Abstract:OBJECTIVE:To explore the relationship between glycemic control and visceral adiposity index (VAI) among type 2 diabetes mellitus (T2DM) patients.METHODS:A community-based epidemiological field study for patients with T2DM aged ≥ 40 years was conducted in China.Every participant underwent physical examinations, biochemical tests of fasting glucose, glycosylated hemoglobin (HbA1c), total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C) and so on, and a questionnaire, including anthropometric characteristics, lifestyle, disease history, family history, and medication use. Those participants with HbA1c ≥7.0% were classified as the poorly controlled in our analysis of relationship between glycemic control and VAI. Anthropometric characteristics, lifestyle, and biochemical indexes of the participants were compared among the groups of different VAI levels. Logistic models were applied in multiple analysis adjusting for possible confounders.RESULTS:A total of 1 607 patients with T2DM were recruited in our analysis with a mean age of (59.4±8.1) years and an average T2DM duration of (7.0±6.4) years. Among them, 78.3% were on hypoglycemic therapy. The cutoff points of quartiles of VAI were calculated for the males and females, respectively. According to the ascending order of the quartiles of VAI, the participants were divided into four groups, i.e. Q1, Q2, Q3, and Q4. The poor glycemic control rate for these groups were 60.6%, 65.7%, 70.1%, and 71.0%, respectively (Trend χ2=12.20, P<0.001). After adjustment for age, gender, systolic blood pressure (SBP), diastolic blood pressure (DBP), LDL-C, smoking, cardio-cerebral vascular disease (CVD) history, hypoglycemic therapy, T2DM duration, and family history of diabetes, the Logistic regression models showed that the glycemic control rate was significantly associated with VAI levels among the patients with T2DM. Compared with the participants in group Q1, the ORs of poor glycemic control for those in groups Q2, Q3, and Q4 were 1.239 (95%CI 0.918 to 1.672), 1.513 (95%CI 1.117 to 2.050), and 1.535 (95%CI 1.128 to 2.088), respectively (trend P=0.003). With each quartile increase in VAI, the OR of poor glycemic control was 1.162 (95%CI 1.054 to 1.282).CONCLUSION:The glycemic control among the patients with T2DM is significantly associated with VAI. High level of VAI is an indicator of poor glycemic control.

Abstract:Introduction & Objective: Safely achieving stringent glycemic control at hemoglobin A1c (HbA1c) ≤ 6.5% is recommended in some diabetes guidelines, and HbA1c < 5.7% is considered a normal level. We aim to examine the association between stringent glycemic control and long-term macrovascular outcomes using real world data in China. Methods: Data were retrospectively extracted from a regional data platform (2006/1/1-2021/10/31) in Yinzhou, Ningbo, China. Adult T2D patients with ≥ 1 HbA1c record from 2007/01/01-2016/10/31 were included to ensure ≥ 5 years of follow-up. Index date was the first HbA1c occurred ≥ 90 days after the initial T2D diagnosis. Index and sustained stringent glycemic control were defined as achieving HbA1c at ≤ 6.5% or < 5.7% at index or every measurement during follow-up, respectively. Cox proportional hazard model was used to examine the association between stringent glycemic control and 3-point major adverse cardiovascular event (3P-MACE) occurred during follow-up, adjusting for demographic and clinical characteristics. Results: A total of 7,049 patients (mean age 61.4±11.4 years, 3,751 [53.7%] female) were analyzed. Over a median follow-up of 6.7 years (interquartile range 5.5-7.9), 3P-MACE occurred in 1565 (22.2%) patients at an incidence rate of 3.4 per 100 person-years. Index and sustained stringent glycemic control were significantly associated with 3P-MACE risk reduction (all p<0.05). For index stringent glycemic control, the adjusted hazard ratio (aHR) was 0.88 [0.79-0.98] for HbA1c ≤ 6.5% and 0.84 [0.70-1.00] for HbA1c < 5.7%. For sustained stringent glycemic control, the aHR was 0.74 [0.63-0.86] for HbA1c ≤ 6.5% and 0.58 [0.42-0.79] for HbA1c < 5.7%. Conclusion: Stringent glycemic control was associated with significant risk reduction in long-term macrovascular outcomes in Chinese people with T2D. Abstract:Background: Diabetic peripheral neuropathy (DPN) impacts approximately 50% of individuals with Type 2 diabetes mellitus (T2DM), leading to severe complications such as foot ulcers and amputations. Notably, visceral adiposity is increasingly recognized as a pivotal factor in augmenting the risk of DPN. We aim to evaluate the correlation between obesity‐related body composition, particularly visceral fat, and DPN to facilitate early identification of high‐risk patients with T2DM. Methods: This cross‐sectional analysis encompassed 113 T2DM patients from the Department of Endocrinology and Metabolism at the Second Affiliated Hospital of Fujian Medical University, conducted between September 2020 and January 2021. Patients were categorized into two cohorts: those with DPN (DPN group) and those without (NDPN group). We utilized bioelectrical impedance analysis (BIA) to determine body measurements, such as weight and visceral fat area, in addition to collecting clinical and biochemical data. Logistic regression was employed to analyze the data. Results: The study uncovered a statistically significant difference in the visceral fat area between the DPN and NDPN groups (p = 0.048). Through multivariate logistic regression analysis, the visceral fat area was identified as an independent risk factor for DPN among T2DM patients (OR 1.027; 95% CI 1.004–1.051, p = 0.022). Other significant risk factors included the duration of diabetes and the presence of diabetic retinopathy. Conclusion: The visceral fat area serves as an independent risk factor for DPN in individuals with T2DM. Implementing measures to assess and manage visceral obesity could be vital in the prevention and management of DPN. This underscores the value of technologies such as BIA in clinical and community settings for early intervention.

Abstract:Background: Type 2 diabetes mellitus (T2DM)-associated mortality and morbidity are strongly dependent on glycemic control. With T2DM prevalence increasing in China, we aimed to assess glycemic control rates in Chinese T2DM outpatients.Material/Methods: A total of 9065 adult T2DM outpatients (5035 men) were assessed in 26 Chinese medical centers between August 2010 and April 2012. Patients were stratified according to BMI (kg/m(2)): <24, 24-28, and >28. Successful glycemic control was defined as glycated hemoglobin A1c (HbA1c) <= 7% or fasting plasma glucose (FPG) <7.0 mmol/L.Results: Among the participants included in this study, 2939 had BMI <24, 3361 had BMI of 24-28, and 2764 had BMI >28. The glycemic control rate was only 32.6%, and the triple control rate for glycemia, blood pressure, and lipidemia was only 11.2%. Glycemic control rates by BMI group were 33.7% (<24), 33.8% (24-28), and 30.2% (>28) (p=0.005), and corresponding incidences of cardiovascular diseases (CVD) were 12.2%, 15.7%, and 15.9% (p<0.001). Multivariate logistic regression analysis demonstrated that older age (p<0.001), higher BMI (p=0.026), larger waist circumference (p<0.001), less education (p<0.001), and recent diagnosis (p<0.001) were independent risk factors for poor glycemic control.Conclusions: The T2DM glycemic control rate in China is currently low, especially in older obese patients with poor education and recent diagnosis.

Abstract:Objective: Visceral fat area (VFA) levels have been found to exhibit a strong association with various conditions such as insulin resistance (IR), inflammation, oxidative stress, metabolic syndrome (MetS), hyperlipidemia, diabetes, and its vascular complications. These complications include hypertension, cardiovascular disease, diabetic retinopathy (DR), albuminuria, and cardiovascular autonomic dysfunction, which is considered one of the main types of diabetic neuropathy. This study aimed to investigate the correlation between visceral fat and peripheral neuropathy in patients with type 2 diabetes (T2DM). Methods: A retrospective analysis of clinical data of patients diagnosed with type 2 diabetes admitted to our hospital was conducted. After excluding 28 cases, a total of 488 patients were included, divided into the group with peripheral neuropathy (207 cases) and the control group without peripheral neuropathy (281 cases). The correlation between VFA and the presence of DPN was assessed using correlation and multiple logistic regression analyses. Results: In terms of general information, the group with peripheral neuropathy had lower BMI but longer duration of diabetes compared to the control group. Regarding biochemical indicators, VFA were lower in the group with peripheral neuropathy, while FPG and HbA1c levels were higher (all P<0.05). Spearman correlation analysis showed a negative correlation between VFA, and the presence of peripheral neuropathy in patients with type 2 diabetes (P<0.05). Logistic regression analysis indicated that VFA, duration of diabetes, and HbA1c level were influencing factors for the occurrence of peripheral neuropathy in patients with type 2 diabetes (P<0.05). Conclusion: This study revealed a correlation between visceral fat and peripheral neuropathy in patients with type 2 diabetes, highlighting the importance of monitoring visceral fat in such patients. In addition to lower levels of VFA, factors such as duration of diabetes and glycated hemoglobin (HbA1c) level were also associated with peripheral neuropathy in patients with T2DM.

Abstract:Aim: Clinical heterogeneity exists in overall obesity and abdominal obesity in terms of insulin secretion and sensitivity. Further, the impact of visceral fat (VF) on the first- and second-phase insulin secretion (FPIS and SPIS) is controversial. We aim to investigate insulin secretion and sensitivity in Chinese patients with T2DM according to different BMI and VF levels. Methods: This study enrolled 300 participants. A dual bioelectrical impedance analyzer was used to assess the visceral and subcutaneous fat area (VFA and SFA). VF levels were categorized as normal or high, with the cutoff value of 100 cm 2 . FPIS and SPIS were evaluated by arginine stimulation test and standardized steamed bread meal tolerance test, respectively. β-cell function (HOMA2-β), insulin resistance (HOMA2-IR), and Gutt's insulin sensitivity index (Gutt-ISI) were also calculated. Spearman's correlation analysis and multivariate linear regression analysis were adopted for statistical analysis. Results: Participants were categorized into four groups: normal weight-normal VF, normal weight-high VF, overweight/obese-normal VF and overweight/obese-high VF. Multivariate linear regression showed that both VFA and SFA were correlated with FPIS, HOMA2-IR and Gutt-ISI after controlling for gender and diabetes duration. After further adjustment for BMI and VFA, some associations of SFA with insulin secretion and sensitivity disappeared. After adjustment for gender, diabetes duration, BMI and SFA, VFA was positively correlated with FPIS, SPIS and HOMA2-IR. Subjects with overweight/obese-high VF were more likely to have higher FPIS, HOMA2-IR and lower Gutt-ISI (all p < 0.05). Conclusion: VF affects both FPIS and SPIS, and worsens insulin sensitivity independent of BMI and subcutaneous fat in Chinese patients with T2DM. Abstract:Aim The objective of this study is to investigate the relationship between visceral fat area (VFA) and diabetic retinopathy (DR) in the context of type 2 diabetes mellitus (T2DM) within Ningbo, China. Methods The data of a total of 3,707 subjects with T2DM treated at The First Affiliated Hospital of Ningbo University were enrolled. The existence and severity of diabetic retinopathy were assessed by employing the 45° two-field stereoscopic digital photography. Subjects were categorized into four distinct groups: those without DR (NDR), individuals with mild non-proliferative DR (mild NPDR), people with moderate non-proliferative DR (moderate NPDR), and those suffering from vision-threatening DR (VTDR). Bio-electrical impedance was employed to estimate the Visceral fat area (VFA). Multinomial logistic regression models were utilized to evaluate the association between VFA and DR. Results The mean VFA in patients without diabetic retinopathy (NDR) was notably lower compared to that of patients with diabetic retinopathy (DR) (85.21 ± 37.78 vs. 97.37 ± 44.58 cm 2 , p < 0.001). As the severity of DR increased, VFA increased gradually but insignificantly (94.41 ± 43.13 cm 2 , 96.75 ± 40.82 cm 2 , 100.84 ± 49.34 cm 2 , p = 0.294). After adjusting the confounding factors, there was an association identified between VFA and the occurrence of DR (OR = 1.020, 95% CI = 1.016–1.024). It showed that regardless of BMI, whether it’s less than 25 kg/m 2 or greater than or equal to 25 kg/m 2 , a higher VFA (≥100 cm 2 ) level came with a higher prevalence of DR ( p < 0.001). Conclusion The outcomes of this research indicate a modest association between VFA and the incidence of DR among Chinese patients who have been diagnosed with T2DM in Ningbo.

Biodegradation of di-n-butyl phthalate in a soil microcosm

Abstract:This study investigated the effects of various culture treatments on di-n-butyl phthalate (DBP) degradation and the survival conditions of DBP-degrading bacterial strains in a soil microcosm. In the previous study, a DBP-degrading strain was isolated from activated sludge and identified by 16S rRNA as Deinococcus radiodurans. In this study, we added D. radiodurans into a soil microcosm and analyzed the structure of the whole bacterial community of the soil using a polymerase chain reaction-denaturing gradient gel electrophoresis (PCR-DGGE) technique. Meanwhile, the optimal conditions for DBP degradation were assessed by varying the temperature and initial pH of the culture, and by adding yeast extract and surfactants. The results show that the optimal conditions for DBP degradation in soil are a temperature of 35°C, a pH of 7, and the addition of Triton X-100 and yeast extract. Furthermore, the addition of D. radiodurans can also enhance DBP degradation in soil. The PCR-DGGE analysis showed that D. radiodurans could survive in the soil microcosm through 24 days of incubation. We hope that these findings may provide some useful information about the remediation of DBP in our environment.

Abstract:Abstract Content The correlation between visceral obesity index (VAI) and diabetes and accuracy of early prediction of diabetes are still controversial. Objective This study aims to review the relationship between high level of VAI and diabetes and early predictive value of diabetes. Data Sources The databases of PubMed, Cochrane, Embase, and Web of Science were searched until October 17, 2023. Study Selection After adjusting for confounding factors, the original study on the association between VAI and diabetes was analyzed. Data Extraction We extracted odds ratio (OR) between VAI and diabetes management after controlling for mixed factors, and the sensitivity, specificity, and diagnostic 4-grid table for early prediction of diabetes. Data Synthesis Fifty-three studies comprising 595 946 participants were included. The findings of the meta-analysis elucidated that in cohort studies, a high VAI significantly increased the risk of diabetes mellitus in males (OR = 2.83 [95% CI, 2.30-3.49]) and females (OR = 3.32 [95% CI, 2.48-4.45]). The receiver operating characteristic, sensitivity, and specificity of VAI for early prediction of diabetes in males were 0.64 (95% CI, .62–.66), 0.57 (95% CI, .53–.61), and 0.65 (95% CI, .61–.69), respectively, and 0.67 (95% CI, .65–.69), 0.66 (95% CI, .60–.71), and 0.61 (95% CI, .57–.66) in females, respectively. Conclusion VAI is an independent predictor of the risk of diabetes, yet its predictive accuracy remains limited. In future studies, determine whether VAI can be used in conjunction with other related indicators to early predict the risk of diabetes, to enhance the accuracy of prediction of the risk of diabetes.

Association of Visceral Adiposity and Its Longitudinal Increase with the Risk of Diabetes in Chinese Adults: A Prospective Cohort Study

Abstract:Objective: A Chinese visceral adiposity index (CVAI) was recently established to estimate the visceral fat area in Chinese adults. This study aimed to investigate the risk of incident prediabetes and diabetes in relation to visceral adiposity calculated by CVAI. Methods: A total of 2558 subjects with normal plasma glucose levels from the Shanghai Changfeng Study were enrolled in a prospective cohort study. The independent associations of basal visceral fat area by CVAI and its longitudinal change with incident prediabetes and diabetes were identified using Cox regression analyses. Results: During an average of 4.4 years of follow-up, 546 (21.3%) and 99 (3.9%) of 2558 nondiabetic subjects developed prediabetes and diabetes, respectively. Visceral fat area by CVAI and its longitudinal increase were independently associated with incident prediabetes and diabetes in Chinese adults. In a multivariable-adjusted regression model, CVAI, as well as its annual change, was the strongest independent predictor of incident prediabetes (HR, 1.383 [1.162-1.647]) and diabetes (HR, 1.607 [1.092-2.364]) compared with other estimates of obesity (BMI and waist circumference). Receiver operating characteristic curve analyses showed that CVAI had better performance than BMI and waist circumference for the prediction of prediabetes and diabetes in Chinese adults. Conclusions: Visceral adiposity plays a pivotal role in the pathogenesis of diabetes, and the visceral adiposity estimated by CVAI is superior to the traditional estimates of obesity for the prediction of incident prediabetes and diabetes in Chinese adults.

Abstract:Relationships between the visceral adiposity index (VAI) and type 2 diabetes mellitus (T2DM) have been underexplored. The purpose of this study is to explore association between VAI and T2DM in adults in the US. Based on the National Health and Nutrition Examination Survey 2007–2018, 11,214 participants aged 20 years or older were included in a cross-sectional study. Multifactorial logistic regression analysis and smoothed curve fitting analysis were performed to investigate links between VAI and the prevalence of T2DM, as well as the stability and incidence between subgroups. In a fully adjusted continuous model, the aggregate population risk of T2DM increased 0.43-fold with each 1-unit increase in VAI [odds ratio (OR) = 1.43; 95% confidence interval (CI) 1.35–1.50]. In the fully adjusted categorical model with VAI scores stratified by quartiles, results showed a higher T2DM advantage among participants in the second, third, and fourth quartiles (Q2: OR 1.35, 95% CI 1.06–1.71; Q3: OR 2.46, 95% CI 1.95–3.11; Q4: OR 4.42, 95% CI 3.55–05.50). Compared with Q1, the prevalence of T2DM in the total population increased 3.42-fold in Q4. The above results indicated that VAI was positively associated with the prevalence of T2DM, which was consistent and nonlinear with the smoothed curve-fitting analysis (P for non-linear = 0). Subgroup analyses after adjusting for covariates showed that keeping with the overall population results, it also was found that there was an interaction between sex and hypertension in the subgroups. VAI was positively associated with the prevalence of T2DM and was more prevalent in women, non-hypertensive than in men, hypertensive populations.

Abstract:This study aimed to evaluate the association between obesity, evaluated by fat mass index (FMI) with the risk of microvascular diseases in patients with type 2 diabetes mellitus (T2DM) and compare the magnitude of associations of FMI, body mass index (BMI), and waist circumference (WC) with the risk of microvascular diseases. We performed a post-hoc analysis of the Action to Control Cardiovascular Risk in Diabetes study. The primary microvascular outcomes of the present study included chronic kidney disease (CKD) progression, retinopathy, and neuropathy. Cox proportional-hazards models were performed to evaluate the association of FMI with microvascular diseases. A discordant analysis was performed to compare the magnitude of associations of FMI, BMI, and WC with the risk of microvascular diseases. Our study included 10,251 T2DM participants with a median of 5 years (interquartile range, 4.2–5.7) of follow-up. A total of 6,184 participants developed CKD progression, 896 participants had retinopathy, and 3,213 participants developed neuropathy (Michigan Neuropathy Screening Instrument, >2.0). After the confounding factors were adjusted for, patients in the highest FMI quartile had a higher risk of CKD progression (HR: 1.26, 95%CI: 1.16–1.36) and neuropathy (HR: 1.93, 95% CI: 1.74–2.15), except for retinopathy (HR: 1.17, 95% CI: 0.96–1.43), than those in the lowest quartile. Discordant analyses found that FMI and WC are better in identifying individuals with obesity-related risk of neuropathy, compared with BMI; neither is better in identifying individuals with obesity-related risk of CKD progression and retinopathy. Obesity is associated with CKD progression and neuropathy in T2DM participants. Further randomized trials are needed to test whether obesity control can improve the outcomes of T2DM participants with CKD or neuropathy. FMI and WC are more useful in identifying obesity-related risk of neuropathy compared with BMI in T2DM patients. Clinical Trial Registration http://www.clinicaltrials.gov , NCT00000620.

Abstract:Aims/Introduction The aim of the present study was to analyze the strength of association between fasting plasma glucose (FPG), 2-h postprandial glucose (2h PPG), hemoglobin A1c (HbA1c), disposition index (DI) and nine anthropometrics measures, to explore the best indicator for hyperglycemia. Materials and Methods Analyses were based on the cross-sectional data of 3,572 adults from the Pinggu Metabolic Disease Study. Anthropometrics were measured, visceral fat area (VFA) and subcutaneous fat area were calculated using an abdominal computed tomography scan. Linear regression was used to analyze the association between FPG, 2h PPG, HbA1c, DI and nine anthropometrics measures (height, weight, waist circumference [WC], body mass index and waist-to-hip ratio [WHR], waist-to-height ratio, VFA, subcutaneous fat area, and visceral-to-subcutaneous ratio). Logistic regression was further carried out to understand the association between per standard increase and risk for hyperglycemia. Results Higher VFA and subcutaneous fat area were associated with higher FPG, 2h PPG, HbA1c and DI after adjusting for other covariates. The strongest association observed after adjustment was WC for FPG, with one standard deviation greater WC being associated with 1.70 increased odds; WHR for 2h PPG, with one standard deviation greater WHR being associated with 1.83 increased odds. The strength of the association between VFA and FPG, 2h PPG, HbA1c, and DI was less than WHR and WC, but slightly stronger than body mass index. Stratified analyses showed that VFA performs better as an anthropometrics indicator in predicting hyperglycemic risk in women than men. Conclusions WHR and WC remain the best indicators for hyperglycemic risk among ahealthy Chinese population.

Abstract:Abstract Background We investigated whether glycemic control affects the relation between endothelial dysfunction and coronary artery disease in patients with type 2 diabetes mellitus (T2DM). Methods In 102 type 2 diabetic patients with stable angina, endothelial function was evaluated using brachial artery flow-mediated dilation (FMD) with high-resolution ultrasound, and significant stenosis of major epicardial coronary arteries (≥ 50% diameter narrowing) and degree of coronary atherosclerosis (Gensini score and SYNTAX score) were determined. The status of glycemic control was assessed by blood concentration of glycated hemoglobin (HbA1c). Results The prevalence of significant coronary artery stenosis (67.9% vs. 37.0%, P = 0.002) and degree of coronary atherosclerosis (Gensini score: 48.99 ± 48.88 vs. 15.07 ± 21.03, P < 0.001; SYNTAX score: 15.88 ± 16.36 vs. 7.28 ± 10.54, P = 0.003) were higher and FMD was lower (6.03 ± 2.08% vs. 6.94 ± 2.20%, P = 0.036) in diabetic patients with poor glycemic control (HbA1c ≥ 7.0%; n = 56) compared to those with good glycemic control (HbA1c < 7.0%; n = 46). Multivariate regression analysis revealed that tertile of FMD was an independent determinant of presence of significant coronary artery stenosis (OR = 0.227 95% CI 0.056–0.915, P = 0.037), Gensini score (β = − 0.470, P 0.05). Conclusion Poor glycemic control negatively influences the association of endothelial dysfunction and coronary artery disease in T2DM patients.

Abstract:Introduction: Empirical cohort have evidenced that central adiposity is the strongest modifiable risk factor for T2DM, however most studies neglect the absolute measures including visceral fat area (VFA) and subcutaneous fat (SFA). This study aims to analyze the strength of association between FPG, 2hPPG, HbA1c and nine anthropometrics measures, to explore the best indicator for hyperglycemic risks. Methods: Analyses were based on the data from the Pinggu Metabolic Disease study in 2013-2014. Excluded participants with self-reported DM, dyslipidemia and hyperuricemia, 2940 were included. Anthropometrics, VFA and SFA were measured. Logistic regression was used to find the association between per standard increase and risk for high FPG (≥7.0 mmol/L), 2hPPG (≥11.1 mmol/L), HbA1c (≥6.5%). Results: Higher VFA and SFA were associated with higher FPG, 2hPPG and HbA1c after adjusting for other covariates. The strongest association was waist to hip ratio (WHR) for FPG and 2hPPG, and waist circumference (WC) for HbA1c. The strength of association between SFA, VFA and FPG, 2hPPG and HbA1c was less than WHR and WC. VSR showed a comparably weak strength of association. See Figure for detail result. Abstract:PURPOSE To compare outcomes of patients with HIV-Burkitt's lymphoma (HIV-BL) and HIV-diffuse large-cell lymphoma (HIV-DLCL) after treatment with CHOP (cyclophosphamide, doxorubicin, vincristine, prednisone) or M-BACOD (methotrexate, bleomycin, cyclophosphamide, etoposide) in pre-highly active antiretroviral therapy (HAART) versus HAART eras. PATIENTS AND METHODS Three hundred sixty-three patients with AIDS-related lymphoma diagnosed from 1982 to 2003 were reviewed retrospectively, including 262 in the pre-HAART (HIV-BL, 117; HIV-DLCL, 145) and 101 in the HAART era (HIV-BL, 18; HIV-DLCL, 83). Pre-HAART included those who did not receive HAART, and HAART era included those diagnosed after January 1997 who received HAART. RESULTS There were no significant differences between groups in terms of age, sex, history of injection drug use, prior AIDS, lactate dehydrogenase level, and disease stage at diagnosis. Compared with HIV-BL, HIV-DLCL was associated with significantly lower CD4 counts in the pre-HAART but not the HAART era. Although the overall median survival was similar for both groups in the pre-HAART era (HIV-BL, 6.4 months v HIV-DLCL, 8.3 months; P = .43), survival was significantly worse in patients with HIV-BL in the HAART era (HIV-BL, 5.7 months v HIV-DLCL, 43.2 months; P = .0003). Failure to attain complete remission and CD4 count less than 100 cells/mm(3) independently predicted for poor survival in the pre-HAART era. In comparison, histology of HIV-BL and no attainment of complete remission were independent poor prognostic factors in the HAART era. CONCLUSION Survival of patients with HIV-DLCL has improved in the HAART era, along with CD4 count, whereas survival of similarly treated patients with HIV-BL remained poor. The current practice of using the same regimen for both groups of patients should be re-evaluated.

Abstract:Purpose: To study beta-cell function and insulin sensitivity in abdominal visceral obesity. Methods: Fifty-two abdominal visceral obesity patients, 47 control subjects and 102 type 2 diabetes mellitus(T2DM) were selected to study. Nine visceral obesity subjects, 3 novisceral obesity patients, 9 control subjects and 7 T2DM were analyzed for beta-cell function and insulin sensitivity by hyperglycemic clamp and regional body fat was measured by MRI. Results: The result showed that the levels of first phase insulin release(FPIR), glucose disposal rates(GDR) and insulin sensitivity index(ISI) were significantly elevated and fasting insulin(FINS). HOMA-IR and proinsulin were remarkably decreased in control group compared with visceral obesity group and T2DM group(P<0.05). As compared with T2DM group, the levels of waist hip ratio(WHR), HOMA-β, fasting blood glucose(FBG), visceral adipose area(VA), subcutaneous adipose area(SA), FPIR, second phase insulin release(SPIR) and maximum insulin release(INS-ax) were increased and FBG decreased in visceral obesity group. Multiple stepwise regression analysis showed FBG was the independent negatively correlated to FPIR, WHR negatively correlated with ISI(P<0.05). Conclusions: The study confirmed that insulin resistance is uniformly present in subjects with visceral obesity who may be had early beta-cell dysfunction.

Abstract:Objective To investigate the influence of abdominal adipose tissue on insulin resistance and other metabolic indicators in the patients with type 2 diabetes mellitus(T2DM)complicating obesity.Methods Fifty-one patients with newly diagnosed T2DM complicating obesity (T2DM group) and 50 cases of simple obesity(control group) in Jishuitan Hospital were enrolled.The two groups were performed QCT for measuring the total abdominal adipose tissue volume (TAV),subcutaneous adipose tissue volume (SAV),visceral adipose tissue volume (VAV),and then the VAV/SAV ratio was calculated.Meanwhile the waist circumference,body mass index(BMI),blood uric acid (UA),total cholesterol (C H O),triglycerides (TG),high-density lipo protein cholesterol (HDL),low density lipoprotein cholesterol(LDL),homocysteine(HCY),dosage of basal insulin(DBIns),dosage of prandial insulin (DPIns),fasting plasma glucose(FBG),fasting blood insulin (FIns) and HbA1c levels were also measured.The relationship between TAV,SAV and VAV with HOMA-RI and other indicators was investigated.The influence of abdominal adipose tissue on insulin resistance and other metabolic indicators was determined.Results The UA,TG,LDL,FPG and HbA1c levels in the T2DM group were increased compared with the control group(P＜0.05).Compared with the control group,the SAV level in the T2DM group was decreased(P＜0.05),the VAV and VAV/SAV levels were increased(P＜0.05).The multivariate regression analysis showed that VAV was the natural logarithm affecting In (HOMA-RI),UA,CHO,TG,LDL,HCY,DBIns (β=0.399,0.398,0.389,0.447,0.440,0.372,0.307;P＜ 0.05).TAV was the main factor affecting the waist circumference and BMI (β=0.868,0.639;P＜0.05).Conclusion The visceral adipose tissue volume has larger influences on insulin resistance,blood lipids,UA,HCY and DBIns.
Nonlinear association between the triglyceride-glucose index and diabetes mellitus in overweight and obese individuals: a cross-sectional retrospective analysis

Abstract:Background: The triglyceride-glucose (TyG) index is linked to both the development and progression of diabetes, while obesity remains a significant risk factor for this disease. However, the relationship between the TyG index and overweight or obese diabetes remains unclear. Methods: This study was a cross-sectional analysis of data from 40,633 participants with body mass index (BMI) ≥ 24 kg/m2 who were screened from January 2018 to December 2023 at Henan Provincial People's Hospital. Participants were divided into groups of overweight or obese individuals with diabetes and those without diabetes according to the diabetes diagnostic criteria. The TyG index, the dependent variable, was determined using the equation ln [fasting triglycerides (mg/dL) × fasting glucose (mg/dL)/2]. We explored the association between TyG index and diabetes in overweight or obese individuals through multivariate logistic regression, subgroup analysis, generalized additive models, smoothed curve fitting, and analysis of threshold effects. Results: Patients who were overweight or obese and had diabetes had higher TyG index levels than those without diabetes. After adjusting for confounders, our findings indicated a significant association between the TyG index and the risk of diabetes in overweight or obese individuals [odds ratio (OR) = 7.38, 95% confidence interval (CI): 6.98-7.81]. There was a J-shaped nonlinear association between TyG index and diabetes. When TyG index was > 4.46, the risk of diabetes increased sharply. Notably, a high baseline TyG index (Q4 group) correlated with a notably greater risk of diabetes than did the Q1 group, with an OR of 22.72 (95% CI: 20.52-25.16). Subgroup analysis revealed that the association between TyG and diabetes was stronger in females than in males (OR = 7.57, 95% CI: 6.76-8.48,), more significant in individuals with a BMI of 24-28 kg/m2 than in those with a BMI ≥ 28 kg/m2 (OR = 8.40, 95% CI: 7.83-9.02), and increased with age (OR = 8.15, 95% CI: 7.25-9.17) (all P for interaction < 0.001). Conclusion: Among overweight or obese individuals, a higher TyG index is associated with an elevated risk of diabetes, especially when TyG is > 4.46. Furthermore, factors such as sex, age, and BMI significantly influence the risk of diabetes in overweight or obese individuals. Specifically, older women with a BMI of 24-28 kg/m2 are at a greater risk of diabetes under similar TyG index conditions.
