Abstract:Aims Genetic testing can be used to improve the safety and effectiveness of commonly prescribed medicines—a concept known as pharmacogenetics. This study aimed to quantify members of the UK public's preferences for a pharmacogenetic service to be delivered in primary care in the National Health Service. Methods Members of the UK population were surveyed via an online panel company. Respondents completed 1 of 2 survey versions, asking respondents to select their preferred pharmacogenetic testing service in the context of a presentation of low mood or pain. A conditional logit model was estimated, before the best functional form for the dataset was identified. Preference heterogeneity was identified via latent class analysis. Coefficients from the final selected models were used to estimate uptake in the context of different hypothetical pharmacogenetic services. Results Responses from 1993 individuals were included in the analysis. There were no differences observed in preference between the 2 clinical scenarios. Conditional logit analysis, using maximum likelihood estimation, indicated that respondents preferred to have noninvasive tests and wanted their data to be shared between different healthcare organizations to guide future prescribing. There was a preference for regional over national data sharing initiatives, and respondents preferred to have access to their data. Predicted uptake varied considerably, ranging from 51% to >99%, depending on design of the service. Conclusion This study identifies public preferences for a pharmacogenetic testing service and demonstrates how predicted uptake can be impacted by relatively minor adaptations. This highlights areas for prioritization during development of future pharmacogenetic services.

Abstract:Background: The implementation of pharmacogenetic (PGx) testing may contribute to health disparities if access to testing is inequitable, as medically underserved patients are prescribed higher rates of drugs with PGx guidelines and often experience the benefits of emerging health technologies last. Limited research has evaluated potential implementation of PGx testing in populations who are medically underserved and none have evaluated their preferences regarding PGx test characteristics and cost. Our study endeavored to assess the willingness to pay for PGx testing and key PGx test preferences in a nationwide cohort of medically underserved respondents. Methods: A survey was developed to assess willingness to pay and preferences for PGx testing through a discrete choice experiment (DCE). Five attributes of PGx tests were included in the DCE: doctor recommendation, wait time, number of actionable results, benefit of the test (avoid a side effect or address a health problem), and out-of-pocket cost. A convenience sample of U.S. adults with an average yearly household income of $42,000 or less was collected utilizing an online survey fielded by Qualtrics Research Services (Provo, UT). For the DCE analysis, conditional logit and mixed-logit regression models were utilized to determine relative utility of attributes and levels, conditional relative importance for each attribute, and marginal willingness to pay. Results: Respondents completed the survey with an 83.1% response completion rate. Following quality control procedures, 1,060 respondents were included in the final nationwide cohort. Approximately, 82% of respondents were willing to pay less than 200. Out-of-pocket cost was the attribute identified as having the greatest relative importance on choice, while wait time had the lowest importance. Greater utility was observed if the PGx test was doctor recommended, had a higher number of actionable results, and resolved major or minor health problems compared with avoiding side effects. Conclusion: This first-of-its-kind study provides important insights into the willingness to pay for PGx testing and PGx test preferences of a large medically underserved population. Applying these findings can potentially lead to improvements in the successful implementation of PGx testing in this population.

Abstract:Abstract Pharmacogenomics describes interpatient genetic variability in drug responses. Information based on whole genome sequencing will soon open up the field of pharmacogenomics and facilitate the use of genomic information relating to drug metabolism and drug responses. We undertook a qualitative study, aiming to explore the potential barriers, opportunities and challenges facing the implementation of pharmacogenomics into primary care. Semi-structured interviews were undertaken with 18 clinical participants (16 GPs and 2 other clinicians). All interviews were recorded and transcribed verbatim. Using a thematic analysis approach, data items were coded, ordered and themes constructed. Most participants were aged 55–60 years and worked as part-time clinical GPs with other clearly defined roles. The emerging themes covered several areas of concern, including the following: the utility of pharmacogenomics and the value of introducing such testing into primary care; how to educate the primary care workforce and ‘mainstream’ pharmacogenomics; the ethical, legal and social aspects of pharmacogenomics and its impact on patients; and potential impacts on the healthcare system particularly around economics and informatics. Most participants had concerns about pharmacogenomics and felt that there were a number of barriers and challenges to its implementation into routine primary care. Most striking were their concerns around the cost-effectiveness of using pharmacogenomics in primary care. At the same time most recognised the increasing availability of direct-to-consumer testing, and felt that this would drive the need to understand the ethical and social implications of using genomic information in primary care. This study has raised important issues that need to be considered when planning the implementation of pharmacogenomics into clinical practice. Prior to the implementation of genomic testing into day-to-day practice in UK primary care, it is important that considerations around education, cost-effectiveness and informatics are addressed, as well as the impact on patients.
Immunotherapy of cancer patients with BCG: summary of ten years experience in Japan.

Abstract:Two thousand and four patients with various types of late stage cancer were treated with living BCG immunotherapy from 1972 to 1982. Fourty-five out of 1,112 patients were able to survive more than 5 years after initiation of this immunotherapy. All of these patients were noncurative operation except for 2 cases of hepatoma who received no surgery. We report the immunological and clinical aspects of the long term survivors of this treatment.

Abstract:Background/Objectives: The integration of pharmacogenetic (PGx) testing into primary care has not been widely implemented, despite its benefits for patients and providers. PGx testing could also reduce health disparities as patients with lower healthcare access are prescribed higher proportions of medications with PGx guidelines. Little is known about the preferences of patients who have experienced PGx testing to inform implementation across the care process. This qualitative study aimed to refine implementation by capturing patient preferences on (1) testing and prescription timing, (2) patient–clinician discussion of results during post-test counseling, and (3) usability of a card during results dissemination. Methods: Interviews were conducted with 25 primary care patients from clinics primarily serving medically underserved populations. Interview transcripts were thematically analyzed using a constant comparative approach. Results: While patients supported both reactive and pre-emptive testing, they valued pre-emptive PGx testing because it is proactive for future health needs, expedites treatment, and is convenient. Patients' preferences for receiving prescriptions depended on several factors: having immediate access to needed medications, avoiding experiencing medication side effects and interactions, avoiding taking ineffective medications, and avoiding inconveniences. Patients identified three issues critical to patient–clinician interactions when receiving testing results: information specific to medications, clarification and further information about their results, and enhanced clinician accessibility related to the results. Lastly, they liked that the results card could facilitate discussions with clinicians and was informative and convenient but said it lacked clarity. Conclusions: These findings should inform implementation strategies for integrating PGx testing in primary care for underserved patients.

Abstract:The field of pharmacogenetics (PGx) is experiencing significant growth, with increasing evidence to support its application in psychiatric care, suggesting its potential to personalise treatment plans, optimise medication efficacy, and reduce adverse drug reactions. However, the perceived utility and practicability of PGx for psychiatric treatment in youth remains underexplored. This study investigated youth-perceived barriers and attitudes towards the implementation of PGx testing to guide antidepressant treatment in primary care. Semi-structured focus groups and interviews were conducted with 17 participants aged between 18 to 24 years. These sessions were recorded and transcribed before thematic analysis was used to identify collective themes. Three key themes were identified, including attitudes towards the medication prescription process, concerns and attitudes towards PGx testing, and perceived barriers to its clinical implementation. Although PGx testing was positively perceived by most participants, all participants shared concerns about PGx testing. Participants voiced concerns about the financial impact of PGx testing, the potential for treatment delays, and the accuracy of PGx testing in guiding antidepressant treatment. Additionally, participants noted that the low awareness and willingness of general practitioners to incorporate PGx testing into routine practice could hinder successful clinical implementation. Prior to the implementation of PGx testing into Australian primary practices, it is essential to acknowledge patient perspectives and ensure that clinical practices remain patient-focused. This study highlights important considerations for integrating PGx testing into antidepressant pharmacotherapy and emphasizes the need for future research to address and mitigate youth-perceived barriers.

Abstract:The Fisher divergence (FD) and Jensen-Shannon divergence (JSD) are used in this work with the aim of providing quantitative measures of the discrepancies between two arbitrary D-dimensional distribution functions, the FD being of local character and the JSD of global one. In doing so, the concepts of Fisher information and Shannon entropy associated to a distribution are the essential quantities for building up these comparative functionals. This kind of relative measures are here applied to the study of the one-particle densities in both conjugated spaces (position and momentum) of neutral atoms, discussing the results as compared to those provided by other previous functional measures. It is clearly shown how these divergences provide relevant information on the atomic shell structure, up to a level which depends on the considered space and measure.

Abstract:We thank Drs. Frye and Nemeroff for their perspective on the promising role of pharmacogenetics (PGx) in the selection of antidepressant treatment [1]. In their article, Frye and Nemeroff discuss several challenges to the wide-scale application of PGx for the treatment of depression. We agree that there are several challenges to address, particularly regarding electronic health record (EHR) integration, in order for PGx to become a widely utilized resource for antidepressant selection and dosing. EHR functionality is required for PGx implementation to succeed, including readily accessible, discrete results and integrated clinical decision support (CDS). PGx laboratory results are shared in several ways with patients and providers: as paper reports which are subsequently scanned into the EHR and/or manually entered into a patient's chart; as electronic files uploaded into the media section of the EHR; in patient or provider-facing websites independent of the EHR; or as laboratory results directly transmitted via HL7 messaging. Returning results and providing CDS in static formats, such as electronic files, do not allow for the interpretation or prescribing recommendations to change as PGx knowledge evolves. For example, the 2023 Clinical Pharmacogenetics Implementation Consortium (CPIC) serotonin reuptake inhibitor guidelines provide actionable recommendations for sertraline based on CYP2B6 metabolizer status, whereas previous CPIC serotonin reuptake inhibitor guidelines did not [2, 3]. CDS returned in electronic file formats prior to 2023 would not contain CYP2B6/sertraline recommendations, therefore not reflecting current consensus. To ensure accurate and high-quality patient care, CDS must be revised periodically and dynamically changed as evidence and treatment recommendations are updated.

Abstract:Computer graphics and animation has been widely used in many fields especially in film industry. The animation industry especially in Malaysia is limited to television commercial. This is because the production of animation film is very expensive. Furthermore the number of animators in Malaysia still needs to be increase. Modeling is the most important aspect in the animation development whereby articulated figure is commonly used in modeling the human or animal model. Kinematics technique is the common approach for control movement in animation. In this study, forward kinematics technique is applied into human animation and the defined angles are trained into neural network. The purpose of using neural network is to get the precise coordinate for “end effector”. The angles of the output are then applied to the model to produce human walking. This technique overcomes the problem of “try and error” in forward kinematics technique in order to get the target coordinate for human walking. This study is able to overcome the deficiency in forward kinematics. Therefore, forward kinematics can be use to produce an animation that involve a simple movement. Abstract:(1) Background: Uptake of pharmacogenomic testing in routine clinical practices is currently slow in China. Pharmacists might play an important role in leveraging care through applying pharmacogenomics, therefore, it is important to better understand clinical pharmacists’ knowledge of and attitudes toward pharmacogenomic testing, which has not been well-studied. (2) Methods: A self-administered survey was developed based on previous knowledge of pharmacogenomic testing and its uptake in China. Participants were recruited through the Committee of Pharmaceutical Affairs Management under the Chinese Hospital Association. (3) Results: A total of 1005 clinical pharmacists completed the questionnaire, among whom 996 (99.10%) had heard of pharmacogenomic testing before participation. More than half of respondents (60.0%, n = 597) rated their knowledge of pharmacogenomic testing as “average”, while 25% rated it “good” or “excellent”. “Guidelines, consensus and treatment paths for disease diagnosis and treatment” (78.7%) were the most preferred sources of information about pharmacogenomic testing. Most respondents (77.0%) believed that pharmacogenomics could “help to improve efficacy and reduce the incidence of adverse reactions”. Our participants also believed that patients would benefit most from pharmacogenomic testing through better prediction of individual drug responses and thus informed treatment decisions. The top challenge for the uptake of pharmacogenomic testing was its high cost or lack of insurance coverage (76.7%). (4) Conclusions: Most Chinese clinical pharmacists who participated in our study had a positive attitude toward pharmacogenomic testing, while the knowledge of pharmacogenomic testing was generally self-assessed as average.

Abstract:OBJECTIVE: To better understand clinician needs and preferences for the display of pharmacogenomic (PGx) information in clinical decision support (CDS) tools. MATERIALS AND METHODS: We developed a semi-structured interview to collect feedback and preferences in six key areas of PGx CDS design, from clinicians who had prior experience with live PGx CDS tools. Eight clinicians from Northwestern Medicine's (NM) General Internal Medicine clinic participated in the study. RESULTS: Clinicians expressed preference for interruptive pop-up alerts during order entry, brief descriptions of relevant drug-gene interactions, and a clear and specific recommended alternative course of action when a medication is contraindicated. They did not wish to see detailed genetic data, preferring phenotypic information predicted from the genotype. Nor did they wish to be interrupted when genetic test results do not indicate a change in treatment plan. Clinicians reported little familiarity with Clinical Pharmacogenetic Implementation Consortium prescribing recommendations but reported trusting recommendations of their professional societies and resources like UpToDate. Analysis of unstructured comments concurred with structured results, indicating a general uncertainty among participants around how to interpret and apply PGx information in practice. DISCUSSION: Results point to several underlying principles that can inform future PGx CDS alert designs: Be Specific and Actionable; Be Brief; Display Phenotypes not Genotypes; Rely on Sources Clinicians Already Trust; and, Be Adaptable to Learning Effects. CONCLUSION: This study is part of a broader socio-technical design approach to PGx CDS design underway at NM and provides a baseline for future PGx CDS development. Designs based on these results have the potential to improve clinician education and adherence levels, and to improve patient outcomes.

Abstract:Pharmacogenetic testing could reduce the time to identify a safe and effective medication for depression; however, it is underutilized in practice. Major depression constitutes the most common mental disorder in the US, and while antidepressant therapy can help, the current trial –and error approach can require patients to endure multiple medication trials before finding one that is effective. Tailoring the fit of pharmacogenetic testing with prescribers' needs across a variety of settings could help to establish a generalizable value proposition to improve likelihood of adoption. We conducted a study to explore the value proposition for health systems using pharmacogenetic testing for mental health medications through prescribers' real‐world experiences using implementation science concepts and systematic interviews with prescribers and administrators from four health care systems. To identify a value proposition, we organized the themes according to the Triple Aim framework, a leading framework for health care policy which asserts that high‐value care should focus on three key metrics: (1) better health care quality and (2) population‐level outcomes with (3) reduced per capita costs. Primary care providers whom we interviewed said that they value pharmacogenetic testing because it would provide more information about medications that they can prescribe, expanding their ability to identify medications that best‐fit patients and reducing their reliance on referrals to specialists; they said that this capacity would help meet patients' needs for access to mental health care through primary care. At the same time, prescribers expressed differing views about how pharmacogenetic testing can help with quality of care and whether their views about out‐of‐pocket cost would prevent them from offering it. Thus, implementation should focus on integrating pharmacogenetic testing into primary care and using strategies to support prescribers' interactions with patients.

It is of particular importance in the management of high-risk patients in whom predictive genetic testing can alter health outcomes.PurposeThis review aims to identify the relative importance and willingness to pay for attributes of genetic testing in hereditary cancer syndromes.Data SourcesSearches were conducted in Medline, Embase, PsycINFO, HMIC, Web of Science, and EconLit using discrete choice experiment (DCE) terms combined with terms related to hereditary cancer syndromes, malignancy synonyms, and genetic testing.Study SelectionFollowing independent screening by 3 reviewers, 7 studies fulfilled the inclusion criteria, being a DCE investigating patient or public preferences related to predictive genetic testing for hereditary cancer syndromes.Data ExtractionExtracted data included study and respondent characteristics, DCE attributes and levels, methods of data analysis and interpretation, and key study findings.Data SynthesisStudies covered colorectal, breast, and ovarian cancer syndromes. Abstract:There is a growing number of evidence-based indications for pharmacogenetic (PGx) testing. We aimed to evaluate clinical relevance of a 16-gene panel test for PGx-guided pharmacotherapy. In an observational cohort study, we included subjects tested with a PGx panel for variants of ABCB1, COMT, CYP1A2, CYP2B6, CYP3A4, CYP3A5, CYP2C9, CYP2C19, CYP2D6, CYP4F2, DPYD, OPRM1, POR, SLCO1B1, TPMT and VKORC1. PGx-guided pharmacotherapy management was supported by the PGx expert system SONOGEN XP. The primary study outcome was PGx-based changes and recommendations regarding current and potential future medication. PGx-testing was triggered by specific drug–gene pairs in 102 subjects, and by screening in 33. Based on PharmGKB expert guidelines we identified at least one “actionable” variant in all 135 (100%) tested patients. Drugs that triggered PGx-testing were clopidogrel in 60, tamoxifen in 15, polypsychopharmacotherapy in 9, opioids in 7, and other in 11 patients. Among those, PGx variants resulted in clinical recommendations to change PGx-triggering drugs in 33 (32.4%), and other current pharmacotherapy in 23 (22.5%). Additional costs of panel vs. single gene tests are moderate, and the efficiency of PGx panel testing challenges traditional cost-benefit calculations for single drug–gene pairs. However, PGx-guided pharmacotherapy requires specialized expert consultations with interdisciplinary collaborations.

Abstract:Many analyses of bone microarchitecture using three-dimensional images of micro CT (microCT) have been reported recently. However, as extirpated bone is the subject of measurement on microCT, various kinds of information are not available clinically. Our aim is to evaluate usefulness of fractal dimension as an index of bone strength different from bone mineral density in in-vivo, to which microCT could not be applied. In this fundamental study, the relation between pixel size and the slice thickness of images was examined when fractal analysis was applied to clinical images. We examined 40 lumbar spine specimens extirpated from 16 male cadavers (30-88 years; mean age, 60.8 years). Three-dimensional images of the trabeculae of 150 slices were obtained by a microCT system under the following conditions: matrix size, 512 x 512; slice thickness, 23.2 em; and pixel size, 18.6 em. Based on images of 150 slices, images of four different matrix sizes and nine different slice thicknesses were made using public domain software (NIH Image). The threshold value for image binarization, and the relation between pixel size and the slice thickness of an image used for two-dimensional and three-dimensional fractal analyses were studied. In addition, the box counting method was used for fractal analysis. One hundred forty-five in box counting was most suitable as the threshold value for image binarization on the 256 gray levels. The correlation coefficients between two-dimensional fractal dimensions of processed images and three-dimensional fractal dimensions of original images were more than 0.9 for pixel sizes < or =148.8 microm at a slice thickness of 1 mm, and < or =74.4 microm at one of 2 mm. In terms of the relation between the three-dimensional fractal dimension of processed images and three-dimensional fractal dimension of original images, when pixel size was less than 74.4 microm, a correlation coefficient of more than 0.9 was obtained even for the maximal slice thickness (1.74 mm) examined in this study.
Scoping review of enablers and challenges of implementing pharmacogenomics testing in the primary care settings

Abstract:Introduction Pharmacogenomic testing (PGx) plays a crucial role in improving patient medication safety, yet ethical concerns and limitations impede its clinical implementation in the primary care settings. Aims To systematically review the current state of PGx in the primary care settings and determine the enablers and challenges of its implementation. Design A scoping review was carried out by adhering to Arksey and O'Malley's 6-stage methodological framework and the 2020 Joanna Briggs Institute and Levac et al. Data sources Cochrane Library, EMBASE, Global Health, MEDLINE and PubMed were searched up to 17 July 2023. Eligibility criteria All peer-reviewed studies in English, reporting the enablers and the challenges of implementing PGx in the primary care settings were included. Date extraction and synthesis Two independent reviewers extracted the data. Information was synthesised based on the reported enablers and the challenges of implementing PGx testing in the primary care settings. Information was then presented to stakeholders for their inputs. Results 78 studies discussing the implementation of PGx testing are included, of which 57% were published between 2019 and 2023. 68% of the studies discussed PGx testing in the primary care setting as a disease-specific themes. Healthcare professionals were the major stakeholders, with primary care physicians (55%) being the most represented. Enablers encompassed various advantages such as diagnostic and therapeutic benefits, cost reduction and the empowerment of healthcare professionals. Challenges included the absence of sufficient scientific evidence, insufficient training for healthcare professionals, ethical and legal aspects of PGx data, low patient awareness and acceptance and the high costs linked to PGx testing. Conclusion PGx testing integration in primary care requires increased consumer awareness, comprehensive healthcare provider training on legal and ethical aspects and global feasibility studies to better understand its implementation challenges. Managing high costs entails streamlining processes, advocating for reimbursement policies and investing in research on innovation and affordability research to improve life expectancy.

Abstract:Background: Striking the right balance between early cancer diagnosis and the risk of excessive testing for low-risk symptoms is of paramount importance. Patient-centred care must also consider patient preferences for testing. Aim: To investigate diagnostic testing preferences of the Australian public for symptoms associated with oesophagogastric (OG), bowel, or lung cancer. Design and Setting: One of three discrete choice experiments (DCEs) related to either OG, bowel, or lung cancer were administered to a nationally representative sample of Australians aged 40 and above. Methods: Each DCE comprised three scenarios with symptom positive predictive values (PPVs) for undiagnosed cancer ranging from 1% to 3%. The numerical risk was concealed from participants. DCE attributes encompassed the testing strategy, GP familiarity, test and result waiting times, travel duration, and test cost. Preferences were estimated using conditional and mixed logit models. Results: A total of 3013 individuals participated in one of three DCEs: OG (n=1004), Bowel (n=1006), and Lung (n=1003). Preferences were chiefly driven by waiting time, test cost followed by the test type. There was preference for more invasive tests. When confronted with symptoms carrying an extremely low risk (symptom PPV of 1% or less), participants were more inclined to abstain from testing. Conclusions: Access-related factors, particularly waiting times and testing costs, emerged as the most pivotal elements influencing preferences, underscoring the substantial impact of these systemic factors on patient choices regarding investigations.
