Abstract:Abstract Background Urolithiasis is considered one of the most common diseases in urological practice. Its prevalence is about 1% to 15% with 30 years old as the peak age of incidence. Medical expulsive therapy (MET) has been used as a conservative treatment for patients with ureteral stones. Nitrergic fibers have been identified to have a relaxant effect on the distal ureteral smooth musculature. The objective of our study was to evaluate the efficacy and safety of the combination of silodosin and vardenafil as a medical expulsive therapy in comparison with each drug alone. Methods One hundred and two male patients with uncomplicated distal ureteric stone 6–10 mm were enrolled in the study. The patients were randomly divided into 3 equal groups, and each one consists of 34 patients. Group I received silodosin 8 mg once daily, group II vardenafil 5 mg once daily and group III combination of silodosin 8 mg and vardenafil 5 mg once daily. The treatment was given for all the patients until stone expulsion or a maximum of 4 weeks. The primary endpoint was the stone expulsion rate, and the secondary endpoints were time to stone expulsion, number of hospital visits for pain, amount of analgesic required and side effects associated with MET. Results Our study showed that the stone expulsion rate was higher in combination = 90.0% than silodosin = 76.7% and vardenafil groups = 60.0% ( P = 0.025), the time to stone expulsion was significantly shorter in combination = 11.23 ± 3.14 than silodosin = 12.50 ± 1.66 and vardenafil groups 14.67 ± 1.24 days ( P < 0.01), the number of hospital visits for pain was statistically significant between the three groups (silodosin 1.35 ± 0.9, vardenafil 1.65 ± 1.09 and combination groups 1.02 ± 0.80) ( P = 0.038) and lesser amount of analgesic required in combination 313.6 ± 2.85.5 than silodosin 613.44 ± 483.62 and vardenafil groups 716.97 ± 685.3 ( P = 0.008). There was no significant difference among the studied groups as regards the drugs side effects except for retrograde ejaculation (silodosin and combination = 86.7% vs vardenafil groups = 0.0%) ( P < 0.05) and increased erection (combination = 26.7%, vardenafil = 23.3% and silodosin groups = 0%) ( P = 0.010). Conclusion The prescription of vardenafil in combination with silodosin is safe and more effective than silodosin or vardenafil alone as a MET.
Comparative Study between Silodosin and Tamsulosin in Expectant Therapy of Distal Ureteral Stones

Abstract:OBJECTIVE: To compare the efficacy and safety of silodosin against tamsulosin as medical expulsion therapeutic agent in stone lower 1/3rd ureter.PATIENTS AND METHODS: One hundred fifty patients divided equally into 2 groups I and II received silodosin 8 mg and tamsulosin 0.4 mg respectively. Patients aged 18 years or older having single unilateral stone 10 mm or less were included in the study. Patients with bilateral or multiple stones, marked hydronephrosis, previous open or endoscopic surgery and having urinary infection were excluded. Patients were followed weekly for 4 weeks by ultrasonography, plain radiography of the urinary tract and CT of the urinary tract when indicated.RESULTS: Silodosin showed better results against tamsulosin as stone expulsion rate in silodosin and tamsulosin groups was 82.4 and 61.5% respectively with significant difference (p = 0.007). Also, the stone expulsion time was significantly lower in silodosin against tamsulosin groups as it was 9.4 ± 3.8 vs. 12.7 ± 5.1 days in group I and II respectively (p = 0.001). The adverse effects were comparable with non-significant more retrograde ejaculation in the silodosin group.CONCLUSION: Silodosin showed better efficacy in the stone expulsion rate and time with comparable safety of both drugs, with nonsignificant more retrograde ejaculation in silodosin.

A comparison of tamsulosin and silodosin versus tadalafil in medical expulsive therapy for distal ureteral stone in men: a prospective study

Abstract:Objectives: To evaluate the efficacy of tamsulosin, silodosin, and tadalafil as medical expulsive therapy for distal ureteral stones in men. Methods: Adult males meeting the inclusion criteria were randomized into one of four treatment arms - dexketoprofen (control group), tamsulosin (Group 1), silodosin (Group 2), and tadalafil (Group 3). The stone expulsion rate after four weeks represented the primary endpoint, while the stone expulsion rate at the end of the study and the incidence of adverse events constituted secondary endpoints. Clinical findings were compared between all four drug groups. Results: No statistically significant difference was observed among the groups in terms of age, body mass index, stone futures, expulsion time, pain episodes, or total analgesic use. Expulsion rates in the fourth week were 42.5%, 80%, 82.5%, and 75%, respectively. The stone expulsion rates in groups 1, 2, and 3 were significantly higher than in the control group (p < 0.001). However, the differences between groups 1, 2, and 3 were not statistically significant. No serious adverse effects were observed during the study period. Conclusion: The study results showed a higher expulsion rate in male patients using tamsulosin, silodosin and tadalafil for distal ureteral stones, but no significant superiority between these. All three are safe, effective, and well-tolerated, causing minimal side-effects.
Evaluation of the efficacy and safety of either or both mirabegron and silodosin, as a medical expulsive therapy for distal ureteric stones

Abstract:Objective: To evaluate efficacy and safety of either or both silodosin and mirabegron as MET for distal ureteric stones ≤ 10 mm. Patients and methods: This study enrolled a total of 105 patients, aged between 20 and 56 years, diagnosed by single radiopaque distal ureteral stone measuring ≤ 10 mm. The recruitment period spanned from May 2020 to December 2021. The patients were randomly divided into three groups, with each group consisting of 35 participants. Group A received a once-daily dose of 8 mg of silodosin, group B received a once-daily dose of 50 mg of mirabegron, and group C received a combination of both medications. Treatment was administered to all patients until the stone was expelled or for a maximum duration of four weeks. The stone-free rate was determined by analyzing KUB films with or without ultrasonography. Results: The rate of stone expulsion was significantly higher in group C compared to groups A and B (P = 0.04 and P = 0.004, respectively). The mean (standard deviation) time for stone expulsion in groups A, B, and C was 14 ± 2.3 days, 11 ± 3.1 days, and 7 ± 2.2 days, respectively. Group C demonstrated a significantly shorter stone expulsion time compared to groups A and B (P = 0.001 and P = 0.04, respectively). The frequency of renal colic in group C was significantly lower than that in groups A and B, resulting in a reduced requirement for analgesics (P < 0.05). Anejaculation occurred at a significantly higher rate in the silodosin group (73.9%) and combination group (84%) compared to the mirabegron group (P < 0.05). Conclusions: The findings of this study suggest that both silodosin and mirabegron are effective treatments for the expulsion of lower ureteric stones. Furthermore, the combination of these medications leads to an increased rate of stone expulsion and a reduced duration of expulsion.
Comparison of Synergistic Action of Alpha Blockers and Tadalafil in the Management of Lower Ureteric Stones as Medical Expulsive Therapy: A Prospective Cohort Study

Abstract:Introduction: Medical Expulsive Therapy (MET) involves the utilisation of different drugs that act on the ureter through various mechanisms. Alpha-1 adrenoceptor and Phosphodiesterase (PDE) regulate ureteric motility thus combination of these drugs can increase the ureteric stone expulsion rate by complementing each other’s actions. Aim: To assess the synergistic role of alpha-blocker and Tadalafil therapy in facilitating the spontaneous expulsion of distal ureteral stones. Materials and Methods: A prospective cohort study was conducted in the Department of Urology at a Tertiary Care Centre (NSCB Medical College) from April 2020 to January 2022. A total of 281 patients diagnosed with lower ureteric stones (5-10 mm) were divided into five study groups: Group A was treated with tamsulosin, group B with silodosin, group C with tadalafil alone, group D with tamsulosin and tadalafil, and group E treated with a combination of silodosin and tadalafil. Corticosteroid (deflazacort 6 mg) was also included in every group. All patients were reassessed after three weeks of treatment for stone expulsion rate, expulsion time, the number of hospital visits for pain, and adverse effects of drugs. The statistical data was analysed using Statistical Package for Social Sciences (SPSS) software (version 21.0, IBM Corp, USA). The Chi-square test and Analysis of Variance (ANOVA) test were used to determine the effect on stone expulsion rate and expulsion time. The confidence interval was 95%, and the significance level of the p-value was set <0.05. Results: The stone expulsion rates in group A, B, C, D, and E were 70.91%, 79.63%, 52.63%, 84.21%, and 86.20%, respectively, which was significant (p=0.00194). The mean time taken for stone expulsion in group A, B, C, D, and E were 8.95±1.73, 8.43±1.57, 9.86±1.90, 7.96±2.03, and 7.75±1.84 days (p=0.0001). Minor side effects were not significant, except for retrograde ejaculation in group B and E, and 22.8% of patients needed hospitalisation in group C (tadalafil alone). Conclusion: Combination therapy is safe, efficacious, and welltolerated as MET for distal ureteric calculi in the 5-10 mm range, thereby avoiding surgical procedures and providing faster relief for the patients.

Abstract:Introduction: If surgical intervention is not indicated, current international guidelines and evidences recommend medical expulsive therapy (MET) involving the administration of drugs to improve spontaneous stone passage. Medical expulsive therapy has now become an established modality of treatment, and employs the use of various drugs that act on the ureter with different mechanisms. It is hypothesised that elevation of cyclic guanosine monophosphate in ureteral smooth muscle by phosphodiesterase type 5 (PDE5) inhibitors may result in ureteral relaxation and increased stone clearance. Aims/ objective: To investigate whether we can achieve better ureteric relaxation and reduction in intramural pressure in order to facilitate stone passage by tamsulosin plus tadalafil combination therapy. Materials and Method: Stone expulsion rate of tamsulosin + tadalafil when compared to tamsulosin was our primary endpoint. (Assessed by subjective response and confirmed by repeat plain X ray abdomen and if needed CT scan abdomen). The efficacy of the individual drugs was analysed and compared with the help of Pearson chi square test. p value <0.05 were considered as statistically significant. Results: 33 patients (66%) receiving tamsulosin and 43 patients (86%) receiving combination therapy showed positive outcome (expulsion of stones). So, tamsulosin plus tadalafil combination therapy had significantly better efficacy. In tamsulosin group 30 patients among 33 (90.9%) expelled the stone in less than 5 days. In tamsulosin plus tadalafil group, 37 patients among 43 (86%) expelled in less than 5 days. But the difference was not statistically significant. Both drugs were well tolerated by the study patients. Conclusion: Tamsulosin plus tadalafil is more efficacious, and well tolerated as medical expulsive therapy for distal ureteric stones compared to tamsulosin. Medical expulsive therapy also doesn’t carry post-operative morbidities as compared to surgical methods.
A comparative study of two means of simultaneous treatment for benign prostate hyperplasia and bladder stone

Abstract:Objective To compare the safety and the clinical efficacy of two means of simultaneous treatment for benign prostate hyperplasia (BPH) and bladder stone. Methods Sixty-three cases of BPH and bladder stones were randomly divided into 2 groups: they were treated by combining mini-incisions cystolithotomy (group A) ,transurethral eletroresection with holmium laser (group B). TURP followed after the removal of stone. The removal of stone time, bladder irrigation time, catheterization time, hospital stay , hospital expenses and therapeutic effects were surveyed. Results Compared with group B, the removal of stone time, hospital cost were significantly superiority in group A(t=2.89,17.87, P<0.05).But compared with group A, the bladder irrigation time, catheterization time, hospital stay were significantly superiority in group B(t=2.21,8.04,2.19, P<0.05). The procedures were successful in all cases, a follow up of 3 to 35 months found a good outcome in both groups. All the patients had normal micturation after the urethral catheter was withdrawn and no severe complication. IPSS, Qmax and RUV were significantly improved in both groups. Conclusion The results showed that the two therapies of BPH with bladder stones were safe and effective.
Effect of Alpha-Blocker on Distal Ureteric Calculi: A Comparative Study

Abstract:Introduction Silodosin is a recently introduced selective alpha- blocker in Medical Expulsive Therapy for the management of distal ureteric calculi. The efficacy and safety of silodosin compared to tamsulosin are controversial, however the former is considered to be more effective. The objective of the study is to assess the efficacy and safety of silodosin compared to tamsulosin in the treatment of ureteral stones less than 10mm.  Methods This prospective randomized study was conducted in the Department of General Surgery, Navodaya Medical College, Raichur, Karnataka. Sixty patients were included in the study who presented with abdominal or loin pain and were diagnosed with unilateral solitary distal ureteric stone measuring <10mm with no complications. Participants in Group I received Tablet Silodosin 8mg OD until the passage of stone or up to two weeks, and Group II received Tablet Tamsulosin 0.4mg OD until the passage of stone or up to two weeks. Analgesic Tablet Diclofenac sodium 50mg was given to both patients on demand.  Results A total of 60 patients were included in the study, divided into 30 patients in each group of Silodosin and tamsulosin respectively. In Group I (silodosin), out of 30 patients, 25 patients expelled the calculus, whereas in Group II (tamsulosin), out of 30 patients, 15 patients expelled the calculus. Group I showed a significantly higher rate of expulsion, with a p-value of 0.005. Stone expulsion time was notably shorter in Group I compared to Group II, with averages of 4.73 days versus 6.33 days (p=0.009). Additionally, analgesic use during the medical expulsive therapy was lower in Group I, averaging 8.25 compared to 4.13 (p=0.0001) in the tamsulosin group. Significant differences were observed in the outcomes such as stone expulsion time, and analgesic requirement. However, no significant differences were found when comparing the groups based on age, gender, stone size, and side of the ureter involved. The 20 patients who failed the medical management underwent ureterorenoscopy with 100% clearance. 3 patients in Group II experienced orthostatic hypotension, and zero patients experienced any side effects in Group I. Compared to tamsulosin, silodosin provides significantly higher stone expulsion rates, fewer colic episodes, and shorter stone expulsion duration than tamsulosin.

Abstract:Rat lung slices incubated in a Krebs ringer bicarbonate buffer respired at a rate of 0.62 mumoles/g lung X min. Respiration was inhibited by cyanide and stimulated by dinitrophenol. PET was insensitive to the action of pyrazole, cyanide or azide. Lung microsomes metabolized ethanol at rates which were less than 1% of PET. They showed an apparent Km for ethanol of the order of 10 mM and a Vmax of about 7 nmoles acetaldehyde/mg prot X min. Oxidation of ethanol by lung slices to acetaldehyde and CO2 was less than 0.3% and 1.15% than PET, respectively. Chromatographic determination of the products of ethanol metabolism by lung slices showed that a metabolite similar to glucuronic acid was formed. It is concluded that PET is a nonoxidative process which from the evidence presented in this work, would seem to involve the formation of ethyl glucuronide.
Medical Expulsive Therapy for Ureteral Calculi with Tamsulosin: Evidence from A Meta-analysis of Randomized Controlled Trials

Abstract:Objective To assess the efficacy of medical expulsive therapy for ureteral calcul i with tamsulosin. Methods We searched PubMed,MEDLINE,EMBASE, BIOSIS,International Pharmaceutical Abstracts(IPA)Database, The Cochrane Library and Chinese Journal Full-text Database from 1995 to September 2006,as well as the proceedings of urological scientificconferences from 2000 to 2006.Randomized controlled trials(RCTs)comparing tamsulosin and other therapies for ureteral calculi among adults were included.Data were extracted by two reviewers independently and synthesized by STATA 9.0 software.Results A total of 16 studies involving 1 521 patients with distal or juxtavesical ureteral calcul i were included.Compared with conservative therapy,tamsulosin showed higher expulsion rate[RR 1.50,95%CI(1.20 to 1.87), P0.000 1],shorter expulsion time[SMD–1.29,95%CI(–2.27, –0.31)]and fewer patients requiring ESWL or ureteroscopy[RR 0.40,95%CI(0.27,0.59),P0.05].Compared with conservative therapy,the combination of tamsulosin plus deflazacort also showed higher expulsion rate[RR 1.59,95%CI(1.31,1.93)], shorter expulsion time[SMD–0.8,95%CI(–1.18,–0.42)] and fewer patients requiring ESWL or ureteroscopy[RR 0.13,95%CI(0.06,0.31),P0.05].Compared with deflazacort alone, the combination of tamsulosin plus deflazacort demonstrated similar expulsion rate[RR 1.31,95%CI(0.78,2.23),P=0.31], but significantly reduced the dosage of analgesics[SMD 15.20,95%CI(14.98,15.52)]and decreased the proportion of patients requiring ESWL or ureteroscopy[RR 0.09,, 95%CI(0.02,0.47),P0.05].Compared with deflazacort plus nifedi pine,the combination of tamsulosin plus deflazacort showed higher expulsion rate[RR 1.20,95%CI(1.07,1.35), P=0.002],but similar expulsion time[SMD–1.34,95%CI(– 3.47,0.79)]and proportion of patients requiring ESWL or ureteroscopy[RR 0.34,95%CI(0.05,2.22),P0.05].As for side effects,tamsulosin-based treatment and conservative therapy were comparable(P0.05).Conclusions Tamsulosin has a beneficial effect on the expulsion of ureteral calculi,especially for distal and juxtavesical ureteral calcul i.Tamsulosin- based medical expulsive therapy at the dosage of 0.4mg daily is effective and safe for patients with distal ureteral calcul i. More large-scale studies are required to define the efficacy of combination therapy of tamsulosin plus deflazacort.
Efficacy of tamsulosin versus tadalafil as medical expulsive therapy on stone expulsion in patients with distal ureteral stones: A randomized double-blind clinical trial

Abstract:PURPOSE: To compare the effects of tadalafil, tamsulosin, and placebo as a medical expulsive therapy (MET) for distal ureteral calculi.MATERIALS AND METHODS: This prospective randomized double-blind clinical trial was conducted on 132 renal colic patients with distal ureteric stones (≤10mm) over a period of 12 months. Patients were randomly divided into three groups. Patients in group A received tamsulosin 0.4mg, in group B received tadalafil 10mg, and in group C received placebo. Therapy was given for a maximum of 4 weeks. The rate of stone expulsion, duration of stone expulsion, the dose and the duration of nonsteroidal anti-inflammatory drugs (NSAIDs), analgesic use, and adverse effects of drugs were recorded.RESULTS: Demographic profiles were comparable between the 3 groups. Although the stone expulsion rate in group A (72.7%) was higher in comparison to group B(63.6%) and group C(56.8%), it was not considered statistically significant (P=0.294). Shorter mean time to stone expulsion was significantly observed in group A (17.75±75), than group B(21.13±1.17) and group C(22.25±1.18) (P=0.47). The mean number of analgesic use was 9.8±5.09 days in group A, 14.6±7.9 days in group B, and 12.6±22.25 days in group C, this difference was significant (P=0.004). The analgesic requirement (doses of NSAIDs and pethidine) in group A was significantly lower than other groups (P< 0.05). Also, patients in group A reported fewer headaches compared to other groups (P=0.011).CONCLUSION: Tamsulosin as medical expulsive therapy is more effective for distal ureteric stones with less need for analgesics and less stone expulsion time than tadalafil.

Abstract:OBJECTIVE: To assess the efficacy and safety of tadalafil in facilitating the spontaneous passage of distal ureteral stones.METHODS: The relevant studies were identified by searching MEDLINE, EMBASE, and the Cochrane Central Register of Controlled Trials up to November 2016. Randomized controlled trials evaluating effects of tadalafil for distal ureteral stones were included.RESULTS: Six publications involving 921 patients were included. Compared with tamsulosin monotherapy, tadalafil monotherapy or combined with tamsulosin has a significantly higher stone expulsion rate (relative risk [RR], 1.16; 95% confidence interval [CI], 1.05 to 1.29; p = 0.004; RR, 1.24; 95% CI, 1.09 to 1.42; p = 0.001, respectively) and shorter time to stone expulsion (mean difference [MD], -1.33 days; 95% CI, -2.44 to -0.23; p = 0.02; MD, -1.98 days; 95% CI, -3.08 to -0.88; p = 0.0004, respectively). Statistically significant differences were noted in pain episodes and analgesic use in favor of group tadalafil alone compared to group tamsulosin alone. The analgesic use was significantly lower in the combined group than in the tamsulosin alone group. Although the occurrence of drug-related adverse events in the tadalafil alone or combined with tamsulosin was higher than that in the use of tamsulosin-alone group, the most common adverse events were mild and tolerable.CONCLUSIONS: Our study suggested that medical expulsive therapy for the distal ureteral stones using tadalafil alone or combined with tamsulosin is safe, efficacious, and well tolerated.

Abstract:Objective: To evaluate the efficacy and safety of add-on therapy with the phosphodiesterase type 5 inhibitor tadalafil for patients with lower urinary tract symptoms secondary to benign prostatic hyperplasia (LUTS/BPH) treated with the α1-adrenoceptor blocker silodosin. Materials and methods: We analyzed 103 patients with LUTS/BPH with an International Prostate Symptom Score (IPSS) of >8 after ≥4 weeks of silodosin treatment from April 2016 through December 2016 at Kori Hospital. The patients subsequently received silodosin 4.0 mg twice daily (monotherapy group) or silodosin 4.0 mg twice daily plus tadalafil 5.0 mg once daily (add-on therapy group) for 8 weeks. We assessed adverse events and evaluated the mean change from baseline to 8 weeks in the IPSS, Overactive Bladder Symptom Score (OABSS), maximum urine flow rate (Qmax), and post-void residual urine volume. Results: Of 103 patients, 101 (98.1%) could continue medical treatment. The IPSS, OABSS, and Qmax showed significantly greater improvement in the add-on therapy than in the monotherapy group (-3.92 vs -1.24, -1.18 vs 0.10, and 1.09 vs -1.04, respectively; all P <.05). Although 4 patients experienced adverse events (add-on therapy: n = 3, 5.7%; monotherapy: n = 1, 2.0%), no significant differences were observed (P = .62). Among patients with overactive bladder (n = 55), the IPSS storage symptom subscore, IPSS urgency subscore, and OABSS urgency subscore showed significantly greater improvement in the add-on therapy than in the monotherapy group (-2.23 vs 0.17, -0.88 vs 0.28, and -1.5 vs -0.48, respectively; all P <.05). Conclusion: Add-on therapy with tadalafil may be effective for patients with LUTS/BPH resistant to silodosin monotherapy.
Evolving Role of Silodosin for the Treatment of Urological Disorders – A Narrative Review

Abstract:Abstract Use of α-androgenic receptor blockers remains a mainstay therapeutic approach for the treatment of urological diseases. Silodosin is recommended over other α-blockers for the treatment of lower urinary tract symptoms (LUTS) and benign prostate hyperplasia (BPH), due to its high α1A uroselectivity. Current research data suggest that silodosin is efficacious in the management of various urological diseases. Thus, we herein review the current evidence of silodosin related to its efficacy and tolerability and appraise the available literature that might ultimately aid in management of various urological conditions at routine clinical practice. Literature reveals that silodosin is beneficial in improving nocturia events related to LUTS/BPH. Silodosin exerts effect on relaxing muscles involved in detrusor obstruction, therefore prolonging the need for patients undergoing invasive surgery. Silodosin treatment, either as a monotherapy or combination, significantly improves International Prostate Symptom Score (IPSS) including both storage and voiding symptoms in patients with BPH/LUTS. Patients on other treatment therapies such as phosphodiesterase 5 inhibitors or other α-blockers are well managed with this drug. Steadily, silodosin has proved beneficial in the treatment of other urological disorders such as chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS), overactive bladder/acute urinary retention (AUR), premature ejaculation (PE), and prostate cancer post brachytherapy-induced progression. In patients with distal ureteral stones, silodosin treatment is beneficial in decreasing stone expulsion time without affecting stone expulsion rate or analgesic need. Moreover, there were significant improvements in intravaginal ejaculation latency time, quality of life scores, and decrease in PE profile among patients with PE. Silodosin has also demonstrated promising results in increasing the likelihood of successful trial without catheter in patients with AUR and those taking antihypertensive drugs. Reports from Phase II studies have shown promising role of silodosin in the treatment of CP/CPPS as well as facilitating ureteral stone passage. From the robust data in this review, further silodosin treatment strategies in the management of different urological conditions need to be focused on.

Abstract:Fresh human cadaveric specimens of occiput (C0) to C3 were subjected to 1.5 nm of flexion, extension and bilateral bending and axial torque. The resulting physiological motions were studied in an unconstrained three-dimensional manner. The effects of sequential transections of the left and right alar ligaments on the relative motion of C0-1 and C1-2 were studied. After transection of the left alar ligament, the percentage increases in neutral zones (NZ) and ranges of motion (ROM) were documented at both the C0-1 and C1-2 joints. In the sagittal plane, the most increase was at C1-2 due to the flexion moment, e.g., 47.4% in NZ and 27.6% in ROM. In lateral bending, the left alar transsection resulted in mostly right lateral bending increases and at the C0-1 joint, 37.1% in NZ and 19.6% in ROM. In axial rotation, changes in the total motion of the C0-2 joint complex were more reliable indicators. For left alar transsection, most increases were in right axial rotation, e.g., 25.6% for right rotation versus 11.2% for left rotation in the NZ parameter. Functional loss of the alar ligaments indicates a potential for instability which, however, must be determined in conjunction with other clinical findings, such as neurological dysfunction, pain and deformity.

Abstract:Introduction: Urinary stones, also known as calculi, are a common urological problem affecting millions of people worldwide. Medical expulsion therapy (MET) is a commonly used treatment for lower ureteral calculi. Objectives: The main objective of the study is to find the role of silodosin as a superior substitute for tamsulosin in medical expulsion therapy for patients with lower ureteral calculi. Material and methods: The study design for this research article would be a randomized controlled trial comparing the efficacy and safety of silodosin and tamsulosin in medical expulsion therapy (MET) for patients with lower ureteral calculi. Patients with lower ureteral calculi who meet the inclusion criteria will be recruited for the study. Inclusion criteria may include age over 18 years, presence of lower ureteral calculus confirmed by imaging, and willingness to participate in the study. Exclusion criteria may include previous history of ureteral surgery, contraindication to alpha-blockers, and pregnancy. Results: A total of 100 participants were randomized to receive either silodosin (n=50) or tamsulosin (n=50) for medical expulsion therapy of lower ureteral calculi. The mean age of the participants was 45 years, and 60% were male. The stone expulsion rate at 4 weeks was 80% in the silodosin group and 75% in the tamsulosin group. There was no statistically significant difference between the two groups (p=0.54). Conclusion: In conclusion, the results of this study indicate that silodosin may not be a superior substitute for tamsulosin in medical expulsion therapy for patients with lower ureteral calculi. Both drugs demonstrated similar rates of stone expulsion and time to stone passage, as well as comparable safety profiles and patient satisfaction rates. Abstract:Objective: Urolithiasis is a common urological diseases and affects the daily life of patients. Medical expulsive therapy has become acceptable for many parents. We conducted a meta-analysis to determine the efficacy and safety of tadalafil compared with tamsulosin for treating distal ureteral stones less than 10 mm in length. Methods: Related studies were identified via searches of the PubMed, Embase, and Cochrane Library databases. All the articles that described the use of tadalafil and tamsulosin for treating distal ureteral stones were collected. Results: A total of 14 studies were included in our meta-analysis. Our results revealed that tadalafil enhanced expulsion rate [odds ratio (OR) = 0.68, 95% confidence interval (CI): 0.47 to 0.98, p = 0.04]; reduced expulsion time [mean difference (MD) = 1.22, 95% CI (0.13, 2.30), p = 0.03]; lowered analgesia use [MD = 38.66, 95% CI (7.56, 69.77), p = 0.01] and hospital visits [MD = 0.14, 95% CI (0.06, 0.22), p = 0.0006]. According to our subgroup analysis, either tadalafil 5 mg or 10 mg did not promote expulsion rate and accelerate expulsion time compared with tamsulosin. But patients receiving 5 mg tadalafil decreased analgesia usage [MD = 101.04, 95% CI (67.56, 134.01), p < 0.00001]. Conclusion: Compared with tamsulosin, tadalafil demonstrates a higher expulsion rate and less expulsion time for patients with distal ureteral stones less than 10 mm with a favorable safety profile.

Abstract:Objective: This study aimed to compare the efficacy of tamsulosin and silodosin as medical expulsive therapy in patients with symptomatic uncomplicated distal ureteric stones. Material and Methods: The data of adult patients who had distal ureteric stones in size between 4 and 10 mm and were treated with medical expulsive therapy between June 2019 and January 2022 were retrospectively documented. Patients were divided into two groups. Patients in Group 1 received silodosin 4 mg, and Group 2 received tamsulosin 0.4 mg. Therapy was given for a maximum of 3 weeks. Stone expulsion rate, time to stone expulsion, stone burden, and stone size were recorded. The efficacy of tamsulosin and silodosin as adjunctive medical therapy was determined. Results: A total of 152 patients were included in the study. Demographic profiles were comparable between the 2 groups. 116 (76.3%) patients were stone-free at the end of the follow-up. The stone expulsion rate was calculated in 47 patients (73.4%) in Group 1, and 69 patients (78.4%) in Group 2 (P = 0.477). The distance of the stone to the ureterovesical junction was significantly associated with successful stone expulsion in multivariate analysis (P=0.032). Conclusion: There was no significant superiority between tamsulosin and silodosin as medical expulsive therapy for distal ureteral stones. The distance of the stone to the ureterovesical junction was the only independent predictor of stone expulsion in multivariate analysis. Keywords: Medical expulsive therapy,
