Abstract:Abstract Background Augmented Reality (AR) is a rapidly emerging technology finding growing acceptance and application in different fields of surgery. Various studies have been performed evaluating the precision and accuracy of AR guided navigation. This study investigates the feasibility of a commercially available AR head mounted device during orthopedic surgery. Methods Thirteen orthopedic surgeons from a Swiss university clinic performed 25 orthopedic surgical procedures wearing a holographic AR headset (HoloLens, Microsoft, Redmond, WA, USA) providing complementary three-dimensional, patient specific anatomic information. The surgeon’s experience of using the device during surgery was recorded using a standardized 58-item questionnaire grading different aspects on a 100-point scale with anchor statements. Results Surgeons were generally satisfied with image quality (85 ± 17 points) and accuracy of the virtual objects (84 ± 19 point). Wearing the AR device was rated as fairly comfortable (79 ± 13 points). Functionality of voice commands (68 ± 20 points) and gestures (66 ± 20 points) provided less favorable results. The greatest potential in the use of the AR device was found for surgical correction of deformities (87 ± 15 points). Overall, surgeons were satisfied with the application of this novel technology (78 ± 20 points) and future access to it was demanded (75 ± 22 points). Conclusion AR is a rapidly evolving technology with large potential in different surgical settings, offering the opportunity to provide a compact, low cost alternative requiring a minimum of infrastructure compared to conventional navigation systems. While surgeons where generally satisfied with image quality of the here tested head mounted AR device, some technical and ergonomic shortcomings were pointed out. This study serves as a proof of concept for the use of an AR head mounted device in a real-world sterile setting in orthopedic surgery.

Abstract:Augmented reality (AR) technology has witnessed remarkable advancements in recent years, revolutionizing various fields, including medicine and surgery. In neurosurgery, AR holds immense promise for improving the accuracy, efficiency, and safety of various procedures. Augmented reality allows a user to visualize digital information such as 3D models, superimposed on their real-world field of view. Lately, there has been increased use of this technology for various procedures such as tumor resection, ventriculostomy, and pedicle screw insertion. Despite this, integration of AR into the field of neurosurgery is still in its infancy. As such, it is imperative that physicians continue to explore and document new clinical uses of AR. In this report, we describe the novel integration of AR into an endoscopic third ventriculostomy (ETV) case. ETV is a minimally invasive technique used to treat hydrocephalus, which involves creating a new pathway for cerebrospinal fluid (CSF) drainage within the brain's ventricular system. The integration of AR into ETV procedures offers unprecedented opportunities to enhance surgical visualization, navigation, and decision-making, ultimately leading to improved patient outcomes. Traditionally, neurosurgeons rely on pre-operative imaging, intraoperative neuronavigation systems, and their anatomical knowledge to perform an ETV. However, the complex neuroanatomy and variability among patients pose challenges to accurate navigation and spatial orientation prior to and during surgery. AR technology addresses these challenges by overlaying digital information—such as three-dimensional models, anatomical landmarks, and surgical trajectories—onto the surgeon's view of the patient in real-time.

Abstract:OBJECTIVE The placement of a ventricular catheter, that is, an external ventricular drain (EVD), is a common and essential neurosurgical procedure. In addition, it is one of the first procedures performed by inexperienced neurosurgeons. With or without surgical experience, the placement of an EVD according to anatomical landmarks only can be difficult, with the potential risk for inaccurate catheter placement. Repeated corrections can lead to avoidable complications. The use of mixed reality could be a helpful guide and improve the accuracy of drain placement, especially in patients with acute pathology leading to the displacement of anatomical structures. Using a human cadaveric model in this feasibility study, the authors aimed to evaluate the accuracy of EVD placement by comparing two techniques: mixed reality and freehand placement. METHODS Twenty medical students performed the EVD placement procedure with a Cushing’s ventricular cannula on the right and left sides of the ventricular system. The cannula was placed according to landmarks on one side and with the assistance of mixed reality (Microsoft HoloLens 2) on the other side. With mixed reality, a planned trajectory was displayed in the field of view that guides the placement of the cannula. Subsequently, the actual position of the cannula was assessed with the help of a CT scan with a 1-mm slice thickness. The bony structure as well as the left and right cannula positions were registered to the CT scan with the planned target point before the placement procedure. CloudCompare software was applied for registration and evaluation of accuracy. RESULTS EVD placement using mixed reality was easily performed by all medical students. The predefined target point (inside the lateral ventricle) was reached with both techniques. However, the scattering radius of the target point reached through the use of mixed reality (12 mm) was reduced by more than 54% compared with the puncture without mixed reality (26 mm), which represents a doubling of the puncture accuracy. CONCLUSIONS This feasibility study specifically showed that the integration and use of mixed reality helps to achieve more than double the accuracy in the placement of ventricular catheters. Because of the easy availability of these new tools and their intuitive handling, we see great potential for mixed reality to improve accuracy.

Abstract:The immunological response of the human host to xenograft bone after combined xenografts of Kiel bone and autologous marrow has been studied by serological methods in twenty-eight patients and by the leucocytic reaction in the peripheral blood in fifteen. The clinical findings in these patients have already been reported. The results suggest that Kiel bone is very weakly antigenic in the human host. These results are discussed in the light of previous experimental findings and a possible explanation put forward. The advantages of osteogenesis by autologous marrow cells are also discussed.

Abstract:Augmented reality (AR) is emerging as a key technology in neurosurgery. Projecting three-dimensional (3D) anatomic models onto the surgical field provides unique operative information to make procedures safer and more efficient. A small footprint, rapid registration AR system was used for bedside guidance during aspiration of a subdural hygroma. A 77-year-old male presented for resection of a suprasellar tumor and subsequently developed a large bilateral subdural hygroma. We performed the aspiration of the hygroma at the bedside using AR guidance. The AR system allowed for precise needle placement during the aspiration. The aim of this report was to demonstrate the clinical feasibility of integrating a novel AR system into the clinical workflow of a bedside procedure. As AR continues to expand in the field, the benefits of this technology for various procedures will become more evident.

871 Bedside Neuronavigation for External Ventriculostomy as a Training Tool for Success: A Single-institution Experience and Meta-Analysis of Patients Outcomes and Procedural Accuracy

Abstract:INTRODUCTION: External ventricular drain (EVD) placement is the most universally performed neurosurgical procedure, frequently by trainees. Freehand insertion, which approximates anatomical landmarks to achieve percutaneous ventricular access, bodes a complication risk as high as 40%. Neuronavigation is widely used for neurosurgical procedures; however, its use case for EVD has been inadequately explored. METHODS: A single-institution retrospective cohort study was performed, including all EVDs inserted in 2022. A PRISMA-guided systematic review was then conducted, querying databases for relevant articles. Primary outcomes of interest were optimal positioning (using Kakarla grading) and number of passes for ventricular access. Secondary outcomes included postoperative ventricular enlargement, hemorrhage, edema, and drain malfunction. Clinical data was pooled between single-institution and systematic review for meta-analyses. Outcomes were compared using risk ratios (RR [95% CI]) and standardized mean differences (SMD). RESULTS: Forty-seven patients (49 EVDs) were included from a single institution (17 neuronavigation; 32 freehand). Through systematic review, 1,289 patients (255 neuronavigation; 1,034 freehand) were identified from 10 eligible studies. Pooled together, patients were predominantly male (52%) and adult-aged (90%). EVD indications most commonly included hemorrhage (61%), obstructive pathology (15%), trauma (10%), and infection (7%). 85% of EVDs were placed by neurosurgical trainees. Frequency of catheter malposition was significantly lower in the neuronavigation cohort compared to the freehand cohort (10% vs. 36%; RR, 0.41 [0.24-0.69]). However, number of catheter passes remained the same between cohorts (mean [SD]; 1.11 [0.42] vs. 1.16 [0.51]; SMD; 0.04 [0.48]). Moreover, frequencies of postoperative ventricular enlargement, hemorrhage, edema, and drain malfunction, although were higher in the freehand cohort, did not differ significantly. CONCLUSIONS: These findings suggest neuronavigation helps trainees achieve safe and accurate ventricular access. Neuronavigation-guided external ventriculostomy may offer a promising, accessible, and bedside-ready approach to minimize morbidity, while mastering the procedural technique.

Abstract:OBJECTIVE The traditional freehand placement of an external ventricular drain (EVD) relies on empirical craniometric landmarks to guide the craniostomy and subsequent passage of the EVD catheter. The diameter and trajectory of the craniostomy physically limit the possible trajectories that can be achieved during the passage of the catheter. In this study, the authors implemented a mixed reality–guided craniostomy procedure to evaluate the benefit of an optimally drilled craniostomy to the accurate placement of the catheter. METHODS Optical marker–based tracking using an OptiTrack system was used to register the brain ventricular hologram and drilling guidance for craniostomy using a HoloLens 2 mixed reality headset. A patient-specific 3D-printed skull phantom embedded with intracranial camera sensors was developed to automatically calculate the EVD accuracy for evaluation. User trials consisted of one blind and one mixed reality–assisted craniostomy followed by a routine, unguided EVD catheter placement for each of two different drill bit sizes. RESULTS A total of 49 participants were included in the study (mean age 23.4 years, 59.2% female). The mean distance from the catheter target improved from 18.6 ± 12.5 mm to 12.7 ± 11.3 mm (p = 0.0008) using mixed reality guidance for trials with a large drill bit and from 19.3 ± 12.7 mm to 10.1 ± 8.4 mm with a small drill bit (p < 0.0001). Accuracy using mixed reality was improved using a smaller diameter drill bit compared with a larger bit (p = 0.039). Overall, the majority of the participants were positive about the helpfulness of mixed reality guidance and the overall mixed reality experience. CONCLUSIONS Appropriate indications and use cases for the application of mixed reality guidance to neurosurgical procedures remain an area of active inquiry. While prior studies have demonstrated the benefit of mixed reality–guided catheter placement using predrilled craniostomies, the authors demonstrate that real-time quantitative and visual feedback of a mixed reality–guided craniostomy procedure can independently improve procedural accuracy and represents an important tool for trainee education and eventual clinical implementation.

Abstract:Objectives: Intraoperative navigation reduces the risk of major complications and increases the likelihood of optimal surgical outcomes. This paper presents an augmented reality (AR)-based simulation technique for ventriculostomy that visualizes brain deformations caused by the movements of a surgical instrument in a three-dimensional brain model. This is achieved by utilizing a position-based dynamics (PBD) physical deformation method on a preoperative brain image. Methods: An infrared camera-based AR surgical environment aligns the real-world space with a virtual space and tracks the surgical instruments. For a realistic representation and reduced simulation computation load, a hybrid geometric model is employed, which combines a high-resolution mesh model and a multiresolution tetrahedron model. Collision handling is executed when a collision between the brain and surgical instrument is detected. Constraints are used to preserve the properties of the soft body and ensure stable deformation. Results: The experiment was conducted once in a phantom environment and once in an actual surgical environment. The tasks of inserting the surgical instrument into the ventricle using only the navigation information presented through the smart glasses and verifying the drainage of cerebrospinal fluid were evaluated. These tasks were successfully completed, as indicated by the drainage, and the deformation simulation speed averaged 18.78 fps. Conclusions: This experiment confirmed that the AR-based method for external ventricular drain surgery was beneficial to clinicians.

Abstract:OBJECTIVE A major obstacle to improving bedside neurosurgical procedure safety and accuracy with image guidance technologies is the lack of a rapidly deployable, real-time registration and tracking system for a moving patient. This deficiency explains the persistence of freehand placement of external ventricular drains, which has an inherent risk of inaccurate positioning, multiple passes, tract hemorrhage, and injury to adjacent brain parenchyma. Here, the authors introduce and validate a novel image registration and real-time tracking system for frameless stereotactic neuronavigation and catheter placement in the nonimmobilized patient. METHODS Computer vision technology was used to develop an algorithm that performed near-continuous, automatic, and marker-less image registration. The program fuses a subject’s preprocedure CT scans to live 3D camera images (Snap-Surface), and patient movement is incorporated by artificial intelligence–driven recalibration (Real-Track). The surface registration error (SRE) and target registration error (TRE) were calculated for 5 cadaveric heads that underwent serial movements (fast and slow velocity roll, pitch, and yaw motions) and several test conditions, such as surgical draping with limited anatomical exposure and differential subject lighting. Six catheters were placed in each cadaveric head (30 total placements) with a simulated sterile technique. Postprocedure CT scans allowed comparison of planned and actual catheter positions for user error calculation. RESULTS Registration was successful for all 5 cadaveric specimens, with an overall mean (± standard deviation) SRE of 0.429 ± 0.108 mm for the catheter placements. Accuracy of TRE was maintained under 1.2 mm throughout specimen movements of low and high velocities of roll, pitch, and yaw, with the slowest recalibration time of 0.23 seconds. There were no statistically significant differences in SRE when the specimens were draped or fully undraped (p = 0.336). Performing registration in a bright versus a dimly lit environment had no statistically significant effect on SRE (p = 0.742 and 0.859, respectively). For the catheter placements, mean TRE was 0.862 ± 0.322 mm and mean user error (difference between target and actual catheter tip) was 1.674 ± 1.195 mm. CONCLUSIONS This computer vision–based registration system provided real-time tracking of cadaveric heads with a recalibration time of less than one-quarter of a second with submillimetric accuracy and enabled catheter placements with millimetric accuracy. Using this approach to guide bedside ventriculostomy could reduce complications, improve safety, and be extrapolated to other frameless stereotactic applications in awake, nonimmobilized patients.

Abstract:Augmented Reality (AR) has been used to facilitate surgical guidance during External Ventricular Drain (EVD) surgery, reducing the risks of misplacement in manual operations. During this procedure, the key challenge is accurately estimating the spatial relationship between pre-operative images and actual patient anatomy in AR environment. This research proposes a novel framework utilizing Time of Flight (ToF) depth sensors integrated in commercially available AR Head Mounted Devices (HMD) for precise EVD surgical guidance. As previous studies have proven depth errors for ToF sensors, we first assessed their properties on AR-HMDs. Subsequently, a depth error model and patient-specific parameter identification method are introduced for accurate surface information. A tracking pipeline combining retro-reflective markers and point clouds is then proposed for accurate head tracking. The head surface is reconstructed using depth data for spatial registration, avoiding fixing tracking targets rigidly on the patient's skull. Firstly, $7.580\pm 1.488 mm$ depth value error was revealed on human skin, indicating the significance of depth correction. Our results showed that the error was reduced by over $85\%$ using proposed depth correction method on head phantoms in different materials. Meanwhile, the head surface reconstructed with corrected depth data achieved sub-millimetre accuracy. An experiment on sheep head revealed $0.79 mm$ reconstruction error. Furthermore, a user study was conducted for the performance in simulated EVD surgery, where five surgeons performed nine k-wire injections on a head phantom with virtual guidance. Results of this study revealed $2.09 \pm 0.16 mm$ translational accuracy and $2.97\pm 0.91$ degree orientational accuracy.

Virtual and Augmented Reality in Ventriculostomy: A Systematic Review

Abstract:Background: Ventriculostomy, one of the most common neurosurgical procedures, involves inserting a draining catheter into the brain's ventricular system to alleviate excessive cerebrospinal fluid accumulation. Traditionally, this procedure has relied on freehand techniques guided by anatomical landmarks, which have shown a high rate of misplacement. Recent advancements in virtual reality (VR) and augmented reality (AR) technologies have opened up new possibilities in the field. This comprehensive review aims to analyze the existing literature, examine the diverse applications of VR and AR in ventriculostomy procedures, address their limitations, and propose potential future directions. Methods: A systematic search was conducted in Web of Science and PubMed databases to identify studies employing VR and AR technologies in ventriculostomy procedures. Review papers, non-English records, studies unrelated to VR/AR technologies in ventriculostomy, and supplementary documents were excluded. In total 29 papers were included in the review. Results: The development of various VR and AR systems aimed at enhancing the ventriculostomy procedure are categorized according to the Data, Visualization and View taxonomy. The study investigates the data utilized by these systems, the visualizations employed, and the virtual or augmented environments created. Furthermore, the surgical scenarios and applications of each method, as well as the validation and evaluation metrics used, are discussed. Discussion: The review delves into the fundamental challenges encountered in the implementation of VR and AR systems in ventriculostomy. Additionally, potential future directions and areas for improvement are proposed, addressing the identified limitations and paving the way for further advancements in the field.

Abstract:The field of augmented reality (AR) has undergone substantial growth, finding diverse applications in the medical industry. This paper delves into various techniques employed in medical surgeries, scrutinizing factors such as cost, implementation, and accessibility. The focus of this exploration is on AR-based solutions, with a particular emphasis on addressing challenges and proposing an innovative solution for ventriculoperitoneal shunt (VP) operations. The proposed solution introduces a novel flow in the pre-surgery phase, aiming to substantially reduce setup time and operation duration by creating 3D models of the skull and ventricles. Experiments are conducted where the models are visualized on a 3D- printed skull through an AR device, specifically the Microsoft HoloLens 2. The paper then conducts an in-depth analysis of this proposed solution, discussing its feasibility, advantages, limitations,and future implications.

Abstract:Most current surgical navigation methods rely on optical navigators with images displayed on an external screen. However, minimizing distractions during surgery is critical and the spatial information displayed in this arrangement is non-intuitive. Previous studies have proposed combining optical navigation systems with augmented reality (AR) to provide surgeons with intuitive imaging during surgery, through the use of planar and three-dimensional imagery. However, these studies have mainly focused on visual aids and have paid relatively little attention to real surgical guidance aids. Moreover, the use of augmented reality reduces system stability and accuracy, and optical navigation systems are costly. Therefore, this paper proposed an augmented reality surgical navigation system based on image positioning that achieves the desired system advantages with low cost, high stability, and high accuracy. This system also provides intuitive guidance for the surgical target point, entry point, and trajectory. Once the surgeon uses the navigation stick to indicate the position of the surgical entry point, the connection between the surgical target and the surgical entry point is immediately displayed on the AR device (tablet or HoloLens glasses), and a dynamic auxiliary line is shown to assist with incision angle and depth. Clinical trials were conducted for EVD (extra-ventricular drainage) surgery, and surgeons confirmed the system’s overall benefit. A “virtual object automatic scanning” method is proposed to achieve a high accuracy of 1 ± 0.1 mm for the AR-based system. Furthermore, a deep learning-based U-Net segmentation network is incorporated to enable automatic identification of the hydrocephalus location by the system. The system achieves improved recognition accuracy, sensitivity, and specificity of 99.93%, 93.85%, and 95.73%, respectively, representing a significant improvement from previous studies.

Abstract:Background: Intracranial arteriovenous malformations (AVMs) are lesions containing complex vessels with a lack of buffering capillary architecture which might result in hemorrhagic cerebrovascular accidents (CVAs). Intraoperative navigation can improve resection rates and functional preservation in patients with lesions in eloquent areas, but current systems have limitations that can distract the operator. Augmented Reality (AR) surgical technology can reduce these distractions and provide real-time information regarding vascular morphology and location. Methods: In this case report, an adult patient was admitted to the emergency department after a fall, and diagnostic imaging revealed a Spetzler-Martin grade I AVM in the right parietal region with evidence of rupture. The patient underwent a stereotactic microsurgical resection with assistance from augmented reality technology, which allowed for a hologram of the angioarchitecture to be projected onto the cortical surface, aiding in the recognition of the angiographic anatomy during surgery. Results: The patient's postoperative recovery went smoothly. At 6-month follow-up, the patient had remained in stable condition, experiencing complete relief from his previous symptoms. The follow-up examination also revealed complete obliteration of the AVMs without any remaining pathological vascular structure. Conclusions: AR-assisted microsurgery makes both the dissection and resection steps safer and more delicate. As several innovations are occurring in AR technology today, it is likely that this novel technique will be increasingly adopted in both surgical applications and education. Although certain limitations exist, this technique may still become more efficient and precise as this novel technology its continues to develop further.

Abstract:Based on the inherent coupling of system states, backstepping controllers are designed for underactuated spacecraft hovering without either the radial or in-track thrust and in the presence of input saturation and unmatched disturbances. Feasible hovering positions for either saturated underactuated case are derived. A Lyapunov-based approach is used to prove the asymptotical convergence of state errors and system robustness against unmatched disturbances. The validity of theoretical analysis is verified by simulations in a J2 -perturbed environment.
