Abstract:Traumatic brain injury (TBI) initiates a complex cascade of neurochemical and signaling changes that leads to neuronal apoptosis, which contributes to poor outcomes for patients with TBI. Previous study indicates that calcium-sensing receptor (CaSR) activation contributes to neuron death in focal cerebral ischemia-reperfusion mice, however, its role in neuronal apoptosis after TBI is not well-established. Using a controlled cortical impact model in rats, the present study was designed to determine the effect of CaSR inhibitor NPS2390 upon neuronal apoptosis after TBI. Rats were randomly distributed into three groups undergoing the sham surgery or TBI procedure, and NPS2390 (1.5 mg/kg) was infused subcutaneously at 30 min and 120 min after TBI. All rats were sacrificed at 24 h after TBI. Our data indicated that NPS2390 significantly reduced the brain edema and improved the neurological function after TBI. In addition, NPS2390 decreased caspase-3 levels and the number of apoptotic neurons. Furthermore, NPS2390 up-regulated anti-apoptotic protein Bcl-2 expression and down-regulated pro-apoptotic protein Bax, and reduced subsequent release of cytochrome c into the cytosol. In summary, this study indicated that inhibition of CaSR by NPS2390 attenuates neuronal apoptosis after TBI, in part, through modulating intrinsic apoptotic pathway.

Abstract:Survival after severe traumatic brain injury (TBI) depends on minimizing or avoiding secondary insults to the brain. Overproduction of reactive oxygen species (ROS) and Ca2+ influx at the damaged site are the key factors that cause secondary injury upon TBI. Herein, a TBI-targeted lipid covered radical scavenger nanoparticle is developed to deliver nimodipine (Np) (CL-PPS/Np), in order to inhibit Ca2+ influx in neurons by Np and to scavenge ROS in the brain trauma microenvironment by poly(propylene sulfide)60 (PPS60 ) and thus prevent TBI-associated secondary injury. In post-TBI models, CL-PPS/Np effectively accumulates into the wound cavity and prolongs the time of systemic circulation of Np. CL-PPS/Np can markedly protect the integrity of blood-brain barrier, prevent brain edema, reduce cell death and inflammatory responses, and promote functional recovery after TBI. These findings may provide a new therapy for TBI to prevent the spread of the secondary injury.

Abstract:Vascular recanalization intervention is the primary recommended approach for the treatment of ischemic stroke. However, the damage caused by cerebral ischemia-reperfusion (I/R) poses a significant challenge for effective treatment. The aim of this study was to address this challenge by developing multifunctional nanoparticles (NPs). Cu4.6O NP was encapsulated by zein-Se-Se-DHA (Dz), the diselenide bond-conjugated zein and docosahexaenoic acid (DHA), and then coated by platelet membrane (PLTM) to form Cu4.6O@Dz@PLTM (MDzCu) NP. MDzCu with the PLTM coating showed strong targeting ability to the cerebral ischemic lesions in the rat model of cerebral I/R injury. Furthermore, in the reactive oxygen species (ROS)-rich brain microenvironment, both Cu4.6O and DHA are released from MDzCu, exerting ROS scavenging, promoting microglia polarization, and providing neuroprotective effects, ultimately alleviating cerebral I/R injury. This study presents several novel findings, such as the use of Cu0/Cu+ NPs for brain-related diseases, ROS scavenger-assisted DHA therapy for the central nervous system (CNS), and the application of stimuli-responsive NPs with diselenides to treat I/R injuries. This study provides insights into the development of stimuli-responsive nanomedicines with cell membrane coatings for the targeted treatment of brain diseases.

Abstract:Bioactive multifunctional nanocomposites (ANG‐MnEMNPs‐Cur, AMEC) mediated antioxidation and anti‐neuroinflammation are developed intrinsically for targeted traumatic brain injury (TBI) theranostics. AMEC effectively protects blood–brain barrier, relieves brain edema, reduces brain tissue loss, and improves the cognition of TBI mice. More notably, T1–T2 dual‐modal magnetic resonance and photoacoustic imaging allow tracking AMEC delivery into the TBI lesion in vivo. Overproduced reactive oxygen species and the induced oxidative stress and neuroinflammation often result in secondary injury, which is associated with unfavorable prognosis in traumatic brain injury (TBI). Unfortunately, current medications cannot effectively ameliorate the secondary injury at traumatic sites. Here, it is reported that intrinsically bioactive multifunctional nanocomposites (ANG‐MnEMNPs‐Cur, AMEC) mediate antioxidation and anti‐neuroinflammation for targeted TBI theranostics, which are engineered by loading the neuroprotective agent curcumin on angiopep‐2 functionalized and manganese doped eumelanin‐like nanoparticles. After intravenous delivery, efficient AMEC accumulation is observed in lesions of TBI mice models established by controlled cortical impact method, evidenced by T1–T2 magnetic resonance and photoacoustic dual‐modal imaging. Therapeutically, AMEC effectively alleviates neuroinflammation, protects blood–brain barrier integrity, relieves brain edema, reduces brain tissue loss, and improves the cognition of TBI mice. Mechanistically, following the penetration into the traumatic tissues via angiopep‐2 mediated targeting effect, the efficacy of AMEC is synergistically improved by combined functional moieties of curcumin and eumelanin. This is achieved by the alleviation of oxidative stress, inhibition of neuroinflammation via M1‐to‐M2 macrophage reprogramming, and promotion of neuronal regeneration. The as‐developed AMEC with well‐defined mechanisms of action may represent a promising targeted theranostics strategy for TBI and other neuroinflammation‐associated intracranial diseases.

Abstract:Ultrastructural alterations occurring in human salivary glands as a result of a variety of diseases are described. Major changes in these organs in cases of cystic fibrosis are probably the result of duct blockage, as indicated by study of chronically inflamed salivary glands. A new disease of salivary glands is reported in which parotid serous granules are distorted by bundles of fine filaments, which may be due to the untoward polymerization of a secretory protein, perhaps proline-rich protein. Finally, a clear-cell adenocarcinoma of palatine gland origin is described in which the nuclei display complex pseudoinclusions and the cytoplasm contains numerous herpes-type viruses.

Abstract:Posttraumatic neuroinflammation is a key driver of secondary injury after traumatic brain injury (TBI). Pyroptosis, a proinflammatory form of programmed cell death, considerably activates strong neuroinflammation and amplifies the inflammatory response by releasing inflammatory contents. Therefore, treatments targeting pyroptosis may have beneficial effects on the treatment of secondary brain damage after TBI. Here, a cysteine-alanine-glutamine-lysine peptide–modified β-lactoglobulin (β-LG) nanoparticle was constructed to deliver disulfiram (DSF), C-β-LG/DSF, to inhibit pyroptosis and decrease neuroinflammation, thereby preventing TBI-induced secondary injury. In the post-TBI mice model, C-β-LG/DSF selectively targets the injured brain, increases DSF accumulation, and extends the time of the systemic circulation of DSF. C-β-LG/DSF can alleviate brain edema and inflammatory response, inhibit secondary brain injury, promote learning, and improve memory recovery in mice after trauma. Therefore, this study likely provided a potential approach for reducing the secondary spread of TBI.

Abstract:A hesperetin nanoparticle that targets neutrophils, T‐Hes, catalyzed by MPO secreted by neutrophil is rationally designed. The T‐Hes can effectively identify inflammatory neutrophils and quickly gather in the injured area after TBI. The sustained release of hesperetin by T‐Hes shows great advantages in anti‐inflammatory treatment, reducing brain edema, and improving neurological function. Traumatic brain injury (TBI) is the main cause of death and disability in people of all ages worldwide. Neuroinflammation plays beneficial and harmful roles in secondary brain injury. Neutrophils play an important role in chemically mediated inflammatory responses through myeloperoxidase (MPO) and inflammation triggered by TBI. Herein, a nanodrug targeting neutrophils and MPO through chemical and biological functions after TBI is designed to enhance the retention and sustained release of drug cargos for improved TBI therapy. 5‐Hydroxytryptamide (5‐HT) is modified on nanoparticles (NPs) loaded with an anti‐inflammatory and antioxidant natural product, hesperetin, to obtain MPO‐ and neutrophil‐targeting NPs, denoted as T‐Hes. In a mouse TBI model, it is confirmed that neutrophil‐targeting NPs can quickly accumulate and remain in the brain tissue, reduce the secretion of inflammatory factors, and the level of microglia and astrocytes, subsequently inducing the transformation of microglia from pro‐inflammatory M1 to anti‐inflammatory M2 cells and promoting the infiltration of regulatory T cells (Tregs). T‐Hes significantly inhibits neuroinflammation and improves neurological deficits through the sustained release of hesperetin in the brain. The findings may open up new avenues for designing clinically translatable probes for TBI treatment.

Abstract:Traumatic brain injury (TBI) often results in damage to the blood–brain barrier (BBB), leading to secondary effects such as neuroinflammation mediated by reactive oxygen species (ROS), which can increase the risk of long-term neurological dysfunction. Therefore, it is crucial to remove excessive ROS and monitor the status of BBB for timely clinical intervention. Here, we developed a fluorescent nanozyme by doping Mn atoms into PbS/CdS quantum dots (QDs) that emit in the near-infrared-II (NIR-IIb, 1500–1700 nm) region, enabling imaging-guided treatment of TBI. The QD-based nanozyme exhibited excellent antioxidant activity due to uniformly distributed Mn atoms on the surface, which effectively scavenged ROS and inhibited the release of pro-inflammatory factors, thus providing neuroprotective effects for TBI treatment. Additionally, the nanozyme demonstrated exceptional NIR-IIb fluorescence performance, allowing for dynamic monitoring of BBB status with high spatiotemporal resolution to guide precise TBI treatment.

Abstract:Traumatic brain injury (TBI) triggers both central and peripheral inflammatory responses. Existing pharmacological drugs are unable to effectively and quickly target the brain inflamed regions, setting up a major roadblock towards effective brain trauma treatments. Nanoparticles (NPs) have been used in multiple diseases as drug delivery tools with remarkable success due to their rapid diffusion and specificity in the target organ. Here, leukocyte-based biomimetic NPs are fabricated as a theranostic tool to directly access inflamed regions in a TBI mouse model. This NP systemic delivery is visualized using advanced in vivo imaging techniques, including intravital microscopy and in vivo imaging system. The results demonstrate selective targeting of NPs to the injured brain and increased NPs accumulation among the peripheral organs 24 h after TBI. Interestingly, increased microglial proliferation, decreased macrophage infiltration, and reduced brain lesion following the NPs treatments compared to sham vehicle-treated mice are also found. In summary, the results suggest that NPs represent a promising future theranostic tool for TBI treatment.

Abstract:Abstract Following a traumatic brain injury (TBI), excess reactive oxygen species (ROS) and lipid peroxidation products (LPOx) are generated and lead to secondary injury beyond the primary insult. A major limitation of current treatments is poor target engagement, which has prevented success in clinical trials. Thus, nanoparticle‐based treatments have received recent attention because of their ability to increase accumulation and retention in damaged brain. Theranostic neuroprotective copolymers (NPC3) containing thiol functional groups can neutralize ROS and LPOx. Immediate administration of NPC3 following injury in a controlled cortical impact (CCI) mouse model provides a therapeutic window in reducing ROS levels at 2.08–20.83 mg kg −1 in males and 5.52–27.62 mg kg −1 in females. This NPC3‐mediated reduction in oxidative stress improves spatial learning and memory in males, while females show minimal improvement. Notably, NPC3‐mediated reduction in oxidative stress prevents the bilateral spread of necrosis in male mice, which is not observed in female mice and likely accounts for the sex‐based spatial learning and memory differences. Overall, these findings suggest sex‐based differences to oxidative stress scavenger nanoparticle treatments, and a possible upper threshold of antioxidant activity that provides therapeutic benefit in injured brain since female mice benefit from NPC3 treatment to a lesser extent than male mice.

Abstract:Following spinal cord injury (SCI), immune cell infiltration creates an inflammatory and oxidative microenvironment (known as the secondary injury), which causes neuron death and spinal cord damage, and dramatically hinders neurological functional recovery. Strategies that inhibit the infiltration and/or function of neutrophils offer promises for SCI treatment because they can reduce the secondary injury; however, such strategies remain largely unexplored. Herein, a strategy using neutrophil membrane-coated nanoparticles (NPs) as decoys (neutrophil decoy, ND) is presented to reduce local neutrophil infiltration and relieve oxidative stress in the injured spinal cord after SCI. Coated with membranes of activated neutrophils, the NDs inherit multiple receptors from the "parent" neutrophils, which can adsorb and neutralize the elevated neutrophil-related cytokines. In addition, polydopamine NPs with multi-antioxidative properties (selected as the core for ND) scavenge excessive reactive oxygen and nitrogen species. In a contusion model of SCI, ND treatment significantly reduces neutrophil infiltration and reprograms the inflammatory and oxidative microenvironment in injured spinal cords. Importantly, ND treatment significantly improves neural regeneration and functional recovery in rats. Such a nano-decoy platform opens up new approaches for efficiently treating SCI.

Abstract:Traumatic brain injury (TBI) is a major source of death and disability worldwide as a result of motor vehicle accidents, falls, attacks and bomb explosions. Currently, there are no FDA-approved drugs to treat TBI patients predominantly because of a lack of appropriate methods to deliver drugs to the brain for therapeutic effect. Existing clinical and pre-clinical studies have shown that minocycline's neuroprotective effects either through high plasma protein binding or an increased dosage requirement have resulted in neurotoxicity. In this study, we focus on the formulation, characterization, in vivo biodistribution, behavioral improvements, neuroprotective effect and toxicity of transferrin receptor-targeted (tf) conjugated minocycline loaded albumin nanoparticles in a blast-induced TBI model. A novel tf conjugated minocycline encapsulated albumin nanoparticle was developed, characterized and quantified using a validated HPLC method as well as other various analytical methods. The results of the nanoformulation showed small, narrow hydrodynamic size distributions, with high entrapment, loading efficiencies and sustained release profiles. Furthermore, the nanoparticle administered at minimal doses in a rat model of blast TBI was able to cross the blood-brain barrier, enhanced nanoparticle accumulation in the brain, improved behavioral outcomes, neuroprotection, and reduced toxicity compared to free minocycline. Hence, tf conjugated minocycline loaded nanoparticle elicits a neuroprotective effect and can thus offer a potential therapeutic effect.

Abstract:Spinal cord injury (SCI) is one of the most complex diseases. After SCI, severe secondary injuries can cause intense inflammatory storms and oxidative stress responses, leading to extensive neuronal apoptosis. Effective regulation of inflammation and oxidative stress after SCI remains an unresolved challenge. In this study, resveratrol-loaded nanoparticles coated with neutrophil membranes (NMR) were prepared using the emulsion-solvent evaporation method and membrane encapsulation technology. Multifunctional biomimetic nanoparticles retain neutrophil membrane-related receptors and possess a strong adsorption capacity for inflammatory factors. As a drug carrier, NMR can sustainably release resveratrol for >72 h. Moreover, co-culture studies in vitro show that the NMR help regulate macrophage polarization to relieve inflammatory response, reduce intracellular reactive oxygen species by approximately 50%, and improve mitochondrial membrane potential to alleviate oxidative stress. After injecting NMR into the injury site, it reduces early apoptosis, inhibit scar formation, and promote neural network recovery to improve motor function. This study demonstrates the anti-inflammatory, antioxidant, and neuroprotective effects of NMR, thus providing a novel therapeutic strategy for SCI.

Abstract:Early brain injury (EBI) refers to the immediate injury and neuroinflammation in the brain after subarachnoid hemorrhage (SAH). The current drugs used to treat SAH are inadequate due to their disappointing ability to penetrate the blood–brain barrier (BBB) and their limited anti-inflammatory effects, leading to unsatisfactory therapeutic outcomes for EBI. Herein, we developed a biomimetic nanozyme (Fe-DMY) using a metal-polyphenol self-assembly method, involving the coordination of ferric ion (Fe 3+ ) and dihydromyricetin (DMY), then encapsulated Fe-DMY in neutrophil membranes (NM) to form NM@Fe-DMY, which targets to neuroinflammatory regions. NM@Fe-DMY exhibits strong catalase- and superoxide dismutase-like catalytic activities, suggesting its potential to reduce oxidative stress in SAH. This leads to reduced lipid peroxidation, increased expression of glutathione peroxidase 4 and prevented ferroptosis. Furthermore, The NM@Fe-DMY has demonstrated effective penetration of the BBB in vivo, accumulating in SAH injury region and exhibiting significant therapeutic efficacy including neural function recovery and improvement of spatial memory. Importantly, NM@Fe-DMY also induces the polarization of microglia from M1-like to M2-like subtypes, interrupting the detrimental cycle of neuroinflammation in EBI. This study introduces a new NM-wrapped biomimetic nanozyme strategy for effectively targeting neuroinflammatory regions and enhancing the treatment of EBI following SAH.

Abstract:Secondary injury has a tremendous impact on the recovery of motor function after traumatic spinal cord injury (SCI). The microenvironmental reactive oxygen species (ROS) and inflammatory bursts in the spinal cord tissues cause secondary injury that can further lead to neuronal apoptosis, which in turn exacerbates the loss of mobility and sensory function. Due to the presence of the blood–spinal cord barrier (BSCB), current clinical medicine is unable to effectively improve the microenvironment after SCI at a safe dose. In this experiment, macrophage membrane-modified ROS-responsive self-assembled methylprednisolone-S-docosahexaenoic acid prodrug nanoparticles (MΦ@MP-S-DHA) were designed to achieve targeted therapy across the BSCB. The monosulfide bond (-S-) responds to ROS in situ and attenuates ROS levels while releasing methylprednisolone (MP) and docosahexaenoic acid (DHA), which exert anti-inflammatory and neuroprotective effects. In vitro experiments have shown that MΦ@MP-S-DHA is released more rapidly by ROS. Cellular and mouse experiments further illustrated that MΦ@MP-S-DHA reduced ROS, malondialdehyde (MDA) and superoxide anion levels and maintained superoxide dismutase (SOD) activity after SCI. Moreover, MΦ@MP-S-DHA reduced the expression of inflammatory factors (TNF-α, IL-1β and IL-6) and the levels of apoptotic factors (caspase-3 and cleaved caspase-3) after SCI. On the 28th day, the SCI mice treated with MΦ@MP-S-DHA achieved a BMS score of 6 and better recovery of hindlimb movement. Therefore, MΦ@MP-S-DHA provides a potential therapeutic strategy to effectively exert multifaceted improvements in the microenvironment after SCI.

Intravenous Immunomodulatory Nanoparticle Treatment for Traumatic Brain Injury

Abstract:Objective: There are currently no definitive disease-modifying therapies for traumatic brain injury (TBI). In this study, we present a strong therapeutic candidate for TBI, immunomodulatory nanoparticles (IMPs), which ablate a specific subset of hematogenous monocytes (hMos). We hypothesized that prevention of infiltration of these cells into brain acutely after TBI would attenuate secondary damage and preserve anatomic and neurologic function. Methods: IMPs, composed of US Food and Drug Administration-approved 500nm carboxylated-poly(lactic-co-glycolic) acid, were infused intravenously into wild-type C57BL/6 mice following 2 different models of experimental TBI, controlled cortical impact (CCI), and closed head injury (CHI). Results: IMP administration resulted in remarkable preservation of both tissue and neurological function in both CCI and CHI TBI models in mice. After acute treatment, there was a reduction in the number of immune cells infiltrating into the brain, mitigation of the inflammatory status of the infiltrating cells, improved electrophysiologic visual function, improved long-term motor behavior, reduced edema formation as assessed by magnetic resonance imaging, and reduced lesion volumes on anatomic examination. Interpretation: Our findings suggest that IMPs are a clinically translatable acute intervention for TBI with a well-defined mechanism of action and beneficial anatomic and physiologic preservation and recovery. Abstract:Traumatic brain injury (TBI) is one of the most common causes of disability and mortality worldwide, but due to the heterogeneity and the complexity of the pathological mechanisms that ensue, there are currently no effective disease‐modifying treatments to prevent TBI secondary injuries. Herein, the current nanotechnological innovations that can potentially be useful to improve TBI recovery are discussed in this review. Traumatic brain injury (TBI) is one of the most common causes of disability and mortality worldwide, creating a large socioeconomic burden annually. Secondary injury physiopathology is known to play a prominent role in exacerbating neurodegeneration post‐TBI and is potentially preventable by therapies. However, due to the heterogeneity of TBI and the complexity of the pathological mechanisms that ensue, there are currently no effective disease‐modifying treatments to prevent TBI‐associated disability and mortality. Nanotechnology has emerged in recent decades as a promising platform for the development of multifunctional neuroprotective agents for TBI. Herein, current multifunctional innovations are explored in this review in nanotechnology, which target the secondary injury pathological mechanisms of TBI and show promise in improving future post‐TBI management. Also, potential new directions for the future development of TBI treatment are discussed.

Abstract:BACKGROUND The ideal endpoint of resuscitation after severe hemorrhage should indicate not only that optimal oxygen delivery has been achieved, but also that oxygen utilization has been restored. A modified Foley catheter for simultaneous assessment of microcirculatory blood flow (TBF) and mitochondrial NADH in the urethral wall was used in the female swine. We hypothesized that changes in mitochondrial NADH and TBF are associated with impaired energy metabolism in the urethra and that these changes correlate with impaired tissue perfusion in the bladder during shock and resuscitation. MATERIAL/METHODS Female swine n=5 underwent laparotomy. TBF was measured by a laser Doppler flowmeter. Mitochondrial function was evaluated by measuring NADH fluorescence in vivo. Multiparameter sensors (pH, pCO2 and pO2) were placed in the bladder mucosa (BM), and in the skeletal muscle (Sk). Animals underwent hemorrhage and their MAP was maintained at 40 mm Hg by appropriate infusing or withdrawing of blood for 10 min. Animals were resuscitated and observed for 20 min. RESULTS Urethral NADH increased during shock and recovered during resuscitation, while TBF showed an opposite effect (r(2)=0.74). Skeletal muscle and bladder pO2 decreased during shock (p<0.01) and recovered after resuscitation. NADH increased significantly (p<0.05) during shock and decreased after resuscitation. CONCLUSIONS Changes in TBF and NADH in the urethral mucosa represent novel markers for the energetic state of the tissue. They could be measured in vivo by a minimally invasive approach and thus could provide important information on the end-points of resuscitation in hemorrhagic shock.
