The Clinical Value of Breast Specific Gamma Imaging and Positron Imaging: An Update

Abstract:In the management of patients with breast cancer (BC), a mammography contributed to screen an early-stage patient, to plan a therapy strategy, to evaluate a therapy outcome, to detect a recurrence, and to reduce a mortality. Currently, various imaging modalities, such as CT, MR, Ultrasound (US), SPECT/CT, PET/CT, PET/MR have been utilized for the management of BC patients. In order to overcome a limited spatial resolution and sensitivity of whole-body systems in nuclear medicine imaging, dedicated breast imaging modalities were developed. One is a gamma imaging system with single/dual head scintillation detectors or semiconductor detectors associated with light compression device for breast parenchyma. Radiopharmaceutical for the gamma imaging is 99mTc-sestamibi. Another is a positron imaging system with opposite-type panel detectors and ring-shaped type detectors. Radiopharmaceutical for positron imaging is 18F-fluorodeoxyglucose. The breast-specific gamma and positron imaging systems were utilized mainly to detect small lesions less than 1 cm in diameter especially in patients with dense breast, to evaluate an effect of preoperative neo-adjuvant therapy, to plan surgical procedures (conservative-surgery vs mastectomy), and to detect a recurrence. By combining higher sensitivity and spatial resolution scanners with new radiopharmaceuticals, an information on molecular-level pathology of BC is increasingly available in an individual patient. This article reviewed clinical impact and future perspective of this field.

Abstract:Breast cancer is the most common malignancy in women worldwide. Recent developments in minimally invasive interventional radiology techniques have significantly improved breast cancer treatment. This study aimed to develop a novel technique for the local management of breast cancers using radiofrequency heat (RFH). We performed both in vitro experiments using human breast cancer cells and in vivo validation in xenograft animal models with magnetic resonance imaging (MRI) and pathological correlation to investigate the feasibility of our approach. Four treatment groups, including (1) no treatment (control), (2) RFH-only, (3) chemo (doxorubicin)-only, and (4) combination therapy with both doxorubicin and RFH, were conducted in each experiment. In vitro combination therapy significantly decreased breast cancer cell proliferation while increased their apoptosis index compared to the other three groups. MRI demonstrated a significant tumor size reduction in animals treated with combination therapy compared to those receiving other treatments in vivo. Such result was further confirmed by pathological examination. In conclusion, our findings suggests that RFH can enhance the therapeutic efficiency of doxorubicin on breast cancers, thus establishing the basis for future development of interventional molecular image-guided local chemotherapy for breast malignancies.

Abstract:Positron emission tomography / magnetic resonance imaging (PET/MRI) is an emerging imaging technology that allows for the acquisition of multiple MRI parameters simultaneously with PET data. In this review, we address the technical requirements of PET/MRI including protocols and tracers, the potential of integrated localized breast PET/MRI exams, and possible applications of whole-body PET/MRI in breast cancer patients. Currently, PET/MRI can be performed on sequential and integrated PET/MRI scanners but, as not all practices can access these dedicated machines, several studies look at PET and MRI exams that are performed separately on separate scanners within a short time frame. This practice likely provides similar clinical data, although exact colocalization for iso-voxel analysis, currently performed only in research, is not possible. In PET/MRI, the MRI sequences are flexible and can be customized according to the aim of the exam. The most commonly used radiotracer is 18 F-FDG; however, tracers that image hypoxia and drug targets such as estrogen receptors and HER2 are in development and may increase the utility of PET/MRI. For dedicated breast PET/MRI, a potential advantage over standard breast MRI alone may be the complementary sensitivities of MRI for extent of disease within the breast and PET for axillary and internal mammary nodal metastases. Moreover, layers of multiparametric MRI and PET metrics derived from the index lesion are being investigated as predictors of response to neoadjuvant therapy. These data may eventually be able to be quantified and mined in a way that furthers radiomics and also precision medicine. Finally, in whole-body imaging of breast cancer patients, single-institution studies have found that PET/MRI detects more metastases than PET at about half the radiation dose, although a survival benefit has not been shown. For now, whole-body PET/MRI in breast cancer patients may be most relevant for young patients who may undergo serial surveillance exams. Level of Evidence: 3 Technical Efficacy: Stage 2 J. Magn. Abstract:Abstract The presence of a communication network in a control loop induces imperfections, such as quantization effects, packet dropouts, time-varying transmission intervals, time-varying transmission delays and communication constraints. The objectives of this paper are to provide a unifying modeling framework that incorporates all these imperfections, and to present novel techniques for the stability analysis for these networked control systems (NCSs). We focus on linear plants, linear controllers and periodic protocols, which leads to a modeling framework for NCSs based on discrete-time switched linear uncertain systems. Using an overapproximated system in the form of a polytopic model with additive norm-bounded uncertainty, we use LMI-based techniques to analyze the input-to-state stability (ISS) of the obtained NCS models with respect to the norm-bounded additive disturbances on plant and controller signals induced by quantization. These ISS conditions will be used to assess closed-loop stability and performance for periodic communication protocols and uniform quantizers, although the framework allows for extensions towards other types of protocols and other types of quantizers as well. We illustrate the effectiveness of the developed theory on a benchmark example of a batch reactor.

There is an unmet need for a standardized patient-centric approach to use different imaging methods for response assessment.

Abstract:Breast cancer, with its global prevalence and impact on women's health, necessitates effective early detection and accurate staging for optimal patient outcomes. Traditional imaging modalities such as mammography, ultrasound, and dynamic contrast-enhanced magnetic resonance imaging (MRI) play crucial roles in local-regional assessment, while bone scintigraphy and 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) aid in evaluating distant metastasis. Despite the proven utility of 18F-FDG PET/CT in various cancers, its limitations in breast cancer, such as high false-negative rates for small and low-grade tumors, have driven exploration into novel targets for PET radiotracers, including estrogen receptor, human epidermal growth factor receptor-2, fibroblast activation protein, and hypoxia. The advent of PET/MRI, which combines metabolic PET information with high anatomical detail from MRI, has emerged as a promising tool for breast cancer diagnosis, staging, treatment response assessment, and restaging. Technical advancements including the integration of PET and MRI, considerations in patient preparation, and optimized imaging protocols contribute to the success of dedicated breast and whole-body PET/MRI. This comprehensive review offers the current technical aspects and clinical applications of PET/MRI for breast cancer. Additionally, novel targets in breast cancer for PET radiotracers beyond glucose metabolism are explored.

Abstract:Since thoracic MR imaging was first used in a clinical setting, it has been suggested that MR imaging has limited clinical utility for thoracic diseases, especially lung diseases, in comparison with x-ray CT and positron emission tomography (PET)/CT. However, in many countries and states and for specific indications, MR imaging has recently become practicable. In addition, recently developed pulmonary MR imaging with ultra-short TE (UTE) and zero TE (ZTE) has enhanced the utility of MR imaging for thoracic diseases in routine clinical practice. Furthermore, MR imaging has been introduced as being capable of assessing pulmonary function. It should be borne in mind, however, that these applications have so far been academically and clinically used only for healthy volunteers, but not for patients with various pulmonary diseases in Japan or other countries. In 2020, the Fleischner Society published a new report, which provides consensus expert opinions regarding appropriate clinical indications of pulmonary MR imaging for not only oncologic but also pulmonary diseases. This review article presents a brief history of MR imaging for thoracic diseases regarding its technical aspects and major clinical indications in Japan 1) in terms of what is currently available, 2) promising but requiring further validation or evaluation, and 3) developments warranting research investigations in preclinical or patient studies. State-of-the-art MR imaging can non-invasively visualize lung structural and functional abnormalities without ionizing radiation and thus provide an alternative to CT. MR imaging is considered as a tool for providing unique information. Moreover, prospective, randomized, and multi-center trials should be conducted to directly compare MR imaging with conventional methods to determine whether the former has equal or superior clinical relevance. The results of these trials together with continued improvements are expected to update or modify recommendations for the use of MRI in near future.

Common long bone fractures in small animal practice--part 1.

Abstract:333 T small animal practitioner can expect to see many long bone fractures over the course of a career. For those who are equipped to manage these surgically, the challenge is to select the appropriate fixation hardware and surgical technique. For many who may be prepared to tackle some intramedullary pin and wire applications or feel comfortable with external coaptation, the trick is to identify which cases will lend themselves to these techniques and which need to be referred. For those who refer most or all such cases, an understanding of what fractures are commonly seen, how they can be managed, and what prognosis goes along with them can be helpful in counseling clients. An analysis of patient records from 282 long bone fractures managed surgically in our practice from September 1983 to December 2002 yielded some valuable information on the location and types of fractures seen most often. The femur is, by far, the bone that is fractured most often in the dog and cat, comprising almost half of all long bone fractures in some surveys (1,2). Forty-five percent (128/282) of our long bone fractures were femoral fractures, 26% (74/282) were tibial fractures. It is interesting to speculate why almost 3 of every 4 long bone fractures in small animals occur in the hind limb. It may be that these represent only those patients that survived the traumatic episode, so as to be treated surgically. It is likely that trauma significant enough to fracture bones in the cranial half of the animal would more frequently cause lethal trauma to the head or chest. Similar trauma to the caudal half of the animal would be less likely to produce life-threatening injury. It may also be that animals see impending trauma coming (automobiles) and, in their effort to flee, they expose their hindquarters to the major force of the impact. Fractures of the radius, ulna, or both were considered together since, concurrent fractures of both bones are seen in the vast majority of cases. These fractures accounted for 16% (46/282) of our cases. The humerus is the long bone that is fractured least often by all accounts (2) and was involved in 13% (36/282) of our cases. Parts 1 and 2 of this topic review the “top 5” categories of long bone fractures recorded in our survey. They are presented in reverse order of frequency of occurrence; nearly 80% of the long bone fractures fell into 1 of the 5 categories.
Unilateral Subtotal Adrenalectomy for Pheochromocytoma in Multiple Endocrine Neoplasia Type 2 Patients: A Feasible Surgical Strategy

Abstract:Objective:To determine the best surgical strategy for pheochromocytoma in multiple endocrine neoplasia type 2 (MEN2) patients. Background:Pheochromocytomas occur in 50% to 60% of MEN2 patients, approximately half of them eventually develop bilateral disease. Unilateral subtotal adrenalectomy as primary surgery for pheochromocytoma in these patients may avoid or postpone the need for corticosteroid replacement therapy and the risk of Addisonian crisis, but is not yet widely accepted. Methods:We conducted a retrospective cohort study including 61 MEN2 patients with pheochromocytoma who were treated at the University Medical Center Utrecht between 1959 and 2010. Surgery was classified into 4 adrenalectomy groups: bilateral total, unilateral total, bilateral subtotal, and unilateral subtotal. Results:Primary surgery involved 22 bilateral total, 30 unilateral total, 2 bilateral subtotal, and 7 unilateral subtotal adrenalectomies. Twenty-one patients developed ipsilateral or contralateral recurrence after a median follow-up of 13.4 ± 10.8 years (range: 0.1–41.8). Unilateral total and unilateral subtotal adrenalectomy had similar rates of recurrence (P = 0.232) and survival time (5.5 versus 8.8 years; P = 0.170). Steroid replacement after bilateral total adrenalectomy led to complications in 8 patients. Reoperations for recurrence included unilateral total adrenalectomy in 12 patients, after which 10 needed steroid replacement (with complications in 3) and unilateral subtotal adrenalectomy in 5 patients, after which none needed replacement therapy. Ipsilateral recurrence after reoperation was similar between these groups. Conclusions:Unilateral subtotal adrenalectomy is a feasible surgical strategy for pheochromocytoma in MEN2 patients. It has comparable recurrence rates and eventually less complications of steroid replacement compared to unilateral total adrenalectomy.

Abstract:MRI is used widely both for screening women who are at increased risk of breast cancer and for treatment selection. Prospective studies confirm that MRI screening of women with known or suspected genetic mutation results in a higher sensitivity for cancer detection than does mammography. However, survival data are not available. In women with breast cancer, MRI detects cancer not identified with other types of screening. In two randomised trials, this increased sensitivity did not translate into improved selection of surgical treatment or a reduction in the number of operations. Data for longer-term outcomes such as ipsilateral breast tumour recurrence rates and contralateral breast cancer incidence are scarce, but to date do not show clear benefit for MRI. MRI is better than other methods of assessing the response to neoadjuvant chemotherapy, and is helpful in identifying the primary tumour in patients who present with axillary adenopathy.

Abstract:Breast cancer is a major disease with high morbidity and mortality in women worldwide. Increased use of imaging biomarkers has been shown to add more information with clinical utility in the detection and evaluation of breast cancer. To date, numerous studies related to PET-based imaging in breast cancer have been published. Here, we review available studies on the clinical utility of different PET-based molecular imaging methods in breast cancer diagnosis, staging, distant-metastasis detection, therapeutic and prognostic prediction, and evaluation of therapeutic responses. For primary breast cancer, PET/MRI performed similarly to MRI but better than PET/CT. PET/CT and PET/MRI both have higher sensitivity than MRI in the detection of axillary and extra-axillary nodal metastases. For distant metastases, PET/CT has better performance in the detection of lung metastasis, while PET/MRI performs better in the liver and bone. Additionally, PET/CT is superior in terms of monitoring local recurrence. The progress in novel radiotracers and PET radiomics presents opportunities to reclassify tumors by combining their fine anatomical features with molecular characteristics and develop a beneficial pathway from bench to bedside to predict the treatment response and prognosis of breast cancer. However, further investigation is still needed before application of these modalities in clinical practice. In conclusion, PET-based imaging is not suitable for early-stage breast cancer, but it adds value in identifying regional nodal disease and distant metastases as an adjuvant to standard diagnostic imaging. Recent advances in imaging techniques would further widen the comprehensive and convergent applications of PET approaches in the clinical management of breast cancer.

Abstract:Breast magnetic resonance imaging (MRI) is the most sensitive imaging modality for breast cancer diagnosis and is widely used clinically. Dynamic contrast-enhanced MRI is the basis for breast MRI, but ultrafast images, T2-weighted images, and diffusion-weighted images are also taken to improve the characteristics of the lesion. Such multiparametric MRI with numerous morphological and functional data poses new challenges to radiologists, and thus, new tools for reliable, reproducible, and high-volume quantitative assessments are warranted. In this context, radiomics, which is an emerging field of research involving the conversion of digital medical images into mineable data for clinical decision-making and outcome prediction, has been gaining ground in oncology. Recent development in artificial intelligence has promoted radiomics studies in various fields including breast cancer treatment and numerous studies have been conducted. However, radiomics has shown a translational gap in clinical practice, and many issues remain to be solved. In this review, we will outline the steps of radiomics workflow and investigate clinical application of radiomics focusing on breast MRI based on published literature, as well as current discussion about limitations and challenges in radiomics.

Abstract:Breast cancer is a significant health concern in females, worldwide. In vivo proton ( 1 H) MR spectroscopy (MRS) has evolved as a non-invasive tool for diagnosis and for biochemical characterization of breast cancer. Water-to-fat ratio, fat and water fractions and choline containing compounds (tCho) have been identified as diagnostic biomarkers of malignancy. Detection of tCho in normal breast tissue of volunteers and in lactating females limits the use of tCho as a diagnostic marker. Technological developments like high-field scanners, multi channel coils, pulse sequences with water and fat suppression facilitated easy detection of tCho. Also, quantification of tCho and its cut-off for objective assessment of malignancy have been reported. Meta-analysis of in vivo 1 H MRS studies have documented the pooled sensitivities and the specificities in the range of 71–74% and 78–88%, respectively. Inclusion of MRS has been shown to enhance the diagnostic specificity of MRI, however, detection of tCho in small sized lesions (≤1 cm) is challenging even at high magnetic fields. Potential of MRS in monitoring the effect of chemotherapy in breast cancer has also been reported. This review briefly presents the potential clinical role of in vivo 1 H MRS in the diagnosis of breast cancer, its current status and future developments.

Abstract:Breast cancer is a common disease that causes great health concerns to women worldwide. During the diagnosis and treatment of breast cancer, medical imaging plays an essential role, but its interpretation relies on radiologists or clinical doctors. Radiomics can extract high-throughput quantitative imaging features from images of various modalities via traditional machine learning or deep learning methods following a series of standard processes. Hopefully, radiomic models may aid various processes in clinical practice. In this review, we summarize the current utilization of radiomics for predicting clinicopathological indices and clinical outcomes. We also focus on radio-multi-omics studies that bridge the gap between phenotypic and microscopic scale information. Acknowledging the deficiencies that currently hinder the clinical adoption of radiomic models, we discuss the underlying causes of this situation and propose future directions for advancing radiomics in breast cancer research.

Abstract:Magnetic resonance (MR) imaging provides useful information for characterization of various ovarian masses as neoplastic or nonneoplastic and, when neoplastic, on a spectrum from benign to malignant. The use of MR imaging for diagnosis of ovarian masses includes consideration of morphologic characteristics and signal intensity characteristics on T1- and T2-weighted images. The morphologic characteristics of cystic masses, cystic and solid masses, and predominantly solid masses provide important information. In general, cystic masses represent benign tumors, whereas cystic and solid masses are strongly associated with malignancy. Predominantly solid masses include benign, borderline malignant, and malignant tumors. T1-weighted images provide useful information for characterization because hemorrhagic adnexal masses (eg, endometriotic cyst) and cystic teratomas can be correctly diagnosed when the mass has high signal intensity. Significant low signal intensity in solid masses on T2-weighted images is indicative of fibrothecomas and Brenner tumors because extensive fibrous tissue produces significant low signal intensity on T2-weighted images. A strategy for diagnosis of ovarian masses with MR imaging incorporates signal intensity characteristics into morphologic characteristics.

Abstract:Breast cancer's heterogeneity poses significant challenges in its diagnosis and treatment, necessitating advancements in non-invasive diagnostic methods. Radiomics, an emerging field that leverages mathematical analysis of medical imaging, offers the potential for designing personalized treatment strategies in breast cancer care. This systematic review aimed to synthesize current research on the application of radiomics in breast cancer diagnosis and treatment assessment, with a focus on Magnetic Resonance Imaging (MRI). Following the PRISMA-ScR protocol, a comprehensive literature search was conducted across major databases and grey literature. The review included peer-reviewed articles published in 2015 onwards, focusing exclusively on studies involving MRI imaging in radiomics. The studies were categorized into four themes including tumor subtype classification, prediction of pathologically complete response (PCR), detection of lymph node metastasis, and prediction of recurrence. The review included 48 articles, revealing significant insights into the use of radiomics for breast cancer analysis. These studies highlighted the superiority of MRI, particularly with 3T scanners and multiparametric protocols in radiomic analysis. The findings underscore the potential of combining radiomics with clinical data for enhanced prediction accuracy in tumor characterization, treatment response, and recurrence risk. The review also illuminated the importance of intratumor heterogeneity, as well as the challenges associated with image acquisition, segmentation, and feature extraction in radiomic studies. Radiomics in MRI shows considerable promise in advancing breast cancer diagnosis and treatment, aligning with the goals of personalized medicine.

Abstract:Neoadjuvant therapy is increasingly being used to treat early-stage triple-negative and human epidermal growth factor 2-overexpressing breast cancers, as well as locally advanced and inflammatory breast cancers. The rationales for neoadjuvant therapy are to shrink tumor size and potentially decrease the extent of surgery, to serve as an in vivo test of response to therapy, and to reveal prognostic information for the patient. MRI is the most accurate modality to demonstrate response to therapy and to help ensure accurate presurgical planning. Changes in lesion diameter, volume, and enhancement are used to predict complete response, partial response, or nonresponse to therapy. However, residual disease may be overestimated or underestimated at MRI. Fibrosis, necrotic tumors, and residual benign masses may be causes of overestimation of residual disease. Nonmass lesions, invasive lobular carcinoma, hormone receptor-positive tumors, nonconcentric shrinkage patterns, the use of antiangiogenic therapy, and late-enhancing foci may be causes of underestimation of residual disease. In patients with known axillary lymph node metastasis, neoadjuvant therapy may be followed by targeted axillary dissection to avoid the potential morbidity associated with an axillary lymph node dissection. Diffusion-weighted imaging, radiomics, machine learning, and deep learning methods are under investigation to improve MRI accuracy in predicting treatment response.©RSNA, 2021.
