Abstract:Mucosal growth in small intestine of the streptozotocin-diabetic rat is increased; insulin treatment decreases growth toward normal. Gastrin is trophic to the alimentary tract. Therefore, we measured gastrin and growth of the alimentary tract in controls, diabetics, and insulin-treated diabetics. Mucosal growth of the small intestine increased at 10 days of diabetes and increased further by 20 days. Serum and antral gastrin did not respond to diabetes at 10 days. At 20 days, mean antral gastrin was increased in all diabetic groups, but the increase was significant only for a hyperphagic ad libitum-fed diabetic group, which also showed increased serum gastrin. Insulin treatment decreased serum gastrin but only partly reversed the increased mucosal growth in diabetics. We conclude that 1) gastrin is not the primary cause for increased small intestinal mucosal growth in diabetes, inasmuch as gastrin did not respond to diabetes at 10 days when mucosal growth was increased, and decreased serum gastrin in insulin-treated groups was associated with increased mucosal growth; and 2) diabetes appears to increase gastrin.
Immunohistochemical characterization of interstitial cells and their spatial relationship to motor neurons within the mouse esophagus

Abstract:Interstitial cells of Cajal (ICC) and PDGFRα + cells regulate smooth muscle motility in the gastrointestinal (GI) tract, yet their function in the esophagus remains unknown. The mouse esophagus has been described as primarily skeletal muscle; however, ICC have been identified in this region. This study characterizes the distribution of skeletal and smooth muscle cells (SMCs) and their spatial relationship to ICC, PDGFRα + cells, and intramuscular motor neurons in the mouse esophagus. SMCs occupied approximately 30% of the distal esophagus, but their density declined in more proximal regions. Similarly, ANO1 + intramuscular ICC (ICC-IM) were distributed along the esophagus, with density decreasing proximally. While ICC-IM were closely associated with SMCs, they were also present in regions of skeletal muscle. Intramuscular, submucosal, and myenteric PDGFRα + cells were densely distributed throughout the esophagus, yet only intramuscular PDGFRα + cells in the lower esophageal sphincter (LES) and distal esophagus expressed SK3. ICC-IM and PDGFRα + cells were closely associated with intramuscular n NOS + , VIP + , VAChT + , and TH + neurons and GFAP + cells resembling intramuscular enteric glia. These findings suggest that ICC-IM and PDGFRα + cells may have roles in regulating esophageal motility due to their close proximity to each other and to skeletal muscle and SMCs, although further functional studies are needed to explore their role in this region. The mixed muscular composition and presence of interstitial cells in the mouse distal esophagus is anatomically similar to the transitional zone found in the human esophagus, and therefore, motility studies in the mouse may be translatable to humans.

Enteric sensory neurons communicate with interstitial cells of Cajal to affect pacemaker activity in the small intestine

Abstract:Enteric sensory neurons (the AH neurons) play a role in control of gastrointestinal motor activity; AH neuron activation has been proposed to change propulsion into segmentation. We sought to find a mechanism underlying this phenomenon. We formulated the hypothesis that AH neurons increase local ICC-MP (interstitial cells of Cajal associated with the myenteric plexus) pacemaker frequency to disrupt peristalsis and promote absorption. To that end, we sought structural and physiological evidence for communication between ICC-MP and AH neurons. We designed experiments that allowed us to simultaneously activate AH neurons and observe changes in ICC calcium transients that underlie its pacemaker activity. Neurobiotin injection in AH neurons together with ICC immunohistochemistry proved the presence of multiple contacts between AH neuron varicosities and the cell bodies and processes of ICC-MP. Generating action potential activity in AH neurons led to increase in the frequency and amplitude of calcium transients underlying pacemaker activity in ICC. When no rhythmicity was seen, rhythmic calcium transients were evoked in ICC. As a control, we stimulated nitrergic S neurons, which led to reduction in ICC calcium transients. Hence, we report here the first demonstration of communication between AH neurons and ICC. The following hypothesis can now be formulated: AH neuron activation can disrupt peristalsis directed by ICC-MP slow wave activity, through initiation of a local pacemaker by increasing ICC pacemaker frequency through increasing the frequency of ICC calcium transients. Evoking new pacemakers distal to the proximal lead pacemaker will initiate both retrograde and antegrade propulsion causing back and forth movements that may disrupt peristalsis.
Involvement of ANO1 currents in pacemaking of PDGFRα-positive specialised smooth muscle cells in rat caudal epididymis

Abstract:The epididymal duct exhibits spontaneous phasic contractions (SPCs) to store and transport sperm. Here, we explored molecular identification of pacemaker cells driving SPCs in the caudal epididymal duct and also investigated properties of pacemaker currents underlying SPCs focusing on ANO1 Ca 2+ -activated Cl − channels (CaCCs). Immunohistochemistry was performed to visualise the distribution of platelet-derived growth factor receptor α (PDGFRα)- or ANO1-positive cells in the rat caudal epididymal duct. Perforated whole-cell patch clamp technique was applied to enzymatically isolated epididymal cells, while SPCs were recorded with video edge-tracking technique. Immunohistochemistry revealed the distribution of α-smooth muscle actin (α-SMA)-positive cells co-expressing both PDGFRα and ANO1 in the innermost smooth muscle layer. Approximately one-third of isolated epididymis cells exhibited spontaneous transient inward currents (STICs) at the holding potential −60 mV. The reversal potential for STICs was close to the calculated chloride equivalent potential depending on intracellular Cl − concentrations. Ani9 (3 μM), the ANO1 specific inhibitor, decreased both amplitude and frequency of STICs, while cyclopiazonic acid (CPA, 30 μM), a sarco-/endoplasmic reticulum Ca 2+ -ATPase (SERCA) inhibitor, abolished STICs. Ani9 (3 or 10 μM) reduced the frequency of SPCs without changing their amplitude. Thus, PDGFRα + , ANO1 + specialised smooth muscle cells (SMCs) appear to function as pacemaker cells to electrically drive epididymal SPCs by generating ANO1-dependnet STICs. STICs arising from spontaneous Ca 2+ release from intracellular Ca 2+ store and subsequent opening of ANO1 result in depolarisations that spread into adjacent SMCs where L-type voltage-dependent Ca 2+ channels are activated to develop SPCs.

Characterization of inhibitory innervation in porcine colonic circular muscle

Abstract:Effects of stimulation of intramural nerves in the circular smooth muscle layer of the porcine colon (Sus scrofa domestica) were studied using the sucrose-gap technique. Electrical field stimulation of the preparation, superfused with Krebs solution at 21 degrees C, induced a transient hyperpolarization of the smooth muscle cell membrane. This hyperpolarization was an inhibitory junction potential (IJP). The responses obtained from circular muscle originating from either the centripetal or centrifugal gyri of the ascending colon did not differ significantly. The IJP was characterized as being mediated by intramural, nonadrenergic, noncholinergic (NANC) nerves. The amplitude and latency of the IJP changed linearly with temperature (15-25 degrees C: +1 mV and -0.1 s per degree Celsius, respectively) reflecting a temperature-dependent synchronization of transmitter release. The membrane resistance decreased during the IJP. The IJP amplitude decreased or increased during conditioning hyperpolarizations or depolarizations, respectively, and reversed at membrane potentials about 30 mV more negative than the resting membrane potential. Potassium conductance blocking agents, barium (1 mM), tetraethylammonium chloride (TEA, 20 mM), 4-aminopyridine (4-AP, 5 mM), apamin (1 microM), and aminacrine (10(-4) M) added to the superfusion medium increased the membrane resistance. Only barium, TEA, and apamin depolarized the smooth muscle cell membrane. The IJP amplitude decreased in the presence of aminacrine and apamin to 75 and 35%, respectively, suggesting that apamin-sensitive Ca2+-activated K+ channels are involved in this response. ATP, adenosine, and related adenine nucleotides in concentrations up to 10(-3) M did not mimic the IJP. Superfusion with ATP for 15 min revealed a gradually increasing attenuation by up to 20% of the IJP. This might suggest that the release of neurotransmitter from intramural NANC nerves is modulated presynaptically via purinoceptors. Exogenously applied vasoactive intestinal polypeptide (VIP) in concentrations of 10(-9) to 10(-4) M did not affect the preparation. Also at elevated temperatures (up to 35 degrees C), VIP (10(-7) to 10(-4) M) did not cause measurable effects. It is concluded that the inhibitory mediator of the intramural NANC nerves present in the circular muscle layers of the porcine colon is neither a purine nor VIP.
Characterization of interstitial cells of Cajal in the subserosal layer of the guinea-pig colon

Abstract:Interstitial cells of Cajal in the subserosa (ICC-SS) of the guinea-pig proximal colon were studied by immunohistochemistry for c-Kit receptors and by transmission electron microscopy. These cells were distributed within a thin layer of connective tissue space immediately beneath the mesothelium and were multipolar with about five primary cytoplasmic processes that divided further into secondary and tertiary processes to form a two-dimensional network. Ultrastructural observations revealed that ICC-SS were connected to each other via gap junctions. They also formed close contacts and peg-and-socket junctions with smooth muscle cells. Three-dimensional analysis of confocal micrographs revealed that the cytoplasmic processes of ICC-SS had contacts with interstitial cells in the longitudinal muscle layer. Taking account of the location and peculiar arrangement of the ICC-SS and the main functions of the proximal colon, i.e. the absorption and transport of fluids, we suggest that the superficial network of ICC-SS acts as a stretch receptor to detect circumferential expansion and swelling of the colon wall and triggers the contraction of the longitudinal muscle to accelerate the drainage of fluids from the colon.

Supported employment as an intervention package: a preliminary analysis of functional variables.

Abstract:In this two-experiment investigation, the long-term (at least 6 months) employment of 51 moderately mentally retarded clients who were placed into 64 supported employment positions was first evaluated relative to 10 training and posttraining components that comprised a supported employment training package. In Experiment 1, chi-square analyses were used to identify three components that differentiated successful (employed for at least 6 months) from unsuccessful clients. In Experiment 2, 4 successful clients were further evaluated in a nonconcurrent multiple baseline design to determine whether the three variables identified during the group analysis (client advocate, collateral behavior, and follow-up plan) were included in the training packages. These results are discussed in terms of the need to establish better the functional variables of supported employment training programs.
ERG K+ currents regulate pacemaker activity in ICC

Abstract:Ether-à-go-go-related gene (ERG) K channels have been implicated in the generation of pacemaker activities in the heart. To study the presence and function of ERG K channels in the pacemaker cells of the small intestine [the interstitial cells of Cajal (ICC)], a combination of patch-clamp techniques, tissue and live cell immunohistochemistry, RT-PCR, and in vitro functional studies were performed. Nonenzymatically isolated ICC in culture were identified by vital staining and presence of rhythmic inward currents. RT-PCR showed the presence of ERG mRNA in the intestinal musculature, and immunohistochemistry on tissue and cultured cells demonstrated that protein similar to human ERG was concentrated on ICC in the Auerbach's plexus region. Whole cell ERG K+ currents were evoked on hyperpolarization from 0 mV (but not from -70 mV) up to -120 mV and showed strong inward rectification. The currents were inhibited by E-4031, cisapride, La3+, and Gd3+ but not by 50 microM Ba2+. The ERG K+ inward current had a typical transient component with fast activation and inactivation kinetics followed by significant steady-state current. E-4031 also inhibited tetraethylammonium (TEA)-insensitive outward current indicating that the ERG K+ current is operating at depolarizing potentials. In contrast to TEA, blockers of the ERG K+ currents caused marked increase in tissue excitability as reflected by an increase in slow-wave duration and an increase in superimposed action potential activity. In summary, ERG K channels in ICC contribute to the membrane potential and play a role in regulation of pacemaker activity of the small intestine.
Kit-negative fibroblast-like cells expressing SK3, a Ca2+-activated K+ channel, in the gut musculature in health and disease

Abstract:The apamin-sensitive component of the inhibitory response of the gastrointestinal musculature involves the small conductance Ca(2+)-activated K(+) channel SK3. Kit-immunoreactive (ir) interstitial cells of Cajal appear to be involved in nitrergic inhibition while the role of the recently described CD34-ir fibroblast-like cells adjacent to, but distinct from, the cells of Cajal remains elusive. The distribution of SK3 was studied by immunohistochemistry in the normal human gut, in motility disorders with a lack of cells of Cajal (infantile hypertrophic pyloric stenosis and Hirschsprung's disease) and in mice deficient in cells of Cajal. SK3 immunoreactivity was observed exclusively in Kit-negative interstitial cells adjacent to, but distinct from, the Kit-ir interstitial cells of Cajal in the normal gut. The distribution of SK3-ir cells was not altered in conditions where cells of Cajal were lacking. These cells were CD34-ir fibroblast-like cells in the human gut and in the mouse stomach, while SK3-ir cells in the mouse intestine were CD34 negative. As SK channels are reportedly involved in inhibitory neurotransmission, our morphological observations suggest that SK3-ir interstitial cells, distinct from the Kit-ir interstitial cells of Cajal, may represent a novel cellular component in the control of excitability of the digestive musculature. Further studies will be required to directly address the function of these cells.

Abstract:In this work, we report the synthesis of periodic nanoporous cobalt ferrite (CFO) that exhibits tunable room temperature ferrimagnetism. The porous cubic CFO frameworks are fabricated by coassembly of inorganic precursors with a large amphiphilic diblock copolymer, referred to as KLE. The inverse spinel framework boasts an ordered open network of pores averaging 14 nm in diameter. The domain sizes of the crystallites are tunable from 6 to 15 nm, a control which comes at little cost to the ordering of the mesostructure. Increases in crystalline domain size directly correlate with increases in room temperature coercivity. In addition, these materials show a strong preference for out-of-plane oriented magnetization, which is unique in a thin film system. The preference is explained by in-plane tensile strain, combined with relaxation of the out-of-plane strain through flexing of the mesopores. It is envisioned that the pores of this ferrimagnet could facilitate the formation of a diverse range of exchange coupled composite materials.

Abstract:Blood insulin complexes examined for insulin activity in vitro by the rat diaphragm assay under specified conditions did not exhibit full insulin activity. Incubation of the blood insulin complexes with crude and purified human and rat adipose tissue extracts resulted in an increase of insulin activity in the resin eluates. Extracts from other tissues, such as human liver, kidney and muscle and from rat diaphragm, did not produce similar results. It is suggested that adipose tissue extracts may contain a system which produces either free insulin from the blood insulin complexes or other forms of insulin which could be utilized by the rat diaphragm.
Xiangbinfang granules enhance gastric antrum motility via intramuscular interstitial cells of Cajal in mice

Abstract:Interdigestive migrating motor complexes (MMC) produce periodic contractions in the gastrointestinal tract, but the exact mechanism of action still remains unclear. Intramuscular interstitial cells of Cajal (ICC-IM) participate in gastrointestinal hormone and neuromodulation, but the correlation between ICC-IM and MMC is also unclear. We found that xiangbinfang granules (XBF) mediated the phase III contraction of MMC. Here, the effects of XBF on gastric antrum motility in mice and the effects of ICC-IM on gastric antrum MMC are reported.To observe the effects of ICC-IM on gastric antrum motility and to establish the mechanism of XBF in promoting gastric antrum motility.The density of c-kit-positive ICC myenteric plexus (ICC-MP) and ICC-IM in the antral muscularis of and wild-type (WT) mice was examined by confocal microscopy. The effects of XBF on gastric antrum slow waves in and WT mice were recorded by intracellular amplification recording. Micro-strain-gauge force transducers were implanted into the gastric antrum to monitor the MMC and the effect of XBF on gastric antrum motility in conscious and WT mice.In the gastric antrum of mice, c-kit immunoreactivity was significantly reduced, and no ICC-IM network was observed. Spontaneous rhythmic slow waves also appeared in the antrum of mice, but the amplitude of the antrum slow wave decreased significantly in mice (22.62 ± 2.23 mV 2.92 ± 0.52 mV, < 0.0001). MMCs were found in 7 of the 8 WT mice but no complete MMC cycle was found in mice. The contractile frequency and amplitude index of the gastric antrum were significantly increased in conscious WT compared to mice (frequency, 3.53 ± 0.18 cpm 1.28 ± 0.12 cpm; amplitude index, 23014.26 ± 1798.65 mV·20 min 3782.16 ± 407.13 mV·20 min; < 0.0001). XBF depolarized smooth muscle cells of the gastric antrum in WT and mice in a dose-dependent manner. Similarly, the gastric antrum motility in WT mice was significantly increased after treatment with XBF 5 mg ( < 0.05). Atropine (0.1 mg/kg) blocked the enhancement of XBF in WT and mice completely, while tetrodotoxin (0.05 mg/kg) partially inhibited the enhancement by XBF.ICC-IM participates in the regulation of gastric antrum MMC in mice. XBF induces MMC III-like contractions that enhance gastric antrum motility ICC-IM in mice.

Abstract:Background To investigate the effect of the distribution and expression of interstitial cells of Cajal (ICCs) and platelet-derived growth factor receptor-α positive (PDGFRα+) cells in different colon segments on colonic motility in children with Hirschsprung disease (HSCR). Methods Smooth muscles of the narrow and dilated segments of the colon were obtained from 16 pediatric patients with HSCR. The proximal margin was set as the control section. The mRNA and protein expressions of c-Kit, PDGFRα, ANO1, and SK3 channels were examined. Circular smooth muscle strips of the colon were prepared for performing electrophysiology experiments using electric field stimulation (EFS) and intervention from different drugs (TTX, NPPB, Apamin, L-NAME, and CyPPA). Results The mRNA and protein expressions of c-Kit, ANO1, PDGFRα, and SK3 were much lower in the narrow segment than those in the dilated and proximal segments of the colon. The narrow segment showed a considerably spontaneous contraction of the muscle strip. After the EFS, the relaxation response decreased from the proximal to the narrow segment, whereas the contraction response increased. TTX blocking did not cause any significant changes in the narrow segment. In contrast, when NPPB, Apamin, L-NAME, and CyPPA were used to intervene in the muscle strips, the proximal segment showed a more sensitive inhibitory or excitatory response than the narrow segment. Conclusions Downregulation of the ICCs and PDGFRα+ cells from the proximal to narrow segment may be responsible for the dysmotility of the colon in pediatric HSCR.

The Mechanism of Action of Zingerone in the Pacemaker Potentials of Interstitial Cells of Cajal Isolated from Murine Small Intestine

Abstract:Background/aims: Zingerone, a major component found in ginger root, is clinically effective for the treatment of various diseases. Interstitial cells of Cajal (ICCs) are the pacemaker cells responsible for slow waves in the gastrointestinal (GI) tract. We investigated the effects of zingerone on the pacemaker potentials of ICCs to assess its mechanisms of action and its potential as a treatment for GI tract motility disorder. Methods: We isolated ICCs from small intestines, and the whole-cell patch-clamp configuration was used to record the pacemaker potentials in cultured ICCs. Results: Under the current clamping mode, zingerone inhibited pacemaker potentials of ICCs concentration-dependently. These effects were blocked not by capsazepine, a transient receptor potential vanilloid 1 (TRPV1) channel blocker, but by glibenclamide, a specific ATP-sensitive K+ channel blocker. Pretreatment with SQ-22536 (an adenylate cyclase inhibitor), LY294002 (a phosphoinositide 3-kinase inhibitor), and calphostin C (a protein kinase C (PKC) inhibitor) did not block the effects of zingerone on the pacemaker potentials relative to treatment with zingerone alone. However, zingerone-induced pacemaker potential inhibition was blocked by 1H-[1,2,4] oxadiazolo [4,3-a] quinoxalin-1-one (ODQ; a guanylate cyclase inhibitor), KT5823 (a protein kinase G (PKG) inhibitor), and L-NAME (a non-selective nitric oxide synthase (NOS) inhibitor). In addition, zingerone stimulated cyclic guanosine monophosphate (cGMP) production in ICCs. Finally, pretreatment with PD98059 (a p42/44 mitogen-activated protein kinase (MAPK) inhibitor), SB203580 (a p38 MAPK inhibitor), and SP600125 (c-Jun N-terminal kinases (JNK)-specific inhibitor) blocked the zingerone-induced pacemaker potential inhibition. Conclusion: These results suggest that zingerone concentration-dependently inhibits pacemaker potentials of ICCs via NO/cGMP-dependent ATP-sensitive K+ channels through MAPK-dependent pathways. Taken together, this study shows that zingerone may have the potential for development as a GI regulation agent.

Abstract:Maintenance of fecal continence requires a continuous or basal tone of the internal anal sphincter (IAS). Paradoxically, the basal tone results largely from high-frequency rhythmic contractions of the IAS smooth muscle. However, the cellular and molecular mechanisms that initiate these contractions remain elusive. Here we show that the IAS contains multiple pacemakers. These pacemakers spontaneously generate propagating calcium waves that drive rhythmic contractions and establish the basal tone. These waves are myogenic and act independently of nerve, paracrine or autocrine signals. Using cell-specific gene knockout mice, we further found that TMEM16A Cl − channels in smooth muscle cells (but not in the interstitial cells of Cajal) are indispensable for pacemaking, rhythmic contractions, and basal tone. Our results identify TMEM16A in smooth muscle cells as a critical pacemaker channel that enables the IAS to contract rhythmically and continuously. This study provides cellular and molecular insights into fecal continence.

Cardiac PDGFRα+ interstitial cells generate spontaneous inward currents that contribute to excitability in the heart

Abstract:The cell types and conductance that contribute to normal cardiac functions remain under investigation. We used mice that express an enhanced green fluorescent protein (eGFP)-histone 2B fusion protein driven off the cell-specific endogenous promoter for Pdgfra to investigate the distribution and functional role of PDGFRα+ cells in the heart. Cardiac PDGFRα+ cells were widely distributed within the endomysium of atria, ventricle, and sino-atrial node (SAN) tissues. PDGFRα+ cells formed a discrete network of cells, lying in close apposition to neighboring cardiac myocytes in mouse and Cynomolgus monkey (Macaca fascicularis) hearts. Expression of eGFP in nuclei allowed unequivocal identification of these cells following enzymatic dispersion of muscle tissues. FACS purification of PDGFRα+ cells from the SAN and analysis of gene transcripts by qPCR revealed that they were a distinct population of cells that expressed gap junction transcripts, Gja1 and Gjc1. Cardiac PDGFRα+ cells generated spontaneous transient inward currents (STICs) and spontaneous transient depolarizations (STDs) that reversed at 0 mV. Reversal potential was maintained when ECl = -40 mV. [Na+ ]o replacement and FTY720 abolished STICs, suggesting they were due to a non-selective cation conductance (NSCC) carried by TRPM7. PDGFRα+ cells also express β2 -adrenoceptor gene transcripts, Adrb2. Zinterol, a selective β2 -receptor agonist, increased the amplitude and frequency of STICs, suggesting these cells could contribute to adrenergic regulation of cardiac excitability. PDGFRα+ cells in cardiac muscles generate inward currents via an NSCC. STICs generated by these cells may contribute to the integrated membrane potentials of cardiac muscles, possibly affecting the frequency of pacemaker activity.
Slow inward and late slow outward currents induced by hyperpolarizing pre-pulses in cat bladder parasympathetic neurones

Abstract:Single-electrode voltage-clamp techniques were used to examine membrane currents recorded as a result of hyperpolarizing pre-pulses in cat bladder parasympathetic neurones. In 84 ganglion cells examined, two types of current were observed in response to hyperpolarizing pre-pulses of 10 ms to 1 s duration from holding potentials of -30 to -60 mV to test potentials of -90 to -130 mV. In 46 cells, a short-duration pulse induced a slow inward current (SIC); with longer pulse durations, an outward current was superimposed on the SIC, resulting in a late slow outward current (LSOC). In the remaining cells, either a SIC (n = 12) or an LSOC (n = 26) was recorded over a range of hyperpolarizing pre-pulse durations. The more depolarized the holding potential, the more hyperpolarized the test potential and the longer the pulse duration, the larger the amplitude of the SIC and LSOC. The SIC and LSOC were associated with an increase in input conductance. The extrapolated reversal potential (Vrev) for the LSOC obtained at a holding potential of -60 mV (where the LSOC seemed to be less contaminated with the SIC) was -89 +/- 4 mV (mean +/- standard error of the mean; n = 5), which is close to the equilibrium potential for the K ion. The LSOC was depressed by a high-K (10-20 mM) solution and potentiated by a low-K (0.47 mM) solution. The SIC was depressed by a low-Na (26.2 mM) solution, but was not affected significantly by a low-Cl (12.2 mM) solution. A low-Ca (0.1 mM)/high-Mg (5 mM) solution depressed the LSOC, while a high-Ca (5 mM) solution potentiated it. Cd (0.5 mM) blocked the SIC almost completely, and suppressed the LSOC. The LSOC but not the SIC was suppressed by tetraethylammonium chloride (10 mM). Superfusing Cs (3 mM) did not affect either the LSOC or the SIC. 4-Aminopyridine (1 mM) and muscarine (10 microM) depressed or replaced the SIC with an outward current, while potentiating the LSOC. These results suggest that a hyperpolarizing pre-pulse induces slow inward Na- and late slow outward Ca-dependent K currents, which are inactivated at depolarized potentials and are de-inactivated by hyperpolarizing pulses in a time-dependent manner.
Interstitial cell of Cajal-like cells in the upper urinary tract

Abstract:Autorhythmicity in the upper urinary tract (UUT) has long been considered to arise in specialized atypical smooth muscle cells (SMC) predominately situated in the most proximal regions of the pyeloureteric system. These atypical SMC pacemakers have been thought to trigger adjacent electrically-quiescent typical SMC to fire action potentials which allow an influx of Ca2+ and the generation of muscle contraction. More recently, the presence of cells with many of the morphological, electrical and immunohistochemical characteristics of interstitial cells of Cajal (ICC), the pacemaker cells of the gastrointestinal tract, have been located in many regions of both the upper and lower urinary tract. This article reviews the evidence from the literature and from our laboratory supporting a role of both atypical SMC and ICC-like cells in the initiation and propagation of pyeloureteric peristalsis in the UUT. We propose a new model in which there are 2 populations of pacemaker cells, high frequency atypical SMC and lower frequency ICC-like cells, both of which can drive electrically-quiescent typical SMC. The relative presence of these 2 populations of pacemaker cells and the relatively-long refractoriness of typical SMC determines the decreasing frequency of contraction with distance from the renal fornix. In the absence of the proximal pacemaker drive from atypical SMC after pyeloureteral/ureteral obstruction or surgery, ICC-like cell pacemaking provides a compensatory mechanism allowing the ureter to maintain rudimentary peristaltic waves and movement of urine from the pyelon towards the bladder.
Subacute administration of Plak-Lite sodium fluorescein solution in hamsters.

Abstract:The purpose of this research was to evaluate the potential toxic effects of Plak-Lite disclosing solution, which contains 0.75% sodium fluorescein U.S.P. in buffered glycerol in hamsters under defined and exaggerated conditions. In oral dosing experiments, volumes calculated at least 50, 200 or 1000 times the human daily use level were administered. For intraoral application the 1000 X human use level was selected. The control group was dosed orally with the 1000 X dose volume of water. All animals were treated once a day for 31 or 92 days. Daily examination of animals revealed no overt drug or dosing-related effects. Weights recorded at 31 or 92 days were similar for all groups. Histological evaluation performed on tongue, oral mucosa, cheek pouch, stomach (3 sections), liver, kidney, spleen, small intestine (3 sections), large intestine and bladder revealed no pathological changes. This study demonstrates the safety of Plak-Lite disclosing solution when administered at exaggerated dose levels via routes simulating human use.
Clinicoepidemiological Profile and Outcome of Children With Wilms Tumor in a Developing Country

Abstract:Background: Outcome of children with Wilms tumor (WT) has improved remarkably because of the advancements in Pediatric Oncology. However, in most developing countries survival rates are still poor. There is a paucity of data regarding the clinical profile and outcome of children with WT in India. Methods: Children with WT who were treated at our center between 2005 and 2014 were retrospectively analyzed. Results: Over the 10-year study period, 108 children with WT were treated in our unit. The median age at presentation was 30 months (male:female ratio of 4:1). Abdominal mass was the most common presenting feature (91%). The majority of children had stage 2 (30%) or stage 3 (35%) disease. Fifty-nine children had large or bilateral tumors requiring preoperative chemotherapy. Of the total 108 children, 11 died during primary treatment and 14 relapsed after completion of treatment. The projected 5-year overall survival is 74%, whereas the event-free survival is 73%. Conclusions: Children with WT in our setting present late with advanced disease and large tumors, which are technically difficult to deliver at surgery. With a multidisciplinary approach, reasonably good survival rates were achieved even in the resource-limited Indian scenario.
