Abstract:Abstract Background Missed nursing care is a global challenge that can have many consequences. Knowing the experiences of clinical nurses can be helpful. Therefore, this study was conducted to explain the experiences of Iranian nurses regarding the consequences of missed nursing care. Methods This qualitative descriptive study was conducted with a content analysis approach. Sampling was done by the purposeful sampling method and continued until data saturation. Data were collected by in-depth semi-structured interviews. Data were analyzed using qualitative content analysis and Graneheim and Lundman’s method. MAXQDA version 10 software was used for data management. Results The participants included 14 nurses with a mean age of 38.7 ± 7.7 years. The data were classified into three categories: patient-related outcomes, nurse-related outcomes, and organization-related outcomes. These categories included nine subcategories entitled "moral distress", "job dissatisfaction", " decreased quality of nursing care "," patient dissatisfaction ","adverse events"," absenteeism ","intention to leave and subsequent turnover", "decreased hospital credit", and "increased hospital costs". Conclusion Missed nursing care can have adverse consequences for the patients, nurses, and organizations. Therefore, it is necessary to adopt management strategies such as providing sufficient manpower and increasing nurses' job satisfaction to reduce the amount of missed nursing care. Further studies are needed to explain the predictors of the missed nursing care consequences.

Unveiling missed nursing care: a comprehensive examination of neglected responsibilities and practice environment challenges

Abstract:Background: The global variable of missed nursing care and practice environment are widely recognized as two crucial contextual factors that significantly impact the quality of nursing care. This study assessed the current status of missed nursing care and the characteristics of the nursing practice environment in Iran. Additionally, this study aimed to explore the relationship between these two variables. Methods: We conducted an across-sectional study from May 2021 to January 2022 in which we investigated 255 nurses. We utilized the Missed Nursing Care Survey, the Nursing Work Index-Practice Environment Scale, and a demographic questionnaire to gather the necessary information. We used the Shapiro‒Wilk test, Pearson correlation coefficient test, and multiple linear regression test in SPSS version 20 for the data analyses. Results: According to the present study, 41% of nurses regularly or often overlooked certain aspects of care, resulting in an average score of 32.34 ± 7.43 for missed nursing care. It is worth noting that attending patient care conferences, providing patient bathing and skin care, and assisting with toileting needs were all significant factors contributing to the score. The overall practice environment was unfavorable, with a mean score of 2.25 ± 0.51. Interestingly, 'nursing foundations for quality of care' was identified as the sole predictor of missed nursing care, with a β value of -0.22 and a p-value of 0.036. Conclusions: This study identified attending patient care interdisciplinary team meetings and delivering basic care promptly as the most prevalent instances of missed nursing care. Unfortunately, the surveyed hospitals exhibited an undesirable practice environment, which correlated with a higher incidence of missed nursing care. These findings highlight the crucial impact of nurses' practice environment on care delivery. Addressing the challenges in the practice environment is essential for reducing instances of missed care, improving patient outcomes, and enhancing overall healthcare quality.

Abstract:Background: Care is one of the essential metaparadigms of nursing. The personal and professional character of the nurse, along with attention to human, financial, and communication factors, can provide an essential part of standard care. Moral sensitivity, as a part of professional ethics, offers responsible and accurate care. Objectives: This study was conducted with the aim of predicting missed nursing care based on moral sensitivity and care-related factors in 2021. Methods: In this cross-sectional-analytical study, 345 nurses working in medical education centers in Ardabil were selected by stratified random sampling from internal, surgical, special care, emergency, and pediatric departments. Data were collected using the modified Lutzen Moral Sensitivity Questionnaire, Kalish's Missed Nursing Care, and Blackman's Missed Care Related factors. The collected data were analyzed using SPSS 19 with descriptive statistics (frequency, percentage, mean, and standard deviation) and inferential statistics (ANOVA, regression, and Pearson). Results: The results of the present study showed that nurses had an average level of moral sensitivity (70.75 ± 10.83). The mean and standard deviation of missed nursing care were (33.93 ± 10.35), which was less than the median of the questionnaire. The components of moral sensitivity (0.32) and related factors (0.08) explain missed care. There was a significant relationship between age, work experience, overtime, and second job with missed care (P < 0.001). Conclusions: Identifying influential factors in reducing missed care is crucial in care, education, and management. It can be said that the dimensions of moral sensitivity and related factors such as human, financial, and communication resources are predictors of missed care. Increasing workload and having a second job increase forgetfulness in caregiving. It is suggested to utilize the experiences of nurses with expertise in care, education, and management, in addition to enhancing moral sensitivity, to ensure comprehensive care while reducing overtime and avoiding second jobs.
Nurses’ professional competency and organizational commitment: Is it important for human resource management?

Abstract:BACKGROUND: Professional competency is a fundamental concept in nursing, which has a direct relationship with quality improvement of patient care and public health. Organizational commitment as a kind of affective attachment or sense of loyalty to the organization is an effective factor for professional competency.OBJECTIVE: This study was conducted to evaluate the nurses´ professional competency and their organizational commitment as well as the relationship between these two concepts.METHODS AND MATERIALS: This descriptive-analytic study was conducted at the hospitals affiliated with a University of Medical Sciences, in the southeast of Iran in 2016. The sample included 230 nurses who were selected using stratified random sampling. Data were gathered by three questionnaires including socio-demographic information, competency inventory for registered nurse (CIRN) and Allen Meyer's organizational commitment.RESULTS: Results showed that professional competency (Mean±SD: 2.82±0.53, range: 1.56-4.00) and organizational commitment (Mean±SD: 72.80±4.95, range: 58-81) of the nurses were at moderate levels. There was no statistically significant correlation between professional competency and organizational commitment (ρ = 0.02; p = 0.74). There were significant differences in professional competency based on marital status (p = 0.03) and work experience (p<0.001).CONCLUSION: The results highlighted that the nurses needed to be more competent and committed to their organizations. Developing professional competency and organizational commitment is vital, but not easy. This study suggests that human resource managers should pursue appropriate strategies to enhance the professional competency and organizational commitment of their nursing staff. It is necessary to conduct more comprehensive studies for exploring the status and gaps in the human resource management of healthcare in different cultures and contexts.

Abstract:Background & Aim: Missed nursing care can reduce nursing care quality by eliminating or delaying any aspect of the patient’s necessary nursing care. Teamwork has been a solution to improve the quality of care and safety of patients. The present study was conducted to determine the relationship between missed nursing care and emergency nurses’ teamwork in the educational-medical centers affiliated to Iran University of Medical Sciences in 2019. Methods & Materials: This cross-sectional study is predictive correlational in nature. It was conducted on 213 nurses working in the emergency department who were selected based on the census method from March 2018 to December 2019. The self-report data were collected using Nursing Teamwork Survey and Missed Nursing Care questionnaires. The data were analyzed through conducting ANOVA, t-test, Pearson correlation, and multiple regression analysis in SPSS version 16. Results: The mean score of missed nursing care in the emergency nurses was 2.25±0.65, and the nurses’ average teamwork was 3.53±0.55. There was a statistically significant yet inverse correlation between teamwork and missed nursing care (p<0.001) (r= -0.29). The determination coefficient of the multiple linear regression modeling was used to predict the missed nursing care based on the total teamwork score indicated that 14% of the missed nursing care was explained under the influence of independent variables. Conclusion: Due to the inverse relationship between teamwork and missed nursing care, it is necessary to design and plan training courses to further enhance the nurses’ ability to perform teamwork and collaborative work.

Abstract:Objective: The aim of this study was to determine the relationship between patients’ reports of nurse caring and patient satisfaction with nursing care. Design: The study used a cross sectional design. The Persian versions of the Revised Caring Behaviors Inventory (CBI) and the Patient Satisfaction Instrument (PSI) were used to establish the strength of association between nurse caring and patient satisfaction with nursing care. Data were analysed using SPSS version 10. Setting: The setting was the educational hospitals affiliated to Iran University of Medical Sciences (IUMS). Heavy workloads and severe staff shortages are common characteristics of Tehran educational hospitals and the time that nurses previously allotted to direct care has been reduced. Subjects: Two hundred and fifty patients who were hospitalised for medical conditions or surgical procedures were selected through a quota sampling method during 2007. Results: The findings indicated a statistically significant relationships between patient reports of nurse caring and satisfaction with nursing care (r=0.72, p=0.000; CI 95%; 178.31‐189.99 for the CBI and 82.81‐86.71 for the PSI). Male patients were more satisfied with nursing care than female patients. Admissions to hospital during the last five years were positively correlated with patients’ perceptions of nurse caring and satisfaction with nursing care. Conclusion: This study which was carried out in Tehran, Iran supports earlier western evidence that nurse caring is associated with patient satisfaction with nursing care. It is important to reflect on the impact of the current health care environment and cultural religion of Iran on caring behaviors of nurses and patient satisfaction with nursing care. Heavy workloads and severe staff shortages are common characteristics of Tehran educational hospitals and the time that nurses previously allotted to direct care has been reduced which may contribute to changes in patients’ perceptions of nurse caring and satisfaction with nursing care possibly reducing the effects of nursing care services. Iranian nurse managers need to consider the religious and cultural barriers to nurse caring and recompense it by providing more human resources.

Abstract:Background Quality of care is one of the most critical issues in nursing care today. Moreover, all health care employees are responsible for providing support and high‐quality, safe care. However, nurses caring for COVID‐19 patients face problems such as unfamiliar work environments, exposure to the disease, lack of experience in their new positions, and close public and media attention. This study aimed to investigate missed nursing care and the reasons for missed nursing care during the coronavirus disease 2019 (COVID‐19) pandemic in Iran. Methods This cross‐sectional study included 135 nurses working in COVID‐19 units. Data were collected using the MISSCARE Survey. The significance level for the statistical tests was set at p < 0.05. Results Most of the participants were women and had a bachelor's degree in nursing. There was a lack of supportive and necessary care more than any other form of care. According to nurses, the most common reasons for missed care were urgent patient situations, inadequate staff, and an unexpected rise in patient volume and/or patient acuity on the unit. Conclusion Generally, the results showed that being positioned in a new situation and unfamiliarity with the disease had a significant impact—among nurses—on the amount of care provided. Managers can use this information to solve existing missed‐nursing‐care problems and contribute to improving the quality of care. These results can be helpful in controlling missed nursing care and finding a more optimal solution for this problem; thus, we can improve the quality of care delivery and increase the satisfaction of nurses and patients. Additionally, an understanding of the kind of missing nursing care during a pandemic can positively enhance the management of similar situations in the future.

Abstract:Background and Objective: Missed nursing care is a global concern. Nursing managers, using leadership skills, have a prominent role in preventing missed nursing care. Ethical leadership is one of the leadership approaches. The present study aimed to describe the impact of ethical leadership perceived by nurses on missed nursing care in 2021. Materials and Methods: In this descriptive correlational study, 230 nurses working in internal medicine and surgery units were selected through a stratified proportional sampling method from two hospitals affiliated with Yazd University of Medical Sciences, Yazd, Iran. Questionnaires on “ethical leadership in nursing” and “missed nursing care” were the data collection tools. Descriptive and inferential statistics were performed for data analysis using SPSS (version 16). Results: The mean score of missed nursing care and perceived ethical leadership was 47.54 ± 2.83 and 109.08 ± 10.75, respectively. The most missed nursing care, in the dimensions of assessment, individual needs, basic care, and planning, were "focused reassessment according to patient’s condition", "emotional support for patient/family", "supervision on feeding patient when the food is still warm", and "attending interdisciplinary care conferences", respectively. The results showed a significant inverse relationship between perceived ethical leadership and missed nursing care (P<0.05). The regression analysis revealed that among the dimensions of ethical leadership, the ethical-oriented dimension can be considered a negative predictor of missed nursing care. Conclusion: Implementing ethical leadership is one of the practical strategies to decrease missed nursing care. The participation and performance of nursing managers in this process are critical to a successful outcome.

Abstract:Purpose. This study aimed to (1) assess the levels and associations of workplace spirituality (WPS), compassion satisfaction (CS), burnout (BO), and secondary traumatic stress (STS) among Iranian nurses; and (2) identify the impact of WPS components on CS, BO, and STS while controlling for demographic variables. Design and Methods. In this descriptive, cross‐sectional, and correlational study, 295 nurses were selected via proportionate stratified sampling from five hospitals affiliated with the Arak University of Medical Sciences in Iran from June 2022 to December 2022. The scale of workplace spirituality and Professional Quality of Life version 5 (ProQOL-5) were used for data collection. Data were analyzed using SPSS24 and descriptive and inferential statistics (Pearson correlation coefficient and multiple regression). Findings. The mean scores of WPS, CS, BO, and STS were 63.27, 36.55, 27.31, and 29.58, respectively. The highest percentages of nurses reported moderate CS, BO, and STS levels, reaching rates of 76.3%, 80%, and 86.1%, respectively. The Pearson correlation coefficient showed that CS had a negative relationship with BO and STS. There was a direct correlation between BO and STS. Participants who reported higher levels of WPS had higher CS and lower BO scores. Results of multiple regression analyses indicate that the model of WPS components and control variables explains 42.7, 34, and 8.6 percent of the variance of CS, BO, and STS, respectively. Higher “meaningful work” is associated with higher CS and lower BO and STS among nurses. Participants higher in “sense of community” had higher CS scores. Higher “alignment with organizational values” is associated with higher STS. Practice Implications. According to the study results, it is possible to provide a suitable platform for improving nurses’ professional quality of life by strengthening workplace spirituality.

Moral sensitivity enables nurses to interpret and respond to clients' needs according to ethical principles. Abstract:Aim: To examine the correlation between moral courage and caregiving behaviour among nurses who are employed in the COVID-19 ward in Iran. Design: This study is a descriptive-correlational and cross-sectional research conducted in 2021 in the city of Tehran, Iran. Methods: A total of 270 nurses employed in COVID-19 wards, who had been working in these wards for at least 1 month, were recruited from XXX hospitals and XXX Hospital. Data collection was conducted from September to November 2021. The data collection tools included a demographic information form, the Caring Dimension Inventory (CDI-25), and the Professional Moral Courage Scale (PMC). The designed questionnaire links were provided to the participants virtually through the Persian platform 'Pars Line' and via WhatsApp. Descriptive statistics and inferential statistics were used to examine the relationship between moral courage and care behaviour. Results: There was a significant difference in the mean of caring behaviour between the age groups (<30 and >40 years old), and the work experience groups (<5 years and >15 years) (p < 0.05). Ethical values also varied significantly between certain age and experience groups (p < 0.05), but no significant differences were found in ethical behaviour across most ages (30-40 years) and work experience categories (>10 years) (p > 0.05). The level of moral courage among the nurses was higher than the median score of the instrument (87.31 ± 10.37). Among the dimensions of moral courage, the highest score was related to the dimension of ethical factors (17.64 ± 2.64), and the lowest score was related to the dimension of multiple values (17.26 ± 2.78). The level of caring behaviours among the nurses was lower than the median score of the instrument (46.11 ± 10.84). Among the dimensions of caring behaviours, the highest score was related to physical-technical behaviours (19.22 ± 5.27), and the lowest score was related to professional behaviours (1.35 ± 0.67). Based on the analysis, moral courage had a weak and inverse correlation with caring behaviours among nurses. The higher moral courage does not necessarily result in improved caregiving behaviours, emphasizing the need for further research to explore and address this relationship. The level of organizational commitment of healthcare workers, including nurses, is closely connected to the productivity and quality of care provided by healthcare institutions. The aims of the present study were to explore nurses' levels of organizational commitment and the impact of key sociodemographic variables on this issue. Materials and Methods: A cross-sectional descriptive quantitative study was conducted at a major public hospital in Saudi Arabia during April and May 2019. A revised validated version of the three-component model (TCM) questionnaire was self-administered to a systematic random sample of 384 nurses. The data were analyzed using Statistical Package for Social Sciences (SPSS) version 22. Mean scores were compared by independent variables using an independent sample t -test and analysis of variance (ANOVA). Multiple linear regression analysis was performed. Results: Out of 384 participants, 337 responded, yielding a response rate of 88%. Overall, 47.88% of the nurses agreed with all items related to the organizational commitment scale, while only 22.3% disagreed. There was a significant difference in the levels of commitment among nurses in the various age groups ( p = 0.024). The continuous commitment subscale received the largest number of positive responses. Conclusion: Most nurses showed a moderate level of job commitment. Greater organizational commitment was positively related to sociodemographic variables, such as age and nationality, and the only positive predictor of overall organizational commitment was age. Nursing policy makers should enhance the organizational commitment of nurses by developing strategies to recruit, attract, and retain committed nurses. Abstract:Policy, Politics, &Nursing Practice, Ahead of Print. Missed nursing care is a multifaceted patient safety issue receiving increased attention among healthcare scholars worldwide. There is limited research on missed nursing care in the Jordanian healthcare context. The current study sought to examine the perceptions of Jordanian nurses toward the amount and types of missed nursing care in medical and surgical wards. We also examined the differences in missed care items between public, private, and university hospitals in Jordan. This was a cross-sectional study using the MISSCARE Survey tool. Data collection spanned 4 months between March and July 2021. The final study sample consisted of 672 registered nurses employed in five public, three private, and two university hospitals in Jordan. Data were analyzed using descriptive statistics, Analysis of variance, and Pearson correlation coefficent test. Of the 672 registered nurses who participated, the majority were females (n = 421; 62.6%). Most participants held a bachelor's degree in nursing (n = 577; 85.9%). The three most common missed nursing activities in the participating hospitals were: ambulation, oral care, and emotional support. Nurses working in public hospitals reported the highest missed nursing care. The age and number of patients under care significantly correlated with missed nursing care. The findings could help nursing managers develop plans to reduce missed nursing care in their healthcare institutions.

Abstract:The issue of missed nursing care is a problem that affects the overall quality of nursing care in hospitals around the world. However, there is limited research about it in Ethiopia. This study assessed missed nursing care and factors among nurses. An institutional-based cross-sectional study was conducted among 485 nurses at the University of Gondar Specialized Hospital. The data were collected from May 15 to June 15, 2022 using a self-administered questionnaire. A simple random sampling technique was used to select study participants. The data was entered to EPI DATA version 4.6 and exported to SPSS version 23 for analysis. Descriptive statistics were computed, and a summative score of missed nursing care was calculated. Then, multiple linear regression analysis was used to identify the factors. A P-value of 0.05 was used to declare significant level. In this study, missed nursing care was 62.5% (95% CI 60.98–64.02). Documentation, vital signs assessment, intravenous care, and assessment as per hospital policy and patient education were the most frequently missed nursing care. Being single in marital status (B = −4.609, P = 0.004), adequacy of nursing staff (B = −2.458, P = 0.003), satisfaction with income (B = −8.753, P = 0.007), working in medical unit (B = −5.708, P = 0.002) and working in both day and night shift (B = 1.731, P = 0.027) were statistically associated factors. More than half of the participants missed basic nursing care. Most of the identified factors correlated with missed nursing care negatively. Efforts should be done to enhance communication between health team members, assign a manageable number of patients per nurse, and adjust shifts and units.

Abstract:Context: An important factor that significantly affects missed nursing care is the nursing work environment. Additionally, it significantly impacts the recruitment and retention of healthcare professionals and indirectly affects the standard of nursing care. Aim: To identify nurses' perception of the professional practice environment, identify types and reasons for missed nursing care, and find the relationship between nurses' perceptions of the professional practice environment and missed nursing care during the Hajj season. Methods: A descriptive correlational design was carried out at seven hospitals in Makkah region. A convenience sample of 463 registered nurse who has at least one year of nursing experience and working in inpatient units was included in this study. Data was collected through an online questionnaire. It included patients' demographic data, the revised professional practice environment scale, and missed nursing care assessment questionnaire. Results: The result of this study reveal that the mean scores of the professional practice environment and its subscales were 3.56±0.46, indicating a favorable environment. Furthermore, it was found that the overall missed nursing care among the studied nurses was 1.56±0.65, which indicates low missed nursing care. The highest reason for missed nursing care was the inadequate staff (2.85±1.12), supplies/equipment not functioning properly when needed (2.74±1.07), and not being available when needed (2.70±1.10). The professional practice environment statistically significantly negatively correlated with missed nursing care (r=-0.32, p=0.000). Conclusion: Missed nursing care in hospitals can be reduced by improving the professional practice environment. The study recommends that the hospital and nursing administrators consider keeping good practice environments for nurses to prevent missed care activities and potentially enhance patient outcomes in nursing units.

Abstract:Background. Missed nursing care, which has been explored in various acute care settings, results in adverse patient outcomes and job dissatisfaction in nurses. However, little is known about missed care in the operating room. Objective. This study tested a hypothesised model to identify relationships between nurses’ age, years of experience in the operating room, job satisfaction, and intention to leave which have direct and indirect effects on the frequency of missed care. The frequency of missed care was hypothesised to be mediated by nurses reported perioperative competence and the reasons for missed care. Design. A cross-sectional design using an online survey of Australian perioperative nurses was undertaken in 2022. Methods. All Australian College of Perioperative Nurses members were invited to participate. Missed nursing care was measured using the MISSCARE Survey-OR. Age, years of experience, and intention to leave were single-item measures. Satisfaction was a three-item scale. Competence was measured by the 18-item Perceived Perioperative Competence Scale-Short Form. Structural equation modelling was used to test our hypothesised model. Results. Of the 5,500 nurses invited, 853 (15.5%) responded, but only 602 (10.9%) participant responses were usable for inclusion in the model. The model demonstrates that participants’ age directly predicted the frequency of missed care, nurse role satisfaction, perceived perioperative competence, and reasons for missed care. The reasons for missed care and perceived perioperative competence were mediators that were negatively associated with the frequency of missed care. Conclusions. While the final model explained 22.6% of the frequency of missed care, other variables not identified in this study may influence this outcome.
