Abstract:With the support of a geriatrics curriculum grant from the Association of American Medical Colleges (AAMC) and the John A. Hartford Foundation, we have successfully designed and implemented the Curriculum on Care in Aging for the Duke University School of Medicine. The curriculum effectively integrates content and experiences in geriatrics medicine across all four years of medical school. We attempt to cover a spectrum of themes in geriatrics, including the science of aging, assessment and management of geriatric syndromes, and orientation to the variety of settings and providers involved in the care of older adults. Accordingly, our teaching strategies range from formal symposia to oneon-one precepting in clinics and long-term care units. Although we have met logistical challenges along the way, we have also received constant and enthusiastic support from administrators, students, and instructors. The following summary provides an introductory overview of the curriculum. We worked with course directors from a number of different departments to create a series of symposia that punctuate the preclinical curriculum with clinical correlates and introductions to research on aging. The Biology of Aging symposium introduces students to the basic theories of aging and provides examples of ongoing research on that topic. Hypertension in the Elderly, a symposium on the pathophysiology of the aging heart, illustrates several age-related changes in cardiac physiology through a case-based review. In the pharmacology and pathology courses as well, we introduced aging-related issues by developing content and cases with course instructors. From the time of their matriculation at Duke, students participate in the practice course, which enhances skills in communication and medical decision making. We provided a brief presentation as a guide to communicating with older adults and assisted in the development of standardized patients with a variety of aging-related problems, including depression, heart disease, social isolation, cognitive impairment, and multiple medications. As part of this course, we also deliver the Care in Aging symposium. This day-long program consists of a morning symposium followed by a practical-experience session in the afternoon. A multidisciplinary group of geriatrics faculty discusses the basic assessment of conditions commonly encountered in older adults, including cognitive impairment, gait instability, functional impairment, and polypharmacy. After lunch, students meet with senior volunteers from the local Durham community to practice these assessments. Following this meeting, students return to campus to meet with faculty in small-group sessions to describe their experiences and provide feedback. In the course of clinical rotations in internal medicine and family practice, students on clinical clerkships participate in learning modules delivered by members of the Division of Geriatrics Medicine. Topics for these sessions include appropriate prescribing, long-term care, advance directives, delirium, prevention of fractures and falls, and screening and health promotion in the elderly. Most of these experiences have brief didactic presentations, followed by small-group case reviews. In addition, students receive an assignment to return to the ward, clinic, or nursing home and perform a follow-up assessment on a patient for whom they are currently providing care. During the fourth year, students can select a month-long experience on geriatric medicine. The rotation offers practical clinical training in long-term and ambulatory-care settings, working with a spectrum of providers that includes geriatricians, nurse practitioners, occupational and physical therapists, and wound and pain specialists. The students read an assigned list of important articles and write a short paper at the end of the month-long rotation. Outside the classroom, students have led the formation of a geriatrics interest group, which in turn has spawned the development of the Senior–Student Partnership Program. In this program, volunteer students meet with seniors to complete a “health-care journal,” which assists older adults in organizing their personal health information. Duke University School of Medicine administrators are very interested in expanding this program as a required activity for all students. At this point, we are very pleased with the impact we have had during the last two years. We believe three key elements account for the curriculum’s success: (1) the enthusiastic support of the medical school faculty and administration, (2) the introduction of a few core concepts that consistently recur throughout the curriculum, and (3) a Web site allowing for easy storage and retrieval of content by both faculty and students. In the following pages, we summarize the process and procedures behind each of these elements and describe their impact on the curriculum’s development, implementation, and evaluation.
