Abstract:Chronic osteomyelitis of the skull base is a commonly reported pathology in existing scientific literature, but chronic osteomyelitis of the skull vault (COSV) is a rarely documented disease. We report the case of a 38 years old Afghan male with a presenting complaint of irregular swelling on the skull vault for six months. The patient had a history of head trauma one year back with a compound depressed fracture which had been surgically managed then. Physical examination revealed a 15 x 15 cm hard, immobile swelling with an old scar mark over the scalp. Neuroimaging was consistent with diffuse, bilateral frontoparietal swelling of bony origin, sparing underlying brain parenchyma. Surgically excision of the lesion was done through a bicoronal skin flap and cranioplasty done at the same time. Histological findings of the specimen reported chronic osteomyelitis. However, microbiology revealed no growth in culture. Patient was discharged on the second postoperative day uneventfully. This case turns minds into keeping skull vault osteomyelitis as differential diagnosis besides other spontaneous bony lesions; e.g. fibrous dysplasia, osteoma and giant cell tumours. Mandibular reconstruction with TMJ prosthesis: management of osteomyelitis after orthognathic surgery (Case Report)

Abstract:This case report presents a mandible reconstruction with temporomandibular joint (TMJ) prothesis for treatment of osteomyelitis infection following a bimaxillary orthognathic and genioplasty surgeries. The patient, a 41-year-old female, presented with facial pain, difficulty in opening her mouth, and mandibular deviation four months after the initial orthognathic surgery. During the four months, the patient had experienced two more surgerys post-operative for treatment of the complications. Examination revealed mobility between osteotomy segments of the right mandibular osteotomomie between body and ramus, and Computer Tomographic scan analysis revealed osteomyelitis and non-union within the osteotomy side of the right mandible from the previous surgery. Due to the advanced state of the disease and significant bone resorption, treatment was planned in two stages, involving bone debridement and then reconstruction with a custom made extended temporomandibular joint prosthesis. The patient had no postoperative complications and achieved satisfactory functional and aesthetic outcomes. This case emphasizes the importance of careful monitoring and prompt management of postoperative complications following orthognathic surgery to prevent rare but serious complications such as osteomyelitis.

Abstract:Osteomyelitis is mainly caused by pyogenic organisms that spread through fractures, the bloodstream, or surgeries. It is a serious bone infection that can be either acute or chronic. It involves an inflammatory process affecting the bone and its surrounding structures. Management of osteomyelitis mainly depends on the nature of the lesion. In pathological fracture, the usual treatment is resection of the lesion with affected margins. However, the reconstruction is usually delayed until the infected bone is completely removed. In this case report, we demonstrate a case of an unusual presentation of osteomyelitis in the right body of the mandible which caused a pathological fracture following the extraction of the lower right first molar. The patient was further treated by segmental resection, coronoidectomy, and reconstruction of the defective site using a recon plate with inferior alveolar nerve preservation using the lateralization technique.
Primary tubercular osteomyelitis of zygoma: A literature review and case report

Abstract:Purpose: This study presents a rare case of primary tubercular osteomyelitis of the zygoma, and addresses how combined surgical and medical treatments are effective managing rare presentations of tubercular osteomyelitis in facial bones. Methods: A 57-year-old man presented with pain and purulent discharge from the right malar region following trauma. Initial treatments with empirical antibiotics had poor response, hence further investigations were done. Comprehensive diagnostic workup included lab tests, Computed tomography with contrast, and histopathological examination. The patient underwent surgical debridement of the zygomatic bone through an intraoral approach, and debrided tissue was sent for microbiological and histopathological examination, which confirmed tubercular osteomyelitis. Post-surgery, the patient was treated with a four-drug anti-tubercular regimen. Results: Following anti-tubercular therapy, the patient showed significant improvement, with no signs of disease recurrence after a year of follow-up. Conclusion: Primary tubercular osteomyelitis of the zygoma is extremely rare but should be considered in differential diagnoses of facial swellings and discharging sinuses. This case underscores the importance of a multidisciplinary approach in treating rare presentations of tubercular osteomyelitis.

Abstract:Purpose: The objective of this study was to retrospectively evaluate and report the associated factors with the diagnosis and management of 24 patients with chronic osteomyelitis of the mandible seen at the authors' institution during the past several years. Patients and methods: Only cases of chronic osteomyelitis of the mandible not associated with antiresorptive medications or radiotherapy to the maxillofacial region were included in the study. After confirmation of the diagnosis, initial clinical and radiologic findings, treatment approach, and outcome were evaluated for each patient. Fourteen male and 10 female patients (average age, 53.75 yr; range, 22 to 83 yr) were included. Results: The peak incidence of the disease was recorded in the fifth and sixth decades of life. An uneventful healing was observed in 20 patients (83.3%). One of 18 patients (5.5%) who underwent segmental resections developed a secondary infection and was managed with intravenously administered antibiotics. Three of 6 patients (50%) who were treated with marginal resections remained symptomatic after surgery. Conclusion: Independent of the cause and presentation of the disease, complete resolution of the infection should be the main focus of management in patients with chronic osteomyelitis of the mandible, and findings of this retrospective study indicate that a conservative surgical approach is more likely to result in a less than ideal outcome.

Abstract:The present study aimed to investigate outcomes following open surgery for extensive skull base ORN. Open surgery through a personalized sequential approach was employed to deal with five cases of extensive skull base ORN. Two patients with mild cases underwent regional debridement and sequestrectomy, and three patients with severe cases underwent extensive resection with reconstruction using free anterolateral thigh (ALT) flap. Biological glues and vascularized flaps were used for obturation of the skull base bony defect to prevent postoperative cerebrospinal fluid (CSF) leakage. The infections were controlled by antibiotic administrations which strictly followed the principles of antimicrobial stewardship (AMS). As results, both regional debridement plus sequestrectomy and extensive resection achieved satisfied outcomes in all patients. No severe complications and delayed hospitalization occurred. During the follow-up period (8-19 months), all patients were alive, pain free, without crusting or purulent discharge, and no sequestration or CSF leakage occurred. In conclusion, a personalized sequential approach including open surgery, pedicled/vascularized free flap reconstruction and AMS was advocated for patients with extensive skull base ORN.

Abstract:This article presents a case report of a cranioplasty performed after a post-craniotomy empyema with osteitis. The skull reconstruction was performed using a bioceramic implant and a combined muscular free flap of latissimus dorsi and serratus anterior. This procedure not only provided coverage of a wide skull defect but also allowed the filling of the intracranial dead space. Clinically, we observed an improvement of the patient's preoperative neurological status with a near-complete correction of her right hemiparaesis and phasic disorders. Eight months after the cranioplasty, (1) no recurrence of infection was noticed; (2) no distortion of the skull was noticeable and (3) the patient again experienced a normal social life. Using computed tomography (CT) scan images, we observed a re-expansion of the left cerebral hemisphere without any dead space or extradural collection. The only observable sequelae were a temporoparietal alopecia (10 cm x 4 cm) and a winging of the scapula, induced by the skin graft and the removal of the lower-third of the serratus anterior muscle, respectively. The use of a muscular free flap associated with a customised biomaterial allows a single-stage reconstruction of extensive skull defect (120 cm(2)) in a previously infected area.

Abstract:Purpose: Mandibular osteomyelitis is relatively rare except in cases of osteoradionecrosis or medication-related osteonecrosis. The purpose of this case report is to highlight a rare but devastating complication of dental implant surgery. Materials and methods: The case of a patient who developed mandibular osteomyelitis after implant placement, which was resistant to long-term antibiotic therapy and required radical surgical treatment with fibular free flap reconstruction, is reviewed as is the related literature. Results: The most frequent etiologies are odontogenic and traumatic; however, hematogenous spread also exists. It usually affects patients with systemic conditions, such as diabetes mellitus, malnutrition, malignancy, or immune deficiency. The infection is usually polymicrobial. Concerning dental implant complications, the literature is comprehensive on the mechanical etiologies of implant failure and the infectious etiologies of peri-implantitis. Mandibular osteomyelitis treatment is a long and challenging process requiring long-term antibiotic therapy and multiple surgeries. Conclusion: The pathophysiology and treatment of mandibular osteomyelitis are discussed.
Ultrastructural morphometry and the control of lymphocyte nuclear size in non-Hodgkin lymphoma: a review.

Abstract:Morphologic features, particularly nuclear characteristics, continue to be a major factor in the classification of the subtypes of non-Hodgkin lymphoma (NHL). Information is limited or nonexistent, however, concerning the mechanisms responsible for nuclear appearance. This is particularly true for nuclear size. In determining which of the nuclear compartments--condensed chromatin or the interchromatinic (euchromatin or nuclear matrix) region--is responsible for nuclear size, ultrastructural morphometric analysis has been a useful tool. Linear regression analysis has been used to compare, in turn, condensed chromatin area (CCA) and interchromatinic area (ICA) against nuclear area (NA). Whether in normal or neoplastic lymphocytes, correlation coefficients are moderately good when CCA is matched against NA, but CCA is only a weak predictor of NA. In contrast, correlation coefficients are remarkably high for ICA compared to NA, and ICA predicts NA to the degree of 94% to 97%. This holds true for all NHLs tested and for reactive hyperplasias. Despite abnormalities in lymphocyte nuclear size in NHL, the size of any nucleus is controlled by the amount of interchromatinic material to an exceptional degree. Even so, the mechanisms responsible for major changes in nuclear volume during transformation of normal lymphocytes are apparently altered in NHL. Morphometric analysis, ultrastructurally, can assist in understanding, testing, and defining diagnostic morphologic criteria in this disease.
Usability Evaluation of a Clinical Decision Support System for Geriatric ED Pain Treatment

Abstract:Summary Background Older adults are at risk for inadequate emergency department (ED) pain care. Unrelieved acute pain is associated with poor outcomes. Clinical decision support systems (CDSS) hold promise to improve patient care, but CDSS quality varies widely, particularly when usability evaluation is not employed. Objective To conduct an iterative usability and redesign process of a novel geriatric abdominal pain care CDSS. We hypothesized this process would result in the creation of more usable and favorable pain care interventions. Methods Thirteen emergency physicians familiar with the Electronic Health Record (EHR) in use at the study site were recruited. Over a 10-week period, 17 1-hour usability test sessions were conducted across 3 rounds of testing. Participants were given 3 patient scenarios and provided simulated clinical care using the EHR, while interacting with the CDSS interventions. Quantitative System Usability Scores (SUS), favorability scores and qualitative narrative feedback were collected for each session. Using a multi-step review process by an interdisciplinary team, positive and negative usability issues in effectiveness, efficiency, and satisfaction were considered, prioritized and incorporated in the iterative redesign process of the CDSS. Video analysis was used to determine the appropriateness of the CDS appearances during simulated clinical care. Results Over the 3 rounds of usability evaluations and subsequent redesign processes, mean SUS progressively improved from 74.8 to 81.2 to 88.9; mean favorability scores improved from 3.23 to 4.29 (1 worst, 5 best). Video analysis revealed that, in the course of the iterative redesign processes, rates of physicians’ acknowledgment of CDS interventions increased, however most rates of desired actions by physicians (such as more frequent pain score updates) decreased. Conclusion The iterative usability redesign process was instrumental in improving the usability of the CDSS; if implemented in practice, it could improve geriatric pain care. The usability evaluation process led to improved acknowledgement and favorability. Incorporating usability testing when designing CDSS interventions for studies may be effective to enhance clinician use.

Abstract:Actinomycosis is an opportunistic infection caused by bacteria of the <i>Actinomyces</i> spp., commonly <i>A. israelii</i>. These are non-pathogenic commensals in the mouth, gut, and female genital tract. An infection may arise following trauma or surgery, such as tooth extraction. More than half of cases of actinomycosis occur in the perimandibular area and are termed cervicofacial actinomycosis. Initially, the infection develops as a painful, rapidly progressive swelling. The lesion may then indurate and is often painless while the overlying skin discolors red to purple-blue. Prolonged treatment with antibiotics and surgery are often required for resolution, unless treatment is promptly started. However, diagnosis may be delayed or missed because of difficult bacterial culturing and frequent confusion with malignancy and other infections. This case study describes six patients who developed cervicofacial actinomycosis following third molar extraction. The purpose of this study is to inform clinicians on this stubborn and deceitful disease entity and to highlight the importance of clinical recognition for quick resolution with minimal morbidity.

Abstract:Osteomyelitis is inflammation of the bone which begins as an infection of the medullary cavity with rapid involvement of the haversian systems and extension to the periosteum. Various etiological factors are involved in origin of the disease, among them fungal origin is rare. Specific feature in fungal osteomyelitis is involvement of maxillary sinus. Maxillary osteomyelitis can be classified based on the following causes: traumatic, rhinogenic, and odontogenic. I hereby report a case of fungal osteomyelitis involving maxilla in a 50-yearold man with uncontrolled diabetes mellitus. This specific case was necrosed bone with analysed using Energy dispersive Xray and Scanning electron microscope analysis. This case report highlights the need of oral physician to rule out fungal origin in osteomyelitis.

Abstract:A 65-year-old male patient had a history of receiving radiotherapy for the treatment of laryngeal Squamous Cell Carcinoma six years prior. Following a mandibular extraction with osteotomy, he began experiencing severe pain. Physical examination revealed extensive bone exposure with purulent discharge, and imaging examinations indicated diffuse bone loss without fracture. To address the condition, a treatment plan was initiated, involving weekly laser therapy and antimicrobial Photodynamic Therapy (aPDT) with 0.1% methylene blue, administered at intra and extraoral points.

Abstract:Osteonecrosis of the jaw is a complex disorder with variable etiology, necessitating a thorough patient history and histopathological assessment of the exposed bone area for accurate treatment. In this case report, we describe a 68-year-old female patient who sought medical attention due to a history of facial trauma, tooth mobility, bone exposure in the maxilla persisting for over three months, and spontaneous tooth avulsion two weeks before the appointment. The patient also had a medical history of Non-Hodgkin's Lymphoma from 2007 to 2017, which was treated with monoclonal antibodies.

Abstract:Skull base osteomyelitis (SBO) is a complex and fatal clinical entity that is often misdiagnosed for malignancy. SBO is commonly a direct complication of otogenic, sinogenic, odontogenic, and rhinogenic infections and can present as central, atypical, or pediatric clival SBO. This review describes the clinical profile, investigational approach, and management techniques for these variants. A comprehensive literature review was performed in PubMed, MEDLINE, Research Gate, EMBASE, Wiley Online Library, and various Neurosurgical and Neurology journals with the keywords including: SBO, central or atypical SBO, fungal osteomyelitis, malignant otitis externa, temporal bone osteomyelitis, and clival osteomyelitis. Each manuscript's reference list was reviewed for potentially relevant articles. The search yielded a total of 153 articles. It was found that with early and aggressive culture guided long-term intravenous broad-spectrum antibiotic therapy decreases post-infection complications. In cases of widespread soft tissue involvement, an early aggressive surgical removal of infectious sequestra with preferentially Hyperbaric Oxygen (HBO) therapy is associated with better prognosis of disease, less neurologic sequelae and mortality rate. Complete resolution of the SBO cases may take several months. Since early treatment can improve mortality rates, it is paramount that the reporting radiologists and treating clinicians are aware of the cardinal diagnostic signs to improve clinical outcomes of the disease. It will decrease delayed diagnosis and under treatment of the condition. However, due to rarity of the condition, complete prognostic factors have not fully been analyzed and discussed in the literature.

Abstract:Valvular heart disease (VHD) refers to the disorder of cardiac valves leading to valvular regurgitation or stenosis. Severe VHD will result in chronic heart failure and cardiac death. The only large population‐based epidemiologic study involving echocardiography was performed in the United States in 2006. In this study, the prevalence of VHD in the United States was estimated to be 2.5%.[1] As a developing country, VHD caused by rheumatic heart disease is still a huge burden in China. It is estimated that there were around 7.07 million patients with rheumatic heart disease in China in 2015.[2] In addition, as life expectancy increases, the prevalence of the degenerative valvular disease increases sharply according to a regional survey in Xinjiang, which reported that the prevalence of degenerative valvular disease in Han elderly (mean age 67.7 years old) was 30.2%. Hence, due to Chinese socioeconomic characters, both rheumatic heart disease and degenerative valve disease contribute to the high burden of VHD in China. However, there is no national cross‐sectional study to uncover the epidemiologic features of VHD in China. Our group is currently conducting the first degenerative valve disease cross‐sectional study in China, aiming to provide a demographic profile of degenerative mitral valve disease nationwide. On this basis, a national multicenter registry will be started to build an adult severe VHD database in China.
