Abstract:AIM: To sum up the clinical and pathological characteristics of solid pseudopapillary tumor (SPT) and the experience with it. METHODS: A total of 553 SPT patients reported in Chinese literature between January 1996 and January 2009 were retrospectively reviewed and analyzed. RESULTS: The mean age of the 553 SPT patients included in this review was 27.2 years, and the male to female ratio was 1:8.37. Their symptoms were nonspecific, and nearly one third of the patients were asymptomatic. Computed tomography and ultrasonography were performed to show the nature and location of SPT. Most of the tumors were distributed in the pancreatic head (39.8%), tail (24.1%), body and tail (19.5%). Forty-five patients (9.2%) were diagnosed as malignant SPT with metastasis or invasion. None of the clinical factors was closely related to the malignant potential of SPT. Surgery was the main therapeutic modality for SPT Local resection, distal pancreatectomy and pancreatoduodenectomy were the most common surgical procedures. Local recurrence and hepatic metastasis were found in 11 and 2 patients, respectively, after radical resection. Four patients died of tumor progression within 4 years after palliative resection of SPT The prognosis of SPT patients was good with a 5-year survival rate of 96.9%. CONCLUSION: SPT of the pancreas is a rare indolent neoplasm that typically occurs in young females. It is a low-grade malignancy and can be cured with extended resection. The prognosis of such patients is good although the tumor may recur and metastasize. (C) 2010 Baishideng. All rights reserved.
Computed Tomography and Magnetic Resonance Imaging Features of Solid Serous Cystadenomas of the Pancreas.

Abstract:Solid serous cystadenomas (SSC) of the pancreas are the rarest benign type of serous cystic neoplasms and are frequently misdiagnosed, resulting in unnecessary invasive surgical procedures. A retrospective study was performed to collect imaging features of SSC using computed tomography (CT) and magnetic resonance imaging. The clinical data and imaging study of 5 female patients with pathologically confirmed SSC (mean age, 44.2 years; range, 23-69 years) diagnosed between January 2006 and July 2017 were retrospectively analyzed. The imaging study was assessed by two experienced radiologists in consensus. All patients had tumors with a mean size of 2.3 cm (range, 1.5-3.2 cm; 4/5 lesions ≤3 cm). The tumors were located in the tail of pancreas in 3 cases and in the neck of the pancreas in 2 cases. The tumors were all well defined, with an ovoid and lobulated shape and fibrous capsules. Consequently, they presented as low density on unenhanced CT images, marked hyperintensity on T2-weighted images in 4 cases and slight hyperintensity in 1 case. However, they exhibited heterogeneous wash-in and wash-out enhancement in 2 cases and moderate and prolonged enhancement in 3 cases. The apparent diffusion coefficient (ADC) values of the lesions were >2×10-3 mm2/sec. Therefore, a small tumor size, signal intensity on T2-weighted images, high ADC values, fibrous capsules and enhancement patterns may be indicative imaging features of SSCs.

Abstract:A 49-year-old female undergoing a periodic health examination at other hospital revealed a mass in the tail of pancreas. The patient denied any personal history of surgery except subtotal hysterectomy because of multiple myomas in uterus 7 years ago, family history of abdominal cancer and trauma. Physical examination and laboratory finding (including tumor marker) were unremarkable. Chest X-ray result was normal. Contrast enhanced ultrasound (CEUS) examination showed a well-defined hypoechoic pancreatic mass which was suggestive of solid pseudopapillary tumor. Contrast enhanced computed tomography (CE-CT) of the abdomen revealed a mass of hypodensity suggestive of intraductal papillary mucinous neoplasm. Because of the risk of bleeding and exclusion of surgical contraindications, patient underwent laparoscopic surgery. Intraoperatively, a solid mass was identified in the tail of pancreas, the intraoperative frozen pathological examination suggested a heterotopic accessary spleen (HAS) with squamous epithelial cyst. Partial pancreatectomy was performed. The uniqueness of this case is that the spleen can be ectopic to the pancreas, what is even more unexpected is that the HAS undergone cystic change. When encountering a pancreatic mass, we need to think about the possibility of HAS. In conclusion, it is important to diagnose HAS with squamous cyst in the pancreatic tail presenting as other pancreatic masses.
Solid Pseudopapillary Neoplasm of the Pancreas: Clinicopathologic Feature, Risk Factors of Malignancy, and Survival Analysis of 53 Cases from a Single Center

Abstract:Introduction . Solid pseudopapillary neoplasm (SPN) of the pancreas is a rare tumor of low malignant potential. The aim of this study was designed to evaluate the clinicopathologic feature, predictive factors of malignancy, and survival from experience of a single center. Methods . 53 consecutive patients who underwent surgery for a pathologically definitive SPN were retrospectively reviewed. Results . A total of 53 cases included 7 male cases and 46 female cases with the median age of 35.4 years (14–67). Abdominal pain and mass were the most common clinical presentations. The radiological presentations were consistent with solid and cystic pattern in 18 cases, solid pattern in 25 cases, and cystic pattern in 10 cases. The predominant location of tumor was pancreatic body and tail. The mean size of the tumors was 6.4 cm. Aggressive en bloc resection combined with organ-preserving should be indicated whenever feasible. Follow-up information was available for 48 patients with a median follow-up time of 48 months. The 5-year disease-specific survival was 95.7%. Incomplete capsule was not only the predictive factor of malignancy but also the significant predictor of disease-specific survival. Conclusion . Incomplete capsule may suggest a malignant SPN and a prognostic indicator of disease-specific survival. We recommend that surgeons consider a more radical resection with an incomplete capsule of tumor.

Abstract:Background: Solid pseudopapillary neoplasm of the pancreas (SPN) is a rare and low-grade malignant tumor. It mainly occurs in women of reproductive age, accounting for approximately 1-3% of all pancreatic tumors. SPN has a low incidence rate and is difficult to diagnose before surgery. Some cases may show local infiltration, but distant metastasis rarely occurs. Currently, there is no standardized treatment protocol for SPN. Patient and methods: We have collected clinical data from 5 patients with solid pseudopapillary neoplasm (SPN) of the pancreas who presented with distant metastasis at our hospital. This study retrospectively analyzes their clinical manifestations, imaging characteristics, pathological findings, and treatment outcomes. The aim is to summarize the clinical features of SPN with distant metastasis, thereby improving the diagnosis, treatment, and prognosis prediction of this disease. This study also reviews relevant literature. Results: The median age of the 5 patients was 32 years old, with a male-to-female ratio of 1:4. All patients underwent enhanced CT scans and were diagnosed with SPN through biopsy or surgical pathology. All 5 patients had liver metastases, and one patient had clavicular lymph node metastasis. Another patient had both lung and clavicular lymph node metastases. Three patients underwent curative surgery, one patient received chemotherapy combined with targeted immunotherapy and subsequently underwent TACE(Transcatheter arterial chemoembolization) and HAIC (Hepatic artery infusion chemotherapy) treatments due to progression. One patient received internal radiation therapy but experienced multiple relapses and eventually died due to complications. The follow-up period ranged from 7 to 53 months, with 2 patients succumbing to the disease. Conclusion: As a low-grade tumor, SPN has a low rate of distant metastasis, typically occurring in only 5%-15% of cases. These metastases often lack characteristic clinical symptoms. Diagnosis can only be confirmed after exclusion of other lesions through imaging and pathological examination. The primary treatment for metastatic SPN is curative surgery, which can lead to a favorable prognosis.

Abstract:Objective To study the clinicopathological characteristics and immunophenotype of solid pseudopapillary neoplasm (SPN) of the pancreas.Methods Retrospective analysis was performed for the clinical materials,pathological features and immunohistochemical phenotype characteristics of the 23 SPN cases.Results Ratio of male to female was 1:10;the average age was 32 years.10(43％) patients had no clinical symptoms,12 (52％) patients had abdominal pain,abdominal distention or diarrhea;1 patient had jaundice.Tumors were in the head of pancreas in 10(43％) cases,and were in the tail or body of pancreas in 13 cases.Grossly,tumors were solid-cystic or solid;the maximum diameter were from 1.3 cm to 17 cm,with the average of 5.9 cm;12 (52％) cases appeared encapsulated.Histopathologically,21 (91％) cases appeared to exhibit a fibrous capsule surrounding the tumors,and the fibrous capsule invasion were observed in all these cases;Pancreas tissues were involved in 8(35％) cases.Tumor cells arranged in sheets,nests and pseudopapillary patterns.Hemorrhage,necrosis and cystic degeneration were often seen.Tumor cells were positive for vimentin,β-catenin,CK,CD10,PR,CD56 and Syn.Tumor were mostly negative for CgA and Ki-67.23 cases were followed up for 2 to 44 months,and all were alive.Conclusion SPN is a low degree malignant tumor often seen in young women with various histological patterns and multiple immunophenotypes.Definite diagnosis of SPN can be made by combining clinicopathological characteristics with a panel of immunohistochemicat marks.SPN grows slowly,often responds to surgical resection and rarely recur.
Management of Solid Pseudopapillary Neoplasms of Pancreas: A Single Center Experience of 243 Consecutive Patients

Abstract:Background: Solid pseudopapillary neoplasm of the pancreas (SPN) is a rare neoplasm, which mainly affects young women. The aim of this study was to investigate the clinicopathological features and surgical management of SPNs in our institution. Methods: Patients who underwent surgery for a pathologically confirmed SPN in our institution between January 2008 and October 2018 were collected. Their clinical characteristics and survival associations were analyzed. Results: In total, 243 pathologically confirmed patients were analyzed in this study, including 181(74.5%) females and 62(25.5%) males. The mean age was 35.3 years old (range: 12-64 years old) with average tumor size of 4.83 cm (range: 0.8-16 cm). 239 patients underwent complete surgical resection. After median follow-up of 46 months (range: 10-118 months), four patients died due to tumor progression. All the other people were absent of local recurrence or distant metastasis. Conclusions: SPN is a latent malignant tumor with excellent prognosis. Surgical resection is recommended even in the presence of liver metastasis. If possible, function-preserving surgery is advocated. High Ki67 index may predict the malignant potential and poor prognosis of SPNs. (C) 2019 Published by Elsevier B.V. on behalf of IAP and EPC.

Abstract:Solid pseudopapillary neoplasm (SPN) is a rare pancreatic tumor accounting for 1%-3% of all pancreatic tumors. SPNs are slow-growing with low malignant potential and are predominantly diagnosed in females with a female-to-male ratio of 10:1. They most commonly occur in the second or third decade of life. The most common presenting symptom is abdominal pain and is present in up to 45% of patients at diagnosis. The standard treatment is surgical resection to mitigate the risk of malignancy. To date, there have been only 8 published cases of acute pancreatitis leading to a SPN diagnosis. Of these, only 2 cases have been in men, both greater than 30 years of age. This case presents a rare instance of SPN in an adolescent male initially diagnosed with acute pancreatitis.

Abstract:Pancreatic solid pseudopapillary neoplasms (SPNs) are rare and relatively benign tumors, with a malignancy ratio of 10–15%. The utility of multiple imaging modalities, combining with age and gender profile, is crucial for the diagnosis of SPNs. At present, surgery remains the only curative method for SPNs. While opinions towards surgical procedures are highly divided due to its rarity, minimally invasive procedures for SPNs are gradually recommended, whether extent of resection or surgical path. Although patients with SPNs always have a favorable prognosis, postoperative follow-ups remain essential. In general, we mainly discussed the diagnosis, treatment, and follow-up for patients with SPNs.

Abstract:Background: Solid pseudopapillary neoplasms (SPNs) of the pancreas are uncommon neoplasms and are potentially malignant. Complete resection is advised due to rare recurrence and metastasis. Duodenum-preserving pancreatic head resection (DPPHR) is indicated for SPNs located in the pancreatic head and is only performed using the open approach. To the best of our knowledge, there are no reports describing laparoscopic DPPHR (LDPPHR) for SPNs. Methods: Herein, we report a case of 41-year-old female presented with a 1-week history of epigastric abdominal discomfort, and founded an SPN of the pancreatic head by abdominal computed tomography/magnetic resonance, who was treated by radical LDPPHR without complications, such as pancreatic fistula and bile leakage. Histological examination of the resected specimen confirmed the diagnosis of SPN. Results: The patient was discharged 1 week after surgery following an uneventful postoperative period. She was followed up 3 months without readmission and local recurrence according to abdominal ultrasound. Conclusion: LDPPHR is a safe, feasible, and effective surgical procedure for SPNs.

Abstract:AIM To describe the imaging features of serous neoplasms of the pancreas using ultrasound, endoscopic ultrasound, computed tomography and magnetic resonance imaging. METHODS This multicenter international collaboration enhances a literature review to date, reporting features of 287 histologically confirmed cases of serous pancreatic cystic neoplasms (SPNs). RESULTS Female predominance is seen with most SPNs presenting asymptomatically in the 5th through 7th decade. Mean lesion size was 38.7 mm, 98% were single, 44.2% cystic, 46% mixed cystic and solid, and 94% hypoechoic on B-mode ultrasound. Vascular patterns and contrast-enhancement profiles are described as hypervascular and hyperenhancing. CONCLUSION The described ultrasound features can aid differentiation of SPN from other neoplastic lesions under most circumstances.

Abstract:Solid pseudopapillary neoplasms (SPN) of the pancreas are presently recognized as low-grade malignant tumors that are frequently observed in young females. This tumor has a low incidence and is associated with an excellent prognosis following surgical resection. Typical SPNs primarily affect the pancreas and tend to have moderate or asymptomatic manifestations. Based on retrospective research, it is anticipated that patients with SPN can achieve disease-free survival, even in cases when metastasis is detected during inspection. However, the incidence of malignant SPN has been consistently underestimated, as evidenced by recent research findings. Malignancy of SPN primarily encompasses invasion and infiltration, metastasis, and recurrence after R0 resection. Imaging technologies such as Ultrasound, Computed Tomography, Magnetic Resonance Imaging, and Position Emission Tomography are capable of preliminarily identifying malignant SPN, which is primarily based on its invasive clinical features. Research on risk factors of malignant SPN revealed that larger tumor size, Ki-67 index, and several other parameters had significant correlations with invasive tumor behavior. Pathologic features of malignant SPNs overlay other pancreatic tumors, nevertheless they can provide valuable assistance in the process of diagnosis. Several confirmed specific pathologic biomarkers are related to its cellular origin, characteristic gene mutation, and cell proliferation. Considering the invasiveness of malignant SPN, it is imperative to enhance the comprehensiveness of its therapy. Tumor resection remains a suggested course of action in line with typical SPN, and additional lymph node dissection is seen as reasonable. Compared to benign SPNs, malignant SPNs have worse prognosis, underscoring the necessity of early identification and treatment in comprehensive medical centers to get improved clinical outcomes.
Solid Pseudopapillary Tumor of the Pancreas: a Case Series of 26 Consecutive Patients

Abstract:OBJECTIVE: Solid pseudopapillary tumor (SPT) of the pancreas, which predominantly affects young women, is a relatively indolent entity with favorable prognosis. The aim of this study is to describe the clinicopathologic features and surgical management of this disease in our institution.METHODS: A retrospective study of clinical data from 26 consecutive patients with SPT managed in a tertiary academic center between January 2002 and December 2007 was performed. Clinicopathologic factors were compared between benign and malignant cases to determine what features of the tumor could suggest malignant potential.RESULTS: The 26 cases included 22 female and 4 male patients, and the average age was 32.3 years (range 15 to 64). Clinical symptoms were nonspecific and included upper abdominal pain or discomfort, abdominal distention, and back pain. The neoplasm was localized in the pancreatic head/neck in 14 patients and in the body/tail in 12 patients. The median diameter of these lesions was 6.25 cm (range 2 to 15). All of the tumors-including 8 pancreaticoduodenectomies, 10 distal pancreatectomies, 6 local resections, 1 total pancreatectomy, and 1 central pancreatectomy-were resected successfully. No patient received chemotherapy or radiotherapy after surgery. All of the patients except 1 were alive at a median follow-up of 32.5 months (range 3 to 69). One of the 2 patients with malignant SPT, in whom Ki-67 immunoreactivity was >25%, developed local recurrence with liver metastasis 4 months and died 6 months after surgery. There were no significant associations between clinicopathologic factors and malignancy.CONCLUSIONS: SPT is a rare neoplasm with low malignant potential. Characteristic computed axial tomography and magnetic resonance imaging scans combined with age and sex profile should be sufficient for the decision to operate. Patients with malignant SPT should have careful follow-up. The high proliferative index assessed by immunohistochemical staining for Ki-67 may predict poor outcome of malignant SPT. Abstract:Introduction: Solid pseudopapillary tumors (SPT) account for 1% to 3% of all pancreatic tumors. They have low malignant potential with a favorable prognosis, and predominantly occur in young women. The pathogenesis and clinical behavior of SPT are still uncertain. In addition, most ruptures of SPT were associated with blunt abdominal trauma, while spontaneous ruptures seemed to be quite rare. Up to now, there have been only 3 spontaneous ruptured SPT cases reported worldwide. Patient concerns: Here, we reported a 22-year-old female patient with left lower abdominal pain. Computed tomography (CT) showed that a hemorrhagic complex solid cystic mass located in the lesser omentum sac. Diagnosis: According to pathological findings of tumor specimen, the diagnosis of solid pseudopapillary tumor (SPT) of the pancreas was made. Interventions: Distal pancreatectomy and splenectomy was carried out. Outcomes: The patient recovered to normal status within 10 days after surgery. Conclusion: Besides, we reviewed about 50 cases in literatures to find out the clinical characteristics and differential diagnostic strategies of SPT.

Abstract:Pancreatic tumors have an annual incidence of 0.19 cases per million in the pediatric population in North America. Solid pseudopapillary neoplasms (SPN), an indolent tumor with low malignant potential, comprise 70% of pediatric pancreatic tumors. It has been described mostly in adolescents, mean age of 13-14 years. SPN commonly occurs in the pancreatic tail with surgery being the mainstay of treatment. We present a case of a 13-year-old female who presented with acute onset abdominal pain found to have SPN based on endoscopic ultrasound assisted biopsy with spontaneous regression of the mass.

We argue—against the commonly advocated “classical method”—that no significant benefits are likely to accrue from unequal weighting in mathematical aggregation. Our argument hinges on the difficulty of constructing reliable and valid measures of substantive expertise upon which to base weights. Practical problems associated with attempts to evaluate experts are also addressed. While our discussion focuses on one specific weighting scheme that is currently gaining in popularity for expert knowledge elicitation, our general thesis applies to externally imposed unequal weighting schemes more generally.
Solid Pseudopaillary Neoplasm of the Pancreas: Analysis of Clinicopathological and Immunohistochemical Features in 10 Cases.

Abstract:Objective: The aim of this study was to investigate the clinicopathological features and immunomarkers of Solid Pseudopapillary Neoplasm of the Pancreas (SPN) and to find the best possible immunomarker combination that can accurately diagnose it. Methods: We retrospectively analyzed 10 patients who underwent surgery for pathologically confirmed SPN from August 2013 to August 2017. Follow-up of the patients was between 9 and 57 months. Results: Clinical symptoms and imaging were atypical. In general, the mass was encapsulated and clearly defined by the surrounding tissues. Cut surface was dusty-red and associated with hemorrhage. The neoplastic cell cytoplasm was eosinophilic or clear, and the nuclei were round or oval, presenting typical features of pseudopapillary distribution around a fibrovascular core. Immunohistochemical results showed that tumor cells were consistently positive for vimentin, CD56, CD10, PR, CD99, beta-catenin and negative for E-cadherin (100%) and chromogranin. CD99 presented a unique dot-like staining pattern in tumor cells. Conclusions: In view of the atypical clinical manifestations and imaging features of SPN, accurate diagnosis mainly relies on the histomorphology and immunomarker combination including PR, CD99, beta-catenin, and E-cadherin, which might be useful method in the diagnosis of SPN.
Features and Treatment of Peritoneal Metastases from Solid Pseudopapillary Neoplasms of the Pancreas

Abstract:Background: Solid pseudopapillary neoplasms (SPNs) of the pancreas may present widespread peritoneal metastases, but the treatment of this malignancy has not been well described and requires further investigation. Material/Methods: Four cases of SPN with significant peritoneal metastases in our department were operated and retrospectively summarized after long-term follow-up. Eight more cases of peritoneal metastatic SPN from the PubMed database were also included in the analysis. Results: Peritoneal metastases of SPNs have different gross features. The benign nodules were tenacious and well encapsulated, while the malignant nodules were soft and prone to slow bleeding. However, neither of these nodules invaded the small intestines or mesentery. Of the 12 disseminated cases, 7 had history of primary tumor rupture, whereas the others had tumors malignant in nature. A total of 14 surgical events were documented, including 3 complete cytoreductive surgeries (CCRS), 9 cytoreductive surgeries (CRS), and 2 debulking surgeries. After follow-up ranging from 0.3 to 6.1 years, the results of the Fisher's exact test showed no difference between CCRS and CRS in treating either low-grade or high-grade malignant SPNs (P=0.257 and P=0.203, respectively). For all cases of SPN with peritoneal metastases, the CCRS procedure could significantly improve tumor-free survival (TFS) compared to the CRS procedure (P=0.046). Conclusions: SPN rupture could cause significant peritoneal metastases, and either disruption or biopsy of these lesions should be avoided. Peritoneal metastases from SPNs vary both in gross features and biological mechanisms. CCRS may offer optimal therapeutic outcomes and longer TFS for individuals with significant peritoneal metastases of SPNs.
