Abstract:Objective: to compare canine tooth stress distribution condition during maxillary canine tooth distalization by different accessories of bracket-free invisible orthodontics technology after removal of maxillary first premolar, and provide basis for clinical design of invisible orthodontics technology. Method: CBCT scanning image of a patient with individual normal occlusion was adopted, Mimics, Geomagic and ProlE software were used for establishing three-dimensional models of maxilla, maxillary dentition, parodontium, invisible orthodontics appliance and accessories, ANSYS WORKBENCH was utilized as finite element analysis tools for analyzing stress distribution and movement pattern of canine tooth and parodontium when canine tooth was equipped with power arm and vertical rectangle accessory. Meanwhile, canine tooth none-accessory design group was regarded as a control. Result: teeth had even bistal surface stress distribution in the power arm group; stress was concentrated on distal tooth neck, and the stress was gradually deviated to mesial-labial side and distal lingual side in vertical rectangle group and none-accessory group. Conclusion: teeth tend to move as a whole in the Power arm group, vertical rectangle group has lower tooth gradient compared with the none-accessory group, teeth are inclined for movement in the none-accessory group, and canine teeth tend to rotate to the distal lingual side.
Numeric simulation of occlusal interferences in molars restored with ultrathin occlusal veneers

Abstract:Statement of problem: Selecting material for a minimally invasive occlusal veneer reconstruction concept requires an understanding of how stresses are distributed during functional and parafunctional forces. Purpose: The purpose of this in vitro study was to investigate stress distribution in a maxillary molar restored with ultrathin occlusal veneers and subjected by an antagonistic mandibular molar to clenching and working and nonworking movements. Material and methods: A maxillary first molar was modeled from microcomputed tomography (micro-CT) data, using medical image processing software, stereolithography editing/optimizing software, and finite element software. Simulated ultrathin occlusal veneer materials were used. The mandibular molar antagonist was a solid nondeformable geometric entity. Loads simulated clenching, working, and nonworking movements with loading of 500 N. The values of the maximum principal stress were recorded. Results: In the clenching load situation, maximum tensile stresses were located at the occlusal veneer (52 MPa for composite resin versus 47 MPa for ceramic). In the working movement, significant additional tensile stresses were found on the palatal root (87 MPa for composite resin and 85 MPa for ceramic). In the nonworking movement, tensile stress on the ultrathin occlusal veneer increased to 118 MPa for composite resin and 143 MPa for ceramic veneers. Tensile stress peaks shifted to the mesiobuccal root (75 MPa for composite resin and 74 MPa for ceramic). Conclusions: The topography of stresses generated by the various occlusal interferences were clearly identified. Significant tensile stress concentrations were found within the restoration's occlusal topography and root, with the nonworking interference being the most harmful and also the most revealing of the difference between the composite resin and ceramic ultrathin occlusal veneers.
In Vitro Investigation Using a New Biomechanical Force–Torque Analysis System: Comparison of Conventional and CAD/CAM-Fixed Orthodontic Retainers

Abstract:(1) Background: After more than a decade since their first description, Inadvertent Tooth Movements (ITMs) remain an adverse effect of orthodontic retainers without a clear etiology. To further investigate the link between ITMs and the mechanical properties of different retainers, the response upon vertical loading was compared in three retainer types (two stainless steel and one nickel–titanium). The influence of different reference teeth was also considered. (2) Methods: Three retainers (R1, R2, R3) were tested in a newly developed biomechanical analysis system (FRANS). They were bonded to 3D-printed models of the lower anterior jaw and vertically displaced up to 0.3 mm. Developing forces and moments were recorded at the center of force. (3) Results: The vertical displacement caused vertical forces (Fz) and labiolingual moments (My) to arise. These were highest in the lateral incisors (up to 2.35 ± 0.59 N and 9.27 ± 5.86 Nmm for R1; 1.69 ± 1.06 N and 7.42 ± 2.65 Nmm for R2; 3.28 ± 1.73 N and 15.91 ± 9.71 Nmm for R3) for all analyzed retainers and with the R3 retainer for all analyzed reference teeth, while the lowest Fz and My values were recorded with the R1 retainer. (4) Conclusions: Displacements of 0.2 mm and larger provided forces and moments which could be sufficient to cause unwanted torque movements, such as ITMs, in all analyzed retainers. Clinicians must be mindful of these risks and perform post-treatment checkups on patients with retainers of all materials.

Stress Analysis of Periodontal Tissue in en Masse Retraction With Integration of Maxillary Anterior Teeth: A Three-Dimensional Finite Element Method Study

Abstract:Objective: To simulate the en masse traction technique with the integration (EMTI) of six maxillary anterior teeth using a finite element model (FEM) and explore various protocols for maxillary protrusion. The study aimed to investigate root displacement and stress distribution in the periodontal ligament (PDL) by varying the retraction position and direction of EMTI applied to the maxillary anterior teeth. No actual participants were involved. Materials and methods: The FEM model included six teeth (central and lateral incisors and canines) with a PDL thickness of 0.3 mm. The model encompassing the alveolar bone (ALB) and EMTI had 180,528 elements and 47,836 nodes. The EMTI integrated six anterior teeth via a 0.9-mm-diameter stainless steel lingual wire, equipped with three moment arms extending toward the root apex: one midline (central arm) and two distal to the canines (lateral arms). The position and direction of the traction force applied to the three moment arms of the EMTI were varied to assess crown and apex displacement, as well as PDL stress. Results: Lingual tipping was consistent across all protocols, emphasizing controlled incisor tipping. The application of horizontal traction at 10 mm and traction at 7 mm from the central and lateral arms of the EMTI, respectively, demonstrated the most uniform stress distribution across the PDL of all six anterior teeth. Conclusions and clinical significance: The FEM analysis results suggest that the new EMTI method, which retracts the maxillary anterior teeth as a unit, is effective for tooth movement and PDL stress distribution. The EMTI technique, with its specific traction protocols and emphasis on controlled tipping, appears to be a promising approach for addressing maxillary protrusions.

Abstract:Introduction: Force is applied through brackets in fixed mechanotherapy which generates stress in periodontal ligament and root of the teeth and is difficult to measure directly. Finite element analysis is used as a solution for understanding this biomechanical response. So the aims and objectives of this study were to evaluate the stress magnitude induced in the labial and lingual surface of mandibular incisors using superelastic NiTi arch wires of different cross-section at different inclination through finite element analysis. Materials and Methods: Finite element model of all mandibular teeth including periodontal ligament, alveolar bone, buccal tubes, brackets, and arch wire were constructed using mechanical elastic properties of the materials. Three different NiTi arch wires, round (0.016′′), square (0.016′′ × 0.016′′), and rectangular (0.016′′ × 0.022′′), were placed into the bracket slots and models were constructed which were analyzed for stress distribution on root surface and periodontal ligament with 3 different cross-sections at 90°, 100°, and 110° with Ansys Version 14 software. Conclusion: Labial stress was found highest in round arch wire as compared to square and rectangular wire. Lingual stress was found to be slightly more in rectangular wire as compared to round and square arch wire.
Three-dimensional finite element analysis of upper anterior teeth retraction and intrusion using clear aligners and mini-implants

Abstract:Objectives: This study aimed to investigate the differences in treatment outcomes of upper anterior teeth retraction and intrusion with clear aligners and different types of elastics on mini-implants via the 3D finite element method. Methods: Cone-beam computed tomography (CBCT) was conducted to construct a 3D finite element model of maxillary bone, dentition, and clear aligners using Mimics, Geomagic, Solidworks, and Ansys. Four model groups were developed. Group 1 was the control group. Group 2 implanted a mini-implant between the upper second premolar and the upper first molar with a force of 0.98 N applied to the canine cut along the retracting direction for each side. Group 3 was based on Group 2, which implanted an additional mini-implant between the upper central incisors with a force of 0.98 N applied to the upper anterior teeth cuts. Group 4 was also based on Group 2, which implanted 2 additional mini-implants between the upper central and lateral incisors with a force of 0.56 N applied to the upper anterior teeth cuts for both sides. Anterior teeth torque changes, displacement patterns, and stress distributions were analyzed. Results: Among all the experimental groups, the anterior teeth demonstrated intrusion and distal inclination, accompanied with varied degrees of torque loss. In Group 3, the maxillary and lateral incisors had the smallest sagittal coronal displacement. In Group 4, the intrusion values of the maxillary and lateral incisors were the largest among all the experimental groups. The maximum stress concentration in Group 2 was the most significant among all the experimental groups. Conclusions: Implanting the mini-implant between the central incisors for traction is conducive to torque control. By contrast, implanting the mini-implants between the central incisors and lateral incisors combined with vertical traction is conducive to simple intrusion, avoiding the "bowing effect" to a certain extent.

Abstract:The finite element method (FEM) is a computational method that can solve all biomechanical problems, including the field of orthodontics. The purpose of this virtual experimental study is to determine the behavior of a real orthodontic system subjected to different systems of loads. To analyze the real orthodontic system, we studied the case of a 21-year-old female patient. We used the InVesalius program, which can transform a set of DICOM-type images taken from cone beam computed tomography (CBCT) into three-dimensional structures. These structures were edited, modified, completed, and analyzed from a geometric point of view with the help of the Geomagic software. The final result of these operations must be a three-dimensional model made up of perfectly closed surfaces so that they can be transformed into virtual solids. The model consisting of perfectly closed surfaces is loaded into computer-aided design (CAD) programs. Bracket and tube components, as well as orthodontic wires, can be added to these models, similar to the analyzed patient's tissues. When the model is complete and geometrically correct, it is exported to a program that uses FEM, such as Ansys Workbench. The simulation was performed for the forces of 0.5, 0.6, 0.7, 0.8, 0.9, and 1 N. The intention was to determine the behavior of the entire orthodontic system for these force values. After running the simulations, result maps were obtained that were composed of displacement, strain, and stress diagrams. It was also found that, in addition to the known rigidity, the orthodontic system has some elasticity due to the orthodontic wires, as well as the periodontal ligaments. Thus, a virtual analysis study can be carried out starting from a real patient with pre-treatment CBCT images and the virtual models of the bracket and tube elements and of the orthodontic wires.

Abstract:Introduction: Orthodontics a unique branch in dentistry is inter-related with biology, mathematics and engineering sciences. Biomechanics is fundamental to the practice of orthodontics. Stresses generated at any point during orthodontic treatment are critical in remodeling of bone and type of tooth movement. Aims and Objectives: The aim of the following study is to determine the initial stresses produced in the tooth, periodontal ligament (PDL) and bone when force is applied on the tooth for intrusion and tipping on labial and lingual sides in the presence of varying alveolar bone heights. Materials and Methods: A geometric model generated using AutoCAD (Autodesk, Inc.). Six three-dimensional finite element models of a maxillary central incisor were designed with PDL and varying alveolar bone heights after applying the boundary conditions of the model, force was applied on the tooth.Results: Results were obtained with color coded three-dimensional fringe patterns. Conclusion: Alveolar bone loss causes an increase in the maximum initial stresses relative to the bone heights.
Stress distribution of the modified clear twin-block aligner on the temporomandibular joint, alveolar bone and teeth: A finite element analysis

Abstract:Objective: The modified clear twin-block aligner (CTBA) was developed to provide a mandibular advancement appliance for the treatment of mandibular retrognathia. The objective of this study was to analyse the stress distribution changes of CTBA with 45°, 60° and 70° bite blocks. Material and methods: A three-dimensional model of the craniomaxillofacial bones and teeth was generated from a spiral computed tomography (CT) scan. The models of the articular disc, capsule, periodontal ligament and CTBA were constructed mathematically. After assigning the appropriate material properties and the boundary condition using ABAQUS software, we simulated the CTBA with different bite blocks to analyse the mechanical effects. Results: In the temporomandibular joint (TMJ) region, the posterior aspect of the condyle and glenoid fossa experienced tensile stress that was approximately about 22 times greater at 70° than at 45°. The Von Mises stress distribution on the articular disc tended to be uniform. The strain direction of the condyle was backward. In the maxillary bone, the stress on the labial alveolar bone was about 5.83MPa at 70° and greater than that on the lingual side. The resulting displacement of the dentition revealed a tendency for the upper teeth to shift backward and the lower teeth to move forward by 0.46 to 0.49mm. The foregoing stress and displacement rose as the angle of the bite blocks increased. Conclusions: CTBA with 70° bite blocks constituted an advantageous biomechanical setting for the treatment of mandibular retrognathia in teenagers and provided a superior therapeutic effect.
Effect of Attachment on Movement Control of the Central Incisor Using Invisible Orthodontics: In-Silico Finite Element Analysis

Abstract:We aimed to study the influence of attachment on central incisor movement control using invisible orthodontics with different translation or rotation targets. In this study, cone-beam computerised tomography was used to scan one 15-year-old volunteer’s teeth. Three-dimensional models of the mandibular central incisor, removable orthodontic appliance, and a rectangular attachment were reconstructed. A non-linear finite element simulation was used to analyse the influence of attachment on the stress distribution of the periodontal ligament of the central incisor under tooth rotation, mesial translation, and labial-lingual translation. For mandibular central incisor correction, using rectangular attachments increased the translation by up to 9.6%, increased the maximum stress on the periodontal ligament during the mesial translation correction, and reduced unwanted labial-lingual tilt by up to 40% during the rotation of the tooth. However, wearing an attachment during labial-lingual translation had no significant effect on the orthodontic results of the mandibular central incisor. In the mandibular central incisor’s invisible orthodontic, the rectangular attachment appliance increased the displacement and alleviated the side effect after applying mesial translation and tooth rotation. For labial-lingual translation, the application of a rectangular attachment had no pronounced effect on the results.

Abstract:Tadpole-type amphiphilic dendrimers consisting of an alkyl chain, a quaternary ammonium bromide (qb) and a poly(amidoamine) dendron (den) (CnqbdenGm, where n represents an alkyl chain with a length of 10,14, or 18 carbons; m is the generation number G of dendron taking the value -0.5, 0.5, or 1.5) were synthesized using N, N-dimethylethylenediamine as a central scaffold. Electrical conductivity, surface tension, pyrene fluorescence, and dynamic light scattering measurements were used to characterize the properties of the dendrimers. In addition, the effect of the alkyl chain length and the generation number of a dendron on these properties was evaluated through a comparison with those of the corresponding previously reported amphiphilic dendrimers with lactobionamide sugar terminal groups (CnqbdenGmLac) and conventional cationic monomeric surfactants (CnTAB). Both critical micelle concentration (cmc) and surface tension (except for C18 series) were lower than those of CnqbdenGmLac and CnTAB with the same alkyl chain length, indicating that the synthesized amphiphilic dendrimers have an excellent micelle-forming ability in solution and high adsorption ability at the air/water interface, in spite of the large bulky dendron structure. When the alkyl chain length and the generation number of the amphiphilic dendrimers were increased, the surface tension became high because of the curved long alkyl chain and the bulky structure of dendron. Further, CnqbdenGm formed micelles with a small size in solution, and the micelles of CnqbdenG(-0.5) had almost a constant size despite the changes in the concentration, while those of CnqbdenG0.5 and CnqbdenG1.5 became smaller with increasing concentration. The difference in the behavior results from the difference in the number of amide groups in the low- and high-generation dendrons.
Stress and movement trend of lower incisors with different IMPA intruded by clear aligner: a three-dimensional finite element analysis

Abstract:Background: During the intrusion of lower incisors with clear aligners (CAs), root disengagement from the alveolar bone often occurs, resulting in serious complications. This study aimed to determine the potential force mechanism of the mandibular anterior teeth under the pressure of CA, providing theoretical data for clinical practice. Methods: In this study, a 3D finite element model was established, including the CA, periodontal ligament, and mandibular dentition. Incisor mandibular plane angles were set as 5 groups: 90°, 95°, 100°, 105°, and 110°. The 4 mandibular incisors were intruded by 0.2 mm, while the canines were the anchorage teeth. The stress, force systems, and potential movement trends of mandibular anterior teeth were obtained. Results: The compressive stress of the incisors was concentrated in the lingual fossa, incisal ridge, and apex. With the increase in IMPA, the moment of central incisors changed from lingual crown moment to labial crown moment, with the turning point between 100° and 105°, but the center of resistance (CR) was always subjected to the force toward the lingual and intrusive direction. The force and moment toward the labial side of the lateral incisors were greater than those toward the central incisors. The canines always tipped distally and received extrusive force with no relationship with IMPA. Conclusions: With the increase in the initial IMPA, the direction of labiolingual force on the mandibular incisors was reversed. However, the root of the lower incisors always tipped labially, which indicated fenestration and dehiscence.
The Biomechanical Mechanism of Invisible Appliance in Pushing the Second Molar Backwards

Abstract:Objective To research the biomechanical mechanism of the invisible appliance technique in the process of pushing the molar backwards.Methods A numerical model of volunteer was formed from sequential computed tomography (CT).Scan images was taken at 0.5 mm intervals by using Mimics,Solidworfs and Abques software.The three dimensional finite element including alveolar bone,upper denture,pericementum and bracketless appliance were established.The research was designed for three groups,pushing backwards 0.2 mm.Results The max stress was in the periodontal membrane of the distal surface of the root of the palate of the maxillary second molar and the palatal surface of the root of the palate of the maxillary first molar.The equivalent stress is concentrated at 7.5-10 Kpa.In the second,the stress of the periodontal membrane of the upper incisor and upper canines were mainly concentrated in 5-7.5 Kpa.Then,for lateral incisors and maxillary second premolar,the equivalent stress was mainly concentrated in 2.5-5 Kpa.Conclusions The Invisalign technique pushing the molar backwards,the max stress is the second molar,followed by maxillary first molar,followed by incisors in the upper jaw,and then followed by canines.Later incisors and premolars are less stressed.Before orthodontics,the thickness of the lip bone should be evaluated.In addition,the thickness of the lip bone should be prevented from being absorbed by protrusion of anterior teeth,and the class Ⅱ elastic should be used timely.

Abstract:Objective To investigate the centre of resistance of the maxillary anterior teeth and the accompanied initial displacement of teeth in en masse retraction with micro-implant anchorage.MethodsA dry skull was scanned with high resolution spiral CT,and the obtained data were used to construct the three dimensional finite element model of "the micro-implant-segmented maxillary anterior teeth en masse retraction" force system.A 2 N horizontal retraction force by micro-implant were added to segmented maxillary teeth in every 2-mm interval from the archwire level and the initial displacement were calculated.ResultsThe centre of resistance was located in the position of 5.1 mm above alveolar ridge.The centre of rotation constants were different among the central incisor,lateral incisor and canine.Conclusion The bodily movement or tipping movement can be easily achieved by altering the horizontal height of the retraction force using the "micro-implant-segmented maxillary anterior teeth en masse retraction" force system.

Abstract:Flux core spheromak sustainment by electrostatic helicity injection is studied in resistive MHD. The geometry has magnetized electrodes at the ends held at a potential difference V. For V>V(c) the central current column is kink unstable. The nonlinear state with V just above V(c) has a large volume of flux surfaces, with rotational transform provided by the helical kinking of the column. As V increases the kink becomes stronger, the tori are destroyed, and the field lines exhibit chaotic scattering. The distribution of field line lengths L, related to confinement and parallel current density, is studied. At larger V or larger Lundquist number S, a limit cycle appears.

Abstract:Background: Overtreatment design of clear aligner treatment (CAT) in extraction cases is currently primarily based on the clinical experience of orthodontists and is not supported by robust evidence on the underlying biomechanics. This study aimed to investigate the biomechanical effects of overtreatment strategies involving different maxillary anterior teeth intrusion patterns during anterior teeth retraction by CAT in extraction cases. Materials and methods: A finite element model of the maxillary dentition with the first premolar extracted was constructed. A loading method of clear aligners (CAs) based on the initial state field was proposed. The iterative method was used to simulate the long-term orthodontic tooth movement under the mechanical load exerted by the CAs. Three groups of CAs were utilized for anterior teeth retraction (G0: control group; G1: incisors intrusion group; G2: anterior teeth intrusion group). Tooth displacement and occlusal plane rotation tendency were analyzed. Results: In G0, CAT caused lingual tipping and extrusion of the incisors, distal tipping and extrusion of the canines, mesial tipping, and intrusion of the posterior teeth. In G1, the incisors showed minimal extrusion, whereas the canines showed increased extrusion and distal tipping tendency. G2 showed the smallest degree of posterior occlusal plane angle rotation, while the inclination tendency of the canines and second premolars decreased. Conclusion: 1. In CAT, tooth displacement tendency may change with increased wear time. 2. During anterior teeth retraction, the incisor intrusion pattern can provide effective vertical control for the lateral incisors but has little effect on the central incisors. Anterior teeth intrusion patterns can alleviate the inclination of canines and second premolars, resulting in partial relief of the roller-coaster effect.

Abstract:Objective: To investigate the influence of light and heavy bite force on the mandibular movement trajectories, and the influence of bite force on virtual occlusal pre-adjustment of digital full crown. Methods: From October 2021 to March 2022, 10 postgraduate volunteers (3 males and 7 females, aged 22-26 years) were recruited from Peking University School and Hospital of Stomatology. Maxillary and mandibular digital models of the participants were obtained by intraoral scanning. Jaw relations were digitally transferred under heavy bite force and mandibular movement trajectories under light and heavy bite force were recorded by jaw motion analyser. Three mandibular markers were chosen, namely the mesial proximal contact point of the central incisor (incisal point) and the mesial buccal cusp tips of the bilateral first molars. The three-dimensional displacements of the markers under two kinds of bite force in the intercuspal position (ICP), the sagittal projection of the three-dimensional displacements in the protrusive edge-to-edge position, and the coronal projection of the three-dimensional displacements in the lateral edge-to-edge position of upper and lower posterior teeth were measured. Single-sample t-test was used to compare the three-dimensional displacements and the corresponding sagittal projection and coronal projection with 0, respectively. The left maxillary central incisor and left mandibular first molar were virtually prepared by the reverse engineering software. Then dental design software was used to design digital full crown using the copy method. The mandibular movement trajectories under light and heavy bite force were separately used to guide virtual occlusal pre-adjustment. The three-dimensional deviations (mean deviations and root mean square) between the lingual surface of the left maxillary central incisor or the occlusal surface of the left mandibular first molar and that of the natural tooth before preparation were calculated (light bite force group and heavy bite force group), and the differences between the two groups were compared by the paired t-test. Results: Under the two kinds of bite force, the three-dimensional displacements of the markers in the ICP were (0.217±0.135), (0.210±0.133) and (0.237±0.101) mm, respectively; the sagittal projection of the three-dimensional displacements of the markers in the protrusive edge-to-edge position were (0.204±0.133), (0.288±0.148) and (0.292±0.136) mm, respectively; the coronal projection of the three-dimensional displacements of the mesial buccal cusp tips of the bilateral first molars in the lateral edge-to-edge position were (0.254±0.140) and (0.295±0.190) mm, respectively. The differences between the above displacements and 0 were statistically significant (P<0.05). The results of occlusal pre-adjustment showed that the mean deviations of the lingual surface of the left maxillary central incisor in the light and heavy bite force groups were (0.215±0.036) and (0.195±0.041) mm (t=3.95, P=0.004), respectively. The mean deviations of the occlusal surface of the left mandibular first molar were (0.144±0.084) and (0.100±0.096) mm (t=0.84, P=0.036), respectively. Conclusions: Both the light and heavy bite force have an influence on the mandibular movement trajectories. Virtual occlusal pre-adjustment of prostheses with mandibular movement trajectories under heavy bite force can obtain morphology of lingual or occlusal surfaces closer to the natural teeth before preparation.
