Abstract:Objectives: The aim of this study was to compare the diagnostic accuracy of an intraoral photostimulable storage phosphor (PSP) plate system and cone-beam computerized tomography scanning (CBCT) for detection of experimentally induced transverse root fractures and to evaluate differences between original images and images enhanced with high-pass filters. Study design: Sixty-nine extracted human teeth, 34 with root fractures and 35 without, were examined under standardized conditions using an intraoral PSP system (Digora Optime; Soredex). The images were saved in original 8-bit format and in a version sharpened with a high-pass filter. The teeth were examined with CBCT (i-Cat; Imaging Sciences) in 2 resolutions: 0.125 mm and 0.25 mm voxel size. Original images were saved together with images enhanced with 2 high-pass filters, sharpen and angio-sharpen. Six observers scored the presence of a root fracture in all modalities in random order. Sensitivity, specificity, and accuracy [(true positives + true negatives)/all scores] were calculated for each modality and each observer. Differences were estimated by analyzing the binary data, assuming additive effects of observer and modality in a generalized linear model. Results: High-resolution original CBCT images had higher sensitivity (P < .05) than lower-resolution images and PSP images (0.125 mm resolution 87%, 0.25 mm resolution 72%, and PSP 74%). Angio-sharpen-filtered images for both CBCT resolutions had higher sensitivities (P < .02) than the original images (0.125 mm resolution: 95% vs. 87%; 0.25 mm resolution: 81% vs. 72%). There was no significant difference between the lower-resolution CBCT and PSP images. Only small differences in specificity were seen between modalities, and accuracy was higher for high-resolution CBCT than for the other modalities (P < .03). Conclusions: High-resolution i-Cat CBCT images resulted in an increase in sensitivity without jeopardizing specificity for detection of transverse root fractures compared with lower-resolution CBCT images, which were not more accurate than periapical PSP images. The angio-sharpen high-pass filter improved sensitivity in the high-resolution CBCT images in this in vitro model.
Vertical root fracture diagnosis in teeth with metallic posts: Impact of metal artifact reduction and sharpening filters

Abstract:Purpose: This study examined the influence of a metal artifact reduction (MAR) tool, sharpening filters, and their combination on the diagnosis of vertical root fracture (VRF) in teeth with metallic posts using cone-beam computed tomography (CBCT). Materials and methods: Twenty single-rooted human premolars - 9 with VRF and 11 without - were individually placed in a human mandible. A metallic post composed of a cobalt-chromium alloy was inserted into the root canal of each tooth. CBCT scans were then acquired under the following parameters: 8 mA, a 5×5 cm field of view, a voxel size of 0.085 mm, 90 kVp, and with MAR either enabled or disabled. Five oral and maxillofacial radiologists independently evaluated the CBCT exams under each MAR mode and across 3 sharpening filter conditions: no filter, Sharpen 1×, and Sharpen 2×. The diagnostic performance was quantified by the area under the receiver operating characteristic curve (AUC), sensitivity, and specificity. These metrics were compared using 2-way analysis of variance with a significance level of α=5%. Intra- and inter-examiner agreement were assessed using the weighted kappa test. Results: Neither MAR nor the application of sharpening filters significantly impacted AUC or specificity (P>0.05). However, sensitivity increased when MAR was combined with Sharpen 1× and Sharpen 2× (P=0.015). The intra-examiner agreement ranged from fair to substantial (0.34-0.66), while the inter-examiner agreement ranged from fair to moderate (0.27-0.41). Conclusion: MAR in conjunction with sharpening filters improved VRF detection; therefore, their combined use is recommended in cases of suspected VRF.
Influence of Artifacts Induced by Dental Implants on The Detection of Vertical Root Fractures in Cone Beam Computed Tomographic Scans. An in-Vitro Study

Abstract:Aim: The aim of this study was to evaluate the influence of implant metal artifacts on the detection of vertical root fracture of teeth adjacent to dental implants. Materials and Methods: Sixty extracted single-rooted teeth were randomly divided into vertical root fracture (VRF) and non-fractured groups (NVF) (n=30). Root fracture was induced in VFR, then teeth were positioned in the right and left posterior areas of epoxy- resin mandible model mesial and distal to two titanium implants. Three CBCT scan protocols were done: roots without implants, roots with implant without artifact reduction and roots with implant with artifact reduction. The images were evaluated by three observers. Area under the receiver-operating characteristic curve (ROC), diagnostic accuracy, sensitivity, and specificity were calculated. Results: Statistically significant difference was found in diagnostic accuracy of CBCT of vertical root fracture for all test groups with highest accuracy in group without implant (p =0.022). Post hoc pairwise comparisons showed statistically significant difference between accuracy of group without implants and group with implants and without algorithm (p=0.019). Sensitivity, specificity, positive and negative predictive values were higher in group without implant but there was no statistically significant difference between the three groups. Conclusions: Implant metal artifacts influenced the diagnostic accuracy, sensitivity and specificity for detection of vertical root fracture but still high levels were achieved and all image possessed high diagnostic quality. Artifact reduction tool improved the accuracy and specificity and can be recommended when the teeth involved are near dental implant.

Comparative diagnostic yield of cone beam CT reconstruction using various software programs on the detection of vertical root fractures

Abstract:Objectives: To evaluate the effect on diagnostic yield in the detection of experimentally induced vertical root fractures on cone beam CT images using four dental software program. Methods: 190 single-rooted extracted human teeth were divided into three groups according to the pulp canal status: unrestored (UR), filled with gutta-percha (GP) and restored with a metallic custom post (Post). One-half of the sample of each group was artificially fractured and the segments repositioned. All teeth were scanned on a cone beam CT device at 0.2 mm nominal voxel resolution (i-CAT Platinum; Imaging Sciences International, Hatfield, PA). The data were exported as digital imaging and communications in medicine files and imported into Dolphin Imaging & Management Solutions, v. 11.5 (Patterson Dental Supply Inc., St Paul, MN), InVivoDental, v. 5.0 (Anatomage Inc., San Jose, CA) and Kodak Dental Imaging Software 3D module, v. 2.1.11 (Carestream Health Inc., Rochester, NY) software. Cross-sectional images in the acquisition (using Xoran CAT™, v. 3.0.34 software; Xoran Technologies, Ann Arbor, MI) and additional software were presented to three calibrated oral radiologists who rated the presence or absence of root fracture on a five-point scale. Receiver operating characteristic analysis was performed, and treatment comparisons compared by analysis of variance and pairwise comparisons were performed using Tukey's test at an a priori value of α < 0.05%. Results: All dental software performed equally at detecting fractures. Fractures were significantly more difficult to detect when posts were present. Conclusions: The diagnosis of root fracture is software-independent. The presence of an intracanal metallic post significantly decreases the detection of artificially created root fractures.

Abstract:A definitive diagonosis of vertical root fracture (VRFs) is often a challenging task for clinicians. Two-dimensional periapical radiographs (PRs) may be not helpful in such a diagnosis when the x-ray beam is not parallel to the plane of the fracture line. This report presents a set of 3 cases in which 1 endodontically treated and 2 nonendodontically treated mandibular molars were diagnosed with VRFs based on findings from clinical, radiographic, and cone-beam computerized tomographic (CBCT) examinations. After extraction, VRFs were confirmed in all of the teeth. Deep and narrow periodontal pockets were detected in 2 molars. A widening of the root canal space was observed in the PR of 1 molar only, and crown cracks were detected in none of these cases. However, in all 3 molars, fracture lines were visible on the CBCT images. Thus, CBCT provided useful information in diagnosing VRFs in both endodontically treated and nonendodontically treated teeth, especially when VRFs could not be confirmed by clinical findings and PRs. Abstract:INTRODUCTION: We aimed to compare the diagnostic accuracy of 5 cone-beam computed tomographic (CBCT) systems in detecting vertical root fractures (VRFs) and to assess whether fracture identification is affected by the presence of root canal filling.METHODS: Eighty extracted posterior teeth were included in this study. They were grouped according to the presence/absence of VRFs and the presence/absence of endodontic treatment. The teeth were then inserted in 5 dry skull/mandible assemblies. CBCT scans were performed using 5 different commercially available systems. Two observers evaluated the resultant multiplanar images twice for VRFs using a 3-point scale.RESULTS: i-CAT (Imaging Sciences International, Hatfield, PA) showed the highest diagnostic accuracy in the detection of VRFs among the 5 investigated CBCT systems. The presence of root canal filling did not significantly decrease the ability to detect VRFs by all the studied systems. In the detection of VRFs in endodontically treated teeth, i-CAT was the most accurate, whereas 3D Accuitomo (J. Morita, Kyoto, Japan) was the least. Interobserver agreement was moderate for the i-CAT and fair for the rest of the studied modalities, whereas intraobserver agreement was good for the Scanora 3D (Soredex, Tuusula, Finland) and moderate for images from the other CBCT machines.CONCLUSIONS: At the specified exposure parameters in the detection of VRFs in non-root canal-filled teeth, i-CAT showed the highest diagnostic accuracy followed by Planmeca Promax 3D (Planmeca, Helsinki, Finland), Scanora 3D, Accuitomo 3D, and Galileos 3D (Sirona Dental Systems, Bensheim, Germany), respectively. In the detection of VRFs in root canal-filled teeth, i-CAT showed the highest diagnostic accuracy followed by Planmeca Promax 3D, Scanora 3D, Galileos Comfort (Sirona Dental Systems), and Accuitomo 3D.

Abstract:Introduction: The purpose of this study was to evaluate the diagnostic accuracy of cone-beam computed tomographic (CBCT) imaging in the detection of subtle vertical root fractures (VRFs) in endodontically treated teeth in vivo and to discuss direct and indirect evidence for the diagnosis of subtle VRFs. Methods: Twenty-nine endodontically treated teeth with suspected VRFs from 29 patients were examined using CBCT imaging. CBCT images were scored based on evaluation of the fracture line and vertical buccopalatal (lingual) bone loss. VRF was diagnosed only when a definite fracture line was detected on CBCT images, and findings of periodontal exploration or surgical extraction were considered the gold standard. The diagnostic sensitivity, specificity, and accuracy were assessed. Results: Of the 29 teeth, 27 were positive and 2 were negative for VRF according to the gold standard. Sensitivity, specificity, and accuracy were 33.3%, 100%, and 37.9%, respectively, based on the direct finding of a fracture line on CBCT images. However, vertical buccopalatal (lingual) bone loss was found in 25 of 27 teeth with VRFs. Conclusions: Although the accuracy of CBCT imaging for the diagnosis of subtle VRFs in endodontically treated teeth in vivo was poor, vertical buccopalatal (lingual) bone loss is an important indirect sign for the diagnosis of VRFs, which can be found on CBCT images.

Abstract:Abstract Objectives The current study aimed to evaluate different CBCT exposure protocols and influencing factors affecting the subjective image quality of scans taken for endodontic indications. Materials and methods Twelve extracted teeth, comprising of two sets of maxillary molars, premolars, canines and incisors, mandibular premolars, and molars, were endodontically treated, and either received a fiber or metal post. The teeth were scanned by CBCT imaging before and after root canal treatment, and after post insertion. Each scan was performed thrice, using an ultra low dose (ULD), standard (SM), and high-resolution mode (HR), respectively. Twelve observers—4 endodontists, 4 periodontists, and 4 radiologists—assessed the subjective image quality using visual analogue scales (VAS). Potential influencing factors were evaluated including acquisition mode, observer specialty, stage of treatment, type of post, and type of tooth, using one-way ANOVA and T test. Results Teeth scanned with the ULD had the highest average VAS score (72.5), followed by HR (70.2), and SM (69.0) for values pooled from all teeth and observers. CBCT acquisition mode was not a significant influencing factor on the VAS scores. Observer specialty, stage of treatment, type of post, and type of tooth were significant influencing factors. Conclusions Based on the present in vitro data, a low-dose CBCT mode seems not to negatively affect the perception of image quality. Clinical relevance The findings from this in vitro study demonstrate that a low-dose CBCT mode might have potential for diagnostics prior to or following endodontic treatment.

Abstract:Abstract Background and aim: Patients with dysplasia in Barrett’s esophagus (BE) have a considerable risk of developing esophageal adenocarcinoma (EAC). The mucosal expression of the pro-inflammatory angiotensin II receptor type 1 (AT1R) is elevated in these patients, suggesting a role in carcinogenesis. The purpose of this study was to determine whether interference with the renin–angiotensin system (RAS) would influence downstream markers of carcinogenesis. Methods: Endoscopic mucosal biopsies from BE patients with low-grade dysplasia (LGD) were sampled before and after a three-week period of RAS-interfering treatment. Thirty patients were randomly allocated to enalapril (ACE inhibitor, 5 mg od), candesartan (AT1R antagonist, 8 mg od), or no drug. The expression of 12 proteins known to be associated with RAS and carcinogenesis was assessed using western blot. Results: We found altered expression of several proteins after enalapril treatment (decreased: NFκB, p = .043; NLRP3, p = .050; AMACR, p = .017; and caspase 3, p = .025; increased: p53, p = .050). Candesartan treatment was associated with increased iNOS expression (p = .033). No significant changes were seen in the no-drug group. Conclusion: Interference with angiotensin II formation was associated with altered expression of inflammation- and carcinogenesis-related proteins. The present results speak in favor of involvement of angiotensin II in BE dysplasia, but the role of AT1R should be investigated further.

Abstract:Individual-level health behaviors and outcomes have multi-level determinants (individual and environmental). Multi-level analysis seeks to explain individual outcomes in terms of both individual and environmental or aggregate variables. Ecologic fallacy (improper inference about individual-level associations based on associations measured only at the aggregate level) can result from confusion about the level of inference that is of ultimate interest. The perspective of multi-level analysis acknowledges the importance of both individual and environmental variables in determining health behaviors and outcomes at the level of the indivisible unit--the individual. The authors review concepts and methods of multi-level analysis and their application to epidemiologic research on health behavior and health outcomes.
Exploring the Use of Enhanced Cone-Beam CT Technique to Diagnose Vertical Root Fracture.

Abstract:OBJECTIVES:To explore the feasibility of using sodium iodide (NaI)+dimethyl sulfoxide (DMSO)+ethyl alcohol+ethyl acetate as a cone-beam CT (CBCT) contrast agent in the diagnosis of vertical root fracture (VRF). METHODS:21 endodontically treated VRF teeth of 21 patients were collected in this study. All these 21 teeth were confirmed subtle fracture lines under transillumination, the number and position of fracture lines were recorded. All these patients had CBCT routine scanning (RS1) before extraction. After extraction, the teeth was performed micro-CT scanning and 3 in vitro CBCT scanning: CBCT routine scanning in vitro(RS2), CBCT enhanced scanning using meglumine diatrizoate (MD) as contrast agent(ES1); and CBCT enhanced scanning using NaI+DMSO+ethyl alcohol+ethyl acetate as contrast agent(ES2). The number of fracture lines was evaluated on all the 5 scanning modes and the accuracy of diagnosis was calculated. RESULTS:In all, there were 43 fracture lines on the 21 teeth. The accuracy of detection of fracture lines of CBCT RS1, RS2, ES1, ES2 and micro-CT was 0%, 20.9% (9/43), 11.6% (5/43), 93% (40/43) and 95.3% (41/43) respectively. Significant differences were found between ES2 vs. RS2, ES2 vs. ES1 (p < 0.01); however, no significant difference was found between ES2 vs. micro-CT (p > 0.05). CONCLUSION:CBCT enhanced scanning using NaI+DMSO+ethyl alcohol+ethyl acetate as contrast agent could be a prospective technique in the diagnosis of VRF.
Comparison of bisecting and parallel intraoral radiography and cone-beam computed tomography for detecting various horizontal angle root fractures

Abstract:ObjectivesThe aim of this study was to compare cone-beam computed tomography (CBCT) with intraoral radiography (parallel and bisecting techniques) for their detection capabilities of various horizontal angle root fractures in human teeth.MethodsEighty-one extracted human upper central incisors were divided into three groups: (1) right-angle group (n = 27), in which a groove was formed perpendicular to the long axis of the root at the middle of the root; (2) 75-degree group (n = 27), which was grooved at 75° in the cervical third of the root; and (3) 55-degree group (n = 27), which was grooved at 55° in the middle third of the root. In an additional control group (n = 9), the teeth remained without grooves. The specimens were examined by the above-mentioned three imaging techniques by seven oral radiologists, and the grooves were visualized as radiolucent lines traversing the roots.ResultsThe detection sensitivities for the parallel technique and CBCT were significantly greater than those for the bisecting technique for the right-angle and 75-degree groups, whereas the bisecting technique and CBCT showed significantly greater detection sensitivities than the parallel technique for the 55-degree group. There were no significant differences in specificity among the three imaging techniques.ConclusionsCBCT demonstrated high diagnostic accuracy for teeth with root fractures at any angle. When fracture lines are not located clearly by intraoral radiography at only one projection angle, additional intraoral radiography with different angles can improve detection.

Abstract:Introduction This study explores the differences between the patterns of bone defects associated with vertical root fracture (VRF) and apical periodontitis (AP) in single-rooted endodontically treated premolars (SRETPs) based on cone-beam computed tomography (CBCT) data. Methods Eighty-four SRETPs were extracted and categorized into the VRF and AP groups. On preoperative CBCT images, the location of bone defects according to the root thirds in buccolingual and mesiodistal directions across the study groups were compared. Results The majority of bone defects in the VRF group were longitudinal and combined, involving more than one root thirds in buccolingual and mesiodistal directions simultaneously. A uniform approach to comparing bone defects using the sites of periradicular area with bone loss as a comparison unit was developed. In the VRF group, bone loss sites in the middle and coronal thirds were detected more often and were located mainly buccolingually compared with the AP group ( P < .001). Conclusion Bone defects in the middle or middle and coronal root thirds in the buccolingual direction may be potential radiographic signs useful in differentiating between VRF and AP in SRETPs. The introduction of the uniform approach to assessment of bone loss patterns will give practitioners a single simple tool and improve the quality of endodontic treatment.

Evaluation Of Dental Root Fracture Using Cone-Beam Computed Tomography

Abstract:Objective: To evaluate the application of cone-beam computed tomography (CBCT) in the detection of dental root fractures and to analyse the demographic profile of these fractures.Methods: The study group comprised 398 teeth that were examined by CBCT for determining whether they had a root fracture. Patient characteristics were recorded, and the location, fractured roots, fracture types and three-dimensional images of the related skeletal structures were analysed. Two experienced oral radiologists independently analysed each case and reached a consensus, and the diagnosis was graded in one of the following three categories: fracture definitely present (FDP), fracture probably present (FPP) and no visible fracture (NVF).Results: Among these teeth, 155 (39.0%) were diagnosed as FDP, 14 (3.5%) as FPP and 229 (57.5%) as NVF in the consensus reading. During follow-up, all teeth diagnosed as FDP and 4 of the 14 teeth diagnosed as FPP were intra-operatively demonstrated to be fractured. In FDP cases, 60.0% of the patients were aged from 50 to 69 years. In total, 107 FDP teeth were non-endodontically treated, and the remaining were endodontically treated with (n = 16) or without (n = 32) crown placement. The maxillary and mandibular molars were most frequently affected (81.9%). The fractured roots were mostly palatal (65.7%) in maxillary molars and mesial (84.2%) in mandibular molars. The fractures were characterised as vertical (n = 84), horizontal (n = 34), oblique (n = 5) and complicated (n = 32) fractures.Conclusion: The application of CBCT is valuable for the diagnosis of root fracture.

Abstract:The {rho}-meson contribution to the {Lambda}{ital N}{r_arrow}{ital NN} process is studied in the nonmesonic decay of hypernuclei. It is found that the central potential of the {rho} exchange is larger than its tensor part and, therefore, must be included. This is in contrast to the {pi}-exchange term which is dominated by the tensor interaction.

Abstract:This study aimed to evaluate the accuracy of detecting vertical root fractures in BiodentineTM-filled teeth using the Promax 3Dmax cone-beam computed tomography (CBCT) unit compared to periapical radiographs. It tested hypotheses regarding CBCT's diagnostic superiority in non-root-filled and BiodentineTM-root-filled maxillary central incisors and assessed the impact of smaller field of view and lower intensity settings on detection accuracy.

Influence of an adjacent zirconium implant, tube current, and metal artifact reduction algorithm on horizontal root fracture diagnosis in cone beam computed tomography

Abstract:Objective: We evaluated the influence of an adjacent zirconium implant, tube current (mA), and a metal artifact reduction algorithm (MARA) on horizontal root fracture (HRF) diagnosis in cone beam computed tomography (CBCT). Study design: Nineteen teeth (9 with HRF, 10 without) were individually placed in a human maxilla. Scan volumes were acquired without and with a zirconium implant adjacent to the tooth at settings of 4, 8, and 10 mA, with MARA disabled and enabled, using a 5×5 cm field of view, 0.085-mm voxel size, and 90 kV. Four maxillofacial radiologists individually assessed the scans. Diagnostic metrics were compared by multiway analysis of variance (α=5%). Inter- and intraexaminer agreements for HRF diagnosis were evaluated with the weighted kappa test. Results: Area under the receiver operating characteristic curve (AUC), sensitivity, and specificity were significantly lower in the presence of the implant (P≤.005). AUC values were higher in scans obtained with 8 and 10 mA compared with 4 mA (P=0.010), but 10 mA without MARA was better with the implant present. MARA did not significantly influence outcomes (P≥0.240). Inter- and intraexaminer agreements ranged from moderate to almost perfect. Conclusions: The presence of the zirconium implant impairs HRF detection. Settings of 8 or 10 mA improve HRF detection regardless MARA condition without the implant. With an adjacent implant, 10 mA without MARA is recommended to improve diagnostic performance.
Accuracy of Periapical Radiography and CBCT in Endodontic Evaluation

Abstract:Introduction: A radiological evaluation is essential in endodontics, for diagnostic purposes, planning and execution of the treatment, and evaluation of the success of therapy. The periapical radiography is nowadays the main radiographic investigations used but presents some limits as 3D anatomic alteration, geometric compression, and possible anatomical structures overlapping that can obscure the area of interest. CBCT (cone beam computed tomography) in endodontics allows a detailed assessment of the teeth and surrounding alveolar anatomy for endodontic diagnosis, treatment planning, and follow-up. Objective: The purpose of this study was to evaluate the accuracy of CBCT in comparison with conventional intraoral radiographs used in endodontic procedures. Materials and methods: Statistical analysis was performed on 101 patients with previous endodontic treatments with the relative radiographic documentation (preoperative, postoperative, and follow-up intraoral X-ray) that had underwent at CBCT screening for surgical reasons. The CBCT scans were evaluated independently by two operators and compared with the corresponding periapical images. Results: Our analysis shows that the two radiological investigations statistically agree in 100% of cases in the group of patients without any endodontic sign. In the group of patients with an endodontic pathology, detected with CBCT, endodontic under extended treatments (30.6%), MB2 canals in nontreated maxillary molars (20.7%), second canals in nontreated mandibular incisors (9%), root fractures (2.7%), and root resorption (2.7%) were not always visible in intraoral X-ray. Otherwise, positivity in the intraoral X-ray was always confirmed in CBCT. A radiolucent area was detected in CBCT exam in 46%, while the intraoral X-ray exam was positive only in 18%. Conclusions: Our study shows that some important radiological signs acquired using CBCT are not always visible in periapical X-ray. Furthermore, CBCT is considered as a II level exam and could be used to solve diagnostic questions, essential to a proper management of the endodontic problems.
