Abstract:Objective: To verify the efficacy and safety of daily oral minodronate in postmenopausal women with established osteoporosis. Methods: In this randomized, double-blinded, placebo-controlled trial, 262 postmenopausal women were enrolled. Patients were randomized to receive daily oral minodronate 1 mg with supplements of 500 mg calcium and 200 U vitamin D3 (n=130) or placebo (n=132) with daily supplements of 500 mg calcium and 200 U vitamin D3, for 48 weeks. The primary endpoint was the average bone mineral density (BMD) change in the lumbar vertebrae 48 weeks post-treatment. Secondary outcome measures was the incidence of vertebral fractures. Safety assessments included the rate of adverse events. Results: At the end of 48 weeks treatment, the average BMD change rate from baseline were: full analysis set results: (3.52±4.82)% in the minodronate group and (2.00±5.74)% in the placebo group; per-protocol set results: (3.99±5.05)% in the minodronate group and (2.07±6.20)% in the placebo group; the differences were all significant (all P<0.05). Vertebral fracture occured in 3 patients (2.3%, 3/132) in the placebo group, and 1 case (0.8%, 1/130) in the minodronate group (P>0.05). The incidence of adverse events was 71.5% (93/130) in the minodronate group and 78.0% (103/132) in the placebo group (P>0.05). Conclusion: Minodronate is effective and safe in the treatment of postmenopausal osteoporosis without severe side effects.

Abstract:The purpose of this study was to perform a meta-analysis to examine the efficacy and safety of denosumab in postmenopausal women with osteoporosis.Medline, Cochrane Library, EMBASE, and Google Scholar databases were searched until October 30, 2014 using combinations of the following search terms: osteoporosis, postmenopause, postmenopausal, women, denosumab. The primary outcome was bone mineral density (BMD) change, and secondary outcomes were change in the bone turnover markers β-isomerized carboxy-terminal cross-linking telopeptide of type I collagen (CTX) and serum procollagen type I amino-terminal propeptide (P1NP), and adverse events.Patients treated with denosumab had significantly increased BMD of the lumbar spine (7.58%), total hip (4.86%), and distal third of the radius (2.92%) than those treated with placebo (all, P < 0.001). Patients treated with denosumab had a significant decrease of CTX (-66.16%) and P1NP (-64.65%) as compared with those treated with placebo (both, P < 0.001). Adverse events were similar between the 2 groups (pooled odds ratio = 1.04, P = 0.625).Denosumab increases BMD and decreases markers of bone turnover in postmenopausal women with osteoporosis, and is not associated with significant side-effects.
[Phase III Clinical Study of Zoledronic Acid in the Treatment of Pain Induced by Bone Metastasis from Solid Tumor or Multiple Myeloma].

Abstract:OBJECTIVETo evaluate the efficacy and safety of zoledronic acid in the treatment of bone pain in patients with bone metastasis from solid tumor or multiple myeloma.METHODSA randomized, double-blind, double-simulated and multi-center phase III clinical trail with pamidronate as control was conducted. Patients with moderate to severe bone pain (VAS > 50 mm) induced by solid tumor or multiple myeloma were randomized to receive intravenous zoledronic acid 4 mg or pamidronate 90 mg. Then the change of VAS and urinary NTX/Cr and CTX/Cr were observed in two groups.RESULTSFrom July 2005 to September 2006, 228 patients with bone pain induced by bone metastasis from 15 cancer centers were randomize into two groups: 116 patients in zoledronic acid group and 112 patients in pamidronate group. The VAS value was decreased gradually after treatment in these two groups. Significant improvement in bone pain after treatment were observed both in zoledronic acid group and the control group when compared with baseline VAS on D8 (-11.77% vs. -10.87%), D15 (-24.60% vs. -21.06%) and D28 (-32.37% vs. -31.26%) (P< or =0.0001), but no significant difference existed between two groups (P =0.6587). Compared with baseline, urine NTX/Cr and CTX/Cr level were decreased rapidly after treatment in both groups, the nadir was on D8, the median decreased on D28, which was -36.9% vs. -32.1% for NTX/Cr (P = 0.7922) and -63.2% vs. -47.9% for CTX/Cr (P =0.834). The frequently observed adverse events were pyrexia (19.0% vs. 31.3%), vomiting (6.0% vs. 8.9%), nausea (4.3% vs. 4.5%), fatigue (3.4% vs. 2.7%) and constipation (2.6% vs. 1.8%) in the two groups. Compared with baseline, the serum creatinine level was not significantly increased throughout the study.CONCLUSIONIntravenous injection of 4 mg zoledronic acid can significantly reduce bone pain and bone resorption marker in urine in the Chinese patients with bone metastasis from solid tumor or multiple myeloma, which is tolerable and also comparable to pamidronate in the efficacy and safety.
Comparison of Clinical Efficacy and Safety Between Denosumab and Alendronate in Postmenopausal Women with Osteoporosis: a Meta-Analysis

Abstract:The aim of this study was to perform a head-to-head comparison of efficacy and safety profile between 60 mg denosumab (Den) subcutaneously (SC) per 6 months (Q6M) and 70 mg alendronate (Aln) orally per week (QW) for postmenopausal women with low bone mineral density. We searched electronic databases comparing efficacy and safety of Den SC Q6M and Aln QW in postmenopausal women. The primary outcomes of efficacy evaluation in included trials were incidence of clinical fracture in both groups and bone mineral density (BMD) at different skeletal sites. And adverse events (AEs), including incidence of neoplasms and infections, were considered as secondary outcomes. Following the instructions of ‘Cochrane Handbook for systematic Reviews of Interventions 5.0.2’, we identified eligible studies, evaluated the methodological quality and abstracted relevant data. Four heterogeneous randomised controlled trials (RCTs) involving 1942 women were identified. The results of review showed low evidence quality that supported the hypothesis the denosumab vs. alendronate could reduce risk of fracture [OR (95% CI) 1.42 (0.84 to 2.40), 11 more women per 1000 (from 4 fewer to 36 more), p = 0.19] but the moderate to high quality evidence suggesting treatment with 60 mg Den SC Q6M was more effective for postmenopausal women in increasing BMD [at distal radius (DR), total hip (TH), lumbar spine (LS), and femoral neck (FN)]. Hazards of neoplasms [OR (95% CI) 1.10 (0.65 to 1.86), 3 more per 1000 (from 10 fewer to 24 more), p = 0.62] or infections [OR (95% CI) 0.95 (0.79 to 1.15), 12 fewer per 1000 (from 53 fewer to 33 more,), p = 0.62] were appeared to be similar.Our review suggested within 1 year 60 mg Den SC Q6M treatment was more effective in increasing bone mass but could not reduce the fracture risk to a greater extent than 70 mg Aln QW therapy. Also the Den SC Q6M therapy did not increase the risks of neoplasms and infections compared with Aln QW.

Abstract:Objective:To investigate whether there is an effective attenuation of osteoporosis treatments on BMD in-crease and whether they are safe with increasing ages and increasing uses of zoledronic acid. To investigate whether there ’s any improvement of health-related quality of life (HRQL) as zoledronic acid improves BMD. Methods: In this self-control, prospective trial, 220 postmenopausal women with osteoporosis (mean age 67 years) received a single infusion of ZOL 5 mg at baseline in the 12th month. BMD and HRQL were measured at baseline, in the 12th and 24th month (before each use of zoledronic acid). The main outcome measures were the changes in lumbar spine and hip BMD and the changes in HRQL by the Short Form-36 questionaire (SF-36). The ANOVA and LSD multiple comparison was used for statistical comparison. Safety evaluation was conducted at the same time. Results: The patients with postmenopausal osteoporosis had significantly higher L1-4 (0.841 ±0.134), total hip (0.730 ±0.096), femoral neck (0.695 ±0.073), Ward’s triangle (0.498 ±0.036) and trochanter (0.559±0.056) BMD (P<0.05) and improved BP, GH and PCS in HRQL (P<0.05) over 2-year treatment of once-yearly ZOL 5mg. No serious adverse events directly related occurred. Conclusions: Zoledronic acid is safe and effective, increases bone mineral density substantially, and increases health-related quality of life, especially in the physical aspects, over 2 years of treatment in women with postmenopausal osteoporosis.
A randomized, double-blind, placebo-controlled study to evaluate the effects of alendronate on bone mineral density and bone remodelling in perimenopausal women with low bone mineral density

Abstract:Background: Perimenopausal women can experience rapid bone loss at skeletal sites with both cortical and cancellous bone, increasing the prevalence of osteoporosis following menopause. Methods: We conducted a 12-month randomized placebo-controlled trial evaluating the effects of alendronate 70 mg with 2800 IU cholecalciferol administered once per week for 12 months in comparison with placebo and cholecalciferol. The primary end-point was the percentage change in the lumbar spine bone mineral density (BMD) from baseline to 12 months. Secondary end-points were the change in BMD at the femoral neck and changes in biochemical markers of bone turnover. Results: Forty-five women were recruited to participate in the study. Five subjects withdrew from the study before randomization for unrelated reasons. Forty subjects were randomly allocated to the alendronate and placebo groups. The mean lumbar spine MD in women treated with alendronate increased by 3.66% (mean paired difference, μd = 0.032; ± 0.008 SE) at 12 months, compared with a reduction of 3.33% (μd = -0.030; ± 0.008 SE) in the control group (P < 0.001). In the femoral neck, the mean BMD in the alendronate group increased by 2.07% (μd = 0.014; ± 0.009 SE) at 12 months, compared with a reduction of 1.87% (μd = -0.014; ± 0.008 SE) in the control group (P = 0.046). There were no differences in BMD between the alendronate and placebo groups at the total hip sites after 12 months. At 12 months, both bone-specific alkaline phosphatase and urinary N-telopeptide were significantly reduced, by 37.79% (μd = -9.90; ± 1.92 SE) and 27.21% (μd = -11.68; ± 4.80 SE) respectively, in the alendronate group; in the control group, these levels increased (P < 0.001). Conclusion: Weekly treatment with alendronate 70 mg and cholecalciferol 2800 IU increases BMD and decreases bone turnover in perimenopausal women.
Moxibustion as adjuvant therapy for preventing bone loss in postmenopausal women: protocol for a randomised controlled trial

Abstract:Introduction Postmenopausal osteoporosis, caused by ageing and oestrogen deficiency, seriously threatens women's physical and mental health. Postmenopausal osteopenia is the transition from healthy bone to osteoporosis, and it may be the key period for preventing bone loss. Moxibustion, a physical therapy of Traditional Chinese Medicine, has potential benefits for osteoporosis treatment and prevention, but it has not been adequately studied. This study aims to explore the clinical effects and safety of moxibustion in delaying bone loss in postmenopausal women. Methods and analysis In this parallel-design, randomised, patient-blind and assessor-blind, controlled clinical study, 150 women with osteopenia at low fracture risk will be randomly assigned to a moxibustion treatment (MT) group or a placebo-moxibustion control (PMC) group in a 1:1 ratio. In addition to the fundamental measures (vitamin D3 and calcium) as recommended by the guidelines, participants of the two groups will receive MT or PMC treatment for 42 sessions over 12 months. The primary outcome will be the bone mineral density (BMD) of the lumbar spine at the end of the 12-month treatment, and secondary outcomes will be the BMD of the femoral neck and total hip, T-scores, bone turnover markers, serum calcium levels, serum magnesium levels, serum phosphorus levels, serum parathyroid hormone levels and 25-hydroxyvitamin D levels, intensity of bone pain, quality of life, incidence of osteoporosis and fractures, usage of emergency drugs or surgery, participant self-evaluation of therapeutic effects and the rate of adverse events. All statistical analyses will be performed based on the intention-to-treat and per-protocol principle. Ethics and dissemination Ethics approval has been obtained from the Ethics Committee on Biomedical Research, West China Hospital of Sichuan University (permission number: 2021-1243). The results are expected to be published in peer-reviewed journals. Trial registration number ChiCTR2100053953.

Efficacy and safety of zoledronic acid in the treatment of osteoporosis: A meta-analysis of randomized controlled trials

Abstract:Purpose: Zoledronic acid can inhibit the activity of osteoclasts, and thus, may slow or inhibit bone loss. The purpose of this study was to systematically evaluate the efficacy and safety of zoledronic acid in the treatment of osteoporosis. Methods: Four databases, PubMed, Embase, Cochrane Library, and Web of Science, were systematically searched up to December 26, 2022. The primary outcomes included bone mineral density (BMD), carboxy-terminal cross-linked telopeptide of type 1 collagen (CTX), bone-specific alkaline phosphatase (BSAP), procollagen type 1 N-terminal prope-ptide (P1NP), adverse events, and fracture. Secondary outcomes included serum sclerostin level, Visual Analogue Scale (VAS) score, and Oswestry Disability Index (ODI). Results: A total of 22 randomized controlled trials were included in this meta-analysis. Meta-analysis results showed that zoledronic acid was effective in increasing BMD of the lumbar spine, femoral neck, trochanter and serum sclerostin level; and reduced CTX, BSAP, P1NP, VAS score, and ODI in patients with osteoporosis. Regarding safety, zoledronic acid could reduce the incidence of fractures but had relatively more adverse events. Conclusion: Zoledronic acid can significantly improve BMD of the lumbar spine, femoral neck and trochanter, and effectively reduce incidence of fracture in patients with osteoporosis, thereby significantly improving patients' quality of life. However, the incidence of adverse events was higher than that of patients treated with placebo.

Abstract:The current study evaluated the efficacy and safety of a denosumab biosimilar, QL1206 (60 mg), compared to placebo in postmenopausal Chinese women with osteoporosis and high fracture risk. At 31 study centers in China, a total of 455 postmenopausal women with osteoporosis and high fracture risk were randomly assigned to receive QL1206 (60 mg subcutaneously every 6 months) or placebo. From baseline to the 12-month follow-up, the participants who received QL1206 showed significantly increased bone mineral density (BMD) values (mean difference and 95% CI) in the lumbar spine: 4.780% (3.880%, 5.681%), total hip :3.930% (3.136%, 4.725%), femoral neck 2.733% (1.877%, 3.589%) and trochanter: 4.058% (2.791%, 5.325%) compared with the participants who received the placebo. In addition, QL1206 injection significantly decreased the serum levels of C-terminal crosslinked telopeptides of type 1 collagen (CTX): −77.352% (−87.080%, −66.844%), and N-terminal procollagen of type l collagen (P1NP): −50.867% (−57.184%, −45.217%) compared with the placebo over the period from baseline to 12 months. No new or unexpected adverse events were observed. We concluded that compared with placebo, QL1206 effectively increased the BMD of the lumbar spine, total hip, femoral neck and trochanter in postmenopausal Chinese women with osteoporosis and rapidly decreased bone turnover markers. This study demonstrated that QL1206 has beneficial effects on postmenopausal Chinese women with osteoporosis and high fracture risk.
The effect of Zoledronic acid on pain and balance in women with postmenopausal osteoporosis

Abstract:Objective: To investigate whether there’s any help r elieving the pain of osteoporosis as Zoledronic acid (ZOL) improving bone mineral density(BMD). To observe the effect of ZOL accompanied by calcium and vitamin D on the balance ability in the course of treatment of postmenopausal osteoporosis. Methods: In this self-control, prospective trial, 220 postmenopausal women with osteoporosis(mean age 67 years) were to receive a single infusion of ZOL 5 mg at baseline and month 12. BMD, numerical rating scale(NRS) pain scores and fall index(FI) were measured at baseline, month 12 and 24 (before each use of zoledronic acid). The main outcome measures were the changes in lumbar spine and hip BMD, the changes in NRS pain scores and fall index . The ANOVA and least significant difference ( LSD ) multiple comparison was used for statistical comparison. Results: The patients with postmenopausal osteoporosis had significantly decreased NRS pain scores(P<0.05) with significantly higher lumbar and hip BMD over 2 year treatment of once-yearly ZOL 5 mg. And FI was decreased(P<0.05) with the oral daily elemental calcium and vitamin D in the treatment course. Conclusion: Zoledronic acid relieves pain with increased BMD over 2 years of treatment in women with postmenopausal osteoporosis. ZOL can help improve the balance ability.
Overall adjustment acupuncture for postmenopausal osteoporosis (PMOP): a study protocol for a randomized sham-controlled trial

Abstract:Background: Osteoporosis is becoming more prevalent in aging societies worldwide, and the economic burden attributable to osteoporotic fractures is substantial. The medications presently available to treat osteoporosis have side effects. Acupuncture is widely used for treating osteoporotic postmenopausal women because it is non-invasive and has fewer side effects, but the powerful clinical evidence for its efficacy remains insufficient. Our study intends to explore the effect of overall adjustment acupuncture (OA) in the treatment of postmenopausal osteoporosis (PMOP). Methods/design: This study is a randomized, sham-controlled, patient- and assessor-blinded trial and aims to evaluate the effect of OA in women with PMOP. We will recruit 104 women aged 45-70 years with a diagnosis of PMOP. Participants will be randomly allocated in a 1:1 ratio to the OA group and the sham acupuncture (SA) group. Both groups will receive real herbal medicine treatment as a basic treatment twice a day for 3 months, the OA group receives real acupuncture treatment and the SA group receives placebo acupuncture treatment (non-penetrating, sham skin-needle therapy, sham cupping). All patients will receive acupuncture treatment twice per week for 3 months. The primary outcome is bone mineral density (BMD) and the secondary outcomes include estradiol (E2), follicle-stimulating hormone (FSH), bone gla protein (BGP), bone alkaline phosphatase (BALP), total antioxidant capacity (TAC), advanced oxidation protein products (AOPP), PPARγ, β-catenin, FoxO3a levels, visual analog pain scale score (VAS), Traditional Chinese medicine (TCM) syndrome scores and quality of daily life score (QOL). Outcome measures will be collected at baseline, middle of the treatment (1.5 months), the end of treatment (3 months). The present protocol followed the SPIRIT guidelines and fulfills the SPIRIT Checklist. Conclusion: This study will be conducted to compare the efficacy of OA versus SA. This trial should help to evaluate whether OA can effectively prevent and treat PMOP by improving the estrogen levels of postmenopausal women. The mechanism is to improve the imbalance of osteogenic differentiation and lipogenesis of bone-marrow cells under oxidative stress. Abstract:Background Osteoporosis is a chronic, progressive, systemic condition of the skeletal tissue that is characterized by reduced bone density, microarchitecture deterioration, and fragile bones, making osteoporotic fractures or fragility fractures more likely to occur. This condition often remains asymptomatic and undiagnosed until it presents with fragility fractures. The condition is associated with a significant socioeconomic burden with disability, morbidity, and mortality. Therefore, early diagnosis, as well as treatment, is needed to prevent fractures. Intravenous zoledronic is an effective bisphosphonate with high patient compliance due to once-yearly dosing. The present study aims to determine whether zoledronic acid effectively treats chronic back pain in people with osteoporosis. Materials and methods Seventy patients above the age of 60 years presented with complaints of chronic low back aches to the outpatient department of orthopedics, R L Jalappa Hospital & Research Centre attached to Sri Devaraj Urs Medical College. The study was conducted between November 2016 and November 2018. Results All the patients found excellent clinical improvement following zoledronic acid infusion in early and long-term follow-ups. Additionally, it was found that zoledronic acid's effectiveness was excellent, with significant improvement in bone mineral density (BMD), T-score, and Z-score. Conclusion Early diagnosis and treatment of vertebral osteoporosis is the most important factor in preventing fragility fractures. Zoledronic acid, an antiresorptive drug with better compliance, is very effective in controlling low back pain, improving bone mineral density, and preventing the occurrence of atraumatic fragility fractures. With all the above factors, zoledronic acid is a preferable bisphosphonate for the treatment and prevention of osteoporosis compared to other modalities of treatment of osteoporosis.
Effect of alendronate sodium plus vitamin D3 tablets on knee joint structure and osteoarthritis pain: a multi-center, randomized, double-blind, placebo-controlled study protocol

Abstract:Osteoarthritis (OA) is a major cause of pain and disability worldwide. Despite the relatively high burden of the disease, the currently available non-surgical treatment options are directed towards symptomatic relief. Therefore, we propose the use of alendronate as a disease modifying agent to help slow and prevent OA. In addition, this study will utilize Whole-Organ Magnetic Resonance Imaging Score (WORMS) to evaluate the structural integrity of cartilage in the study population. High-quality evidence, limited to a few well-conducted randomized trials, highlights contradictory results on the effect of bisphosphonates on knee function and progression of OA. Therefore, a placebo-controlled, randomized trial is needed to evaluate the combined effect of alendronate and vit D on the structure of cartilage utilizing the WORMS score and its ability to treat knee pain in OA patients.

The Effects of Health Education and Comprehensive Lifestyle Modification on Postmenopausal Osteoporosis Women Treatment with Alendronate Sodium

Abstract:Objective To evaluate the effects of an intervention programme of health education and life style modification on postmenopausal osteoporosis women. Methods A total of 120 postmenopausal osteoporosis women were enrolled in this one-year randomized controlled follow-up study and assigned to the intervention group ( Group A, n = 60) or the control group ( Group B, n = 60). Both groups were treated with alendronate sodium. In Group A, education program was performed once a season in the form of face-to-face consultation or group session. In Group B, no additional intervention was used. The primary outcome was patients' compliance in follow-up. The secondary outcomes were change in bone mineral density (BMD).BMD was measured by dual-X-ray absorptiometry (DXA) on lumbar spine and hip at baseline and 12 months after the intervention. Results After one-year intervention,51 subjects in Group A and 38 in Group B completed the follow-up. Groups A showed better compliance. BMD on lumbar spine and hip was significantly increased in both groups when compared with baseline. The changes of BMD on lumbar (0.042+0.067 vs 0.026±0.070,P=0. O29) or Words region (0.029 +0. 129 vs 0.023±0. 143,P=0. 041 ) showed statistical significance between the two groups. Conclusion For alendronate sodium treatment, health management ensures the effectiveness of the therapy and improves the compliance of the patients.
Study of Ibandronate and Alendronate Treatment for Postmenopausal Osteoporosis

Abstract:Objective To evaluate the effect of ibandronate and alendronate on bone mass in postmenopausal osteoporosis.Methods Sixty-four postmenopausal women aged 47 to 80 years old were randomly divided into two groups.Patients in Group I were administered oral ibandronate 150 mg per month,and patients in Group II were administered oral alendronate 70 mg per week.All patients received calcium 500 mg and vitamin D 200 IU daily for one year.Bone mineral density in the lumbar vertebra and the hip were examined before and after the treatment.Results BMDs in the lumbar spine of patients in Group I were increased by 6.25% and 9.64%,respectively,after a half-year and one-year treatment(P0.05),and increased in patients in Group II by 6.82% and 11.4%,respectively(P0.05).In both group,BMDs of the hip also increased.However,the most obvious BMD increased occurred in the lumbar spine.Conclusions Treatment with both bisphosphonates improved BMD in postmenopausal osteoporosis.Their clinical efficacies were similar.However,the clinical tolerance of ibandronate was better.
A multicenter randomized, double-blind and double dim comparative studies on zoledronic acid and pamidronate disodium: Phase II trials from 12 centers for treating bone metastasis pain

Abstract:Objective: To evaluate the clinical effect and safety profiles of zoledronic acid in the treatment of bone metastasis pain in cancer patients. Methods: A multicenter, double-blind and double-dim clinical trial on pamidronate was conducted in 237 cancer patients with bone metastasis pain from 12 centers in China. The patients were randomized into 2 groups, i.e., the experiment group and control group. Zoledronic acid (4 mg) was injected for 15 min in the experiment group and pamidronate (90 mg) i.v. within 4 h in the controls. The daily change of bone ache recorded by a visual analog scale (VAS) and the side effects were observed within 4 weeks as well. Results: The total 219 of 239 cases were eligible for final analysis, 112 from experimental group and 107 from control group were involved in the clinical response evluation. For the best clinical response within 14 days, after injection with those two biphosphanates, the complete response rate (CR rate), partial response rate (PR rate ) and clinical benefit rate was 10.08%, 53.78% and 90.75% respectively in the experiment group, compared with those data of 5.08%, 54.24% and 83.90% in the control group. There was no significant difference between the 2 groups. The daily clinical improvements of pain reduction and/or relief in the experiment group during the 5th, 7th till 12th day were absolutely better than those from the controls.There was no significant difference between the 2 groups for the consideration of pain improvement with 28 days . The medium maintain time (MMT) of complete response within 14 days were 5.51 and 3.01 days respectively between the 2 groups (P=0.0302), with the contrast of the MMT of effective responses (CR+PR) were 9.02 and 8.02 days (P>0.05). The incidence rate of the main side effects, like fever, hypocalcaemia, and fatigue was 23.94% and 27.98% respectively in the experiment group and the controls. No severe side effect was found in both groups. Conclusion: Zoledronic acid can effectively relieve bone metastasis pain. The response rate and side effect were similar to those of pamidronate.

Abstract:Aim: Oral risedronate is effective in the treatment of postmenopausal osteoporosis when administered daily, weekly, or monthly. In this 1-year, randomized, double-blind, multicenter study we compared the weekly 35-mg and daily 5-mg risedronate dosing regimens in the treatment of Chinese postmenopausal women with osteoporosis or osteopenia. Methods: Postmenopausal women with primary osteoporosis or osteopenia were randomly assigned to the weekly group or daily group ( n =145 for each) that received oral risedronate 35 mg once a week or 5 mg daily, respectively, for 1 year. The subjects' bone mineral densities (BMDs), bone turnover markers (P1NP and β-CTX), new vertebral fractures, and adverse events were assessed at baseline and during the treatments. Results: All subjects in the weekly group and 144 subjects in the daily group completed the study. The primary efficacy endpoint after 1 year, ie the mean percent changes in the lumbar spine BMD (95% CI) were 4.87% (3.92% to 5.81%) for the weekly group and 4.35% (3.31% to 5.39%) for the daily group. The incidences of clinical adverse events were 48.3% in the weekly group and 54.2% in the daily group. Conclusion: The weekly 35-mg and daily 5-mg risedronate dosing regimens during 1 year of follow-up show similar efficacy in improving BMDs and biochemical markers of bone turnover in Chinese postmenopausal women with osteoporosis or osteopenia. Moreover, the two dosing regimens exhibit similar safety and tolerability.

Abstract:Osteoporosis is an insidious disease characterized by low bone mass and microarchitectural damage, [ 1 ] and postmenopausal women are a major risk group, with approximately 62% of postmenopausal women suffering from osteoporosis. [ 2 ] Fragility fractures due to osteoporosis have become a major public health threat owing to their significant morbidity, mortality, and healthcare burden. [ 3 ] Vertebral fractures are the most common type of fragility fractures. However, given that back pain, height shortening, and other symptoms can be dismissed as aging-related symptoms, the symptoms of vertebral fractures are often ignored. Furthermore, its low diagnosis and treatment rate greatly reduces the quality of life of patients and increases the risk of death. [ 4 ] Therefore, the prevention and control of osteoporosis in postmenopausal women are particularly important. The guidelines state that bone loss can be prevented through adequate calcium and vitamin D3 intake, smoking and alcohol cessation, and exercise therapy and recommend that medication should be administered for patients with reduced bone mass or low bone mass and a history of fragility fractures of the hip or spine. [ 5 ] Some scholars have suggested that menopause-related problems such as osteoporosis can be mitigated by delaying or prolonging the childbearing age of women. [ 6–8 ]

Abstract:Objective To investigate the clinical efficacy and compliance of oral administration or intravenous injection of bisphosphonates for the treatment of postmenopausal osteoporosis. Methods Fifty patients with postmenopausal osteoporosis were randomly divided into 2 groups: the Fosamax group and the Aclasta group. Patients in Fosamax group received alendronate sodium tablet once a week. Patients in Aclasta group received an intravenous drip of zoledronic acid once a year. Patients in both groups were given calcium and Vitamin D3 at the same time. Visual analogue score( VAS) and bone mineral density( BMD) were determined and compared before and after 3-week and 12-month treatment. Results The VAS after the treatment in both groups decreased significantly compared with that before the treatment. BMD increased significantly,and the difference was significant. Conclusion Both the oral administration and the intravenous injection of bisphosphonates are safe and effective. Whereas the annual intravenous drip of bisphosphonates can improve the compliance for the treatment of postmenopausal osteoporosis.

Abstract:INTRODUCTION:Osteoporosis is characterised by decreased bone mass and weakened bones, with an increased risk of fractures. Osteoporotic fracture, the most serious complication of osteoporosis, is related not only to lower bone mineral density (BMD), but also falls. Osteoporosis and fractures are associated with a decreased health-related quality of life (HRQL). Zoledronic acid (ZOL) is an intravenous once-yearly bisphosphonate that has been shown to be effective and safe in improving BMD and reducing fracture risk in controlled clinical trials.MATERIAL AND METHODS:In this self-controlled, prospective trial, 220 postmenopausal women with osteoporosis (mean age 67 years) received a single infusion of ZOL 5 mg at baseline and month 12. BMD, HRQL and Fall Index (FI) were measured at baseline, and months 12 and 24 (before each use of ZOL). The main outcome measures were the changes in lumbar spine and hip BMD and the changes in HRQL, the Short Form-36 questionnaire (SF-36). Additional comparisons were based on the FI. LSD multiple comparisons were used in the comparisons of BMD, SF-36 domain scores and FI.RESULTS:The patients had significantly higher L1-4, total hip, femoral neck and trochanter BMD (P < 0.05) with improved HRQL (P < 0.05) over two years of treatment of once-yearly ZOL 5mg. FI was reduced (P < 0.05) with oral daily elemental calcium and vitamin D in the treatment course.CONCLUSIONS:ZOL improves BMD and HRQL, especially in the physical aspects, over two years of treatment in women with postmenopausal osteoporosis, and can help improve balance ability.
