Abstract:Aspergillus is a genus of filamentous, ubiquitous, and opportunistic pathogenic fungi. Patients with underlying lung disease are susceptible to Aspergillus due to their immunodeficiency, but the symptoms become atypical as other lung diseases are contracted (1-3). Aspergillus fumigatus, A. flavus, and A. niger are the primary pathogenic species in most literatures (4, 5). The principal forms of pulmonary aspergillosis (PA) are allergic bronchopulmonary aspergillosis (ABPA), chronic (and saprophytic) pulmonary aspergillosis (CPA), and invasive pulmonary aspergillosis (IPA). In non-neutropenic patients, various forms are considered a semi-continuous spectrum of aspergillosis (4). The analysis of the clinical features can deepen our understanding of the spectrum of clinical manifestations of Aspergillus infection and provide references for its diagnosis.

Utility of CT Assessment in Hematology Patients with Invasive Aspergillosis: a Post-Hoc Analysis of Phase 3 Data

Abstract:Pulmonary computed tomography (CT) scans are commonly used as part of the clinical criteria in diagnostic workup of invasive fungal diseases like invasive aspergillosis, and may identify radiographic abnormalities, such as halo signs or air-crescent signs. We assessed the diagnostic utility of CT assessment in patients with hematologic malignancies or those who had undergone allogeneic hematopoietic stem cell transplantation in whom invasive aspergillosis was suspected. This post-hoc analysis assessed data from a prospective, multicenter, international trial of voriconazole (with and without anidulafungin) in patients with suspected invasive aspergillosis (IA; proven, probable, or possible, using 2008 European Organisation for Research and Treatment of Cancer/Invasive Fungal Infections Cooperative Group and the National Institute of Allergy and Infectious Diseases Mycoses Study Group criteria) [NCT00531479]. Eligible patients received at least one baseline lung CT scan. Of 395 patients included in this post-hoc analysis, 240 patients (60.8%) had ‘confirmed’ proven (9/240, 3.8%) or probable (231/240, 96.3%) invasive aspergillosis (cIA) and 155 patients (39.2%) had ‘non-confirmed’ invasive aspergillosis (all nIA; all possible IA (de Pauw et al., Clin Infect Dis 46:1813–21, 2008)). Mean age was 52.3 and 50.5 years, 56.3 and 60.0% of patients were male, and most patients were white (71.7 and 71.0%) in the cIA and nIA populations, respectively. Median baseline galactomannan was 1.4 (cIA) and 0.2 (nIA), mean Karnofsky score was 65.3 (cIA) and 66.8 (nIA), and mean baseline platelet count was 48.0 (cIA) and 314.1 (nIA). Pulmonary nodules (46.8% of all patients), bilateral lung lesions (37.5%), unilateral lung lesions (28.4%), and consolidation (24.8%) were the most common radiographic abnormalities. Ground-glass attenuation (cIA: 24.2%; nIA: 11.6%; P < 0.01) and pulmonary nodules (cIA: 52.5%; nIA: 38.1%; P < 0.01) were associated with cIA. Other chest CT scan abnormalities (including halo signs and air-crescent signs) at baseline in patients with hematologic malignancy or hematopoietic stem cell transplantation, and suspected IA, were not associated with cIA. These findings highlight the limitations in the sensitivity of chest CT scans for the diagnosis of IA, and reinforce the importance of incorporating other available clinical data to guide management decisions on individual patients, including whether empirical treatment is reasonable, pending full evaluation. [Chronic Necrotizing Pulmonary Aspergillosis: Diagnosis and Treatment].

Abstract:OBJECTIVE:The purpose of this study was to describe the approaches to the diagnosis and the treatment of chronic necrotizing pulmonary aspergillosis (CNPA).METHODS:The study included 8 patients with CNPA confirmed by histopathological studies. The patients were admitted to the First Affiliated Hospital of College of Medicine of Zhejiang University during 2000-2004. The clinical data were analyzed, and related literature was reviewed. The clinical, radiological and pathological characteristics, and the approaches to the diagnosis and the treatment were described.RESULTS:CNPA was a rare form of pulmonary aspergillosis. Common presentations included productive cough (8/8), haemoptysis (7/8) and constitutional symptoms (5/8). Radiographically, the air crescent sign was suggestive of the diagnosis, which was found in 4 patients. Consolidation was also common in the other 4 cases. The histological feature of CNPA included the presence of tissue invasion by Aspergillus, tissue necrosis and granulomatous inflammation.CONCLUSIONS:The clinical manifestations of CNPA are non-specific. Radiography plays a pivotal role in the diagnosis of CNPA. Treatment with antifungal medications is indicated once the diagnosis is made. Pulmonary resection should be considered when patients have prolonged illness or frequent haemoptysis.
[Extramedullary plasmocytoma of the nasal cavity: a case report].

Abstract:PURPOSE OF STUDY To establish a diagnosis and therapeutic management in patients with extramedullar plasmocytomas. METHOD There is no consensus concerning extramedullar plasmocytoma treatment. One patient with a nasal cavity tumor location and a review of literature are reported. RESULTS Extramedullar plasmocytoma is a rare tumor that occurs most frequently in the upper respiratory tract. The most common location is the nasal cavity. Diagnosis can only be made after histological and immunohistochemical examinations. Localized tumors are treated by radiation therapy, combined or not with surgery. Disseminated diseases are treated by chemotherapy. CONCLUSION Diagnosis can only be confirmed after the exclusion of a systemic disease (multiple myeloma).

Abstract:OBJECTIVE: Patients with acute-on-chronic liver failure (AoCLF) are prone to various infections, including invasive pulmonary aspergillosis (IPA). This retrospective study investigated the relationship between AoCLF and IPA in a large patient cohort. METHODS: Twenty-nine out of 470 patients with AoCLF, who were diagnosed with definite or probable IPA by clinical and laboratory parameters and were treated with voriconazole, were analysed for predisposing factors for IPA and clinical outcome. RESULTS: Imaging findings, such as the halo sign and increased white blood cell count, contributed to the early diagnosis of IPA. Patients with AoCLF and IPA experienced fever, cough and chest pain and, despite treatment with antifungal therapy, most (25/29 patients) died within 7 days. Possible risk factors for IPA included prolonged antibiotic therapy and dexamethasone exposure. CONCLUSIONS: AoCLF with prolonged antibiotic therapy and dexamethasone exposure carries a high risk for IPA. AoCLF patients with IPA involved in this study exhibited fever, cough, chest pain and increased white blood cell count, and their imaging findings were useful for the early diagnosis of IPA.

Abstract:Chronic pulmonary aspergillosis (CPA) is an uncommon but dreaded complication of many respiratory diseases occurring in non- or mildly immunocompromised patients [1]. CPA affects approximately three million people worldwide, an estimation which is undoubtedly affected by under-reporting of this neglected disease [2]. The morbidity and mortality is high, with 5-year survival rates of 15–60% depending on comorbidities and age [3, 4]. The diagnosis of CPA is typically established on a combination of clinical, radiological and microbiological criteria all present for at least 3 months and the absence of an alternative diagnosis [1]. The most common form of CPA is chronic cavitary pulmonary aspergillosis (CCPA), besides other disease entities, including chronic fibrosing pulmonary aspergillosis (CFPA), Aspergillus nodule(s) and single aspergilloma. Subacute invasive pulmonary aspergillosis (SAIA), also called chronic necrotising pulmonary aspergillosis, usually occurs in moderately immunocompromised patients and comprises a more rapidly progressive clinical course (<3 months) [1].

Abstract:The prevalence of pulmonary aspergillosis is increasing in patients with chronic obstructive pulmonary disease (COPD) and can manifest in different forms such as invasive pulmonary aspergillosis (IPA), chronic pulmonary aspergillosis (CPA) and allergic bronchopulmonary aspergillosis (ABPA). With the variations of individual conditions such as immune status, these forms of the disease may transform into each other or even overlap. Moreover, the atypical clinical manifestations and the limited use of invasive sampling techniques have posed a challenge to the diagnosis and treatment of invasive pulmonary aspergillosis in patients with COPD. To provide recommendations for the management of pulmonary aspergillosis in patients with COPD and to construct a clinical workflow, the consensus panel reviewed the evidence and critically appraised the existing studies. As the majority of the recommendations were supported by low levels of evidence, the evidence levels were not listed in the consensus and the strong and weak recommendations were expressed as "recommend" and "suggest", respectively.Recommendations for COPD with IPA: The Panel recommends that high-resolution chest computed tomography (HRCT) be performed in patients suspected with IPA. If IPA cannot be excluded by CT scanning, mycological examination of sputum and bronchoalveolar lavage fluid (BALF) is recommended. Bronchoscopy and BALF Aspergillus-related examination are recommended in COPD patients with respiratory symptoms such as dyspnea despite the use of broad-spectrum antibiotics and systemic glucocorticoids and pulmonary infiltrates observed on chest CT. If the diagnosis is in doubt in patients with probable IPA, histopathological examination is recommended. In COPD patients with an acute infection of more than 10 days' duration, the Panel recommended the detection of Aspergillus-specific IgG antibodies in peripheral blood to aid in the diagnosis of IPA, especially in those who cannot obtain BALF. It is not recommended to initiate antifungal therapy based on clinical symptoms such as cough, fever, and dyspnea empirically in COPD patients. In critically ill patients (such as those admitted to ICU and those with respiratory failure) who are unresponsive to broad-spectrum antibiotic treatment and have imaging findings consistent with IPA, patients with HRCT or bronchoscopy findings consistent with airway invasive aspergillosis, patients with a history of oral or intravenous glucocorticoid use in the past 3 months, or patients with a history of airway Aspergillus infection or colonization, empirical antifungal therapy may be initiated after a comprehensive evaluation of Aspergillus infection risk, and at the same time, pathogen examination should be started as early as possible. Voriconazole, isavuconazole, and posaconazole are recommended as the first-line treatments for COPD with IPA. Echinocandins and amphotericin B may be used as alternative options. Antifungal treatment for COPD with IPA should be continued for at least 6-12 weeks. The duration of antifungal therapy should be determined based on clinical symptoms, pulmonary imaging, and microbiological test results. Significant lesion absorption and stabilization, as well as the elimination of related risk factors, are important references for discontinuation of treatment.Recommendations on COPD with CPA: Chest CT scan and dynamic observation are recommended for COPD with suspected CPA. Peripheral blood Aspergillus-specific IgG antibody testing is recommended in COPD patients with suspected CPA. For those who are difficult to diagnose by routine methods or need further differential diagnosis, pulmonary tissue histopathological examination is recommended. Oral itraconazole solution or voriconazole tablets are recommended as the first-line treatment options for COPD with CPA. Oral isavuconazole capsules or enteric-coated posaconazole tablets can be used as an alternative. Intravenous administration of echinocandins or amphotericin B (deoxycholate or lipid formulations) are suggested as a second-line treatment options in cases of triazole treatment failure, resistance, or intolerance. Antifungal treatment for COPD with CPA should be continued for at least 6 months, and for patients with CCPA for at least 9 months. In those with cavities communicating with the bronchial lumen, if systemic antifungal therapy is ineffective or cannot be tolerated due to adverse reactions, and surgery is also not feasible, the Panel suggests considering nebulized inhalation of amphotericin B and intracavitary injection of amphotericin B or azoles (voriconazole, itraconazole) to control recurrent hemoptysis.Recommendations on COPD with Aspergillus sensitization: When COPD patients present with refractory wheezing and/or rapid decline in lung function, it is recommended that an assessment for Aspergillus sensitization be performed, including Aspergillus-specific IgE, skin Aspergillus antigen test, Aspergillus-specific IgG, total IgE, blood eosinophil count, and sputum examination. The Panel recommends that antifungal therapy should not be routinely initiated in COPD patients with Aspergillus sensitization. For those who meet the diagnostic criteria for ABPA, antifungal therapy is suggested. The most commonly used medication is oral itraconazole solution, but other azoles such as voriconazole, isavuconazole and posaconazole enteric-coated tablets can also be chosen. The general course of antifungal therapy is 3-6 months.Recommendations on the use of glucocorticoids in COPD with pulmonary aspergillosis: In exacerbating COPD patients with secondary IPA or subacute invasive aspergillosis, the Panel suggests that the use of glucocorticoids should be controlled. For COPD patients with concomitant CPA who experience exacerbations with predominantly wheezing, it is suggested that short-term, low-dose glucocorticoids be considered on the basis of antifungal treatment to control symptoms. Glucocorticoid use for COPD exacerbations is suggested to be guided by peripheral blood eosinophil count. It is recommended to avoid systemic glucocorticoids and long-term or high-dose inhaled glucocorticoids (ICS) in stable COPD patients with concomitant CPA. In patients with concomitant Aspergillus sensitization and persistent wheezing despite standardized COPD treatment or patients with ABPA, the Panel recommends systemic glucocorticoids in combination with antifungal therapy and consideration of the use of ICS to reduce the dose of systemic glucocorticoids. Close monitoring for progression to IPA or subacute invasive aspergillosis is essential during treatment.
Clinical Analysis on Ninty-Six Cases of Secondary Aspergillosis in COPD Patients

Abstract:Objective To discuss the clinical features and control measure of respiratory infection associated with aspergillosis secondary to chronic obstructive pulmonary disease.Method 96 cases of COPD patients with aspergillus in their sputum culture,admitted into the First Affiliated Hospital of Zhejiang University from June 2007 to June 2008,were collected and their clinical manifestations,treatment and treatment outcome were analyzed.Result Among the 96 cases of COPD patients with aspergillosis,9 cases patients died after treatment,87 cases improved including 45 cases treated with non-antifungal agents,38 cases with fluconazde and 13 cases with the addition of Voriconazole.Conclusion COPD patients with aspergillosis were increasing in recent years.The risk factors of aspergillus infection includes repeatedly long-term hospitalization,advance in age,irrational antibiotics and hormones application,as well as latrogenic and invasive operation.Early diagnosis,early treatment and rational precautionary can be a key to reduce the occurrence of respiratory infection by aspergillus.
Clinical Findings in 19 Cases of Invasive Pulmonary Aspergillosis with Liver Cirrhosis

Abstract:BACKGROUND:Aspergillus infection was mostly reported with high mortality rates and a bad prognosis in immunocompromised patients, but data were lacking on the clinical characteristics of aspergillus infection in liver cirrhosis. The aim of this study was to retrospectively assess the morbidity and mortality rate, clinical manifestation, risk factors, and medication of invasive pulmonary aspergillosis (IPA) in liver cirrhosis in The First Affiliated Hospital, College of Medicine, Zhejiang University.METHODS:Patients with liver cirrhosis who had been diagnosed with proven or probable IPA by clinical and laboratory parameters from 1st December 2008 to 1st May2012 were retrospectively evaluated for predisposing factors for IPA and clinical outcome. The follow up ended on 30th July2012. IPA was defined according to European Organization for Research and Treatment of Cancer/Mysoses Study group criteria.RESULTS:In total, 6,600 patients with liver cirrhosis were enrolled, and 19 out of these developed IPA. Seventeen out of 19 patients died. Imaging findings such as the halo sign and lower respiratory tract infection symptoms contributed to the early diagnosis of IPA. Possible risk factors for IPA included a high Child-Turcotte-Pugh (CTP) score, broad antibiotic usage and steroid exposure. The use of antifungal compounds may prolong a patient's life.CONCLUSIONS:The mortality of liver cirrhosis with IPA is high. Liver cirrhosis should be considered a risk factor of IPA. Once patients with high CTP scores and steroid and broad spectrum antibiotics exposure present cough and fever, IPA should be taken into consideration and antifungal agents should be used as soon as possible.

Abstract:Pulmonary aspergillosis is a significant global health concern, although still too often neglected and underdiagnosed. It affects people who are immunocompromised, those with underlying lung disease, or even previously healthy individuals. In the human lung, the ubiquitous fungus Aspergillus causes a wide spectrum of disease that cannot be untangled from the host's immune state. In this issue of Seminars of Respiratory and Critical Care Medicine , we aim to elucidate the spectrum of pulmonary aspergillosis in all its facets. In the first chapter, the authors unravel how Aspergillus can take up residence in the lung and how the interaction between host and pathogen determines the development of distinct and overlapping clinical syndromes, providing a complete overview of the disease spectrum from an immunological point of view.[1] We are all breathing in many fungal spores every day, but usually, these do not cause illness. In patients with structural airway damage, asymptomatic colonization, or inappropriate type 2 immune responses may occur, and when the host environment looks welcoming, such as in the presence of preexistent cavities, an aspergilloma may start to grow in a saprophytic way. When the host immune response falls short, chronic cavitary pulmonary aspergillosis may develop, and in the severely immunocompromised, invasive aspergillosis might install. In clinical practice, one of the main challenges remains the diagnosis of pulmonary aspergillosis—for instance, to assess the relevance of Aspergillus spp. cultured from a respiratory specimen, or to interpret nonspecific findings on chest computed tomography in high-risk patients. Four subsequent chapters of this issue are, therefore, dedicated to the diagnostic toolbox available for pulmonary aspergillosis, among which many tools are part of the established diagnostic criteria for the different disease entities. First, the existing microbiological diagnostic modalities and their future prospects are being discussed.[2] Second, the increasing problem of antifungal resistance in Aspergillus spp. is reviewed, including the mechanisms of resistance and their therapeutic implications, emphasizing the urgency of surveillance and research in this area.[3] Third, Jensen and Becker provide a comprehensive and richly illustrated overview of histopathological diagnosis as a gold standard in differentiating between fungal contamination, commensalism, and disease states—from angioinvasive infection to hypersensitivity pneumonitis.[4] Fourth, Laurent et al. highlight the radiological features that aid in the diagnosis and classification of pulmonary aspergillosis, and touch upon promising new imaging strategies such as magnetic resonance and artificial intelligence.[5] Before diving deeper into the different clinical entities of pulmonary aspergillosis, the current therapeutic antifungal landscape is explored, offering a glimpse of the promising antifungal pipeline.[6] Many of these novel therapies hold significant potential to treat different forms of pulmonary aspergillosis—also beyond the classical risk groups and presentations. Toxicity, drug–drug interactions, and resistance development warrant an expansion of our antifungal treatment arsenal—not only in acute but also in chronic pulmonary aspergillosis (CPA), where oral formulations are the preferred treatment. One of the challenges for the mycological community is, therefore, to ensure capacity building to set up randomized trials in these neglected populations. In addition, further research is needed to explore therapies directed at the host immune response against Aspergillus infection. In the second half of this series, we are zooming in on clinical presentation and management of the main disease entities within the pulmonary aspergillosis spectrum. Although acute invasive pulmonary aspergillosis (IPA) is traditionally considered in classical risk groups such as prolonged and severe neutropenia, the authors outline the importance of looking beyond the EORTC/MSGERC host factors, such as critical illness, chronic obstructive pulmonary disease, liver disease, and the increasingly recognized phenomenon of virus-associated IPA.[7] In contrast, CPA remains one of the most neglected forms of Aspergillus disease, both in terms of diagnostic and therapeutic options. Optimized Antifungal Therapy for Chronic Pulmonary Aspergillosis

Abstract:Chronic pulmonary aspergillosis (CPA) represents a spectrum of lung disorders caused by local proliferation of Aspergillus hyphae in individuals with non-systemic or mildly systemic immunodepression or altered pulmonary integrity due to underlying disease. While long-term systemic antifungal treatment is still the mainstay for management, surgery is considered mainly in rarer invasive disease manifestations such as sinusitis and osteomyelitis. Optimal application of existing antifungal agents with suitable pharmacokinetic properties is important for the treatment of diseases such as CPA, which requires long-term use. Appropriate management of side effects by therapeutic drug monitoring, maintenance of adherence, and assessment of drug resistance to Aspergillus can provide safe and effective treatment in the future. Most available antifungal agents for the management of mycoses in humans have disadvantages that can limit their use in clinical practice. By contrast, second generation antifungals such as triazoles have advantages of extended antifungal spectrum and availability in both oral and intravenous formulations. Isavuconazole, a new extended spectrum triazole, has been shown to be effective against Aspergillus. The safety profile and excellent pharmacokinetic characteristics of isavuconazole make it an attractive option for treatment of invasive fungal infections including CPA. With this drug now available in Japan, new evidence is expected to expand treatment options. This review focuses on the selection of antifungal agents based on national and international guidelines and the characteristics of each agent for their appropriate use in CPA.
Evaluation of a Commercial Quantitative Aspergillus Fumigatus-Specific IgM Assay for the Diagnosis of Invasive Pulmonary Aspergillosis.

Abstract:Background The early diagnosis of chronic pulmonary aspergillosis (CPA) remains challenging. Currently, a new quantitative IgM antibody assay that enveloped purified galactomannan extracted from Aspergillus fumigatus as antigen was commercially available in China, but its diagnostic value remains to be established. Methods Results To evaluate the role of this commercial IgM assay for diagnosis of CPA, a multi-center prospective study was performed in 12 hospitals in Zhejiang Province, from January 1 to December 31, 2016. Adult inpatients without severe immunocompromised condition and those with persistent clinical symptoms/radiological findings >= 3 months or a consistent appearance in chest CT that was suggestive of CPA were included. The clinical data were compiled using a structured questionnaire. A total of 87 cases were enrolled, including 43 CPA. The sensitivity ranged from 16.3% to 46.5%, and the specificity from 77.3% to 95.5% when cutoffs were from 50 to 80 arbitrary unit (AU)/mL. The receiver operating characteristic analysis revealed an area under the curve of 0.627. When a cutoff of 38.12 AU/mL was applied, the sensitivity and specificity were 69.8% and 56.8%, respectively, which represents the best performance. Conclusions To our knowledge, this study was the first to evaluate this commercial A. fumigatus-specific IgM antibody assay in CPA patients in China and indicates that this IgM assay has a limited value and should not be a prior recommendation for CPA diagnosis.

Abstract:Patients with chronic pulmonary aspergillosis (CPA) have a poor prognosis and CPA occurs in patients with various underlying diseases. Recently, the number of patients with CPA complicated by nontuberculous mycobacteria (NTM) has increased. Additionally, complications of both diseases have several problems like drug interactions. Since the impact of NTM on the outcome of CPA is not well understood, we investigated the risk factors for developing CPA and the clinical characteristics of CPA patients with or without NTM. We retrospectively investigated the medical records of NTM and CPA patients who were admitted to Nagasaki University Hospital between April 2008 and September 2013. Comorbid diseases, causative microorganisms, radiological findings, and outcomes were evaluated. During the study period, 82 and 41 patients were diagnosed as having NTM and CPA, respectively. Nine patients were coinfected with NTM and CPA, and cavitary type NTM and steroid usage were independent risk factors of development of CPA. Mortality rates in the coinfection group were significantly higher than those of the NTM without CPA group (P = .003, log-rank test). The rate of treatment initiation in the co-infection group (33.3%) was significantly lower than in the CPA without NTM group (84.4%) (P = .006). However, there were no significant differences in cumulative survival rate between both groups (P = .760, log-rank test). Cavity formation and steroid usage were the independent risk factors for NTM patients to develop CPA within long observation period, and development of CPA made outcomes poor. It is important to diagnose the development of CPA early and initiate treatment for CPA.

Abstract:Objective: The aim of this study was to describe clinical features in different subtypes of chronic pulmonary aspergillosis (CPA)-simple aspergilloma (SA), chronic cavitary pulmonary aspergillosis (CCPA), chronic fibrosing pulmonary aspergillosis (CFPA), aspergillus nodule (AN), and subacute invasive aspergillosis (SAIA), respectively, and identify long-term prognosis of CPA. Methods: We reviewed patients diagnosed with different subtypes of CPA from 2002 to 2020 at Nanjing Jinling Hospital, China. We analyzed the clinical and survival information of five different subgroups. A Cox regression model was used to explore proper antifungal duration and long-term survival factors of CCPA and SAIA. Results: A total of 147 patients with CPA were included, consisting of 11 SA, 48 CCPA, 5 CFPA, 12 AN, and 71 SAIA. The most common underlying pulmonary disease was pulmonary tuberculosis (n = 49, 33%), followed by bronchiectasis (n = 46, 31.3%) and chronic obstructive pulmonary disease (COPD) or emphysema (n = 45, 30.6%), while in SAIA and CFPA groups, the most common was COPD or emphysema (45.1 and 100%). Cough (85%), expectoration (70.7%), hemoptysis (54.4%), and fever (29.9%) were common symptoms, especially in CCPA, CFPA, and SAIA groups. The common imaging manifestations included cavitation (n = 94, 63.9%), fungal ball (n = 54, 36.7%), pleural thickening (n = 47, 32.0%), and bronchiectasis (n = 46, 31.3%). SAIA and CFPA groups had a lower value of hemoglobin (HB) and serum albumin (ALB) with higher C-reactive protein and erythrocyte sedimentation rate. The positive rate of sputum culture, serum galactomannan (GM), and bronchoalveolar lavage fluid GM was 32.7% (36/110), 18.4% (18/98), and 48.7% (19/39), respectively. There were 64.6% (31/48) patients with CCPA and 25.4% (18/71) patients with SAIA who received surgery and the 5-year cumulative survival rate was 92.1 and 66.6%, respectively. SAIA, old age, male, low body mass index (BMI), COPD or emphysema, multiple distribution, low serum ALB, and positive sputum culture were adverse prognosis factors for SAIA and CCPA group, and BMI ≤ 20.0 kg/m2 was independently associated with increased mortality (hazard ratio (HR) 5.311, 95% CI 1.405-20.068, p = 0.014). Multivariable Cox regression indicated that surgery (HR 0.093, 95% CI 0.011-0.814, p = 0.032) and antifungal duration >6 months (HR 0.204, 95% CI 0.060-0.696 p = 0.011) were related to improved survival. Conclusion: The clinical features and laboratory test performance are different among SA, CCPA, CFPA, AN, and SAIA. Low BMI was an independent risk factor for survival. Selective surgery and antifungal duration over 6 months were associated with improved survival.

Abstract:Abstract Background Chronic pulmonary aspergillosis (CPA) is an underrecognized but common complication of pulmonary tuberculosis (TB). In Nigeria, a TB-endemic country, there is currently no provision to monitor the development of CPA in patients treated for TB. This study determined the prevalence and incidence of CPA in Lagos, Nigeria. Methods A prospective longitudinal study of patients previously managed for TB was conducted between June 2021 and May 2022. The study cohorts were assessed at three monthly intervals with the following parameters collected: sociodemographic data, chest x-ray findings, sputum for fungal culture, and venous blood for Aspergillus IgG estimation. CPA cases were determined using the case definition for resource-constrained countries. Descriptive and inferential statistics were employed and significance was set at a probability of 5% (p<0.05). Results Of 141 patients recruited, 79 (56.0%) belonged to retreatment TB and 62 (44.0%) were post-TB treated patients. The median (interquartile range) age was 40 (30-52) years, with a male-to-female ratio of 1.1:1. Ninety-seven patients (69%) had a GeneXpert test done, of which, 63/97 (64.9%) were GeneXpert negative. Cough was the commonest symptom, with 15 (11%) having haemoptysis. The rate of CPA increased steadily as the study progressed: 44 (31.2%) at commencement, 45 (34.9%) at three months, 49 (42.6%) at six months, and 51 (54.3%) at nine months. Thus, the overall prevalence of CPA was 49.7% and the incidence was 6.1%. Conclusion CPA is common in Nigeria and its true burden may still be underestimated. Increased awareness of CPA as a post-TB lung disease is advocated. Evaluation for CPA should be incorporated in patients’ work-up for TB

Purpose: This study aimed to evaluate the diagnostic value of the Dynamiker quantitative Aspergillus fumigatus -specific IgG antibody in different types of CPA with the aim of providing a reference for clinical work. Methods: This prospective study collected the clinical data of patients with suspected CPA admitted to the hospital from January 2020 to December 2022 and divided them into two groups: CPA and non-CPA. The study analyzed clinical characteristics and Aspergillus -specific IgG antibody test's diagnostic value, and a receiver operating characteristic (ROC) curve was used to evaluate diagnostic efficacy. Results: We enrolled 54 CPA patients and 132 non-CPA patients. The average admission age of the CPA group was 61.0 (43.8, 70.0) years, and the sex ratio was 32/22 (male/female). The level of Aspergillus fumigatus -specific IgG antibody in the CPA group was significantly higher than the non-CPA group (95.2 (31.3, 213.3) vs 47.5 (34.0, 80.3) AU/mL, p = 0.001). The area under the ROC curve was 0.653 (95% confidence interval [CI]: 0.580– 0.721, p = 0.003). The cutoff with the best diagnostic efficacy was 87 AU/mL, and the sensitivity and specificity were 57.4% and 77.3%, respectively. There was no significant difference in the level of specific IgG antibody among the five CPA types ( p = 0.543); however, it was relatively higher in chronic cavitary pulmonary aspergillosis (CCPA). Conclusion: Aspergillus -specific IgG antibody is valuable diagnostic marker for CPA, while its value in differential diagnosis among different types of CPA is limited. To date, over 300 species of Aspergillus have been reported, and approximately 50 species are pathogenic to humans, most commonly Aspergillus fumigatus (A. fumigatus) . 1–3 Aspergillosis is defined as different kinds of diseases caused by Aspergillus , and pulmonary aspergillosis (PA) is the main clinical manifestation. 4 The PA spectrum includes invasive pulmonary aspergillosis (IPA), allergic bronchopulmonary aspergillosis (ABPA) and chronic pulmonary aspergillosis (CPA). CPA is comprised of a series of progressive lung parenchymal diseases, including simple pulmonary aspergilloma, aspergillus nodules, chronic cavitary pulmonary aspergillosis (CCPA), chronic fibrosing pulmonary aspergillosis (CFPA) and chronic necrotizing pulmonary aspergillosis (CNPA), also known as sub-acute invasive aspergillosis (SAIA). CPA usually exists in immunocompetent individuals but is accompanied by underlying lung diseases; the most common ones are patients with cavitary lesions in the lungs caused by tuberculosis or nontuberculous mycobacterial infections. 5,6 In recent years, the morbidity and mortality of PA have gradually increased, and approximately 3 million people worldwide suffer from CPA. 7 CPA has been regarded as an emerging fungal infectious disease with public health importance, therefore, early diagnosis and timely treatment are essential to reduce mortality and improve the prognosis of CPA. Abstract:Objective To get a better understand of pulmonary aspergillosis. Method A retrospective analysis of the clinical features of 18 pulmonary aspergillosis cases was performed.Result 83% of the patients had accompanying underlying diseases;The incidence of symptoms was 89%.Chest imaging findings:Lesions were mainly in the right lung,more common in the upper lobes,and mostly located in the peripheral;The cases of Lobar pneumonia-type accounted for 17% of all the patients,aspergilloma-type 50%,nodular or mass-type 33%,bronchopneumonia-type 6% and diffuse lesion-type 22%,with 22% of cases complicated with pleural effusion. Histopathological examination showed septate hyphae branching at an angle of approximately 45°.50% of patients underwent lobectomy and received antifungal medication after operation,while the other 50% received antifungal medication only; 89% of patients were fully recovered or improved,while 11% died.Conclusion Most of pulmonary aspergillosis patients have underlying diseases.Some types of pulmonary aspergillosis have typical imaging changes; Diagnosis requires a combination of past history,clinical manifestations,chest imaging findings and related laboratory examinations,and confirmed diagnosis is according to histopathological examination.To treat the diseases,most of patients need drug application,and some patients may undergo surgery.

Abstract:Background Chronic pulmonary aspergillosis (CPA) is known to complicate patients with post‐tubercular lung disease. However, some evidence suggests that CPA might co‐exist in patients with newly‐diagnosed pulmonary tuberculosis (P.TB) at diagnosis and also develop during therapy. The objective of this study was to confirm the presence of CPA in newly diagnosed P.TB at baseline and at the end‐of‐TB‐therapy. Materials and Methods This prospective longitudinal study included newly diagnosed P.TB patients, followed up at third month and end‐of‐TB‐therapy with symptom assessment, anti‐Aspergillus IgG antibody and imaging of chest for diagnosing CPA. Results We recruited 255 patients at baseline out of which 158 (62%) completed their follow‐up. Anti‐Aspergillus IgG was positive in 11.1% at baseline and 27.8% at end‐of‐TB‐therapy. Overall, proven CPA was diagnosed in 7% at baseline and 14.5% at the end‐of‐TB‐therapy. Around 6% patients had evidence of aspergilloma in CT chest at the end‐of‐TB‐therapy. Conclusions CPA can be present in newly diagnosed P.TB patients at diagnosis and also develop during anti‐tubercular treatment. Patients with persistent symptoms or developing new symptoms during treatment for P.TB should be evaluated for CPA. Whether patients with concomitant P.TB and CPA, while receiving antitubercular therapy, need additional antifungal therapy, needs to be evaluated in future studies.
