Abstract:Objective: To compare the efficacy of inhaled salbutamol with salmeterol for the treatment of arterial hypoxaemia in anaesthetized horses. Study design: Prospective, randomized, clinical study. Animals: A total of 108 client-owned horses (American Society of Anesthesiologists status I-V) anaesthetized for elective and emergency procedures. Methods: Horses were premedicated with acepromazine [intramuscularly 0.1 mg kg-1 or intravenously (IV) 0.05 mg kg-1] and xylazine (0.6 mg kg-1 IV). Midazolam (0.06 mg kg-1 IV) and ketamine (2.2 mg kg-1 IV) were combined to induce anaesthesia, and isoflurane in oxygen/air mixture (inspired oxygen fraction 0.7) was used for maintenance of anaesthesia. Mechanical ventilation was initiated without delay using the following ventilator settings: tidal volume 10 mL kg-1, respiratory rate 8 breaths minute-1, inspiratory-to-expiratory time ratio 1:2, no positive end-expiratory pressure. If arterial blood gas analysis revealed PaO2 < 100 mmHg (13.3 kPa), the administration of either inhaled salbutamol (2 μg kg-1) or salmeterol (0.5 μg kg-1) was randomly assigned Blood gas analysis was repeated 15 and 30 minutes after treatment. The intervention was considered successful when PaO2 after treatment ≥ 1.2 × PaO2 before treatment (i.e. ≥20% increase). PaO2 at 15 and 30 minutes was compared between groups using Mann-Whitney U test; p < 0.05 was considered significant. Results: Of the 108 horses, 60 were administered salbutamol, 65% and 60% responded successfully at 15 and 30 minutes, increasing their initial PaO2 by 38% and 44%, respectively. The other 48 horses were administered salmeterol, 35% responded successfully at 15 and 30 minutes, increasing their initial PaO2 by 3% and 4%, respectively. PaO2 was significantly higher after salbutamol than after salmeterol at 15 and 30 minutes. Conclusions and clinical relevance: Using the described protocol, inhaled salbutamol was more effective than salmeterol in improving PaO2 in anaesthetized horses with value < 100 mmHg (13.3 kPa).

The efficacy of treatment on performance is unknown.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Hypothesis</h3><p>Treatment targeting lung inflammation improves O<sub>2</sub>peak in horses with mild asthma. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Animals</h3><p>Thoroughbred polo horses (n = 12) with smoke‐induced mild asthma. Horses were exposed to increased ambient particulate matter (35.51 μg/m<sup>3</sup> [PM<sub>2.5</sub>]; day mean, centrally measured) from day −33 to 0, from bushfire smoke (natural model). </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>Prospective, randomized, placebo‐controlled, double‐blinded clinical trial. All horses completed 3 O<sub>2</sub>peak tests, measuring aerobic and anaerobic variables: day 0 ‐baseline; day 16 ‐after dexamethasone (20 mg IM q24h; DEX, n = 6) or saline treatment (SALINE, n = 6), under improved ambient PM<sub>2.5</sub> concentrations (7.04 μg/m<sup>3</sup>); day 17‐15‐30mins after inhaled salbutamol (1500 μg). Bronchoalveolar lavage and mucus scoring were performed on day −8 and day 20. Linear mixed effects models were used to examine the effects of timepoint and treatment group on BAL differential cell counts, mucus scores, aerobic and anaerobic variables. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Horses' mucus scores improved significantly from day −8 to 20 by 1.27 ± .38 (<i>P</i> = .01). There was a significant increase in O<sub>2</sub>peak of 15.5 ± 4.0 mL(min.kg)<sup>−1</sup> from day 0 to 17 (<i>P</i> = .002), representing an average (mean) increase in O<sub>2</sub>peak of 13.2%. There was no difference in V˙O<sub>2</sub>peak between treatment groups (SALINE versus DEX) at any timepoint. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions and Clinical Importance</h3><p>This study highlighted the key role of improved air quality on functionally important airway inflammation. Evidence provided is central to increasing owner compliance regarding improved air quality for the treatment and prevention of mild asthma.</p></section>

Abstract:Aim: Salbutamol has multiple effects on the lungs. Computed cardiopulmonography (CCP) is a new method for assessing lung function by evaluating the homogeneity of lung emptying [1]. In order to better understand the responsiveness of measures, we have evaluated the effect of inhaled salbutamol in patients with T2-high asthma and healthy controls. Methods: 22 healthy controls (43±24 yr) and 77 patients (57±12 yr) with severe T2-high asthma were studied. All participants underwent a protocol which included CCP (a 12-min test as outlined in [1]) and standard spirometry. Assessments were conducted before and 20 min after the inhalation of salbutamol (400 μg) via a spacer. Results: Figure 1 shows the effects of salbutamol on forced expiratory volume in 1 sec (FEV 1 ) % predicted, and for CCP, a measure of ventilation inhomogeneity (lnCL) and anatomical deadspace (VD). Salbutamol increased FEV 1 % pred and decreased lnCL in both groups. However, salbutamol increased VD in only the healthy group. Apart from lnCL in the patient group, age did not influence the size of any effect associated with salbutamol. Conclusions: Salbutamol improved lung function in both groups. The absence of any effect on VD in asthma is intriguing and may perhaps relate to airway remodelling. Reference: 1. doi.org/10.1152/japplphysiol.00436.2022

Effects of dobutamine, dopamine, phenylephrine and noradrenaline on systemic haemodynamics and intestinal perfusion in isoflurane anaesthetised horses

Abstract:Background: In the horse, effects of cardiovascular-active drugs on local perfusion of the gastrointestinal tract are poorly understood. Objectives: To determine the effect of drugs commonly used to support blood pressure, on local intestinal blood flow and tissue oxygenation under isoflurane anaesthesia. Study design: In vivo randomised crossover experiment. Methods: Ten horses were anaesthetised with isoflurane. After 90 min of equilibration three doses (μg/kg bwt/min) of dobutamine (DOB 0.5/1/3), dopamine (DA 1/2/5), noradrenaline (NA 0.1/0.2/0.5) and phenylephrine (PHE 0.5/1/3) were infused for 15 min, in a randomised order, with a 45 min washout-period. Blood flow and tissue oxygenation (sO2 ) of jejunum, colon and stomach were measured using white light remission spectrophotometry and laser doppler flowmetry; heart rate (HR), mean arterial blood pressure (MAP), cardiac output (CO) were measured and systemic vascular resistance (SVR) calculated. Results: Compared to baseline high dose dobutamine significantly increased CO, HR, MAP (P<0.001) and blood flow to the jejunum (+47 ± 26%, P = 0.001) and colon (+29 ± 15%, P<0.001) (mean ± s.d.). Dopamine (DA5) increased CO but decreased colonic blood flow (-39 ± 21% from baseline, P<0.001), as well as SVR and MAP compared to baseline (P<0.001). Noradrenaline had no significant influence on intestinal perfusion, but increased MAP and SVR from baseline (P<0.001). Phenylephrine (PHE3) caused a significant decrease in blood flow and sO2 , most profoundly at the colon compared to baseline (flow -44 ± 21%; sO2 -16 ± 3%, P<0.001), while MAP and SVR increased and CO and HR decreased (P<0.001). Main limitations: The measurement technique only allows for flow measurements in arbitrary units, which can limit comparability to other techniques. Conclusion: At the investigated doses dobutamine improved systemic and peripheral haemodynamics, while dopamine decreased MAP and peripheral perfusion. Noradrenaline increased MAP and SVR while peripheral blood flow was maintained, phenylephrine increased MAP, but reduced both local and systemic perfusion.

Abstract:Objective To study salbutamol pretreatment on chronic obstructive pulmonary disease respiratory mechan-ics of laparoscopic surgery.Methods March 2012-May 2013 laparoscopic surgery in our hospital 50 patients with COPD were randomly divided into observation group and control group,25 cases in the observation group before induction of anesthesia albuterol sulfate inhalation aerosol,control group,no treatment were compared preoperative and intraoperative blood gas analysis and respiratory mechanics parameters.Results The pH value and PaCO2values T0 ～T4 no significant fluctuations during the period(P > 0.05),PaO2/FiO2decreased in T1-T3,T4 rise,T0-T4 statistically significant fluctuations during the period(P < 0.05),pH value of the control group,PaCO2values and the PaO2/FiO2at T0-T4 period there were significant fluctuations(P < 0.05),the observation group T2,T3 were significantly higher when the pH value,T2～T4 PaCO2values were significantly lower than the control group,T1～T4 PaO2/FiO2were significantly higher(P <0.05).Observation group T1～T4 Ppeak,Pplat,Raw significant fluctuations,showed increased and then decreased(P <0.05),Compl fluctuations were not significant(P > 0.05),the control group T1～T4 Ppeak,Pplat,Raw,Compl were there were significant fluctuations,Ppeak,Pplat,Raw were significantly increased and then decreased,Compl rebounded significantly lower T4(P < 0.05).Conclusion Salbutamol pretreatment can effectively alleviate the chronic obstructive pulmonary disease in patients undergoing airway stress response,inhibiting airway pressure and resistance increases,increased pulmonary compliance increased oxygen utilization,improve lung function,help to improve the anesthesia security.
Effects of the Balance in Activity of RhoA and Rac1 on the Shock-Induced Biphasic Change of Vascular Reactivity in Rats

Abstract:Objective:To investigate the effects of the balance in activity of RhoA and Rac1 on the shock-induced biphasic change in vascular reactivity and the related mechanism. Background:Vascular reactivity after hemorrhagic shock shows a biphasic change. RhoA and Rac1 are the main members of a family of Rho GTPases; whether and how they participate in the regulation of the biphasic change in vascular reactivity after shock is not known. Methods:The relationship of the balance of the activity RhoA and Rac1 with the changes in vascular reactivity after hemorrhagic shock, the effects of artificially changing the balance of RhoA and Rac1 activity on vascular reactivity, and the roles of Rho kinase and p21-activated kinase (PAK) in RhoA/Rac1 regulation of vascular reactivity were observed in isolated superior mesenteric arteries (SMAs) from hemorrhagic shocked rats and hypoxia-treated vascular smooth muscle cells (VSMCs). Results:The reactivity of SMAs and VSMCs to norepinephrine after shock or hypoxia was positively correlated with changes in the RhoA and Rac1 activity ratio. Artificially changing the balance in activity of RhoA and Rac1 significantly changed the shock-induced biphasic response of vascular reactivity. Specific antagonist of Rho kinase and PAK (Y-27632 and PAK-18) respectively abolished the effect of activation of RhoA and Rac1. Activation of RhoA significantly increased the activity of Rho kinase and inhibited the activity of Rac1 in SMAs. Rac1 activation significantly increased the activity of PAK and decreased the activity of RhoA. Conclusions:The balance in the activity of RhoA and Rac1 participated in the biphasic vascular reactivity seen after hemorrhagic shock. RhoA and Rac1 regulation of the vascular reactivity after shock are closely related to Rho kinase and the PAK pathway. RhoA regulates vascular reactivity mainly through activation of Rho kinase and inhibition of Rac1. Rac1 regulates vascular reactivity mainly through inhibition of RhoA and activation of PAK. These findings have potential significance for the treatment of vascular hyporesponsiveness.
Effects of consecutive protected specimen brushing and bronchoalveolar lavage on gas exchange and hemodynamics in ventilated patients

Abstract:Objective: To assess cardiovascular effects and the oxygenation status of mechanically ventilated patients undergoing protected specimen brushing (PSB) and bronchoalveolar lavage (BAL) under fiberoptic bronchoscopy (FOB). Design: A prospective study. Setting: Polyvalent intensive care unit in a university hospital. Patients: Twelve consecutive, critically ill, intubated, and mechanically ventilated patients with hemodynamic failure requiring invasive monitoring with an indwelling radial artery catheter and indwelling Swan-Ganz catheter were included in the study. Interventions: Hemodynamic measurements, arterial and mixed-venous blood gas analyses, and arterial blood lactate analysis were performed before and at the end of a 10-min period of mechanical ventilation with a fractional concentration of oxygen in the inspired gas (FIO2) of 1.0. The same measurements and blood samplings were repeated at the end of the PSB procedure, at the end of the BAL procedure, and 1 h after the end of the BAL. During the study period the ECG, arterial oxygen saturation (SaO2), and mixed-venous oxygen saturation (SvO2) were continuously monitored. Main results: A moderate increase in both mean arterial pressure and mean pulmonary arterial pressure was observed during the FOB procedure (p < 0.05). One hour after the end of BAL, the PaO2 decreased when compared with values recorded at the beginning of the procedure with the same FIO2 (p < 0.05). An increase in intrapulmonary shunt was observed at the end of BAL (p < 0.01). A moderate increase in PaCO2 was also observed after PSB (p < 0.05) and after BAL (p < 0.01). Monitoring of SaO2 permitted us to observe a significant and sustained decrease after the end of the FOB procedure from 10 to 60 min. The decrease in SvO2 was less pronounced but reached statistical significance. Conclusions: We conclude that PSB and BAL under FOB are well tolerated in critically ill, mechanically ventilated patients with hemodynamic disturbances requiring inotropic or vasopressor agents (or both); however, a modest impairment in arterial oxygenation was observed after the end of the FOB procedure.
Comparing the Effects of Two Different Levels of Hyperoxygenation on Gas Exchange During Open Endotracheal Suctioning: A Randomized Crossover Study

Abstract:Background: Endotracheal suctioning is required for mechanically ventilated patients to maintain a patent airway. Studies show that open endotracheal suctioning affects respiratory mechanics and gas exchange. The aim of this study was to compare the effectiveness of hyperoxygenation with FIO2 + 0.20 above baseline and hyperoxygenation with FIO2 1.0 in preventing hypoxemia, and to determine the impact of open endotracheal suctioning on the pulmonary ventilation of critical subjects receiving mechanical ventilatory support. Methods: This prospective randomized crossover study was conducted in the ICU. Sixty-eight mechanically ventilated subjects with FIO2 ≤ 0.6 and requiring endotracheal suctioning were included in this study. Open endotracheal suctioning was performed using 2 different intervention sequences: hyperoxygenation of 0.20 above baseline FIO2 (FIO2 + 0.20) and 1.0 hyperoxygenation (FIO2 1.0). Oxygenation was assessed via oxygen saturation as measured by pulse oximetry (SpO2 ), and changes in lung ventilation were measured via alveolar gas volume (V̇A), alveolar minute volume (V̇A min), carbon dioxide (CO2) production (V̇CO2 ), mixed exhaled partial pressure of CO2 (P̄ECO2 ), exhaled tidal CO2 volume (VTCO2), end-tidal CO2 (PETCO2 ), anatomical dead space to tidal volume ratio (VD/VT), and anatomical dead space volume of each breath (VD) using volumetric capnography, and breathing frequency (f), heart rate, and mean arterial pressure using a multiparameter monitor. Results: SpO2 levels were significantly higher within interventions (FIO2 + 0.20 and 1.0) 1 min before and after suction. Also, there was a significant increase in PETCO2 , P̄ECO2 , heart rate, and mean arterial pressure immediately after the procedure compared with baseline, and in VTCO2, only for FIO2 + 0.20. Baseline values were not found to be significantly different between the groups in case of any dependent variable. Conclusions: In mechanically ventilated adult subjects, hyperoxygenation with FIO2 + 0.20 above baseline prevents hypoxemia. Also, transient changes in pulmonary ventilation with open circuit suctioning were confirmed by volumetric capnography analysis. (Trial registration: ClinicalTrials.gov NCT02440919).

Tidal volume selection in volume-controlled ventilation guided by driving pressure versus actual body weight in healthy anesthetized and mechanically ventilated dogs: A randomized crossover trial

Abstract:Objective: To compare static compliance of the respiratory system (CstRS) and the ratio of partial pressure of end-tidal to arterial carbon dioxide (Pe'CO2/PaCO2), in healthy dogs using two approaches for tidal volume (VT) selection during volume-controlled ventilation: body mass based and driving pressure (ΔPaw) guided. Study design: Randomized, nonblinded, crossover, clinical trial. Animals: A total of 19 client-owned dogs anesthetized for castration and ovariohysterectomy. Methods: After a stable 10 minute baseline, each dog was mechanically ventilated with a VT selection strategy, randomized to a constant VT of 15 mL kg-1 of actual body mass (VTBW) or ΔPaw-guided VT (VTΔP) of 7-8 cmH2O. Both strategies used an inspiratory time of 1 second, 20% end-inspiratory pause, 4 cmH2O positive end-expiratory pressure and fraction of inspired oxygen of 0.4. Respiratory frequency was adjusted to maintain Pe'CO2 between 35 and 40 mmHg. Respiratory mechanics, arterial blood gases and Pe'CO2/PaCO2 were assessed. Continuous variables are presented as mean ± SD or median (interquartile range; quartiles 1-3), depending on distribution, and compared with Wilcoxon signed-rank tests. Results: The VT was significantly higher in dogs ventilated with VTΔP than with VTBW strategy (17.20 ± 4.04 versus 15.03 ± 0.60 mL kg-1, p = 0.036). CstRS was significantly higher with VTΔP than with VTBW strategy [2.47 (1.86-2.86) versus 2.25 (1.79-2.58) mL cmH2O-1 kg-1, p = 0.011]. There were no differences in Pe'CO2/PaCO2 between VTΔP and VTBW strategies (0.94 ± 0.06 versus 0.92 ± 0.06, p = 0.094). No discernible difference in ΔPaw was noted between the strategies. Conclusions and clinical relevance: While no apparent difference was observed in the Pe'CO2/PaCO2 between the VT selection strategies employed, CstRS significantly increased during the VTΔP approach. A future trial should explore if VTΔP improves perioperative gas exchange and prevents lung damage.

Abstract:Background: Impairment of ventilation and perfusion (V/Q) matching is a common mechanism leading to hypoxemia in patients with acute respiratory failure requiring intensive care unit (ICU) admission. While ventilation has been thoroughly investigated, little progress has been made to monitor pulmonary perfusion at the bedside and treat impaired blood distribution. The study aimed to assess real-time changes in regional pulmonary perfusion in response to a therapeutic intervention. Methods: Single-center prospective study that enrolled adult patients with ARDS caused by SARS-Cov-2 who were sedated, paralyzed, and mechanically ventilated. The distribution of pulmonary perfusion was assessed through electrical impedance tomography (EIT) after the injection of a 10-ml bolus of hypertonic saline. The therapeutic intervention consisted in the administration of inhaled nitric oxide (iNO), as rescue therapy for refractory hypoxemia. Each patient underwent two 15-min steps at 0 and 20 ppm iNO, respectively. At each step, respiratory, gas exchange, and hemodynamic parameters were recorded, and V/Q distribution was measured, with unchanged ventilatory settings. Results: Ten 65 [56-75] years old patients with moderate (40%) and severe (60%) ARDS were studied 10 [4-20] days after intubation. Gas exchange improved at 20 ppm iNO (PaO2/FiO2 from 86 ± 16 to 110 ± 30 mmHg, p = 0.001; venous admixture from 51 ± 8 to 45 ± 7%, p = 0.0045; dead space from 29 ± 8 to 25 ± 6%, p = 0.008). The respiratory system's elastic properties and ventilation distribution were unaltered by iNO. Hemodynamics did not change after gas initiation (cardiac output 7.6 ± 1.9 vs. 7.7 ± 1.9 L/min, p = 0.66). The EIT pixel perfusion maps showed a variety of patterns of changes in pulmonary blood flow, whose increase positively correlated with PaO2/FiO2 increase (R2 = 0.50, p = 0.049). Conclusions: The assessment of lung perfusion is feasible at the bedside and blood distribution can be modulated with effects that are visualized in vivo. These findings might lay the foundations for testing new therapies aimed at optimizing the regional perfusion in the lungs.

Methods A randomized,placebo-controlled and double-blinded trial was designed.Twenty-six patients were recruited and randomly allocated to salbutamol group(group E,n=13) and placebo group(group C,n=13).Eligible patients were elderly patients with ASA physical status Ⅱ-Ⅲ,a body mass index(BMI) between 18 and 30 kg/m2,well-defined clinical diagnosis of COPD.Routine monitoring(consists of a three-lead ECG,pulse oximetry,noninvasive blood pressure and expired gas analysis) was instituted and 500 mL Ringer's lactate solution was administered.Bispectral index(BIS) monitoring was initiated prior to induction.All subjects were received inhaled aerosol 200 μg according to manufacturers' recommendations 30 minutes before induction of anesthesia.Controlled ventilation was applied after the trachea was intubated.When stable hemodynamics was maintained for 5 minutes,fresh gas flow was set to 2 L/min with 2% sevoflurane in admixture,then HR,invasive arterial blood pressure(IABP),SpO2,PETCO2,bispectral index(BIS),minimum alveolar concentration(MAC),concentrations of inhaled sevoflurane(FI) and end-tidal(FE) were recorded at 1,2,3,4,5,7,10,and 15 minutes after inhalation of sevoflurne.The Ppeak and Pplat were also measured in 1,5,and 10 minutes after the successful endotracheal intubation. Results Compared to the placebo group,the FE was significantly higher at 2,3,4,and 5 minutes in experiment group.Peak airway pressure and plateau pressure in experiment group were strikingly lower than control group(P<0.05).There was no statistical significance about BIS variations between these groups in respective time in spite of increased MAC(P>0.05). Conclusions The increasing rate of alveolar concentration of sevoflurane was accelerated after the administration of inhaled salbutamol aerosol(200 μg) 30 minutes before induction of anesthesia,it might increase the uptake of sevoflurane.

Abstract:BACKGROUND: Hypoxemia in acute lung injury/acute respiratory distress syndrome (ALI/ARDS) patients represents a common finding in the intensive care unit (ICU) and frequently does not respond to standard ventilatory techniques.OBJECTIVE: To study whether the early short-term application of high-frequency percussive ventilation (HFPV) can improve gas exchange in hypoxemic patients with ALI/ARDS or many other conditions in comparison to conventional ventilation (CV) using the same mean airway pressure (P(aw)), representing the main determinant of oxygenation and hemodynamics, irrespective of the mode of ventilation.METHODS: Thirty-five patients not responding to CV were studied. During the first 12 h after admission to the ICU the patients underwent CV. Thereafter HFPV was applied for 12 h with P(aw) kept constant. They were then returned to CV. Gas exchange was measured at: 12 h after admission, every 4 h during the HFPV trial, 1 h after the end of HFPV, and 12 h after HFPV. Thirty-five matched patients ventilated with CV served as the control group (CTRL).RESULTS: PaO(2)/FiO(2) and the arterial alveolar ratio (a/A PO(2)) increased during HFPV treatment and a PaO(2)/FiO(2) steady state was reached during the last 12 h of CV, whereas both did not change in CTRL. PaCO(2) decreased during the first 4 h of HFPV, but thereafter it remained unaltered; PaCO(2) did not vary in CTRL. Respiratory system compliance increased after HFPV.CONCLUSIONS: HFPV improved gas exchange in patients who did not respond to conventional treatment. This improvement remained unaltered until 12 h after the end of HFPV.

Abstract:Background: Electrical impedance tomography (EIT) has been an essential tool for assessing pulmonary ventilation in several situations, such as the alveolar recruitment maneuver (ARM) in PEEP titration to maintain the lungs open after atelectasis reversion. In the same way as in humans and dogs, in horses, this tool has been widely used to assess pulmonary aeration undergoing anesthesia, mechanical ventilation, recruitment maneuver, standing horses, or specific procedures. Objectives: The present study aimed to evaluate the distribution of regional ventilation during ARM based on lung monitoring assessment by EIT, with a focus on better recruitment associated with less or no overdistention. Methods: Fourteen horses of 306 ± 21 kg undergoing isoflurane anesthesia in dorsal recumbency were used. The animals were mechanically ventilated with a tidal volume of 14 ml kg-1 and a respiratory rate of 7-9. An alveolar recruitment maneuver was instituted, increasing the PEEP by five cmH2O every 5 min until 32 cmH2O and decreasing it by five cmH2O every 5 min to 7 cmH2O. At each step of PEEP, arterial blood samples were collected for blood gas analysis, EIT images, hemodynamic, and respiratory mechanics. Results: Associated with the CoV-DV increase, there was a significant decrease in the DSS during the ARM and a significant increase in the NSS when PEEP was applied above 12 cmH2O compared to baseline. The ComplROI showed a significant increase in the dependent area and a significant decrease in the non-dependent area during ARM, and both were compared to their baseline values. The driving pressure decreased significantly during the ARM, and Cst, PaO2, and PaO2/FiO2 ratio increased significantly. The VD/VT decreased significantly at DEPEEP17 and DEPEEP12. There was an HR increase at INPEEP27, INPEEP 32, and DEPEEP17 (p < 0.0001; p < 0.0001; and p < 0.05, respectively), those values being above the normal reference range for the species. The SAP, MAP, DAP, CI, and DO2I significantly decreased INPEEP32 (p < 0.05). Conclusion: The ARM by PEEP titration applied in the present study showed better ventilation distribution associated with better aeration in the dependent lung areas, with minimal overdistention between PEEP 17 and 12 cmH2O decreasing step. Those changes were also followed by improvements in static and regional compliance associated with increased oxygenation and pulmonary ventilation. ARM promoted a transitory decrease in arterial blood pressure and depression in CI with a concomitant drop in oxygen delivery, which should be best investigated before its routine use in clinical cases.

Abstract:Pyrethroids are commonly used commercial and domestic insecticides that infrequently result in human intoxication. Type I pyrethroids delay closure of voltage-gated sodium channels leading to protracted sodium infl ux (tail current) and membrane depolarization. 1,2 At high doses, the sodium tail current may be suffi cient to prevent further action potential generation. Additional chloride channel inhibition by Type II pyrethroids such as Bifenthrin and Fenvalerate lowers the threshold for action potential generation and increases neuronal hyperexcitability. 2,3 causing profuse salivation, ataxia, hyperactivity, choreoathetosis and seizures in animals. 4 In humans, dermal exposure leads to paresthesias and ingestion gives rise to nausea, vomiting, abdominal pain, increased secretions and dysphagia. Systemic effects such as dizziness, headache, fatigue, palpitations, chest tightness, blurred vision and convulsions occur by 4 – 48 h following ingestion. Mortality is low (1 – 2%) and most patients recover within 6 days following exposure. 1,4 Defi nitive diagnostic testing is not freely available and management is supportive as no specifi c antidote exists. Type II pyrethroid poisoning resulting in seizures in humans has been reported only in cases of suicidal or accidental ingestion but not after inhalational exposure. Low systemic toxicity by Type II pyrethroids may be due to poor dermal absorption and 2000-fold higher selectivity for insects over mammals. 1

Abstract:Based on the denoising and stratifying of the well logs as well as strata correlation, a regular grid surface model was given to describe the geologic model of the aquifer. The 3D aquifer model was built by using the kriging spatial prediction method. The spatial distribution of the water-bearing layers and the water-resisting layers was well known by using the generalized tri-prism model to cut and clip the 3D aquifer model. Short-term effects of noisy pressure support ventilation in patients with acute hypoxemic respiratory failure

Abstract:IntroductionThis study aims at comparing the very short-term effects of conventional and noisy (variable) pressure support ventilation (PSV) in mechanically ventilated patients with acute hypoxemic respiratory failure.MethodsThirteen mechanically ventilated patients with acute hypoxemic respiratory failure were enrolled in this monocentric, randomized crossover study. Patients were mechanically ventilated with conventional and noisy PSV, for one hour each, in random sequence. Pressure support was titrated to reach tidal volumes approximately 8 mL/kg in both modes. The level of positive end-expiratory pressure and fraction of inspired oxygen were kept unchanged in both modes. The coefficient of variation of pressure support during noisy PSV was set at 30%. Gas exchange, hemodynamics, lung functional parameters, distribution of ventilation by electrical impedance tomography, breathing patterns and patient-ventilator synchrony were analyzed.ResultsNoisy PSV was not associated with any adverse event, and was well tolerated by all patients. Gas exchange, hemodynamics, respiratory mechanics and spatial distribution of ventilation did not differ significantly between conventional and noisy PSV. Noisy PSV increased the variability of tidal volume (24.4 ± 7.8% vs. 13.7 ± 9.1%, P <0.05) and was associated with a reduced number of asynchrony events compared to conventional PSV (5 (0 to 15)/30 min vs. 10 (1 to 37)/30 min, P <0.05).ConclusionsIn the very short term, noisy PSV proved safe and feasible in patients with acute hypoxemic respiratory failure. Compared to conventional PSV, noisy PSV increased the variability of tidal volumes, and was associated with improved patient-ventilator synchrony, at comparable levels of gas exchange.Trial registrationClinicialTrials.gov, NCT00786292

Abstract:OBJECTIVES: In mechanical ventilation, normoventilation in terms of PCO2 can be achieved by titration of the respiratory rate and/or tidal volume. Although a linear relationship has been found between changes in respiratory rate and resulting changes in end-tidal cO2 (△PetCO2) as well as between changes in respiratory rate and equilibration time (teq) for mechanically ventilated patients without lung injury, it is unclear whether a similar relationship holds for acute lung injury or altered hemodynamics.DESIGN: We performed a prospective randomized controlled animal study of the change in PetCO2 with changes in respiratory rate in a lung-healthy, lung-injury, lung-healthy + altered hemodynamics, and lung-injury + altered hemodynamics pig model.SETTING: University research laboratory.SUBJECTS: Twenty mechanically ventilated pigs.INTERVENTIONS: Moderate lung injury was induced by injection of oleic acid in 10 randomly assigned pigs, and after the first round of measurements, cardiac output was increased by approximately 30% by constant administration of noradrenalin in both groups.MEASUREMENTS AND MAIN RESULTS: We systematically increased and decreased changes in respiratory rate according to a set protocol: +2, -4, +6, -8, +10, -12, +14 breaths/min and awaited equilibration of Petco2. We found a linear relationship between changes in respiratory rate and △PetCO2 as well as between changes in respiratory rate and teq. A two-sample t test resulted in no significant differences between the lung injury and healthy control group before or after hemodynamic intervention. Furthermore, exponential extrapolation allowed prediction of the new PetCO2 equilibrium and teq after 5.7 ± 5.6 min.CONCLUSIONS: The transition between PetCO2 equilibria after changes in respiratory rate might not be dependent on moderate lung injury or cardiac output but on the metabolic production or capacity of cO2 stores. Linear relationships previously found for lung-healthy patients and early prediction of PetCO2 equilibration could therefore also be used for the titration of respiratory rate on the PetCO2 for a wider range of pathologies by the physician or an automated ventilation system.

Abstract:Objective To assess the effect of salbutamol inhalation on expiratory flow limitation (EFL) in patients with chronic obstructive pulmonary disease(COPD). Methods EFL in 28 moderate to severe COPD patients (aged 70±10 years) was measured by applying negative expiratory pressure (NEP) at the mouth during tidal expiration. EFL was present in all of these patients. Pulmonary function tests and NEP ( 5cmH 2O) test were performed at rest in a sitting position before and 20 min after inhalation of salbutamol 400μg. Results After salbutamol administration, all patients exhibited a significant increase in FEV 1,FVC and FEF50 iso vol , an obvious decrease in FL index (P0.01). Eleven patients had reversal of their bronchial obstruction (FEV 1≥15% in positive group) and the others had no such reversal in negative group. The changes in FL index and FEF50 iso vol had no difference between these 2 groups, FL index had a significant correlation with FEF50 iso vol (r=0.508, P0.01). Conclusions COPD patients might benefit from bronchodilators. After inhalation of salbutamol, EFL might decreased significantly. The change in EFL is correlated with that in FEF50 iso vol
[Application of capnography and SpO2 measurement in the evaluation of respiratory failure in patients with chronic obstructive pulmonary disease]

Abstract:Objective: To investigate the accuracy and potential error range of noninvasive estimation of CO2 pressure (PCO2), arterial O2 pressure (PaO2), and oxygenation index (OI) by measuring the end-tidal CO2 pressure (PETCO2) and pulse oxygen saturation (SpO2) in patients with chronic obstructive pulmonary disease (COPD) and respiratory failure, and assess the feasibility of this method for dynamic monitoring of arterial CO2 pressure (PaCO2) and PaO2 in the primary care facilities where arterial blood gases analysis is not available. Methods: All the 30 patients with COPD and respiratory failure received routine clinical treatment including bronchodilators, mucolytics, glucocorticosteroid, antibiotics and oxygen therapy (titrated to keep SpO2 above 90%) for 5-7 days. A subgroup of the patients also received NIPPV treatment. All the patients were tested with both the eupnea method and prolonged expiratory method before and after the treatment to obtain the data of PCO2 and SpO2 were respectively performed before and after treatment. Results: The PETCO2 with eupnea (PETCO2(Q)) was 50.72-/+8.93 mmHg, significantly lower than PaCO2 (71.25-/+9.08 mmHg, Plt;0.01), but the PETCO2(P) (70.35-/+8.91 mmHg) was comparable with PaCO2 (P>0.05). Similar results were obtained after the treatment. The PETCO2(P) before treatment and after treatment was positively correlated to PaCO2 (r=0.96 and 0.97, respectively, P<0.01). The PaO2(Y) before the treatment derived from the oxygen dissociation curve based on SpO2 measurement was close to SpO2 (59.96-/+1.42 mmHg vs 59.07-/+2.22 mmHg, P>0.05). The OI derived from PaO2 and OI(Y) from PaO2 (Y) was also similar (215.70-/+22.77 vs 219.15-/+24.63, P>0.05). Linear regression analysis showed positive correlations between PaO2(Y) to PaO2 (r=0.81, P<0.01) and between OI(Y) and OI (r=0.95, P<0.01). Conclusions: In patients with COPD (especially those with also type II respiratory failure), the modified monitoring method of PCO2 and maintenance of SpO2 above 90% can precisely estimate PaCO2 and PaO2. This method is feasible for clinical noninvasive and dynamic evaluation of respiratory failure in COPD patients, especially in primary care facilities where arterial blood gases analysis is not available.
