Abstract:Health equity in Bangladesh faces a large chasm over the economic conditions, socio-cultural factors and geographic location despite the push for digitalization of the health sector. While some research has been conducted assessing the viability of digital health solutions in Bangladesh, gender dynamics of digital healthcare have been absent. This study dived into healthcare equity for women with a focus on reproductive health services delivered through mobile devices. This paper reported the findings of a qualitative study employing in-depth interviews conducted among 26 women about their behavioral intention to use mHealth services for reproductive health and the underlying factors influencing this intention with the help of the Integrative Model of Planned Behavior (IMPB). A snowball sampling technique were used to interview those university educated women, aged 21-31, based on their familiarity and exposure of mHealth services from seven universities in Bangladesh. The findings suggested that users of mHealth services find it more convenient and secure compared to visiting healthcare facilities, especially for trivial issues and inquiries regarding their reproductive health. Although promoting such services is lagging behind traditional healthcare, the attitude toward reproductive health services in Bangladesh is generally favorable resulting increasing adoption and use. Because such information-related mobile services (apps, websites, and social media) served as a first base of knowledge on reproductive health among many young girls and women in Bangladesh, who are generally shy to share or talk about their menstruation or personal health problems with family members, peers, or even health professionals due to socio-cultural factors and stigmatization. Conversely, urban centric services, availability of experts, quality management, security of privacy, authenticity of the information, digital divide, lack of campaign initiatives, lack of equipment and technology, lack of sex education, and outdated apps and websites were identified as obstacles that constrain the widespread use of reproductive mHealth services in Bangladesh. This study also concluded that promotion will be crucial in reforming conservative norms, taboos, and misconceptions about women's health and recommended such endeavors to be initiated by the policy makers as there is a substantive need for a specific policy regulating emerging digital health market in Bangladesh. Notwithstanding, women-only sample, low sample size, narrow focus on mHealth users and absence of perspectives from healthcare providers were among shortcomings of this study which could be addressed in future research. Further quantitative explorations are must to determine the usage patterns of reproductive mHealth services and their effectiveness that would identify implementation challenges in terms of customization and personalization in reproductive healthcare in a developing country like Bangladesh.

Abstract:Bangladesh, a developing country with a large and dense population, has recently seen significant economic as well as technological developments. The growth of technology has resulted in a dramatic increase in the number of smartphone users in Bangladesh, and as such, mobile apps have become an increasingly important part of peoples' life, even encompassing healthcare services. However, the apps used in healthcare (telemedicine to be specific) in Bangladesh are yet to be studied from the perspective of their features as per the voices of the users as well as service providers. Therefore, in this study, we focus on the features of the telemedicine apps used in Bangladesh. First, we evaluated the present status of existing telemedicine apps in Bangladesh, as well as their benefits and drawbacks in the context of HCI. We analyzed publicly accessible reviews of several Bangladeshi telemedicine apps (N = 14) to evaluate the user impressions. Additionally, to ascertain the public opinion of these apps, we performed a survey in which the patients (N = 87) participated willingly. Our analysis of the collected opinions reveals what users experience, what they appreciate, and what they are concerned about when they use telemedicine apps. Additionally, our study demonstrates what users expect from telemedicine apps, independent of their past experience. Finally, we explore how to address the issues we discovered and how telemedicine may be used to effectively offer healthcare services throughout the country. To the best of our knowledge, this study is the first to analyze the perception of the people of Bangladesh towards telemedicine apps from the perspective of features of the apps.

Abstract:Abstract Objective. To demonstrate proof of concept for a smart phone-empowered community health worker (CHW) model of care for breast health promotion, clinical breast examination (CBE), and patient navigation in rural Bangladesh. Methods. This study was a randomized controlled trial; July 1 to October 31, 2012, 30 CHWs conducted door-to-door interviews of women aged 25 and older in Khulna Division. Only women who disclosed a breast symptom were offered CBE. Arm A: smart phone with applications to guide interview, report data, show motivational video, and offer appointment for women with an abnormal CBE. Arm B: smart phone/applications identical to Arm A plus CHW had training in “patient navigation” to address potential barriers to seeking care. Arm C: control arm (no smart phone; same interview recorded on paper). Outcomes are presented as the “adherence” (to advice regarding a clinic appointment) for women with an abnormal CBE. This study was approved by Women's College Hospital Research Ethics Board (Toronto, Ontario, Canada) and district government officials (Khulna, Bangladesh). Funded by Grand Challenges Canada. Results. In 4 months, 22,337 women were interviewed; <1% declined participation, and 556 women had an abnormal CBE. Control group CHWs completed fewer interviews, had inferior data quality, and identified significantly fewer women with abnormal breast exams compared with CHWs in arms A and B. Arm B had the highest adherence. Conclusion. CHWs guided by our smart phone applications were more efficient and effective in breast health promotion compared with the control group. CHW “navigators” were most effective in encouraging women with an abnormal breast examination to adhere to advice regarding clinic attendance.
Paving the pathway for women's empowerment? A review of information and communication technology development in Bangladesh

Abstract:This paper reviews the recent situation in information and communication technology (ICT) development in Bangladesh, with particular reference to mobile phone technologies. While mass access to ICT is often portrayed as the great socio-economic leveller, we show that in the Bangladesh case the existing evidence illustrates something quite different. Despite the high profile of certain female politicians, Bangladesh performs very poorly on international measures of gender equality and the reasons are deeply rooted in social and cultural norms. As a consequence, programmes aimed at poverty alleviation through widespread deployment of ICT in rural areas are disproportionately beneficial to men, even when targeted primarily at women. We conclude that unless ICT programmes are more sensitive to the causes of gender inequity, the ICT revolution in Bangladesh is likely to deepen rather than ameliorate the gender divide.
A New Algorithm for a High Level of Protection Against Pacemaker‐Mediated Tachycardia

Abstract:Pacemaker‐mediated tachycardia (PMT) is a well‐known complication of DDD pacing. PMT needs a permeable V‐A conduction and is usually initiated by a premature ventricular or atrial systole, artifact sensing, or misprogramming (long AV delay [AVD]). Today, protection against PMT is expected from pacemaker multiprogrammability. Unfortunately, this prevention is often ineffective; postventricular atrial refractory period (PVARP) must be prolonged, which limits the upper tracking rate and the patient capacity. The new Chorus ELA Medical DDD pacemaker provides classic protection against PMT (PVARP prolongation after premature V or A complex, magnet application, noise sensing), hut also automatically reduces an eventual PMT and adjusts AVD or PVARP for a high level of protection. The process is divided in four steps: (1) a sensing step for 16 cycles, with V‐P conduction analysis; (2) confirmation of the presence of the PMT and analysis of V‐A conduction time; (3) a termination step, by extending the PVARP after the following ventricular heart beat; and (4) in case of immediate recurrence of the tachycardia, reprogramming of the AVD and eventually of the PVARP. By first reducing AVD, before reprogramming PVARP, the pacemaker preserves point 2:1, providing a higher exercise capacity. This algorithm was successfully tested in three patients who had a permeable V‐A conduction, without any adverse effect.

Abstract:In the West African country of Ghana, depression is a significant issue affecting a large number of women. Despite its importance, the issue received insufficient attention during the COVID-19 pandemic. In developed countries, mobile phones serve as a convenient medium for accessing health information and providers. However, in Ghana, women's access to mobile phones is limited by cultural, social, and financial constraints, hindering their ability to seek mental health information and support. While some women in deprived areas can afford feature phones, such as the Nokia 3310, the lack of advanced smartphone features further restricts their access to necessary health information. This paper reviews the potential of Unstructured Supplementary Service Data (USSD) technology to address these challenges. Unlike Short Messaging Service (SMS), USSD can facilitate data collection, complex transactions, and provide information access without the need for internet connectivity. This research proposes studying the use of USSD to improve access to mental health resources for resource-deprived women in Ghana.

Abstract:BackgroundMobile phone-based technology has been used in improving the delivery of healthcare services in many countries. However, data on the effects of this technology on improving primary healthcare services in resource-poor settings are limited. The aim of this study is to develop and test a mobile phone-based system to improve health, population and nutrition services in rural Bangladesh and evaluate its impact on service delivery.MethodsThe study will use a quasi-experimental pre-post design, with intervention and comparison areas. Outcome indicators will include: antenatal care (ANC), delivery care, postnatal care (PNC), neonatal care, expanded programme on immunization (EPI) coverage, and contraceptive prevalence rate (CPR). The study will be conducted over a period of 30 months, using the existing health systems of Bangladesh. The intervention will be implemented through the existing service-delivery personnel at various primary-care levels, such as community clinic, union health and family welfare centre, and upazila health complex. These healthcare providers will be given mobile phones equipped with Apps for sending text and voice messages, along with the use of Internet and device for data-capturing. Training on handling of the Smartphones, data-capturing and monitoring will be given to selected service providers. They will also be trained on inputs, editing, verifying, and monitoring the outcome variables.DiscussionMobile phone-based technology has the potential to improve primary healthcare services in low-income countries, like Bangladesh. It is expected that our study will contribute to testing and developing a mobile phone-based intervention to improve the coverage and quality of services. The learning can be used in other similar settings in the low-and middle-income countries.

Abstract:Background: Menstruation is a normal physiological process for women during their reproductive cycle, typically beginning during adolescence. During this stage, lack of knowledge, social taboos, and shyness act as barriers to proper menstrual hygiene management, rendering adolescent girls more vulnerable. This issue is highly prevalent in low- and middle-income countries. In rural areas of Bangladesh, there is a deficiency in menstrual hygiene management due to inadequate information and knowledge among adolescent girls. Therefore, this study aims to assess the effect of mHealth education on the knowledge and practices of menstrual hygiene management among school-going adolescent girls in rural Bangladesh. Methods: This is a quasi-experimental study conducted from early June to December 2023 at a secondary high school in Chandpur, Bangladesh. Participants' data will be collected through face-to-face interviews using a structured questionnaire covering socioeconomics, knowledge of menstrual hygiene management, and practices. Pre-test data will be collected at baseline, followed by a 6-month mHealth education intervention. Afterward, post-test data will be collected using the same questionnaire. The data will be analyzed as frequency and percentage for descriptive statistics, and a paired t-test will be used to compare the pre-and post-test data. Results: In the study, 172 participants were enrolled at baseline. Among them, 69.8% were aged 10-14 years. The outcome of this study will be published in a peer-reviewed journal. The findings will provide evidence-based information for the government, researchers, and policymakers on menstrual hygiene management using mobile health technology. Conclusion: mHealth education can be posited as a significant tool for increasing knowledge and practices related to menstrual hygiene management in rural regions of Bangladesh.
Applying the principles for digital development to improve maternal and child health in the Peri-urban areas of Karachi, Pakistan

Abstract:Low- middle-income countries, including Pakistan, are facing significant obstacles in their efforts to achieve the global targets for maternal, newborn, and child health (MNCH) defined by the Sustainable Development Goals. Barriers at the individual, health system, and contextual levels undermine healthcare access for pregnant women and children, disproportionately affecting those in low-resource settings. To address these challenges in the high-mortality, peri-urban areas of Karachi, VITAL Pakistan Trust and Aga Khan University launched a digital health intervention (DHI) to stimulate demand for health services and streamline care management for health workers at the primary care level. In this case study, we present a narrative review of the design, development, and deployment of the DHI, an Android-based application, in accordance with the Principles for Digital Development. We draw on the initial experience with implementation to reflect on how each of the nine Principles was considered during different phases of the project lifecycle, focusing on the lessons learned and challenges encountered during this process. By engaging with end-users and understanding the community, we were able to map existing relationships and workflows onto a digital platform to address major challenges hindering service delivery. Leveraging insights from field observations and user feedback, we collaborated with experts in healthcare and technology to develop the DHI, which has now scaled to 44 peri-urban settlements in Karachi. Our experience underscores the value of substantiated frameworks like the Principles. However, on-ground challenges reveal important caveats requiring further assessment. These include building community trust in new digital systems and ensuring the ethical use of health data, particularly in low digital and data literacy contexts. Based on this understanding, we share recommendations for conditions central to the effective integration and uptake of technology in healthcare, specifically within the context of digital health for MNCH.

Abstract:Women from low- and middle-income countries face challenges in accessing and utilising quality healthcare. Technologies can aid in overcoming these challenges and the present scoping review is aimed at summarising the range of technologies used by women and assessing their role in enabling Indian women to learn about and access healthcare services. We conducted a comprehensive search from the date of inception of database till 2022 in PubMed and Google Scholar. Data was extracted from 43 studies and were thematically analysed. The range of technologies used by Indian women included integrated voice response system, short message services, audio–visual aids, telephone calls and mobile applications operated by health workers. Majority of the studies were community-based (79.1%), from five states (60.5%), done in rural settings (58.1%) and with interventional design (48.8%). Maternal and child health has been the major focus of studies, with lesser representation in domains of non-communicable and communicable diseases. The review also summarised barriers related to using technology – from health system and participant perspective. Technology-based interventions are enabling women to improve awareness about and accessibility to healthcare in India. Imparting digital literacy and scaling up technology use are potential solutions to scale-up healthcare access among women in India.

The overarching objective of this initiative is to bolster public health and foster heightened awareness regarding personal well-being. Frequently, individuals fail to retain ambulance contact information or remain unaware of the specific contact details of nearby hospitals, exacerbating emergency situations. To address this, the proposed application will not only provide users with access to nearby hospital information based on their current location but also facilitate direct calls to hospital ambulances. Furthermore, the ability to schedule appointments remotely will streamline the process, alleviating the inconvenience associated with traditional appointment booking methods, such as phone calls or direct interactions with administrative staff. Additionally, users will receive daily health tips and notifications, emphasizing preventive care and lifestyle adjustments over reliance on medication. Through integrated chat functionality, patients can consult with healthcare professionals remotely, eliminating the need for in-person visits for non-urgent inquiries. This innovative utilization of modern technology in healthcare delivery represents a significant leap forward, offering swift access to medical advice and support in a manner that transcends traditional clinic-based consultations, thereby enhancing overall healthcare accessibility and efficiency.
Exploring the System Determinants Associated with Senior Women’s Access to Medical Care in Rural Bangladesh

Abstract:Senior women's access to medical care in rural Bangladesh is a major public health concern. This study aims to explore the system determinants that impact on rural senior women's utilization of healthcare. Following a qualitative critical social research design, healthcare staff and senior women living in three rural villages of Bangladesh were approached to participate in face-to-face audio-recorded interviews. A total of 11 staff and 25 senior women were interviewed with questions about health policy, healthcare services and management of aged care. Data was analyzed using a blend of critical discourse and thematic analysis methods. Several healthcare system determinants were identified that were complex and cross-sectional. Three major themes emerged from the system determinants: legal framework of aged care; inadequate healthcare support; and professional knowledge and skills of healthcare staff that led the rural senior women to avoid or delay access to hospitals and clinics. The findings revealed that a lack of health focus and professional skills among healthcare staff can be considered as critical. This study recommends that policy and organizational changes are made to improve the women's access to rural hospitals and clinics in Bangladesh.

Abstract:We aim to investigate the obstacles faced by elderly indigenous individuals in the Chittagong Hill Tracts, Bangladesh when accessing healthcare services. A qualitative research approach was utilized, and data collection was carried out in three distinct regions of the aforementioned area. A total of 30 in-depth, semi-structured interviews and participant observations were conducted to achieve the research objectives. Thematic analysis utilizing both a deductive and inductive approach was employed to analyze the data. The Granheim method and Nvivo-12 software were utilized to process, analyze and code the data. The study's findings indicate that a lack of knowledge about healthcare needs, geographical barriers, poor financial conditions, higher cost of medical services, scarcity of hospitals nearby and communication barriers all contribute to inadequate access to healthcare services. By recognizing the factors that impede access to healthcare services in this region, this study offers valuable insight for policymakers and healthcare providers on how to enhance healthcare services for the indigenous population, especially the elderly. Furthermore, the government can adopt a more efficient approach to include these elderly individuals in various social safety net programs.
NGOs' initiatives and grassroots approach for accessing to health care services for the slum people in Dhaka

Abstract:Objective: This study holds significant importance as it aims to delve into the impactful NGOs' initiatives and grassroots approaches instrumental in providing healthcare services to Dhaka's underserved slum people. It focuses on understanding how these factors influence the use and access to health services, which is a crucial aspect for researchers, policymakers, and healthcare professionals. Study design: This study was meticulously designed, utilizing a comprehensive cross-sectional mixed-methods design. By incorporating qualitative and quantitative data collection methods, we ensured a thorough understanding of NGOs' initiatives and grassroots approaches to providing healthcare services to slum dwellers in Dhaka, thereby instilling confidence in the validity of our research for the audience. Methods: A face-to-face interview was used to survey the participants (n = 722) using semi-structured questionnaires, following a systematic sampling technique. Four focus group discussions (FGDs) were also conducted with the slum people. Binary logistic regression was performed to know NGOs' initiatives, roles, and grassroots approach as predictors or independent variables and healthcare services as an outcome or dependent variable. The quantitative data were analyzed using SPSS version 23.0. At the same time, thematic analysis was conducted following Philip Adu's Qualitative data analysis process and Braun and Clarke's six steps of the thematic analysis system, integrating the 11 subthemes with the quantitative findings to highlight the interpretative findings of the qualitative data. Findings: Major findings revealed that NGOs' initiative roles and grassroots approach had a significant impact on slum dwellers' use and access to healthcare services. The initiatives included affordable health services (OR = 22.86, 95% CI = 3.87, 35.00, P = 0.01), special health services (OR = 5.63, 95% CI = 3.36, 9.42, P = 0.00), engagement of responsible community leaders (OR = 1.72, 95% CI = 1.14, 2.59, P = 0.01), distribution of health and medicine items (OR = 1.92, 95% 2 CI = 1.40, 2.63, P = 0.01), provision of updated information to slum dwellers (OR = 1.37, 95% CI = .99, 1.90, P = 0.05), telehealth and telemedicine (OR = 1.82, 95% CI = 1.55, 2.13, P = 0.01), BCC strategy (OR = 1.26, 95% CI = 1.00, 1.57, P = 0.05), and doorstep services as NGOs' grassroots approach (OR = 1.84, 95% CI = 1.00, 3.38, P = 0.05). Qualitative findings supported the quantitative findings through 2 main themes and 11 sub-themes, which were integrated with quantitative findings to highlight the interpretative findings of qualitative data. Conclusions: Health services and other facilities for urban slum people through NGOs' initiatives and grassroots approaches are highly affordable and practical, special health services with the involvement of special exceptional health professionals, community supportive services, BCC strategies, and doorstep health services may trigger the use and access to health services for slum dwellers. Results suggest and recommend capitalizing and investing in such initiatives and grassroots approaches from the government, policymakers, and donors with NGOs to find accessible, affordable health services for the unprivileged slum people.

Abstract:The production protocol of many whole cell/virion vaccines involves an inactivation step with β-propiolactone (BPL). Despite the widespread use of BPL, its mechanism of action is poorly understood. Earlier work demonstrated that BPL alkylates nucleotide bases, but its interaction with proteins has not been studied in depth. In the present study we use ellipsometry to analyze the influence of BPL treatment of two H1N1 influenza strains, A/Brisbane/59/2007 and A/New Caledonia/20/1999, which are used for vaccine production on an industrial scale. Analyses were conducted using a mixed lipid monolayer containing ganglioside GM3, which functions as the viral receptor. Our results show that BPL treatment of both strains reduces viral affinity for the mixed monolayer and also diminishes the capacity of viral domains to self-assemble. In another series of experiments, the pH of the subphase was reduced from 7.4 to 5 to provoke the pH-induced conformational change of hemagglutinin, which occurs following endocytosis into the endosome. In the presence of the native virus the pH decrease caused a reduction in domain size, whereas lipid layer thickness and surface pressure were increased. These observations are consistent with a fusion of the viral membrane with the lipid monolayer. Importantly, this fusion was not observed with adsorbed inactivated virus, which indicates that BPL treatment inhibits the first step of virus-membrane fusion. Our data also indicate that BPL chemically modifies hemagglutinin, which mediates the interaction with GM3.

Abstract:Bangladesh is a rapidly developing South Asian country with a massive population. Around 70% of Bangladesh’s population lives in rural regions, making immediate access to healthcare extremely challenging. Doctors and healthcare-related services are in short supply in Bangladesh’s rural areas. For which e-Health service is very important in Bangladesh especially in rural parts of the nation. As a result, endeavors are being made to improve e-health services in Bangladesh step by step and also e-health services are being given through different portable applications. The primary purpose of this study is to examine the current condition of e-health services in Bangladesh and to identify the issues that it faces. To accomplish this goal, the main challenges of e-health services are identified, and arrangements of these issues are contemplated. An online-based survey has also been conducted to identify the current situation of e-health in Bangladesh. The aftereffects of the survey show that 70% of people know about e-Health services, 55.9% of people utilize this service regularly, and currently, 67.4% of people are satisfied with the services of e-health organizations in Bangladesh. Because of that a few challenges of e-Health services and exhortation to overcome these difficulties are given in this paper.

Abstract:The main goal of this paper is to make a multifunctional robot along with a database management system named “MEDBOT”. That will reduce the problem of spreading any viral or infectious disease by maintaining social distance. A robot is designed and implemented will replace the ordinary system in the medical sector in a digital way with fewer life risks. Being a multifunctional robot, it can perform as a receptionist, entertainer, nurse, cleaner, and deliveryman. As a nurse, it will check a patient’s temperature, Heart Rate, Oxygen Saturation, and Heart condition in a contactless way. Furthermore, these vitals are easily stored in its database system. These vitals are broadcast on the device’s display and server, where the doctor can quickly monitor them in real-time. A mobile application-based remote-control system is equipped in this robot to control it remotely. Moreover, a disinfecting system and UV-C light are used for sterilizing purposes. The previously invented medical robot can perform one or two tasks, but a revolutionary change will come into the medical sector by making this robot. At last, it can be said that the ‘Medbot’ can reduce infectious disease, save time moreover digitalize the medical industry by doing these works. Finally, building a digital Bangladesh, it will work along with the medical sector as a warrior.
