Abstract:Objective: To investigate the expression of intercellular adhesion molecule-1 (ICAM-1) and vascular cell adhesion molecule-1 (VCAM-1) in allograft with chronic rejection (CR) after heart transplantation. Methods: Seventy-two SD rats receiving transplantation of the heats of 71 Wistar rats were divided into 4 equal groups: Group A, undergoing heterotopic cervical heart transplantation (transplanted with donor's heart subcutaneously at the neck) and receiving none intervention, and with the transplanted hearts taken out 3 d after transplantation to the end of CR; Group B, injected intravenously with the splenocytes (SPCs) of the donors on day 0, injected with cyclophomide (CP) on d2, transplanted with the donor hearts on d15, with the transplanted hearts taken out 15 - 120 days after transplantation; Group C, transplanted with the donor's heart and injected intraperitoneally with cyclosporine A (Cs A) 10 mg/kg every other day for 8 - 10 times, and with the transplanted hearts taken out 60 d after transplantation to the end of CR; and Group D, injected intravenously with the SPCs of the donors on day 0, injected with CP on d2, transplanted with the donor hearts on d15, with the transplanted hearts taken out 150 - 420 days after transplantation. Immunohistochemical method was used to detect the expression of ICAM-1 and VCAM-1 in the allografts. Results: The cardiac allograft survival time of Group B was the longest. Less ICAM-1 and VCAM-1 expression and lymphocyte infiltration were observed in SPC and CP-pretreated group. Reversely, protein expression levels of ICAM-1 and VCAM-1 were both high and significant lymphocyte infiltration was seen in Groups B and D. There was no significant difference in the expression of ICAM-1 and VCAM-1 between Groups A and C (P > 0.05). Conclusion: The expression levels of ICAM-1 and VCAM-1 are associated with the occurrence and development of rejection and have positive correlation with the severity of graft rejection. Determination of ICAM-1 and VCAM-1 can predict the function of allograft and provide evidence for early diagnosis and prevention of CR.

Abstract:Background Acute rejection is still one of the main complications which enhances the cost and the risk to renal graft failure. Chemokines, interacting with respective receptors, can recruit leukocytes into grafts and mediate allograft rejection. In this study, we aimed to analyze gene expression of chemokines including CCL5/RANTES, CXCL10/IP-10, CXCL13/BCA-1, and receptors of CCR5, CXCR3, CXCR5 in peripheral blood mononuclear cells (PBMCs) during acute renal allograft rejection Methods Gene expression of all these chemokines and receptors in PBMCs were analyzed by real-time PCR from 14 stable recipients, 32 biopsy-proven acute rejection (AR), and 5 acute tubular necrosis (ATN). Results Gene expression of CCL5, CXCL10, CXCL13, and CCR5 were up-regulated both in AR and ATN group compared to stable recipients (fold change > 2, P < 0.05). Serum creatinine recovered to baseline level after anti-rejection therapy was defined as AR-sensitive and creatinine maintained above 200 μmol/L as AR-resistant. Expression of CXCL10 and CXCL13 were 5.98-, 2.94-, and 20.5, 10.8-fold change in AR-resistant and AR-sensitive compared to stable recipients, respectively. The expression of CXCL10 and CXCL13 was a twofold change in AR-resistant compared to AR-sensitive recipients ( P < 0.05). Five out of ten AR-resistant recipients lost graft function in the follow-up. Conclusion CXCL10 and CXCL13 expression were highly up-regulated in PBMCs in acute renal allograft rejection, especially in poor response to anti-rejection therapy and detrimental prognosis.
FcγRIIIb Inhibits Renal Transplant Rejection by Inducing Neutrophils Apoptosis

Abstract:Fcγ receptors (FcÎ³R) have been proposed to be involved in the pathogenesis of acute rejection (AR) after organ transplantation. However, the mechanisms by which FcγRIIIB mediate this process are still unclear. Previous studies suggested that the FCGR3B gene copy number variation (CNV) is associated with the risk of autoimmune diseases. In this study, we delineated the association of FCGR3B-CNV with end-stage renal disease (ESRD) and risk of rejection post kidney transplantation in 414 kidney transplant recipients and 219 healthy subjects. Our data showed that the percentage of kidney transplant recipients with high copy number of FCGR3B gene was significantly lower than that of healthy controls, suggesting that individuals with low copy of FCGR3B gene are more prone to develop ESRD. Besides, patients with low copy of the FCGR3B gene are associated with susceptibility to AR following kidney transplantation. In mice, transgenic expression of human FcγRIIIB effectively inhibited the AR and improved the function of the transplanted kidney in a model of allogeneic kidney transplantation. This protective effect was associated with an inhibition of T lymphocytes infiltration and Th1 cell immune response. Finally, we showed that FcγRIIIB cross-linking combined with TNF-α treatment in vitro promoted neutrophil apoptosis and was associated with increased activation of cleaved caspase-8 and caspase-3. Abstract:Background: Effective non-invasive monitoring method to tell histopathology is a big challenge in renal transplantation.Methods: We used 70-mer long oligonucleotide array with 449 immune related genes to determine gene expression profiles of peripheral blood mononuclear cells (PBMCs) under different immune status including stable renal function (TX), acute tubular necrosis (ATN), biopsy conformed acute rejection (AR), clinical rejection with pathology of borderline changes (BL), clinical rejection without biopsy proven/presumed rejection (PR) and renal dysfunction without rejection (NR).Results: Distinct molecular expression signatures in each group were found to correlate with histopathology. And we concluded that B cell chemokine CXCL13 and mast cell may play a role in renal allograft rejection through significant difference analysis and functional pathway analysis.Conclusions: It provides a potential non-invasive method for monitoring renal allograft function and immune status of renal transplant recipients. Abstract:OBJECTIVE:To determine the diagnostic and clinical significance of C4d accumulation in renal allografts followed by acute rejection. METHODS:A total of 158 graft biopsies performed from December 1997 to December 2002 were classified, according to the Banff-97 criteria, into hyperacute rejection (HAR, three cases), acute vascular rejection (AVR, 27), acute cellular rejection (ACR, 24), borderline rejection (BR, 38), acute tubular necrosis (ATN, five), stable graft function (SGF, 30) and baseline kidney (31). Immunohistochemical technique was used to determine the C4d deposition level. RESULTS:The percentages of C4d positive in HAR, AVR, ACR, BR, ATN, SGF and baseline kidney groups were 100% (3/3), 77.8% (21/27), 37.5% (9/24), 23.7% (9/38), 0% (0/5), 3.3% (1/30), 0% (0/31), respectively. In acute rejection patients, the peak serum creatinine (sCr) level in C4d(ptc)-positive group (41 cases) was 334.82 +/- 238.37 micromol/L, with that of C4d(ptc)-negative group (47 cases) being 220.20 +/- 176.94 micromol/L (p < 0.01). After treatment, the trough sCr level in C4d(ptc)-positive group and C4d(ptc)-negative group were 176.87 +/- 111.80 and 121.75 +/- 34.59 micromol/L (p < 0.01), respectively. In each AVR, ACR and BR subgroups, the peak sCr level, the trough sCr level, after 3 or 6 months of AR, the sCr level in C4d(ptc)-positive subgroup was higher than that of C4d(ptc)-negative subgroup. There were more resistance against steroid therapy [65.9% (27/41) vs. 36.2% (17/47), p = 0.005] and a higher rate of graft loss [29.3% (12/41) vs. 6.4% (3/47), p = 0.001] in C4d(ptc)-positive group than those of C4d(ptc)-negative group. In each C4d(ptc)-positive subgroup of AVR, ACR and BR the complete reversion was 57.1, 56 and 66.7%, respectively, it is almost same. CONCLUSION:The C4d deposition level is of great value in diagnosis of acute rejection caused by humoral immune components. It is a significant predictor of graft survival and will be of great help when treating acute rejection.
Urinary C-X-C Motif Chemokines 13 is a Noninvasive Biomarker of Antibody-Mediated Renal Allograft Rejection

Abstract:Background Since acute rejection remains one of the major complications which necessitate periodic surveillance, noninvasive diagnostic/prognostic methods are preferred by renal transplant recipients. Here we explored whether urinary C-X-C motif chemokines 13(CXCL13) could be apotential candidate to reflect ongoing immune processes within the renal graft. Methods We investigated urinary CXCL13 levels by a cross-sectional analysisof 146 renal allograft recipients and 40 healthy controls. Besides, a subset of patients (n = 57) were followed-up for kinetic monitoring of immune status. Results Urinary CXCL13/Cr was lower in normal transplants compared to those withacute tubular necrosis (ATN, P = 0.001), chronic allograft nephropathy (CAN, P = 0.01) andacute rejection (AR, P < 0.0001), which yielded a good diagnosis performance of urinary CXCL13 for AR (AUC = 0.818, P< 0.0001). Interestingly, urinary CXCL13 furtherdistinguished acute antibody mediated rejection (ABMR)from acute cellular rejection, with an AUC of 0.856. Besides,patients with steroid-resistant acute rejection had distinctly greater urinary CXCL13/Cr levels than patients with reversible acute rejection, P =0.001. Follow-up data revealed that urinary CXCL13/Cr varied in line with the occurrence of ABMR. Furthermore, elevated levels of urinary CXCL13/Cr within the first monthwas predictive of graft function at 3, 6 months, P=0.044 and 0.4. Conclusion This study demonstrated that monitoring of urinary CXCL13/Cr might be a valuable noninvasive approach for the detection of AR, especially ABMR. Additionally, high urinary CXCL13/Cr levels related to the poor response to steroid treatment and predicted a compromised graft function after AR. Cell Type of Renal Allograft Glomerulitis and Its Relationship with Peritubular Capillary C4d Deposition

Abstract:Objective To investigate the cell types of renal allograft intimal arteritis in posttransplantation patients and its relationship with peritubular capillaries (PTCs)C4d deposition. Methods Twenty allograft kidney transplant recipients from Jun 2006 to Jun 2008 were enrolled in the retrospective study. Twenty-one biopsy specimens with acute vascular rejection were immunostained for macrophages, T cells and C4d. In each biopsy specimen, arterial intimal macmphages and T cells were counted, and mean number of macrophages per "artery and T cells per artery were determined. The recipients were divided into C4d + and C4d- group according to whether C4d deposition in the PTCs. Results In the intimal arteritis of biopsies diagnosed as acute vascular rejection, the infiltrating cells were predominantly macrophages including C4d+ and C4d- group. T cells were the minority. The mean macrophage number per artery cross-section was significantly higher than the mean T cell number per artery cross-section in the C4d + group (12.45±9.86 vs 3.91±3.03, P=0.007) and in the C4d- group (3.47±1.89 vs 1.45±1.37, P=0.006). Forther more, the mean number of macrophage per artery cross-section in the C4d+ group was significantly higher than that in the C4d- group (P=0.007). Conclusions In the intimal arteritis of biopsies diagnosed as acute vascular rejection, the infiltrating cells are predominantly macrophages which has relationship with peritubular capillary C4d deposition. The mean number of macmphage per artery cross-section in the C4d+ biopsies is significantly higher than that in the C4d- biopses.
Preliminary Investigation of the Biomarkers of Acute Renal Transplant Rejection Using Integrated Proteomics Studies, Gene Expression Omnibus Datasets, and RNA Sequencing.

Abstract:A kidney transplant is often the best treatment for end-stage renal disease. Although immunosuppressive therapy sharply reduces the occurrence of acute allograft rejection (AR), it remains the main cause of allograft dysfunction. We aimed to identify effective biomarkers for AR instead of invasive kidney transplant biopsy. We integrated the results of several proteomics studies related to AR and utilized public data sources. Gene ontology (GO) and pathway analyses were used to identify important biological processes and pathways. The performance of the identified proteins was validated using several public gene expression omnibus (GEO) datasets. Samples that performed well were selected for further validation through RNA sequencing of peripheral blood mononuclear cells of patients with AR (n = 16) and non-rejection (n = 19) from our medical center. A total of 25 differentially expressed proteins (DEPs) overlapped in proteomic studies of urine and blood samples. GO analysis showed that the DEPs were mainly involved in the immune system and blood coagulation. Pathway analysis showed that the complement and coagulation cascade pathways were well enriched. We found that immunoglobulin heavy constant alpha 1 (IGHA1) and immunoglobulin κ constant (IGKC) showed good performance in distinguishing AR from non-rejection groups validated with several GEO datasets. Through RNA sequencing, the combination of IGHA1, IGKC, glomerular filtration rate, and donor age showed good performance in the diagnosis of AR with ROC AUC 91.4% (95% CI: 82–100%). Our findings may contribute to the discovery of potential biomarkers for AR monitoring.
Expression of Anti-Mica Antibodies and Its Influence on Acute Rejection and Allograft Function of Sensitized Renal Recipients in Early Post-Transplant Course

Abstract:Objective To explore the expression of anti-MICA antibodies and evaluate its influence on acute rejection and renal function in early period after renal transplantation. Methods A total of 29 sensitized subjects (PRA＞20 %) were enrolled in this study. All the patients underwent protein A immunoabsorption treatment and the expression of anti-MICA antibodies was detected before and after treatment. Triple immunosuppressive regimen consisting of tacrolimus, mycophenolate mofetil (MMF) and steroid was given to prevent graft rejection. The correlation between the expression of anti-MICA antibodies and acute rejection or serum creatinine (SCr) level was analyzed.Results The expression of anti-MICA antibodies was detected in 8 candidates (27. 6 % ,8/29) ,and 6 kinds of anti-MICA antibodies simultaneously expressed were found in one individual, 3 kinds in one case,and sole kind in 6 patients. There was no significant difference in acute rejection rate between positive anti-MICA antibodies group and negative group [37.5 % (3/8) vs 38. 1% (8/21), P＞0.05). The positive expression rate of anti-MICA antibodies in the recipients with PRA ≥40% was higher than that in those with PRA ＜40% [43. 8 % (7/16) vs 7. 7 % (1/13),P＜0.05]. The SCr level in patients positive for anti-MICA antibodies was markedly higher than that in those negative anti-MICA antibodies at the 1st week postoperatively ( 135.4 ± 21.4 vs 108. 6 -+ 31.6 μmol/L, P＜0.05). The SCr level in the patients with positive anti-MICA antibodies, however, was reduced to the normal range at the 2nd week after surgery (P＞0.05). The levels of anti-MICA antibodies were continuously decreased in the candidates undergoing protein A irnmunoadsorption treatment. Conclusion Higher expression of anti-MICA antibodies exists in sensitized recipients and possesses an influence on the recovery of renal function in early postoperative period. Protein A immunoadsorption can eliminate anti-MICA antibodies effectively in sensitized recipients.

Abstract:OBJECTIVE:To explore the changes of inflammation cytokines during acute renal transplantation rejection and decipher the functions of their protein-protein interaction network. METHODS:Serum samples were collected from renal transplantation patients with stable renal function or acute rejection (n = 6 each) to measure the expression level of 40 inflammatory factors by APIX protein array. The differentially expressed proteins were selected and their protein-protein interaction networks constructed. And biologic processes were analyzed by the online tools of String and Network Ontology Analysis. The analyses of protein-protein association network showed that these proteins were correlated and involved in such biological processes as taxis, chemotaxis, inflammatory reactions, wound responses and leukocytic migration. CONCLUSIONS:Comparing the inter-group differences of inflammatory cytokines and further developing and analyzing the protein-protein interaction network may help us to explore the mechanisms of acute renal transplantation rejection. And the differential cytokines can be used as candidate diagnostic biomarkers and intervention targets.
Expression of Regulatory T Cells and Related Cytokines in Mice Allogenic Corneal Transplantation Rejection

Abstract:OBJECTIVE:To study the changes of CD4(+)CD25(+) and CD8(+)CD103(+) regulatory T cells and their relevant cytokines in corneal allograft transplantation in mice.METHODS:BABL/c (H-2(d)) mice received corneal allografts from C3H/He (H-2(k)) mice were served as the experimental group. BABL/c (H-2(d)) mice received corneal from BABL/c (H-2(d)) mice were used as the control group. Corneal graft survival time was recorded pre-, 3rd day, 7th day, 4th week, 8th week post-operatively. Infiltration of inflammatory cells and corneal neovascularization were evaluated by histopathologic examination. The percentage of CD4(+)CD25(+) and CD8(+)CD103(+) T cell in peripheral blood and spleen was determined by flow cytometry. The expression of interleukin-10 (IL-10), IL-4, gamma-interferon (IFN-gamma) and IL-1beta in serum and aqueous humor was measured by enzyme-linked immunospecific assay (ELISA).RESULTS:The graft rejection in experimental group occurred at 7 days to 4 weeks after the operation, averaged (14.79 +/- 1.02) days. The grafts in the control group remained clear within 8 weeks observation and the survival time is much longer than that of the allografts (chi(2) = 46.934, P = 0.000). Flow cytometry showed that the percentage of CD4(+)CD25(+) T cell in peripheral blood in the control group after surgery was (3.36 +/- 0.29)%, (4.09 +/- 0.44)%, (5.44 +/- 0.35)%, (5.73 +/- 0.53)% at day 3, day 7, 4th week, and 8th week, respectively, which was significantly higher than that in the experimental group [(2.50 +/- 0.39)%, (3.24 +/- 0.25)%, (4.20 +/- 0.45)%, (4.18 +/- 0.14)%; t = 3.828, 2.898, 3.780, 4.892; P < 0.05]. On the other hand, CD8(+)CD103(+) T cell in peripheral blood in the experimental group after surgery was (2.20 +/- 0.33)%, (2.79 +/- 0.57)%, (4.55 +/- 1.03)%, (4.31 +/- 0.07)% at different periods, which was much higher than that in the control group (t = 7.133, 4.876, 5.196, 19.960; P < 0.05). The changes of CD4(+)CD25(+) and CD8(+)CD103(+) T cells in the spleen was earlier than those in peripheral blood. ELISA showed the expression of IL-10 and IL-4 in the serum in experimental group after surgery was much lower than that in the control group (t = 3.203, 3.141, 3.012, 2.869 and 2.340, 6.681, 8.839, 8.574; P = 0.011, 0.012, 0.013, 0.019 and 0.053, 0.000, 0.000, 0.000). The serum levels of IFN-gamma and IL-1beta in experimental group were higher than that in the control group (t = 3.508, 3.265, 4.402, 5.539 and 3.630, 5.796, 1.728, 0.660; P = 0.006, 0.011, 0.002, 0.000 and 0.005, 0.000, 0.115, 0.524). Furthermore, the cytokine levels in the aqueous humor behaved similarly with those in the serum.CONCLUSION:The down-regulation of CD4(+)CD25(+) Treg, IL-10 and IL-4 and the up-regulation of CD8(+)CD103(+) Treg, IFN-gamma and IL-1beta in mice allograft corneal rejection may play an important role in allograft rejection.

Abstract:Acute T cell mediated rejection of allografts remains a significant risk factor for early graft loss. Our prior work defined a population of graft-specific CD8+ T cells positive for the activated receptor CD43 (expressing the 1B11 epitope) that form during acute rejection, leading us to further understand the in vivo fate and clinical relevance of this population. We found that during acute rejection, the CD43+ICOS+ phenotype was sensitive for proliferative graft-specific CD8+ T cells. We evaluated whether CD43 1B11 signaling could impact graft survival, and found that CD43 1B11 mAb treatment could overcome costimulation-blockade induced tolerance in the majority of mice. Using an adoptive transfer approach, we investigated the fate of CD43 1B11+ and CD43 1B11- CD8+ T cell populations, and found that CD43 1B11+ CD8+ T cells were more persistent three weeks after transplantation. A portion of CD43 1B11- CD8+ T cells converted to CD43 1B11+, while CD43 1B11+ CD8+ T cells retained CD43 1B11+ status. In healthy human donors, we found that the CD43 1D4 clone, which identifies the large CD43 isoform, defines a population of antigen-experienced CD8+ T cells independent of the canonical CD8+ T cell populations. CD43 1D4+ CD8+ T cells were efficient cytokine-producers after stimulation. In scRNA-seq analysis of graft-infiltrating cells from renal transplant patients experiencing acute rejection, a population of SPN+GCNT1+ CD8+ T cells had an effector phenotype that includes high expression of IFNG, ICOS, and perforins/granzymes. Together, these data provide evidence that the CD43 1B11 expression defines a proliferative and persistent population of CD8+ T cells in mice, and that an analogous population of antigen-experienced CD8+ T cells that participate in allograft rejection.

Abstract:Objective: To observe the expression of iNOS in the transplanted renal tissue and the effect of Aminoguanidine(AG)/ Tacrolimus on acute allograft rejection following renal transplantation.Methods: Rat models of renal orthotopic transplantation were divided into an isograft group(n=5,SD to SD rats) and an allograft group,the latter including 3 subgroups(SD to Wistar rats): acute rejection(n=5),AG treatment(n=5) and Tacrolimus treatment(n=5).The grafts harvested on day 7 were subjected to histopathological examination,RT-PCR analysis of the iNOS mRNA expression and immunohistochemical measurement of the iNOS activity.The effects of AG /Tacrolimus on the acute allograft rejection following renal transplantation were studied.Results: iNOS expressed strongly in the acute allograft rejection group,with statistically significant differences from the AG,Tacrolimus and isograft groups(P<0.01).Conclusion: During acute allograft rejection in rat renal orthotopic transplantation,the expression of iNOS increased markedly and the increase was closely related to the severity of acute allograft rejection.AG and Tacrolimus could inhibit the expression of iNOS and attenuate acute allograft rejection in the transplanted renal tissue.

Abstract:Renal transplantation is the best treatment of some end-stage renal diseases. Unfortunately, not every transplant is successful due to the rejection or dysfunction of the transplanted kidney. Many cytokines participate in rejection by inducing inflammation or apoptosis. In this study, the expressions of TRAIL, DR4, and DR5 in rejected renal tissue and of serum soluble TRAIL (sTRAIL) in patients with kidney rejection were investigated by immunohistochemical staining and sandwich enzyme-linked immunosorbent assay, respectively. The results showed that the expression of TRAIL, DR4 and DR5, and serum sTRAIL levels were markedly upregulated among renal transplant patients. Since both membrane and soluble forms of TRAIL can induce apoptosis of DR4/DR5-expressing cells via recruiting FADD and caspase 8, elevated TRAIL and its receptors may participate in renal graft rejection.
Single-Cell Rna Sequencing Reveals The Immunological Profiles Of Renal Allograft Rejection In Mice

Abstract:Allograft rejection is a common immunological feature in renal transplantation and is associated with reduced graft survival. A mouse renal allograft rejection model was induced and single-cell RNA sequencing (scRNA-seq) data of CD45(+) leukocytes in kidney allografts on days 7 (D7) and 15 (D15) after operation was analyzed to reveal a full immunological profiling. We identified 20 immune cell types among 10,921 leukocytes. Macrophages and CD8(+) T cells constituted the main populations on both timepoints. In the process from acute rejection (AR) towards chronic rejection (CR), the proportion of proliferating and naive CD8(+) T cells dropped significantly. Both B cells and neutrophils decreased by about 3 folds. On the contrary, the proportion of macrophages and dendritic cells (DCs) increased significantly, especially by about a 4.5-fold increase in Ly6c(lo)Mrc1(+) macrophages and 2.6 folds increase in Ly6c(lo)Ear2(+) macrophages. Moreover, myeloid cells harbored the richest ligand and receptor (LR) pairs with other cells, particularly for chemokine ligands such as Cxcl9, Cxcl10, Cxcl16 and Yars. However, macrophages with weak response to interferon gamma (IFNg) contributed to rejection chronicization. To conclude, reduction in CD8 T cells, B cells and neutrophils while increasing in Ly6c(lo)Mrc1(+) macrophages and Ly6c(lo)Ear2(+) macrophages, may contribute significantly to the progress from AR towards CR.

Abstract:OBJECTIVE:To investigate the expression of non-T cell derived cytokines in acute rejection after heart transplantation.METHODS:The hearts of 26 C57BL/6 mice were transplanted into the abdominal cavities of 26 Balb/c mice (acute rejection group). The hearts of 26 Balb/c mice were transplanted into the abdominal cavities of another 26 Balb/c mice (syngeneic control group). After the operation, heart beat was felt with finger at the abdomen every day to determine the survival of the transplanted heart. One, three, five, and seven days after the operation the transplanted hearts were taken out from 5 mice in each group respectively to undergo microscopy. RT-PCR was used to detect the mRNA expression of these cytokines: interleukin (IL)-2, IL-15, tumor necrosis factor (TNF)-alpha, and interferon (IFN)-gamma. Western blotting was used to detect the protein expression of these cytokines. Six mice in each group, totally 12, were used to observe the survival time of transplanted heart.RESULTS:The mice of the control group recovered more rapidly in comparison with the nice of the acute rejection group. The survival times of the transplanted hearts of the control group were all longer than 100 days, significantly longer than those of the acute rejection group (7-9 days, P < 0.01). No sign of acute rejection was found in the hearts of the control group, and significant lymphocyte infiltration was seen in the myocardium of the acute rejection group 3-7 days after the operation. In the control group, no mRNA expression of IL-2 and IFN-was seen at any time-point, and IL-15 mRNA and TNF-alpha mRNA were expressed at low level at any time-point. However, in the acute rejection group the mRNA expression of IL-2, TNF-alpha, and IL-15 could be detected since the first day post-operatively and peaked on the day 5, but the mRNA expression of IFN-gamma could be detected and peaked only on the 5 th post-operative day. The mRNA expression levels of these 4 cytokines at any time point of the acute rejection group were all significantly higher than those of the control group (P < 0.01 or P < 0.05). The proteins of the 4 cytokines were all expressed at low level at any time point in the control group. However, in the acute rejection group the protein expression levels of IL-15 and IL-2 began to increase since the third day after operation and the protein expression levels of TNF-alpha and IFN-gamma began to increase since the first day after operation.CONCLUSION:Many cytokines, including T cell derived and non-T cell derived cytokines, are involved in the pathogenesis of acute rejection after allogeneic organ transplantation.

Abstract:In solid-organ transplantation, microRNAs (miRNAs) have emerged as key players in the regulation of allograft cells function in response to injury. To gain insight into the role of miRNAs in antibody-mediated rejection, a rejection phenotype histologically defined by microvascular inflammation, kidney allograft biopsies were subjected to miRNA but also messenger RNA (mRNA) profiling. Using a unique multistep selection process specific to the BIOMARGIN study (discovery cohort, N=86; selection cohort, N=99; validation cohort, N=298), six differentially expressed miRNAs were consistently identified: miR-139-5p (down) and miR-142-3p/150-5p/155-5p/222-3p/223-3p (up). Their expression level gradually correlated with microvascular inflammation intensity. The cell specificity of miRNAs target genes was investigated by integrating their in vivo mRNA targets with single-cell RNA sequencing from an independent allograft biopsy cohort. Endothelial-derived miR-139-5p expression correlated negatively with MHC-related genes expression. Conversely, epithelial-derived miR-222-3p overexpression was strongly associated with degraded renal electrolyte homeostasis and repressed immune-related pathways. In immune cells, miR-150-5p regulated NF-κB activation in T lymphocytes whereas miR-155-5p regulated mRNA splicing in antigen-presenting cells. Altogether, integrated omics enabled us to unravel new pathways involved in microvascular inflammation and suggests that metabolism modifications in tubular epithelial cells occur as a consequence of antibody-mediated rejection, beyond the nearby endothelial compartment.
