Abstract:Objectives: Last October, the nuclear medicine departments were informed of the closure of the chromium-51 production line for clinical use. This radionuclide has different diagnostic indications in nephrology and hematology. It was therefore essential to set up alternative exploration protocols to overcome this production stoppage. Methods: Chromium-51 EDTA has been replaced by technetium-99m DTPA for the determination of glomerular filtration rates. Sodium chromate was substituted by sodium pertechnetate for the determination of globular volumes. A retrospective analysis of the chromium-51 data was performed followed by a prospective study, from January to December 2019 for technetium tracers. Results: One hundred and forty-four patients were included in the study. Forty-two EDTA-51Cr and 30 DTPA-99mTc exams were conducted and compared. There were no significant differences between the methods used to assess renal function (P=0.355). For the determination of blood cell and plasma volumes, 47 tests with 51Cr and 125I and 25 tests with 99mTc and 125I were performed and compared. There were no significant differences in the determination of total (P=0.325) and globular (P=0.148) volumes. Conclusions: The study carried out shows that there is no significant difference between the results obtained with chromium-51 and technetium tracers. As a result, clinical activity was maintained in good conditions.

Abstract:Background: The chromium-51-labeled posttransfusion recovery (PTR) study has been the gold-standard test for assessing red blood cell (RBC) quality. Despite guiding RBC storage development for decades, it has several potential sources for error. Methods: Four healthy adult volunteers each donated an autologous, leukoreduced RBC unit, aliquots were radiolabeled with technetium-99m after 1 and 6 weeks of storage, and then infused. Subjects were imaged by single-photon-emission computed tomography immediately and 4 hours after infusion. Additionally, from subjects described in a previously published study, adenosine triphosphate levels in transfusates infused into 52 healthy volunteers randomized to a single autologous, leukoreduced, RBC transfusion after 1, 2, 3, 4, 5, or 6 weeks of storage were correlated with PTR and laboratory parameters of hemolysis. Results: Evidence from one subject imaged after infusion of technetium-99m-labeled RBCs suggests that, in some individuals, RBCs may be temporarily sequestered in the liver and spleen immediately following transfusion and then subsequently released back into circulation; this could be one source of error leading to PTR results that may not accurately predict the true quantity of RBCs cleared by intra- and/or extravascular hemolysis. Indeed, adenosine triphosphate levels in the transfusates correlated more robustly with measures of extravascular hemolysis in vivo (e.g., serum iron, indirect bilirubin, non-transferrin-bound iron) than with PTR results or measures of intravascular hemolysis (e.g., plasma free hemoglobin). Conclusions: Sources of measurement error are inherent in the chromium-51 PTR method. Transfusion of an entire unlabeled RBC unit, followed by quantifying extravascular hemolysis markers, may more accurately measure true posttransfusion RBC recovery.
Investigation of NK cell function against two target hematological cell line using radioactive chromium assay

Abstract:NK (Natural killer) cells are a special group of peripheral blood lymphocytes that kill virus-infected cells as well as tumor cells. For testing NK cell function, the classic gold standard assay has been used for a long time, determining the activity from target tumor cells using radioactive chromium in cell cultures for 4h. In this study two hematological cell lines K562 and MDS where used and target and results showed different sensitivity to killing by NK cells separated from healthy volunteers. Results have been shown that MDS release significantly more radioactive chromium indicating higher degree of necrosis during cell culture. In addition, K562 cell line is better target for NK killing in all different E:T ratio in comparison to MDS cell line previously described. Based on this, it is suggested that K562 cells be continues used in the future as better target for investigation NK killing.

Abstract:In the1980s, radiolabeled cells helped understand the pathology of hemato-oncology. In the 1990s, preclinical trials evaluated radiolabeled immunotherapy with monoclonal antibodies (MoAbs) such as anti-CD20 agents labeled with Iodine-131 (Bexxar) or Yttrium-90 (Zevalin). Due to the safe and durable responses of radiolabeled MoAbs, the Food and Drug Administration approved these agents in the 2000s. Despite radioimmunotherapy's long journey, its application has recently decreased. This review will discuss the historical timeline of radioimmunotherapy, debate on advantages and difficulties, and explore trials. We will examine future directions of radioligand therapy in hemato-oncology, considering emerging molecules that may become the next theragnostic trend.
Red blood cells metabolome changes upon treatment with different X-ray irradiation doses

Abstract:The upholding of red blood cells (RBC) quality and the removal of leukocytes are two essential issues in transfusion therapy. Leukodepletion provides optimum results, nonetheless there are cases where irradiation is recommended for some groups of hematological patients such as the ones with chronic graft-vs-host disease, congenital cellular immunodeficiency, and hematopoietic stem cell transplant recipients. The European guidelines suggest irradiation doses from 25 to 50 Gray (Gγ). We evaluated the effect of different prescribed doses (15 to 50 Gγ) of X-ray irradiation on fresh leukodepleted RBCs bags using a novel protocol that provides a controlled irradiation. Biochemical assays integrated with RBCs metabolome profile, assessed by nuclear magnetic resonance spectroscopy, were performed on RBC units supernatant, during 14 days storage. Metabolome analysis evidenced a direct correlation between concentration increase of three metabolites, glycine, glutamine and creatine, and irradiation dose. Higher doses (35 and 50 Gγ) effect on RBC mean corpuscular volume, hemolysis, and ammonia concentration are considerable after 7 and 14 days of storage. Our data show that irradiation with 50 Gγ should be avoided and we suggest that 35 Gγ should be the upper limit. Moreover, we suggest for leukodepleted RBCs units the irradiation with the prescribed dose of 15 Gγ, value at center of bag, and ranging between 13.35-15 Gγ, measured over the entire bag volume, may guarantee the same benefits of a 25 Gγ dose assuring, in addition, a better quality of RBCs.
The isolated Xenopus laevis tail: a preparation for studying the central nervous system and metamorphosis in culture.

Abstract:The Xenopus tail preparation described here was initially developed for studying the action of substances on the vertebrate nervous system. For this both physiological preparations and whole animals have their disadvantages. The former are short-lived; the latter may die before any major chemical or structural changes have had time to occur. It was therefore thought desirable to seek a relatively long-lived preparation that possessed a functional central nervous system, but yet was not dependent on the conduction of nerve impulses for its survival. The tail of a Xenopus laevis tadpole seemed to be highly suitable for this purpose, as it contained complete reflex arcs and was sufficiently small to carry out its necessary exchanges by diffusion. Moreover, it was so transparent that nerves and muscle striations could be studied microscopically during life. Although agents to which the skin was impermeable could easily be injected into the tail, it was thought best to seek media that would be suitable for partially flayed tails.

Abstract:Although the diagnosis of anemia is a relatively simple and straightforward endeavor, since it is mostly based on the measurement of hemoglobin in whole blood, the accurate characterization of red blood cell (RBC) disorders has emerged as a mainstay in the era of precision (laboratory) medicine. The many technological advances occurred in laboratory medicine over recent times have enabled the introduction of a vast array of innovations also for RBC diagnostics, mostly represented by modular automation, digital hematology, innovative erythrocyte parameters and adaptation of disruptive technologies to laboratory hematology, especially mass spectrometry and molecular biology. These important breakthroughs have enormously contributed to broadening the diagnostic armamentarium and making more efficient and sustainable the diagnostics of RBC disorders. Some on these important innovations will be discussed in this article.
Need to reconsider national quality standards for red cell components: Evidence from a retrospective observational analysis

Abstract:Background & objectives Red cell concentrates (RCCs) must comply with applicable quality control (QC) standards to achieve the desired therapeutic effect in the recipient. In this study, we assessed the effect of change in the component preparation process on the quality of RCCs and their compliance with different QC standards. Methods A retrospective analysis of data for QC testing of RCCs over a period of 10 years, (from 2009 to 2019), was undertaken. QC testing parameters [volume, haematocrit (Hct), haemoglobin (Hb) content, white blood cell (WBC) content and percentage (%) haemolysis] were used to assess compliance with three national and three international QC standards. Linear regression analysis was done to assess the influence of donor variables. Results Data from 5,218 RCC units was included in the analysis. A majority (>50%) of RCCs prepared did not meet the three national QC standards either for volume or for Hct. The criteria for volume, Hct and Hb content, as defined in different international standards, were fulfilled by a majority (>75%) of RCCs evaluated. RCCs prepared by the buffy coat method had overall better compliance with QC standards compared to the platelet-rich plasma (PRP) method. The method of component preparation was found to influence Hb content, WBC content and percentage haemolysis. Male gender was associated with better Hb content. Interpretation & conclusions RCC prepared at our centre was found to have better compliance with international QC criteria compared to national standards. There is a need to reconsider the current national QC criteria for red cells with due consideration to the volume of whole blood collected and the method used for RCC preparation.

A Radioactive Method for Assessing Microbial Safety of an Infusion Set for Contrast-Enhanced Imaging

Abstract:1185 Objectives Multiple uses of automatic contrast injection systems can save costs. However, cross-contaminations with blood-borne pathogens of infectious diseases may occur. We developed a radioactive method for quantitative safety evaluation on a new replaceable patient-delivery system. By mimicking pathogens with a radiotracer, we evaluated the feasibility of using the system repeatedly without septic risks. Methods Twelve patient-delivery systems were tested using an automatic contrast dual-injector and one filling-injecting set. Protocols with normal saline (n=6) and contrast agent plus saline (n=6) loaded in the injection system were compared. Each patient-delivery system was connected to the ear vein of a rabbit that was intravenously pre-injected with a radiotracer 99mTc-dimercaptopropionyl-human serum albumin (185MBq/rabbit). Aliquots from filling-injecting set, patient line and blood were counted after replacing each patient-delivery system. Results With saline, radioactivity was found in blood (1655903±593221 CPM) and in patient line (52894±33080 CPM) but in none of samples from the filling-injecting set (8±3 CPM), relative to the background (7±3 CPM) (p=0.726). Contrast plus saline showed radioactivity in blood (1119107±183174 CPM) and in patient line (32991±20232 CPM) but in none of samples from the filling-injecting set (6±6 CPM), relative to the background (6±4 CPM) (p=0.955). Conclusions This radioactive method appears accurate and reliable. The patient-delivery system proves safe and convenient. By replacing the patient delivery system, cross-contamination risks can be avoided without changing the main part of injection system. This method can be applied for evaluation of similar devices before human use. Animal experiments with radioactive techniques allow sensitive real-time analyses.
Cherry-red plasma: Beyond the assumption of hemolysis

Abstract:Hemolysis is the most prevalent pre-analytical interfering factor and a major source of error in laboratory analysis. The examination of samples post-centrifugation can provide valuable information regarding pre-analytical interferences. In this unusual case, a patient's plasma specimen was cherry-red after centrifugation, which is most usually indicative of hemolysis. However, subsequent investigations ruled out common hemolysis causes. We eventually determined that the patient's cherry-red plasma was most likely caused by other factors in the patient's medical history, including cancer treatment with PV-10 (rose bengal disodium 10%). We then conducted an interference study to comprehensively assess the effects of PV-10 on various biochemical tests, especially liver function tests and bilirubin levels. The findings indicate that PV-10 has varying effects on different biochemical assays and test results should be examined individually. This report underlines the need for awareness of potential drug interference on laboratory tests for better result interpretation and making clinical decisions.

Abstract:SUMMARY: The measurement of red blood cell (RBC) survival has a long history, and a wide variety of methods have been utilized for this purpose. Current methods are of 2 types. First, those that label a representative sample of RBCs of all ages from the blood and then measure their rate of disappearance upon reinfusion. This category includes the (51)Cr and biotin labels. Second, those that use a metabolic precursor or product to determine the turnover of hemoglobin. Examples of these are carbon monoxide production and incorporation of labeled glycine. Recent studies with the covalent, nonradioactive biotin label show its unique suitability for both the accurate measurement of red cell survival and the determination of changes in red cell properties as they age in vivo.

Abstract:Abstract— Hand-held digital mobile phones generate pulsed magnetic fields associated with the battery current. The peak value and the waveform of the battery current were measured for seven different models of digital mobile phones, and the results were applied to compute approximately the magnetic flux density and induced currents in the phone-user’s head. A simple circular loop model was used for the magnetic field source and a homogeneous sphere consisting of average brain tissue equivalent material simulated the head. The broadband magnetic flux density and the maximal induced current density were compared with the guidelines of ICNIRP using two various approaches. In the first approach the relative exposure was determined separately at each frequency and the exposure ratios were summed to obtain the total exposure (multiple-frequency rule). In the second approach the waveform was weighted in the time domain with a simple low-pass RC filter and the peak value was divided by a peak limit, both derived from the guidelines (weighted peak approach). With the maximum transmitting power (2 W) the measured peak current varied from 1 to 2.7 A. The ICNIRP exposure ratio based on the current density varied from 0.04 to 0.14 for the weighted peak approach and from 0.08 to 0.27 for the multiple-frequency rule. The latter values are considerably greater than the corresponding exposure ratios 0.005 (min) to 0.013 (max) obtained by applying the evaluation based on frequency components presented by the new IEEE standard. Hence, the exposure does not seem to exceed the guidelines. The computed peak magnetic flux density exceeded substantially the derived peak reference level of ICNIRP, but it should be noted that in a near-field exposure the external field strengths are not valid indicators of exposure. Currently, no biological data exist to give a reason for concern about the health effects of magnetic field pulses from mobile phones.

Abstract:In 85 patients between 15 and 95 years of age (40 male patients and 45 females) without any signs of renal disease (concentration of serum creatinine below 1,4 mg/100ml, normal diastolic blood pressure and normal urine analysis) a highly significant negative correlation was found between glomerular filtration rate (GFR, determined by 51Cr-EDTA - slope-clearance) and renal plasma flow (RPF, determined by 131J-oJHS-iodinehippuricacid-totalclearance) on one side and age on the other. Beginning with a basic value of 160 ml in males and 144 ml in females for GFR, the average decrease per year was 1,2 ml/1,0 ml respectively. The basis value of RPF was 854 ml for males and 673 ml for females, the decrease per year was 5,4 ml in men and 2,9 ml in women. The importance of physiological diminution of clearance values in elderly patients and its consequences for therapy are discussed. The more exact and methodically simpler 51Cr-EDTA-slope-clearance without urine collection should be used instead of the somewhat inaccurate determination of endogenous creatinine clearance especially in older patients.

Abstract:Automated techniques for red cell [red blood cell (RBC)] exchange or depletion of malignant cells from the peripheral blood have allowed patients with life-threatening conditions to survive long enough to receive definitive treatment. Examples of such conditions include acute chest syndrome in sickle cell disease (SCD) or acute respiratory insufficiency due to leukostasis in acute leukemia. Conversely, other patients with SCD undergo RBC exchanges on a chronic basis to maintain a reasonable quality of life and prevent another stroke. In this review, we will discuss the techniques as well as indications for RBC exchange, leukocytapheresis, and thrombocytapheresis. To illustrate the uses of these therapeutic apheresis procedures, the authors included a summary of the most common diagnoses that comprise their use.

Radiochromium distribution in aged rats.

Abstract:Fragmentary studies suggest that tissue chromium (Cr) levels decrease with age. Regardless of the mechanism for such a decline, decreased tissue exchange with administered radiochromium (51Cr) should result. Accordingly, body retention, urinary excretion, and serum (plasma) and tissue levels of 51Cr were determined in 2-month-old male control rats and in 9-18-month-old experimental male rats 3 days after the intravenous injection of high specific activity trivalent 51Cr. The older rats retained relatively less 51Cr than the 2-month-old rats in comparison to body weight but had similar urinary excretions of 51Cr. Serum (plasma) 51Cr levels were generally higher and tissue 51Cr levels generally lower in the older rats, with the exception of spleen 51Cr content, which increased. Skeletal 51Cr content was markedly decreased, by 30-85%. These data suggest that aging alters 51Cr distribution by decreasing cellular Cr content and transport. Other mechanisms must be operative in bone since skeletal Cr is primarily extracellular.

Validation of low-volume blood collection tubes for routine hematologic testing

Abstract:Background: Low-volume blood tubes offer several advantages in facilitating blood collection, reducing iatrogenic anemia and spurious hemolysis, but their clinical reliability must be validated. We planned this investigation for establishing the reliability of routine hematologic testing in low-volume tubes before their implementation into clinical practice. Methods: Blood was drawn from 44 ostensibly healthy laboratory professionals into three blood tubes, as follows: 3.0 mL of blood into a 3.0 mL K2EDTA standard blood tube, 0.5 mL of blood into a second 3.0 mL K2EDTA standard blood tube, and 0.5 mL of blood into a 0.25-0.5 mL K2EDTA low-volume blood tube. Hematologic testing was performed on Sysmex XN-10 hematology analyzer. Results: Statistically significant differences were observed in total white blood cell count, neutrophil count, lymphocyte count, red blood cell count, platelet count, hemoglobin, hematocrit, mean corpuscular volume (MCV), mean corpuscular hemoglobin concentration (MCHC), and mean platelet volume in both 0.5 mL-filled 3.0 mL standard blood tubes as well as in the 0.5 mL-filled low-volume blood tubes. Although none of these variations was found to be clinically significant in the 0.5 mL-filled low-volume blood tube compared to the desirable specifications, hematocrit, MCV and MCHC displayed a clinically significant bias in the 0.5 mL- filled 3.0 mL K2EDTA standard blood tube. Conclusions: These results suggests that K2EDTA low-volume blood tubes could safely replace standard blood tubes for preventing the receipt of insufficient samples, but also for facilitating blood collection in patients with difficult veins and reducing the risk of iatrogenic anemia and spurious hemolysis.

Effect of irradiation on red cell cation content and transport

Abstract:The effect of irradiation on red cell (RBC) cation content and membrane transport was studied. When units of RBCs collected in AS-1 (n = 8) were exposed to radiation (2000 cGy) and then stored at 4 degrees C, there was a significant increase in external K and internal Na and a decrease in internal K relative to control units (n = 3). Similar changes in internal cation contents were noted for RBCs collected in CPD; because of the higher hematocrit, the external K concentration was higher than that in AS-1-collected RBCs. Cellular K loss and Na gain were balanced, and the total cation content remained constant; however, both control and irradiated cells swelled over time as a consequence of the decreased pH and 2.3 DPG content. These changes in volume and cation content were reversed when the cells were incubated at 37 degrees C. A specific effect of radiation on the Na-K pump and Na-K-Cl and K-Cl cotransport systems was ruled out, because no significant differences between irradiated cells and controls were found. These data indicate that radiation induces an increased permeability to Na and K on the part of the RBC membrane lipid bilayer and that this increase is balanced and reversible.

National Survey of Neonatal Transfusion Practices: I. Red Blood Cell Therapy

Abstract:Neonatal blood component transfusion practices during 1989 were surveyed via a questionnaire developed by the Pediatric Hemotherapy Committee of the American Association of Blood Banks. Of 1790 questionnaires mailed, 452 were selected to form the database for this analysis because they were from institutions in which neonates were transfused. Nearly all institutions contained intensive care units directed by neonatologists and were involved in the management of high-risk infants. Results from institutions serving as the primary pediatric teaching hospital of a medical school were compared with those with no medical school affiliation. Thirty-six percent of primary pediatric teaching hospitals and 52% of hospitals with no medical school affiliation performed pretransfusion testing in excess of that required, resulting in additional blood loss in neonates. Sixty-six percent of primary pediatric teaching hospitals used fresh frozen plasma to adjust the hematocrit of red blood cell concentrates prior to transfusion (a practice increasing donor exposure), compared with only 29% of hospitals with no medical school affiliation. The usual indication for small-volume red blood cell transfusions in severely ill neonates was to maintain a desired hematocrit level, whereas for stable infants, red blood cell transfusions were given to treat symptomatic anemia, rather than to maintain a predetermined hematocrit.
