Abstract:With many variables to adjust, conventional manual forward planning for Gamma Knife (GK) radiosurgery is very complicated and cumbersome. The resulting plan quality heavily depends on planners skills, experiences and devoted efforts, and varies significantly among cases, planners, and institutions. Quality control for GK planning is desired to consistently provide high-quality plan to each patient. In this study, we proposed a quality control method for GK planning by building a database of high-quality GK plans. Patient anatomy was described by target volume, target shape complexity, and spatial relationship between target and nearby organs, which determine GK planning difficulty level. Plan quality was evaluated using target coverage, selectivity, intermediate dose spillage, maximum dose to 0.1 cc of brainstem, mean dose of ipsilateral cochlea, and beam-on time. When a new plan is created, a high-quality plan that has the most similar target volume size and shape complexity will be identified from the database. A model has also been built to predict the dose to brainstem and cochlea based on their overlap volume histograms. The identified reference plan and the predicted organ dose will help planners to make quality control decisions accordingly. To validate this method, we have built a database for vestibular schwannoma, which are considered to be challenging for GK planning due to the irregularly-shaped target and its proximity to brainstem and cochlea. Five cases were tested, among which one case was considered to be of high quality and four cases had a lower plan quality than prediction. These four cases were replanned and got substantially improved. Our results have demonstrated the efficacy of our proposed quality control method. This method may also be used as a plan quality prediction method to facilitate the development of automatic treatment planning for GK radiosurgery.

The isocenters selected from last round and their neighbors on a finer resolution were used as new isocenter candidates for next round of optimization. After plan optimization, shot sequencing was performed to group the optimized sectors to deliverable shots supported by GK treatment units. We have tested our algorithm on 6 GK cases previously treated in our institution (2 meningioma cases, 3 cases with single metastasis and 1 case with 6 metastases). Compared with manual planning, achieving same coverage and similar selectivity, our algorithm improved the gradient index averagely from 3.1 to 2.9 and reduced the maximum dose of brainstem from 8.0Gy to 5.6Gy. The average beam-on time was also reduced by from 103.8 mins to 87.4 mins. A linear programming approach to inverse planning in Gamma Knife radiosurgery

Abstract:Leksell Gamma Knife is a stereotactic radiosurgery system that allows fine-grained control of the delivered dose distribution. We describe a new inverse planning approach that both resolves shortcomings of earlier approaches and unlocks new capabilities. We fix the isocenter positions and perform sector-duration optimization using linear programming, and study the effect of beam-on time penalization on the trade-off between beam-on time and plan quality. We also describe two techniques that reduce the problem size and thus further reduce the solution time: dualization and representative subsampling. The beam-on time penalization reduces the beam-on time by a factor 2-3 compared with the naive alternative. Dualization and representative subsampling each leads to optimization time-savings by a factor 5-20. Overall, we find in a comparison with 75 clinical plans that we can always find plans with similar coverage and better selectivity and beam-on time. In 44 of these, we can even find a plan that also has better gradient index. On a standard GammaPlan workstation, the optimization times ranged from 2.3 to 26 s with a median time of 5.7 s. In conclusion, we present a combination of techniques that enables sector-duration optimization in a clinically feasible time frame.

Abstract:AIM To investigate the effects of 2-(8-hydroxy-6-methoxy-1-oxo-1H-2-benzopyran-3-yl) propionic acid (NM-3) alone and in combination with carboplatin on tumor growth and apoptosis in mouse models of human gastric cancer constructed by subcutaneous implantation of histologically intact tumor tissue. METHODS Human gastric cancer SGC-7901 tissues were implanted into the dorsal subcutis of nude mice. One week after tumors reached to a volume of 50-100 mm(3) for around 1 wk, these mice were randomly divided into 8 groups (n = 10). NM-3 was injected peritoneally at the dose of 10 mg/kg, 20 mg/kg or 40 mg/kg every other day for 5 wk, combined with carboplatin (5 mg/kg) every third day for 4 wk. As controls of combined treatment, another 4 groups of mice were injected with either NM-3 at 10 mg/kg, 20 mg/kg or 40 mg/kg, or with carboplatin alone (5 mg/kg). The control mice received normal saline. Tumor weight, tumor growth inhibition (TGI), and intratumoral microvessel density (MVD) were evaluated. Apoptosis of human gastric cancer was detected by TUNEL method and flow cytometry analysis, respectively. RESULTS The mean tumor volume (692.40 +/- 58.43 mm(3), 548.30 +/- 66.02 mm(3), 382.13 +/- 43.52 mm(3)) after treatment with carboplatin combined NM-3 at the dose of 10 mg/kg, 20 mg/kg or 40 mg/kg was lower than that after treatment with either NM-3 at the dose of 10 mg/kg, 20 mg/kg or 40 mg/kg or with carboplatin alone. Compared with the normal saline group, NM-3 administered at 10 mg/kg, 20 mg/kg or 40 mg/kg significantly reduced the tumor weight in these groups (P < 0.05). Carboplatin used alone at 5 mg/kg showed minimal effects. But NM-3 in combination with carboplatin had greater effects of tumor weight than either NM-3 or carboplatin alone. NM-3 alone at the dose 10 mg/kg or in combination with carboplatin had no obvious effects on body changes. Two mice died of diarrhea in each of the two groups treated with 40 mg/kg NM-3 or with 40 mg/kg NM-3 in combination with carboplatin. A significant increase in apoptosis was observed in the NM-3 treated groups, and the effect was more significant in the groups treated with carboplatin in combination with NM-3 at 10 mg/kg, 20 mg/kg and 40 mg/kg, than in the control group. The induction of apoptosis was positively associated with the dose of NM-3. NM-3 significantly reduced the neo-microvascular formation of gastric cancer. The MVD was lower in the groups treated with NM-3 or with NM-3 in combination with carboplatin than in the group treated with carboplatin or in the normal saline group (P < 0.05). CONCLUSION The results suggest that the inhibitory effect of NM-3 on gastric cancer growth is mediated through decreased angiogenesis and the increased induction of apoptosis. Furthermore, NM-3 alone at the dose of 10 mg/kg or in combination with carboplatin has no obvious effects on body changes, indicating that NM-3 in combination with carboplatin may be effective in the treatment of gastric cancer. The toxicity of NM-3 needs further studies.

Abstract:Abstract Background Craniospinal irradiation (CSI) poses a challenge to treatment planning due to the large target, field junction, and multiple organs at risk (OARs) involved. The aim of this study was to evaluate the performance of knowledge-based planning (KBP) in CSI by comparing original manual plans (MP), KBP RapidPlan initial plans (RP I ), and KBP RapidPlan final plans (RP F ), which received further re-optimization to meet the dose constraints. Patients and methods Dose distributions in the target were evaluated in terms of coverage, mean dose, conformity index (CI), and homogeneity index (HI). The dosimetric results of OARs, planning time, and monitor unit (MU) were evaluated. Results All MP and RP F plans met the plan goals, and 89.36% of RP I plans met the plan goals. The Wilcoxon tests showed comparable target coverage, CI, and HI for the MP and RP F groups; however, worst plan quality was demonstrated in the RP I plans than in MP and RP F . For the OARs, RP F and RP I groups had better dosimetric results than the MP group ( P < 0.05 for optic nerves, eyes, parotid glands, and heart). The planning time was significantly reduced by the KBP from an average of 677.80 min in MP to 227.66 min ( P < 0.05) and 307.76 min ( P < 0.05) in RP I , and RP F , respectively. MU was not significantly different between these three groups. Conclusions The KBP can significantly reduce planning time in CSI. Manual re-optimization after the initial KBP is recommended to enhance the plan quality.

Abstract:Purpose Linear accelerator‐based stereotactic radiosurgery (SRS) has become a mainstay for simultaneous management of multiple intracranial targets. Recent improvements in treatment planning systems (TPS) have enabled treatment of multiple brain metastases using dynamic conformal arcs (DCA) and a single treatment isocenter. However, as the volume of healthy tissue receiving at least 12 Gy (V12) is linked to the probability of developing radionecrosis, balancing target coverage while minimizing V12 is a critical factor affecting SRS plan quality. Current TPS allow users to adjust various parameters influencing plan optimization. The purpose of this work is to quantify the effect of negative margins on V12 for cranial SRS plans managing multiple brain metastases. Methods Using the Brainlab Elements v3.0 TPS (Brainlab, Munich, Germany), we calculated V10, V12, V15, monitor units, and conformity index for seventeen SRS plans treating 2–10 metastases on our Elekta Versa HD (Elekta, Stockholm, Sweden) linear accelerator. We compared plans optimized using 70%‐90% prescription isodose lines (IDL) in 5% increments. Clinical Evaluation of the Inverse Planning System Utilized in Gamma Knife Lightning

Abstract:Objectives The purpose of this study is to independently compare the performance of the inverse planning algorithm utilized in Gamma Knife (GK) Lightning Treatment Planning System (TPS) to manual forward planning, between experienced and inexperienced users, for different types of targets. Materials and Methods Forty patients treated with GK stereotactic radiosurgery (SRS) for pituitary adenoma (PA), vestibular schwannoma (VS), post-operative brain metastases (pBM), and intact brain metastases (iBM) were randomly selected, ten for each site. Three inversely optimized plans were generated for each case by two experienced planners (OptExp1 and OptExp2) and a novice planner (OptNov) using GK Lightning TPS. For each treatment site, the Gradient Index (GI), the Paddick Conformity Index (PCI), the prescription percentage, the scaled beam-on time (sBOT), the number of shots used, and dosimetric metrics to OARs were compared first between the inversely optimized plans and the manually generated clinical plans, and then among the inversely optimized plans. Statistical analyses were performed using the Student’s t-test and the ANOVA followed by the post-hoc Tukey tests. Results The GI for the inversely optimized plans significantly outperformed the clinical plans for all sites. PCIs were similar between the inversely optimized and clinical plans for PA and VS, but were significantly improved in the inversely optimized plans for iBM and pBM. There were no significant differences in the sBOT between the inversely optimized and clinical plans, except for the PA cases. No significant differences were observed in dosimetric metrics, except for lower brain V 12Gy and PTV D 98% in the inversely optimized plans for iBM. There were no noticeable differences in plan qualities among the inversely optimized plans created by the novice and experienced planners. Conclusion Inverse planning in GK Lightning TPS produces GK SRS plans at least equivalent in plan quality and similar in sBOT compared to manual forward planning in this independent validation study. The automatic workflow of inversed planning ensures a consistent plan quality regardless of a planner’s experience.

Abstract:Treatment of patients with multiple brain metastases using a single-isocenter volumetric modulated arc therapy (VMAT) has been shown to decrease treatment time with the tradeoff of larger low dose to the normal brain tissue. We have developed an efficient Projection Summing Optimization Algorithm to optimize the treatment geometry in order to reduce dose to normal brain tissue for radiosurgery of multiple metastases with single-isocenter VMAT. The algorithm: (a) measures coordinates of outer boundary points of each lesion to be treated using the Eclipse Scripting Application Programming Interface, (b) determines the rotations of couch, collimator and gantry using three matrices about the cardinal axes, (c) projects the outer boundary points of the lesion on to Beam Eye View projection plane, (d) optimizes couch and collimator angles by selecting the least total unblocked area for each specific treatment arc and (e) generates a treatment plan with the optimized angles. The results showed significant reduction in the mean dose and low dose volume to normal brain, while maintaining the similar treatment plan qualities on the thirteen patients treated previously. The algorithm has the flexibility with regard to the beam arrangements and can be integrated in the treatment planning system for clinical application directly.

Abstract:Fractionated radiotherapy was established in the 1920s based upon two principles: (1) delivering daily treatments of equal quantity, unless the clinical situation requires adjustment, and (2) defining a specific treatment period to deliver a total dosage. Modern fractionated radiotherapy continues to adhere to these century-old principles, despite significant advancements in our understanding of radiobiology. At UT Southwestern, we are exploring a novel treatment approach called PULSAR (Personalized Ultra-Fractionated Stereotactic Adaptive Radiotherapy). This method involves administering tumoricidal doses in a pulse mode with extended intervals, typically spanning weeks or even a month. Extended intervals permit substantial recovery of normal tissues and afford the tumor and tumor microenvironment ample time to undergo significant changes, enabling more meaningful adaptation in response to the evolving characteristics of the tumor. The notion of dose painting in the realm of radiation therapy has long been a subject of contention. The debate primarily revolves around its clinical effectiveness and optimal methods of implementation. In this perspective, we discuss two facets concerning the potential integration of dose painting with PULSAR, along with several practical considerations. If successful, the combination of the two may not only provide another level of personal adaptation ("adaptive dose painting"), but also contribute to the establishment of a timely feedback loop throughout the treatment process. To substantiate our perspective, we conducted a fundamental modeling study focusing on PET-guided dose painting, incorporating tumor heterogeneity and tumor control probability (TCP).

Abstract:Dose painting has been developed to modulate the required dose in the target area without increasing the toxicity in healthy areas. Apart from determining the accurate location and size of tumors, quantitative functional imaging can be used to implement the dose painting. Functional imaging, such as multi-parameter MRI and PET CT, allows us to achieve biological dose escalation by increasing the dose in certain areas or voxels that are therapy-resistant in the gross tumor volume while reducing the dose in the less aggressive area or voxels. Functional imaging can serve as an indicator of therapeutic intervention in radiotherapy due to microscopic tissue properties. With such biological indicators, the personalized radiation dose can be tailored to a specific contour or a voxel using dose painting. In this review, we firstly discuss several quantitative functional imaging techniques including PET-CT and multi-parameter MRI. Furthermore, theoretical and experimental comparisons for dose painting by contours (DPBC) and dose painting by numbers (DPBN), along with outcome analysis after dose painting are provided. Finally, we conclude major challenges and future directions in this field through which we hope to inspire exciting developments and fruitful research avenues.
Guaranteed epsilon-optimal treatment plans with minimum number of beams for stereotactic body radiation therapy

Abstract:Stereotactic body radiotherapy (SBRT) is characterized by delivering a high amount of dose in a short period of time. In SBRT the dose is delivered using open fields (e.g., beam's-eye-view) known as "apertures". Mathematical methods can be used for optimizing treatment planning for delivery of sufficient dose to the cancerous cells while keeping the dose to surrounding organs at risk (OARs) minimal. Two important elements of a treatment plan are quality and delivery time. Quality of a plan is measured based on the target coverage and dose to OARs. Delivery time heavily depends on the number of beams used in the plan since the setup times for different beam directions constitute a large portion of the delivery time. Therefore the ideal plan, in which all potential beams can be used simultaneously, will be associated with a long impractical delivery time. We use the dose to OARs in the ideal plan to find the plan with the minimum number of beams which is guaranteed to be epsilon-optimal (i.e., a predetermined maximum deviation from the ideal plan is guaranteed). Since the treatment plan optimization is inherently a multicriteria optimization problem, the planner can navigate the ideal dose distribution Pareto surface and select a plan of desired target coverage versus OARs sparing, and then use the proposed technique to reduce the number of beams while guaranteeing epsilon-optimality. We use mixed integer programming (MIP) for optimization. To reduce the computation time for the resultant MIP, we use two heuristics: a beam elimination scheme and a family of heuristic cuts, known as "neighbor cuts", based on the concept of "adjacent beams". We show the effectiveness of the proposed solution technique on two clinical cases, a liver and a lung case. Based on our technique we propose an algorithm for fast generation of epsilon-optimal plans.

Dosimetric Impact of Optimization Resolutions Using the New Photon Optimizer Algorithm in Eclipse Treatment Planning System

Abstract:Objective To investigate the effect of optimization resolutions of new photon optimizer in Eclipse treatment planning system on the dose to planning target volume (PTV) and organs-at-risk (OAR) of preoperational simultaneous-integrated-boosting volumetric modulated arc therapy plans for patients with rectal cancer,and provide clinical guidelines of resolution selection to strike a balance between plan quality and efficiency.Methods After the RapidPlan model applied in the research was set with a resolution of 1.25,2.50 and 5.00 mm,respectively,the dose-volume histogram (DVH) were estimated,respectively,and Application Programming Interface (API) script was used to export these data for comparison.Herein,3 plans using different resolutions were generated for 20 patients,and their DVH was exported.We applied the in-house MATLAB code to calculate the mean DVH,and SigmaPlot and SPSS soltware for plotting and statistical analysis.Time for DVH estimation and optimization were also recorded and analyzed.Results According to the API exportation,the DVH estimation and optimized parameters acquired by various resolutions were exactly the same.However,the optimization time using a resolution of 1.25 mm was about 3-4 times of that using the resolution of 2.50 and 5.00 mm.All the resolutions satisfied clinical requirements for target dose coverage.The differences in the conformity index of PTV and planning gross tumor volume,and the homogeneity index of PTV were not statistically significant (P＞0.05).Though the homogeneity index of planning gross tumor volume was significantly different among 3 plans,the magnitude of deviations was within 0.01,which was considered clinically negligible.Relative to the resolutions of 2.50 and 5.00 mm,1.25 mm did not always guarantee the best OAR sparing.The differences in mean DVH were marginal.Conclusion The impacts of optimization resolutions on the dose to target areas and OAR are trivial,but the differences in optimization time are dramatic.The plan with a resolution of 2.50 mm was most cost-effective.

Abstract:Radiotherapy planning is a critical aspect of cancer treatment, where the optimal selection of beam directions and dose distributions significantly impacts treatment efficacy and patient outcomes. Traditionally, this process involves time-consuming manual trial-and-error methods, leading to suboptimal treatment plans. To address this challenge, optimization strategies based on advanced artificial intelligence (AI) techniques have been explored. This paper presents an investigation into the application of AI-driven optimization methods for beam direction and dose distribution selection in radiotherapy planning. The study proposes an approach utilizing Convolutional Neural Networks (CNN) to learn the relationship between patient anatomy and optimal beam orientations. The CNN model is trained on a dataset comprising anatomical features and corresponding beam orientations, derived from a column generation (CG) algorithm. Additionally, Particle Swarm Optimization (PSO) and Grey Wolf Optimization (GWO) algorithms are employed to optimize the CNN's weights and biases to attain the Fluence Map Optimization (FMO) objective function. Experiments are conducted using data from 70 clinical prostate cancer patients. The results demonstrate the effectiveness of the CNN-PSO and CNN-GWO approaches in generating beam orientations that yield treatment plans with dose distributions comparable to those obtained through traditional CG. DVH analysis of the resulting plans for different anatomical structures validates the accuracy and feasibility of the CNN-GWO model in radiotherapy planning. The findings of this study highlight the potential of AI-driven optimization strategies to revolutionize radiotherapy planning by significantly reducing planning time and enhancing treatment plan quality.

Abstract:The aim of this study was to evaluate the potential to increase the tumor control probability (TCP) with 'dose painting by numbers' (DPBN) plans optimized in a treatment planning system (TPS) compared to uniform dose plans. The DPBN optimization was based on our earlier published formalism for prostate cancer that is driven by dose-responses of Gleason scores mapped from apparent diffusion coefficients (ADC). For 17 included patients, a set of DPBN plans were optimized in a TPS by maximizing the TCP for an equal average dose to the prostate volume (CTVT) as for a conventional uniform dose treatment. For the plan optimizations we applied different photon energies, different precisions for the ADC-to-Gleason mappings, and different CTVT positioning uncertainties. The TCP increasing potential was evaluated by the DPBN efficiency, defined as the ratio of TCP increases for DPBN plans by TCP increases for ideal DPBN prescriptions (optimized without considering radiation transport phenomena, uncertainties of the CTVT positioning, and uncertainties of the ADC-to-Gleason mapping). The median DPBN efficiency for the most conservative planning scenario optimized with a low precision ADC-to-Gleason mapping, and a positioning uncertainty of 0.6 cm was 10%, meaning that more than half of the patients had a TCP gain of at least 10% of the TCP for an ideal DPBN prescription. By increasing the precision of the ADC-to-Gleason mapping, and decreasing the positioning uncertainty the median DPBN efficiency increased by up to 40%. TCP increases with DPBN plans optimized in a TPS were found more likely with a high precision mapping of image data into dose-responses and a high certainty of the tumor positioning. These findings motivate further development to ensure precise mappings of image data into dose-responses and to ensure a high spatial certainty of the tumor positioning when implementing DPBN clinically.

Iterative Dataset Optimization in Automated Planning: Implementation for Breast and Rectal Cancer Radiotherapy

Abstract:PURPOSE:To develop a new automated treatment planning solution for breast and rectal cancer radiotherapy.METHODS:The automated treatment planning solution developed in this study includes selection of the iterative optimized training dataset, dose volume histogram (DVH) prediction for the organs at risk (OARs), and automatic generation of clinically acceptable treatment plans. The iterative optimized training dataset is selected by an iterative optimization from 40 treatment plans for left-breast and rectal cancer patients who received radiation therapy. A two-dimensional kernel density estimation algorithm (noted as two parameters KDE) which incorporated two predictive features was implemented to produce the predicted DVHs. Finally, 10 additional new left-breast treatment plans are re-planned using the Pinnacle3 Auto-Planning (AP) module (version 9.10, Philips Medical Systems) with the objective functions derived from the predicted DVH curves. Automatically generated re-optimized treatment plans are compared with the original manually optimized plans.RESULTS:By combining the iterative optimized training dataset methodology and two parameters KDE prediction algorithm, our proposed automated planning strategy improves the accuracy of the DVH prediction. The automatically generated treatment plans using the dose derived from the predicted DVHs can achieve better dose sparing for some OARs without compromising other metrics of plan quality.CONCLUSIONS:The proposed new automated treatment planning solution can be used to efficiently evaluate and improve the quality and consistency of the treatment plans for intensity-modulated breast and rectal cancer radiation therapy.

Abstract:Purpose The purpose of this study is to establish and assess a practical delivery quality assurance method for stereotactic radiosurgery with Cyberknife by analyzing the geometric and dosimetric accuracies obtained using a PTW31016 PinPoint ionization chamber and EBT3 films. Moreover, this study also explores the relationship between the parameters of plan complexity, target volume, and deliverability parameters and provides a valuable reference for improving plan optimization and validation. Methods One hundred fifty cases of delivery quality assurance plans were performed on Cyberknife to assess point dose and planar dose distribution, respectively, using a PTW31016 PinPoint ionization chamber and Gafchromic EBT3 films. The measured chamber doses were compared with the planned mean doses in the sensitive volume of the chamber, and the measured planar doses were compared with the calculated dose distribution using gamma index analysis. The gamma passing rates were evaluated using the criteria of 3%/1 mm and 2%/2 mm. The statistical significance of the correlations between the complexity metrics, target volume, and the gamma passing rate were analyzed using Spearman’s rank correlation coefficient. Results For point dose comparison, the averaged dose differences (± standard deviations) were 1.6 ± 0.73% for all the cases. For planar dose distribution, the mean gamma passing rate for 3%/1 mm, and 2%/2 mm evaluation criteria were 94.26% ± 1.89%, and 93.86% ± 2.16%, respectively. The gamma passing rates were higher than 90% for all the delivery quality assurance plans with the criteria of 3%/1 mm and 2%/2 mm. The difference in point dose was lowly correlated with volume of PTV, number of beams, and treatment time for 150 DQA plans, and highly correlated with volume of PTV for 18 DQA plans of small target. DQA gamma passing rate (2%/2 mm) was a moderate significant correlation for the number of nodes, number of beams and treatment time, and a low correlation with MU. Conclusion PTW31016 PinPoint ionization chamber and EBT3 film can be used for routine Cyberknife delivery quality assurance. The point dose difference should be within 3%. The gamma passing rate should be higher than 90% for the criteria of 3%/1 mm and 2%/2 mm. In addition, the plan complexity and PTV volume were found to have some influence on the plan deliverability.

Abstract:Radiotherapy inverse planning often requires planners to modify parameters in the treatment planning system's objective function to produce clinically acceptable plans. Due to the manual steps in this process, plan quality can vary depending on the planning time available and the planner's skills. This study investigates two hyperparameter-tuning methods for automated inverse planning. Because this framework does not train a model on previously-optimized plans, it can be readily adapted to practice pattern changes, and plan quality is not limited by that of a training cohort. We selected 10 patients who received lung SBRT using manually-generated clinical plans. We used random sampling (RS) and Bayesian optimization (BO) to tune parameters using linear-quadratic utility functions based on 11 clinical goals. Normalizing all plans to have PTV D95 equal to 48 Gy, we compared plan quality for the automatically-generated and manually-generated plans. We also investigated the impact of iteration count on the automatically-generated plans, comparing planning time and plan utility for RS and BO plans with and without stopping criteria. Without stopping criteria, the median planning time was 1.9 and 2.3 hours for RS and BO plans. The OAR doses in the RS and BO plans had a median percent difference (MPD) of 48.7% and 60.4% below clinical dose limits and an MPD of 2.8% and 3.3% below clinical plan doses. With stopping criteria, the utility decreased by an MPD of 5.3% and 3.9% for RS and BO plans, but the median planning time was reduced to 0.5 and 0.7 hours, and the OAR doses still had an MPD of 42.9% and 49.7% below clinical dose limits and an MPD of 0.3% and 1.8% below clinical plan doses. This study demonstrates that hyperparameter-tuning approaches to automated inverse planning can reduce active planning time with plan quality that is similar to or better than manually-generated plans.

Impact of the skull contour definition on Leksell Gamma Knife® Icon™ radiosurgery treatment planning

Abstract:Introduction: The Gamma Knife® planning software (TMR 10, Elekta Instruments, AB, Sweden) affords two ways of defining the skull volume, the "historical" one using manual measurements (still perform in some centers) and the new one using image-based skull contours. Our objective was to assess the potential variation of the dose delivery calculation using consecutively in the same patients the two above-mentioned techniques. Materials and methods: We included in this self-case-control study, 50 patients, treated with GKRS between July 2016 and January 2017 in Lausanne University Hospital, Switzerland, distributed among four groups: convexity targets (n = 18), deep-seated targets (n = 13), vestibular schwannomas (n = 11), and trigeminal neuralgias (n = 8). Each planning was performed consecutively with the 2 skull definition techniques. For each treatment, we recorded the beam-on time (min), target volume coverage (%), prescription isodose volume (cm3), and maximal dose (Gy) to the nearest organ at risk if relevant, according to each of the 2 skull definition techniques. The image-based contours were performed using CT scan segmentation, based upon a standardized windowing for all patients. Results: The median difference in beam-on time between manual measures and image-based contouring was + 0.45 min (IQR; 0.2-0.6) and was statistically significant (p < 0.0001), corresponding to an increase of 1.28% beam-on time per treatment, when using image-based contouring. The target location was not associated with beam-on time variation (p = 0.15). Regarding target volume coverage (p = 0.13), prescription isodose volume (p = 0.2), and maximal dose to organs at risk (p = 0.85), no statistical difference was reported between the two skull contour definition techniques. Conclusion: The beam-on time significantly increased using image-based contouring, resulting in an increase of the total dose delivery per treatment with the new TMR 10 algorithm. Other dosimetric parameters did not differ significantly. This raises the question of other potential impacts. One is potential dose modulation that should be performed as an adjustment to new techniques developments. The second is how this changes the biologically equivalent dose per case, as related to an increased beam on time, delivered dose, etc., and how this potentially changes the radiobiological effects of GKRS in an individual patient.
