Abstract:Abstract Hypoxia-inducible factors (HIFs) are the key regulators of oxygen homeostasis in response to hypoxia. In diabetes, multiple tissues are hypoxic but adaptive responses to hypoxia are impaired due to insufficient activation of HIF signalling, which results from inhibition of HIF-1α stability and function due to hyperglycaemia and elevated fatty acid levels. In this review, we will summarise and discuss current findings about the regulation of HIF signalling in diabetes and the pathogenic roles of hypoxia and dysregulated HIF signalling in the development of diabetes and its complications. The therapeutic potential of targeting HIF signalling for the prevention and treatment of diabetes and related complications is also discussed. Graphical abstract

Abstract:Type 2 diabetes is a chronic disease marked by hyperglycemia; impaired insulin secretion by pancreatic β-cells is a hallmark of this disease. Recent studies have shown that hypoxia occurs in the β-cells of patients with type 2 diabetes and hypoxia, in turn, contributes to the insulin secretion defect and β-cell loss through various mechanisms, including the activation of hypoxia-inducible factors, induction of transcriptional repressors, and activation of AMP-activated protein kinase. This review focuses on advances in our understanding of the contribution of β-cell hypoxia to the development of β-cell dysfunction in type 2 diabetes. A better understanding of β-cell hypoxia might be useful in the development of new strategies for treating type 2 diabetes.

Abstract:Type 2 diabetes (T2D) is caused by insulin resistance and insufficient insulin secretion. Evidence has increasingly indicated that pancreatic β-cell dysfunction is the primary determinant of T2D disease progression and remission. High plasticity is an important feature of pancreatic β-cells. During T2D development, pancreatic β-cells undergo dynamic adaptation. Although β-cell death/apoptosis in later-stage T2D is the major cause of β-cell dysfunction, recent studies have revealed that β-cell dedifferentiation and reprogramming, which play critical roles in β-cell functional regulation in the early and middle T2D progression stages, are characterized by (i) a loss of mature β-cell-enriched genes; (ii) dedifferentiation to a progenitor-like state; and (iii) transdifferentiation into other cell types. The roles of transcription factors (TFs) in the establishment and maintenance of β-cell identity during pancreatic development have been extensively studied. Here, we summarize the roles and underlying mechanisms of TFs in the maintenance of β-cell identity under physiological and type 2 diabetic conditions. Several feasible approaches for restoring islet functions are also discussed. A better understanding of the transcriptional control of β-cell identity and plasticity will pave the way for developing more effective strategies, such as β-cell regeneration therapy, to treat T2D and associated metabolic disorders.
Repression of hypoxia-inducible factor-1 contributes to increased mitochondrial reactive oxygen species production in diabetes

Abstract:Excessive production of mitochondrial reactive oxygen species (ROS) is a central mechanism for the development of diabetes complications. Recently, hypoxia has been identified to play an additional pathogenic role in diabetes. In this study, we hypothesized that ROS overproduction was secondary to the impaired responses to hypoxia due to the inhibition of hypoxia-inducible factor-1 (HIF-1) by hyperglycemia. The ROS levels were analyzed in the blood of healthy subjects and individuals with type 1 diabetes after exposure to hypoxia. The relation between HIF-1, glucose levels, ROS production and its functional consequences were analyzed in renal mIMCD-3 cells and in kidneys of mouse models of diabetes. Exposure to hypoxia increased circulating ROS in subjects with diabetes, but not in subjects without diabetes. High glucose concentrations repressed HIF-1 both in hypoxic cells and in kidneys of animals with diabetes, through a HIF prolyl-hydroxylase (PHD)-dependent mechanism. The impaired HIF-1 signaling contributed to excess production of mitochondrial ROS through increased mitochondrial respiration that was mediated by Pyruvate dehydrogenase kinase 1 (PDK1). The restoration of HIF-1 function attenuated ROS overproduction despite persistent hyperglycemia, and conferred protection against apoptosis and renal injury in diabetes. We conclude that the repression of HIF-1 plays a central role in mitochondrial ROS overproduction in diabetes and is a potential therapeutic target for diabetic complications. These findings are timely since the first PHD inhibitor that can activate HIF-1 has been newly approved for clinical use. Abstract:Type 1 diabetes (T1D) results in immune-mediated destruction of insulin-producing beta cells I the pancreas. B cells have been identified as critical antigen presenting cells and their specificity drives disease progression. More recently, they have also been shown to have the capacity to develop into suppressive, regulatory B cells that ameliorate autoimmune disease in animal models of rheumatoid arthritis and multiple sclerosis. In these models, signaling through hypoxia-inducible factor 1α (HIF-1α) drives a glycolytic flux that facilitates expansion of regulatory B cells. Here we examine the relationship between B cell development, cellular metabolism, and HIF-1α to reveal that in a mouse model of autoimmune diabetes, B cells have distinct metabolic characteristics that change with disease progression. Further, response to hypoxia in autoimmune B cells is distinct from the response by non-autoimmune control B cells. Together, these data suggest that dysregulated HIF signaling may drive T1D progression and activation of HIF-1α to expand regulatory B cell populations may be a viable option for immune modulation.

Abstract:Hypoxia-inducible factors (HIFs), a family of transcription factors activated by hypoxia, consist of three α-subunits (HIF1α, HIF2α and HIF3α) and one β-subunit (HIF1β), which serves as a heterodimerization partner of the HIFα subunits. HIFα subunits are stabilized from constitutive degradation by hypoxia largely through lowering the activity of the oxygen-dependent prolyl hydroxylases that hydroxylate HIFα, leading to their proteolysis. HIF1α and HIF2α are expressed in different tissues and regulate target genes involved in angiogenesis, cell proliferation and inflammation, and their expression is associated with different disease states. HIFs have been widely studied because of their involvement in cancer, and HIF2α-specific inhibitors are being investigated in clinical trials for the treatment of kidney cancer. Although cancer has been the major focus of research on HIF, evidence has emerged that this pathway has a major role in the control of metabolism and influences metabolic diseases such as obesity, type 2 diabetes mellitus and non-alcoholic fatty liver disease. Notably increased HIF1α and HIF2α signalling in adipose tissue and small intestine, respectively, promotes metabolic diseases in diet-induced disease models. Inhibition of HIF1α and HIF2α decreases the adverse diet-induced metabolic phenotypes, suggesting that they could be drug targets for the treatment of metabolic diseases.

Abstract:Type 1 diabetes (T1D), which prevalence is currently on the rise, is a major risk factor for cardiovascular disease (CVD). Although T1D induces endothelial dysfunction, a precursor and contributor to CVD, its etiology remains ill-defined. Aberrant increases in endothelial cell (EC) glycolysis mainly via increased expression/ activity of its regulatory enzyme, 6-phosphofructo-2- kinase/fructose-2, 6-bisphosphatase 3 (PFKFB3) contribute to several vascular disorders. Nevertheless, whether alterations in EC glycolysis impairs endothelium dependent relaxation (EDR) is unknown. Using Akita mice, a genetic model of T1D, and a newly developed assay to measure EC glycolytic capacity in aortic explants via Seahorse analyzer, we tested the hypothesis that T1D impairs endothelial function via increasing EC glycolysis. Aortic explants from Akita mice exhibited a 1.7-fold increase in glycolysis to WT (P<0.05). EC-denudation abolished these increases which identify EC as the source of increased vascular glycolysis. These metabolic changes were accompanied by a 2-fold increase in EC PFKFB3 expression in aortas of Akita mice and inhibition of PFKFB3 using 3PO restored EDR in Akita mice. Interestingly, increases in EC glycolysis in situ via transduction of adenoviral vectors to overexpress either PFKFB3 (Ad-PFKFB3) or a constitutively active form of PFK2 to drive glycolysis independent of PFKFB3 (Ad-GlycoHi) reproduced the endothelial dysfunction associated with T1D. Also, EC transduced in vitro with Ad-PFKFB3 depicted increased expression of the ROS–producing enzyme, NADPH oxidase homolog, Nox1, and its co-activator, NoxA1(p<0.05). Similarly, aorta EC from Akita mice revealed increased Nox1 and NoxA1 expression. Inhibition of Nox1 fully restored EDR in Akita and Ad-PFKFB3-transduced WT aortas. Consistently, aortas from Nox1 KO mice were protected from Ad-PFKFB3-induced endothelial dysfunction. T1D markedly increased the aortic expression of the Advanced Glycation End products (AGE) precursor methylglyoxal (MG) (p<0.05), thus, we tested the contribution of AGE to T1D-induced EC glycolysis. Aortic rings exposed to MG showed increased EC PFKFB3 and Nox1 expression and impaired EDR. The latter was restored in rings incubated with 3PO or in rings from EC-specific PFKFB3 deficient mice (p<0.05). Also, T1D and exposure to MG upregulated hypoxia-inducible factor 1α(HIF1α) expression in EC. HIF1α inhibition blunted MG-mediated PFKFB3 upregulation in aorta EC (p<0.05). In Conclusion, this study identified for the first time a role for endothelial PFKFB3-mediated glycolysis in T1D-induced endothelial dysfunction. The underlying mechanism involves AGE and HIF1α as upstream regulators and NOX1 as a downstream target for PFKFB3. Abstract:BACKGROUND:Type 1 diabetes (T1D) is a major cause of endothelial dysfunction. Although cellular bioenergetics has been identified as a new regulator of vascular function, whether glycolysis, the primary bioenergetic pathway in endothelial cells (EC), regulates vascular tone and contributes to impaired endothelium-dependent relaxation (EDR) in T1D remains unknown.METHODS:Experiments were conducted in Akita mice with intact or selective deficiency in EC PFKFB3 (6-phosphofructo-2-kinase/fructose-2,6-bisphosphatase 3), the main regulator of glycolysis. Seahorse analyzer and myography were employed to measure glycolysis and mitochondrial respiration, and EDR, respectively, in aortic explants. EC PFKFB3 (Ad-PFKFB3) and glycolysis (Ad-GlycoHi) were increased in situ via adenoviral transduction.RESULTS:T1D increased EC glycolysis and elevated EC expression of PFKFB3 and NADPH oxidase Nox1 (NADPH oxidase homolog 1). Functionally, pharmacological and genetic inhibition of PFKFB3 restored EDR in T1D, while in situ aorta EC transduction with Ad-PFKFB3 or Ad-GlycoHi reproduced the impaired EDR associated with T1D. Nox1 inhibition restored EDR in aortic rings from Akita mice, as well as in Ad-PFKFB3-transduced aorta EC and lactate-treated wild-type aortas. T1D increased the expression of the advanced glycation end product precursor methylglyoxal in the aortas. Exposure of the aortas to methylglyoxal impaired EDR, which was prevented by PFKFB3 inhibition. T1D and exposure to methylglyoxal increased EC expression of HIF1α (hypoxia-inducible factor 1α), whose inhibition blunted methylglyoxal-mediated EC PFKFB3 upregulation.CONCLUSIONS:EC bioenergetics, namely glycolysis, is a new regulator of vasomotion and excess glycolysis, a novel mechanism of endothelial dysfunction in T1D. We introduce excess methylglyoxal, HIF1α, and PFKFB3 as major effectors in T1D-mediated increased EC glycolysis.

Abstract:Type 1 diabetes mellitus is a disease involving changes to energy metabolism. Chronic hyperglycemia is a major cause of diabetes complications. Hyperglycemia induces mechanisms that generate the excessive production of reactive oxygen species, leading to the development of oxidative stress. Studies with animal models have indicated the involvement of mitochondrial dysfunction in the pathogenesis of diabetic cardiomyopathy. In the current review, we aimed to collect scientific reports linking disorders in mitochondrial functioning with the development of diabetic cardiomyopathy in type 1 diabetes mellitus. We also aimed to present therapeutic approaches counteracting the development of mitochondrial dysfunction and diabetic cardiomyopathy in type 1 diabetes mellitus.
HIF-1α/FOXO1 axis regulated autophagy is protective for β cell survival under hypoxia in human islets

Abstract:β cells suffer from hypoxia due to the rapid metabolic rate to supply insulin production. Mechanistic study of β cell survival under hypoxia may shed light on the β cell mass loss in type 2 diabetes mellitus (T2DM). Here, we found that the expressions of LC3 and p62/SQSTM1, two key autophagy regulators, were significantly higher in β cells than that in non-β endocrine cells in both non-diabetic and T2DM human pancreases, and the autophagy process was accelerated upon Cobalt Chloride (CoCl2) treatment in ex vivo cultured primary human islets. Meanwhile, CoCl2 induced the upregulation of FOXO1 in human islets, where HIF-1α played a key role. CoCl2 treatment caused the increase of β cell apoptosis, yet inhibiting autophagy by Chloroquine or by FOXO1 knockdown further aggravated apoptosis, suggesting that FOXO1-regulated autophagy is protective for β cell survival under hypoxia. Immunofluorescence staining showed that LC3 and p62/SQSTM1 expressions were significantly decreased in T2DM patients and negatively correlated with HbA1c, indicating that the autophagy capacity of β cells is impaired along with the progression of the disease. Our study revealed that HIF-1α/FOXO1 regulated autophagy benefits β cell survival under hypoxia and autophagy dysregulation may account for β cell mass loss in T2DM. BRIEF SUMMARY: Our study revealed that HIF-1α/FOXO1 regulated autophagy benefits β cell survival under hypoxia and autophagy dysregulation may account for β cell mass loss in T2DM.

Abstract:Hypoxia-inducible factor 1 alpha (HIF-1 alpha), an essential transcription factor which mediates the adaptation of cells to low oxygen tensions, is regulated precisely by hypoxia and hyperglycemia, which are major determinants of the chronic complications associated with diabetes. The process of HIF-1 alpha stabilization by hypoxia is clear; however, the mechanisms underlying the potential deleterious effect of hyperglycemia on HIF-1 alpha are still controversial, despite reports of a variety of studies demonstrating the existence of this phenomenon. In fact, HIF-1 alpha and glucose can sometimes influence each other: HIF-1 alpha induces the expression of glycolytic enzymes and glucose metabolism affects HIF-1 alpha accumulation in some cells. Although hyperglycemia upregulates HIF-1 alpha signaling in some specific cell types, we emphasize the inhibition of HIF-1 alpha by high glucose in this review. With regard to the mechanisms of HIF-1 alpha impairment, the role of methylglyoxal in impairment of HIF-1 alpha stabilization and transactivation ability and the negative effect of reactive oxygen species (ROS) on HIF-1 alpha are discussed. Other explanations for the inhibition of HIF-1 alpha by high glucose exist: the increased sensitivity of HIF-1 alpha to Von Hippel-Lindau (VHL) machinery, the role of osmolarity and proteasome activity, and the participation of several molecules. This review aims to summarize several important developments regarding these mechanisms and to discuss potentially effective therapeutic techniques (antioxidants eicosapentaenoic acid (EPA) and metallothioneins (MTs), pharmaceuticals cobalt chloride (CoCl2), dimethyloxalylglycine (DMOG), desferrioxamine (DFO) and gene transfer of constitutively active forms of HIF-1 alpha) and their mechanisms of action for intervention in the chronic complications in diabetes.

Abstract:Blood flow of the superior (SVC) and inferior venae cavae (IVC) was simultaneously and separately measured with two cannulating type electromagnetic blood flow probes inserted into the SVC and IVC of thirty two dogs during hypotensive and low cardiac output states in hemorrhagic shock and cardiogenic shock. Blood flow ratio of teh IVC to total venous return was expressed by IVCF/CO (per cent). It was 66.3 per cent in normal condition, but was decreased to 54.3 per cent in hemorrhagic shock. In four cardiogenic shock models, however, IVCF/CO changes little from the control to the initial phase of shock state. At least in part, the marked decrease of venous return from the IVC region in hemorrhagic shock was considered due to the increase of sympathetic nerve activity in that region mediated by baro-receptors in the high pressure system. The unchanged blood flow ratio of the SVC and IVC in cardiogenic shock was assumed that the high atrial or high ventricular endodiastolic pressure was an inhibitory factor of the sympathetic nerve activity by activating the stretch receptors existing the low pressure system including left ventricle. This hypothesis was proved by the experiments in which the renal vascular resistance was suppressed by stretch of the left atrium by either extraatrial mechanical traction or intraatrial balloon inflation in hemorrhagic shock or intracardiac decompression during cardiopulmonary by pass.

Abstract:Patients with type 1 diabetes mellitus (T1DM) experience multiple episodes of hypoglycemia, resulting in dysfunctional counter-regulatory responses with time. The recent experimental study by Jin et al explored the role of intestinal glucagon-like peptide-1 (GLP-1) in impaired counter-regulatory responses to hypoglycemia. They identified intestinal GLP-1 along with GLP-1 receptor (GLP-1R) as the new key players linked with impaired counter-regulatory responses to hypoglycemia in type 1 diabetic mice. They also demonstrated that excessive expression of GLP-1 and GLP-1R was associated with attenuated sympathoadrenal responses and decreased glucagon secretion. The study has enormous clinical relevance as defective counter regulation and hypoglycemia unawareness negatively impacts the intensive glycemic management approach in this group of patients. However, the physiological processes must be validated in dedicated human studies to comprehensively understand the pathophysiology of this complex relationship, and to clarify the true extent of impaired hypoglycemia counter regulation by intestinal GLP-1. For now, following the results of the index study and other similar studies, GLP-1 analogues usage in T1DM must be carefully monitored, as there is an inherent risk of worsening the already impaired counter-regulatory responses in these patients. Further studies in the future could identify other key players involved in this clinically relevant interaction.

Abstract:This study critically examines the novel findings presented by Jin et al , which explores the role of intestinal glucagon-like peptide-1 (GLP-1) in impaired counterregulatory responses to hypoglycemia in mice with type 1 diabetes. The study identifies intestinal GLP-1 as a significant determinant in the physiological responses to hypoglycemia, offering new insights into its potential implications for diabetes management. The editorial synthesizes these findings, discusses their relevance in the context of current diabetes research, and outlines potential avenues for future investigation of intestinal GLP-1 as a therapeutic target. This analysis underscores the need for continued research into the complex mechanisms underlying impaired hypoglycemia responses and highlights the potential of targeting intestinal GLP-1 pathways in therapeutic strategies for type 1 diabetes.

Third eyelid resection as a treatment for suspected squamous cell carcinoma in 24 horses

Abstract:Between October 2000 and January 2007, 24 horses were presented with suspected squamous cell carcinoma (SCC) of the third eyelid. The hospital’s medical records were analysed retrospectively to gain data about the cases, and telephone follow-up was obtained from the owners and referring veterinary surgeons. The resected third eyelid was submitted for histological examination in 21 cases; in the other three cases the tissue was not submitted at the owners’ request, for economic reasons. SCC was confirmed in 16 of these 21 cases, three cases were diagnosed histologically as lymphoid hyperplasia, one as a mast cell tumour and one as a sebaceous gland adenocarcinoma. Long-term follow up over a median period of 41 months revealed no recurrence of SCC or associated problems; six of the 16 confirmed SCC cases were euthanased for unrelated reasons during the follow-up period.
Hypoxia-inducible factor 1a protects peripheral sensory neurons from diabetic peripheral neuropathy by suppressing accumulation of reactive oxygen species

Abstract:Diabetic peripheral neuropathy (DPN) is one of the most common diabetic complications. Mechanisms underlying nerve damage and sensory loss following metabolic dysfunction remain large unclear. Recently, hyperglycemia-induced mitochondrial dysfunction and the generation of ROS have gained attention as possible mechanisms of organ damage in diabetes. Hypoxia-inducible factor 1(HIF1a) is a key transcription factor activated by hypoxia, hyperglycemia, nitric oxide as well as ROS, suggesting a fundamental role in DPN susceptibility. Genetically-modified mutant mice, which conditionally lack HIF1a in peripheral sensory neurons (SNS-HIF1a-/-), were analyzed longitudinally up to 6 months in the streptozotocin (STZ) model of type1 diabetes. Behavioral measurements of sensitivity to thermal and mechanical stimuli, quantitative morphological analyses of intraepidermal nerve fiber density and measurements of reactive oxygen species (ROS) in sensory neurons in vivo were undertaken over several months post-STZ injections to delineate the role of HIF1a in DPN. Longitudinal behavioral and morphological analyses at 5, 13 and 24 wks post-STZ treatment revealed that SNS-HIF1a-/- developed stronger hyperglycemia-evoked losses of peripheral nociceptive sensory axons associated with stronger losses of mechano- and heat sensation with a faster onset than HIF1afl/fl mice. Mechanistically, these histomorphologic and behavioral differences were associated with significantly higher level of STZ-induced production of ROS in sensory neurons of SNS-HIF1a-/- mice as compared with HIF1afl/fl. Our results indicate that HIF1a is as an upstream modulator of ROS in peripheral sensory neurons and exerts a protective function in suppressing hyperglycemia-induced nerve damage by limiting ROS levels. HIF1a stabilization may be thus a new strategy target for limiting sensory loss, a debilitating late complication of diabetes.
Glycolysis Inhibition Induces Functional and Metabolic Exhaustion of CD4+ T Cells in Type 1 Diabetes

Abstract:In Type 1 Diabetes (T1D), CD4+ T cells initiate autoimmune attack of pancreatic islet β cells. Importantly, bioenergetic programs dictate T cell function, with specific pathways required for progression through the T cell lifecycle. During activation, CD4+ T cells undergo metabolic reprogramming to the less efficient aerobic glycolysis, similarly to highly proliferative cancer cells. In an effort to limit tumor growth in cancer, use of glycolytic inhibitors have been successfully employed in preclinical and clinical studies. This strategy has also been utilized to suppress T cell responses in autoimmune diseases like Systemic Lupus Erythematosus (SLE), Multiple Sclerosis (MS), and Rheumatoid Arthritis (RA). However, modulating T cell metabolism in the context of T1D has remained an understudied therapeutic opportunity. In this study, we utilized the small molecule PFK15, a competitive inhibitor of the rate limiting glycolysis enzyme 6-phosphofructo-2-kinase/fructose-2,6- biphosphatase 3 (PFKFB3). Our results confirmed PFK15 inhibited glycolysis utilization by diabetogenic CD4+ T cells and reduced T cell responses to β cell antigen in vitro. In an adoptive transfer model of T1D, PFK15 treatment delayed diabetes onset, with 57% of animals remaining euglycemic at the end of the study period. Protection was due to induction of a hyporesponsive T cell phenotype, characterized by increased and sustained expression of the checkpoint molecules PD-1 and LAG-3 and downstream functional and metabolic exhaustion. Glycolysis inhibition terminally exhausted diabetogenic CD4+ T cells, which was irreversible through restimulation or checkpoint blockade in vitro and in vivo. In sum, our results demonstrate a novel therapeutic strategy to control aberrant T cell responses by exploiting the metabolic reprogramming of these cells during T1D. Moreover, the data presented here highlight a key role for nutrient availability in fueling T cell function and has implications in our understanding of T cell biology in chronic infection, cancer, and autoimmunity.

Abstract:Type 1 diabetes (T1D) is a chronic autoimmune disease in which destruction or damaging of the beta-cells in the islets of Langerhans results in insulin deficiency and hyperglycemia. We only know for sure that autoimmunity is the predominant effector mechanism of T1D, but may not be its primary cause. T1D precipitates in genetically susceptible individuals, very likely as a result of an environmental trigger. Current genetic data point towards the following genes as susceptibility genes: HLA, insulin, PTPN22, IL2Ra, and CTLA4. Epidemiological and other studies suggest a triggering role for enteroviruses, while other microorganisms might provide protection. Efficacious prevention of T1D will require detection of the earliest events in the process. So far, autoantibodies are most widely used as serum biomarker, but T-cell readouts and metabolome studies might strengthen and bring forward diagnosis. Current preventive clinical trials mostly focus on environmental triggers. Therapeutic trials test the efficacy of antigen-specific and antigen-nonspecific immune interventions, but also include restoration of the affected beta-cell mass by islet transplantation, neogenesis and regeneration, and combinations thereof. In this comprehensive review, we explain the genetic, environmental, and immunological data underlying the prevention and intervention strategies to constrain T1D.
