Abstract:3586 Background: IBI351 (GFH925) is an irreversibly covalent inhibitor of KRASG12C and has demonstrated promising anti-tumor activity with acceptable safety in advanced solid tumors. Here, we report a pooled analysis of two phase I studies (NCT05005234, NCT05497336) evaluating the efficacy and safety of IBI351 (GFH925) monotherapy for metastatic colorectal cancer (CRC) harboring KRASG12C mutation. Methods: Eligible metastatic CRC patients (pts) with KRASG12C mutation were included. Pts received IBI351 (GFH925) orally at dose levels of 700mg once daily (QD), or 450/600/750mg twice daily (BID). The primary endpoint was objective response rate (ORR) assessed by investigator per RECIST v1.1. Data cutoff for the analyses was November 30, 2022 unless otherwise specified. Results: A total of 45 pts were enrolled (median age: 58.0 years; male: 57.8%; ECOG PS 1: 71.1%; pts with ≥2 prior lines of treatment: 66.7%), including 3, 4, 37, and 1 pts in 700mg QD, 450mg BID, 600mg BID, and 750mg BID cohorts, respectively. The median exposure to therapy was 84 days (range, 7-286), and 36 pts (80.0%) were still on treatment including one patient at 450mg BID with treatment exposure for 286 days. As of December 15, 2022, ORR was 47.5% (19/40, 95% CI: 31.5%-63.9%) and disease control rate (DCR) was 85.0% (95% CI: 70.2%-94.3%) for the efficacy-evaluable pts across all dose levels. The median duration of response (DOR) was not reached. For 32 efficacy-evaluable patients at 600mg BID, ORR and DCR were 43.8% (14/32) and 87.5%, respectively. Treatment-related adverse events (TRAEs) occurred in 40 (88.9%) pts, with the most common being anaemia, white blood cell count decreased, alanine aminotransferase increased, and pruritus. Grade ≥ 3 TRAEs occurred in 9 (20.0%) pts. No drug-related adverse events leading to treatment discontinuation or death occurred. Conclusions: IBI351 (GFH925) monotherapy demonstrated promising anti-tumor activity with manageable safety profile in metastatic CRC patients harboring KRASG12C mutation. Abstract:Background IBI351 is an irreversible and covalent inhibitor of KRAS G12C. Despite FDA approval of two KRAS G12C inhibitors, there are still significant unmet clinical needs in Chinese patients and ongoing concerns about the optimal dosage. Herein, we presented the phase Ia/Ib study of IBI351 monotherapy in Chinese patients with advanced solid tumors harboring KRAS G12C mutation. Methods In phase Ia dose escalation, IBI351 at 250/450/700/900 mg once daily and 450/600/750 mg twice daily (BID) were evaluated. Potentially efficacious doses and optimal recommended phase 2 dose (RP2D) were further evaluated in patients with advanced non-small cell lung cancer (NSCLC) in phase Ia dose expansion and phase Ib. Safety, pharmacokinetics, and investigator-assessed tumor response were evaluated. Results As of June 13, 2023, 176 patients were enrolled. IBI351 was well tolerated with no dose-limiting toxicity reported across all evaluated doses. The RP2D was determined as 600 mg BID by considering safety, efficacy and pharmacokinetics. A total of 168 patients (95.5 %) had at least one treatment-related adverse event (TRAE), and 64 patients (36.4 %) had grade 3 or higher TRAEs, most commonly gamma-glutamyl transferase increased (10.2 %) and anemia (6.8 %). For patients with NSCLC, the confirmed objective response rate (ORR) was 45.5 % across all doses. At 600 mg BID, the confirmed ORR was 46.8 % and median progression-free survival was 9.6 months with a median follow-up of 6.9 months. Conclusions IBI351 was well tolerated in patients with advanced solid tumors and showed promising antitumor activity in advanced NSCLC patients with KRAS G12C mutation.

Abstract:Introduction: D-1553 (garsorasib) is a potent and selective oral KRASG12C inhibitor. We report results from a phase I dose-escalation and dose-expansion study of D-1553 in patients with KRAS G12C-mutated NSCLC in multiple sites in the People's Republic of China. Methods: Patients with KRAS G12C-mutated NSCLC have administrated D-1553 600 mg orally once daily, 800 mg once daily, 1200 mg once daily, 400 mg twice a day, or 600 mg twice a day in dose escalation. In dose-expansion, all patients received 600 mg twice a day. The safety, pharma-cokinetics, and efficacy of D-1553 were evaluated.Results: Among a total of 79 treated patients, 75 patients (94.9%) reported treatment-related adverse events with 30 patients experiencing grade 3 or 4 events (38.0%). Most of the adverse events were manageable and the patients tolerated the study treatment well. Among 74 patients assessable for efficacy analysis, 30 patients had a partial response and 38 had stable disease with a confirmed objective response rate (ORR) and disease control rate (DCR) of 40.5% and 91.9%, respectively. The median progression -free survival was 8.2 months, and the median duration of response was 7.1 months. Among 62 patients assessable for response at the recommended phase 2 dose, partial response occurred in 24 patients (ORR, 38.7%) and stable disease in 32 patients (DCR, 90.3%). The median progression-free survival and duration of response were 7.6 months and 6.9 months, respectively. In patients with brain metastasis, ORR and DCR were 17% and 100%, respectively. Conclusions: D-1553 represents a promising therapeutic option for patients with KRAS G12C-mutated NSCLC with a well-tolerated safety profile and encouraging antitumor activity.& COPY; 2023 International Association for the Study of Lung Cancer. Abstract:8635 Background: The combination of immune checkpoint inhibitors (ICIs) and chemotherapy has shown high efficacy in non-small cell lung cancer (NSCLC) patients, but further exploration of the predictive biomarkers is needed. KRAS is the most common driver oncogene in patients with NSCLC. However, the influence of different KRAS mutation subtypes on immunotherapy response remains controversial. We aimed to evaluate the value of KRAS mutation subtypes for predicting response to immunotherapy in NSCLC patients and to further analyze the related tumor immune microenvironment (TiME) phenotypes. Methods: In this retrospective study, clinical data were extracted from the medical records of patients with advanced NSCLC treated with ICIs and whose available molecular analysis included KRAS mutation information between January 2017 and December 2022. NSCLC tissue samples were collected for TiME analysis. Differences in immune cell subsets between different groups were analyzed using the Mann‒Whitney U test. Clinical characteristics, ICI treatment efficacy, progression-free survival (PFS), and overall survival (OS) were analyzed using multivariate Cox models. Results: A total of 1913 eligible KRAS-mutated NSCLC patients were screened, and 137 patients who received systemic immunotherapy were enrolled (108 males vs 29 females), with a median age of 62 (range, 39-84) years. Among them, 126 (92.0%) patients had adenocarcinoma, 99 (72.3%) patients were systemic therapy naïve, and 124 (90.5%) patients received combined immunotherapy. Among the patients with KRAS mutations, 48 (35.0%) had G12C, 20 (14.6%) had G12V, 27 (19.7%) had G12D and 42 (30.7%) had other subtypes. PFS was significantly longer in the G12C subgroup than in the other mutation subgroup (median PFS: 11.3 vs 6.8 months, P = 0.001). The median PFS times in the G12C/G12V subgroup and the other groups were 11.1 months and 6.5 months, respectively ( P= 0.0005). Multivariate analysis of the immunotherapy cohort revealed that G12C/G12V was an independent predictor of PFS ( P = 0.003, hazard ratio (HR): 2.021, 95% CI: 1.265-3.227). Further TiME analysis suggested that the infiltration of myeloid-derived suppressor cells (MDSCs, CD33+CD11b+) in the tumor and stromal areas in the G12C/G12V subgroup was significantly lower than that in the other subgroup (tumor area, P= 0.045; stroma area, P= 0.015). However, no significant difference in PD-L1 expression was found between the G12C/G12V group and the other groups. Conclusions: KRAS G12C and G12V were associated with favorable combined ICI treatment efficacy in patients with NSCLC. Patients with G12C or G12V mutations had longer PFS and lower infiltration of MDSCs. KRAS mutation subtypes are potential predictive biomarkers disturbed by MDSCs for combination therapy in NSCLC patients.

Phase I dose escalation had an accelerated titration design for dose level 250mg QD and a BOIN design with 450/700/900mg QD. The primary end points were safety and tolerabilityThe secondary end points were pharmacokinetics (PK) and anti-tumor activity of IBI351(GFH925) monotherapy per RECIST v1.1. Results: As of Feb 07 2022, 15 pts (13 men, 2 women; median age: 62 yrs, range: 48–74 yrs) were enrolled, among whom 12 had non-small cell lung cancer (NSCLC), and 3 had colorectal cancer (CRC). 4 pts had ≥3 prior lines of treatment (tx). Median tx duration was 66.5 ds (range: 21–98 ds). No dose-limiting toxicity (DLT) or any ≥grade 3 treatment-related adverse events were observed in any dose cohorts. A total of 12 patients (80.0%) had treatment-related adverse events (grade 1, n = 6; grade 2, n = 6). By investigator-assessment, tumor response was evaluated in 9 pts (4 with ≥2 assessments); 6 pts had not reached their first assessment. 2 pts had PR (1 NSCLC at wks 12, 450mg, tx ongoing; 1 CRC at wks 6, 700mg, tx ongoing), 4 pts (NSCLC) had SD, and 3 pts had PD (1 NSCLC at wks 12, 2 CRC at wks 6). As data cut-off date, 11 pts were continuing to receive IBI351 (GFH925). Conclusions: IBI351(GFH925) was well-tolerated without unanticipated adverse events across all doses explored in pts with advanced solid tumors harboring the KRAS p.G12C mutation. Data from prior data cutoffs (primary: Feb, 2022; initial update: July, 2022) showed that IBI351 (GFH925) was well-tolerated and demonstrated promising efficacy in patients (pts) with advanced solid tumors harboring KRAS p.G12C mutation. Here, we update results of this phase I study evaluating IBI351 (GFH925) in pts with advanced solid tumors again. Methods: Pts with locally advanced, recurrent or metastatic solid tumors with KRASG12C mutation for whom standard therapy had failed were enrolled. Phase I dose escalation had an accelerated titration design for dose level 250mg once daily (QD) and a Bayesian optimal interval (BOIN) design with 450-900mg QD and 450-750mg twice daily (BID). The primary endpoints were safety and tolerability. The secondary endpoints were pharmacokinetics (PK), anti-tumor activity of IBI351 (GFH925) monotherapy per RECIST v1.1, and overall survival. Results: As of November 30th, 2022, 74 pts (1 at 250mg QD, 3 at 450mg QD, 9 at 700mg QD, 5 at 900mg QD, 21 at 450mg BID, 31 at 600mg BID and 4 at 750mg BID; 62 men, 12 women; median age: 64 yrs, range: 42-76 yrs) were enrolled, among whom 67 pts had non-small cell lung cancer (NSCLC). Among 67 NSCLC pts, 44.8% pts received ≥2 prior lines of treatment (tx), 38.8% pts had brain metastases; adenocarcinoma was the most common histology (n=66, 98.5%). All 74 pts were included for safety analysis. No dose-limiting toxicity (DLT) were observed in any dose cohorts. The overall safety profile was consistent with the latest previous report, with no new safety signals identified. As of December 15th, 2022, among 67 response-evaluable NSCLC pts across all dose levels, the ORR (by investigator assessment) was 58.2% (95% CI, 45.5-70.2), and the confirmed ORR was 44.8% (95% CI, 32.6-57.4); the disease control rate (DCR) was 92.5% (95% CI, 83.4-97.5). At the 600mg BID dose level (RP2D), the ORR was 63.3% (95% CI, 43.9-80.1), and the confirmed ORR was 50.0% (95% CI, 31.3-68.7), the DCR was 96.7% (95% CI, 82.8-99.9). With a median progression-free survival (PFS) follow-up of 5.5 months (95% CI, 5.3-6.8) for NSCLC pts at the 600mg BID dose level, 21 (70%) pts were continuing treatments, and the median duration of response (DoR) and PFS were not reached. Thirteen out of 15 confirmed responders were still in response. Conclusion: IBI351 (GFH925) was well-tolerated across all doses explored in patients with advanced solid tumors harboring KRAS p.G12C mutation. Abstract:94 Background: LY3537982 is an oral, potent, and highly selective inhibitor of GDP-bound KRAS G12C with unique pharmacologic properties that achieve high target occupancy at low absolute exposures. Here we present results of GI tumors treated on LOXO-RAS-20001, a phase 1 study of LY3537982 in patients (pts) with a KRAS G12C mutation. Methods: Dose escalation followed a mTPI-2 method. Dose expansion included a combination with cetuximab in colorectal cancer (CRC). All pts were KRAS G12C inhibitor naïve. Key objectives were to determine the RP2D, safety, PK, and antitumor activity per RECIST v1.1. Results: As of 24 July 2023, 73 pts with CRC (32), PANC (24), BTC (10), and other GI tumors (7) were treated with 50-200 mg BID LY3537982. Median age was 62 yrs (range, 36-85) and median number of prior lines of therapies was 3 (range, 0-11). No DLTs were observed. Grade 1 diarrhea was the highest frequency TEAE regardless of attribution (33%). At a median time on treatment of 4 months (range, 0.1-18), 20 pts are ongoing and 53 discontinued treatment. In the combination cohort, 46 pts with CRC were treated with 100 or 150 mg BID LY3537982 and cetuximab. Median age was 57 yrs (range, 35-77) and median number of lines of prior therapies was 3 (range, 1-8). 1 pt at 100 mg BID had a DLT (ALT/AST increased) and required a dose reduction. TEAEs ≥30% were dermatitis acneiform (59%), diarrhea (44%), dry skin (44%), hypomagnesemia (33%), and fatigue (30%). Vomiting, pruritus, skin fissures, headache, nausea, pyrexia, and rash were mostly grade 1 with 20-24% occurrence. At a median time on treatment of 6 months (range, 0.6-11), 37 pts are ongoing and 9 discontinued treatment (none due to AE). Table shows efficacy data. Abstract:468214 Background: KRAS G12C mutation occurs in approximately 4% of non-small cell lung cancers (NSCLC) in China. Glecirasib (Jacobio, Beijing, PRC) is a potent and highly selective covalent oral inhibitor of KRAS G12C. Methods: This phase 2 single-arm study (NCT05009329) of glecirasib was conducted in China at 43 sites, enrolling participants with locally advanced or metastatic KRAS G12C mutated NSCLC. Subjects must have received prior platinum-based therapy and an immune checkpoint inhibitor (ICI) as well as appropriate targeted agents for any actionable mutations per local standards. Primary study objective is overall response rate (ORR) assessed by independent review committee (IRC). The secondary study objectives include duration of response (DOR), progression-free survival (PFS), overall survival (OS), disease control rate (DCR) and safety. Here we report topline results from this pivotal study. Results: A total of 119 patients with NSCLC harboring KRAS G12C mutation were enrolled and treated with single agent glecirasib at 800mg daily (median follow up 10.4 months). The median age was 62 years, 79% was male and 21% was female, and 80.7% had ECOG PS 1. The prior lines of therapy include 1-3, with 94.1% (112/119) patients having received prior platinum-based therapy and ICI. Confirmed ORR was 47.9% (56/117; 95% CI, 38.5%-57.3%) and confirmed DCR was 86.3% (101/117; 95% CI, 78.7%-92%) assessed by IRC. Median PFS per IRC was 8.2 months (95% CI, 5.5 – 13.1), and median OS was 13.6 months (95% CI, 10.9 – NE). Median DOR has not been reached (95% CI, 7.2 – NE ). Overall, treatment-related adverse events (TRAEs) of any grade were observed in 97.5% of subjects. Most common TRAEs [≥15%] were anemia (56.3%), blood bilirubin increased (48.7%), alanine aminotransferase increased (35.3%), aspartate aminotransferase increased (35.3%), hypertriglyceridemia (28.6%) and γ-Glutamyl transferase increased (15.1%). Grade 3 and 4 TRAEs were observed in 39.5%. No grade 5 TRAEs occurred. Most of patients can stay on the treatment and 5.0% patients discontinued the treatment due to TRAE. In contrast to other FDA approved KRAS G12C inhibitor, Gleciasib has 6.7% of nausea, 7.6% vomiting, 3.4% diarrhea in which only one grade 3 nausea is reported. Conclusion: The pivotal phase 2 study of Glecirasib has met the primary endpoints and demonstrates impressive ORR and PFS with promising clinical activity for patients with KRAS G12C mutated advanced NSCLC. Glecirasib is well tolerated, with an extremely low gastrointestinal toxicity profile, which may improve patient compliance with oral therapy. Clinical trial information: NCT05009329 .

Abstract:8605 Background: KRAS G12C mutation is present in about 12-14% of NSCLC population. Ifebemtinib (IN10018) is a highly potent and selective oral inhibitor of focal adhesion kinase (FAK). D-1553 (garsorasib) is a novel oral and potent KRAS G12C inhibitor. Preclinical data showed that ifebemtinib in combination with KRAS G12C inhibitors had synergistic anti-cancer effect in multiple KRAS G12C mutant cancer models. This study is to evaluate the safety and antitumor activity of ifebemtinib combined with D-1553 in solid tumors with KRAS G12C mutation. Here we report the preliminary data from phase II part of front-line NSCLCs with KRAS G12C mutation. Methods: Locally advanced or metastatic NSCLC subjects without prior systemic anti-cancer therapy were enrolled. Subjects were required to have KRAS G12C mutation in tumor tissues. All subjects were assigned to the recommended phase II dose (RP2D) of ifebemtinib (100mg qd) + D-1553 (600mg bid). Results: As of 31 January 2024, 33 front-line NSCLC subjects with KRAS G12C mutation were enrolled and received the combination therapy. Median age was 65yrs (range 58, 83), 93.9% were male, and 81.8% had stage IV metastatic disease. Median study follow-up (FU) was 2.2 months (range 1.1, 9.2). The safety profile of the combination therapy is comparable to each single agent without additive toxicities. No ifebemtinib- or D-1553-related death was reported. Four subjects (12.1%) had ifebemtinib-related SAEs (diarrhea, enteritis, oedema peripheral and proteinuria) as assessed by investigators, and all SAEs were also considered D-1553 related. Six subjects (18.1%) had ≥ Grade 3 ifebemtinib-related AEs as assessed by investigators, and all were also D-1553 related AEs. The majority of ifebemtinib-related AEs were CTCAE Grade 1 or 2. There was no event leading to study treatment discontinuation. We have observed continuing tumor shrinkage in almost all subjects who had at least 2 tumor assessments. Among 16 subjects with ≥ 3 months FU, preliminary efficacy found that 14 had best overall response of partial response (PR) including 12 confirmed PRs and 2 unconfirmed but still in treatment, 1 had stable disease (SD), and 1 had progressive disease (PD). The overall response rate (ORR) was 87.5% (95%CI: 56.6, 96.2), and disease control rate (DCR) was 93.8% (95%CI: 63.6, 98.5). The median duration of response (DOR), median progression-free survival (PFS) and overall survival (OS) have not been reached. Conclusions: The combination of ifebemtinib and D-1553, as a dual-oral regimen, showed promising antitumor activity and manageable safety profile in KRAS G12C mutant front-line NSCLC population. Abstract:9112 Background: The KRAS G12C mutation occurs in 12-14% of NSCLC and in 3-5% of CRC patients and is recognized as an oncogenic driver mutation. Recently the discovery of a cryptic binding pocket within the effector region of the KRAS G12C oncoprotein has led to the rapid clinical development of a new series of specific inhibitors. GEC255 is a novel small molecule which showed excellent target engagement ability and a favorable pharmacological profile. Methods: This is a phase 1, open-label, multicenter study (CTR20212486) to evaluate the safety, tolerability, pharmacokinetics (PK), and efficacy of GEC255 in patients (pts) with advanced solid tumors bearing KRAS G12C mutation. The primary endpoint is safety and key secondary endpoints include PK and ORR. Key inclusion criteria: KRAS G12C mutation identified through digital PCR, measurable or evaluable disease, ECOG PS ≤1, and life expectancy > 3 months. Key exclusion criteria: previous administration of specific KRAS G12C inhibitors, uncontrollable plasma cavity effusion. Phase 1a is an accelerated titration, dose-escalating study to determine the maximum tolerated dose (MTD) or recommended phase 2 dose (RP2D). Phase 1b is a dose expansion study which will enroll pts with NSCLC, CRC, and other solid tumors harboring the KRAS G12C mutation. GEC255 will be administered PO QD until disease progression, intolerance, or withdrawal of consent. Results: 16 NSCLC pts (15 men, 1 woman, median age 58 y) were enrolled in Phase 1 dose escalation for the first 4 dose cohorts followed with dose escalation in chosen doses. No dose-limiting toxicity was observed and MTD has not been identified yet. GEC255 showed the linear increase in AUC exposure and C max with increasing doses. T max was 2-4 hours in all dosing groups and C trough within the 24-hour period was higher than pERK IC 90 even in low dose group. The half-life of GEC255 in 24h period is about 8-9h and there is no increase in AUC upon daily dosing for 28 consecutive days. Among the 16 pts, most pts (n = 13) had ≥1 prior lines of treatment (tx). A total of 15 patients had treatment-related adverse events and most AEs are G1 or G2 (93.3%); the most frequent AEs are diarrhea (56.3%), ALT increase (37.5%), rashes (25%), and anemia (25%). Tumor response was evaluated in 13 pts. 10 pts (76.9%) had objective response (complete or partial response) and 12 pts (92.3%) had disease control (objective response or stable disease). In 600mg dose group, the ORR was 83.3% and DCR was 100% (n = 6). 7 pts are continuing to receive GEC255. Conclusions: GEC255 has been well tolerated at the doses tested and has shown encouraging anticancer activity to advanced NSCLC patients with KRAS G12C mutation pts. Abstract:Molecular modifiers of KRAS G12C inhibitor (KRAS G12Ci) efficacy in advanced KRAS G12C-mutant NSCLC are poorly defined. In a large unbiased clinico-genomic analysis of 424 NSCLC patients, we identified and validated co-alterations in KEAP1, SMARCA4 and CDKN2A as major independent determinants of inferior clinical outcomes with KRAS G12Ci monotherapy. Collectively, co-mutations in these three tumor suppressor genes segregated patients into distinct prognostic subgroups and captured ~50% of those with early disease progression (PFS≤3 months) with KRAS G12Ci. Pathway-level integration of less prevalent co-alterations in functionally related genes nominated PI3K/AKT/MTOR pathway and additional baseline RAS gene alterations, including amplifications, as candidate drivers of inferior outcomes with KRAS G12Ci, and revealed a possible association between defective DNA damage response/repair and improved KRAS G12Ci efficacy. Our findings propose a framework for patient stratification and clinical outcome prediction in KRAS G12C-mutant NSCLC that can inform rational selection and appropriate tailoring of emerging combination therapies.

Abstract:Background: Mutations in KRAS are among the most frequent oncogenic drivers with the G12C mutation found in up to ~13% of NSCLC. LY3537982 is an oral, highly selective, and potent inhibitor of KRAS G12C, which preclinically delivers >90% sustained target occupancy. We present the initial results from LOXO-RAS-20001, a phase 1 study of LY3537982 in patients (pts) with KRAS G12C-mutant advanced solid tumors (NCT04956640). Methods: LY3537982 monotherapy dose escalation followed a mTPI-2 method. Dose expansion cohorts included combinations with pembrolizumab (NSCLC) and cetuximab (CRC). Key objectives were to determine the RP2D of LY3537982, safety, PK, and antitumor activity per RECIST v1.1. Results: As of 17 August 2022, 56 pts with NSCLC (16), CRC (17), PANC (8), and other tumor types (15) were treated with LY3537982 monotherapy on doses from 50-200 mg BID. Median number of prior systemic therapies was 2 (range, 0-8). No DLTs were observed, MTD was not reached, and RP2D determination is ongoing. Median time on treatment was 3 months (range, 0.3-13), 33 pts are ongoing, and 23 pts discontinued (none due to a related AE). TEAEs observed in ≥10% of pts were diarrhea (38%), constipation (16%), fatigue (16%), peripheral oedema (13%), and nausea (11%), mostly grade 1. The only grade ≥3 treatment related AE was neutropenia (n=1), and no pneumonitis or grade ≥2 transaminitis was observed. There were no treatment related serious AEs or deaths. Dose proportional steady-state exposures were observed through 150 mg BID. Table shows preliminary efficacy data. Conclusions: LY3537982 demonstrated a favorable safety profile, including the absence of high-grade liver toxicity, and tolerance in pts previously intolerant to other KRAS G12C inhibitors. Preliminary efficacy was observed with LY3537982 monotherapy across multiple tumor types. Updated data in more than 100 pts, including data in combination with pembrolizumab and cetuximab will be presented. Abstract:Introduction D-1553 (garsorasib) is a potent and selective oral KRAS G12C inhibitor. We report results from a phase I dose-escalation and dose-expansion study of D-1553 in patients with KRAS G12C–mutated NSCLC in multiple sites in the People's Republic of China. Methods Patients with KRAS G12C–mutated NSCLC have administrated D-1553 600 mg orally once daily, 800 mg once daily, 1200 mg once daily, 400 mg twice a day, or 600 mg twice a day in dose escalation. In dose-expansion, all patients received 600 mg twice a day. The safety, pharmacokinetics, and efficacy of D-1553 were evaluated. Results Among a total of 79 treated patients, 75 patients (94.9%) reported treatment-related adverse events with 30 patients experiencing grade 3 or 4 events (38.0%). Most of the adverse events were manageable and the patients tolerated the study treatment well. Among 74 patients assessable for efficacy analysis, 30 patients had a partial response and 38 had stable disease with a confirmed objective response rate (ORR) and disease control rate (DCR) of 40.5% and 91.9%, respectively. The median progression-free survival was 8.2 months, and the median duration of response was 7.1 months. Among 62 patients assessable for response at the recommended phase 2 dose, partial response occurred in 24 patients (ORR, 38.7%) and stable disease in 32 patients (DCR, 90.3%). The median progression-free survival and duration of response were 7.6 months and 6.9 months, respectively. In patients with brain metastasis, ORR and DCR were 17% and 100%, respectively. Conclusions D-1553 represents a promising therapeutic option for patients with KRAS G12C–mutated NSCLC with a well-tolerated safety profile and encouraging antitumor activity.

Abstract:3008 Background: Resistance to KRAS G12C inhibitor in non-small cell lung cancer (NSCLC) can be overcome by SHP2 blockade. This concept was tested in Phase I study of glecirasib combined with JAB-3312 in patients (pts) with KRAS p.G12C mutated tumors. Promising preliminary data of safety and response were reported at the 2023 ESMO (653O). Here, we present the updated safety and efficacy data with treatment response and progression-free survival (PFS) of the combination therapy in front-line NSCLC. Methods: The phase 1/2a study [NCT05288205] evaluated glecirasib plus JAB-3312 in patients with KRAS p.G12C mutated solid tumors. The primary objective of the dose-escalation phase was to assess the safety and tolerability of the combination therapy, and the primary objective of the dose-expansion phase was to assess efficacy. Efficacy endpoints included objective response rate (ORR), disease control rate (DCR), duration of response (DoR), PFS by investigator per RECIST1.1 and overall survival. Glecirasib 400 mg or 800 mg daily was combined with various JAB-3312 doses and schedules. Results: As of December 1st, 2023, 179 pts were enrolled, including 157 with NSCLC, 17 with colorectal cancer, and five with other tumor types. Treatment-related adverse events (TRAE) ≥ grade 3 occurred in 41.9% of all pts. There were no treatment-related deaths. Discontinuation of either glecirasib or JAB-3312 due to TRAE occurred in 7.8% of all pts. The AE profile was unchanged from the last ESMO report. The most common ( > 20%) TRAEs included anemia, ALT/AST increased, hypertriglyceridemia, bilirubin increased, neutropenia/leukopenia, creatine kinase increased, and edema. No overlapped toxicity was observed for this combination therapy. Amongst all pts enrolled, 88 had NSCLC and received the combination therapy as front-line treatment in seven dosing groups. The median follow-up duration was 6.1 (range:0.5-16.7) months. Eighty pts had at least once post-treatment efficacy assessment. The ORR was 72.5% (58/80) and DCR was 96.3% (77/80). In the group of glecirasib 800mg QD + JAB-3312 2mg one week on/one week off, the ORR was 77.8% (21/27) and DCR was 92.6% (25/27). The median PFS was not mature by the data cut-off date. The 6-month and 12-month PFS rate were 67.3% and 53.7%, respectively. Additional safety and efficacy data will be presented at the meeting. Conclusions: Glecirasib plus JAB-3312 has a manageable safety profile and a promising ORR and PFS as a front-line treatment for patients with KRAS p.G12C NSCLC. Abstract:The recent accelerated approval of the KRASG12C mutant-selective inhibitor sotorasib (AMG 510) for the treatment of 2nd line KRASG12C mutation-positive NSCLC patients marks the first approved targeted therapy for tumors with any KRAS mutation. While KRASG12C inhibitors deliver clinical benefit, most patients who achieved an objective response ultimately progressed. Recent insights into clinical KRASG12C inhibitor resistance identified reactivation of the RAS/MAPK pathway as a common putative driver mechanism of resistance. Multiple ongoing trials seek to augment responses to KRASG12C inhibitors through rational combination strategies, including the pan-KRAS SOS1 inhibitor BI 1701963. Here we use different preclinical experimental approaches to interrogate KRASG12C inhibitor resistance mechanisms with the aim to identify strategies to overcome resistance. To predict on-target resistance, Ba/F3 cells were transduced with KRASG12C, ENU-mutagenized and chronically exposed to KRASG12C inhibitors. Resistant clones were screened for secondary KRAS mutations, highlighting that KRAS G12C/Y96D and Y96S cis mutations did confer resistance to KRASG12C inhibition but could be overcome by MEK inhibitor combination with a SOS1 inhibitor. As second strategy a high-complexity single site variant library of KRASG12C encompassing all possible secondary KRAS mutations was employed to establish Ba/F3 transgenic cell pools. The response of this KRASG12C Ba/F3 clone library harboring a comprehensive set of secondary mutations was tested following treatment with KRASG12C inhibitors alone and in combination with a pan-KRAS SOS1 inhibitor. In parallel, acquired KRASG12C inhibitor resistance was generated in solid cancer cells following long-term MRTX849 treatment. Clones were characterized and their response to KRASG12C inhibition and combination therapy was analyzed. Both in the Ba/F3 cell pool as well as in KRASG12C inhibitor resistant clones combining SOS1 inhibition to KRASG12C inhibition proved to be beneficial. Finally, bringing these findings in vivo, SW837(CRC) tumor-bearing mice were long-term treated with adagrasib until tumors relapsed after initial regression and resistant tumors were randomized for second line treatments. In this KRASG12C inhibitor resistant context, treatment with adagrasib plus cetuximab resulted in tumor stasis and adagrasib plus SOS1i resulted in tumor regression. While more work is currently being undertaken to map the resistance mechanisms in both our in vitro and in vivo settings, all results highlight the benefit of combining a SOS1 inhibitor with a KRASG12C inhibitor to prevent and/or overcome acquired resistance. Abstract:604 Background: KRAS G12C mutation is an oncogenic driver and a validated therapeutic target in solid tumors. Glecirasib, a highly selective, covalent oral KRAS G12C inhibitor, has demonstrated promising clinical activity in NSCLC and CRC; however, the efficacy and safety of glecirasib are unknown in PDAC and other solid tumors with KRAS G12C mutation. Methods: Two phase 1/2 trials (NCT05009329 in China; NCT05002270 in US, Europe and Israel) are evaluating the safety and efficacy of glecirasib in patients (pts) with solid tumor harboring KRAS G12C mutations. As both trials have similar inclusion and exclusion criteria, we have pooled the data from them to assess the effect of glecirasib monotherapy in various types of solid tumor with low incidence of KRAS G12C mutation. Here we report the preliminary efficacy and safety results of glecirasib in pts with PDAC and other solid tumors (excluding NSCLC and CRC) from the pooled population of both trials. Results: As of Sep 8, 2023, a total of 48 pts have received glecirasib monotherapy (41 from NCT05009329 and 7 from NCT05002270) and 47 were evaluable for efficacy. This includes 28 PDAC and 19 other solid tumors (7 biliary tract, 3 gastric, 3 small bowel, 1 appendiceal, 1 hepatocellular, 1 peritoneal, 1 posterior bronchial mediastinal, 1 ameloblastic carcinoma and 1 cervical). At baseline, pts had previously received a median of one line of systemic therapy (range: 0 to 4; with 23 pts have received one prior line); 85% Asian, 15% Caucasian; most (45 pts) at 800 mg QD. Among 28 pts with PDAC, 13 achieved confirmed partial response (PR) with the confirmed objective response rate (ORR) of 46.4% (13/28) and disease control rate (DCR) of 96.4%; the median duration of response (DOR) and progression-free survival (PFS) were 4.1 months and 5.5 months (95%CI 1.2, 13.1), respectively. Among 19 pts with other tumor types, the confirmed ORR of 52.6% (10/19) and DCR of 84.2% (16/19) were observed; the median DOR and PFS were 8.3 months and 7.0 months (95% CI 1.1 to 15.2), respectively. Treatment-related AE (TRAE) of any grade occurred in 89.6% (43/48) pts; the most common (≥10%) TRAE were anemia (52.1%), blood bilirubin increased (39.6%), white blood cell count decreased (18.8%), AST increased (18.8%), diarrhea (16.7%), ALT increased (14.6%), asthenia (14.6%), hypertriglyceridemia (10.4%), and nausea (10.4%); ≥Grade 3 TRAE occurred in 25% (12/48) pts; no TRAE were fatal or led to treatment discontinuation. Conclusions: Glecirasib monotherapy is well tolerated and has a manageable safety profile and exhibits promising anti-tumor activity in pts with KRAS G12C mutated PDAC and other solid tumors. Abstract:3563 Background: KRAS G12C mutation is an oncogenic driver that occurs in 3-4% of colorectal cancer (CRC). D-1553 is a novel oral and potent KRAS G12C inhibitor. This phase I/II open-label study (NCT04585035) is an international multicohort study evaluating the safety, tolerability, pharmacokinetics (PK) and efficacy of D-1553 in patients (pts) with KRAS G12C mutated locally advanced or metastatic solid tumors. The Phase I part was conducted to determine the recommended phase 2 dose (RP2D) of D-1553. The Phase II part enrolled multiple expansion cohorts of different cancer types. The endpoints of the study include clinical activity, safety and PK. Here we report preliminary data from pts with locally advanced unresectable or metastatic CRC receiving ≥ RP2D of D-1553 monotherapy. Methods: Pts with locally advanced unresectable or metastatic CRC with progression after standard treatment were enrolled in the Phase I and Phase II parts of the study. Pts were required to have KRAS G12C mutations in tumor or ctDNA samples and no prior KRAS G12C directed therapy. The current analysis includes CRC patients who were treated with D-1553 at RP2D (600 mg BID in Phase I and II) and above (800 mg BID in Phase I) as monotherapy. Results: As of 30 December 2022, 24 pts with previously heavily treated locally advanced or metastatic CRC (54.2% male; median age, 61.5 years [range 44, 74]; ECOG PS 0/1: 45.8%/54.2%) were enrolled and received D-1553 600 mg (n = 23) or 800 mg (n = 1) BID monotherapy. 95.8% of pts had stage IV disease. 66.7% had ≥ 2 prior lines of therapy (median: 2 [range, 1, 6]). Median treatment duration was 5.75 (range 1.51, 11.83) months (mo) with a median follow-up of 6.64 (range 2.46, 13.11) mo. Confirmed ORR was 20.8% (5/24) (95% CI: 7.1%-42.2%), and DCR was 95.8% (23/24). Median PFS was 7.62 mo (95% CI, 2.89 to 9.53 mo). At the data cutoff date, 37.5% (9/24) of pts remain on study treatment. Treatment-related adverse events (TRAEs) of any grade occurred in 50% (12/24), most were grade 1 or 2 in severity. Two pts had grade 3/4 TRAEs (alanine aminotransferase increased, diarrhea, hypertension and hypokalaemia). No TRAEs were fatal or resulted in D-1553 discontinuation. The most common (≥ 5%) TRAEs (any grade) were increased alanine aminotransferase or aspartate aminotransferase, increased total bilirubin or conjugated bilirubin, diarrhea, hypothyroidism and nausea. Conclusions: D-1553 demonstrated a tolerable safety profile and promising monotherapy activity in pts with heavily pretreated locally advanced or metastatic CRC and KRAS G12C mutations. This study is ongoing to further evaluate the safety and efficacy of D-1533 as monotherapy and in combination with cetuximab or chemotherapy in pts with locally advanced or metastatic CRC. Abstract:Background: D3S-001 is a second-generation (2G) GDP-bound KRAS G12C inhibitor (G12Ci). Its high covalent potency and rapid target engagement kinetics correlated with robust anti-tumor activity preclinically and translated into promising clinical activity in a phase 1 first-in-human trial (FIH). Here we report clinical data from D3S-001 FIH trial (NCT05410145). Methods: D3S-001 was administered as monotherapy to patients (pts) with KRAS G12C mutated advanced solid tumors at 6 planned dose levels (50-900mg QD) from 3 countries. The key objectives were safety, PK, antitumor activity, and ctDNA kinetics. Results: As of 6 Feb 2024, a total of 41 pts (25 non-small cell lung cancer [NSCLC], 12 colorectal cancer [CRC] and 4 pancreatic cancer [PDAC]) were dosed. Median follow-up was 5.1 months (m, range 0.2-15.4m) and 28 pts (68.3%) remain on-study treatment. No treatment-related DLT or deaths were observed. Treatment-related AEs (TRAE) of any grade occurred in 30 pts (73.2%). Of which, 6 (14.6%) were Grade 3 (no Grade 4 or higher). D3S-001 dose was modified in 11 pts (26.8%) due to TRAE, and no pts had dose discontinuation due to TRAE. PK analysis indicates dose/exposure linearity and terminal plasma T1/2 of ~11 hours. Of 37 pts with post-baseline tumor assessments, 33 were naïve to G12Ci and 4 were G12Ci pre-treated. In 33 G12Ci naïve pts, PRs were achieved in 25/33 overall population (75.8%, 95% CI 57.7, 88.9%), in 14/20 NSCLC pts (70.0%, 95% CI 45.7, 87.2%), in 7/9 CRC pts (77.8%, 95% CI 40.2, 96.1%), and in 4/4 PDAC pts (100.0%, 95%CI 39.6, 100.0%). CNS efficacy was observed in 2/6 NSCLC pts. Of all G12Ci naive pts (n=36), 6-month PFS rate is 69.1% (95% CI 46.3, 83.7%), but median PFS and DoR are not yet mature. Deeper tumor reductions were observed in 5 out of 9 G12Ci naïve pts after intra-subject dose escalation. All 4 G12Ci pre-treated pts (2 NSCLC, 2 CRC) showed stable disease at the first post-baseline assessment. Serial assessment of ctDNA showed significant declines in G12C VAF as early as on Day 8 of treatment. 600mg QD was selected as RP2D. Conclusions: D3S-001, a 2G G12Ci, is well tolerated and delivers promising anti-tumor activity in pts with NSCLC, CRC, and PDAC carrying KRAS G12C mutation across all dose levels in FIH trial. Adagrasib: A Landmark in the KRASG12C‐mutated NSCLC

Abstract:A recent study published in the New England Journal of Medicine by Pasi A. Jänne et al.1 presented the Phase II cohort results from a clinical trial (KRYSTAL-1, NCT03785249), which evaluated adagrasib (MRTX849, a KRASG12C inhibitor) in KRASG12C-mutated nonsmall-cell lung cancer (NSCLC) previously treated with chemotherapy and antiprogrammed death 1 (PD-1) or antiprogrammed death ligand 1 (PD-L1) therapy. Adagrasib showed encouraging efficacy and acceptable safety, offering novel therapeutic avenues for KRASG12C-mutated NSCLC (Figure 1A). This study enrolled 116 patients with histologically confirmed unresectable or metastatic NSCLC with KRASG12C mutation who had accepted treatment with at least one platinum-based chemotherapy regimen and immune checkpoint inhibitors (ICIs) therapy. 98.3% of the patients accepted both chemotherapy and immunotherapy previously. The patients received a 600-mg dose of adagrasib twice daily, with a fasted state. Among 112 patients with measurable disease at baseline, the confirmed objective response rate (ORR) was 42.9%. The median response duration of 48 patients with a response was 8.5 months (95% confidence interval [CI], 6.2 to 13.8), and the median progression-free survival (PFS) among 112 evaluable patients was 6.5 months (95% CI, 4.7 to 8.4). The median overall survival (OS) was 12.6 months (95% CI, 9.2 to 19.2), with updated data on January 15, 2022. In 33 patients with radiographically evaluable CNS metastases, the intracranial confirmed ORR was 33.3% (95% CI, 18.0 to 51.8). 97.4% of the patients underwent treatment-related adverse events, the most common of which were diarrhea, nausea, vomiting, and fatigue, leading to a 6.9% drug discontinuation rate. The ORRs were similar across PD-L1 expression subgroups, indicating that the efficacy of adagrasib effectiveness was not affected by PD-L1 expression. The ORRs in patients with coalterations in STK11, KEAP1, TP53, and CDKN2A were satisfying (range 28.6%-58.3%). While the ORR in those who had STK11 wildtype with KEAP1 comutation was lower (14.3%). More evidence on this issue is warranted since precise stratification of KRAS-mutated NSCLC patients is necessary before clinical decisions. Adagrasib is a robust, orally available, small molecule covalent inhibitor binding to the KRASG12C cysteine 12 residue, locking the protein in its inactive GDP-bound conformation, thus blocking KRAS-dependent signaling and exerting antitumor efficacy in tumor models2 (Figure 1B). The registered interventional clinical trials of adagrasib in cancer were summarized (Figure 1C). As the first KRASG12C inhibitor approved by FDA (Food and Drug Administration), sotorasib yielded a durable clinical benefit in KRASG12C-mutated NSCLC (NCT03600883). In NSCLC, adagrasib was superior to sotorasib in ORR (sotorasib 37.1%, adagrasib 42.9%), and they were similar in median OS (sotorasib 12.5 months, adagrasib 12.6 months) and median PFS (sotorasib 6.8 months, adagrasib 6.5 months) (NCT03785249 and NCT03600883). In addition, pharmacologic differences between adagrasib and sotorasib are the drug half-life, dose-dependent extended exposure with adagrasib, and potential central nervous system (CNS) penetration of adagrasib. Approximately 27%-42% of KRASG12C-mutated NSCLC patients have CNS metastases at diagnosis and are of dismal prognosis. Under the Response Assessment in Neuro-Oncology Brain Metastases (RANO-BM) criteria, Jänne et al.1 identified 42 patients with CNS metastases at baseline, the median intracranial PFS of whom was 5.4 months (95% CI, 3.3. to 11.6). A total of 33 patients of them could be evaluated radiographically, the intracranial confirmed ORR was 33.3% (95% CI, 18.0 to 51.8), and the median duration of intracranial response was 11.2 months (95% CI, 2.99 to not evaluable). In a previous report, sotorasib led to a 13% intracranial response rate in 16 KRASG12C-NSCLC patients with stable, treated CNS metastases (CodeBreaK 100, NCT03600883). However, the CNS penetration ability of adagrasib warrants further investigation since all patients with CNS metastases recruited were previously treated and neurologically stable, and the number of patients with CNS metastases was limited. The good news is that at the 2022 American Society of Clinical Oncology (ASCO) Annual Meeting, Mirati Therapeutics Inc announced the first clinical data demonstrating CNS-specific activity of KRASG12C inhibitor (adagrasib) in NSCLC patients with active and untreated CNS metastases (the Phase Ib cohort of NCT03785249): the intracranial ORR was approximately 32% under RANO-BM criteria and the intracranial disease control rate was about 84% (NCT03785249, Meeting Abstract | 2022 ASCO Annual Meeting II). The adoption of patients with active CNS disease in Jänne et al.'s study also received attention from Kotecha et.al, who gave their concerns and suggestions about this issue.3 We expect more evidence derived from large-scale cohorts. The efficacy comparisons we made between adagrasib and sotorasib were preliminary work based on the current data from different cohorts. More solid evidence about these comparisons should be referred to head-to-head RCTs (randomized control trials). In NSCLC, the KRASG12C mutation frequency is 14% in adenocarcinoma and 0.5%–4% in squamous carcinoma.1 Although most patients in the study of Jänne et al.1 were ECOG 1 and had previously received both chemotherapy and ICI therapy, the efficacy of adagrasib was better than traditional second-line chemotherapy. In NSCLC second-line docetaxel treatment, the ORR was 14%, the PFS was 3.0 months (IQR 1.4-6.9), and the median OS was 9.1 months (IQR 4.2-18.0) (NCT01168973). Therefore, adagrasib could bring hope to a substantial late-stage NSCLC patient irresponsive to traditional chemotherapy. We believe that the clinical outcome of adagrasib would be improved when applied at an earlier stage. The safety of adagrasib is acceptable. Although treatment-related adverse events were observed in 97.4% of the patients, the drug discontinuation rate was low (6.9%). The most common events were gastrointestinal-related events, similar to other KRASG12C inhibitors. We expect the ongoing evaluation of an additional dose level (400 mg orally twice daily)1 which might offer an opportunity to relieve the adverse events. Moreover, KRASG12C mutation occurs in 3%-4% of CRC (Colorectal Cancer). Durable blockade of KRASG12C might be of significance in CRC. In 2021, Weiss et al. reported that adagrasib demonstrated promising clinical efficacy and safety in KRASG12C -mutant CRC, suggesting the extensive use of this medicine (NCT03785249). Neoadjuvant immunotherapy has improved the prognosis of NSCLC in the past five years, while resistance is an issue that cannot be bypassed. It is never too early to deliberate the combined utilization of KRASG12C inhibitors and immunotherapy. A meta-analysis in 2022 reported that anti-PD-(L)1 therapy with or without chemotherapy achieved more prolonged survival than mono-chemotherapy for KRAS-mutant NSCLC (https://doi.org/10.1007/s00262-021-03031-1). Therefore, combining ICIs and KRASG12C-targeted medicine seems an obvious approach. A preclinical study reported that promoting an antitumor environment and lasting cures were found in mice models treated with sotorasib combined with the anti-PD-1 regimen, supporting the strategy of the combination of ICIs and KRAS inhibitor.4 However, a recent study showed that the successful combination of KRASG12C inhibition (MRTX1257) and ICIs was not universal in the NSCLC tumor models, the synergistic benefit was only seen in the most immunogenic tumor models, and most tumor refractory to ICIs was not resensitized by combination with KRASG12C inhibition.5 Therefore, the confronting questions are: (1) How to select the NSCLC patients who might benefit from the combinational therapy of KRASG12C inhibitors and ICIs? (2) What are the underlying mechanisms and solutions of resistance in the combination of KRASG12C inhibitors and ICIs? Therefore, more efforts are warranted to explore the interactions between KRASG12C inhibitors and cancer immunity. We look forward to the results of KRYSTAL-7 (NCT04613596), a Phase II study evaluating the efficacy and safety of adagrasib monotherapy and in combination with pembrolizumab in cohorts of patients with advanced/metastatic KRASG12C-mutated NSCLC.
