Abstract:Introduction: Sleep is one of the most crucial basic human needs in Maslow's hierarchy, while sleep disorders are the most commonly occurring complications of chemotherapy. Objective: This study was conducted to determine the effect of progressive muscle relaxation techniques on the sleep quality of patients with cancer undergoing chemotherapy. Methods: This study was a randomized clinical trial performed on patients with cancer undergoing chemotherapy. Ninety cancer patients were selected by the convenience sampling method and randomly assigned into two groups; the progressive muscle relaxation group and the control group. Before the intervention, the Pittsburgh Sleep Quality Questionnaire (PSQQ) was completed for patients in both groups. In the intervention group, progressive muscle relaxation was performed for 4 weeks (twice daily and 20 min each time). In the control group, patients only received routine care. After the intervention, the PSQQ was completed again for patients in both groups. The data were analyzed using SPSS version 23. Results: The mean and standard deviation of age were 45.95 ± 10.26 years. The samples of the intervention and control groups were homogeneous in terms of demographic characteristics. The results showed a significant improvement in the Pittsburgh Sleep Quality Index in the intervention group before and after the intervention (P < .001, Z = 5.09). However, this rate was not different in the control group. The sleep level of the intervention and control groups was not different at the beginning of the study. However, at the end of the study, the sleep score in the intervention group had decreased significantly (P < .001, Z = 3.52). Conclusion: The results of the present study showed that progressive muscle relaxation improves the sleep quality of patients with cancer undergoing chemotherapy. Therefore, relaxation can be considered a useful and effective intervention for improving sleep quality of patients with cancer.
Comparing the effects of warm footbath and foot reflexology on depression of patients undergoing radiotherapy: A randomized clinical trial

Abstract:Background: Cancer diagnosis not only impacts physical health but also mental well-being, often leading to significant stress, fear, and depression among patients.The utilization of CAM has shown a rising trend, influenced by the availability of different modalities offered by healthcare services, sometimes in an ad hoc fashion. This study seeks to examine and compare the respective impacts of warm foot baths and foot reflexology on depression in patients undergoing radiotherapy. Methods: A randomized clinical trial was conducted at Mashhad University of Medical Sciences in Iran in 2019, following CONSORT guidelines. Participants included non-metastatic cancer patients aged 18-60 undergoing a 28-day radiotherapy course. Patients were randomly assigned to receive either warm footbaths or foot reflexology as interventions, performed daily for 20 min over 21 days. The data were analyzed using appropriate statistical tests. Results: Statistical analysis indicated no significant differences in demographic attributes between the two groups. Both interventions led to a significant reduction in depression scores post-treatment compared to pre-treatment assessments. Foot reflexology showed a greater reduction in depression scores compared to footbaths with warm water. Conclusions: Both warm footbaths and foot reflexology are effective in alleviating depression in patients undergoing radiotherapy, with foot reflexology showing a greater impact on improving depression levels. The study recommends foot reflexology as a preferred intervention for managing depression in these patients if conditions and facilities permit.

Abstract:Integrative Cancer Therapies, Volume 23, Issue , January-December 2024. Background:Among leukemia patients, sleep disruptions are prevalent and can profoundly affect their overall quality of life. Acupressure and foot reflexology, modalities rooted in traditional Chinese medicine, have garnered attention for their potential to address sleep disturbances and mitigate associated symptoms.Methods:This research utilized a randomized controlled trial with a pretest-posttest design involving 102 leukemia patients admitted to Imam Khomeini Hospital in Urmia. Participants were randomly allocated to 3 groups: acupressure (n = 34), reflexology (n = 34), or control (n = 34). Prior to the intervention, patients completed a demographic survey and the Pittsburgh Sleep Quality Index (PSQI) for baseline assessments. Acupressure involved stimulation of the SP6 point twice daily for 10 minutes over 4 weeks, while reflexology entailed daily 10-minute sessions with sweet almond oil on the soles for the same duration. The control group received standard care without additional interventions. Following the 4-week intervention period, post-intervention evaluations were conducted using identical measurement tools.Results:The findings underscored the efficacy of both acupressure and foot reflexology in significantly improving sleep quality within the intervention groups (P .05). Subsequently, pairwise comparisons adjusted with Bonferroni corrections revealed significant disparities in sleep quality between the acupressure and reflexology groups compared to the control group (P < .001). However, post-intervention analysis indicated no statistically significant variance in enhancing sleep quality between the acupressure and foot reflexology groups (P < .05).Conclusion:This study demonstrates that acupressure and foot reflexology interventions can enhance sleep quality in individuals with leukemia. These findings support the effectiveness of these complementary modalities, offering targeted relief and relaxation. While these non-invasive therapies show promise in improving well-being, further research is needed to confirm and expand upon these results due to study limitations.

Abstract:OBJECTIVE: To evaluate the effects of reflexology treatment on quality of life, sleep disturbances, and fatigue in breast cancer patients during radiation therapy.METHODS/SUBJECTS: A total of 72 women with breast cancer (stages 1-3) scheduled for radiation therapy were recruited.DESIGN: Women were allocated upon their preference either to the group receiving reflexology treatments once a week concurrently with radiotherapy and continued for 10 weeks or to the control group (usual care).OUTCOME MEASURES: The Lee Fatigue Scale, General Sleep Disturbance Scale, and Multidimensional Quality of Life Scale Cancer were completed by each patient in both arms at the beginning of the radiation treatment, after 5 weeks, and after 10 weeks of reflexology treatment.RESULTS: The final analysis included 58 women. The reflexology treated group demonstrated statistically significant lower levels of fatigue after 5 weeks of radiation therapy (p < 0.001), compared to the control group. It was also detected that although the quality of life in the control group deteriorated after 5 and 10 weeks of radiation therapy (p < 0.01 and p < 0.05, respectively), it was preserved in the reflexology group, which also demonstrated a significant improvement in the quality of sleep after 10 weeks of radiation treatment (p < 0.05). Similar patterns were obtained in the assessment of the pain levels experienced by the patients.CONCLUSIONS: The results of the present study indicate that reflexology may have a positive effect on fatigue, quality of sleep, pain, and quality of life in breast cancer patients during radiation therapy. Reflexology prevented the decline in quality of life and significantly ameliorated the fatigue and quality of sleep of these patients. An encouraging trend was also noted in amelioration of pain levels.

Abstract:Viral mRNAs from lesions containing human papillomavirus type 6 (HPV-6) have previously been mapped on the viral DNA but relatively little is known about the control of mRNA production, or whether the mapped RNA termini correspond to promoters. By analysis of run-off transcripts synthesized in vitro, primer extension and measurements of promoter activity in fragments of the viral DNA introduced into cells, we have identified three promoters in the early region of the HPV-6b genome. These are: (i) at the end of the long control region upstream of the E6 open reading frame; (ii) upstream of E7 and (iii) upstream of E1. The promoter upstream of E1 was the most active. These results contrast with results of similar assays with HPV-18, in which the strongest promoter was that controlling expression of the transforming genes E6 and E7. In addition, a novel promoter was detected close to E5a, upstream of the late genes.

Abstract:Objectives This study aimed to evaluate the effect of progressive muscle relaxation exercises (PMRE) on sleep quality in patients undergoing chemotherapy treatment and experiencing disturbed sleep. Methods The prospective randomized controlled study was conducted between March and September 2022 with 69 patients (intervention group: 34 patients, control group: 35 patients) in a hospital chemotherapy unit. During the data collection process, the "Personal Information Form" and "Pittsburgh Sleep Quality Index (PSQI)" were utilized. Patients in the intervention group performed PMRE twice a day for 8 weeks. Patients in the control group received routine care at the clinic without additional intervention. For data analysis, Student's t -test, Mann–Whitney U test, Fisher's exact test, and chi-square test were used. Results The sociodemographic attributes of patients within both the intervention and control groups exhibited comparability. However, notable distinctions emerged in the PSQI Global sleep score and PSQI subdimension scores, encompassing sleep latency and duration, subjective sleep quality, habitual sleep efficiency, sleep disturbance, and daytime dysfunction between the two groups. The study found a notable difference in scores between the patients in the intervention group and those in the control group. The patients who received the intervention had significantly lower scores ( P < .001). Conclusion The study revealed that PMRE was beneficial in improving sleep quality in cancer patients undergoing chemotherapy who had poor sleep quality. Implications for Nursing Practice Oncology nurses may consider using PMRE to improve the sleep quality of cancer patients receiving chemotherapy.

Abstract:Cyclic AMP‐dependent protein kinase A (PKA) type I has been established as an acute inhibitor of T cell activation. For this reason, we investigated the possible role of PKA type I in HIV‐induced T cell dysfunction. T cells from HIV‐infected patients have increased levels of cAMP and are more sensitive to inhibition by cAMP analog than are normal T cells. A PKA type I‐selective antagonist increases the impaired proliferation of T cells from HIV‐infected patients to normal or subnormal levels (up to 2.8‐fold). Follow‐up of patients after initiation of highly active antiretroviral treatment revealed that a majority of patients have a persistent T cell dysfunction that is normalized by incubation of T cells with Rp‐8‐Br‐cAMPS. These observations imply that increased activation of PKA type I may contribute to the progressive T cell dysfunction in HIV infection and that PKA type I may be a potential target for immunomodulating therapy.—Aandahl, E. Clinical Research of Acupuncture on Malignant Tumor Patients for Improving Depression and Sleep Quality.

Abstract:Objective: To study the effect of acupuncture on depression and insomnia of malignant tumor patients.Methods: Eighty cases of cancer related depression and sleep disorders were randomly divided into 2 groups: treatmentl group (acupuncture n=40) and control group (Fluoxetione n=40). All patients were assessed by Self-rating Depression Scale (SDS), Hemilton Depression Rating Scale (HAMD) and Pittsburgh Sleep Quality Index (PSQI) after treatment for 30 days.Results: Before treatment the SDS and HAMD scores of the trial group were 64.12 +/- 5.34 and 20.92 +/- 2.38, those of the control group were 64.24 +/- 4.98 and 20.72 +/- 2.74, after treatment the SDS and HAMD scores of the trial group were 43.64 +/- 5.28 and 9.88 +/- 1.27 respectively, significantly lower than those of the control group (50.76 +/- 5.42 and 13.72 +/- 2.05 respectively, both P < 0.05). Before treatment the PSQI score of the trial group and the control group were 14.48 +/- 1.71 and 13.92 +/- 2.59, after treatment the PSQI score of the trial group was 7.92 +/- 1.22 after treatment, significantly lower than those of the control group (11.44 +/- 1.89, P < 0.01).Conclusion: Acupuncture can effectively reduce malignant-related depression, improve sleep quality, and help to improve the quality of life of cancer patients.
The effects of foot reflexology on symptoms of discomfort in palliative care: a feasibility study

Abstract:Background: In palliative care, the relief of discomfort is sought by an overall approach, combining prescribed medication and additional therapies, such as foot reflexology (FR). The main objective of this study was to assess the feasibility of FR in a population of inpatients in a palliative care unit (PCU).The precariousness of the patients led us to perform a feasibility study and not a cohort study from the outset. Its secondary objective was to assess the impact of an FR session on some symptoms of discomfort (anxiety, pain, troubled sleep, and psychological distress). Methods: This is a feasibility study designed as a randomized controlled two-arm therapeutic trial. One arm tested FR, the other an active control, massage therapy (MT). The evaluators were blinded. Results: FR was feasible for 14 patients out of the 15 included in the FR group (95% CI [68%; 100%]). These patients were in the palliative care phase of cancer, motor neuron disease, or terminal organ failure. Concerning the symptoms of discomfort, ESAS sleep quality score was on average 3.9 (± 2.5) before a session in the FR group. It was improved to an average of 3 (± 2.3) on the day after the session (effect-size = 0.38 [0.03; 0.73]). Conclusion: This study confirms the feasibility of an FR session for patients hospitalized in a PCU. It resulted in a slight improvement in sleep quality. For other discomfort symptoms such as anxiety, pain and distress, FR yielded a non-significant improvement. Significant results would have needed a larger cohort.

Abstract:Annual asthma hospitalization rates were calculated for California's ethnically diverse population from 1983 through 1996. Trends were examined for four race/ethnicity groups: Hispanics, African-Americans, non-Hispanic Caucasians, and Asians/Pacific Islanders. The overall rate decreased by 30% during the time period. African-Americans had the highest rate, more that three times greater than the rate for Caucasians. Among children, the rates for Caucasians decreased by one-third, while rates increased for Hispanics and Asians. The rate for African-American children remained generally constant and was four times higher than the rate for Caucasians. Data from 1996 were assessed for repeat admissions, age and sex differences in rates, costs, and progress toward national goals.
Effects of Aromatherapy on Cancer Patients' Sleep and Fatigue: A Systematic Review and Meta-Analysis

Abstract:Objective: This study aims at investigating the effectiveness of aromatherapy in enhancing sleep quality and reducing fatigue among cancer patients based on randomized controlled trials (RCTs) with a high level of evidence. Design: A systematic review and meta-analysis. Methods: Two reviewers screened and reviewed the full texts of search results obtained from six English databases and four Korean databases, and they extracted data from the selected studies independently. Risk of bias (RoB) was assessed using the revised Cochrane RoB tool for randomized trials. The review protocol was registered with the International Prospective Register of Systematic Reviews (CRD42021277515). Results: Eleven RCTs were included in this study. Aromatherapy significantly improved sleep quality among cancer patients compared with those in the control groups (routine care, no intervention, and placebo) (standardized mean difference [SMD] = -0.92, 95% confidence interval [CI]: -1.39 to -0.46; p < 0.001, I2 = 89%). However, aromatherapy did not significantly reduce fatigue among such patients (SMD = -0.40, 95% CI: -0.81 to 0.01). The results of the meta-regression confirmed that increased intervention sessions resulted in the enhanced effectiveness of aromatherapy in improving sleep quality among cancer patients (Z = -3.86, p < 0.001). Conclusion: This study showed that aromatherapy significantly improves sleep quality among cancer patients. However, aromatherapy does not significantly reduce fatigue among cancer patients. It is possible that the statistical power of the meta-analysis was weak due to the small number of included studies. Therefore, in future studies, the effects of aromatherapy on fatigue among cancer patients should be assessed further using a larger number of included studies. Further, the effects of aromatherapy should be evaluated with a focus on specific subsets of patients, such as specific cancer diagnoses and specific modalities for cancer treatment.

Abstract:BackgroundNondrug treatments are potentially beneficial for cancer patients. However, the effect of sleep on cancer-related fatigue (CRF) and quality of life (QOL) in cancer patients remains unclear. We conducted a meta-analysis of randomized controlled trials to examine the efficacy of sleep in cancer patients undergoing treatment.MethodsThe PubMed, Ovid, Embase, Cochrane Central Register of Controlled Trials, and China National Knowledge Infrastructure databases were searched to identify suitable studies. Stata 15.0 software was used for statistical analyses. Sensitivity analyses were conducted. Fourteen studies (6 in English and 8 in Chinese) involving 1151 patients were included in the meta-analysis. Ten, five, and six studies that assessed the effects of sleep on CRF, QOL, and quality of sleep, respectively, in cancer patients undergoing treatment were identified.ResultsSleep interventions significantly affected overall CRF (standardized mean difference (SMD) = −1.52, P < 0.01), overall QOL (SMD = 1.20, P < 0.01), physical fatigue (SMD = −0.66, P < 0.01), cognitive fatigue (SMD = −0.38, P = 0.015), and physical function (SMD = 0.64, P < 0.01). Comprehensive intervention measures focusing on sleep, sleep nondrug interventions, and interventions for ≥3 or <3 months affect CRF. However, no significant effects on emotional fatigue, emotional function, perpetual fatigue, depression, or quality of sleep were observed.ConclusionsComprehensive interventions focusing on sleep are helpful for CRF. Sleep interventions may only affect physiological function and have no effect on emotional function, perpetual function, or sleep quality. Future research should focus on how to combine sleep interventions with psychological, social, cognitive, and emotional interventions and provide targeted comprehensive nursing measures to better improve CRF, sleep quality, and QOL.

Abstract:BackgroundSleep problems are one of the most common symptoms experienced by cancer patients. The causes of poor sleep quality might be due to treatment and its side effects. Thus, we conducted this systematic review and meta-analysis with the aims of investigating sleep quality during treatment in cancer patients.MethodsComprehensive search strategy was conducted in the following original databases: PubMed, Web of Science (ISI), Scopus, Embase, PsycINFO, and Ovid, from 1950 to 15th February 2021. Studies that investigated the sleep quality during treatment in cancer patients were included. Two investigators extracted all relevant data, independently. For deriving mean difference, random-effects meta-analyses were used. We assessed quality of studies by Newcastle–Ottawa Scale (NOS).ResultsA total of 27 studies (1884 participants) were included in the syntheses on sleep quality. The mean global Pittsburgh Sleep Quality Index (PSQI) in cancer patients before the initiation of treatment was 7.11 (95% CI: 6.48, 7.74), during 8.31 (95% CI: 6.34, 10.27), after the treatment 7.10 (95% CI: 6.54, 7.66), and finally, the mean global PSQI in the time of follow-up was estimated 7.33 (95% CI: 6.27, 8.39); all the results were meaningful (P < 0.001). The mean difference showing the quality of sleep was better before the initiation of treatment compared to after the treatment.ConclusionCancer patients who underwent cancer treatment face lots of problems and adverse effects caused by treatment. Our results revealed that cancer patients experience poor quality of sleep during the whole trajectory of cancer even after a year from the initiation of treatment. After the end of treatment, sleep quality got better compared to during the treatment and returned to before the treatment level, but it is still poor and needs more sleep-related interventions to improve.

Abstract:Objective: Chemotherapy-associated insomnia is a highly prevalent complaint in breast cancer patients. This study was undertaken to evaluate the safety, feasibility, and preliminary effectiveness of electroacupuncture plus auricular acupressure for chemotherapy-associated insomnia in patients with breast cancer. Materials and Methods: In this randomized, wait-list controlled trial, thirty breast cancer patients under or post chemotherapy with insomnia were randomly allocated to the acupuncture or wait-list control group. Participants in acupuncture group received electroacupuncture plus auricular acupressure treatment twice weekly for 6 weeks. Participants in wait-list group received the same regimen of treatment after 6-week of waiting period. Insomnia Severity Index (ISI) served as the primary outcome measurement. Secondary outcomes were sleep parameters recorded with sleep diary and actiwatch, as well as the scores of Pittsburgh Sleep Quality Index (PSQI), Hospital Anxiety and Depression Scale (HADS), and Functional Assessment of Cancer Therapy-Breast Cancer (FACT-B). Results: Twenty-eight participants completed study (13 in the acupuncture group vs 15 in the wait-list control group). At week-6 post-intervention, ISI score change from baseline showed significant between-group difference favoring acupuncture group of −2.9 points (95% CI: −5.2 to −0.6, P = .014). The acupuncture group showed greater improvements in the total sleep time recorded by sleep diary ( P = .026), scores of PSQI ( P = .012), HADS-depression ( P = .020), and FACT-B ( P < .001) compared with the control group. Improvements were maintained at week-10 and week-14 follow-ups. Conclusions: Acupuncture is safe, feasible, and effective for chemotherapy-associated insomnia in breast cancer patients under or post chemotherapy. A larger sample size randomized clinical trial is warranted to confirm the present findings. Clinical Trial Registration: NCT03762694.

On the other hand, an essential part of the quality of life is the quality of sexual life and sexual dysfunction that will affect people's perception of the quality of sexual life. Reflexology massage is one of the complementary and alternative therapies combined with conventional medical treatments to reduce disease and treat its complications. This study aimed to determine the effect of foot reflexology on hemodialysis patients' quality of sexual life. This was a parallel randomized sham-controlled trial which conducted in hemodialysis center of Imam Reza (AS) Hospital in Sirjan, Iran. Forty-seven patients under hemodialysis were divided into two groups of foot reflexology ( n = 24) and sham ( n = 23) using block randomization method. The intervention group received foot reflexology treatment during dialysis for 4 weeks, 3 times a week, 30 min each time (15 min per foot). In the sham group, non-specific foot massage was conducted without squeezing the standard points of reflexology with the same conditions and duration as the intervention group. Quality of sexual life was assessed using female sexual quality of life questionnaire (SQOL-F) and male sexual quality of life questionnaire (SQOL-M) before the intervention, immediately, and one month after the intervention. The results showed that the quality of sexual life of participants in the reflexology group immediately after the intervention (69.74) was significantly higher than before (47.73) and one month after the intervention (52.27). There was no significant difference in the average quality of sexual life of participants in the sham group at different times (before intervention 46.67, immediately after intervention 45.66, and 1 month after intervention 48.18). Immediately after the reflexology intervention, the quality of sexual life was significantly higher than in the sham group ( p < 0.001). This study highlights the potential benefit from reflexology in improving the quality of sexual life in patients undergoing hemodialysis. Trial registration IRCT20200712048085N1. Abstract:BACKGROUND: Esophageal cancer patients experience severe symptoms and poor quality of life.OBJECTIVE: We examined the effects of a rehabilitation program on quality of life, sleep, rest-activity rhythms, anxiety, and depression of esophageal cancer patients.METHODS: Forty-four patients with esophageal cancer were randomly assigned to an experimental group, which underwent a 12-week brisk walking and diet education program, or a control group, which received standard care. Health-related quality of life, subjective and objective sleep quality, rest-activity rhythms, anxiety, and depression were assessed at baseline and post intervention.RESULTS: A generalized estimating equation analysis revealed that, after intervention, compared with the control group, the experimental group exhibited significantly improved reflux (P = .022; effect size, 0.32) and marginally improved emotional (P = .069; effect size, 0.27) and social (P = .069; effect size, 0.27) functions; constipation (P = .050; effect size, 0.29), eating difficulty (P = .058; effect size, 0.27), anxiety (P = .050; effect size, 0.29), and total sleep time (P = .068; effect size, 0.39).CONCLUSIONS: The rehabilitation program may improve health-related quality of life and sleep and alleviate anxiety in patients with esophageal cancer.IMPLICATIONS FOR PRACTICE: A rehabilitation program comprising exercise and diet education is a feasible and low-cost intervention for improving quality of life of patients with esophageal cancer. Healthcare team members may consider it as a nonpharmacological treatment option for patients.

Abstract:Objective: To explore the effects of music therapy combined with aerobic exercise on sleep quality among breast cancer patients during the course of chemotherapy after radical mastectomy. Methods: Totally 100 cases of female breast cancer patients receiving chemotherapy were randomly assigned to the intervention group (n=51) and control group (n=49). Implementation of routine care of patients in the control group, the intervention group received music therapy combined with aerobic exercise on top of traditional routine care. Pittsburgh Sleep Quality Index (PSQI) was used to measure the quality of sleep among the two groups at 10 days after the surgery, and after the ifrst, third, and sixth chemotherapy courses, then the results were compared between two groups Results: PSQI scores in the intervention group after the ifrst, third, and sixth chemotherapy, course were lower than the control group, the differences were statistically signiifcant (P<0.05). Conclusion: Music therapy in combination with aerobic exercise can improve the patient's quality of sleep, also shows continuous improvements in various aspects of sleep reaction.
