female patient was referred to BDMS Wellness Clinic due to a fracture of the maxillary left central incisor. An immediate dental implant was planned to restore tooth 21 with esthetic crown lengthening of upper front teeth and new zirconia crowns for teeth 11, 12, 13, 21, 22, and 23. Digital impression was made using a digital scanner (PRIMESCAN®, Dentsply Sirona, Bensheim, Germany); surgical guide (Cerec Guide 3, Dentsply Sirona, Bensheim, Germany) was designed by using a designing software (Galileos Software, Dentsply Sirona, Bensheim, Germany) and was milled by using a milling machine (MCXL milling machine, Dentsply Sirona, Bensheim, Germany) to create a precise surgical guide. 3D smile design was done by using the Digital Smile Design (DSD) program, the crown lengthening guide was designed according to DSD designed by using a designing software (Inlab 19 Software, Dentsply Sirona, Bensheim, Germany), and the guide was fabricated by a 3D printer (FormLabs Form 2, Formlab, MA, USA). Provisional crowns (splinted) for teeth 11, 12, 13, 21, 22, and 23 were milled by using polymer (VIPI BLOCK TRILUX®, VIPI Industria, Pirassununga, SP, Brazil) using a milling machine (MCX5, Dentsply Sirona, Bensheim, Germany). The zirconia crowns were designed by using software (Inlab19 Software, Dentsply Sirona, Bensheim, Germany) and milled using the same milling machine. At the implant position, Ti-base is cemented by using an abutment (Multilink Hybrid Abutment, Ivoclar Vivadent, Schaan, Lichtenstein, Germany) with zirconia coping (Cercon base white shade, Dentsply Sirona, Bensheim, Germany) utilized as the abutment. The zirconia crowns (Cercon Xt, Dentsply Sirona, Bensheim, Germany) were sintered and characterized and then cemented. The patient was satisfied with the esthetic outcome of the treatment.

Abstract:Although their effects cannot be shown to statistically alter coronary heart disease mortality or morbidity, voluntary health organizations throughout the world clearly play an important role in bringing about favorable changes in the natural history of this disease and in the community phase of its management. In particular, voluntary organizations are able to conduct research surveys and field trials and by so doing favorably influence state health administration. They are able to correct temporary deficiencies in health services, particularly in the form of psychosocial support and cardiac health education. Observers note that the major change in community phase management has occurred with the wider use of coronary bypass surgery since 1975. More objective data, especially relating to psychosocial factors, can be expected when further research (especially the MONICA Study) is completed. Throughout the world, however, existing voluntary health organizations could be more active in the community phase of cardiac rehabilitation. It would seem an area where such organizations could well do more. A challenging question that should be constantly reviewed is 'Can we do more to reduce the effects of invalidism in cardiac patients?'

Abstract:Objective: To explore and evaluate a three-dimensional (3D) digital simulated design and implementation technique in esthetic rehabilitation. Methods: Thirty patients with esthetic deficiency, who came to the Department of Prosthodontics, Peking University School and Hospital of Stomatology from December 2017 to July 2019, were recruited and randomly assigned into 2 groups. Four males and 11 females which were (36.0±10.5) years old in the experimental group, 6 males and 9 females which were (32.0±6.7) years old in the control group, were enrolled. In the experimental group, 3D digital simulated design was used to predict the post-treatment effect; and the final restorations were designed duplicating from the pre-treatment digital design confirmed by the patient and milled. In the control group, the final restorations were manufactured by the dental technician according to the design of two-dimensional (2D) digital smile design. The simulation degree of digital design and the similarity between preoperative design and postoperative rehabilitation were scored by the patients with visual analogue scales (VAS) in both groups; and the satisfaction rate to the restorations was evaluated by the patients. The quality of the restorations was accessed by a prosthodontist who did not know the grouping of patients according to the modified criteria of United States Public Health Service (USPHS). Results: Three-dimensional digital simulated design and implementation technique was successfully established. The VAS score on the simulation degree of digital design in the experimental group (8.5±0.5) was higher than that in the control group (7.2±0.7) (P<0.01); the VAS score on the similarity between preoperative design and postoperative rehabilitation in the experimental group (9.6±0.3) was higher than that in the control group (7.0±0.9) (P<0.01). The satisfaction rate of the patients to the restorations was significantly higher in the experimental group than in the control group (P<0.05). There was no significant difference of the quality of the restorations between the two groups on the anatomic form, the marginal adaption and the surface quality (P>0.05). Conclusions: Three-dimensional digital simulated design and implementation technique can help achieving 3D digital simulated design before treatment and duplication to the final restorations, and can improve the patients' satisfaction in esthetic rehabilitation.
Computer-assisted prosthetic planning and implant design with integrated digital bite registration: a treatment protocol

Abstract:The aim of this clinical study is to present an integrated digital project through the description of a clinical case, made entirely in digitized form, taking advantage of the opportunity offered by instrumental diagnostic software. A case report participant is a 65-year-old female patient presents with loss of diffuse bone support, caused by periodontal disease. After a sign of an informed consent and an explication of a plan of treatment, technical intraoral and extraoral pictures and intraoral digital impressions were taken. The digital images improved from the 2D Smile Lynx Software and the scanner stereolithographic (STL) file was matched into the CAD Lynx to obtain a virtual previsualization of teeth and smile design, and to mill the provisional and the definitive crowns. The digital prosthetic design allows the evaluation of the dental parameters in relation to the parameters of the patient's face for the new prosthetic project and the radiological examination using CBCT guides the insertion of the fixtures for the rehabilitation phase. The surgical and prosthetic design are subsequently integrated. The evaluation of the bone bases is carried out with a radiological diagnostic software for CT (Real Guide 5.0-3Diemme, Cantù-Italy) which can virtually design the implant insertion. The functional examination of the patient is carried out through an occlusion-postural examination that uses digital electromyographic assessments. The integrated digital protocol proposal inserts in the rehabilitation path the digital recording of the free mandibular movement, as well as the scan of the patient's face, data that will be integrated into the CAD software for the design of temporary and definitive prosthetic artifacts, made using the CAM method. This study showed guided implant placement and the application of fixed implant-supported prosthetic restorations carried out with a fully digital workflow, dependent on the functional digital evaluation of the patient's occlusion. The proposed protocol described the correct use of digitalization of clinical, surgical, and prosthetic procedures, and the matching of the data into a computerized environment, to improve team communication and to take advantage of the combination of collected data to not lose information using classic manual steps.
Efficacy of digital guide-assisted implant restoration in anterior teeth aesthetics and its impact on labial bone mass

Abstract:Objective: To analyze the efficacy of digital guide-assisted implant restoration technique in enhancing the anterior teeth aesthetics and its impact on labial bone mass. Methods: We retrospectively analyzed clinical data from 90 patients who underwent maxillary anterior teeth implant restoration at The First People's Hospital of Fuyang, Hangzhou, from January 2021 to September 2023. The patients were divided into two groups: a conventional group (n=45, 45 implants, standard implant restoration) and a digital group (n=45, 45 implants, digital guide-assisted implant). We compared implant positional deviations, changes in dental plaque index (PLI), aesthetic effect scores, labial bone mass differences, and the occurrence of adverse reactions post-treatment between the two groups. Results: The digital group exhibited significantly less deviation in root position in the buccolingual and vertical directions, less neck deviation in the buccolingual and vertical directions, and less apical deviation than the conventional group (P=0.021, P=0.005, P=0.016, P=0.008, P=0.026, respectively). Three months postoperatively, the digital group demonstrated a significantly lower mean PLI (P<0.001), higher white and pink aesthetic scores (P=0.021, P=0.005), and increased alveolar ridge height and coronal and middle labial bone mass (P=0.006, P=0.015, P=0.008). Additionally, this group experienced lower incidence of adverse reactions (4.44% vs. 17.78%) compared with the conventional group (P=0.044). Conclusion: The digital guide-assisted implant restoration significantly enhances implant accuracy, reduces bone resorption, improves aesthetic outcomes, and ensures higher safety.

Abstract:Objective Occlusal reconstruction is a critical intervention for patients with dental hard tissue defects, temporomandibular joint (TMJ) disorders, and jaw position abnormalities. Clinical efficiency and outcomes of these procedures have improved with advances in digital technologies. This case report aims to illustrate a comprehensive digital workflow for occlusal reconstruction in a patient with congenital dentition defects, emphasizing the application of digital technologies to enhance treatment outcomes. Clinical considerations A 28‐year‐old woman with previously installed porcelain‐fused‐to‐metal bridge restorations presented with a fractured prosthesis and TMJ symptoms. A multidisciplinary approach was adopted involving the use of digital facebow, intraoral scanners, digital smile design, and CAD/CAM technologies. The process included the extraction of defective restorations, temporary restorations to refine jaw position, and final permanent restorations. The digital workflow facilitated precise diagnostics and treatment, culminating in the successful installation of permanent restorations. Regular follow‐ups at one‐ and three‐months post‐treatment confirmed stable occlusal function and high patient satisfaction. Conclusions This case report showcases the potential of multiple digital technologies to streamline complex dental treatments and achieve high‐quality results. Clinical significance The integration of digital technologies in occlusal reconstruction treatments offers significant benefits in terms of precision, patient comfort, and esthetic outcomes.

Abstract:Introduction In every prosthetic treatment, the final purpose of the restoration is to restore function and esthetics for the patient, respecting biology and anatomical parameters. Regarding full coverage restorations, there are some factors to take into consideration: like the finish line (chamfer, shoulder, featheredge, etc.), the management of the provisional restoration and tissue healing, the material for the final restoration, and the impression technique. All these aspects will determine the treatment process and its possible results. In recent years, vertical preparation and more specifically biologically oriented preparation technique has generated great interest since its introduction in prosthodontics, changing the periodontal tissues, and providing long‐term stability. Materials and Methods With this technique, the objective is to change the position of the gingival margin, moving it more apically or coronally, depending on the modifications of the provisional. The relining of the provisional is a fundamental step of this technique: the intrasulcular part of the provisional is modified giving a new emergence profile and providing an augmentation of the tissue in the healing process. Discussion In vertical preparation, there is no predetermined margin. The dental technician should position the finish line more apically or coronally, depending on the depth of the sulcus and on the esthetic needs. Ceramic restorations should not invade the epithelial attachment, otherwise it may cause biological and mechanical problems, repetition of the impression, and of the crown restoration, which lead to increased clinical and laboratory time usage as well as expenses. One challenge is communicating the shape and contour of the temporary restoration to the dental lab to ensure accurate information for the final restoration. Conclusion Finally, the objective of this article is to describe a technique for transferring all necessary information of the provisional and obtaining a predictable result for the final restoration by using an intraoral scanner system and the fabrication of two 3D printed prototypes. One for the final volume of the crowns and the other for the finish line position. Clinical Significance The fabrication of 3D printed prototypes allows for a more predictable final restoration, reducing possible communication errors between clinician and technician when restoring with biologically oriented preparation technique. In any esthetic treatment where the restoration does not have a predetermined margin, it is possible with this additional clinical step to provide a more esthetic outcome.

Abstract:This case report highlights the importance of standardized and fully digital sequential treatment in complex occlusal rehabilitation cases. To fully resolve the patient's dental needs, such cases often require multidisciplinary interventions including periodontal therapy, endodontic treatment, anterior esthetics, implant restoration, and prosthetic rehabilitation. A fully digital workflow (including facial scanners, intraoral scanners, jaw motion tracking systems, virtual articulators, and computer-aided design software) streamlined the complex treatment, enhancing workflow simplicity, efficiency, visibility, and precision.
