Abstract:Context : Patellofemoral pain (PFP) is a prevalent chronic condition characterized by retropatellar or peripatellar pain exacerbated by various knee flexion-based activities. Previous research has highlighted the impact of psychological constructs on pain and function in chronic musculoskeletal pain conditions, yet their influence on physical activity in PFP cohorts remains unexplored. We aimed to evaluate whether pain self-efficacy and pain catastrophizing predict variations in steps per day and moderate to vigorous physical activity (MVPA) among individuals with PFP. Design : Cross-sectional observational study. Methods : Thirty-nine participants (11 males) with PFP were included. Dependent variables were steps per day and minutes of MVPA. Independent variables were pain self-efficacy and pain catastrophizing, measured by the pain self-efficacy questionnaire and the pain catastrophizing scale. Participants were given an ActiGraph wGT3X-BT for 7 days to assess physical activity. Correlations were assessed between psychological measures and physical activity, and a simple linear regression was performed on psychological variables that correlated with physical activity. Alpha was set a priori at P < .05. Results : Pain self-efficacy scores displayed a moderate association with steps per day (rho = .45, P = .004) and a weak association with MVPA (rho = .38, P = .014). Pain catastrophizing scores exhibited no significant associations with physical activity ( P < .05). Regression models affirmed pain self-efficacy scores as significant predictors of both steps per day ( F 1,37 = 10.30, P = .002) and MVPA ( F 1,37 = 8.98, P = .004). Conclusions : Psychological measures continue to demonstrate value to clinicians treating PFP. Pain self-efficacy scores were moderately associated with steps per day and weakly associated with MVPA, explaining nearly a fifth of the variation in physical activity. Clinicians should prioritize the assessment of pain self-efficacy when treating individuals with PFP, potentially employing psychological interventions to improve physical activity in the PFP population.

Abstract:Background Patellofemoral pain (PFP) is a prevalent musculoskeletal disorder associated with functional impairments. Although postural control is commonly assessed in people with PFP, there are inconsistent results regarding potential postural control deficits in this population. Research question This review aims to evaluate whether postural control is impaired in people with patellofemoral pain (PFP) and the effectiveness of interventions on postural control measures. Methods We searched six databases from their inception to May 5, 2023. We included studies assessing clinic- or laboratory-based postural control measures in people with PFP compared to pain-free controls, and intervention studies with PFP populations. We assessed risk of bias using the Joanna Briggs Institute critical appraisal checklists and the Cochrane Risk of Bias 2 tool. We assessed the certainty of evidence using the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) approach. We used random-effects meta-analyses considering subgroups based on type of task, measure, and intervention. Results Fifty-three studies were included. Very low certainty evidence indicated that people with PFP have shorter anterior (SMD=0.53, 95%CI:0.16,0.90), posteromedial (SMD=0.54, 95%CI:0.04,1.03) and posterolateral (SMD=0.59, 95%CI:0.11,1.07) reach distance, and worse composite score (SMD=0.46, 95%CI:0.22,0.70). Very low to moderate certainty evidence indicated that people with PFP have worse anterior-posterior and overall stability indexes during single-leg stance (SMD=-0.71, 95%CI:-1.29,-0.14; SMD=-0.63, 95%CI:-0.94,-0.32) and overall stability index during double-leg stance (SMD=-0.39, 95%CI:-0.78,-0.00), but no differences in center of pressure area during stair ascent (SMD=0.32, 95%CI:-2.72, 3.36). Low certainty evidence indicated that kinesio taping improved anterior reach distance (SMD=-0.49, 95%CI:-0.89,-0.09), while no significant differences were observed between pre- and post-intervention outcomes for conventional rehabilitation and rigid taping. Significance Clinicians should use clinic- (star excursion or Y-balance tests) and laboratory-based (stability indexes) measures to identify impairments of postural control in people with PFP. Low certainty of evidence suggests short-term improvement in postural control with kinesio taping.

Abstract:Objective To systematically review and synthesise patellofemoral joint reaction force (PFJRF) in healthy individuals and those with patellofemoral pain and osteoarthritis (OA), during everyday activities, therapeutic exercises and with physical interventions (eg, foot orthotics, footwear, taping, bracing). Design A systematic review with meta-analysis. Data sources Medline, Embase, Scopus, CINAHL, SportDiscus and Cochrane Library databases were searched. Eligibility criteria Observational and interventional studies reporting PFJRF during everyday activities, therapeutic exercises, and physical interventions. Results In healthy individuals, the weighted average of mean (±SD) peak PFJRF for everyday activities were: walking 0.9±0.4 body weight (BW), stair ascent 3.2±0.7 BW, stair descent 2.8±0.5 BW and running 5.2±1.2 BW. In those with patellofemoral pain, peak PFJRF were: walking 0.8±0.2 BW, stair ascent 2.5±0.5 BW, stair descent 2.6±0.5 BW, running 4.1±0.9 BW. Only single studies reported peak PFJRF during everyday activities in individuals with patellofemoral OA/articular cartilage defects (walking 1.3±0.5 BW, stair ascent 1.6±0.4 BW, stair descent 1.0±0.5 BW). The PFJRF was reported for many different exercises and physical interventions; however, considerable variability precluded any pooled estimates. Summary Everyday activities and exercises involving larger knee flexion (eg, squatting) expose the patellofemoral joint to higher PFJRF than those involving smaller knee flexion (eg, walking). There were no discernable differences in peak PFJRF during everyday activities between healthy individuals and those with patellofemoral pain/OA. The information on PFJRF may be used to select appropriate variations of exercises and physical interventions.

Influence of unilateral versus bilateral patellofemoral pain on physical activity, pain self-efficacy, and disability

Abstract:Background: Patellofemoral pain (PFP) affects physical and psychosocial health; however, it is unclear if unilateral and bilateral PFP induce similar levels of impairments. Objectives: We aimed to compare physical activity (PA), pain self-efficacy, and disability between individuals with unilateral and bilateral PFP, and to compare these groups against pain-free controls. Design: Cross-sectional case-control. Method: Sixty-two individuals with PFP (25 unilateral and 37 bilateral) and 20 pain-free controls completed the PA assessment using an accelerometer by daily steps, light PA, and moderate-to-vigorous PA (MVPA). We also calculated variability in each PA variable. Pain self-efficacy and disability were measured using the Pain Self-Efficacy Questionnaire and Anterior Knee Pain Scale, respectively. Results: Individuals with bilateral PFP took fewer daily steps compared to pain-free controls (9568.1 ± 3827.0 vs. 12,285.8 ± 2821.2 steps/day; P = 0.018, Cohen d = 0.79), whereas individuals with unilateral PFP did not (11,099.2 ± 3547.1 steps/day; P = 0.503, Cohen d = 0.37). Individuals with bilateral PFP showed greater variability in MVPA compared to individuals with unilateral PFP (42.9 ± 23.1 vs. 29.0 ± 15.9; P = 0.011, Cohen d = 0.91) and pain-free controls (42.9 ± 23.1 vs. 17.6 ± 6.1; P < 0.001, Cohen d = 1.21). Individuals with unilateral and bilateral PFP similarly exhibited less time spent in MVPA, greater variability in daily steps, lower pain self-efficacy, and greater disability compared to pain-free controls (P < 0.001, Cohen d = 1.14-3.89). Conclusions: Versus pain-free controls, individuals with bilateral PFP displayed greater variability in MVPA than individuals with unilateral PFP. However, it is important to note that unilateral PFP influenced time spent in MVPA, variability in daily steps, pain self-efficacy, and disability at similar levels to bilateral PFP.

Abstract:Background: Vertical ground reaction force (VGRF) can lead to the development knee osteoarthritis. This review systematic and meta-analysis aimed to investigate the VGRF in patellofemoral pain patients (PFP) during weight bearing tasks. Methods: Search strategy was conducted in databases: Science Direct, Scopus, PubMed and Google Scholar from Sep 2020 to Jun 2021. The VGRF components we measured were passive impact (Fz1)/peak and propulsion/active peak (Fz2). The quality of the studies was evaluated with Down and Black index and it was divided into three groups: low quality (LQ), medium quality (MQ) and high quality (HQ). The standardized mean difference between PFP and healthy individuals was used to calculate the effect size. Results: Nine articles were selected for systematic review and meta-analysis of which 5 studies was HQ, 3 studies were MQ and 1 study was LQ were classified. PFP compared to healthy individuals with moderate and small effect size have reduced impact and propulsion respectively. Conclusion: The in PFP, VGRF is influenced psychological, behavioral and biomechanical factors. Therefore, psychosomatic therapeutic approaches may have a long time effectiveness on the rehabilitation of PFP.

The Relationship between Patellofemoral Pain Syndrome and Hip Biomechanics: A Systematic Review with Meta-Analysis

Abstract:(1) Background and purpose: Muscular control and motor function in a patient with Patellofemoral pain syndrome (PFPS) have not yet been investigated systematically. Therefore, this review synthesis the previous results about the association of PFPS with gluteus muscle activation, hip strength, and kinematic characteristic of the hip and knee joint, to deepen understanding of the PFPS etiology and promote the establishment of an effective treatment strategy. (2) Methods: A literature search was conducted from January 2000 to July 2022 in four electronic databases: Medline, Embase, Google scholar, and Scopus. A total of 846 articles were initially identified, and after the screening process based on the inclusion criteria, 12 articles were eventually included. Means and SDs of gluteus medius (GMed), gluteus maximus (GMax), hip strength, and kinematic variation of hip and knee were retrieved from the present study. (3) Results and conclusion: Regarding kinematic variation, moderate evidence indicates that an increased peak hip adduction was found in PFPS groups during running and single leg (SL) squat activities. There is no difference in the GMed and GMax activation levels between the two groups among the vast majority of functional activities. Most importantly, strong evidence suggests that hip strength is weaker in individuals with PFPS, showing less strength of hip external rotation and hip abduction compared to the control group. However, without prospective studies, it is difficult to determine whether hip strength weakness is a cause or a result of PFPS. Therefore, further research is needed to evaluate the hip strength level in identifying individuals most likely to associated with PFPS development is needed.

Association of altered frontal plane kinematics and physical activity levels in females with patellofemoral pain

Abstract:Background: Females with patellofemoral pain (PFP) present with altered frontal plane kinematics, decreased physical activity, and elevated psychological features. However, the relationship between these factors has not been evaluated. Research question: Does a relationship exist between frontal plane kinematics, physical activity levels, and fear avoidance beliefs in females with PFP. Methods: 16 females with PFP (Age = 23.2 ± 4.9years; Height = 166.1 ± 5.9 cm; Mass = 66.3 ± 13.5 kg) completed a 3-D biomechanical assessment during a single leg squat, step-down, and jogging task. Physical activity was collected with an activity monitor over 2-weeks and the Fear Avoidance Belief Questionnaire physical activity subscale was used to assess fear avoidance beliefs. Frontal plane kinematics were correlated with physical activity and fear avoidance, with significant variables included in a multiple regression. Results: Significant correlations were identified between physical activity and single leg squat hip adduction (r=-0.626, p = 0.01), step-down knee abduction (r=-0.783, p < 0.001) and jogging hip adduction (r=-0.639, p = 0.008). Step-down knee abduction predicted activity level and accounted for 61% of the total variance. Correlations between fear avoidance belief and single leg squat hip adduction (r = 0.477, p = 0.049), step-down knee abduction (r = 0.644, p = 0.007), jogging knee abduction (r = 0.558, p = 0.025), and jogging hip adduction (r = 0.557, p = 0.025) were also identified. Knee abduction during the step-down and single leg squat hip adduction accounted for 37.5% of the variance in the Fear Avoidance Belief Questionnaire. Significance: Altered frontal plane kinematics during step-down and jogging provide insight for clinicians to identify females with patellofemoral pain that may be less physically active, while step-down and squatting kinematics related to fear avoidance beliefs. While these relationships were found, it is essential to identify the underlying mechanism for this activity modification so clinicians and researchers can evaluate successful interventions.
Influence of Pain, Risk Factors, and Functional Ability on Physical Activity Levels in Women with Anterior Knee Pain: A Cross-Sectional Study

Abstract:Background and Objectives: Anterior knee pain (AKP) refers to chronic prepatellar pain and is one of the most common knee complaints in physically active women. This condition can significantly affect daily activities and overall quality of life. This study aims to assess the impact of pain, risk factors, and functional ability on different levels of physical activity (comparing low versus moderate activity) in women with AKP. Materials and Methods: This cross-sectional study involved fifty-six women diagnosed with AKP (aged 20-45 years) who were equally allocated into low and moderate physical activity groups. Their AKP and functional ability were assessed using the visual analog scale (VAS) and double squats and step-down tests, respectively. Possible risk factors were assessed using the Q-angle, modified Thomas test, sit and reach test, and patellar glide test. A person's correlation coefficient and independent t-tests were used to determine the relationship and the differences between these variables while keeping the confidence interval level at 95%. Result: Women with moderate activity levels showed significantly higher scores on the VAS than those with low activity levels (p = 0.040). However, both groups had no significant difference in their functional ability or potential risk factors (p > 0.05). Additionally, their functional ability (double squat) showed a positive association with hamstring flexibility (Pearson correlation coefficient [r]:0.3; p = 0.006). Conclusions: Women with AKP who were engaged in moderate physical activity experienced higher levels of pain compared to those with low activity levels. These findings underscore the urgent need for further investigation into different levels of physical activity to develop appropriate prevention and treatment strategies for women with AKP.
Physical activity level and physical fitness in subjects with chronic musculoskeletal pain: a cross-sectional study

Abstract:Background Low physical activity (PA) levels and low physical fitness (PF) have been reported in subjects with temporality-based chronic pain; however, it is unknown whether there are differences in subjects with nociplastic pain (NP) compared with subjects with non-nociplastic pain (NNP). Objective The aim was to compare the levels of PA and PF in patients with chronic, nociplastic, and non-nociplastic musculoskeletal pain. Methods This is an analytical, cross-sectional study. The sample comprised 30 patients receiving ambulatory physiotherapy treatment. Pain was classified as NP or NNP according to the International Association for the Study of Pain categorization system. The PA level was measured with the International Physical Activity Questionnaire–Short Form, and the PF level was measured with the hand grip strength test (HGS) to assess upper limb strength, the five Repetition Sit-to-Stand Test (5R-SRTS) to assess lower limb strength and power, and the YMCA 3 Min Step Test (YMCA-3MST) to estimate peak VO 2 . The results were compared with independent samples t-tests (with p < 0.05 considered significant). Cohen’s d was calculated to determine the effect size. Results The NP group reported a significantly lower PA level than the NNP group, specifically the vigorous PA ( p = 0.0009), moderate PA ( p = 0.0002), and total PA ( p = 0.005) dimensions. The NP group also showed significantly lower 5R-STS ( p = 0.000) and HGS ( p = 0.002) results compared with the NNP group. There were no significant differences in the YMCA-3MST between the NP and NNP groups ( p = 0.635). Conclusion It is possible that the neurophysiological and neuromuscular changes related to NP are associated with a reduced ability to perform vigorous PA. Clinicians should identify the presence of NP comorbidities in conjunction with the diagnosis when establishing the therapeutic goals.

Abstract:OBJECTIVES This study aims to evaluate the effect of intra-articular tranexamic acid (TA) administration on the amount of blood in the drains, the amount of blood transfusion, length of hospital stay, and the total cost in total knee arthroplasty (TKA) patients. PATIENTS AND METHODS The study included a total of 118 patients (24 males, 94 females; mean age 67.0 years, range 52 to 81 years) who underwent primary unilateral TKA surgery. Patients were divided into two groups as 58 patients who were performed intra-articular injection of 2 g TA after closure of articular capsule (TA group; 10 males, 48 females, mean age 65.6 years; range 55 to 80 years) and 60 patients who were not performed any intra-articular injection (non-TA group; 14 males, 46 females; mean age 68.3 years; range 52 to 81 years). The maximum difference between pre- and postoperative hemoglobin levels, the amount of blood in the drains, the amount of blood transfusion, length of hospital stay, and cost of each patient were recorded. RESULTS The mean difference between pre- and postoperative hemoglobin levels were higher in the non-TA group (1.7 g/dL vs 2.6 g/dL, p<0.05). The mean amounts of blood in drains (327.5 mL vs 800.0 mL, p<0.05), the amounts of blood transfusions (0 units vs 10 units, p<0.05), the mean lengths of hospital stay (4.03 days vs 4.53 days, p<0.05), and the hospital costs (1,935.26 United States dollars vs 1,959.64 United States dollars, p<0.05) were lower in TA group. CONCLUSION Administration of intra-articular TA during primary TKA may reduce blood loss and the need for blood transfusion while significantly shortening the postoperative length of hospital stay and reducing the total hospital costs.

Altered hip and knee kinematics and increases in weekly distance and running pace are often associated with PFP development and exacerbation.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Research question</h3><p>Are altered movements and training load characteristics (weekly distance and running pace) relate to pain intensity or physical function level in runners with PFP?</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Forty recreational runners with PFP (20 males and 20 females) participated in this cross-sectional observational study. Three-dimensional hip and knee kinematics were quantified during the stance phase of running. Weekly distance was defined as the average weekly kilometers of running and running pace as the average pace of the activity measured as minutes per kilometer. A visual analogue scale was used to evaluate worst knee pain during the last week. The anterior knee pain scale (AKPS) was used to evaluate knee functional score. A Pearson correlation matrix was used to investigate the association between each dependent variable (worst pain in the last week and AKPS score) and the independent variables (knee and hip kinematics, weekly distance and running pace)</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>There was no significantly correlation between kinematic variables, pain and functional score for both males and females separately and combined. Weekly distance (km/week) was found to positively correlate to pain intensity (r = 0.452; p < 0.05) in females with PFP. A simple linear regression revealed that weekly distance was significant predictor emerged of pain in females with PFP. Females exhibited significantly greater peak hip adduction and hip adduction ROM than the males and males had significantly greater running pace compared to females.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Significance</h3><p>Weekly distance should be considered in the clinical context during rehabilitation of PFP in females runners aiming at pain reduction.</p>

Abstract:Objective To investigate the relationship between accelerometer-measured physical activity and real-time pain in individuals with femoroacetabular impingement syndrome (FAIS). We tested the hypothesis that increased duration of high intensity activity would contribute to momentary increases in pain Design Observational study Setting Participants' natural, day-to-day environment. Participants Population-based sample of 33 individuals with unilateral FAIS. Important eligibility criteria included no concomitant hip disorders or previous hip surgery. Key sociodemographic features include that all participants were required to have a smartphone. Interventions Not applicable. Main Outcome Measures Duration and intensity of physical activity as measured by a waist-worn accelerometer, and instantaneous pain reported in real-time smartphone-based ecological momentary assessment surveys. Physical activity variables included each person's average sedentary time, light physical activity (LPA) and moderate to vigorous physical activity (MVPA) in the 90 minutes proceeding all pain surveys as well as fluctuation in sedentary, LPA, and MVPA above or below average prior to each individual survey. Results Linear mixed models revealed that the significant predictors of pain included fluctuation in sedentary time (B=-0.031, p<.001), average LPA (B=0.26, p=.035), and the interaction between fluctuation in LPA and fluctuation in MVPA (B=0.001, p<.001). Fluctuation in sedentary time above a person's average was associated with lower pain, while average LPA and fluctuations above average in both LPA and MVPA were associated with higher pain. Conclusions These results suggest that individuals with FAIS can engage in health-enhancing MVPA but should focus on avoiding concurrent increase above average in both high intensity and light physical activity in the same 90 minute period. Future work is warranted testing the efficacy of such an approach on pain. Additionally, given that high levels of LPA may arise from a host of socioeconomic factors, additional research is needed to disentangle the impact of LPA on pain in FAIS.

The aim of this study was to monitor physical activity, assessed by the number of steps per day, over time in people with CMSP and identify factors that could be associated with this activity feature. Methods: This prospective study involved people undergoing rehabilitation following an orthopedic trauma that had led to CMSP. At entry, participants completed self-reported questionnaires assessing pain, anxiety, depression, catastrophyzing, kinesiophobia, and behavioural activity patterns (avoidance, pacing and overdoing). They also underwent functional tests, assessing walking endurance and physical fitness. To determine daily step counts, participants wore an accelerometer for 1 week during rehabilitation and 3 months post-rehabilitation. The number of steps per day was compared among three time points: weekend of rehabilitation (an estimate of pre-rehabilitation activity; T1), weekdays of rehabilitation (T2), and post-rehabilitation (T3). Linear regression models were used to analyze the association between daily steps at T2 and at T3 and self-reported and performance-based parameters. Results: Data from 145 participants were analyzed. The mean number of steps was significantly higher during T2 than T1 and T3 (7323 [3047] vs. 4782 [2689], p < 0.001, Cohen's d = 0.769, and 4757 [2680], p < 0.001, Cohen's d = 0.693), whereas T1 and T3 results were similar (p = 0.92, Cohen's d = 0.008). Correlations of number of steps per day among time points were low (r ≤ 0.4). Multivariable regression models revealed an association between daily steps at T2 and pain interfering with walking, anxiety and overdoing behaviour. Daily steps at T3 were associated with overdoing behaviour and physical fitness. Conclusions: Despite chronic pain, people in rehabilitation after an orthopedic trauma increased their physical activity if they were given incentives to do so. When these incentives disappeared, most people returned to their previous activity levels. A multimodal follow-up approach could include both therapeutic and environmental incentives to help maintain physical activity in this population.

Abstract:Background Patellofemoral pain (PFP) is characterized by chronic pain in the anterior aspect of the knee during loading activities. Many studies investigating muscle morphology changes for individuals with PFP focus on the proximal joints, however, few studies have investigated muscles of the foot and ankle complex. This study aimed to explore the differences in peroneal muscle size and activation between individuals with PFP and healthy controls using ultrasound imaging in weight‐bearing. Methods A case‐control study in a university lab setting was conducted. Thirty individuals with PFP (age: 20.23 ± 3.30 years, mass: 74.70 ± 27.63 kgs, height: 161.32 ± 11.72 cm) and 30 healthy individuals (age: 20.33 ± 3.37 years, mass: 64.02 ± 11.00 kgs, height: 169.31 ± 9.30 cm) participated. Cross‐sectional area (CSA) images of the peroneal muscles were taken in non‐weight bearing and weight‐bearing positions. The functional activation ratio from lying to single‐leg standing (SLS) was calculated. Results There was a statistically significant (p = 0.041) group (PFP, healthy) by position (non‐weight‐bearing, weight‐bearing) interaction for the peroneal muscle CSA with a Cohen's d effect size of 0.2 in non‐weight‐bearing position and 0.7 in weight‐bearing position. The functional activation ratio for the healthy group was significantly more (p = 0.01) than the PFP group. Conclusion Peroneal muscles were found to be smaller in size in those with PFP compared to the healthy subjects in the weight‐bearing SLS position. This study found that those with PFP have lower activation of peroneal muscles in functional position.

Abstract:Abstract Objective To investigate the effects of core strengthening exercises on pain, mobility, and lower extremity muscle strength in patients with Patellofemoral Pain Syndrome (PFP). Design Six databases were searched from inception until August 11, 2023. Pain, function and muscle strength related outcomes were extracted and the quality of the studies was assessed using the PEDro scale and the level of evidence was assessed using the GRADE. Results Nineteen studies involving 1138 patients were included. Very low-grade evidence supported the short-term pain-relieving effect of core training (SMD = -0.60 95% CI [-0.95, -0.25]), high-grade evidence supported the short-term functional improvement effects of core training (WMD = 3.61 95% CI [1.44, 5.78]), which was similarly significant within 3-12 months of follow-up. The results of the subgroup analyses suggested that hip-knee training was most advantageous in relieving pain and enhancing motor function. Conclusions Although training that includes trunk core is clearly superior to knee strengthening alone, the effectiveness of hip-knee training, which is also a core training program for pain and function, is more pronounced. The available evidence supports that hip-knee training is the most valuable treatment option for patients with PFP.

Abstract:Background: The distance between the tibial tubercle (TT) and trochlear groove (TT-TG distance) is known to be greater in patients with patellar instability. However, the potential role and prevalence of pathological TT-TG distances in a large cohort of skeletally mature patients with isolated patellofemoral pain (PFP) are not clear. Purpose: To determine if the mean TT-TG distance is greater in patients with PFP, who lack a history of patellar dislocations, knee trauma, or osteoarthritis, relative to healthy controls. Study Design: Cross-sectional study; Level of evidence, 3. Methods: A total of 50 knees (38 patients) with PFP and 60 knees (56 controls) without PFP formed the basis of this study. Magnetic resonance imaging was used to determine the TT-TG distance from 3-dimensional static scans. Results: The cohort with PFP demonstrated a significantly greater mean TT-TG distance relative to asymptomatic controls (13.0 vs 10.8 mm, respectively; P = .001). Among the cohort with PFP, 15 knees (30%) demonstrated TT-TG distances ≥15 mm, and 3 knees (6%) demonstrated TT-TG distances ≥20 mm. Conclusion: Most adult patients with isolated PFP have elevated TT-TG distances compared with controls, which likely contributes to the force imbalance surrounding the knee.

Abstract:Background: Patellar maltracking is widely accepted as an underlying mechanism of patellofemoral pain. However, methodological differences in the literature hinder our ability to generate a universal quantitative definition of pathological patellofemoral kinematics (patellar maltracking) in patellofemoral pain, leaving us unable to determine the cause of patellofemoral pain. Purpose: To systematically review the literature to provide evidence regarding the influence of confounding variables on patellofemoral kinematics. Study Design: Systematic review and random effects meta-analysis of control-case studies. Methods: A literature search of case-control studies that evaluated patellofemoral kinematics at or near full extension and were written in English was conducted using Embase, PubMed, Scopus, and Web of Science up to September 2019. Cases were defined as patients with patellofemoral pain. Studies were eliminated if they lacked quantitative findings; had a primary aim to assess therapy efficacy; or included participants with osteoarthritis and/or previous trauma, pathology, or surgery. A quality assessment checklist was employed to evaluate each study. Meta-analyses were conducted to determine the influence of confounding variables on measures of patellofemoral kinematics. Results: Forty studies met the selection criteria, with quality scores ranging from 13% to 81%. Patient characteristics, data acquisition, and measurement methods were the primary sources of methodological variability. Active quadriceps significantly increased lateral shift (standardized mean difference [SMD] shift = 0.33; P = .0102) and lateral tilt (SMD tilt = 0.43; P = .006) maltracking. Individuals with pain secondary to dislocation had greater effect sizes for lateral maltracking than had those with isolated patellofemoral pain (ΔSMD shift = 0.71, P = .0071; ΔSMD tilt = 1.38, P = .0055). Conclusion: This review exposed large methodological variability across the literature, which not only hinders the generalization of results, but ultimately mitigates our understanding of the underlying mechanism of patellofemoral pain. Although our meta-analyses support the diagnostic value of maltracking in patellofemoral pain, the numerous distinct methods for measuring maltracking and the limited control for cofounding variables across the literature prohibit defining a single quantitative profile. Compliance with specific standards for anatomic and outcome measures must be addressed by the scientific and clinical community to establish methodological uniformity in this field.
The Effects of Exercise on Decreasing Pain and Increasing Function in Patients With Patellofemoral Pain Syndrome

Abstract:Context: Exercise or rest is commonly prescribed as treatment for patellofemoral pain syndrome. Study Selection: This study is based on Level I or II research studies examining the effects of exercise and rest on decreasing pain (visual analog scale) and increasing function (Kujala Scoring Questionnaire) using human participants. Articles were limited to those printed in English from PubMed (1966–September 2010), CINAHL (1982–September 2010), and SPORTDiscus (1972–September 2010). Data Extraction: Weighted aggregate effect sizes and 95% confidence intervals were calculated from means and standard deviations extracted from 10 studies, resulting in an analysis of 433 patients. Results: A very large effect for exercise was found for patient-reported functional outcomes ( d = 2.19) and perceived pain ( d = −1.24) in treated patients, which were larger than functional outcomes ( d = 0.77) and pain ( d = −0.14) in controls. Short-term follow-up of 191 patients from 4 data sets in 2 studies revealed a large effect for functional outcomes ( d = 1.04) and pain ( d = −0.82) in patients who performed an exercise intervention. One study reported moderate effect sizes for functional outcomes ( d = 0.59) and pain ( d = −0.35) at 3 months postintervention. Conclusions: Exercise is the more effective treatment for immediate decrease in pain and increase in function although these differences appear to be less distinguishable over time.
