Abstract:This book is based on over 40 years work in public health at a time of unprecedented change and challenge. The emphasis is on the practical aspects of working at different levels of action, very much ‘how to do it and how it was done’. As such it is a personal account. This period marked a new era in which the previous medical paradigm, dating from the mid-nineteenth century, was replaced by a broader, multidisciplinary approach, grounded in social science, the humanities, ecology, and public engagement with the politics of health once more coming into focus. The author uses case studies, storytelling, and real-life experience of establishing a new and revitalized public health system in the North West of England to bring the subject alive for a new generation of students and practitioners. Building on historic insights and timeless lessons from the Victorian and early-twentieth-century pioneers, he traces the evolution of the new thinking and its translation into action. The volume offers a rich menu of examples of responses to an array of new challenges ranging from new infections, such as HIV/AIDS and Ebola, to the lifestyle diseases of the new age, and the application of public health thinking to mental health and the problems of an ageing population. The external threats to health from the environment and as a result of man-made disasters and emergencies are extensively covered. The author brings a fresh approach to public health and the communication of public health issues. This work is accessible and stimulating, speaking to a wide range of audiences and sharing his passion for the subject.

Abstract:This article provides an historical perspective within which two recent, alternative directions for health education are examined. Each direction is seen as reflecting a unique vision of health promotion, with the first focusing primarily on personal be havior change and the latter on a broad empowerment/environmental model of health promotion. Key historical developments in the evolution of these two perspectives are examined, as are some of the assumptions and ideological values underlying these alter native approaches. The World Health Organization's "Healthy Cities Project" then is used to illustrate the broader vision of health promotion in practice. While recognizing that the health educator has contributions to make on both the micro and macro change levels, a case is made for moving the field of health education further in the direction of this broader model of health promotion, and roles for the health educator within such a paradigm are outlined.

Abstract:Thirty years ago, the World Health Organization (WHO) Ottawa Charter for Health Promotion created a paradigm shift in addressing major public health challenges. Traditional approaches to health education focused on personal health ‘risks’ and lifestyle choices were quickly overshadowed by the attention given to more comprehensive policy and environmental interventions. Since that time health education has evolved in content, media use and sophistication of communication to fulfil a wider range of purposes. The concept of health literacy has been useful in sustaining this change. As the tools for communication have been transformed by digital communication, and the marketplace for communication has become more crowded and complex, health education has continued to evolve to reflect these changes, enabling people to navigate competing sources of information and to engage meaningfully with social and economic determinants of health. Equitable access to quality health education and lifelong learning remain the cornerstones of modern health promotion.

Abstract:This article provides a comprehensive exploration of the application and development of Traditional Chinese Medicine (TCM) within modern healthcare systems. It begins with a historical overview of TCM and delves into its fundamental principles, including concepts of Yin-Yang, the Five Elements, Qi, blood, and body fluids. The role of TCM in contemporary healthcare, particularly its effectiveness and potential in treating chronic diseases and mental health issues, is then analyzed. Additionally, the paper discusses future trends of TCM in global healthcare and strategies to enhance its international influence and acceptance. The study highlights the unique value of TCM in offering more holistic, preventative, and personalized medical services, despite facing challenges of further scientific validation and standardization. The article concludes by emphasizing the integration of TCM with modern medical practices as a novel approach to global health and well-being.

Abstract:This article is the second in a new series in this regular feature. The intention of the series is to look at important global developments in health science libraries. These articles will serve as a road map, describing the key changes in the field and exploring factors driving these changes. The present article by two Canadian librarians identifies important national developments which are shaping the profession such as the centralisation of health care services, the challenge of providing consumer health information in the absence of a national strategy, government recognition of the need to recognise and respond to the health needs of indigenous peoples and the growing emphasis on managing research data. Although their profession is strong, health science librarians must find ways of providing enhanced services with fewer staff and demonstrate value to organisations. JM.

Modifying and developing health behavior

Abstract:The literatures on both behavior modification and behavioral development have engendered innovations in public health programs, addressing problems of patient adherance to preventive and therapeutic regimens, delay in seeking diagnosis of illness symptoms, risk-taking behavior, and other aspects of lifestyle associated with health. Because most of this literature derives from psychology, there has been a distinct bias in the construction of interventions, pointing them directly at individuals, usually in a counseling or small group mode of delivery. These developments served public health well enough during a decade or so when the preoccupation was with utilization of health services and medical management of chronic diseases. With the publication of the Lalonde Report in Canada in 1974, the passage of Public Law 94-317 in 1976 in the United States, and similar initiatives in other English-speaking and European countries, the recognition of the greater complexities of lifestyle development and modification in the absence of symptoms has taken hold. Policy makers and public health workers seek a more efficient and equitable set of strategies to meet the behavioral health challenges of modern society without placing the entire weight of responsibility for behavior on the individual or on therapeutic practitioners. Concurrently, on a more global scale and in the developing countries, a concern has emerged for strategies that give individuals, families, and communities a greater role in deciding their own health priorities. The convergence of these two trends--one seeking to distribute responsibility for lifestyle more equitably and the other seeking to distribute responsibility for planning health programs more equitably --calls for policies, strategies, and interventions that will place similar emphasis on health education and organizational, economic, and environmental supports for health behavior. The combination of these elements of support for behavior calls, in turn, for research and more inventive applications of theory from sociology, political science, economics, and anthropology. Public health workers will need to become more conversant and facile in these social sciences, as they have in psychology and its applications in the recent past.

Abstract:Health promotion is conceived as a unifying concept for improving the health of populations. This means addressing the socio-cultural, economic and commercial causes of ill-health, which are necessarily informed by past policies and socio-cultural contexts. However, historical scholarship has rarely figured in health promotion practice or scholarship. This gap resides in the determinants of health, and notably in the analyses of tobacco control and skin cancer prevention, two long-running campaigns that have shaped modern health promotion in Australia. Both highlight a need for understanding the profound impact of history on the present and the value of learning from past successes and failures. Doing so requires integrating historical analyses into existing health promotion scholarship. To achieve this aim, we present a new 'public health humanities' methodology. This novel interdisciplinary framework is conceived as a spectrum in which historical studies integrate with existing health promotion disciplines to solve complex health problems. We draw on the many calls for more interdisciplinarity in health promotion and derive this methodology from proposals in the medical humanities and cognate fields that have wrestled with combining history and present-focused disciplines. Using tobacco control and skin cancer prevention as case studies, we demonstrate how public health humanities uses interdisciplinary teams and shared research questions to generate valuable new knowledge unavailable with traditional methods. Furthermore, we show how it creates evaluation criteria to consider the powerful impact of issues like colonialism on current inequities that hinder health promotion strategies, and from which lessons may be derived for the future.

Abstract:Patient preference has driven a number of trends in the past 100 years of medicine including the rise of concierge practices, telemedicine, and same-day surgery among many other examples. In the end, convenience and access have driven all these trends; patients would like to more easily and completely access their doctor’s expertise. They would like to do this without leaving their home or spending too much time in the doctor’s waiting room, and given the busy constraints of modern life, this is entirely understandable.

Abstract:Abstract Background Community health worker (CHW) programmes are again receiving more attention in global health, as reflected in important recent WHO guidance. However, there is a risk that current CHW programme efforts may result in disappointing performance if those promoting and delivering them fail to learn from past efforts. This is the first of a series of 11 articles for a supplement entitled “Community Health Workers at the Dawn of a New Era”. Methods Drawing on lessons from case studies of large well-established CHW programmes, published literature, and the authors’ experience, the paper highlights major issues that need to be acknowledged to design and deliver effective CHW programmes at large scale. The paper also serves as an introduction to a set of articles addressing these issues in detail. Results The article highlights the diversity and complexity of CHW programmes, and offers insights to programme planners, policymakers, donors, and others to inform development of more effective programmes. The article proposes that be understood as actors within community health system(s) and examines five tensions confronting large-scale CHW programmes; the first two tensions concern the role of the CHW, and the remaining three, broader strategic issues: What kind of an actor is the CHW? A lackey or a liberator? Provider of clinical services or health promoter? Lay versus professional? Government programme at scale or nongovernmental organization-led demonstration project? Standardized versus tailored to context? Vertical versus horizontal? Conclusion CHWs can play a vital role in primary healthcare, but multiple conditions need to be met for them to reach their full potential.

Abstract:This paper critiques the topic of postmodernism and how it is represented in nursing and social science literature. This critique classified the debates into three identifiable constructs, those being: dissatisfaction; fragmentation and integration. The authors propose a solution from the integration debate by putting forward the notion of an 'integrated postmodern turn'. The 'solution' is situated within feminism and draws on modernist and postmodernist theory. The integrated model is grounded in nursing clinical examples which demonstrate the usefulness and workability of this approach when caring for those who are marginalized by nurses because of their ethnicity, gender, cultural and/or spiritual beliefs.

Abstract:Health promotion programs often lack a clearly specified theoretical foundation or are based on narrowly conceived conceptual models. For example, lifestyle modification programs typically emphasize individually focused behavior change strategies, while neglecting the environmental underpinnings of health and illness. This article compares three distinct, yet complementary, theoretical perspectives on health promotion: behavioral change, environmental enhancement, and social ecological models. Key strengths and limitations of each perspective are examined, and core principles of social ecological theory are used to derive practical guidelines for designing and evaluating community health promotion programs. Directions for future health promotion research are discussed, including studies examining the role of intermediaries (e.g., corporate decision-makers, legislators) in promoting the well-being of others, and those evaluating the duration and scope of intervention outcomes.
Contemporary models of change in the health professions

Abstract:This article reviews the special position that health professionals have occupied and the ways in which changes threaten the foundations of professional work. The application of modern management principles to health care runs the risk of overriding the "action orientation" that is a defining component of professional work. One goal of health workforce design should be the engineering of opportunities for the preservation of "professional voice" as a countervailing force to ensure high quality health care. Contemporary models of change applied to health care workforce include: (1) the system of professions models in which securing and maintaining jurisdiction are the mechanisms that professions employ to sustain their position, (2) a strategic adaptation model by which professions attempt to adjust to changing environments, (3) a model of redesigning patient care which applies Total Quality Management (TQM) and other "industrial techniques" to the health care workplace, and, (4) model of "consumer sovereignty" in which groups of citizens come together to determine the nature of care services and professional work, with the participation of the organizations and providers.
Alterations in the Concentration of Cholesterol in Bile after Administration of Human Growth Hormone

Abstract:Extract: To determine the influence of human growth hormone (hGH) on gall bladder bile composition, bile (concentration and molar percentage of cholesterol, bile acids, and phospholipids) was examined in three growth hormone-deficient patients and in one control subject with constitutional short statute, but with adequate secretion of growth hormone. Investigations were made before and after 0.3, 4, and 12 months of therapy with exogenous hGH, 3 U/week. Two patients responded to hGH with accelerated growth (increase in height velocity from 1.3 and 0.2 cm/year to 10.5 and 3.0 cm/year, respectively), but, during the same time, the cholesterol concentration in bile increased progressively, raising the “lithogenic index” from 1.00 and 0.93 to 1.66 and 1.10, respectively. Normally, the “lithogenic index” is 1.00 or less. The third patient also showed some growth acceleration, but his bile did not become oversaturated with cholesterol. The bile of the control subject was excessively saturated with cholesterol after 0.3 months of treatment with hGH, raising the “lithogenic index” from 0.10 to 2.94, the highest yet reported in man. However, this abnormal response to hGH was not sustained, and bile composition returned to normal later on. The data obtained in this study suggest, but do not prove, that hGH might influence cholesterol secretion into bile.Speculation: Normally, biliary cholesterol is kept in micellar solution by bile acids and phospholipids. An increase in the concentration of cholesterol in bile, without a concomitant increase of bile acids and/or phospholipids, causes oversaturation of bile with cholesterol, as expressed in changes of the lithogenic index. The lithogenic index is a quantitative expression of the cholesterol saturation in bile by a single figure which represents all three major biliary lipids. Oversaturation of bile with cholesterol is now recognized as a probable prerequisite for the formation of cholesterol gall stones. Further studies should now define the influence of hGH on biliary cholesterol secretion rates.

In addition to the 3 5 cities formally associated with the WHO Europe project, there are now 18 national networks and hundreds of towns and cities actively involved in Europe, North America, and, increasingly, the developing world.This article reviews the concept, its evolution and current practice, considers some of the problems in applying the concept, and speculates on its potential future development.

Abstract:The fourth article of the Happy Life Club series explores the patient-centred approach as spirit of the chronic disease management initiative.Patient-centred healthcare builds on biopsychosocial model that aims on responding patient′s needs both of physical and mental health and eventually improving quality of life;understands patient as a whole person;collaborating between supply side and demand side through changing patient-doctor relationship from egalitarian to guiding approach;and uses psychotherapeutic skills as essential for personalised service and effective health outcome.Patient-centeredness is spirit of the Happy Life Club that adding motivational interviewing into routine clinical diabetes management and promoting improvement of quality of life through assisting change behaviours.Physical,mental and social health is considered during patient assessment,intervention implementation and evaluation.Health coach and participant are encouraged to share responsibility,jointly own diabetes management and involve in quality improving activity.Trained health coach applies processes of behaviour change in individualized coaching service.In addition,health coach is encouraged jointly use medical and psychological skills in routine service.The final part of the article is to explore implications of the Happy Life Club spirit on regional community health and general practice development.

Abstract:Traditional Chinese medicine (TCM) is an integral part of mainstream medicine in China. Due to its worldwide use, potential impact on healthcare and opportunities for new drug development, TCM is also of great international interest. Recently, a new era for modernisation of TCM was launched with the successful completion of the Good Practice in Traditional Chinese Medicine Research in the Post-genomic Era (GP-TCM) project, the European Union’s Seventh Framework Programme (FP7) coordination action on TCM research. This 3.5-year project that involved inputs from over 200 scientists resulted in the production of 20 editorials and in-depth reviews on different aspects of TCM that were published in a special issue of Journal of Ethnopharmacology (2012; volume 140, issue 3). In this narrative review, we aim to summarise the findings of the FP7 GP-TCM project and highlight the relevance of TCM to modern medicine within a historical and international context. Advances in TCM research since the 1950s can be characterised into three phases: Phase I (1950s-1970s) was fundamental for developing TCM higher education, research and hospital networks in China; Phase II (1980s-2000s) was critical for developing legal, economic and scientific foundations and international networks for TCM; and Phase III (2011 onwards) is concentrating on consolidating the scientific basis and clinical practice of TCM through interdisciplinary, interregional and intersectoral collaborations. Taking into account the quality and safety requirements newly imposed by a globalised market, we especially highlight the scientific evidence behind TCM, update the most important milestones and pitfalls, and propose integrity, integration and innovation as key principles for further modernisation of TCM. These principles will serve as foundations for further research and development of TCM, and for its future integration into tomorrow’s medicine.

What Are Healthy Societies? A Thematic Analysis of Relevant Conceptual Frameworks

Abstract:Background: While support for the idea of fostering healthy societies is longstanding, there is a gap in the literature on what they are, how to beget them, and how experience might inform future efforts. This paper explores developments since Alma Ata (1978) to understand how a range of related concepts and fields inform approaches to healthy societies and to develop a model to help conceptualize future research and policy initiatives. Methods: Drawing on 68 purposively selected documents, including political declarations, commission and agency reports, peer-reviewed papers and guidance notes, we undertook qualitative thematic analysis. Three independent researchers compiled and categorised themes describing the domains of a potential healthy societies approach. Results: The literature provides numerous frameworks. Some of these frameworks promote alternative endpoints to development, eschewing short-term economic growth in favour of health, equity, well-being and sustainability. They also identify values, such as gender equality, collaboration, human rights and empowerment that provide the pathways to, or underpin, such endpoints. We categorize the literature into four "components": people; places; products; and planet. People refers to social positions, interactions and networks creating well-being. Places are physical environments-built and natural-and the interests and policies shaping them. Products are commodities and commercial practices impacting population health. Planet places human health in the context of the 'Anthropocene.' These components interact in complex ways across global, regional, country and community levels as outlined in our heuristic. Conclusion: The literature offers little critical reflection on why greater progress has not been made, or on the need to organise and resist the prevailing systems which perpetuate ill-health.

Abstract:The term planetary health - denoting the interconnections between the health of person and place at all scales - emerged from the environmental and holistic health movements of the 1970-80s; in 1980, Friends of the Earth expanded the World Health Organization definition of health, stating: "health is a state of complete physical, mental, social and ecological well-being and not merely the absence of disease - personal health involves planetary health". By the 1990s, the concept of planetary health was part of the fabric of integrative medicine; more recently, after the 2015 Lancet Commission on Planetary Health report, the concept has penetrated mainstream academic and medical discourse. Here, we explore this history and describe its relevance to contemporary healthcare; integrative medicine is uniquely positioned to educate and advocate on behalf of patients and communities (current and future generations), helping to safeguard health of person, place and planet. We use the emerging microbiome science as a way to illustrate the interconnectivity and health implications of ecosystems (including social/political/economic systems) at all scales. As highlighted in the Canmore Declaration, mainstream planetary health discourse will be strengthened by inter-professional healthcare perspectives, and a more sophisticated understanding of the ways in which social dominance orientation and medical authoritarianism compromise the World Health Organization's broad vision of global health. Planetary health isn't a "new discipline"; it is merely an extension of a concept that was understood by our ancestors, and remains the vocation of all healthcare providers. Discourse on the topic requires cultural competency, critical consciousness and a greater appreciation of marginalized voices.
