Abstract:Background: Arterial stiffness index (ASI) is a potential risk factor for cerebrovascular and cardiometabolic diseases, but the causal links between them are inconclusive. The aim is to evaluate the causal effects of ASI on cerebrovascular and cardiometabolic diseases by Mendelian randomization (MR). Methods: Two-sample MR analysis was performed to infer causal links. Genetic variants significantly associated with ASI were extracted. The inverse variance weighted method was used for estimating the effects. Sensitivity analysis was performed to test heterogeneity or pleiotropy. Results: MR analysis indicated an effect of genetically predicted ASI on the risk of ischemic stroke (IS) of all causes (OR = 1.894, 95% CI 1.210–2.965, p = 0.005). No links were identified between genetically predicted ASI and other cerebrovascular or cardiometabolic diseases (all p > 0.05). Subgroup analysis of IS etiologies found a suggestive association between genetically predicted ASI and large artery atherosclerosis stroke (LAS) (OR = 3.726, 95% CI 1.230–11.286, p = 0.020). There were no effects of ASI on IS due to cardioembolism or small vessel occlusion. Conclusion: The current MR analysis suggested that genetically predicted ASI was associated with higher risk of IS of all causes. The results and the underlying pathways or mechanisms between ASI and IS needs further investigation.
Genetically Determined Tobacco and Alcohol Use and Risk of Atrial Fibrillation

Abstract:BackgroundThe causality between the use of alcohol and cigarettes and atrial fibrillation (AF) remains controversial. We conducted a Mendelian randomization (MR) study to evaluate the association of genetic variants related to tobacco and alcohol use with AF.MethodsSingle nucleotide polymorphisms (SNPs) related to smoking initiation (N=374), age at initiation of regular smoking (N=10), cigarettes per day (N=55), and smoking cessation (N=24) were derived from a genome-wide association studies (GWAS) of tobacco use (N=1.2million individuals). SNPs related to heavy alcohol use (N=6) were derived from a GWAS of UK biobank (N=125,249 individuals). The genetically matching instrumented variables were obtained from the GWAS of AF (N=588,190 individuals). The estimates between tobacco and alcohol use and AF were combined by inverse-variance weighted (IVW), simple median, weighted median, MR-robust adjusted profile score method, MR-PRESSO, and multivariable MR.ResultsA total of 65,446 AF patients and 522,744 referents were included. In the IVW analysis, the odds ratio per one-unit increase of smoking initiation was 1.11 (95% CI, 1.06-1.16; P=3.35x10(-6)) for AF. Genetically predicted age at initiation of regular smoking, cigarettes per day and smoking cessation were not associated with AF. The IVW estimate showed that heavy alcohol consumption increased AF risk (OR, 1.11; 95% CI, 1.04-1.18; P=0.001). The results were consistent in complementary analyses and multivariable MR.ConclusionOur MR study indicated that regular smoking was associated with increased risk of AF, no matter the age at initiation of regular smoking, or the number of cigarettes smoked per day. Genetically predicted heavy alcohol consumption increased the risk of AF.

Abstract:Background: To understand a causal role of modifiable lifestyle factors in angiotensin-converting enzyme 2 (ACE2) expression (a putative severe acute respiratory syndrome coronavirus 2 [SARS-CoV-2] receptor) across 44 human tissues/organs, and in coronavirus disease 2019 (COVID-19) susceptibility and severity, we conducted a phenome-wide two-sample Mendelian randomization (MR) study. Methods: More than 500 genetic variants were used as instrumental variables to predict smoking and alcohol consumption. Inverse-variance weighted approach was adopted as the primary method to estimate a causal association, while MR-Egger regression, weighted median, and MR pleiotropy residual sum and outlier (MR-PRESSO) were performed to identify potential horizontal pleiotropy. Results: We found that genetically predicted smoking intensity significantly increased ACE2 expression in thyroid (beta=1.468, p=1.8x10(-8)), and increased ACE2 expression in adipose, brain, colon, and liver with nominal significance. Additionally, genetically predicted smoking initiation significantly increased the risk of COVID-19 onset (odds ratio=1.14, p=8.7x 10(-5)). No statistically significant result was observed for alcohol consumption. Conclusions: Our work demonstrates an important role of smoking, measured by both status and intensity, in the susceptibility to COVID-19.

Abstract:A 10‐year‐old girl and a 10‐month‐old girl, both with ambiguous genitalia, were found to have 45,X/46,X,mar and 45,X/46,X,r(?) mosaicism. The marker chromosomes in both girls were very small. Polymerase chain reaction, with synthetic oligonucleotide primers from Y‐specific DNA sequences pY‐80 and pY53.3 containing the sex‐determining region Y(SRY), proved the marker chromosomes to contain the Y short arm material. In situ hybridization with probe pY‐80 confirmed that the marker chromosomes included the Y short arms. These findings, together with ambiguous genitalia in the girls, indicate that the marker chromosomes include the testis‐determining factor gene.
The causal relationship between smoking and thoracic aortic aneurysm: Evidence from Mendelian randomization analysis

Abstract:The potential role of smoking as a risk factor for thoracic aortic aneurysm is still a subject of debate. Therefore, it is important to systematically investigate the causal relationship between smoking and thoracic aortic aneurysm using Mendelian randomization methods. Genetic data were obtained from genome-wide association studies using the inverse variance weighting method as the primary approach. A thorough sensitivity analysis was conducted to ensure the reliability of the findings. Instrumental variables were assessed using the F statistic, and meta-analysis was employed to assess the average genetic predictive effect between smoking and thoracic aortic aneurysm. Our Mendelian randomization study found a positive association between smoking and thoracic aortic aneurysm. The odds ratios (OR) in the inverse variance weighting method were OR = 1.23 (95% confidence interval [CI] = 1.00–1.51; P = .053) and OR = 2.07 (95% CI = 1.10–3.91; P = .024). Furthermore, meta-analyses consistently demonstrated a positive causal relationship between ferritin and myocardial infarction, although statistical significance was not observed. The analysis results did not indicate any horizontal pleiotropy. Despite the presence of heterogeneity, the Mendelian randomization analysis still yielded significant results. This study employed Mendelian randomization to establish a positive association between smoking levels and the risk of thoracic aortic aneurysm. The genetic evidence reveals a causal relationship between the two, offering new insights for future interventions targeting thoracic aortic aneurysms.

Abstract:We aimed to explore the causal association between type 2 diabetes (T2D) and increased arterial stiffness. We performed a Mendelian randomization (MR) analysis in 11,385 participants from a well-defined community study in Shanghai during 2011-2013. We genotyped 34 T2D-associated common variants identified in East Asians and created a genetic risk score (GRS). We assessed arterial stiffness noninvasively with the measurement of brachial-ankle pulse wave velocity (baPWV). We used the instrumental variable (IV) estimator to qualify the causal relationship between T2D and increased arterial stiffness. We found each 1-SD increase in T2D_GRS was associated with 6% higher risk in increased arterial stiffness (95% CI 1.01, 1.12), after adjustment of other metabolic confounders. Using T2D_GRS as the IV, we demonstrated a causal relationship between T2D and arterial stiffening (odds ratio 1.24, 95% CI 1.06, 1.47; P = 0.008). When categorizing the genetic loci according to their effect on insulin secretion or resistance, we found genetically determined decrease in insulin secretion was associated with increase in baPWV (βIV = 122.3 cm/s, 95% CI 41.9, 204.6; P = 0.0005). In conclusion, our results provide evidence supporting a causal association between T2D and increased arterial stiffness in a Chinese population.
Smoking and coronary artery disease risk in patients with diabetes: A Mendelian randomization study

Abstract:BackgroundPrevious observational studies have shown an association between smoking and coronary artery disease (CAD) in patients with diabetes. Whether this association reflects causality remains unestablished. This study aimed to explore the causal effect of smoking on CAD in patients with diabetes. MethodsGenetic signatures for smoking were extracted from a large genome-wide association study (GWAS), consisted of up to 1.2 million participants. Four smoking phenotypes were included: smoking initiation, cigarettes per day, age at initiation of regular smoking, and smoking cessation. Genetic associations with CAD in patients with diabetes were extracted from another GWAS, which included 15,666 participants (3,968 CAD cases and 11,696 controls). The analyses were performed using the univariable and multivariable Mendelian randomization (MR) method. ResultsMR analysis revealed that smoking initiation was positively related to CAD risk in patients with diabetes (OR = 1.322, 95% CI = 1.114 - 1.568, P = 0.001), but this association was attenuated when adjusted for cardiovascular risk factors (OR = 1.212, 95% CI = 1.008 - 1.457, P = 0.041). Age at initiation of regular smoking was negatively related to CAD in patients with diabetes (OR = 0.214, 95% CI = 0.070 - 0.656, P = 0.007), but this association became insignificant when adjusted for cardiovascular risk factors. ConclusionsThis study supported the effect of smoking initiation on the risk of CAD in patients with diabetes.
Smoking and heart failure: a Mendelian randomization and mediation analysis

Abstract:Aims We performed a Mendelian randomization (MR) study to elucidate the associations of ever smoking, lifelong smoking duration, and smoking cessation with heart failure (HF) risk. Methods and results We extracted genetic variants associated with smoking initiation, age at initiation of regular smoking, cigarettes per day, and smoking cessation from the genome-wide association study and Sequencing Consortium of Alcohol and Nicotine use (1.2 million individuals), as well as a composite lifetime smoking index from the UK Biobank (462 690 individuals). The associations between smoking phenotypes and HF were explored in the Heart Failure Molecular Epidemiology for Therapeutic Targets Consortium (47 309 cases; 930 014 controls) employing inverse variance-weighted meta-analysis and multivariable MR. The mediation effects of coronary artery disease and atrial fibrillation on smoking-HF risk were explored using mediation analysis. The odds ratios (ORs) for HF were 1.28 [95% confidence interval (CI), 1.22-1.36; P = 1.5 x 10(-18)] for ever regular smokers compared with never smokers and 1.25 (95% CI, 1.09-1.44; P = 1.6 x 10(-3)) for current smokers vs. former smokers. Genetic liability to smoking more cigarettes per day (OR, 1.37; 95% CI, 1.20-1.58; P = 6.4 x 10(-6)) and a higher composite lifetime smoking index (OR, 1.49; 95% CI, 1.31-1.70; P = 2.5 x 10(-9)) were associated with a higher risk of HF. The results were robust and consistent in all sensitivity analyses and multivariable MR after adjusting for HF risk factors, and their associations were independent of coronary artery disease and atrial fibrillation. Conclusions Genetic liability to ever smoking and a higher lifetime smoking burden are associated with a higher risk of HF.
Arterial Stiffness and Kidney Function: A Mendelian Randomization Study

Abstract:Background and Aims: Although observational studies have suggested that arterial stiffness is associated with kidney function decline, the causal nature remains unclear. We aimed to determine the potential causal relationship between arterial stiffness and kidney function using a two-sample Mendelian randomization study.Methods: We used four single nucleotide polymorphisms (SNPs) as instrumental variables for carotid-femoral pulse wave velocity (PWV). Multiple Mendelian randomization methods were performed in this study, including inverse-variance weighted method, weighted median-based method and MR-Egger method.Results: The results showed genetically instrumented higher arterial stiffness was associated with lower estimated glomerular filtration rate (eGFR) (beta, -0.007, 95% CI: -0.011 to -0.002, p = 0.002), whereas not associated with the risk of CKD (odds ratio, 1.027, 95% CI: 0.863 to 1.097, p = 0.821) or BUN (beta, -0.012, 95% CI: -0.023 to 0.001, p = 0.032).Conclusions: Our findings suggest a causal relationship between arterial stiffness and eGFR. However, we did not find evidence of a causal association between arterial stiffness and risk of CKD or blood urea nitrogen. Abstract:BackgroundLimited studies have explored the association between sexual factors [age at first sexual intercourse (AFS) and lifetime number of sexual partners (LNSP)] and cardiovascular diseases (CVDs), leaving the causality inconclusive.MethodsWe performed a bi-directional Mendelian randomization (MR) study to investigate the causality between sexual factors and CVDs, including coronary artery disease, myocardial infarction, atrial fibrillation (AF), heart failure (HF), and ischemic stroke (IS). Single-nucleotide polymorphisms (SNPs) for sexual factors were extracted from the UK Biobank. Statistics for each CVD were derived from two different databases. MR estimates were calculated per outcome database and were combined through meta-analysis. Several complementary sensitivity analyses were also performed.ResultsThe primary analysis suggested that AFS was causally associated with the risk of CVDs; the odds ratios (ORs) ranged from 0.686 [95% confidence interval (CI), 0.611-0.770] for HF to 0.798 (95% CI, 0.719-0.886) for AF. However, the association between AFS and IS (OR, 0.844; 95% CI, 0.632-1.126) was not consistent in the meta-analysis after excluding SNPs related to confounders. Moreover, non-significant associations were found between LNSP and CVDs. Reverse direction MR analysis showed that CVDs were not associated with sexual factors.ConclusionsGenetic evidence suggested that AFS was causally associated with the risk of CVDs except for IS, whereas non-significant association of LNSP with CVDs was detected. Further investigation into AFS could be warranted in preventing the progression of CVDs.

Abstract:Human rotavirus (HRV) serotypes were studied from diarrheal stool specimens in children in 7 regions of Japan (Sapporo, Tokyo, Maizuru, Osaka, Kagawa, Kurume, and Saga) from 1984 to 1997 by enzyme immunoassay (EIA) with serotype-specific monoclonal antibodies against serotypes 1, 2, 3, and 4. In addition, reverse transcription-polymerase chain reaction (RT-PCR) was conducted for analysis of "others" which included nonserotypable and mixed-serotype rotavirus specimens by EIA. In 3756 rotavirus-positive specimens, serotype 1 was detected in 2649 (70.5%), serotype 2 in 362 (9.6%), serotype 3 in 232 (6.2%) and serotype 4 in 196 (5.2%). Overall, serotype 1 was predominant from 1984 to 1997, although there were a few cases in which serotype 2, 3 and 4 became predominant based on area and year. The frequency of serotype 1 has gradually increased since 1993. Twenty two, 2, 3 and 1 among 57 specimens of "others" by EIA from Tokyo, Maizuru, Sapporo and Kurume in 1995-1996 and 1996-1997 were determined as serotypes 1, 2, 3, and 9 by RT-PCR, respectively.
Cigarette Smoking and Schizophrenia: Mendelian Randomisation Study.

Abstract:BackgroundThe link between schizophrenia and cigarette smoking has been well established through observational studies. However, the cause–effect relationship remains unclear.AimsWe conducted Mendelian randomisation analyses to assess any causal relationship between genetic variants related to four smoking-related traits and the risk of schizophrenia.MethodWe performed a two-sample Mendelian randomisation using summary statistics from genome-wide association studies (GWAS) of smoking-related traits and schizophrenia (7711 cases, 18 327 controls) in East Asian populations. Single nucleotide polymorphisms (SNPs) correlated with smoking behaviours (smoking initiation, smoking cessation, age at smoking initiation and quantity of smoking) were investigated in relation to schizophrenia using the inverse-variance weighted (IVW) method. Further sensitivity analyses, including Mendelian randomisation-Egger (MR-Egger), weighted median estimates and leave-one-out analysis, were used to test the consistency of the results.ResultsThe associated SNPs for the four smoking behaviours were not significantly associated with schizophrenia status. Pleiotropy did not inappropriately affect the results.ConclusionsCigarette smoking is a complex behaviour in people with schizophrenia. Understanding factors underlying the observed association remains important; however, our findings do not support a causal role of smoking in influencing risk of schizophrenia.
The causal relationship between smoking, alcohol consumption, and renal clear cell carcinoma: a Mendelian randomization study

Abstract:Introduction: Observational studies have found a correlation between the consumption of tobacco and alcohol and the likelihood of developing renal cell carcinoma. However, whether these associations indicate causal relationships is unclear. Methods: To establish if these connections indicate causal relationships, we performed a Mendelian Randomization (MR) analysis using a two-sample approach. For the number of daily cigarettes, lifetime smoking index, smoking initiation, and weekly drinking, we employed 44, 108, 174, and 76 single nucleotide polymorphisms (SNPs) as instrumental variables. Outcome data were obtained from the FinnGen Alliance, which included a combined total of 429,290 individuals. The MR analysis was conducted using the inverse-variance weighted (IVW) method to estimate causal effects. To address potential violations of MR assumptions due to directional pleiotropy, we performed MR-Egger regression and MR-PRESSO (Mendelian Randomization Pleiotropy RESidual Sum and Outlier) analysis. Results: Genetically influenced smoking initiation was directly associated with the risk of developing renal cell carcinoma (OR = 1.55, 95% CI: 1.04–2.33; p = 0.03). No causal relationship was found between daily cigarette consumption and lifetime smoking index with the risk of renal cell cancer. Genetic predisposition for weekly alcohol consumption showed a reduced risk of renal cell cancer (OR = 0.45, 95% CI: 0.26–0.81; p = 0.007). Discussion: Our study suggests a potential causal relationship between alcohol consumption and reduced risk of renal cell cancer, while no such association was observed with smoking. Further research is needed to confirm these findings.

Abstract:Background: The Mendelian randomization approach uses genetic variants as instrumental variables to study the causal association between the risk factors and health outcomes of interest. We aimed to examine the relation between alcohol consumption and cardiovascular risk factors using two genetic variants as instrumental variables: alcohol dehydrogenase 1B ( ADH1B ) rs1229984 and aldehyde dehydrogenase 2 ( ALDH2 ) rs671. Methods: Using data collected in the Taiwan Biobank—an ongoing, prospective, population-based cohort study—our analysis included 129,032 individuals (46,547 men and 82,485 women) with complete data on ADH1B rs1229984 and ALDH2 rs671 genotypes and alcohol drinking status. We conducted instrumental variables regression analysis to examine the relationship between alcohol drinking and body mass index (BMI), systolic blood pressure (SBP), diastolic blood pressure (DBP), fasting glucose, glycated hemoglobin (HbA1c), triglycerides, high-density lipoprotein cholesterol (HDLc), and low-density lipoprotein cholesterol (LDLc). Results: In the rs1229984-instrumented analysis, alcohol drinking was only associated with higher levels of SBP in men and lower levels of DBP in women. In the rs671-instrumented analysis, alcohol drinking was associated with higher levels of BMI, SBP, DBP, fasting glucose, triglycerides, HDLc and lower levels of LDLc in men; alcohol drinking was associated with higher levels of HDLc and lower levels of SBP, HbA1c, and triglycerides in women. Conclusion: Using Mendelian randomization analysis, some of our study results among men echoed findings from the previous systematic review, suggesting that alcohol drinking may be causally associated with higher levels of BMI, SBP, DBP, fasting glucose, triglycerides, HDLc, and lower levels of LDLc. Although alcohol drinking is beneficial to a few cardiovascular risk factors, it is detrimental to many others. The assumptions that underlie the Mendelian randomization approach should also be carefully examined when interpreting findings from such studies.

Abstract:BACKGROUND:Evidence suggests that nonalcoholic fatty liver disease (NAFLD) is associated with cardiovascular diseases (CVDs). However, the results are inconsistent, and the causality remains to be established.OBJECTIVE:We aimed to investigate the potential causal relationship between NAFLD and CVDs, including arterial stiffness, coronary artery disease, heart failure, stroke, ischemic stroke and its subtypes using two-sample Mendelian randomization (MR).METHODS:Genetic instruments were used as proxies for NAFLD. Publicly available summary-level data were obtained from the UK Biobank, the CARDIoGRAMplusC4D Consortium, the MEGASTROKE Consortium, and other consortia. Six complementary MR methods were performed, including inverse variance weighted method (IVW), MR-Egger, weighted median, weighted mode, MR-PRESSO, and MR-RAPS.RESULTS:NAFLD was significantly associated with arterial stiffness (β = 0.04 [95%CI, 0.02-0.06], P = 5.53E-04). Moreover, the results remained consistent and robust in the sensitivity analysis. As for heart failure, the IVW method suggested that NAFLD was significantly associated with heart failure (OR = 1.08, 95%CI: 1.02-1.14, P = 0.005) in the absence of pleiotropy. However, there were no significant associations of NAFLD with coronary artery disease, stroke, ischemic stroke, or any ischemic stroke subtype.CONCLUSION:The MR study supported the causal effect of NAFLD on arterial stiffness. However, the study did not provide enough evidence suggesting the causal associations of NAFLD with heart failure, coronary artery disease, and any stroke subtypes.

Abstract:BackgroundThe nature and underlying mechanisms of the association between type 2 diabetes and the risk of increased arterial stiffness are unclear. We aimed to study this association using the Mendelian randomisation approach.MethodsWe did a Mendelian randomisation analysis in 11 385 participants, aged 40 years or more, recruited from two communities in Baoshan district, Shanghai, China, in 2011–13. We genotyped 34 common variants associated with type 2 diabetes that had previously been identified and validated in east Asians, and created the type 2 diabetes genetic risk score. Arterial stiffness was assessed non-invasively with measurement of brachial-ankle pulse wave velocity (baPWV). The effect of type 2 diabetes on the increased risk of arterial stiffness was quantified by instrumental variable estimator. The institutional review board of Rui-Jin Hospital, Shanghai Jiao-Tong University School of Medicine, approved the study protocol. All participants gave written informed consent.FindingsWe found that each ten-point increase in the genetic risk score was associated with a 1·39 times increment in type 2 diabetes risk and an odds ratio (OR) for increased arterial stiffness of 1·21 (95% CI 1·06–1·38). Compared with the lowest quartile of the genetic risk score, the highest quartile was associated with a 1·22 (95% CI 1·06–1·40) times increment risk of increased arterial stiffness. Linear regression adjusted for age, sex, and body-mass index showed that every ten-point increase of the genetic risk score was associated with a rise in baPWV by 36·71 cm/s (p<0·0001). Mendelian randomisation analysis showed a causal relation between type 2 diabetes and risk of increased arterial stiffness (OR 1·23 [95% CI 1·04–1·46], p=0·02), which was smaller than the association seen between type 2 diabetes and arterial stiffness risk (2·09 [1·88–2·32], p<0·0001).InterpretationType 2 diabetes has a causal effect on the presence of arterial stiffness, measured by increased baPWV in a Mendelian randomisation analysis with a large sample size, and has an important relation with increased arterial stiffness. ASSESSING CAUSALITY BETWEEN CAROTID-FEMORAL PULSE WAVE VELOCITY AND COGNITIVE FUNCTION: A MENDELIAN RANDOMIZATION STUDY

Abstract:Arterial stiffness is usually significantly associated with cognitive function. We conducted a Mendelian randomization study to investigate the causal relationship between measures of arterial stiffness and cognitive function in a cross-sectional survey. A total of 1516 adults aged more 30 years old in the CardioVascular Disease risk FACTors in two townships Study (CVDFACTS) with wave 6 and wave 7 during 2013-2020 were evaluated for artery stiffness with carotid-femoral pulse wave velocity (CFPWV) and cognitive function with Mine-Mental Short Evaluation (MMSE). A genome-wide association study was used to identify the genetic variants associated with CFPWV. Mendelian Randomization analysis was conducted using the R software. A total of 87 SNP were identified (p-value<10-6) in the KDM4B gene (Chromosome-19) and 3 SNPs [rs10085112, rs2613776 and rs147498368] of them were independently associated with CFPWV. We constructed a polygenetic risk score (PRS, 1-6) with the 3 significant SNPs. PRS was the genetic variant and instrumental variable in the Mendelian Randomization. PRS was significantly associated with CFPWV (beta = -0.37, p = 0.0003), the “exposure”, and MMSE (beta = 0.14, p = 0.031), the “outcome”. In contrast, PRS was not associated with age, sex, systolic/diastolic blood pressure, body mass index, fasting glucose, total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, triglycerides, C-reactive protein, creatinine and uric acid, the “confounders”. Mendelian Randomization estimate (95% confidence intervals) of the causal effect of the exposure (CFPWV) on the outcome (MMSE) was -0.433 (-0.810, -0.056, p-value = 0.0244). Our study provides evidence that KDM4B gene may be instrumental in the causal relationship between artery stiffness measured by CFPWV and cognitive function measured by MMSE in a Han Chinese population.

Causal relationship between smoking and spinal stenosis: Two-sample Mendelian randomization

Abstract:Objective: Currently, the number of patients with spinal stenosis is increasing, and most of the patients are found to have a history of smoking in the clinic. In this study, we used the Mendelian randomization (MR) method to investigate the causal relationship between smoking and spinal stenosis. Methods: Genetic loci independently associated with smoking and spinal stenosis in people of European ancestry were selected as instrumental variables using pooled data from large-scale genome-wide association studies (GWAS). Three MR analyses, MR-Egger, Weighted median and inverse variance weighting (IVW), were used to investigate the causal relationship between smoking and spinal stenosis. The results were tested for robustness by heterogeneity and multiplicity tests, and sensitivity analyses were performed using the "leave-one-out" method. Results: The IVW results showed an OR (95% CI) of 2.40 (0.31-18.71), P = .403, indicating that there was no causal relationship between smoking and spinal stenosis. And no heterogeneity and multiplicity were found by the test and sensitivity analysis also showed robust results. Conclusion: In this study, genetic data were analyzed and explored using 2-sample MR analysis, and the results showed that there is a causal relationship between smoking and the occurrence of spinal stenosis, and more studies need to be included.

Abstract:Background: Previous studies suggested that smoking behavior (e.g., smoking status) was associated with an elevated risk of chronic kidney disease (CKD), yet whether this association is causal remains uncertain. Methods: We used data for half million participants aged 40 to 69 years from the UK Biobank cohort. In the traditional observational study, we used Cox proportional hazards models to calculate the associations between 2 smoking indices—smoking status and lifetime smoking index and incident CKD stages 3 to 5. Mendelian randomization (MR) approaches were used to estimate a potential causal effect. In one-sample MR, genetic variants associated with lifetime smoking index were used as instrument variables to examine the causal associations with CKD stages 3 to 5, among 344,255 UK Biobank participants with white British ancestry. We further validated our findings by a two-sample MR analysis using information from the Chronic Kidney Disease Genetics Consortium genome-wide association study. Results: In the traditional observational study, both smoking status [hazard ratio (HR): 1.26, 95% confidence interval (CI): 1.22 to 1.30] and lifetime smoking index (HR: 1.22, 95% CI: 1.20 to 1.24) were positively associated with a higher risk of incident CKD. However, both our one-sample and two-sample MR analyses showed no causal association between lifetime smoking index and CKD (all P > 0.05). The genetic instruments were validated by several statistical tests, and all sensitivity analyses showed similar results with the main model. Conclusion: Evidence from our analyses does not suggest a causal effect of smoking behavior on CKD risk. The positive association presented in the traditional observational study is possibly a result of confounding.
