Abstract:OBJECTIVE:To evaluate the early and middle-term results of in situ single screw fixation and subtrochanteric osteotomy of the femur with external fixator for slipped capital femoral epiphysis (SCFE). METHODS:From June 1998 to July 2008, 11 patients (seven male [eight hips] and four female [four hips]) of average age 14.3 years (range, 9-18 years) were treated in our hospital. According to the Southwick measurement, seven hips were mildly affected (<30°), two moderately (30-50°), and three severely (>50°). The average body mass index (BMI) was 31.1 g/m(2) (range, 27-35 g/m(2) ). All the mild and moderate cases underwent in situ single screw fixation, and the three severe cases subtrochanteric osteotomy of femur with external fixator. All cases were evaluated both clinically and radiographically at 3 months, 6 months and every year postoperatively. RESULTS:All cases were followed up for 2-12 years (average 5.6 years). The Harris hip score increased from a mean of 74.8 (70 ∼ 85) points preoperatively to 90.6 (70 ∼ 100) points postoperatively. X-ray films showed epiphyseal closure in the in situ single screw fixation cases, and no further increase in epiphyseal-shaft angle. Partial reduction of slipped epiphysis with normal joint spaces was observed after subtrochanteric osteotomy of the femur in the external fixator cases. CONCLUSION:In situ single screw fixation for treating mild and moderate SCFE has a satisfactory clinical outcome, and the advantage of prevention of further slippage. Subtrochanteric osteotomy of the femur with external fixator is suitable for severe SCFE with late deformity; the realignment procedure can correct deformity and postpone or prevent subsequent osteoarthritis.

Abstract:Biosynthetically directed fractional 13C labeling of proteinogenic amino acids is achieved by expression of proteins on a minimal medium which contains a mixture of [13C6]glucose and glucose with natural isotope abundance as the sole carbon source. Subsequent hydrolysis of the proteins yields the free amino acids. The observation of 13C-13C spin-spin scalar coupling fine structures in sensitive two-dimensional heteronuclear [13C,1H]-correlation spectroscopy (2D [13C,1H]-COSY) allows one to identify non-random 13C-labeling patterns arising from the incorporation of intact two-carbon and three-carbon fragments from a single source molecule of glucose into the amino acids. Since 2D [13C,1H]-COSY suffices to resolve all relevant resonances, the mixture of amino acids can be analyzed without further separation of its components. Probabilistic equations relate the observed multiplet intensities of the 13C fine structures to the relative abundance of the intact carbon fragments. They enable a quantitative analysis of the carbon flux in the network of biosynthetic pathways, thus using the proteinogenic amino acids as probes to study intermediary metabolism. This paper shows that biosynthetically directed fractional 13C labeling of amino acids provides an efficient analytical tool to quantitatively investigate glycolysis, pyruvate metabolism, pentose phosphate pathway, tricarboxylic acid cycle and C1 metabolism. Possible applications of the method include both the exploration of unknown biosynthetic pathways and the rapid elucidation of the response of a known biosynthetic reaction network to changes in growth conditions or genetic manipulations. In conjunction with the relatively low costs for isotopes, manpower and NMR instrument time, this makes biosynthetic fractional 13C labeling of proteinogenic amino acids particularly attractive to support process design and metabolic engineering in biotechnology, since screening procedures become feasible which enable a systematic characterization of the cell's metabolic state as a function of parameters that are involved in the optimization of biotechnological processes.

Abstract:This study evaluates the Patient Acceptable Symptom State (PASS) in patients with slipped capital femoral epiphysis (SCFE) treated with in situ fixation, focusing on medium to long-term outcomes and quality of life. Its primary goal is to establish a subjective well-being cutoff, using subjective methods and the iHOT33 scale, for assessing patients in future studies. Additionally, it explores functionality differences between mild and moderate-severe SCFE, case series epidemiology and potential complications. A retrospective analysis of 63 patients (73 hips), treated for SCFE between 2000 and 2017 at our facility using in situ fixation, was conducted. These patients underwent clinical, anamnestic, and radiological assessments, with PASS determined based on iHOT33 questionnaire results and statistical analysis. The mean age at surgery was 12.95 years (±1.64, range 9–17), with an average follow-up of 11 years (±4.60, range 5–20). At follow-up, 87% of patients reported achieving PASS, with higher iHOT33 scores correlating to PASS. A cutoff of >68 on the iHOT33 scale showed strong predictive ability for assessing PASS (area under the curve 0.857, 88.89% sensitivity, 79.69% specificity). The findings indicate that 87% of patients achieved PASS at medium to long-term follow-up, with better clinical function than those who did not report PASS. The iHOT33 scale’s effectiveness in predicting PASS, especially with a cutoff of >68, suggests this method’s efficacy. Given these positive outcomes, including in moderate-severe cases treated with in situ fixation, this approach is considered a viable therapeutic option.

Controversies and disparate trends derive from differences in clinical presentation, various classifications, and a variety of surgical procedures that have been described. Currently, there are no evidence-based recommendations. Surgical procedures vary, and they can be divided into fixation in situ, compensatory osteotomies, and direct corrections of the deformity at the head–neck junction. The first and second group of procedures have so far not gained optimal control over the risk of avascular necrosis or cannot achieve an anatomically aligned epiphysis with normal blood supply. On the other hand, the third technique can achieve this target and prevent residual deformity and the development of early hip arthritis, but it is not widely accepted, because of its surgical complexity. The purpose of this work is to present an overview of current knowledge and provide an orientation on clinical and surgical management of the patient suffering from SCFE. Limited angular remodelling after in-situ fixation for slipped capital femoral epiphysis

Abstract:Abstract Background In Sweden, most children with slipped capital femoral epiphysis (SCFE) are operated on with a single smooth pin or a short-threaded screw, allowing further growth of the femoral neck. Using the Swedish Pediatric Orthopaedic Quality registry, SPOQ, we investigated whether angular remodelling occurs adjacent to the proximal femoral epiphysis after fixation of SCFE using implants, allowing continued growth of the femoral neck. Methods During 2008–2010 a total national population of 155 children were reported to the SPOQ registry. Following our strict inclusion criteria, radiographs of 51 hips were further assessed. The lateral Head Shaft Angle (HSA), the Nötzli 3-point α-angle, the anatomic α-angle, and the Anterior Offset Ratio (AOR) on the first postoperative radiographs and at follow-up were measured to describe the occurrence of remodelling. Slip severity was categorised as mild, moderate or severe according to postoperative HSA. Results Mean and SD values for the change in HSA were 3,7° (5,0°), for 3-point α-angle 6,8° (8,9°), and anatomic α-angle 13,0° (16,3°). The overall increase in AOR was 0,038 (0.069). There were no significant differences between the slip severity groups. Conclusions We found limited angular remodelling after in situ fixation with smooth pins or short threaded screws for SCFE. The angular remodelling and the reduction of the CAM deformity was less than previously described after fixation of SCFE with similar implants. Results about the same magnitude with non-growth sparing techniques suggest that factors other than longitudinal growth of the femoral neck are important for angular remodelling.

Favourable outcome of severe, unstable grade III slipped capital femoral epiphysis managed by closed reduction percutaneous pinning with mid-term follow up: A case report and literature review

Abstract:Introduction and importance: Slipped capital femoral epiphysis (SCFE) is one of the most common hip pathology in adolescents. Outcome of SCFE management largely depends on the grading of the pathology. Severe, unstable SCFE poses high risk for avascular necrosis (AVN). The objective of this study is to report a good outcome without AVN in an acute, unstable, high grade SCFE managed by closed reduction and percutaneous pinning along with factors that need to be given consideration. Case presentation: A 13 years old boy was unable to bear weight due to severe pain on right hip after trauma since 2 weeks. Patient was diagnosed with acute, unstable high grade SCFE, patient underwent closed reduction and percutaneous pinning (CRPP) using cannulated screw and K wire augmented with spica cast. At 12 weeks patient was already fully active. At 18 month follow up there was no sign of AVN with full hip range of movement. Clinical discussion: Treatment for unstable, severe SCFE is still challenging. Unreduced severe slippage will deliver serious impingement and end up with early degenerative arthritis. The magnitude of reduction in a severe, unstable SCFE poses high risk for AVN. While some studies claimed reduction is justified only when it is serendipitous, we successfully and purposely performed CRPP. Conclusion: Even after 2 weeks from onset, an acute and severe slippage can still be reduced closely and fixed percutaneously. The hip regain full range of motion with no sign of AVN on x ray at 18 months follow up.

Abstract:Introduction: The purpose of this study was to describe proximal femoral deformity after contralateral hip prophylactic fixation of slipped capital femoral epiphysis (SCFE) in patients and the association of relative skeletal immaturity with this deformity. Methods: A retrospective review of patients presenting with a SCFE was conducted from 2009 to 2015. Inclusion criteria were (1) radiographic evidence of a unilateral SCFE treated with in situ fixation, (2) contralateral prophylactic fixation of an unslipped hip, and (3) at least 3 years of follow-up. Measurements were made on radiographs and included greater trochanter height relative to the center of the femoral head, femoral head-neck offset, and femoral neck length. Skeletal maturity was evaluated by assessing the status of the proximal femoral physis and triradiate cartilage (TRC) of the hip, in addition to the length of time to closure of these physes. Values were compared from initial presentation to final follow-up. Statistical analysis included descriptive statistics and linear regression. Results: Twenty-seven patients were included. Bivariable linear regression demonstrated that an increased relative trochanteric overgrowth was associated with TRC width (β = 3.048, R = 0.585, P = 0.001) and an open TRC (β = −11.400, R = 0.227, P = 0.012). Time to proximal femoral physis closure (β = 1.963, R = 0.444, P = 0.020) and TRC closure (β = 1.983, R = 0.486, P = 0.010) were predictive of increased deformity. In addition, multivariable elimination linear regression demonstrated that TRC width (β = 3.048, R = 0.585, P = 0.001) was predictive of an increased relative trochanteric overgrowth. Discussion: Patients with an open TRC and increased TRC width are associated with increased relative trochanteric overgrowth when undergoing prophylactic fixation for a unilateral SCFE. Increased caution should be exercised when considering contralateral hip prophylactic fixation in skeletally immature patients presenting with a unilateral SCFE. Abstract:Background: Fractures of the distal tibial epiphysis in children are often accompanied by articular surface disruption. With increased displacement, internal fixation is frequently performed with an epiphyseal screw to close the fracture gap. Despite limited, high-level clinical evidence to support implant removal, epiphyseal screws are commonly removed after fracture healing due to potentially increased contact forces on the tibiotalar joint. The purpose of this study was to investigate and compare outcomes and complications in children that underwent surgical treatment of distal tibial epiphyseal fractures with placement of an epiphyseal screw(s) and had the implant(s) retained versus removed at a minimum of 2-year postoperative follow-up. Methods: Children younger than 18 years from two urban tertiary care centers who underwent operative management of distal tibia Salter-Harris III and IV fractures using epiphyseal screws (2013-2020) were divided into two cohorts: retained epiphyseal screws and implant removed. Demographics, intraoperative, postoperative, and radiographic data were collected. Patient-reported outcomes (PROs) using the Foot and Ankle Ability Measure (FAAM) and Single Assessment Numeric Evaluation (SANE) questionnaires were collected at the final follow-up. Statistical analysis, including power analysis, was performed. Results: Fifty-two children were included (30 males, 22 females) with a mean age of 13.3 years at the time of injury (range, 7.7-16.4 years). Thirty-five children retained the implants; seventeen had implants removed. All completed the FAAM questionnaires at a mean follow-up of 4.4 ± 1.9 years, while 29 completed the SANE questionnaire at a mean follow-up of 4.4 ± 1.7 years. No statistically significant difference in patient demographics, surgical variables, or PROs was observed. Six children experienced complications from the initial surgery, including infections and complex regional pain syndrome, with no difference in complication rates between the cohorts ( P =0.08). Furthermore, no complication was observed as a result of implant removal. Conclusions: Children with retained epiphyseal implants have similar functional outcomes as compared with those who had implants removed after distal tibial epiphyseal fracture fixation and union. Valgus Slipped Capital Femoral Epiphysis: Pathophysiology of Motion and Results of Intracapsular Realignment

Abstract:Objectives: The purpose of this study was to report (1) a different but specific pattern of impingement in hips involved with valgus slipped capital femoral epiphysis (valgus SCFE) and (2) the results of surgical treatment using intracapsular realignment techniques. Design: Case series. Setting: Multiple academic centers. Patients: Six patients with 8 involved hips referred for valgus alignment of proximal femoral epiphysis (valgus SCFE). Intervention: Intracapsular realignment osteotomy combined with periacetabular osteotomy if needed. Main outcome measurement: The clinical and radiographical results and pathophysiology of motion. Results: Eight hips in 6 patients were treated with subcapital (5 hips) or femoral neck (3 hips) osteotomy for realignment. The medially prominent metaphysis created an inclusive impingement at the anterior acetabular wall, whereas the high coxa valga favored impacting impingement at the posterior head-neck junction. The mean preoperative epiphyseal-shaft angle of 110.5 (range 90-125 degrees) was reduced to 62 degrees (range 55-70 degrees) postoperatively. At the last follow-up, all but 1 hip were pain-free and impingement-free, with normal range of motion. One hip was replaced after repeated attempts of correction. The overall hip functional result using modified Merle d'Aubigne scoring system was excellent in 5 hips (18-16 points), good in 2 hips (16-15 points), and poor in 1 hip (6 points). Conclusions: Impingement in valgus SCFE deformity is specific and complex. Anatomical realignment can lead to favorable results by the restoration of normal morphology and impingement-free range of motion. Level of evidence: Therapeutic Level IV. [Mid-Term Follow-Up Study of Hip Arthroplasty for Internal Fixation Failure of Femoral Intertrochanteric Fractures in the Elderly].

Abstract:OBJECTIVE:To retrospective analysis the mid-term follow-up effect of hip joint replacement in elderly patients with failure of intertrochanteric fractures of the hip joint internal fixation.METHODS:From December 2008 to December 2011, 32 elderly patients underwent arthroplasty after intertrochanteric fracture fixation failure, of which, 4 death cases were excluded from the study, and the remaining 28 cases were in the study group. The age of patients ranged from 69 to 83 years old with a mean of 75 years old. The time from the internal fixation to the hip replacement were 8 to 72 months. Among them, 6 patients were Evans I type, 11 patients were Evans II type, 9 patients were Evans III type, and 2 patients were Evans IV type. Nine cases showed fracture of the lateral plate before operation, while 15 cases were femoral head screw cut-out and 4 cases were screw loosening. Harris score was used to compare the changes of hip function before operation with the final follow-up. Imaging results(X-ray) and erythrocyte sedimentation rate(ESR) were performed during the follow-up.RESULTS:All patients were followed up from 4 to 7 years with an average of 5.3 years. Pain was significantly reduced or disappeared in patients compared with pre-operation. And hip function was significantly improved. Two cases had moderate pain after the physical activity and 4 cases had mild pain after the physical activity. At the final follow-up, 19 patients resumed free walking, 8 patients required walking with walking sticks, and 1 patient needed walking aid. The Harris scores improved from preoperative 34.9±2.4 to 83.4±5.7 at the final follow-up, among them, 15 cases were classified as excellent, 10 as good, 2 as fair, and 1 as poor. X-ray examination showed no prosthesis loosening and sinking fracture.CONCLUSIONS:Salvage THA surgery could improve the hip function and the quality of life for old patients with intertrochanteric fracture fixation failure, and the middle-term follow-up results support that.
Association between NPHS1 and NPHS2 gene variants and nephrotic syndrome in children.

Abstract:INTRODUCTION Nephrin and podocin proteins, encoded by NPHS1 and NPHS2 genes, are essential for the integrity of the glomerular filter. The present study was aimed to investigate whether NPHS1 rs437168 and NPHS2 rs61747728 genetic variants are involved in the susceptibility to nephrotic syndrome (NS). MATERIALS AND METHODS This case-control study was performed on 108 children with NS and 97 healthy children. Genomic DNA was extracted from whole blood using the salting-out method. Polymorphism of the NPHS1 rs437168 and NPHS2 rs61747728 were detected by amplification refractory mutation system- and tetra primers amplification refractory mutation system-polymerase chain reaction, respectively. RESULTS The results showed that the NPHS1 rs437168 GA as well as GA+AA genotypes increased the risk of NS in comparison with GG genotype (odds ratio, 4.76, 95% confidence interval, 2.31 to 9.80; P < .001 and odds ratio, 4.57; 95% confidence interval, 2.31 to 9.04, ; P < .001, respectively). The A allele was associated with increased risk of NS (odds ratio, 3.53; 95% confidence interval, 1.94 to 6.42, ; P < .001) in comparison to the G allele. No association was observed between NPHS2 rs61747728 polymorphism and NS. CONCLUSIONS Our findings indicate that NPHS1 rs437168, but not NPHS2 rs61747728 variant, is associated with NS.
Evaluation and discussion of material strength of flat bar in TETRIX for design and manufacture of demonstration equipment for automatic control theory

Abstract:In this paper, we focus on the strength of a flat bar made of the aluminum alloy in expansion kit TETRIX of LEGO MindStorms, in order to design and manufacture a demonstration equipment for an automatic control theory. The tensile and bending tests were carried out to investigate the mechanical properties of the flat bar. And, the strength design of the demonstration equipment was discussed when the flat bar is subjected to compression in long axis direction, and to combined load of bending and torsion. 0 2% proof stress and Young's modulus of flat bar obtained by tensile test were 174[MPa] and 76[GPa], respectively. Two 0.2% proof stresses obtained by bending tests were depend on bending direction (203[MPa] and 340[MPa]). The critical buckling load and the yield criterion of the flat bar subject to the combined load of bending and torsion were discussed based on the actual mechanical properties.

Abstract:Background: Slipped capital femoral epiphysis (SCFE) is a common hip problem in children. The resulting deformity can cause impingement similar to cam-type idiopathic femoroacetabular impingement (FAI). Although there are similarities between FAI and SCFE, deformity patterns, severity, and time of onset of symptoms varies, which may impact management. The purpose of this study was to describe patterns of articular cartilage damage in patients undergoing surgical hip dislocation for sequelae of SCFE in comparison to patients undergoing arthroscopic surgery for primary FAI. Methods: Patients were identified who underwent surgical treatment for hip pain due to primary FAI (cam type) or sequelae of SCFE. Clinical data and radiographic measurements were recorded. Cartilage was assessed intraoperatively. Severity was classified using the modified Beck classification, while location was classified into 6 sectors. Statistical analysis was performed to test for differences in demographic and radiographic characteristics between the SCFE and FAI patients. χ2 or Fisher exact tests were used to evaluate trends in patterns of acetabular and femoral cartilage wear between SCFE and FAI groups. Results: The SCFE group had 28 hips compared with 304 in the FAI group. SCFE patients were younger (19 vs. 32, P<0.001), had higher body mass index (30±5.9 vs. 24±4.8, P<0.001), and were more often male (61% vs. 27%, P<0.001). Deformity severity based on α-angle was higher in the SCFE group [AP 74 vs. 55 (P=0.001) and Dunn 72 vs. 58 (P<0.001)]. There were no significant differences with regards to lateral center edge angle, anterior center edge angle, or Tonnis angle. In both groups the most common locations for cartilage lesions in both groups were the anterior peripheral and superolateral peripheral regions with fewer but more widely distributed femoral head lesions. The SCFE group had higher rates of femoral head and superolateral central cartilage lesions compared with the FAI group. There was no statistical difference between high-grade femoral or acetabular cartilage lesions between groups. Conclusions: Patients with SCFE were younger at the time of surgery and presented with more severe deformity based on radiographic α-angle compared to patients with FAI. Our results suggest higher prevalence of femoral head lesions and more diffuse cartilage injury in patients with SCFE. This study can be used to support early surgical intervention in patients with symptomatic sequelae of SCFE due to risk of premature joint damage. Level of evidence: Level III-prognostic study.
Gait Outcomes of Patients With Severe Slipped Capital Femoral Epiphysis After Treatment by Flexion-Rotation Osteotomy

Abstract:BACKGROUND: Flexion-rotational osteotomy of the proximal femur is an accepted intervention in the management of severe deformity and femoral acetabular impingement secondary to slipped capital femoral epiphysis (SCFE). The impact of this surgical intervention on gait kinematics and kinetics, validated functional questionnaires, and patient outcomes has not been well studied. The purpose of this study was to analyze the changes in standard gait parameters of patients with moderate to severe SCFE who were treated with a flexion-rotational osteotomy.METHODS: This study is a retrospective review of 8 patients treated for a unilateral moderate and severe SCFE with a flexion-rotational osteotomy. All patients had 3-D computerized gait analysis studies completed preoperatively and 1-year postoperatively. Additional data analyzed preoperatively and postoperatively included: anterior/posterior hip radiographs, standard physical examination measures, and Pediatric Outcomes Data Collection Instrument (PODCI), completed by parents.RESULTS: The Gait Deviation Index, a composite of gait kinematics, showed a significant improvement from 64.9 to 88.0 (P<0.001). Radiographically, significant improvement toward normal values were found in the epiphyseal-shaft angle on the AP view from 123 to 139 degrees (P=0.005) and on the frog lateral view from 61 to 16 degrees (P=0.00001). Hip abduction range of motion on physical examination increased from 15 to 27 degrees and hip external rotation decreased from 51 to 25 degrees after surgery (P<0.05). The PODCI significantly improved in the categories of basic mobility, sports function, and global function (P<0.05).CONCLUSIONS: Longstanding deformity as a result of a severe SCFE may lead to osteoarthritis of the hip, disabling pain, and functional deficits. Although radiographic evidence of degenerative disease may take years to develop, changes in gait parameters can be immediately evident in this population. A flexion-rotation osteotomy in the adolescent and young adult population can improve gait kinematics, radiographic measures, range of motion, and short-term functional outcome scores. It is felt that normalization of these parameters may reduce the risk of long-term hip deterioration and its related sequelae.LEVEL OF EVIDENCE: Level IV.

Abstract:This conceptual paper introduces the important link between law and knowledge resources. We analyse the three key types of institutions: common law, civil law, and social norms, conventions and how they define the nature of property rights for knowledge resources and assets. Knowledge, because of its intangible nature and value, raises fundamental issues of, measurement and property rights that do not arise for more traditional, tangible assets. We provide a conceptual analysis on how to measure and assess the value of property rights for knowledge assets and knowledge networks. A key conclusion from our analysis is that informal institutions such as social norms and conventions are relatively superior to legal institutions such as common law and civil law in the analysis of property rights of knowledge assets and knowledge networks.
Clinical and Radiographic Results Following Treatment of Traumatic Syndesmotic Instability Utilizing a Novel Screw-Suture Syndesmotic Fixation Device

Abstract:Objectives: The objective of this study was to report early outcomes of a novel screw-suture syndesmotic device compared to suture button fixation devices when treating traumatic syndesmotic instability. Methods : Design : Retrospective chart review. Setting: Single academic Level-1 Trauma Center Patient Selection Criteria: All adult patients who had syndesmotic fixation with the novel device (NSRI group) compared to a suture button device (SB group) between January 2018 and December 2022. Outcome Measures and Comparisons : Medial clear space (MCS) and tibiofibular overlap (TFO) measurements were compared immediately post-operatively and at final follow-up. Patients were followed for a minimum of 1-year or skeletal healing. Results: Fifty-nine patients (25 female) with an average age of 47 years (range 19-78 years) were in the NSRI group compared to 52 patients (20 female) with an average age of 41 years (range 18-73 years) in the SB group. There were no significant differences when comparing Body Mass Index, diabetes, or smoking status between groups (p>0.05). There was no difference when comparing the post-operative and final MCS measurements in the NSRI group compared to the SB group (p=0.86; 95% CI [-0.32, 0.27). There was no difference when comparing the post-operative and final TFO measurements in the NSRI group compared to the SB group (p=0.79; 95% CI [-0.072, 0.09). There were 3 cases of implant removal in the NSRI group compared to 2 in the SB group (p=0.77). There was one failure in the NSRI group and none in the SB group. The remaining patients were all fully ambulatory at final follow-up (p=0.35). Conclusions: A novel screw-suture syndesmotic implant provides the fixation of a screw and the flexibility of a suture had similar radiographic outcomes compared to suture button fixation devices in treating ankle syndesmotic instability. Abstract:Purpose Treatment of moderate to severe stable slipped capital femoral epiphysis (SCFE) remains a challenging problem. Open reduction by modified Dunn procedure carries a considerable risk of osteonecrosis (ON). Imhauser osteotomy is capable of realigning the deformity without the risk of ON, but the remaining metaphyseal bump is implicated with significant chondro-labral lesions and accelerated osteoarthritis. We conducted this study to evaluate the efficacy and safety of Imhauser osteotomy combined with osteochondroplasty (OCP) through the surgical hip dislocation (SHD) approach. Methods A prospective series of 23 patients with moderate-severe stable SCFE underwent Imhauser osteotomy and OCP through SHD. The mean age was 14.4 years (13 to 20) and the mean follow-up period was 45 months (24 to 66). The outcome measures included clinical and radiological parameters and Harris hip score (HHS) was used as a functional score. Results The mean HHS improved significantly from 65.39 to 93.3. The limb length discrepancy improved by a mean of 1.72 cm. The mean flexion and abduction arcs showed a significant improvement (mean increase of 37.5° and 18.5°, respectively). The mean internal rotation demonstrated the most significant improvement (mean increase of 38.5°). All the radiographic parameters improved significantly; including anterior and lateral slip angles (mean improvement 37.52° and 44.37°, respectively). The mean alpha angle decreased by 39.19°. The articulo-trochanteric distance significantly increased to a mean of 23.26 mm. No cases of ON or chondrolysis were identified. Conclusion Combined Imhauser osteotomy and OCP through the surgical dislocation approach provide a comprehensive and safe management of moderate to severe stable SCFE. Level of evidence IV

Abstract:Despite advances in the treatment of coronary artery disease, the search for ways to preserve myocardium threatened by ischaemic or reperfusion injury continues. A group ofproteins known as the heat shock or stress proteins represent the cell's innate ability to protect itself. Do these proteins have a role in protecting the heart in coronary artery disease and can this self preservation mechanism be exploited therapeutically? Heat shock proteins were originally described in the 1960s' and over the next two decades the concept of thermotolerance developed. It was discovered that cells and organisms exposed to sublethal hyperthermia synthesised stress proteins2 and were rendered tolerant or resistant to subsequent hyperthermic stress. This group ofinducible stress proteins is phylogenetically well preserved, being present in various organisms from bacteria to humans.' It is unlikely that their expression in mammals is merely an evolutionary remnant of little relevance to homoeothermic animals, since their induction is not confined to the stimulus of heat. Other stimuli include various metabolic stressors such as cellular hypoglycaemia, reactive oxygen species (hydrogen peroxide and superoxide), and ischaemia4 5-all factors that suppress the synthesis of other cellular proteins. Animal studies have shown that after preconditioning with a brief period of ischaemia the heart is temporarily protected from further ischaemic episodes,6 and recent evidence suggests that this phenomenon also exists in humans.7 In addition, a constitutively (basally) expressed stress protein of the 70 kDa heat shock protein (HSP70) family is selectively inducible in rabbit heart by ischaemia.5 It is well recognised that heat shock and the subsequent synthesis of stress proteins is protective;8 whether stress proteins contribute to myocardial preconditioning remains controversial. Attention to date has focused on the 70 kDa stress protein, which has been well reviewed.89 A role for stress proteins in myocardial preconditioning was supported by work that showed that rendering animals hyperthermic (thus inducing the heat shock protein response) before an episode of cardiac ischaemia had a protective effect both during the ischaemic period and on restoration of perfusion.'01' However, a few minutes of myocardial ischaemia has been shown to exert a protective effect, preconditioning the heart against subsequent ischaemic episodes.67 Such a time course suggests that this preconditioning effect is unlikely to be due to stress protein induction: increased expression of HSP70 mRNA is not seen in rabbits until an hour after a short ischaemic period, and an increase in the stress protein itself is only detected after two hours, peaking at 24 hours.5 In addition, it has been shown that protein synthesis is not a prerequisite for preconditioning."2 A further group of stress proteins may contribute to the myocyte's defences. The glucose-regulated stress proteins are induced by cellular hypoglycaemia, a state that occurs during ischaemia when the synthesis of other proteins is suppressed." Though little is known of their functions, they may modulate the response ofthe myocyte to hypoxia, perhaps by regulating the folding and assembly of proteins disrupted by hypoxia.'4 Stress proteins also seem to have a role in myocardial adaptation (by hypertrophy), to haemodynamic overload.'5 Cardiomyocytes adapt rapidly to pressure overload with increased protein synthesis. Though the bulk of such proteins comprise contractile units, increased expression of the HSP70 gene is detected as early as an hour after the onset of pressure overload, followed by the appearance of the HSP70 protein. This induction is a short-lived phenomenon; concentrations of HSP70 mRNA return to baseline after 24 hours despite continuous pressure overload. Although the transience of their induction suggests that stress proteins are not the sole cause of compensatory hypertrophy, their perinuclear distribution implies that they may have a permissive role in mediating overloadinduced hypertrophy. By the mechanisms discussed below they may permit the rapid recovery of protein synthesis interrupted by the onset of overload, thus enabling the cell to survive the initial stress and to adapt by hypertrophy to continued overload. What are the mechanisms by which stress proteins provide protection? It has been hypothesised that they recognise and repair hypoxia-induced damage to cellular proteins; 14 16 rescue protein synthesis and mRNA splicing halted by the onset of stress;'7 18 and are involved in protein folding and unfolding, assembly and disassembly, and translocation.'9 A further possibility is that stress proteins exert their effect by affording protection against the oxidative stress of reactive oxygen species liberated during post-ischaemic reperfusion: heat shock is accompanied by the induction of the antioxidant catalase within the myocardium.'0 Inhibition of catalase induction has been shown to lessen the protective effect of heat shock.2 Do stress proteins protect the heart by other antioxidant mechanisms? A low molecular weight stress protein (SP32) is present in the heart2' and induced by the reactive oxygen species, hydrogen peroxide.22 Reactive oxygen species are released early during post-hypoxic reperfusion and have been implicated in myocardial reperfusion injury. SP32 has been identified as haem oxygenase-1,' the rate limiting enzyme in the degradation ofhaem to bilirubin. Bilirubin is a potent scavenger of reactive oxygen species, its efficacy increasing with the decrease in pH'8 associated with ischaemia. Haem oxygenase-l (SP32) is active only in the presence of oxygen. It is therefore possible that during reperfusion (for example after thrombolysis for myocardial infarction) the induction ofhaem oxygenase-1 mediated by reactive oxygen species has a protective antioxidant effect.
