Abstract:Sir, Trichofolliculoma is an uncommon benign follicular hamartoma that presents mostly during adulthood as an asymptomatic papule or nodule on the face.[1] Herein, we describe a case of trichofolliculoma with dermoscopic features. A 33-year-old man presented with a 2-year history of an asymptomatic papule on the forehead. There was no history of trauma or bleeding at the same site. The patient was in good health with an unremarkable medical history. Physical examination showed a flesh-colored papule with a central dilated pore and tufted white hairs perforating [Figure 1a]. Dermoscopy revealed an "erupting volcano" pattern composed of a central dilated pore with tufted white hairs perforating surrounded by linear vessels in a radial arrangement on the pink background [Figure 1b]. The lesion was resected intact and sent for a pathologic examination. Pathology manifested a cystically dilated follicle filling with keratinous debris and hair shaft fragments and radially arranged hair follicles in different stages of development arising from the wall [Figure 1c]. A diagnosis of trichofolliculoma was made and no recurrence was detected after one year of follow-up.Figure 1: (a) A flesh-colored papule with a central dilated pore and tufted white hairs perforating on the forehead. However, this special clinical presentation only emerges in about 15% of patients and the majority have no distinguishing features.[2] The dermoscopic manifestations of lesions without hairs perforating were multifarious and lack of commonality.[1] The lesions with hairs perforating had common dermoscopic presentations presenting a central dilated pore with tufted white hairs perforating surrounded by dilated capillaries.[3,4] Two of these cases had multiple dilated pores.[3,4] The clinical phenomenon of central hairs perforating from a papule is a crucial diagnostic clue; however, it is not unique to trichofolliculoma. This phenomenon also can be seen in folliculosebaceous cystic hamartoma.[5] Nevertheless, the dermoscopic manifestations are different between them. Multiple islands of yellowish-orange areas are found in folliculosebaceous cystic hamartoma that represents the dermal sebaceous component.[5] Dermoscopy can supply more useful diagnostic clues about skin adnexal neoplasms.
