Abstract:Branchial cleft cysts are congenital anomalies that form during fetal development and originate from the second branchial cleft. They typically manifest as painless masses on the side of the neck and can become symptomatic when infected. These cysts can create a cavity that may foster infection and, in rare instances, facilitate the spread of primary tumors. It is unusual to find ectopic thyroid tissue within a brachial cyst and it is even rarer to see papillary thyroid carcinoma developing from this tissue. Whenever physicians find a case of lateral neck cyst containing thyroid neoplasm without a known primary in the thyroid, there is always a confusion about whether it is a case of metastatic disease with an undetected primary tumor, or is a carcinoma originating from ectopic thyroid tissue. This is a case report of a papillary thyroid cancer that was unintentionally discovered inside a branchial cyst. So far, only five cases akin to this have been documented. There was no sign of an underlying primary thyroid tumor after the patient had a complete thyroidectomy and selected neck dissection, according to a comprehensive evaluation. This article touches on the development of thyroid tissue within branchial cysts and discusses the etiology of lateral neck tumors. The outcome for such patients appears to be favorable after cyst excision and total thyroidectomy. This article also emphasizes the importance of doing routine histopathological examinations on surgically removed samples that look benign.

Abstract:BackgroundPapillary thyroid microcarcinoma (PTMC), one specific subtype of papillary thyroid carcinoma (PTC) which measures less than 10 mm in maximum dimension, presents with a high risk of insidious lymph node metastasis escaping from preoperative examinations (cN0). Given the complications of lymph node dissection (LND) and metastasis risk, proper stratification of PTMC for performing prophylactic LND bears great importance.MethodsFrom June 2015 to December 2017, 338 PTMC patients undergoing thyroidectomy were included in the present study. Potential risk factors, including age, gender, maximal tumor size, etc. were collected and analyzed for association with thyroid lymph node metastasis.ResultsAmong the 338 patients, 87 patients (25.7%) presented with right central lymph node metastasis (CLNM) and 28 patients (8.3%) had posterior right recurrent laryngeal nerve lymph node metastasis (PRRLN-LNM). The maximal tumor was prone to occur at the middle part of the lower pole (35.3%) in patients with right CLNM, while the proportion of tumors located in the middle part of the upper pole (17.2%) was highest in PRRLN-LNM patients. Ages younger than 45 years old, male gender, and a tumor size of more than 0.5 cm were correlated independently with right CLNM and PRRLN-LNM. Presence of capsular invasion also had a significant association with the occurrence of PRRLN-LNM.ConclusionsAges younger than 45, male gender, and a maximal tumor size larger than 0.5 cm, in addition to capsular invasion, were independent risk factors for stratification of PTMC patients. PTMC patients with these clinical characteristics were suggested to receive prophylactic LND in their initial thyroid surgeries.
Malignant Proliferative Ependymoma of the Neck with Lymph Node Metastasis: A Case Report.

Abstract:BACKGROUND:Malignant proliferating trichilemmal tumor (MPTT) is an infrequent malignant neoplasm originating from cutaneous appendages, with only a handful of documented cases. This report delineates a unique instance of MPTT situated in the neck, accompanied by lymph node metastasis. A comprehensive exposition of its clinical trajectory and imaging manifestation is presented, aiming to enhance comprehension and management of this atypical ailment.CASE SUMMARY:Patient concerns: A 79-year-old male presented with a longstanding right neck mass persisting for over six decades, exhibiting recent enlargement over the past year. Diagnoses: Enhanced magnetic resonance imaging of the neck unveiled an elliptical mass on the right neck side, characterized by an ill-defined border and a heterogeneous signal pattern. The mass exhibited subdued signal intensity on T1-weighted imaging (T1WI) and a heterogeneous high signal on T2-weighted imaging (T2WI), interspersed with a lengthy T1 and T2 cystic signal motif. Close anatomical association with the submandibular gland joint was noted, and intravenous gadolinium diethylene triamine pentaacetic acid administration facilitated conspicuous enhancement. Substantial enhancement of the solid segment prompted an initial preoperative diagnosis of malignant nerve sheath tumor. However, post-surgery histopathological and immunohistochemical analysis conclusively confirmed the diagnosis as malignant hyperplastic external hair root sheath tumor. Intervention: Complete excision of the tumor was successfully executed. Outcomes: The patient experienced a favorable postoperative recovery.CONCLUSION:Malignant proliferative trichilemmal tumor external hair root sheath tumor is a cystic-solid lesion, appearing as low signal on T1WI images or high signal on T2WI with enhancement of the solid component. Suspicions of malignancy are heightened when the tumor border is indistinct, tissue planes are breached, or when linear or patchy high signals are observed in the subcutaneous tissue on T1 liver acquisition with volume acceleration enhanced images along with intermediate signal on T2WI and restricted diffusion on diffusion-weighted imaging images. Strong consideration for malignancy should arise if there are signs of compromised adjacent tissue relationships or direct invasion evident on imaging. We have incorporated the above-mentioned content into the entire manuscript.
Metastatic Squamous Cell Carcinoma of the Gingiva Appearing As a Solitary Branchial Cyst Carcinoma: Diagnostic Role of PET/CT.

Abstract:We herein present a case of a left cervical cystic mass, for which the initial pathological diagnosis was branchial cleft cyst carcinoma (following complete mass excision). Thorough postoperative examinations, including with FDG positron emission tomography/computed tomography (PET/CT), revealed a primary tumor in the retromolar region of the left mandible. A 52-year-old female presented with a 2-month history of a painless, progressively enlarged left-sided neck mass. Fine-needle aspiration biopsy suggested a branchial cleft cyst. Physical examination revealed a 3 × 3-cm smooth, tender mass in the upper-left neck and anterior border of the sternocleidomastoid muscle. Examination using nasendoscopy and a strobolaryngoscope revealed no abnormalities of the nasal cavity, nasopharynx, oropharynx, hypopharynx or larynx. MRI of the neck revealed a solitary, round, cystic mass under the left parotid gland. The mass was excised completely. Pathologic results indicated a branchial cleft cyst carcinoma. According to the diagnostic criteria for a branchial cleft cystic carcinoma, PET/CT was performed to detect the occult primary site. PET/CT revealed high FDG uptake in the tooth root of the left mandible. Frozen sections of the mass were indicative of moderate, differentiated squamous cell carcinoma. The carcinoma in the retromolar region of the left mandible was locally excised under general anesthesia. A partial left maxillectomy, partial mandibulectomy, and left radical neck dissection were performed. The patient received postoperative concurrent chemoradiotherapy, and was disease-free at the 8-month follow-up. True branchial cleft cyst carcinoma is rare: once diagnosed, it should be distinguished from metastatic cystic cervical lymph and occult primary carcinoma. FDG PET/CT is useful in the identification of occult primary tumor.

Abstract:Background: The simultaneous occurrence of Branchial Cleft Cyst (BCC) and Papillary Thyroid Carcinoma (PTC) represents an unusual malignant tumor, with cases featuring associated lymph node metastasis being particularly rare. This combination underscores an increased potential for metastasis, and the assessment of neck masses, particularly on the lateral aspect, may inadvertently overlook the scrutiny of the thyroid. Therefore, healthcare providers should exercise vigilance, especially in patients over the age of 40, regarding the potential for neck masses to signify metastasis from thyroid malignancies. Currently, surgical intervention stands as the primary effective curative method, while the postoperative administration of radioactive iodine therapy remains a topic of ongoing debate. Case report: In the presented case, a 48-year-old male patient with a right neck mass underwent surgical intervention. The procedure included the excision of the right neck mass, unilateral thyroidectomy with isthmus resection, and functional neck lymph node dissection under tracheal intubation and general anesthesia. Postoperative pathology findings revealed the coexistence of a BCC with metastatic PTC in the right neck mass, as well as papillary carcinoma in the right thyroid lobe. Lymph node metastasis was observed in the central and levels III of the right neck. Conclusion: The rare amalgamation of a BCC with PTC and concurrent lymph node metastasis underscores the invasive nature of this malignancy. Healthcare professionals should be well-acquainted with its clinical presentation, pathological characteristics, and diagnostic criteria. A multidisciplinary approach is strongly recommended to enhance patient outcomes.

Abstract:Thyroglossal duct cyst carcinoma is uncommon. Its prevalence is less than one percent. Papillary carcinoma is the most common pathologic finding of the thyroglossal duct cyst carcinoma. Metastasis of the cervical lymph node is less common. Its prevalence is less than 8%. A 38-year-old woman presented with a complaint of painless mass in the midline of the neck. Ultrasonography of the neck revealed a solid cystic mass, measuring 5.5 × 2.5 cm in the submental area. Ultrasonography of the neck additionally revealed suspicion lymph node at bilateral level Ib. A preoperative Computed Tomography (CT) showed a 4 cm size MIX heterogeneous lesion in the anterior left-sided hyoid bone. The patient underwent sistrunk surgery, which was the result of the patient’s frozen section papillary carcinoma arising from the thyroglossal duct cyst. The patient had local metastasis to the neck, so she underwent total thyroidectomy and central N-D surgery and bilateral modified radical N-D. Diagnosis of thyroglossal duct cyst carcinoma before surgery is unusual. The definitive diagnosis is often after surgery and is based on the patient’s histopathological findings. The best surgical procedure is sistrunk. But if the patient has local metastasis to the neck, total thyroidectomy and modified radical N-D are performed.
Papillary Thyroid Carcinoma Within Thyroglossal Duct Cyst

Abstract:Abstract A 55-year-old female with medical history of hypothyroidism and fibrocystic disease of the breast presented with complains of a painful anterior neck mass, difficulty swallowing and hoarseness of the voice. Symptoms had progressed over a period of 5 months. CT neck with contrast indicated the presence of an ectopic thyroid tissue anterior to the thyroid cartilage measuring approximately 1.7 x 1.2 x 3.1 cm, with indistinct inferior margins and internal calcifications. The hyoid bone or thyroid cartilage had no irregularities. The thyroid gland itself was unremarkable except for small complex thyroid nodules in both lobes. No masses within the pharynx or larynx were noted. Family history was significant for lymphoma in her father. On physical exam, a hard, mobile right anterior neck mass was appreciated. Labs showed normal TSH of 1.05 uIU/mL and normal free T4 of 1.2 ng/dL. Further evaluation with a dedicated neck US showed a right submandibular mass, superior to the thyroid, lobulated and heterogeneous measuring 2.0 x 1.0 x 2.3 cm with multiple areas of calcifications and internal Doppler flow. The thyroid gland had normal size and texture with bilateral sub centimeter non-concerning nodules. After ENT evaluation and an unremarkable flexible fiberoptic nasolaryngoscope, patient underwent surgical excisional biopsy of the neck mass. Pathology was consistent with thyroglossal duct cyst with the presence of thyroid follicles. An incidental finding of a 0.9 cm papillary microcarcinoma was noted, which was encapsulated with focal extracapsular follicular structures showing papillary nuclear features with no perineural or lymphovascular invasion. The tumor cells were immunoreactive for TTF-1 and PAX8. Development of papillary thyroid cancer within the thyroglossal duct cyst is a rare event, reportedly occurring in 1% of thyroglossal duct cysts. There are no well-established management guidelines. Current management strategies consist of monitoring with serial neck ultrasound versus total thyroidectomy with consideration of postsurgical I-131 treatment, based on pathology results. Our patient opted for undergoing total thyroidectomy.

Abstract:A thyroglossal duct cyst is the most common congenital disease in the neck. There are two age groups usually associated with thyroglossal duct cysts: 1-11 years in children and 30-60 years in adults. These midline neck masses are typically located anteriorly in the neck, inferior to the hyoid bone. We report an extremely rare case of an intralaryngeal thyroglossal duct cyst without a neck mass, presenting with hoarseness as the sole symptom. A 64-year-old man presented with a 3-month history of hoarseness. On physical examination, no neck mass or swelling was observed during cervical palpation. Laryngostroboscopy revealed a large submucosal mass in the right glottis and supraglottis, and mobility of the right vocal cord was restricted. Surgery was performed via an external approach to completely resect the cyst, together with the middle part of the hyoid bone. Histopathologic examination of the cyst led to a diagnosis of thyroglossal duct cyst. The patient recovered well and his voice returned to normal. Attention should be paid to the occurrence of rare types of thyroglossal duct cyst in unusual clinical sites. Adequate radiological examinations should be performed, and reading the computed tomography or magnetic resonance imaging scans carefully before surgery is important to avoid misdiagnosis.

Abstract:ABSTRACT Papillary thyroid carcinoma coexisting with lymphangioma is a rare presentation of a cystic neck mass. Here, we report a case of papillary thyroid carcinoma masquerading as lymphangioma in the neck clinically and radiologically. Our patient was a 31-year-old man who presented with swelling on the left side of the neck for four months. A detailed history and investigation are summarized, and a review of published literature on similar cases is obtained. The initial computed tomography (CT) scan reported it as lymphangioma, and the fine-needle aspiration cytology (FNAC) reported it as a cystic lesion. However, the excision biopsy showed a picture of metastatic papillary carcinoma of the thyroid with cystic changes and associated lymphangioma. The patient finally underwent total thyroidectomy, which again revealed multifocal papillary thyroid carcinoma with capsular invasion and lymph node metastasis on histopathological examination (HPE). Primary occult carcinoma of the thyroid with metastatic deposits presenting as lymphangioma is a very rare presentation of a cystic neck mass, and very few cases have been reported in the English language literature.

Abstract:The results of splenectomy for hematologic disorders in 200 patients have been evaluated. Anticipated good results were achieved in all patients with hereditary spherocytosis and over 80% of patients with idiopathic thrombocytopenic purpura. A hematologic improvement in patients with thrombotic thrombocytopenic purpura, thalassemia, myeloid metaplasia, and malignant hemopathy occurred in a significant percentage of cases. The overall in-hospital mortality was 7% but was totally related to the primary disease process. It was concluded that the results justify broadening the indications for splenectomy in patients with hematologic disorders.

Abstract:To the Editor.— The elegant and important data reported by Farmer and colleagues 1 raise several issues that deserve comment. A different interpretation of their data would be to exclude from analysis those twin pairs with one twin with an affective disorder and one schizophrenic twin, on the grounds that there is a reasonable possibility of a misdiagnosed affective psychosis in the schizophrenic twin. Reanalysis of the remainder of the sample lends support to the possibility of a single-gene dominant form of schizophrenia. The co-occurrence of schizophrenia and affective psychosis in the same identical twin pair is more likely (in any individual case) to be due to misdiagnosis of the "schizophrenic" twin than to a genetic relationship between schizophrenia and mood-incongruent affective disorders (as suggested by Farmer and associates). Patients with early-onset affective psychoses often receive schizophrenic diagnoses because they tend to have more bizarre and psychotic symptoms than patients

Abstract:Retropharyngeal goitre extending to the oropharyngeal level is rare. We present a case of papillary thyroid carcinoma (PTC) with a retropharyngeal cystic goitre extending to the uvular level. A woman in her 50s presented with swelling of the neck and dyspnoea. CT and MRI findings showed a primary tumour in the left lobe of the thyroid gland and a retropharyngeal dumbbell-shaped cystic goitre extending to the uvular level. Total thyroidectomy, central neck dissection and tracheostomy were performed. During the surgery, we opened the retropharyngeal space, and no mass was found. The pathological findings showed that the primary PTC (pT2) was surrounded by benign lesions, including the dumbbell-shaped cystic goitre. We speculated that the dumbbell-shaped cystic goitre extended from the visceral space (VS) into the pharyngeal mucosal space (PMS) and reached the uvular level because the thyroid gland is in the VS, and the VS and PMS are continuous spaces.

Abstract:INTRODUCTION: Papillary carcinoma originating from a thyroglossal cyst is rare and peculiar, with majority of cases detected after surgery. Despite an excellent prognosis, its management remains controversial. Herein, we report the case of a 53-year-old woman who underwent Sistrunk procedure for a thyroglossal duct cyst and was subsequently confirmed to have papillary thyroid carcinoma.PRESENTATION OF CASE: A 53-year-old woman presented with an anterior midline neck mass for 7 years. The patient had no symptoms of hypo-or hyperthyroidism. Additionally, she had no history of compressive symptoms. Neck ultrasound revealed a well-defined 3.5 cm × 2.2 cm × 3 cm-sized cystic lesion inferior to the hyoid bone, with a peripheral solid component. Neck computed tomography revealed a well-defined 3.7 cm × 3.4 cm × 2.7 cm-sized cystic lesion with an enhanced central solid component with focal calcifications, inferior to the hyoid bone, and in contact with the anterior wall of the thyroid cartilage. Sistrunk procedure was performed. The patient was then diagnosed with papillary thyroid carcinoma with TNM stage pT2 and underwent total thyroidectomy as a follow-up procedure.DISCUSSION: Thyroglossal duct cyst carcinoma is usually detected in the fourth decade of life with a higher prevalence in women. Neck ultrasound is performed during the initial radiological workup to assess the cyst and confirm the presence of the thyroid gland.CONCLUSION: The Sistrunk procedure is highly effective in low-risk patients. A more aggressive approach is required for high-risk patients.
Branchial Cyst Masquerading as Myositis and Abscess of Sternocleidomastoid Muscle and Necrotic-Cystic Lymphadenopathy

Abstract:Branchial cleft cysts are considered one of the most common cystic lesions in neck and are commonly seen in pediatric age group and young adulthood with most of the cases presenting within second and third decade. Here we intend to discuss a case of a 43 year old male who presented with a very short history of a painful lateral neck mass and was clinico-radiologically suggested as either myositis and abscess of sternocleidomastoid muscle or necrotic/cystic lymphadenopathy. An ultrasound guided needle aspiration cytology did not show any microorganism thus an excisional biopsy of the mass was done suspecting it to be lymph nodal mass. However the histopatholgical examination confirmed it to be an inflamed branchial cyst. Thus we would like to highlight the importance of keeping branchial cleft cysts as a possible differential while managing lateral neck masses of any duration in adults as well as in children.
Metastatic Follicular Carcinoma of the Thyroid: A Case Report

Abstract:Neck lumps may indicate metabolic disorders of the parathyroid and thyroid glands commonly present in the anterior aspect of the neck. Some neck lumps are detected as follicular thyroid cancer. Follicular thyroid cancer is a malignant epithelial tumor that shows evidence of follicular cell differentiation but lacks the characteristic nuclear features of papillary thyroid carcinoma (PTC). Iodine-deficient regions have higher rates of follicular carcinoma. There has been a decline in the incidence of this kind of tumor in recent years. As with PTC, prior radiation therapy increases the risk of follicular cancer, but to a lesser extent. In some cases, patients have distant metastases that involve the bones. Follicular cancer is divided into minimally invasive and widely invasive types. Early detection is important. Treatment usually consists of thyroidectomy and radioactive iodine therapy, and hormone replacement therapy may be necessary. Fine needle aspiration cytology is an efficient and cost-effective tool for the diagnosis of neck swelling and has the potential to diagnose the mass. We report the case of a 60-year-old Indian woman who had been experiencing neck swelling for the last 12 years. The neglected neck mass was confirmed as a hyperechoic mass with microcalcifications on ultrasound, representative of metastatic follicular thyroid carcinoma, which further spread to the ribs and the vertebrae. The patient was followed without complications.

Approximately 250 cases of papillary thyroid cancer (PTC) within a thyroglossal duct cyst (TGDC) have been reported worldwide. The presence of PTC within a TGDC is rare representing less than 1 % of all TGDC cases. This report describes the case of a young male with PTC in a TGDC suspicious nodules. We present this case along with a review of the literature related to this very uncommon finding. Papillary thyroid carcinoma is a very rare disease but always should be considered when speaking of a mass at the level of the central neck. There is a considerable controversy regarding the role of thyroidectomy to treat thyroglossal duct cyst carcinoma since the treatment of choice is Sistrunk procedure and the only indications to make a thyroidectomy will be aggressive disease and more than 45 years old according to some articles but without any consensus now. With a cure rate of 95 % and recurrence rate of 3 -10 % when this cancer is treated by Sistrunk operation, it doesn’t seem necessary to do an extent surgery if there is not enough thyroid compromised. Long term follow up is necessary to exclude the development of thyroid cancer. Presentation Date: Saturday, June 17, 2023

Abstract:Branchial cleft carcinoma is a rare malignancy, there still exist some controversies regarding to the differentiation of branchial cleft carcinomas. This article is aimed to familiarize clinicians with the presentation and treatment of the tumour and explores its origin. A 41-year old man who was seen with a large lesion of branchial cleft carcinoma with a slowly growing mass in the area of the left parotid gland was reported. The histological finding of dysplastic epithelium was next to direct invasive carcinoma. The diagnosis of a branchial cleft carcinoma requires the fulfillment of strict criteria. This case supports the origin of the carcinoma as being from an epithelial-lined cyst.
Synchronous lateral lymph node metastases from papillary and follicular thyroid carcinoma: case report and review of the literature

Abstract:Abstract Background Follicular thyroid carcinomas (FTCs) rarely metastasize to regional lymph nodes, and descriptions of synchronous lateral lymph node metastases of FTC and papillary thyroid carcinoma (PTC) are lacking. Case Presentation We describe a 43-year-old female with a preoperative cytology indicating a right-sided PTC with lateral lymph node metastases. She underwent a total thyroidectomy and central and lateral lymph node dissection, and histopathology confirmed a multifocal tall cell variant PTC together with a 12 mm minimally invasive FTC in the ipsilateral lobe. While the central compartment demonstrated metastatic PTC, the lateral compartment contained PTC metastases alongside a 15 mm large follicular-patterned mass in a separate lymph node. As the cells lacked PTC associated nuclear changes, the possibility of a lateral lymph node metastasis of FTC was considered, with the possibility of ectopic thyroid tissue as a differential diagnosis. By utilizing next-generation sequencing, a Q61R NRAS mutation was pinpointed, thus proving the tissue as tumorous. The patient underwent radioiodine treatment and is currently monitored following a negative whole-body scan. Conclusions This is probably the first case report of a patient with co-existing lateral lymph node PTC and FTC metastases. Consulting previous publications, there is currently a gap of knowledge in terms of how patients with regional FTC metastases should be followed-up and treated, especially when co-occurring with spread high-risk PTC subtypes. Moreover, what guides a seemingly indolent FTC to spread via the lymphatic system remains to be defined from a molecular standpoint.
Papillary Carcinoma Developing from a Thyroglossal Duct Cyst: A Case Report

Abstract:Background: Thyroglossal duct cyst is the most common midline congenital anomaly of the neck. Papillary carcinoma in the thyroglossal duct cyst and papillary carcinoma in the thyroid tissue can be seen together. In this article, we present a case of papillary carcinoma of the thyroglossal ductus cyst in a 43-year-old male patient. In the postoperative second year ultrasonography, our patient had multiple reactive lymph nodes in the submandibular region due to tooth infection and about 4x3 mm cystic nodule was detected in the thyroid left lobe. Total thyroidectomy was recommended to the patient when our patient wanted to exit long -term follow -up. In our case, although the focus of papillary carcinoma is less than 1 cm and the capsule is intact in the thyroglossal cyst material, the focus of 4 mm papillary carcinoma was found in the total thyroidectomy material. Even if the focus of papillary carcinoma in the thyroglossal cyst is smaller than 1 cm and the capsule is intact, it should be kept in mind that there may be a focus of papillary ca in the thyroid tissue and patients should not be excluded from follow-up.
